To study the effect of hyaluronic acid (HA) on local anaesthetic
chondrotoxicity Chondrocytes were harvested from bovine femoral condyle cartilage
and isolated using collagenase-containing media. At 24 hours after
seeding 15 000 cells per well onto a 96-well plate, chondrocytes
were treated with media (DMEM/F12 + ITS), PBS, 1:1 lidocaine (2%):PBS,
1:1 bupivacaine (0.5%):PBS, 1:1 lidocaine (2%):HA, 1:1 bupivacaine (0.
5%):HA, or 1:1 HA:PBS for one hour. Following treatment, groups
had conditions removed and 24-hour incubation. Cell viability was
assessed using PrestoBlue and confirmed visually using fluorescence
microscopy.Objective

Muscle atrophy has been demonstrated in patients suffering from osteoarthritis of the hip, but little is known about muscular recovery after total hip replacement (THR). A total of 20 patients with unilateral osteoarthritis of the hip were assessed before, six months and two years after THR. The cross-sectional area and radiological density of the muscles of the hip, thigh, calf and back were measured using CT. We hypothesised that the muscles would not recover fully after operation. After two years comparison of the limb with the THR with the healthy limb showed that there was such a reduction in the cross-sectional area in iliopsoas (7.0%; p = 0.006) and the hip adductors (8.4%, p = 0.003) and in the radiological density in gluteus maximus (10.1 Hounsfield units; p <
0.001), gluteus medius/minimus (5.6 Hounsfield units; p = 0.011), iliopsoas (3.9 Hounsfield units; p <
0.001) and the adductors (2.4 Hounsfield units; p = 0.022). Thus, there was persistent muscle atrophy in muscles acting about the hip two years after THR. We suggest that an earlier operation or a more intensive rehabilitation may reverse these changes.
The aims of this study were to examine the repeatability of measurements of bone mineral density (BMD) around a cemented polyethylene Charnley acetabular component using dual-energy x-ray absorptiometry and to determine the longitudinal pattern of change in BMD during the first 24 months after surgery. The precision of measurements of BMD in 19 subjects ranged from 7.7% to 10.8% between regions, using a four-region-of-interest model. A longitudinal study of 27 patients demonstrated a transient decrease in net pelvic BMD during the first 12 months, which recovered to baseline at 24 months. The BMD in the region medial to the dome of the component reduced by between 7% and 10% during the first three months, but recovered to approximately baseline values by two years. Changes in BMD in the pelvis around cemented acetabular components may be measured using dual-energy x-ray absorptiometry. Bone loss after insertion of a cemented Charnley acetabular component is small, transient and occurs mainly at the medial wall of the acetabulum. After two years, bone mass returns to baseline values, with a pattern suggesting a uniform transmission of load to the acetabulum.
Clinical experience of impaction bone grafting for revision knee arthroplasty is limited, with initial stability of the tibial tray emerging as a major concern. The length of the stem and its diameter have been altered to improve stability. Our aim was to investigate the effect of the type of stem, support of the rim and graft impaction on early stability of the tray. We developed a system for impaction grafting of trays which we used with morsellised bone in artificial tibiae. Trays with short, long thick or long thin stems were implanted, with or without support of the rim. They were cyclically loaded while measuring relative movement. Long-stemmed trays migrated 4.5 times less than short-stemmed trays, regardless of diameter. Those with support migrated 2.8 times less than those without. The migration of short-stemmed trays correlated inversely with the density of the impacted groups. That of impaction-grafted tibial trays was in the range reported for uncemented primary trays. Movements of short-stemmed trays without cortical support were largest and sensitive to the degree of compaction of the graft. If support of the rim was sufficient or a long stem was used, impacted morsellised bone graft achieved adequate initial stability.
Balancing service provision and surgical training is a challenging issue that affects all healthcare systems. A multicentre prospective study of 1501 total hip replacements was undertaken to investigate whether there is an association between surgical outcome and the grade of the operating surgeon, and whether there is any difference in outcome if surgeons’ assistants assist with the operation, rather than orthopaedic trainees. The primary outcome measure was the change in the Oxford hip score (OHS) at five years. Secondary outcomes included the rate of revision and dislocation, operating time, and length of hospital stay. There was no significant difference in ΔOHS or complication rates between operations undertaken by trainers and trainees, or those at which surgeons’ assistants and trainees were the assistant. However, there was a significant difference in the duration of surgery, with a mean reduction of 28 minutes in those in which a surgeons’ assistant was the assistant. This study provides evidence that total hip replacements can be performed safely and effectively by appropriately trained surgeons in training, and that there are potential benefits of using surgeons’ assistants in orthopaedic surgery.
In this study of 41 patients, we used proteomic, Western blot and immunohistochemical analyses to show that several reactive oxygen species scavenging enzymes are expressed differentially in patients with primary osteoarthritis and those with non-loosening and aseptic loosening after total hip replacement (THR). The patients were grouped as A (n = 16, primary THR), B (n = 10, fixed THR but requiring revision for polyethylene wear) and C (n = 15, requiring revision due to aseptic loosening) to verify the involvement of the identified targets in aseptic loosening. When compared with Groups A and B, Group C patients exhibited significant up-regulation of transthyretin and superoxide dismutase 3, but down-regulation of glutathione peroxidase 2 in their hip synovial fluids. Also, higher levels of superoxide dismutase 2 and peroxiredoxin 2, but not superoxide dismutase 1, catalase and glutathione perioxidase 1, were consistently detected in the hip capsules of Group C patients. We propose that dysregulated reactive oxygen species-related enzymes may play an important role in the pathogenesis and progression of aseptic loosening after THR.
We have evaluated the clinical effectiveness
of a metal resurfacing inlay implant for osteochondral defects of
the medial talar dome after failed previous surgical treatment.
We prospectively studied 20 consecutive patients with a mean age
of 38 years (20 to 60), for a mean of three years (2 to 5) post-surgery.
There was statistically significant reduction of pain in each of
four situations (i.e., rest, walking, stair climbing and running;
p ≤ 0.01). The median American Orthopaedic Foot and Ankle Society
ankle-hindfoot score improved from 62 (interquartile range (IQR)
46 to 72) pre-operatively to 87 (IQR 75 to 95) at final follow-up
(p <
0.001). The Foot and Ankle Outcome Score improved on all
subscales (p ≤ 0.03). The mean Short-Form 36 physical component
scale improved from 36 (23 to 50) pre-operatively to 45 (29 to 55)
at final follow-up (p = 0.001); the mental component scale did not
change significantly. On radiographs, progressive degenerative changes
of the opposing tibial plafond were observed in two patients. One
patient required additional surgery for the osteochondral defect.
This study shows that a metal implant is a promising treatment for
osteochondral defects of the medial talar dome after failed previous

A consecutive series of 372 patients who underwent surgery for disorders of the rotator cuff involving arthroscopic subacromial decompression and open or arthroscopic repairs of the cuff were prospectively investigated as to the comparability of subjective and objective assessment scores of shoulder function. Assessments were made before operation and at 3, 6, 12, 18 and 24 months after surgery using the Disabilities of the Arm, Shoulder, and Hand score, the Oxford shoulder score and the Constant-Murley score, which was used as a reference. All scores were standardised to a scale of 0 to 100 for comparison. Statistical analysis compared the post-operative course and the mean score for the subjective Disabilities to the Arm, Shoulder and Hand score and Oxford shoulder score, with the objective Constant score at each interval. A strong correlation was evident between both subjective scores and the Constant score. We concluded that both the subjective scores would be useful substitutes for the Constant score, obviating the need for a trained investigator and the specialist equipment required to perform the Constant score.
Objective patellar instability has been correlated with dysplasia of the femoral trochlea. This This study has provided objective biomechanical data to support the use of trochleoplasty in the treatment of patellar instability associated with femoral trochlear dysplasia.
Biomechanical studies involving all-wire and hybrid types of circular frame have shown that oblique tibial fractures remain unstable when they are loaded. We have assessed a range of techniques for enhancing the fixation of these fractures. Eight models were constructed using Sawbones tibiae and standard Sheffield ring fixators, to which six additional fixation techniques were applied sequentially. The major component of displacement was shear along the obliquity of the fracture. This was the most sensitive to any change in the method of fixation. All additional fixation systems were found to reduce shear movement significantly, the most effective being push-pull wires and arched wires with a three-hole bend. Less effective systems included an additional half pin and arched wires with a shallower arc. Angled pins were more effective at reducing shear than transverse pins. The choice of additional fixation should be made after consideration of both the amount of stability required and the practicalities of applying the method to a particular fracture.
The most prevalent disorders of the shoulder are related to the
muscles of rotator cuff. In order to develop a mechanical method
for the evaluation of the rotator cuff muscles, we created a database
of isometric force generation by the rotator cuff muscles in normal
adult population. We hypothesised the existence of variations according
to age, gender and dominancy of limb. A total of 400 healthy adult volunteers were tested, classified
into groups of 50 men and women for each decade of life. Maximal
isometric force was measured at standardised positions for supraspinatus,
infraspinatus and subscapularis muscles in both shoulders in every
person. Torque of the force was calculated and normalised to lean body
mass. The profiles of mean torque-time curves for each age and gender
group were compared.Objectives

The outcome of high tibial osteotomy (HTO) deteriorates
with time, and additional procedures may be required. The aim of
this study was to compare the clinical and radiological outcomes
between unicompartmental knee replacement (UKR) and total knee replacement
(TKR) after HTO as well as after primary UKR. A total of 63 patients (63
knees) were studied retrospectively and divided into three groups:
UKR after HTO (group A; n = 22), TKR after HTO (group B; n = 18)
and primary UKR (group C; n = 22). The Oxford knee score (OKS),
Knee Society score (KSS), hip–knee–ankle angles, mechanical axis
and patellar height were evaluated pre- and post-operatively. At
a mean of 64 months (19 to 180) post-operatively the mean OKS was
43.8 (33 to 49), 43.3 (30 to 48) and 42.5 (29 to 48) for groups
A, B and C, respectively (p = 0.73). The mean KSS knee score was
88.8 (54 to 100), 88.11 (51 to 100) and 85.3 (45 to 100) for groups
A, B and C, respectively (p = 0.65), and the mean KSS function score
was 85.0 (50 to 100) in group A, 85.8 (20 to 100) in group B and
79.3 (50 to 100) in group C (p = 0.48). Radiologically the results
were comparable for all groups except for patellar height, with
a higher incidence of patella infra following a previous HTO (p

We have evaluated four different fixation techniques for the reconstruction of a standard Mason type-III fracture of the radial head in a sawbone model. The outcome measurements were the quality of the reduction, and stability. A total of 96 fractures was created. Six surgeons were involved in the study and each reconstructed 16 fractures with 1.6 mm fine-threaded wires (Fragment Fixation System (FFS)), T-miniplates, 2 mm miniscrews and 2 mm Kirschner (K-) wires; four fractures being allocated to each method using a standard reconstruction procedure. The quality of the reduction was measured after definitive fixation. Biomechanical testing was performed using a transverse plane shear load in two directions to the implants (parallel and perpendicular) with respect to ultimate failure load and displacement at 50 N. A significantly better quality of reduction was achieved using the FFS wires (Tukey’s The ultimate failure load was similar for the FFS wires (parallel, 196.8 N ( The fixation of a standard Mason type-III fracture in a sawbone model using the FFS system provides a better quality of reduction than that when using conventional techniques. There was a significantly better stability using FFS implants, miniscrews and K-wires than when using miniplates.
We report the outcome at a mean of 93 months (73 to 110) of 71 patients with an acute fracture of the scaphoid who were randomised to Herbert screw fixation (35) or below-elbow plaster cast immobilisation (36). These 71 patients represent the majority of a randomised series of 88 patients whose short-term outcome has previously been reported. Those patients available for later review were similar in age, gender and hand dominance. There was no statistical difference in symptoms and disability as assessed by the mean Patient Evaluation Measure (p = 0.4), or mean Patient-Rated Wrist Evaluation (p = 0.9), the mean range of movement of the wrist (p = 0.4), mean grip strength (p = 0.8), or mean pinch strength (p = 0.4). Radiographs were available from the final review for 59 patients. Osteoarthritic changes were seen in the scaphotrapezial and radioscaphoid joints in eight (13.5%) and six patients (10.2%), respectively. Three patients had asymptomatic lucency surrounding the screw. One non-operatively treated patient developed nonunion with avascular necrosis. In five patients who were treated non-operatively (16%) there was an abnormal scapholunate angle ( >
60°), but in four of these patients this finding was asymptomatic. No medium-term difference in function or radiological outcome was identified between the two treatment groups.
We hypothesised that a large acromial cover with
an upwardly tilted glenoid fossa would be associated with degenerative
rotator cuff tears (RCTs), and conversely, that a short acromion
with an inferiorly inclined glenoid would be associated with glenohumeral
osteoarthritis (OA). This hypothesis was tested using a new radiological parameter,
the critical shoulder angle (CSA), which combines the measurements
of inclination of the glenoid and the lateral extension of the acromion
(the acromion index). The CSA was measured on standardised radiographs of three groups:
1) a control group of 94 asymptomatic shoulders with normal rotator
cuffs and no OA; 2) a group of 102 shoulders with MRI-documented
full-thickness RCTs without OA; and 3) a group of 102 shoulders
with primary OA and no RCTs noted during total shoulder replacement.
The mean CSA was 33.1° (26.8° to 38.6°) in the control group, 38.0°
(29.5° to 43.5°) in the RCT group and 28.1° (18.6° to 35.8°) in
the OA group. Of patients with a CSA >
35°, 84% were in the RCT
group and of those with a CSA <
30°, 93% were in the OA group. We therefore concluded that primary glenohumeral OA is associated
with significantly smaller degenerative RCTs with significantly
larger CSAs than asymptomatic shoulders without these pathologies.
These findings suggest that individual quantitative anatomy may
imply biomechanics that are likely to induce specific types of degenerative

Minimally invasive total knee replacement (MIS-TKR)
has been reported to have better early recovery than conventional
TKR. Quadriceps-sparing (QS) TKR is the least invasive MIS procedure,
but it is technically demanding with higher reported rates of complications
and outliers. This study was designed to compare the early clinical
and radiological outcomes of TKR performed by an experienced surgeon
using the QS approach with or without navigational assistance (NA),
or using a mini-medial parapatellar (MP) approach. In all, 100 patients
completed a minimum two-year follow-up: 30 in the NA-QS group, 35
in the QS group, and 35 in the MP group. There were no significant
differences in clinical outcome in terms of ability to perform a
straight-leg raise at 24 hours (p = 0.700), knee score (p = 0.952),
functional score (p = 0.229) and range of movement (p = 0.732) among
the groups. The number of outliers for all three radiological parameters
of mechanical axis, frontal femoral component alignment and frontal
tibial component alignment was significantly lower in the NA-QS
group than in the QS group (p = 0.008), but no outlier was found
in the MP group. In conclusion, even after the surgeon completed a substantial
number of cases before the commencement of this study, the supplementary
intra-operative use of computer-assisted navigation with QS-TKR
still gave inferior radiological results and longer operating time,
with a similar outcome at two years when compared with a MP approach. Cite this article:
It has been suggested that extracorporeal shockwave
therapy is a safe and effective treatment for pain relief from recalcitrant
plantar fasciopathy (PF). However, the changes in gait and associated
biomechanical parameters have not been well characterised. We recruited
12 female patients with recalcitrant PF who had a mean age of 59
years (50 to 70) and mean body mass index of 25 kg/m2 (22
to 30). The patients reported a mean duration of symptoms of 9.3
months (6 to 15). Shockwave therapy consisting of 1500 impulses
(energy flux density 0.26 mJ/mm2) was applied for three
sessions, each three weeks apart. A pain visual analogue scale (VAS)
rating, plantar pressure assessment and motion analysis were carried
out before and nine weeks after first shock wave therapy. It was demonstrated
that patients increased their walking velocity and cadence as well
indicating a decrease in pain after shockwave therapy. In the symptomatic
foot, the peak contact pressure over the forefoot increased and
the contact area over the digits decreased. The total foot impulse
also decreased as did stance duration. The duration the centre of
pressure remained in the hindfoot increased in the symptomatic foot
after shockwave therapy. The differences in centre of pressure trajectory
at baseline decreased at final follow-up. In conclusion, shockwave
therapy not only decreased the pain VAS rating but also improved
the gait parameters of the symptomatic foot in PF patients. Cite this article:
Different calcaneal plates with locked screws were compared in an experimental model of a calcaneal fracture. Four plate models were tested, three with uniaxially-locked screws (Synthes, Newdeal, Darco), and one with polyaxially-locked screws (90° ± 15°) (Rimbus). Synthetic calcanei were osteotomised to create a fracture model and then fixed with the plates and screws. Seven specimens for each plate model were subjected to cyclic loading (preload 20 N, 1000 cycles at 800 N, 0.75 mm/s), and load to failure (0.75 mm/s). During cyclic loading, the plate with polyaxially-locked screws (Rimbus) showed significantly lower displacement in the primary loading direction than the plates with uniaxially-locked screws (mean values of maximum displacement during cyclic loading: Rimbus, 3.13 mm ( The increased stability of a plate with polyaxially-locked screws demonstrated during cyclic loading compared with plates with uniaxially-locked screws may be beneficial for clinical use.
We have investigated the role of the penetration of saline on the shear strength of the cement-stem interface for stems inserted at room temperature and those preheated to 37°C using a variety of commercial bone cements. Immersion in saline for two weeks at 37°C reduced interfacial strength by 56% to 88% after insertion at room temperature and by 28% to 49% after preheating of the stem. The reduction in porosity as a result of preheating ranged from 71% to 100%. Increased porosity correlated with a reduction in shear strength after immersion in saline (r = 0.839, p <
0.01) indicating that interfacial porosity may act as a fluid conduit.
This study investigates the use of porous biphasic ceramics as graft extenders in impaction grafting of the femur during revision hip surgery. Impaction grafting of the femur was performed in four groups of sheep. Group one received pure allograft, group two 50% allograft and 50% BoneSave, group three 50% allograft and 50% BoneSave type 2 and group four 10% allograft and 90% BoneSave as the graft material. Function was assessed using an index of pre- and post-operative peak vertical ground reaction force ratios. Changes in bone mineral density were measured by dual energy X ray absorptiometry (DEXA) scanning. Loosening and subsidence were assessed radiographically and by histological examination of the explanted specimens. There was no statistically significant difference between the four groups after 18 months of unrestricted functional loading for all outcome measures.
