The transfer of part of the ulnar nerve to the musculocutaneous nerve, first described by Oberlin, can restore flexion of the elbow following brachial plexus injury. In this study we evaluated the additional benefits and effectiveness of quantitative electrodiagnosis to select a donor fascicle. Eight patients who had undergone transfer of a simple fascicle of the ulnar nerve to the motor branch of the musculocutaneous nerve were evaluated. In two early patients electrodiagnosis had not been used. In the remaining six patients, however, all fascicles of the ulnar nerve were separated and electrodiagnosis was performed after stimulation with a commercially available electromyographic system. In these procedures, recording electrodes were placed in flexor carpi ulnaris and the first dorsal interosseous. A single fascicle in the flexor carpi ulnaris in which a high amplitude had been recorded was selected as a donor and transferred to the musculocutaneous nerve. In the two patients who had not undergone electrodiagnosis, the recovery of biceps proved insufficient for normal use. The most appropriate protocol for the biopsy of musculoskeletal tumours is controversial, with some authors advocating CT-guided core biopsy. At our hospital the initial biopsies of most musculoskeletal tumours has been by operative core biopsy with evaluation by frozen section which determines whether diagnostic tissue has been obtained and, if possible, gives the definitive diagnosis. In order to determine the accuracy and cost-effectiveness of this protocol we have undertaken a retrospective audit of biopsies of musculoskeletal tumours performed over a period of two years. A total of 104 patients had biopsies according to this regime. All gave the diagnosis apart from one minor error which did not alter the management of the patient. There was no requirement for re-biopsy. This protocol was more labour-intensive and 38% more costly than CT-guided core biopsy (AU$1804
Disruption of the interosseous membrane is easily
missed in patients with Essex-Lopresti syndrome. None of the imaging
techniques available for diagnosing disruption of the interosseous
membrane are completely dependable. We undertook an investigation to identify whether a simple intra-operative
test could be used to diagnose disruption of the interosseous membrane
during surgery for fracture of the radial head and to see if the
test was reproducible. We studied 20 cadaveric forearms after excision of the radial
head, ten with and ten without disruption of the interosseous membrane.
On each forearm, we performed the radius joystick test: moderate
lateral traction was applied to the radial neck with the forearm
in maximal pronation, to look for lateral displacement of the proximal radius
indicating that the interosseous membrane had been disrupted. Each
of six surgeons (three junior and three senior) performed the test
on two consecutive days. Intra-observer agreement was 77% (95% confidence interval (CI)
67 to 85) and interobserver agreement was 97% (95% CI 92 to 100).
Sensitivity was 100% (95% CI 97 to 100), specificity 88% (95% CI
81 to 93), positive predictive value 90% (95% CI 83 to 94), and
negative predictive value 100%). This cadaveric study suggests that the radius joystick test may
be useful for detecting disruption of the interosseous membrane
in patients undergoing open surgery for fracture of the radial head

Autologous retransfusion and no-drainage are
both blood-saving measures in total hip replacement (THR). A new combined
intra- and post-operative autotransfusion filter system has been
developed especially for primary THR, and we conducted a randomised
controlled blinded study comparing this with no-drainage. A total of 204 THR patients were randomised to autologous blood
transfusion (ABT)

We aimed to obtain anthropometric data on Korean
knees and to compare these with data on commonly available total
knee arthroplasties (TKAs). The dimensions of the femora and tibiae
of 1168 knees were measured intra-operatively. The femoral components
were found to show a tendency toward mediolateral (ML) under-coverage
in small femurs and ML overhang in the large femurs. The ML under-coverage
was most prominent for the small prostheses. The ML/anteroposterior
(ML/AP) ratio of Korean tibiae was greater than that of tibial components. This study shows that, for different reasons, current TKAs do
not provide a reasonable fit for small or large Korean knees, and
that the ‘gender-specific’ and ‘stature-specific’ components help
for large Korean femurs but offer less satisfactory fits for small
femurs. Specific modifications of prostheses are needed for Asian
knees.
We used a modification of the wake-up test to monitor spinal cord function in 102 consecutive scoliotic patients undergoing Harrington instrumentation. Four patients were found to have a neural deficit when they were awakened during the operation. Three recovered immediately after partial release of the distracting force; one required immediate removal of the rod and was left with a slight neural deficit. Using our method, we have encountered no problem in performing the wake-up test, although attention is drawn to the difficulty in repeating the test if the patient is re-anaesthetised with diazepam. There were no false negative results in this series.
Aims. Dual-mobility acetabular components (DMCs) have improved total hip arthroplasty (THA) stability in femoral neck fractures (FNFs). In osteoarthritis, the direct anterior approach (DAA) has been promoted for improving early functional results compared with the posterolateral approach (PLA). The aim of this study was to compare these two approaches in FNF using DMC-THA. Methods. A prospective continuous cohort study was conducted on patients undergoing operation for FNF using DMC by DAA or PLA. Functional outcome was evaluated using the Harris Hip Score (HHS) and Parker score at three months and one year. Perioperative complications were recorded, and radiological component positioning evaluated. Results. There were 50 patients in the DAA group and 54 in the PLA group. The mean HHS was 85.5 (SD 8.8) for the DAA group and 81.8 (SD 11.9) for the PLA group (p = 0.064). In all, 35 patients in the DAA group and 40 in the PLA group returned to their pre-fracture Parker score (p = 0.641) in both groups. No statistically significant differences between groups were found at one year regarding these two scores (p = 0.062 and p = 0.723, respectively). The DAA was associated with more intraoperative complications (p = 0.013). There was one dislocation in each group, and four revisions for DAA and one for PLA, but this difference was not statistically significant. There were also no significant differences regarding blood loss, length of stay, or operating time. Conclusion. In DMC-THA for FNF, DAA did not achieve better functional results than PLA, either at three months or at one year. Moreover, DAA presented an increased risk of
Aims. The goals of this study were to define the risk factors, characteristics,
and chronology of fractures in 5417 revision total hip arthroplasties
(THAs). . Patients and Methods. From our hospital’s prospectively collected database we identified
all patients who had undergone a revision THA between 1969 and 2011
which involved the femoral stem. The patients’ medical records and
radiographs were examined and the relevant data extracted. Post-operative
periprosthetic fractures were classified using the Vancouver system.
