Aims

The aim of this study was to report patient and clinical outcomes following robotic-assisted total knee arthroplasty (RA-TKA) at multiple institutions with a minimum two-year follow-up.

Methods

This was a multicentre registry study from October 2016 to June 2021 that included 861 primary RA-TKA patients who completed at least one pre- and postoperative patient-reported outcome measure (PROM) questionnaire, including Forgotten Joint Score (FJS), Knee Injury and Osteoarthritis Outcomes Score for Joint Replacement (KOOS JR), and pain out of 100 points. The mean age was 67 years (35 to 86), 452 were male (53%), mean BMI was 31.5 kg/m2 (19 to 58), and 553 (64%) cemented and 308 (36%) cementless implants.

Management of complex posterior malleolar fractures requires a detailed appreciation of ligamentous and bony anatomy for optimal fracture fixation and restoration of articular congruency. Pre operative planning is vitally important to determine the surgical strategy for complex ankle fractures. We evaluated pre operative planning strategy pre and post implementation of BOAST 12 guidelines (2016) focussing on pre operative CT scans prior to definitive fixation at a major trauma centre.

A multi-surgeon retrospective review of prospectively collected data from 2013 to 2018 was performed at a major trauma centre. Patients who had sustained a posterior malleolar fracture and definitive fixation were identified. Information was collated from PICS, PACS, the trauma database and operative notes.

134 patients were identified over a 5 year period who had sustained a posterior malleolar fracture and had definitive fixation. (Pre BOAST guidelines = 61, Post BOAST guidelines = 73). Prior to the implementation of BOAST guidelines ¼ with posterior malleolar fractures did not have a pre operative CT scan (15/61). Post implementation of BOAST 12 90% (66/73)patients with fixation of posterior malleolus fractures had a pre operative CT scan. Posterior malleolus surgery most commonly took place In patients between 18–30 years.

Following implantation of BOAST 12 guidelines there was a 15% increase in pre operative CT scanning for ‘complex ankle fractures'. Changes in national guidelines have heavily influenced pre operative planning strategy for ankle fractures at University Hospitals Birmingham. A detailed appreciation of fracture pattern pre operatively helps guide surgical strategy.

Internal fixation remains the treatment of choice for non-displaced femoral neck fractures in elderly patients, whereas, arthroplasty is preferred for displaced fracture patterns. Given technological advancements in implant design and excellent long-term outcomes, arthroplasty may provide improved outcomes for the treatment of non-displaced femoral neck fractures. The aim of our study was to conduct a systematic review of the orthopaedic literature (1) to investigate the outcomes of internal fixation for the treatment of non-displaced and minimally displaced femoral neck fractures in elderly patients and (2) to compare the outcomes of patients treated with internal fixation to arthroplasty in this patient population.

Relevant articles were identified using PubMed, Embase, and CENTRAL databases. Manuscripts were included only if they contained (1) patients 60 years or older with (2) nondisplaced or minimally displaced (Garden I or II) femoral neck fractures (3) treated with internal fixation or arthroplasty or (4) separately reported outcomes in this patient population. The primary outcome was reoperation. Secondary outcomes included mortality, patient reported outcomes, length of hospital stay, infection, and transfusions. An a priori decision was made to classify studies into comparative or non-comparative groups. Comparative studies directly compared arthroplasty to internal fixation in the specific study population while the non-comparative studies included separate cohorts of patients treated with arthroplasty or internal fixation. A fixed-effects model was used to quantitatively pool study outcomes.

Twenty-five non-comparative studies were identified with a total of 22,020 patients, all of which were treated with internal fixation. The pooled incidence of reoperation after internal fixation was 14.4% (95% CI: 10.8 – 18.8). The incidence of mortality within one-year of injury was 14.4% (95% CI: 6.7 – 28.3), based on the reporting in 14 studies.

Three comparative studies were identified with a total of 360 patients (128 treated with arthroplasty and 232 treated with internal fixation). All three studies reported reoperation rates. The overall risk of reoperation was 3.1% in the arthroplasty group compared to 9.5% in the internal fixation group (relative risk: 0.30, 95% CI: 0.10 – 0.84, p= 0.02). Only two studies reported mortality. The relative risk of mortality in patients treated with arthroplasty compared to internal fixation was 2.54 (95% CI: 1.38 – 4.70, p= 0.003).

Internal fixation of minimally displaced femoral neck fractures in the elderly is associated with a risk of reoperation and mortality that exceeds 14%. Treatment with arthroplasty may reduce the risk of reoperation by 70%. However, this benefit maybe tempered by a potential increased risk of mortality associated with arthroplasty in this patient population.

Background

The Nottingham Hip Fracture Score (NHFS) is a risk stratifying score that estimates the 30-day and 12-month mortality rates of hip fracture patients. To date, it has only been validated in few centres in the UK. Our study aims to see how our mortality rates compare with those predicted by the NHFS.

Methods

The Nottingham Hip Fracture Database was reviewed for patients presenting to our unit from August 2012 - March 2013 with a neck of femur fracture. Patient information was obtained from the database and our online electronic patient records for NHFS calculation. Patients with incomplete data were excluded.

Background

Low back pain (LBP) is widespread in all populations and is a worldwide health problem, which poses substantial challenges for clinical management. Individuals with LBP may reduce their symptoms by implementing self-managed at-home interventions. The theoretical design of the LumbaCurve™ promotes the principles of a passive gravity-assisted traction (PGAT) stretch of the lumbar and sacral region in order to reduce LBP. This study aimed to assess the clinical effectiveness of the LumbaCurve™ in the management of LBP when compared to a control group of standardised care.

Methods

Following a screening form using Red Flags and STarT Back tools, 60 individuals with LBP were recruited to the 4-week intervention. Eligible participants completed a pre-intervention questionnaire and were randomly allocated to either ‘standardised care’ or ‘standardised care PLUS LumbaCurve™’ group. Intervention material was trialled for 4 weeks consecutively, followed by a post-intervention questionnaire. Pre and post assessments applied the Roland Morris Disability Questionnaire (RMDQ), Patient Reported Outcome Measures (PROMS).

Background

Back Pain is a worldwide problem that often interferes with work, daily activities and sleep. The stress and disability experienced at night by individuals with Low Back Pain (LBP) could be reduced by targeted treatments designed specifically for this period. This study aimed to assess the clinical effectiveness of a spinal alignment cushion in the management of LBP when compared to a control group of standardised care.

Methods & Results

71 individuals (30 males, 41 females) with LBP were recruited to the 4-week intervention after screening using the Red Flags and STarT Back tools. Participants were randomly assigned to either the control (standardised care) or intervention group (standardised care plus spinal alignment cushion). Pre and post assessments were taken using the Roland Morris Disability Questionnaire (RMDQ) (0–24), the Core Outcomes Measure Index (COMI) (0–10), and a Pain and Comfort during Sleep VAS Assessment. Each post assessment was analysed using ANCOVA with corresponding pre-assessment as covariate.

Significant differences were seen in the RMDQ in favour of the intervention group (P = 0.034) over the four week period. Significant differences were also seen in favour of the intervention group in the COMI score (P=0.008), the frequency (P=0.004) and intensity of back pain (P=0.000), joint/muscle stiffness (P=0.046) and intensity of back stiffness (P=0.022).

Background

Back pain has become a worldwide problem and excessive, repetitive rotation has been shown to cause tissue damage. A sleeping posture similar to that of the foetal position has been suggested to limit unnecessary rotation of the lumbar spine. The Rophi™ cushion, utilises this theory to provide spinal alignment and improved sleeping posture. This study aims to assess the subjective experience and biomechanical effects of the Rophi™ cushion in participants with simple mechanical lower back pain (LBP).

Methods & Results

Fifteen participants (aged 44 ± 9.7 years) with simple mechanical LBP were recruited using the Red Flags screening form. The kinematics of the pelvis, lower limbs, lumbar and thoracic spine were analysed in six degrees of freedom whilst the participants lay in a semi-foetal position. Visual analogue scales were used to measure participant pain and discomfort levels during sleep pre and post a one week cushion intervention.

Kinematic results show the main significant difference in joint angles occurred at the hip in all three planes, and between the lower lumbar region and the pelvis in the coronal plane. Subjective experience showed a reduction in the number of days with poor sleep quality and a significant reduction in frequency and intensity of lower back pain and stiffness when waking.

The fixation of comminuted femoral fractures with intramedullary nails is commonplace but there remains little work on the mechanical ability of the different diameters of nail available to resist bending. What previous work there is has produced conflicting conclusions. The bending stiffness against the intramedullary nail diameter and the extent of the comminuted fracture is clinically important due to the impact on fracture healing and implant failure.

Intramedullary nails of differing diameters (10 mm, 11 mm and 13 mm) were loaded axially in fourth generation composite femurs with increasing mid shaft bone defects, namely 3cm, 5cm, 8cm and 10cm bones. The loading versus the displacement was recorded for each nail.

A one-way ANOVA analysis demonstrated a significant difference between intramedullary nail diameters and the bending stiffness, with p values of less than 0.012; 3cm mean 12.26 (CI 9.06-15.46) mm, p=0.012; 5 cm mean 10.63 (CI 8.35-12.92) mm, p=<0.001; 8 cm mean 11.04 (CI 8.35-13.74) mm, p=<0.001; 10 cm mean 11.68 (CI 7.86-15.50) mm, p=<0.001. For the 11 mm diameter intramedullary nail, failure occurred at around two times the body weight of an average individual or 1400 to 1800 N. A repeated measure ANOVA analysis of the effect of the increasing bone defect showed a mixed picture, with a significant difference between the 5 cm and 8 cm gap and only a trend towards significance between 5 cm and 10 cm.

Caution should be advised when considering using a cannulated femoral intramedullary nail in a patient with a fracture gap of greater than 5 cm. Further, the mechanical effect of comminuted fractures treated with nails suggests reduced stiffness with increasing length of fracture gap although the picture is complex and explains the divergence of research conclusions.

Several authors have used 3D motion analysis to measure upper limb kinematics, but none have focused solely on wrist movements, in six degrees of freedom, during activities of daily living (ADL). This study aimed to determine the role of the different planar wrist movements during three standardised tasks, which may be affected by surgical procedures.

Nine volunteers (age range 22-45) were recruited and each participant performed three simulated ADLs: using a door lever, a door knob and opening/closing a jam jar. The ADLs were simulated using a work-sim kit on an isokinetic dynamometer. Motion analysis was performed by a 10-camera Oqus system (Qualisys Medical AB, Gothenburg, Sweden). All raw kinematic data were exported to Visual3D (C-Motion Inc.), where the biomechanical model was defined and joint kinematics calculated.

Table 1 shows a similar range of radial-ulnar deviation and flexion-extension as previous studies. However a substantial amount of wrist rotation also occurred in all tasks. This was significantly greater when using the door lever compared with the door knob and jam jar tasks.

Previous studies have stated that a negligible degree of rotation occurs at the wrist. This study found a maximum mean of 31.7 degrees of wrist rotation. This indicates that considerable rotational movement occurs at the wrist during certain functional tasks. Surgical approaches and clinical pathology may disrupt structures responsible for rotational stability. Further investigation of this rotational component of carpal movement during additional ADLs is proposed in both normal and clinical subjects, to explore the potential relationship between carpal surgery and rotational laxity.

Squeaking in ceramic total hip joint replacements has become a controversial topic. This study aims to document the incidence of squeaking and other noise generation in a single surgeon series for ceramic on ceramic total hip joint replacements. Possible aetiological for squeaking causes are explored.

All patients from public and private who received ceramic on ceramic total hip joint replacements (Stryker trident-accolade) from 2002 to 2007 were identified via the New Zealand Joint registry. Following ethics approval all patients were contacted for a phone interview to question as to whether they had noted any noise generation. Patients who demonstrated noise generation were reviewed in clinic for full history and examination. Data including age, sex, weight, primary diagnosis, head size and cup size were obtained from clinical notes. Post operative x-rays were reviewed to analyse cup abduction and version.

Forty one ceramic total hip joint replacements in a total of thirty seven patients were reviewed via telephone interviews. Three patients complained of squeaking in the ceramic bearing while one patient complained of a grinding and one other of clicking. Two of the three who had recognised the squeaking were both able to reproduce the squeaking in the clinic room. The third patient was noted to have crepitus from anterior patello-femoral osteoarthritis. There was no statistical difference in age, weight, primary diagnosis or head size. In terms of abduction and version of the acetabular cups that squeaked, one had twenty seven degrees of ante-version and forty seven degrees of abduction and the other fifteen degrees of anteversion and thirty degrees of anteversion. Four cups lay outside the recommended fifteen-thirty five degrees of anteversion and thirty five-fifty five degrees of abduction yet showed no squeaking. Neither patient is troubled by the squeaking and neither would seek revision surgery. The incidence of squeaking in ceramic on ceramic total hip joint replacements appears to be around five percent with a similar number of patients experiencing other noises. The position of the acetabular cup does not appear to be the sole contributor to the noise and other aetiological causes need to be further investigated.

The compression produced by and the resistance to pullout of the 6.5 mm cannulated Herbert screw were compared with those of ASIF headed screws. The latter were tested with and without washers and in the following sizes: 4.5 mm cortical, 6.5 mm cancellous with a 16 mm threaded segment, and 6.5 mm cancellous with a 32 mm threaded segment. Polyurethane foam was used as a substitute for cancellous bone and ASIF artificial bone for corticocancellous bone. The compression produced by a cancellous lag screw with a washer was significantly greater than that produced by a Herbert screw of equivalent size (p < 0.05). When the screws were tested using the corticocancellous composite the ASIF cancellous screw without a washer produced significantly greater compression (p < 0.05); when used with a washer the difference was highly significant (p < 0.001). The dual pitch Herbert screw is not appropriate for the management of fractures in which compression is of greater importance than the need to avoid prominence of the screw head.
