The technique of femoral cement-in-cement revision is well established, but there are no previous series reporting its use on the acetabular side at the time of revision total hip replacement. We describe the technique and report the outcome of 60 consecutive acetabular cement-in-cement revisions in 59 patients at a mean follow-up of 8.5 years (5 to 12). All had a radiologically and clinically well-fixed acetabular cement mantle at the time of revision. During the follow-up 29 patients died, but no hips were lost to follow-up. The two most common indications for acetabular revision were recurrent dislocation (46, 77%) and to complement femoral revision (12, 20%).

Of the 60 hips, there were two cases of aseptic loosening of the acetabular component (3.3%) requiring re-revision. No other hip was clinically or radiologically loose (96.7%) at the latest follow-up. One hip was re-revised for infection, four for recurrent dislocation and one for disarticulation of a constrained component. At five years the Kaplan-Meier survival rate was 100% for aseptic loosening and 92.2% (95% CI 84.8 to 99.6), with revision for any cause as the endpoint.

These results support the use of cement-in-cement revision on the acetabular side in appropriate cases. Theoretical advantages include preservation of bone stock, reduced operating time, reduced risk of complications and durable fixation.

Introduction

Melorheostosis is a rare bone dysplasia characterized by its classic radiographic appearance resembling dripping candle wax. The condition was originally described by Leri and Joanny in 1922. Its etiology is not fully known and treatment in most instances has been symptomatic. There are nearly 350 reported cases on melorheostosis, joint replacement has been successfully attempted in the shoulder and knee joint. We describe a case of severe melorheostosis affecting the left hip causing secondary osteo-arthritis, which was treated with a total hip replacement (THR). To the best of our knowledge this is the first reported case of its kind in the World literature.

Case history

A 52-year-old male of Indian origin with known melorheostosis of the left leg for over 30 years, presented with symptoms suggestive of severe osteo-arthritis of the left hip. Previously he had been treated for melorheostosis of the knee joint (fig 1a & 1b) with excisions and decompression of the medial femoral condyle. His left hip became more painful over the last few years. He had a fixed flexion deformity of 20° of the hip, severe muscle wasting and the affected leg was 3 cm longer than the right leg. Radiographs (fig 2a & 2b) confirmed the presence of sclerotic new bone in the acetabulum eroding the femoral head. He had the classical dripping candle wax appearance along the medial border of the neck and shaft of the femur. He underwent a THR using a Corail-Pinnacle un-cemented prosthesis using ceramic on polyethylene bearing surfaces (fig 3a & 3b). Post operatively he made a quick recovery and there was a marked improvement in his symptoms and functional outcome scores at 6 weeks.

INTRODUCTION

The number of patients undergoing total hip replacement surgery is rising and thus the number of periprosthetic fractures is set to increase. The risk factors for periprosthetic fractures include osteolysis, rheumatoid arthritis, osteoporosis and use of certain types of implants. Evidence from literature suggests that the mortality rate within one year is similar to that following treatment for hip fractures thus as surgeons it is important for us to understand the various management strategies of these fractures.

MANAGEMENT

Acetabular periprosthetic fractures are uncommon and classified into Type I, in which the acetabular component is radiographically stable and Type II, in which the acetabular component is unstable. It is better to prevent than to treat these fractures.

Femoral periprosthetic fractures have several classifications the most commonly used is the Vancouver classification (fig 1).

Type-A fractures are proximal and can involve the greater or lesser trochanter. These are often related to osteolytic wear debris and therefore revision of the bearing surface with bone grafting is recommended. AG involves the greater trochanter and AL involves the lesser, and these can usually be stabilised by cerclage wires supplemented by screws or plates if required (fig 2).

Management of type B fractures is more controversial and will be discussed in depth with reference to all recent papers at the meeting and data from the Swedish Joint Registry. In summary the management is shown in fig 3.

In type-C fractures, one should ensure the fixation device bypasses the femoral stem by at least 2 diaphyseal diameters. Management is as shown in fig 4.


We report the results of a modified Fulkerson technique of antero-medialisation of the tibial tubercle, combined with microfracture or abrasion arthroplasty in patients under 60 with patello-femoral osteoarthritis.

All patients operated on between September 1992 and October 2007 were reviewed by an independent observer in clinic or by postal questionnaire, using the Oxford Knee Score, Melbourne Patella Score and a Satisfaction Score. Only patients with Outerbridge Grade 3 – 4 osteoarthritis of the patello-femoral joint were included. They were assessed pre-operatively with plain x-rays, MRI scans (as well as tracking scans in the last 10 years) and arthroscopically. All patients with tracking scans showed lateral subluxation of the patella.

The surgical procedure was a modification of Fulkerson's tibial tubercle osteotomy, with an advancement of 1-1.5 cms and a medialisation of 1.5 cms. The exposed bone of the patella and trochlea was drilled in the early cases and in the later cases an arthroscopic microfracture or abrasion using a power burr was carried out.

Between September 1992 and October 2007, 103 procedures were carried out in 84 patients, 19 patients having staged bilateral procedures. The mean follow up was 84 months (range 24 – 204 months). The mean age was 45 (range 26 – 59) and the female to male ratio was 7.6:1. 70 patients were reviewed giving a follow up rate of 82%. The mean Oxford Knee Score was 18.5 pre-operatively (range 3- 32) and 34.3 post-operatively (range 11- 47). The Melbourne Patella Score was 9.6 pre-operatively (range 3- 30) and 20 post-operatively (range 11- 30). Patient Satisfaction Scores were excellent (54%), good (29%), fair (8.5%) and poor (8.5%). 4 knees in 3 patients were converted to a patello-femoral arthroplasty, giving a 10 year survival rate of 96.1%.

This procedure offers an alternative to patello-femoral arthroplasty for younger patients with isolated patello-femoral arthritis.

Restoration of an anatomical hip centre frequently requires limb lengthening, which increases the risk of nerve injury in the treatment of Crowe 4 Developmental Dysplasia of the Hip (DDH). Prospective evaluation of the use of subtrochanteric derotational femoral shortening with a cemented Exeter stem.

15 female patients (18 hips – 3 bilateral) with a mean age at time of operation of 51 years followed-up for a mean of 114 months (range 52 to 168). 16 cemented and 2 uncemented acetabular components were implanted. Exeter cemented DDH stems were used in all cases. No patient lost to follow up. All 18 Crowe IV hips reviewed. Charnley-D’Aubigne-Postel score for pain, function and range of movement were improved from a mean of 2-2-3 to 5-4-5 respectively. One osteotomy failed to unite at 14 months and revised successfully. Clinical healing was achieved at a mean of 6 months while radiological evidence of union at a mean of 9 months. The mean length of the excised segment was 3 cm and the mean true limb lengthening was 2 cm. 3.5mm DCP plate with unicortical screws was used to reduce the osteotomy, and intramedullary autografting performed in all cases. Mean subsidence was 1 mm and no stem was found to be loose at the latest follow up. No sciatic nerve palsy was observed and there were no post-operative dislocations.

Cemented Exeter femoral components perform well in the treatment of Crowe IV DDH with when a subtrochanteric derotational shortening osteotomy (SDSO) was necessary. A transverse osteotomy is necessary to achieve derotation and reduction can be maintained with a DCP plate. Intramedullary autografting prevents cement interposition at the osteotomy site and promotes healing.

The demand for primary hip replacements has grown steadily over the past 20 years and continues to do so. The challenge is to deliver a high quality and cost-effective service by developing short-stay pathways to optimise patient care and outcome.

A four-day hip protocol was established, with attention to detail on analgesia, thrombo-prophylaxis, mobilisation and patient motivation. A multi-disciplinary team was trained to deliver the pathway on a designated ward for primary hip replacements with the aim to discharge patients on the third post-operative day (four day stay in total). Before the initiation of this audit our average stay for primary hips was nine days. The national average stay is 10.6 days, with the top 10 trusts averaging 6.7 days.

In the initial three-month trial, 66 hip replacements (39 females and 27 males) done by the hip team at Princess Elizabeth Orthopaedic Centre in Exeter were audited against the four-day hip protocol. Average age was 72.4 years (range 44 – 92 years). Two patients were discharged on the second post-operative day and another 15 patients on the third post-operative day. As a result, we managed to get 25.8% of the patients home within four days. Overall, the average length of stay was only 6.3 days.

In conclusion, while we only achieved our target in 25.8% of patients, the project improved our overall length of stay remarkably. It is clearly evident that a targeted pathway focused to patient care in all aspects using a multi-disciplinary approach can improve the overall service delivery.

We evaluated all cases involving the combined use of a subtrochanteric derotational femoral shortening osteotomy with a cemented Exeter stem performed at our institution. With severe developmental dysplasia of the hip an osteotomy is often necessary to achieve shortening and derotation of the proximal femur. Reduction can be maintained with a 3.5 mm compression plate while the implant is cemented into place. Such a plate was used to stabilise the osteotomy in all cases. Intramedullary autograft helps to prevent cement interposition at the osteotomy site and promotes healing. There were 15 female patients (18 hips) with a mean age of 51 years (33 to 75) who had a Crowe IV dysplasia of the hip and were followed up for a mean of 114 months (52 to 168). None was lost to follow-up. All clinical scores were collected prospectively. The Charnley modification of the Merle D’Aubigné-Postel scores for pain, function and range of movement showed a statistically significant improvement from a mean of 2.4 (1 to 4), 2.3 (1 to 4), 3.4 (1 to 6) to 5.2 (3 to 6), 4.4 (3 to 6), 5.2 (4 to 6), respectively. Three acetabular revisions were required for aseptic loosening; one required femoral revision for access. One osteotomy failed to unite at 14 months and was revised successfully. No other case required a femoral revision. No postoperative sciatic nerve palsy was observed.

Cemented Exeter femoral components perform well in the treatment of Crowe IV dysplasia with this procedure.

The cement-cement interface has been proven to have greater shear strength than a new bone-cement interface after removing a well-fixed cement mantle. This study reviewed a series of acetabular revision procedures with a minimum 2-year follow-up where the original cement mantle was left intact. From 1988 to 2004, 60 consecutive cement-in-cement revisions of the acetabular component were performed at our institution. Outcome was based on functional assessment using the Oxford, Charnley, and Harris scoring systems as well as radiographic analysis using the DeLee and Charnley criteria.

In total 60 procedures were performed in 60 patients (40 female and 20 male), whose mean age at surgery was 75 years (range 40 to 99 years). 80% were performed for recurrent dislocation, 13.3% during femoral component revision, 5% for acetabular component wear, and 1.7% for pain. No case was lost to follow-up.

There was one re-revision for aseptic cup loosening at 7 years, with 1 further case of radiological loosening identified at the latest review. There were 6 further cases of dislocation 4 of which were treated with further in-cement revisions. All other cases showed well-fixed components on radiographic analysis and no evidence of failure at the most recent follow up.

The cement-in-cement technique already has a good body of evidence based on revision of the femoral component and this study shows that the technique can be applied to acetabular revisions as well with good functional and radiological results in the short to medium term.

All other cases showed well-fixed components on radiographic analysis and no evidence of failure at the most recent follow up.
