The primary aim of this independent prospective randomised trial
was to compare serum metal ion levels for ceramic-on-metal (CoM)
and metal-on-metal (MoM) bearing surfaces in total hip arthroplasty
(THA). Our one-year results demonstrated elevation in metal ion
levels above baseline with no significant difference between the
CoM and MoM groups. This paper reviews the five-year data. The implants used in each patient differed only in respect to
the type of femoral head (ceramic or metal). At five-year follow-up
of the 83 enrolled patients, data from 67 (36 CoM, 31 MoM) was available

The length of the tourniquet time during total knee arthroplasty (TKA) is related to the incidence of post-operative deep vein thrombosis (DVT). Our aim in this study was to investigate the effect of the early release of the tourniquet on the incidence of DVT in patients undergoing TKA. A total of 200 patients who underwent TKA between November 2015 and November 2016 were prospectively enrolled. The tourniquet was inflated before surgery and released immediately after the introduction of the components (early release group). This group was compared with a retrospective cohort of 200 primary TKAs, in which the tourniquet was released after the dressings had been applied (late release group). The presence of a DVT was detected using bilateral lower limb ultrasonography. Peri-operative clinical and follow-up data were collected for analysis.Objectives

An eight-week-old boy developed severe thoracic
spondylodiscitis following pneumonia and septicaemia. A delay in
diagnosis resulted in complete destruction of the T4 and T5 vertebral
bodies and adjacent discs, with a paraspinal abscess extending into
the mediastinum and epidural space. Antibiotic treatment controlled
the infection and the abscess was aspirated. At the age of six months,
he underwent posterior spinal fusion in situ to
stabilise the spine and prevent progressive kyphosis. At the age
of 13 months, repeat imaging showed lack of anterior vertebral body re-growth
and he underwent anterior spinal fusion from T3 to T6 and augmentation
of the posterior fusion. At the age of five years, he had no symptoms
and radiographs showed bony fusion across the affected levels. Spondylodiscitis should be included in the differential diagnosis
of infants who present with severe illness and atypical symptoms.

To assess the accuracy of patient-specific instruments (PSIs) CT scans were obtained from five female cadaveric pelvises. Five osteotomies were designed using Mimics software: sacroiliac, biplanar supra-acetabular, two parallel iliopubic and ischial. For cases of the left hemipelvis, PSIs were designed to guide standard oscillating saw osteotomies and later manufactured using 3D printing. Episodic, or bundled payments, is a concept now
familiar to most in the healthcare arena, but the models are often
misunderstood. Under a traditional fee-for-service model, each provider
bills separately for their services which creates financial incentives
to maximise volumes. Under a bundled payment, a single entity, often
referred to as a convener (maybe the hospital, the physician group,
or a third party) assumes the risk through a payer contract for
all services provided within a defined episode of care, and receives
a single (bundled) payment for all services provided for that episode.
The time frame around the intervention is variable, but defined
in advance, as are included and excluded costs. Timing of the actual payment
in a bundle may either be before the episode occurs (prospective
payment model), or after the end of the episode through a reconciliation
(retrospective payment model). In either case, the defined costs

The purpose of the present study was to evaluate the impact of
intravenous tranexamic acid on the reduction of blood loss, transfusion
rate, and early post-operative clinical outcome in total shoulder
arthroplasty. A randomised, placebo-controlled trial which included 54 patients
undergoing unilateral primary stemless anatomical or stemmed reverse
total shoulder arthroplasty was undertaken. Patients received either
100 ml saline (placebo, n = 27), or 100 ml saline together with
1000 mg of tranexamic acid (TXA, n = 27) intravenously prior to
skin incision and during wound closure. Peri-operative blood loss
via an intra-articular drain was recorded and total blood loss was
calculated. The post-operative transfusion rate was documented.

decisions more on dural sac cross-sectional area (DSCA) than on
the morphology of the dural sac. We carried out a survey among members
of three European spine societies. The axial T2-weighted MR images
from ten patients with varying degrees of DSCA and morphological
grades according to the recently described morphological classification
of lumbar spinal stenosis, with DSCA values disclosed in half the
assessed images, were used for evaluation. We provided a clinical
scenario to accompany the images, which were shown to 142 responding
physicians, mainly orthopaedic surgeons but also some neurosurgeons
and others directly involved in treating patients with spinal disorders.

by itself. Prospectively, we performed the procedure on 30 malunited
dorsally-angulated radii using fixed angle volar locked plates without
bone grafting. At the final follow-up, 22 wrists were available.

The direct anterior approach in total hip replacement anatomically offers the chance to minimise soft-tissue trauma because an intermuscular and internervous plane is explored. This motivated us to abandon our previously used transgluteal approach and to adopt the direct anterior approach for total hip replacement. Using MRI, we performed a retrospective comparative study of the direct anterior approach with the transgluteal approach. There were 25 patients in each group. At one year post-operatively all the patients underwent MRI of their replaced hips. A radiologist graded the changes in the soft-tissue signals in the abductor muscles. The groups were similar in terms of age, gender, body mass index, complexity of the reconstruction and absence of symptoms. Detachment of the abductor insertion, partial tears and tendonitis of gluteus medius and minimus, the presence of peri-trochanteric bursal fluid and fatty atrophy of gluteus medius and minimus were significantly less pronounced and less frequent when the direct anterior approach was used. There was no significant difference in the findings regarding tensor fascia lata between the two approaches.
