To evaluate the neck strength of school-aged rugby players, and
to define the relationship with proxy physical measures with a view
to predicting neck strength. Cross-sectional cohort study involving 382 rugby playing schoolchildren
at three Scottish schools (all male, aged between 12 and 18 years).
Outcome measures included maximal isometric neck extension, weight,
height, grip strength, cervical range of movement and neck circumference.Objectives

This study demonstrates a significant correlation
between the American Knee Society (AKS) Clinical Rating System and
the Oxford Knee Score (OKS) and provides a validated prediction
tool to estimate score conversion. A total of 1022 patients were prospectively clinically assessed
five years after TKR and completed AKS assessments and an OKS questionnaire.
Multivariate regression analysis demonstrated significant correlations between
OKS and the AKS knee and function scores but a stronger correlation
(r = 0.68, p <
0.001) when using the sum of the AKS knee and
function scores. Addition of body mass index and age (other statistically
significant predictors of OKS) to the algorithm did not significantly
increase the predictive value. The simple regression model was used to predict the OKS in a
group of 236 patients who were clinically assessed nine to ten years
after TKR using the AKS system. The predicted OKS was compared with
actual OKS in the second group. Intra-class correlation demonstrated
excellent reliability (r = 0.81, 95% confidence intervals 0.75 to
0.85) for the combined knee and function score when used to predict
OKS. Our findings will facilitate comparison of outcome data from
studies and registries using either the OKS or the AKS scores and
may also be of value for those undertaking meta-analyses and systematic

Advanced MRI cartilage imaging such as T1-rho
(T1ρ) for the diagnosis of early cartilage degradation prior to morpholgic
radiological changes may provide prognostic information in the management
of joint disease. This study aimed first to determine the normal
T1ρ profile of cartilage within the hip, and secondly to identify
any differences in T1ρ profile between the normal and symptomatic
femoroacetabular impingement (FAI) hip. Ten patients with cam-type
FAI (seven male and three female, mean age 35.9 years (28 to 48))
and ten control patients (four male and six female, mean age 30.6
years (22 to 35)) underwent 1.5T T1ρ MRI of a single hip. Mean T1ρ relaxation
times for full thickness and each of the three equal cartilage thickness
layers were calculated and compared between the groups. The mean
T1ρ relaxation times for full cartilage thickness of control and
FAI hips were similar (37.17 ms ( These results suggest that 1.5T T1ρ MRI can detect acetabular
hyaline cartilage changes in patients with FAI.
We investigated the incidence and risk factors
for the development of avascular necrosis (AVN) of the femoral head in
the course of treatment of children with cerebral palsy (CP) and
dislocation of the hip. All underwent open reduction, proximal femoral
and Dega pelvic osteotomy. The inclusion criteria were: a predominantly
spastic form of CP, dislocation of the hip (migration percentage,
MP >
80%), Gross Motor Function Classification System, (GMFCS) grade
IV to V, a primary surgical procedure and follow-up of >
one year. There were 81 consecutive children (40 girls and 41 boys) in
the study. Their mean age was nine years (3.5 to 13.8) and mean
follow-up was 5.5 years (1.6 to 15.1). Radiological evaluation included
measurement of the MP, the acetabular index (AI), the epiphyseal
shaft angle (ESA) and the pelvic femoral angle (PFA). The presence
and grade of AVN were assessed radiologically according to the Kruczynski
classification. Signs of AVN (grades I to V) were seen in 79 hips (68.7%). A
total of 23 hips (18%) were classified between grades III and V. Although open reduction of the hip combined with femoral and
Dega osteotomy is an effective form of treatment for children with
CP and dislocation of the hip, there were signs of avascular necrosis
in about two-thirds of the children. There was a strong correlation

We assessed the effect of social deprivation
upon the Oxford knee score (OKS), the Short-Form 12 (SF-12) and patient
satisfaction after total knee replacement (TKR). An analysis of
966 patients undergoing primary TKR for symptomatic osteoarthritis
(OA) was performed. Social deprivation was assessed using the Scottish
Index of Multiple Deprivation. Those patients that were most deprived
underwent surgery at an earlier age (p = 0.018), were more likely
to be female (p = 0.046), to endure more comorbidities (p = 0.04)
and to suffer worse pain and function according to the OKS (p <
0.001). In addition, deprivation was also associated with poor mental
health (p = 0.002), which was assessed using the mental component
(MCS) of the SF-12 score. Multivariable analysis was used to identify
independent predictors of outcome at one year. Pre-operative OKS,
SF-12 MCS, back pain, and four or more comorbidities were independent
predictors of improvement in the OKS (all p <

A variety of radiological methods of measuring
version of the acetabular component after total hip replacement (THR)
have been described. The aim of this study was to evaluate the reliability
and validity of six methods (those of Lewinnek; Widmer; Hassan et
al; Ackland, Bourne and Uhthoff; Liaw et al; and Woo and Morrey)
that are currently in use. In 36 consecutive patients who underwent
THR, version of the acetabular component was measured by three independent
examiners on plain radiographs using these six methods and compared
with measurements using CT scans. The intra- and interobserver reliabilities
of each measurement were estimated. All measurements on both radiographs
and CT scans had excellent intra- and interobserver reliability
and the results from each of the six methods correlated well with
the CT measurements. However, measurements made using the methods
of Widmer and of Ackland, Bourne and Uhthoff were significantly
different from the CT measurements (both p <
0.001), whereas
measurements made using the remaining four methods were similar
to the CT measurements. With regard to reliability and convergent
validity, we recommend the use of the methods described by Lewinnek,
Hassan et al, Liaw et al and Woo and Morrey for measurement of version
of the acetabular component.
We undertook clinical and ultrasonographic examination of the shoulders of 420 asymptomatic volunteers aged between 50 and 79 years. MRI was performed in selected cases. Full-thickness tears of the rotator cuff were detected in 32 subjects (7.6%). The prevalence increased with age as follows: 50 to 59 years, 2.1%; 60 to 69 years, 5.7%; and 70 to 79 years, 15%. The mean size of the tear was less than 3 cm and tear localisation was limited to the supraspinatus tendon in most cases (78%). The strength of flexion was reduced significantly in the group with tears (p = 0.01). Asymptomatic tears of the rotator cuff should be regarded as part of the normal ageing process in the elderly but may be less common than hitherto believed.
We carried out a prospective study over a period of 12 months to measure the exposure to radiation of the hands of a dedicated foot and ankle surgeon. A thermoluminescent dosimeter ring (TLD) was used to measure the cumulative dose of radiation. Fluoroscopy was used in operations on the foot and ankle. The total screening time was 3028 s, with a mean time per procedure of 37.4 s (0.6 to 197). This correlated positively with the number of procedures performed (r = 0.92, p <
0.001), and with the dose of radiation in both the left (r = 0.85, p = 0.0005) and right TLDs (r = 0.59, p = 0.419). There was no significant difference in the dose of radiation between the two hands ( This is a simple and convenient method for evaluating the exposure of a single surgeon to radiation. The radiation detected was well below the annual dose limit set by the International Commission on Radiological Protection.
Operative fixation is the treatment of choice for a rupture of the distal tendon of biceps. A variety of techniques have been described including transosseous sutures and suture anchors. The poor quality of the bone of the radial tuberosity might affect the load to failure of the tendon repair in early rehabilitation. The aim of this study was to determine the loads to failure of different techniques of fixation and to investigate their association with the bone mineral density of the radial tuberosity. Peripheral quantitative computed tomography was carried out to measure the trabecular and cortical bone mineral density of the radial tuberosity in 40 cadaver specimens. The loads to failure in four different techniques of fixation were determined. The Endobutton-based method showed the highest failure load at 270 N ( The transosseous technique is an easy and cost-saving procedure for fixation of the distal biceps tendon. TwinFix-QuickT 5.0 mm had significantly lower failure loads, which might affect early rehabilitation, particularly in older patients.
This study aims to assess the correlation of CT-based structural
rigidity analysis with mechanically determined axial rigidity in
normal and metabolically diseased rat bone. A total of 30 rats were divided equally into normal, ovariectomized,
and partially nephrectomized groups. Cortical and trabecular bone
segments from each animal underwent micro-CT to assess their average
and minimum axial rigidities using structural rigidity analysis.
Following imaging, all specimens were subjected to uniaxial compression
and assessment of mechanically-derived axial rigidity.Objectives
Methods
Impingement syndrome in the shoulder has generally been considered to be a clinical condition of mechanical origin. However, anomalies exist between the pathology in the subacromial space and the degree of pain experienced. These may be explained by variations in the processing of nociceptive inputs between different patients. We investigated the evidence for augmented pain transmission (central sensitisation) in patients with impingement, and the relationship between pre-operative central sensitisation and the outcomes following arthroscopic subacromial decompression. We recruited 17 patients with unilateral impingement of the shoulder and 17 age- and gender-matched controls, all of whom underwent quantitative sensory testing to detect thresholds for mechanical stimuli, distinctions between sharp and blunt punctate stimuli, and heat pain. Additionally Oxford shoulder scores to assess pain and function, and PainDETECT questionnaires to identify ‘neuropathic’ and referred symptoms were completed. Patients completed these questionnaires pre-operatively and three months post-operatively. A significant proportion of patients awaiting subacromial decompression had referred pain radiating down the arm and had significant hyperalgesia to punctate stimulus of the skin compared with controls (unpaired The presence of either hyperalgesia or referred pain pre-operatively resulted in a significantly worse outcome from decompression three months after surgery (unpaired These observations confirm the presence of central sensitisation in a proportion of patients with shoulder pain associated with impingement. Also, if patients had relatively high levels of central sensitisation pre-operatively, as indicated by higher levels of punctate hyperalgesia and/or referred pain, the outcome three months after subacromial decompression was significantly worse.
We undertook a prospective study to analyse the
outcome of 48 malunited pronation-external rotation fractures of the
ankle in 48 patients (25 females and 23 males) with a mean age of
45 years (21 to 69), treated by realignment osteotomies. The interval
between the injury and reconstruction was a mean of 20.2 months
(3 to 98). In all patients, valgus malalignment of the distal tibia and
malunion of the fibula were corrected. In some patients, additional
osteotomies were performed. Patients were reviewed regularly, and
the mean follow-up was 7.1 years (2 to 15). Good or excellent results were obtained in 42 patients (87.5%)
with the benefit being maintained over time. Congruent ankles without
a tilted talus (Takakura stage 0 and 1) were obtained in all but
five cases. One patient required total ankle replacement.
We report the outcome of 32 patients (37 knees) who underwent hemicallostasis with a dynamic external fixator for osteoarthritis of the medial compartment of the knee. There were 16 men (19 knees) and 16 women (18 knees) with a mean age at operation of 54.6 years (27 to 72). The aim was to achieve a valgus overcorrection of 2° to 8° or mechanical axis at 62.5% (± 12.5%). At a mean follow-up of 62.8 months (51 to 81) there was no change in the mean range of movement, and no statistically significant difference in the Insall-Salvati index or tibial slope (p = 0.11 and p = 0.15, respectively). The mean hip-knee-ankle angle changed from 190.6 (183° to 197°) to 176.0° (171° to 181°), with a mean final position of the mechanical axis of 58.5% (35.1% to 71.2%). The desired alignment was attained in 31 of 37 (84%) knees. There were 21 excellent, 13 good, two fair and one poor result according to the Oxford knee score with no correlation between age and final score. This score was at its best at one year with a statistically significant deterioration at two years (p = 0.001) followed by a small but not statistically significant deterioration until the final follow-up (p = 0.17). All the knees with Ahlback grade 1 osteoarthritis had excellent or good results. Complications included pin tract infections involving 16.4% of all pins used, delayed union in two, knee stiffness in four, fracture of the lateral cortex in one and ring sequestrum in one. In conclusion, hemicallostasis provides precision in attaining the desired alignment without interfering with tibial slope or patellar height, and is relatively free of serious complications.
Up to 20% of patients are not satisfied with the outcome following total knee replacement (TKR). This study investigated the pre- and post-operative predictors of dissatisfaction in a large cohort of patients undergoing TKR. We assessed 1217 consecutive patients between 2006 and 2008 both before operation and six months after, using the Short-form (SF)-12 health questionnaire and the Oxford Knee Score. Detailed information concerning comorbidity was also gathered. Satisfaction was measured at one year when 18.6% (226 of 1217) of patients were unsure or dissatisfied with their replacement and 81.4% (911 of 1217) were satisfied or very satisfied. Multivariate regression analysis was performed to identify independent predictors of dissatisfaction. Significant (p <
0.001) predictors at one year included the pre-operative SF-12 mental component score, depression and pain in other joints, the six-month SF-12 score and poorer improvement in the pain element of the Oxford Knee Score. Patient expectations were highly correlated with satisfaction. Satisfaction following TKR is multifactorial. Managing the expectations and mental health of the patients may reduce dissatisfaction. However, the most significant predictor of dissatisfaction is a painful total knee replacement.
Patients with acetabular dysplasia commonly undergo
peri-acetabular osteotomy after skeletal maturity to reduce the risk
of the late development of osteoarthritis. Several studies have
suggested that deformity of the femoral head influences the long-term
outcome. We radiologically examined 224 hips in 112 patients with
acetabular dysplasia and early-stage osteoarthritis. There were
103 women and nine men with a mean age of 37.6 years (18 to 49).
A total of 201 hips were placed in the acetabular dysplasia group
and 23 in a normal group. The centre–edge angle and acetabular head
index were significantly smaller (both p <
0.001), and the acetabular
angle, acetabular roof angle and roundness index were significantly
greater in the acetabular dysplasia group than those in the normal
group (all p <
0.001). There were significant correlations between
the roundness index and other parameters. Femoral head shape may

Version of the femoral stem is an important factor
influencing the risk of dislocation after total hip replacement (THR)
as well as the position of the acetabular component. However, there
is no radiological method of measuring stem anteversion described
in the literature. We propose a radiological method to measure stem
version and have assessed its reliability and validity. In 36 patients
who underwent THR, a hip radiograph and CT scan were taken to measure
stem anteversion. The radiograph was a modified Budin view. This
is taken as a posteroanterior radiograph in the sitting position
with 90° hip flexion and 90° knee flexion and 30° hip abduction.
The angle between the stem-neck axis and the posterior intercondylar
line was measured by three independent examiners. The intra- and
interobserver reliabilities of each measurement were examined. The
radiological measurements were compared with the CT measurements

We undertook a prospective study in 51 male patients aged between 17 and 27 years to ascertain whether immobilisation after primary traumatic anterior dislocation of the shoulder in external rotation was more effective than immobilisation in internal rotation in preventing recurrent dislocation in a physically active population. Of the 51 patients, 24 were randomised to be treated by a traditional brace in internal rotation and 27 were immobilised in external rotation of 15° to 20°. After immobilisation, the patients undertook a standard regime of physiotherapy and were then assessed clinically for evidence of instability. When reviewed at a mean of 33.4 months (24 to 48) ten from the external rotation group (37%) and ten from the internal rotation group (41.7%) had sustained a futher dislocation. There was no statistically significant difference (p = 0.74) between the groups. Our findings show that external rotation bracing may not be as effective as previously reported in preventing recurrent anterior dislocation of the shoulder.
We prospectively studied the benefits and risks of prolonged treatment with the Pavlik harness in infants with idiopathic developmental dysplasia of the hip. Bracing was continued as long as abduction improved. It was started at a mean age of four months (1 to 6.9). Outcome measures were the number of successful reductions, the time to reduction, the acetabular index and evidence of avascular necrosis at follow-up at one year. In 50 infants with 62 subluxated and dislocated hips (42 Graf type III and 20 type IV), 37 were reduced successfully with a mean time to reduction of 13.4 weeks ( Prolonged treatment with the Pavlik harness for developmental dysplasia of the hip over the age of one month can be beneficial in type-III hips, but it is unclear as to whether this is the optimal treatment, since it may postpone the need for closed or open reduction to a more unfavourable age. The use of the Pavlik harness in type-IV hips in this age group is questionable, but if used, prolonged bracing is not advised.
Several factors have been implicated in unsatisfactory
results after total hip replacement (THR). We examined whether femoral
offset, as measured on digitised post-operative radiographs, was
associated with pain after THR. The routine post-operative radiographs
of 362 patients (230 women and 132 men, mean age 70.0 years (35.2
to 90.5)) who received primary unilateral THRs of varying designs
were measured after calibration. The femoral offset was calculated
using the known dimensions of the implants to control for femoral
rotation. Femoral offset was categorised into three groups: normal
offset (within 5 mm of the height-adjusted femoral offset), low
offset and high offset. We determined the associations to the absolute
final score and the improvement in the mean Western Ontario and
McMaster Universities osteoarthritis index (WOMAC) pain subscale
scores at three, six, 12 and 24 months, adjusting for confounding
variables. The amount of femoral offset was associated with the mean WOMAC
pain subscale score at all points of follow-up, with the low-offset
group reporting less WOMAC pain than the normal or high-offset groups
