Purpose

Periacetabular osteotomy (PAO) is a versatile acetabular reorienting procedure that is most commonly used to provide greater femoral head coverage in adolescent hip dysplasia. However, PAO can also be used to reorient the acetabulum in the opposite direction to treat femoroacetabular impingement (FAI) due to acetabular over-coverage. We describe the indications, surgical technique, and early results of reverse PAO to reduce femoral head coverage in symptomatic hips with FAI due to acetabular over-coverage.

Methods

IRB approval was obtained to retrospectively review cases of symptomatic acetabular over-coverage treated with reverse PAO and that had a minimum of two years follow-up. All hips had atypical intraoperative positioning of the acetabular fragment to uncover the lateral and anterior aspects of the femoral head, with or without anteverting the acetabulum. Prospectively collected pre- and post-operative demographics, WOMAC scores, Modified Harris Hip Score (MHHS), and radiographic measurements consisting of the lateral center edge angle (LCEA), Tönnis angle (TA), and anterior center edge angle (ACEA) were compared using student's t-test.

Introduction

Dual modular femoral stems for total hip arthroplasty were initially introduced to optimize joint biomechanics. These implants have been recalled due to fretting and crevice corrosion at the stem-neck interface, ultimately necessitating revision in a significant number of patients. At our institution we had experience with the Rejuvenate (Stryker, Mahwah, NJ) dual modular stem from 2009 until 2011 before it's recall in 2012. This study identifies complications encountered in patients requiring revision of this prosthesis.

Methods

We retrospectively identified all patients who had one particular dual modular stem using our registry database. All patients’ charts and imaging was reviewed using our electronic medical records and digital imaging programs. Patients’ age, gender, revision date, intraoperative and postoperative complications, need for subsequent surgery were identified.

Introduction

Risks and benefits of bilateral total knee arthroplasty (TKA), whether simultaneous, sequential single-staged, or staged is a topic of debate. Similarly, computer-assisted navigation for TKA is controversial regarding complications, cost-effectiveness, and benefits over conventional TKA. To our knowledge, no studies have compared computer-assisted and conventional techniques for sequential bilateral TKA. We hypothesise that the computer-assisted technique has fewer complications.

Methods

We retrospectively reviewed 40 computer-assisted and 36 conventional bilateral sequential TKAs from 2007–2011 with 1 year follow-up for complications. Groups were matched by age, gender, body mass index (BMI), Charlson Comorbidity Index (CCI), and American Society of Anesthesiologists Classification (ASA). Pearson's Chi-square, Fisher's exact test, and independent samples t-test were used to compare groups.
