A total of 397 hips were randomised to receive
Metasul metal-on-metal (MoM), metal-on-conventional polyethylene (MoP)
or ceramic-on-polyethylene (CoP) bearings using a cemented triple-tapered
polished femoral component (MS-30). There were 129 MoM hips in 123
patients (39 male and 84 female, mean age 63.3 years (40.7 to 72.9)),
137 MoP hips in 127 patients (39 male and 88 female, mean age 62.8
years (24.5 to 72.7)) and 131 CoP hips in 124 patients (51 male
and 73 female, mean age 63.9 years (30.6 to 73.8)). All acetabular
components were cemented Weber polyethylene components with the
appropriate inlay for the MoM articulation. Clinical evaluation
was undertaken using the Harris hip score (HHS) and radiological
assessments were made at two, five and seven years. The HHS and radiological
analysis were available for 341 hips after seven years. The MoM
group had the lowest mean HHS (p = 0.124), a higher rate of revision
(p <
0.001) and a higher incidence of radiolucent lines in unrevised
hips (p <
0.001). In all, 12 revisions had been performed in
12 patients: eight in the MoM group (four for infection, four for
aseptic loosening, three in the MoP group (one each of infection,
dislocation and pain) and one in the CoP group (infection). Our findings reveal no advantage to the MoM bearing and identified
a higher revision rate and a greater incidence of radiolucent lines
than with the other articulations. We recommend that patients with

The acetabular labrum is a soft-tissue structure
which lines the acetabular rim of the hip joint. Its role in hip
joint biomechanics and joint health has been of particular interest
over the past decade. In normal hip joint biomechanics, the labrum
is crucial in retaining a layer of pressurised intra-articular fluid
for joint lubrication and load support/distribution. Its seal around
the femoral head is further regarded as a contributing to hip stability through
its suction effect. The labrum itself is also important in increasing
contact area thereby reducing contact stress. Given the labrum’s
role in normal hip joint biomechanics, surgical techniques for managing
labral damage are continuously evolving as our understanding of
its anatomy and function continue to progress. The current paper
aims to review the anatomy and biomechanical function of the labrum

We report the long-term survival and functional
outcome of the Birmingham Hip Resurfacing (BHR) in patients aged <
50 years at operation, and explore the factors affecting survival.
Between 1997 and 2006, a total of 447 BHRs were implanted in 393
patients (mean age 41.5 years (14.9 to 49.9)) by one designing surgeon.
The mean follow-up was 10.1 years (5.2 to 14.7), with no loss to
follow-up. In all, 16 hips (3.6%) in 15 patients were revised, giving
an overall cumulative survival of 96.3% (95% confidence interval
(CI) 93.7 to 98.3) at ten years and 94.1% (95% CI 84.9 to 97.3)
at 14 years. Using aseptic revision as the endpoint, the survival
for men with primary osteoarthritis (n = 195) was 100% (95% CI 100
to 100) at both ten years and 14 years, and in women with primary
osteoarthritis (n = 109) it was 96.1% (95% CI 90.1 to 99.9) at ten
years and 91.2% (95% CI 68.6 to 98.7) at 14 years. Female gender
(p = 0.047) and decreasing femoral head size (p = 0.044) were significantly
associated with an increased risk of revision. The median Oxford
hip score (OHS, modified as a percentage with 100% indicating worst
outcome) at last follow-up was 4.2% (46 of 48; interquartile range
(IQR) 0% to 24%) and the median University of California, Los Angeles
(UCLA) score was 6.0 (IQR 5 to 8). Men had significantly better
OHS (p = 0.02) and UCLA scores (p = 0.01) than women. The BHR provides
excellent survival and functional results in men into the second

Until now, there has been no consensus as to whether stemmed
acetabular components are appropriate for use in patients undergoing
revision total hip arthroplasty (THA) who have major acetabular
defects or pelvic discontinuity. We wished to address this deficiency
in the literature. We carried out a retrospective study of 35 patients (six men
and 29 women) with a mean age of 68 years (37 to 87), with major
acetabular defects who underwent revision THA between 2000 and 2012.Aim
Patients and Methods
The ‘jumbo’ acetabular component is now commonly
used in acetabular revision surgery where there is extensive bone
loss. It offers high surface contact, permits weight bearing over
a large area of the pelvis, the need for bone grafting is reduced
and it is usually possible to restore centre of rotation of the
hip. Disadvantages of its use include a technique in which bone
structure may not be restored, a risk of excessive posterior bone
loss during reaming, an obligation to employ screw fixation, limited
bone ingrowth with late failure and high hip centre, leading to increased
risk of dislocation. Contraindications include unaddressed pelvic
dissociation, inability to implant the component with a rim fit,
and an inability to achieve screw fixation. Use in acetabulae with
<
50% bone stock has also been questioned. Published results
have been encouraging in the first decade, with late failures predominantly because
of polyethylene wear and aseptic loosening. Dislocation is the most
common complication of jumbo acetabular revisions, with an incidence
of approximately 10%, and often mandates revision. Based on published results,
a hemispherical component with an enhanced porous coating, highly
cross-linked polyethylene, and a large femoral head appears to represent
the optimum tribology for jumbo acetabular revisions. Cite this article:
We compared a modular neck system with a non-modular system in a cementless anatomical total hip replacement (THR). Each group consisted of 74 hips with developmental hip dysplasia. Both groups had the same cementless acetabular component and the same articulation, which consisted of a conventional polyethylene liner and a 28 mm alumina head. The mean follow-up was 14.5 years (13 to 15), at which point there were significant differences in the mean total Harris hip score (modular/non-modular: 98.6 (64 to 100)/93.8 (68 to 100)), the mean range of abduction (32° (15° to 40°)/28 (0° to 40°)), use of a 10° elevated liner (31%/100%), the incidence of osteolysis (27%/79.7%) and the incidence of equal leg lengths (≥ 6 mm, 92%/61%). There was no disassociation or fracture of the modular neck. The modular system reduces the need for an elevated liner, thereby reducing the incidence of osteolysis. It gives a better range of movement and allows the surgeon to make an accurate adjustment of leg length.
We investigated the development of CT-based bony
radiological parameters associated with femoroacetabular impingement
(FAI) in a paediatric and adolescent population with no known orthopaedic
hip complaints. We retrospectively reformatted and reoriented
225 abdominal CTs into standardised CT pelvic images with neutral
pelvic tilt and inclination (244 female and 206 male hips) in patients
ranging from two to 19 years of age (mean 10.4 years). The Tönnis
angle, acetabular depth ratio, lateral centre–edge angle, acetabular
version and α-angle were assessed. Acetabular measurements demonstrated increased acetabular coverage
with age and/or progressive ossification of the acetabulum. The α-angle
decreased with age and/or progressive cortical bone development
and resultant narrowing of the femoral neck. Cam and pincer morphology
occurred as early as ten and 12 years of age, respectively, and
their prevalence in the adolescent patient population is similar
to that reported in the adult literature. Future aetiological studies
of FAI will need to focus on the early adolescent population. Cite this article:
In Japan, osteoarthritis (OA) of the hip secondary
to acetabular dysplasia is very common, and there are few data concerning
the pathogeneses and incidence of femoroacetabular impingement (FAI).
We have attempted to clarify the radiological prevalence of painful
FAI in a cohort of Japanese patients and to investigate the radiological findings.
We identified 176 symptomatic patients (202 hips) with Tönnis grade
0 or 1 osteoarthritis, whom we prospectively studied between August
2011 and July 2012. There were 61 men (65 hips) and 115 women (137
hips) with a mean age of 51.8 years (11 to 83). Radiological analyses
included the α-angle, centre–edge angle, cross-over sign, pistol
grip deformity and femoral head neck ratio. Of the 202 hips, 79
(39.1%) had acetabular dysplasia, while 80 hips (39.6%) had no known
aetiology. We found evidence of FAI in 60 hips (29.7%). Radiological
FAI findings associated with cam deformity were the most common.

T-cells are considered to play an important role in the inflammatory response causing arthroplasty failure. The study objectives were to investigate the composition and distribution of CD4+ T-cell phenotypes in the peripheral blood (PB) and synovial fluid (SF) of patients undergoing revision surgery for failed metal-on-metal (MoM) and metal-on-polyethylene (MoP) hip arthroplasties, and in patients awaiting total hip arthroplasty. In this prospective case-control study, PB and SF were obtained from 22 patients (23 hips) undergoing revision of MoM (n = 14) and MoP (n = 9) hip arthroplasties, with eight controls provided from primary hip osteoarthritis cases awaiting arthroplasty. Methods
We investigated differences in the location and
mode of labral tears between dysplastic hips and hips with femoroacetabular
impingement (FAI). We also investigated the relationship between
labral tear and adjacent cartilage damage. We retrospectively studied
72 symptomatic hips (in 68 patients: 19 men and 49 women) with radiological
evidence of dysplasia or FAI on high-resolution CT arthrography.
The incidence and location of labral tears and modes of tear associated
with the base of the labrum (Mode 1) or body of the labrum (Mode
2) were compared among FAI, mildly dysplastic and severely dysplastic
hips. The locations predominantly involved with labral tears were
different in FAI and mild dysplastic hips (anterior and anterosuperior
zones) and in severely dysplastic hips (anterosuperior and superior
zones) around the acetabulum. Significant differences were observed
in the prevalence of Mode 1 Hip pathology is significantly related to the locations and modes
of labral tears. Mode 1 tears may be a risk factor for the development

The purpose of this study was to determine patient-reported
outcomes of patients with mild to moderate developmental dysplasia
of the hip (DDH) and femoroacetabular impingement (FAI) undergoing
arthroscopy of the hip in the treatment of chondrolabral pathology.
A total of 28 patients with a centre-edge angle between 15° and
19° were identified from an institutional database. Their mean age
was 34 years (18 to 53), with 12 female and 16 male patients. All
underwent labral treatment and concomitant correction of FAI. There
were nine reoperations, with two patients requiring revision arthroscopy,
two requiring periacetabular osteotomy and five needing total hip arthroplasty. Patients who required further major surgery were more likely
to be older, male, and to have more severe DDH with a larger alpha
angle and decreased joint space. At a mean follow-up of 42 months (24 to 89), the mean modified
Harris hip score improved from 59 (20 to 98) to 82 (45 to 100; p

0.001). The mean Western Ontario and McMaster Universities
Osteoarthritis Index score improved from 30 (1 to 61) to 16 (0 to

0.001). Median patient satisfaction was 9.0/10 (1 to
10). Patients reported excellent improvement in function following
arthroscopy of the hip. This study shows that with proper patient selection, arthroscopy
of the hip can be successful in the young patient with mild to moderate

Dislocation is a common and well-studied complication after total hip replacement. However, subluxation, which we define as a clinically recognised episode of incomplete movement of the femoral head outside the acetabulum with spontaneous reduction, has not been studied previously. Out of a total of 2521 hip replacements performed over 12 years by one surgeon, 30 patients experienced subluxations which occurred in 31 arthroplasties. Data were collected prospectively with a minimum follow-up of two years. Subluxation occurred significantly more frequently after revision than after primary hip replacement, and resolved in 19 of 31 cases (61.3%). In six of the 31 hips (19.4%) the patient subsequently dislocated the affected hip, and in six hips (19.4%) intermittent subluxation continued. Four patients had a revision operation for instability, three for recurrent dislocation and one for recurrent subluxation. Clinical and radiological comparisons with a matched group of stable total hips showed no correlation with demographic or radiological parameters. Patients with subluxing hips reported significantly more concern that their hip would dislocate, more often changed their behaviour to prevent instability and had lower postoperative Harris hip scores than patients with stable replacements.
We report the clinical and radiological outcomes
of a series of contemporary cementless ceramic-on-ceramic total hip
replacements (THRs) at ten years in patients aged ≤ 55 years of
age. Pre- and post-operative activity levels are described. A total
of 120 consecutive ceramic cementless THRs were performed at a single
centre in 110 patients from 1997 to 1999. The mean age of the patients
at operation was 45 years (20 to 55). At ten years, four patients
had died and six were lost to follow-up, comprising ten hips. The
mean post-operative Harris hip score was 94.7 (55 to 100). Radiological
analysis was undertaken in 90 available THRs of the surviving 106
hips at final review: all had evidence of stable bony ingrowth,
with no cases of osteolysis. Wear was undetectable. There were four
revisions. The survival for both components with revision for any
cause as an endpoint was 96.5% (95% confidence interval 94.5 to
98.7). The mean modified University of California, Los Angeles activity
level rose from a mean of 6.4 (4 to 10) pre-operatively to 9.0 (6
to 10) at the ten-year post-operative period. Alumina ceramic-on-ceramic bearings in cementless primary THR
in this series have resulted in good clinical and radiological outcomes
with undetectable rates of wear and excellent function in the demanding

We report the results at a mean of 24.3 years
(20 to 32) of 61 previously reported consecutive total hip replacements carried
out on 44 patients with severe congenital hip disease, performed
with reconstruction of the acetabulum with an impaction grafting
technique known as cotyloplasty. The mean age of the patients at
operation was 46.7 years (23 to 68) and all were women. The patients
were followed post-operatively for a mean of 24.3 years (20 to 32), using
the Merle d’Aubigné and Postel scoring system as modified by Charnley,
and with serial radiographs. At the time of the latest follow-up,
28 acetabular components had been revised because of aseptic loosening
at a mean of 15.9 years (6 to 26), and one at 40 days after surgery
because of repeated dislocations. The overall survival rate for aseptic
failure of the acetabular component at ten years was 93.1% (95%
confidence interval (CI) 86.5 to 96.7) when 53 hips were at risk,
and at 23 years was 56.1% (95% CI 49.4 to 62.8), when 22 hips remained
at risk. These long-term results are considered satisfactory for
the reconstruction of an acetabulum presenting with inadequate bone

We investigated the changes seen on serial metal
artefact reduction magnetic resonance imaging scans (MARS-MRI) of
metal-on-metal total hip arthroplasties (MoM THAs). In total 155
THAs, in 35 male and 100 female patients (mean age 70.4 years, 42
to 91), underwent at least two MRI scans at a mean interval of 14.6
months (2.6 to 57.1), at a mean of 48.2 months (3.5 to 93.3) after
primary hip surgery. Scans were graded using a modification of the
Oxford classification. Progression of disease was defined as an
increase in grade or a minimum 10% increase in fluid lesion volume
at second scan. A total of 16 hips (30%) initially classified as
‘normal’ developed an abnormality on the second scan. Of those with
‘isolated trochanteric fluid’ 9 (47%) underwent disease progression,
as did 7 (58%) of ‘effusions’. A total of 54 (77%) of hips initially
classified as showing adverse reactions to metal debris (ARMD) progressed,
with higher rates of progression in higher grades. Disease progression
was associated with high blood cobalt levels or an irregular pseudocapsule
lining at the initial scan. There was no association with changes
in functional scores. Adverse reactions to metal debris in MoM THAs
may not be as benign as previous reports have suggested. Close radiological

While an increasing amount of arthroplasty articles
report comorbidity measures, none have been validated for outcomes.
In this study, we compared commonly used International Classification
of Diseases-based comorbidity measures with re-operation rates after
total hip replacement (THR). Scores used included the Charlson,
the Royal College of Surgeons Charlson, and the Elixhauser comorbidity
score. We identified a nationwide cohort of 134 423 THRs from the
Swedish Hip Arthroplasty Register. Re-operations were registered
post-operatively for up to 12 years. The hazard ratio was estimated
by Cox’s proportional hazards regression, and we used C-statistics
to assess each measure’s ability to predict re-operation. Confounding
variables were age, gender, type of implant fixation, hospital category,
hospital implant volume and year of surgery. In the first two years only the Elixhauser score showed any significant
relationship with increased risk of re-operation, with increased
scores for both one to two and three or more comorbidities. However,
the predictive C-statistic in this period for the Elixhauser score
was poor (0.52). None of the measures proved to be of any value between
two and 12 years. They might be of value in large cohort or registry
studies, but not for the individual patient. Cite this article:
This study was designed to provide normal reference values for the centre-edge angle of Wiberg in the Chinese population by measuring 1494 radiographs according to the criteria of Tönnis. The mean angle was 23.1° (4.7° to 46.4°) in childhood (four to nine years), increasing to 28.9° (6° to 48°) in adolescence (10 to 18 years) and reaching 32.8° (13.7° to 58.8°) in adults. The angle was positively correlated with age, increasing by a mean of 0.78° annually before adulthood and by 0.070° annually in adults. A relative upward tendency after middle age might contribute to degenerative changes. No gender difference was discovered. The change in the angle with age was similar to that observed in other ethnic groups, showing moderate differences in the average of individual age groups, but the ethnic differences are not sufficient to explain the variation of incidence of developmental dysplasia of the hip in different races.
Reported rates of dislocation in hip hemiarthroplasty
(HA) for the treatment of intra-capsular fractures of the hip, range
between 1% and 10%. HA is frequently performed through a direct
lateral surgical approach. The aim of this study is to determine
the contribution of the anterior capsule to the stability of a cemented
HA through a direct lateral approach. A total of five whole-body cadavers were thawed at room temperature,
providing ten hip joints for investigation. A Thompson HA was cemented
in place via a direct lateral approach. The cadavers were then positioned
supine, both knee joints were disarticulated and a digital torque
wrench was attached to the femur using a circular frame with three
half pins. The wrench applied an external rotation force with the
hip in extension to allow the hip to dislocate anteriorly. Each
hip was dislocated twice; once with a capsular repair and once without
repairing the capsule. Stratified sampling ensured the order in
which this was performed was alternated for the paired hips on each
cadaver. Comparing peak torque force in hips with the capsule repaired
and peak torque force in hips without repair of the capsule, revealed
