Background

As the number of primary or revision TKA with stem extension cases are growing simultaneously, the number of periprosthetic fracture in these cases has also increased accordingly. However, there have been few reports on the classification and treatment of periprosthetic fracture following stemmed TKA and lack of information about the treatment outcome. The purposes of this study were 1) to demonstrate classification and management of periprosthetic fractures after stemmed TKA and 2) to report treatment outcome after the periprosthetic fractures.

Materials and Methods

This retrospective study included 17 knees (15 patients) with an average age of 69.7 years. All cases were revision TKA cases, and there were 13 female and 2 male patients. The patients were treated nonoperatively or underwent operation by orthopedic principle. The period of union was evaluated by confirming the formation of callus crossing fragments in radiographs. We reviewed the complications and functional outcomes after treatment of periprosthetic fracture following revision TKA by assessing FF, FC and scoring WOMAC and KSS.

Background

Joint line, patellar height and posterior condylar offset (PCO) are related to functional outcome such as stability and range of motion after revision total knee arthroplasty (TKA). The purpose of this study was (1) to determine whether revision TKA can restore the joint line, patella height and condylar offset after surgery, and (2) to assess factors associated with failed restoration.

Materials and Methods

We retrospectively reviewed 27 consecutive patients who underwent revision TKA. Among 27 patients, 11 patients had two-stage revision TKA due to periprosthetic joint infection while 14 patients underwent revision TKA due to aseptic loosening. In addition, there were 2 patients who had traumatic event causing a periprosthetic fracture which led to revision TKA. The joint line was measured using the distance from the adductor tubercle of the femur to the most distal portion of the medial femoral component on knee anterior-posterior radiographs. Patella height was assessed using the Caton-Deschamps method. In addition, PCO were measured relative to the tangent of the posterior cortex of the femur using knee lateral radiograph. All parameters were compared between pre- and postoperative radiographs after revision TKA.

Purpose

Unicompartmental Knee Arthroplasty (UKA) has been indicated for inactive elderly patients over 60, but for young and active patients less than 60 years old, it has been regarded as a contraindication. The purpose of this study is to evaluate the usefulness of UKA performed on young Asian patients under 60 years of age by analyzing clinical outcomes, complications and survival rate.

Materials and Methods

The subjects were 82 cases, which were followed up for at least 5 years (from 5 to 12 years). Only Oxford phase III® (Biomet Orthopedics, Inc, Warsaw, USA) prosthesis was used for all cases. The clinical evaluation was done by the range of motion, Knee society score (KSS), WOMAC score. The radiographic evaluation was performed on weight bearing long-leg radiographs, AP and lateral view of the knee and skyline view of the patella. The survival rate was estimated by Kaplan-Meier survival analysis.

Summary Statement

Flow chambers have been implemented in stem cell research to apply controlled dilational (volume changing) and deviatoric (shape changing) mechanical cues to living cells. Studies implementing such chambers demonstrate that controlled delivery of mechanical cues correlates strongly to changes in stem cell shape, structure, and fate.

Introduction

A custom designed flow chamber, capable of delivering highly controlled stresses at the cellular scale, enables the study of flow-induced normal and shear stresses on cell behavior. Specifically, computational fluid dynamics (CFD) and multiphysics modeling (coupling of CFD with finite element models) allow for controlled delivery of mechanical cues via fluid flow and cell seeding protocols, concomitant to optical mapping of cell displacements due to mechanical load, and calculation of flow velocities, imbued stresses, and cellular strains within a given volume of interest. Akin to conducting a mechanical loading test on single cells and groups of cells, paired experimental and computational experiments using the custom-designed chamber enabled calculation of the flow field's effect on the cell(s) as well as the cells’ effect on the flow field, a critical step in predicting the local stress and strain fields at the cell-fluid interface within the chamber, during exposure to fluid flow. These stresses-strains experienced by stem cells demonstrate significant correlation to cell gene expression, and strongly suggest that stresses at the cell-fluid interface influence cell fate. The current study uses a parametric approach to define next steps to prospectively guide mechanically-modulated lineage commitment.

As a result of lateral skin numbness that quite commonly occurs after knee joint operation, injury of the Infrapatellar branch of Saphenous nerve is often underestimated and regarded as a trivial complications. However, there are many complaints and worries from the patients in relation to the injury of this nerve never seems to stop.

The authors wanted to report the results of preserving this nerve during the unicompartmental knee arthroplasty for preventing lateral skin numbness.

The targets of this study were 100 cases of the unicompartmental knee arthroplasty by a single surgeon. All of the cases were medial compartmental osteoarthritis and in which a minimally invasive technique was used, with the average follow up of two years and eight months (range 24 to 42 months). The results were recorded in terms of classification of the nerve by location, preservation after surgery, sensory changes of the lateral skin flap, and complications. The classification by the location of this nerve was observed as either Mochida Type I with 76 people (76%), Type II with 16 people (16%), and unclassified types with eight people(8%). In Type I, this nerve was saved in 62 cases (82%). However in Type II, it could not be preserved in any cases because of the surgical procedure. The results of our study showed that while most of the nerve (76%) on average had a distance of 9.13mm (range 2 to 19mm) from the medial joint line to the nerve and passed inferiorly. This results allowed us to predict ahead of the location of this nerve and careful incision during the operation can preserve this nerve.

The authors discovered that in cases of unicompartmental knee arthroplasty, the nerve can be easily preserved, as 62 people(82%) of type I had this nerve completely preserved. Even if five extra minutes is necessary in order to preserve this nerve, when we think of the patient’s satisfaction it is thought of as a meaningful procedure.

Spontaneous osteonecrosis of the knee (SPONK) usually involves a single condyle or plateau. The medial femoral condyle is most often involved and spontaneous osteonecrosis of medial tibial plateau is a rare condition, representing only 2 % of all necrosis reported in the knee. Therefore, SPONK with both involvement of medial femoral condyle(MFC) and medial tibial plateau(MTP) might be extremely rare. SPONK in each MFC or MTP respectively might be extended into corresponding side of the knee at their advanced final stage, howevere, in that situations, significant degenerative change would accompany and it might be difficult to differentiate final staged SPONK form severe osteoarthritis. To the best of our knowledge, SPONK affecting both medial femoral condyle and medial tibial plateau without significant secondary osteoarthritis changes is not reported, even though it was difficult to know which occurred first. We experienced 3 patients with histologically proven osteonecrosis of the medial tibial condyle and medial tibial plateau, and report their radiologic features. All 3 patients showed similar ridiograhic patterns. Medial portion of medial tibial plateau and lateral portion of medial femoral condyle showed longitudinal fracture like-subchondral collapse. Standing anteroposterior radiograph at 30 degree knee flexion showed well fitted features such as “locked” medial condyle. Varus angulation was present. Significant degenerative changes was not shown except for subchondral sclerosis. T1-weighted coronal and Fat suppressed T2-weighted MR images showed subchondral collapse with ill-defined diffuse bone marrow edema changes on both tibial and femoral condyles. At surgical findings, longitudinal track-like groove was shown in both medial femoral condyle and medial tibial plateau. Articular cartilage was denuded and showed glistening surface with bone defect of lateral side of medial femoral condyle and medial side of tibial articular surface. Histological analysis shows necrotic bone, surrounded by an area of fibrovascular granulation tissue on both femoral and tibial sides. Total knee arthoplasty was performed in all 3 patients. As a result of very low prevalence of both involvement of MFC and MTP and limited number of our cases, we could not conclude that radiologic features in our cases are typical radiologic pattern of both involvement. However, based on our cases, we believe that this characteristic radiologic features may considered as one of the possible various radiologic findings of simultaneous involvement in MFC and MTP and allow diagnosis for SPONK with both involvement in MFC and MTP to be facilitated.

Arthrodesis is used most commonly as a salvage procedure for failed total knee arthroplasty (TKA). For successful arthrodesis, a stable fusion technique and acceptable limb mechanical alignment are needed. Although the use of intramedullary alignment rods might be helpful in terms of achieving an acceptable limb mechanical axis, fat embolism and intramedullary dissemination of an infection or reactivation of latent infection might occur in failed TKA cases. However, computer-assisted surgery allows precise cuts to be made without breaching medullary cavities. Here, the authors describe a case of knee arthrodesis performed by computer navigation and the Ilizarov method in a patient with a past history of infection. A 45-year-old man visited our hospital with failed total knee arthroplasy. Fortunately, even though infection was treated by debridement with component retention, mild heating was present over the knee, but ESR(erythrocyte sedimentation rate) and CRP(C-reactive protein) were within normal ranges. X-ray showed subsidence of the femoral component and a radiolucent line around the femoral component. Arthrodesis was planned for this patient due to disabling pain, a long-lasting severe functional deficit, failure of the primary TKA for ankylosed knee, and the patient’s poor economic status and his strong desire for arthrodesis. The computer navigation surgery system and the Ilizarov method were used for two reasons. The first reason was that the patient had a past history of infection. At pre-operative evaluation, even though ESR and CRP levels were within normal range, we could not completely rule out the possibility of latent infection due to suspicious findings such as long lasting disabling knee pain, mild heating over the knee, severe osteolytic radiographic changes around the femoral component. In that situation, inserting an IM rod to achieve acceptable mechanical alignment might have reactivated and disseminated a possible latent infection to the femoral or tibial medullary canals. The second reason was that we wanted to reduce the possibility of fat embolism by using computer navigation without instrumentation within the medullary canal. A CT-free, wireless computer navigation system was applied, with trackers fixed to the femur and tibia and no requirement for the use of an IM rod with component retention. Navigated femoral and tibial bone resections were then performed using Stryker software. The femoral resection was conducted at 0° of flexion to the sagittal axis, and the tibial resection at 7 ° of flexion to the sagittal axis. Arthrodesis was held in proper axial and rotational alignment with bone surfaces compressed together. Finally, knee arthrodesis was completed using the Ilizarov method. Based on our experience of the described case, we believe that arthrodesis for failed TKR, especially failure secondary to intraarticular infection, can be considered as another indication for computer navigation.

Background: Metaphyseal fracture healing presents special biomechanical challenges in orthopaedic surgery. The void typically created by damage to the metaphyseal cancellous bone must usually be filled in order to recover the biomechanical integrity of the bone. While autologous bone grafting is a standard treatment for these fractures, bone graft substitutes delivered with or without pharmacologic agents may augment healing.

Hypothesis: Tricalcium phosphate (TCP) is a known osteoconductive bone filler and OP-1 an osteoinductive bone morphogenetic protein; both have been used in the past in diaphyseal fractures with success. PTH (parathyroid hormone) has been recently shown to enhance osteoblastic activity, to have a net anabolic effect on bone mass, and to enhance healing of diaphyseal fractures. Each of these agents may also enhance healing of metaphyseal fractures.

Objective: The potential of all above factors to accelerate metaphyseal fracture healing has been evaluated in a new metaphyseal fracture model developed in our laboratory in a rabbit model.

Material and Methods: A metaphyseal wedge osteotomy was created in the distal tibia of 16-week-old female New Zealand White rabbits (n=20). The osteotomy was bridged with a custom-made external fixator. The osteotomy gap was filled with TCP containing OP-1 (n=4), TCP alone with daily subcutaneous injections of 10μg/Kgr BW PTH (n=4), or TCP alone with daily subcutaneous administration of 40μg/Krg BW PTH (n=4). Two control groups, TCP alone (n=4) and normal healing (n=4), were also included. Assessment methods included biomechanical testing in both compression and torsion, radiographic examination, and QCT scans.

Results: Healing was observed in both PTH treated groups as well as in the OP-1 group at 4 weeks post-surgery. PTH appeared to have a systemic effect on bone formation, whereas the effect of OP-1 was local to the osteotomy site. In comparison, healing was delayed in the normal healing and TCP alone groups.

Conclusion: PTH and OP-1 both enhance metaphyseal fracture healing. The different systemic vs. local effects of these two agents, suggest that PTH and OP-1 may have potential synergism in accelerating healing of metaphyseal fractures.
