Osteoid osteoma is treated primarily by radiofrequency
(RF) ablation. However, there is little information about the distribution
of heat in bone during the procedure and its safety. We constructed
a model of osteoid osteoma to assess the distribution of heat in
bone and to define the margins of safety for ablation. Cavities
were drilled in cadaver bovine bones and filled with a liver homogenate
to simulate the tumour matrix. Temperature-sensing probes were placed
in the bone in a radial fashion away from the cavities. RF ablation
was performed 107 times in tumours <
10 mm in diameter (72 of
which were in cortical bone, 35 in cancellous bone), and 41 times
in cortical bone with models >
10 mm in diameter. Significantly
higher temperatures were found in cancellous bone than in cortical
bone (p <
0.05). For lesions up to 10 mm in diameter, in both
bone types, the temperature varied directly with the size of the
tumour (p <
0.05), and inversely with the distance from it. Tumours
of >
10 mm in diameter showed a trend similar to those of smaller
lesions. No temperature rise was seen beyond 12 mm from the edge
of a cortical tumour of any size. Formulae were developed to predict

Unicompartmental knee arthroplasty (UKA) has
advantages over total knee arthroplasty but national joint registries report
a significantly higher revision rate for UKA. As a result, most
surgeons are highly selective, offering UKA only to a small proportion
(up to 5%) of patients requiring arthroplasty of the knee, and consequently
performing few each year. However, surgeons with large UKA practices
have the lowest rates of revision. The overall size of the practice
is often beyond the surgeon’s control, therefore case volume may
only be increased by broadening the indications for surgery, and
offering UKA to a greater proportion of patients requiring arthroplasty
of the knee. The aim of this study was to determine the optimal UKA usage
(defined as the percentage of knee arthroplasty practice comprised
by UKA) to minimise the rate of revision in a sample of 41 986 records
from the for National Joint Registry for England and Wales (NJR). UKA usage has a complex, non-linear relationship with the rate
of revision. Acceptable results are achieved with the use of 20%
or more. Optimal results are achieved with usage between 40% and
60%. Surgeons with the lowest usage (up to 5%) have the highest
rates of revision. With optimal usage, using the most commonly used
implant, five-year survival is 96% (95% confidence interval (CI)
94.9 to 96.0), compared with 90% (95% CI 88.4 to 91.6) with low
usage (5%) previously considered ideal. The rate of revision of UKA is highest with low usage, implying
the use of narrow, and perhaps inappropriate, indications. The widespread
use of broad indications, using appropriate implants, would give
patients the advantages of UKA, without the high rate of revision. Cite this article:
The widespread use of MRI has revolutionised
the diagnostic process for spinal disorders. A typical protocol
for spinal MRI includes T1 and T2 weighted sequences in both axial
and sagittal planes. While such an imaging protocol is appropriate
to detect pathological processes in the vast majority of patients,
a number of additional sequences and advanced techniques are emerging.
The purpose of the article is to discuss both established techniques
that are gaining popularity in the field of spinal imaging and to
introduce some of the more novel ‘advanced’ MRI sequences with examples

We conducted a longitudinal study including patients with the same type of primary hybrid total hip replacement and evaluated patient activity and femoral osteolysis at either five or ten years post-operatively. Activity was measured using the University of California, Los Angeles scale. The primary outcome was the radiological assessment of femoral osteolysis. Secondary outcomes were revision of the femoral component for aseptic loosening and the patients’ quality of life. Of 503 hip replacements in 433 patients with a mean age of 67.7 years (30 to 91), 241 (48%) were seen at five and 262 (52%) at ten years post-operatively. Osteolytic lesions were identified in nine of 166 total hip replacements (5.4%) in patients with low activity, 21 of 279 (7.5%) with moderate activity, and 14 of 58 (24.1%) patients with high activity. The risk of osteolysis increased with participation in a greater number of sporting activities. In multivariate logistic regression adjusting for age, gender, body mass index and the inclination angle of the acetabular component, the adjusted odds ratio for osteolysis comparing high In conclusion, of patients engaged in high activity, 24% had developed femoral osteolysis five to ten years post-operatively.
The early failure and revision of bimodular primary
total hip arthroplasty prostheses requires the identification of the
risk factors for material loss and wear at the taper junctions through
taper wear analysis. Deviations in taper geometries between revised
and pristine modular neck tapers were determined using high resolution
tactile measurements. A new algorithm was developed and validated
to allow the quantitative analysis of material loss, complementing

Because there have been no standard methods to determine pre-operatively
the thickness of resection of the proximal tibia in unicompartmental
knee arthroplasty (UKA), information about the relationship between
the change of limb alignment and the joint line elevation would
be useful for pre-operative planning. The purpose of this study
was to clarify the correlation between the change of limb alignment
and the change of joint line height at the medial compartment after
UKA. A consecutive series of 42 medial UKAs was reviewed retrospectively.
These patients were assessed radiographically both pre- and post-operatively
with standing anteroposterior radiographs. The thickness of bone
resection at the proximal tibia and the distal femur was measured
radiographically. The relationship between the change of femorotibial
angle (δFTA) and the change of joint line height, was analysed.Objectives

This study aims to assess the correlation of CT-based structural
rigidity analysis with mechanically determined axial rigidity in
normal and metabolically diseased rat bone. A total of 30 rats were divided equally into normal, ovariectomized,
and partially nephrectomized groups. Cortical and trabecular bone
segments from each animal underwent micro-CT to assess their average
and minimum axial rigidities using structural rigidity analysis.
Following imaging, all specimens were subjected to uniaxial compression
and assessment of mechanically-derived axial rigidity.Objectives
Methods
Avascular necrosis (AVN) is a serious complication
of high-dose chemotherapy for haematological malignancy in childhood.
In order to describe its incidence and main risk factors and to
evaluate the current treatment options, we reviewed 105 children
with a mean age of 8.25 years (1 to 17.8) who had acute lymphoblastic
or acute myeloid leukaemia, or a non-Hodgkin’s lymphoma. Overall,
eight children (7.6%) developed AVN after a mean of 16.8 months (8
to 49). There were four boys and four girls with a mean age of 14.4
years (9.8 to 16.8) and a total of 18 involved sites, 12 of which
were in the femoral head. All these children were aged >
nine years
(p <
0.001). All had received steroid treatment with a mean cumulative
dose of prednisone of 5967 mg (4425 to 9599) compared with a mean
of 3943 mg (0 to 18 585) for patients without AVN (p = 0.005). No
difference existed between genders and no thrombophilic disorders
were identified. Their initial treatment included 11 core decompressions
and two bipolar hip replacements. Later, two salvage osteotomies
were done and three patients (four hips) eventually needed a total
joint replacement. We conclude that AVN mostly affects the weight-bearing
epiphyses. Its risk increases with age and higher steroid doses.

The optimal management of the tibial slope in
achieving a high flexion angle in posterior-stabilised (PS) total
knee replacement (TKR) is not well understood, and most studies
evaluating the posterior tibial slope have been conducted on cruciate-retaining
TKRs. We analysed pre- and post-operative tibial slope differences,
pre- and post-operative coronal knee alignment and post-operative
maximum flexion angle in 167 patients undergoing 209 TKRs. The mean
pre-operative posterior tibial slope was 8.6° (1.3° to 17°) and
post-operatively it was 8.0° (0.1° to 16.7°). Multiple linear regression
analysis showed that the absolute difference between pre- and post-operative
tibial slope (p <
0.001), post-operative coronal alignment (p
= 0.02) and pre-operative range of movement (p <
0.001) predicted post-operative
flexion. The variance of change in tibial slope became larger as
the post-operative maximum flexion angle decreased. The odds ratio
of having a post-operative flexion angle <
100° was 17.6 if the
slope change was >
2°. Our data suggest that recreation of the anatomical
tibial slope appears to improve maximum flexion after posterior-stabilised

Periprosthetic femoral fracture (PFF) is a potentially
devastating complication after total hip arthroplasty, with historically
high rates of complication and failure because of the technical
challenges of surgery, as well as the prevalence of advanced age
and comorbidity in the patients at risk. This study describes the short-term outcome after revision arthroplasty
using a modular, titanium, tapered, conical stem for PFF in a series
of 38 fractures in 37 patients. The mean age of the cohort was 77 years (47 to 96). A total of
27 patients had an American Society of Anesthesiologists grade of
at least 3. At a mean follow-up of 35 months (4 to 66) the mean
Oxford Hip Score (OHS) was 35 (15 to 48) and comorbidity was significantly
associated with a poorer OHS. All fractures united and no stem needed
to be revised. Three hips in three patients required further surgery
for infection, recurrent PFF and recurrent dislocation and three
other patients required closed manipulation for a single dislocation.
One stem subsided more than 5 mm but then stabilised and required
no further intervention. In this series, a modular, tapered, conical stem provided a versatile
reconstruction solution with a low rate of complications. Cite this article:
This cohort study investigated the influence
of psychological factors, including perception of illness, anxiety
and depression on recovery and functional outcome after total knee
replacement surgery. A total of 100 patients (55 male; 45 female) with a mean age
of 71 (42 to 92) who underwent a primary total knee replacement
for osteoarthritis were recruited into this study. In all 97 participants
completed the six week and 87 the one year follow-up questionnaires. Pre-operatively patients completed the revised Illness Perception
Questionnaire, Hospital Anxiety and Depression Scale and Recovery
Locus of Control Scale. Function was assessed pre-operatively, at
six weeks and one year using Oxford Knee Score (OKS) and the goniometer-measured
range of movement (ROM). The results showed that pre-operative function had the biggest
impact on post-operative outcome for ROM and OKS. In addition questionnaire
variables and depression had an impact on the OKS at six weeks.
Depression and anxiety were also associated with a higher (worse)
knee score at one year but did not influence the ROM at either six weeks
or one year. Recovery from total knee replacement can be difficult to predict.
This study has identified psychological factors that play an important
role in recovery from surgery and functional outcome. These should
be taken into account when considering patients for total knee replacement. Cite this article:
To conduct a pilot randomised controlled trial to evaluate the
feasibility of conducting a larger trial to evaluate the difference
in Victorian Institute of Sports Assessment-Achilles (VISA-A) scores
at six months between patients with Achilles tendinopathy treated
with a platelet-rich plasma (PRP) injection compared with an eccentric
loading programme. Two groups of patients with mid-substance Achilles tendinopathy
were randomised to receive a PRP injection or an eccentric loading
programme. A total of 20 patients were randomised, with a mean age
of 49 years (35 to 66). All outcome measures were recorded at baseline,
six weeks, three months and six months.Objectives
Methods
We investigated the extent to which improved
balance relative to pain relief correlates with the success of total knee
replacement (TKR). A total of 81 patients were recruited to the
study: 16 men (19.8%) and 65 women (80.2%). Of these, 62 patients
(10 men, 52 women) with a mean age of 73 (57 to 83) underwent static
and dynamic assessment of balance pre-operatively and one year post-operatively.
The parameters of balance were quantified using commercially available
and validated equipment. Motor function and self-reported outcome
were also assessed. There was a significant improvement in dynamic balance (p <
0.001) one year after TKR, and better balance correlated with improved
mobility, functional balance and increased health-related quality
of life. As it seems that balance, and not only pain relief, influences
the success of TKR, balance skills should be better addressed during
the post-operative rehabilitation of patients who undergo TKR.
There is little published information on the
health impact of frozen shoulder. The purpose of this study was
to assess the functional and health-related quality of life outcomes
following arthroscopic capsular release (ACR) for contracture of
the shoulder. Between January 2010 and January 2012 all patients
who had failed non-operative treatment including anti-inflammatory
medication, physiotherapy and glenohumeral joint injections for
contracture of the shoulder and who subsequently underwent an ACR
were enrolled in the study. A total of 100 patients were eligible;
68 underwent ACR alone and 32 had ACR with a subacromial decompression
(ASD). ACR resulted in a highly significant improvement in the range
of movement and functional outcome, as measured by the Oxford shoulder
score and EuroQol EQ-5D index. The mean cost of a quality-adjusted
life year (QALY) for an ACR and ACR with an ASD was £2563 and £3189,
respectively. ACR is thus a cost-effective procedure that can restore relatively
normal function and health-related quality of life in most patients
with a contracture of the shoulder within six months after surgery;
and the beneficial effects are not related to the duration of the

Concerns have been raised that deformation of
acetabular shells may disrupt the assembly process of modular prostheses.
In this study we aimed to examine the effect that the strength of
bone has on the amount of deformation of the acetabular shell. The
hypothesis was that stronger bone would result in greater deformation.
A total of 17 acetabular shells were inserted into the acetabula
of eight cadavers, and deformation was measured using an optical
measuring system. Cores of bone from the femoral head were taken
from each cadaver and compressed using a materials testing machine.
The highest peak modulus and yield stress for each cadaver were used
to represent the strength of the bone and compared with the values
for the deformation and the surgeon’s subjective assessment of the
hardness of the bone. The mean deformation of the shell was 129
µm (3 to 340). No correlation was found between deformation and
either the maximum peak modulus (r² = 0.011, t = 0.426, p = 0.676) or
the yield stress (r² = 0.024, t = 0.614, p = 0.549) of the bone.
Although no correlation was found between the strength of the bone
and deformation, the values for the deformation observed could be
sufficient to disrupt the assembly process of modular acetabular

Our aim was to determine the effect of the initial pattern of fracture and the displacement of fragments on the outcome of proximal humeral fractures treated conservatively. We followed 93 consecutive patients prospectively for one year. Final movement and strength were compared with those of the contralateral side. The final American Shoulder and Elbow Society score and the Disabilities of Arm, Shoulder and Hand and Short-Form 36 questionnaires were compared with those provided by the patient on the day of the injury. Radiographs and CT scans with three-dimensional reconstruction were obtained in all patients. The pattern of the fracture and the displacement of individual fragments were analysed and correlated with the final outcome. There were two cases of nonunion and six of avascular necrosis. The majority of the fractures (84 patients; 90%) followed one of the following four patterns: posteromedial (varus) impaction in 50 patients (54%), lateral (valgus) impaction in 13 (14%), isolated greater tuberosity in 15 (16%), and anteromedial impaction fracture in six (6%). Head orientation, impaction of the surgical neck and displacement of the tuberosity correlated strongly with the outcome. In fractures with posteromedial impaction, a poor outcome was noted as the articular surface displaced inferiorly increasing its distance from the acromion. A poorer outcome was noted as a fractured greater tuberosity displaced medially overlapping with the posterior articular surface. Lateral impaction fractures had a worse outcome than other patterns of fracture.
The aim of this study was to investigate the
relationship between the geometry of the proximal femur and the incidence
of intra-operative fracture during uncemented total hip arthroplasty
(THA). We studied the pre-operative CT scans of 100 patients undergoing
THA with an uncemented femoral component. We measured the anteroposterior
and mediolateral dimensions at the level of division of the femoral
neck to calculate the aspect ratio of the femur. Wide variations
in the shape of the femur were observed, from round, to very narrow
elliptic. The femurs of women were narrower than those of men (p
<
0.0001) and small femurs were also narrower than large ones.
Patients with an intra-operative fracture of the calcar had smaller
and narrower femurs than those without a fracture (p <
0.05)
and the implanted Corail stems were smaller in those with a fracture

of division contributes to the understanding of the causation of

This study was designed to provide normal reference values for the centre-edge angle of Wiberg in the Chinese population by measuring 1494 radiographs according to the criteria of Tönnis. The mean angle was 23.1° (4.7° to 46.4°) in childhood (four to nine years), increasing to 28.9° (6° to 48°) in adolescence (10 to 18 years) and reaching 32.8° (13.7° to 58.8°) in adults. The angle was positively correlated with age, increasing by a mean of 0.78° annually before adulthood and by 0.070° annually in adults. A relative upward tendency after middle age might contribute to degenerative changes. No gender difference was discovered. The change in the angle with age was similar to that observed in other ethnic groups, showing moderate differences in the average of individual age groups, but the ethnic differences are not sufficient to explain the variation of incidence of developmental dysplasia of the hip in different races.
Although the importance of sound statistical principles in the design and analysis of data has gained prominence in recent years, biostatistics, the application of statistics to the analysis of biological and medical data, is still a subject which is poorly understood and often mishandled. This review introduces, in the context of orthopaedic research, the terminology and the principles involved in simple data analysis, and outlines areas of medical statistics that have gained prominence in recent years. It also lists and provides an insight into some of the more common errors that occur in published orthopaedic journals and which are frequently encountered at the review stage in papers submitted to the
Vascularised fibular grafts (VFGs ) are a valuable
surgical technique in limb salvage after resection of a tumour.
The primary objective of this multicentre study was to assess the
risk factors for failure and complications for using a VFG after
resection of a tumour. The study involved 74 consecutive patients (45 men and 29 women
with mean age of 23 years (1 to 64) from four tertiary centres for
orthopaedic oncology who underwent reconstruction using a VFG after
resection of a tumour between 1996 and 2011. There were 52 primary
and 22 secondary reconstructions. The mean follow-up was 77 months
(10 to 195). In all, 69 patients (93%) had successful limb salvage; all of
these united and 65 (88%) showed hypertrophy of the graft. The mean
time to union differed between those involving the upper (28 weeks;
12 to 96) and lower limbs (44 weeks; 12 to 250). Fracture occurred
in 11 (15%), and nonunion in 14 (19%) patients. In 35 patients (47%) at least one complication arose, with a
greater proportion in lower limb reconstructions, non-bridging osteosynthesis,
and in children. These complications resulted in revision surgery
in 26 patients (35%). VFG is a successful and durable technique for reconstruction
of a defect in bone after resection of a tumour, but is accompanied
by a significant risk of complications, that often require revision
surgery. Union was not markedly influenced by the need for chemo-
or radiotherapy, but should not be expected during chemotherapy.
Therefore, restricted weight-bearing within this period is advocated. Cite this article:
