The Manchester–Oxford Foot Questionnaire (MOXFQ) is a validated
16-item, patient-reported outcome measure for evaluating outcomes
of foot or ankle surgery. The original development of the instrument
identified three domains. This present study examined whether the
three domains could legitimately be summed to provide a single summary
index score. The MOXFQ and Short-Form (SF)-36 were administered to 671 patients
before surgery of the foot or ankle. Data from the three domains
of the MOXFQ (pain, walking/standing and social interaction) were
subjected to higher order factor analysis. Reliability and validity
of the summary index score was assessed.Objectives
Methods
The patient-reported Manchester-Oxford Foot Questionnaire (MOXFQ), with 3 dimensions (Walking/standing(W/S), Pain, Social interaction(SI)) has previously been validated in patients undergoing hallux valgus surgery. A preliminary cross-sectional evaluation of its measurement properties relating to surgery on different regions of the foot and ankle is presented here. Within 4 weeks prior to foot or ankle surgery, 671of 764 consecutive eligible patients (87.8% response rate) were assessed with the: (i) MOXFQ questionnaire, (ii) SF-36 general health survey (iii) American Orthopaedic Foot & Ankle Society (AOFAS) scales. Sample characteristics: mean age 53 years 427 (63.6%) female. Patients were booked for (main) surgery on Hallux (210, 31.3%), Lesser Toes (119, 17.7%), Mid foot (22, 3.3%), Ankle/Hind foot (311, 46.3%) and whole foot/multiple regions (9, 1.3%) MOXFQ item response rates were high (<2% missing on any one item) with responses generally well distributed across response categories. Item-total correlations within each dimension were generally above recommended levels. Internal consistency, as assessed by Cronbach's alpha, were 0.93, 0.84 and 0.71 for the W/S, Pain and SI dimensions, respectively. Within Hallux, Lesser toes and Ankle/hind foot surgical subgroups (low numbers precluded mid foot and whole/multiple region analyses), alphas for these subscales were similar. Convergent validity of MOXFQ was demonstrated by correlations between the 3 dimensions (W/S, Pain, SI) with similar dimensions of the SF-36 and relevant AOFAS scales. A priori hypotheses were generally supported. The MOXFQ demonstrates good baseline measurement properties in patients undergoing a range of foot and ankle surgery, suggesting its suitability as an outcome measure for clinical trials of foot and ankle surgery in general. The SI dimension, concerned with cosmesis and social participation has somewhat weaker properties but remains useful. Evidence for the MOXFQ's responsiveness across the range of foot and ankle surgery is yet to be assessed.
The responsiveness of the Manchester–Oxford Foot
Questionnaire (MOXFQ) was compared with foot/ankle-specific and
generic outcome measures used to assess all surgery of the foot
and ankle. We recruited 671 consecutive adult patients awaiting
foot or ankle surgery, of whom 427 (63.6%) were female, with a mean
age of 52.8 years (18 to 89). They independently completed the MOXFQ,
Short-Form 36 (SF-36) and EuroQol (EQ-5D) questionnaires pre-operatively
and at a mean of nine months (3.8 to 14.4) post-operatively. Foot/ankle
surgeons assessed American Orthopaedic Foot and Ankle Society (AOFAS)
scores corresponding to four foot/ankle regions. A transition item measured
perceived changes in foot/ankle problems post-surgery. Of 628 eligible
patients proceeding to surgery, 491 (78%) completed questionnaires
and 262 (42%) received clinical assessments both pre- and post-operatively. The
regions receiving surgery were: multiple/whole foot in eight (1.3%),
ankle/hindfoot in 292 (46.5%), mid-foot in 21 (3.3%), hallux in
196 (31.2%), and lesser toes in 111 (17.7%). Foot/ankle-specific
MOXFQ, AOFAS and EQ-5D domains produced larger effect sizes (>
0.8)
than any SF-36 domains, suggesting superior responsiveness. In analyses
that anchored change in scores and effect sizes to patients’ responses
to a transition item about their foot/ankle problems, the MOXFQ
performed well. The SF-36 and EQ-5D performed poorly. Similar analyses,
conducted within foot-region based sub-groups of patients, found
that the responsiveness of the MOXFQ was good compared with the
AOFAS. This evidence supports the MOXFQ’s suitability for assessing
all foot and ankle surgery.
The most common injury in rugby resulting in
spinal cord injury (SCI) is cervical facet dislocation. We report
on the outcome of a series of 57 patients with acute SCI and facet
dislocation sustained when playing rugby and treated by reduction
between 1988 and 2000 in Conradie Hospital, Cape Town. A total of
32 patients were completely paralysed at the time of reduction.
Of these 32, eight were reduced within four hours of injury and
five of them made a full recovery. Of the remaining 24 who were
reduced after four hours of injury, none made a full recovery and only
one made a partial recovery that was useful. Our results suggest
that low-velocity trauma causing SCI, such as might occur in a rugby
accident, presents an opportunity for secondary prevention of permanent
SCI. In these cases the permanent damage appears to result from
secondary injury, rather than primary mechanical spinal cord damage.
In common with other central nervous system injuries where ischaemia
determines the outcome, the time from injury to reduction, and hence
reperfusion, is probably important. In order to prevent permanent neurological damage after rugby
injuries, cervical facet dislocations should probably be reduced
within four hours of injury.
Metal on metal hip resurfacing (MMHR) is a popular procedure for the treatment of osteoarthritis in young patients. Several centres have observed masses, arising from around these devices, we call these inflammatory pseudotumours. They are locally invasive and may cause massive soft tissue destruction. The aim of this study was to determine the incidence and risk factors for pseudotumours that are serious enough to require revision surgery. In out unit, 1,419 MMHRs were performed between June 1999 and November 2008. All revisions were identified, including all cases revised for pseudotumour. Pseudotumour diagnosis was made by histological examination of samples from revision. A Kaplan-Meier survival analysis was performed, Cox regression analysis was used to estimate the independent effects of different factors. The revision rate for pseudotumour increased with time and was 4% (95% CI: 2.2% to 5.8%) at eight years. Female gender was a strong risk factor: at eight years the revision rate for pseudotumours in men was 0.5% (95% CI 0% to 1.1%), in women over 40 it was 6% (95% CI 2.3% to 10.1%) and in women under 40 it was 25% (95% CI 7.3% to 42.9%) (p<
0.001). Other factors associated with an increase in revision rate were, small components (p=0.003) and dysplasia (p=0.019), whereas implant type was not (p=0.156). We recommend that resurfacings are undertaken with caution in women, especially those younger than 40 years of age, but they remain a good option in men. Further work is required to understand the patho-aetiology of pseudotumours so that this severe complication can be avoided.
Patello-femoral instability (PFI) affects 40 individuals per 100,000 population and causes significant morbidity. The causes of patello-femoral instability are multi-factorial, and an isolated anatomical abnormality does not necessarily indicate instability. Patello-femoral subluxation ranges from 0% (stable patella tracking) to 100% (dislocation) and there is an established relationship between the amount of subluxation and anterior knee pain. Traditionally, magnetic resonance (MR) imaging and standard radiographs are used to guide the clinician towards a suitable corrective procedure for PFI. The multi-factorial nature of patello-femoral instability is not addressed with current imaging techniques. This study aims to address which anatomical variables assessed on MR images are most relevant to patello-femoral subluxation. This information will aid surgical decision making, particularly in selecting the most appropriate reconstructive surgery. A retrospective analysis of MR studies of 60 patients with suspected patello-femoral instability was performed. All patients were graded for degree of subluxation using a dynamic MR scan. The patient scans were assessed for the presence of a specific range of anatomical variables:
patella alta, (modified Insall-Salvatti) patella type (Wiberg classification) trochlea sulcus angles for bone and cartilage surfaces the distance of the vastus medialis obliquis (VMO) muscle from the patella trochlea and patella cartilage thickness the horizontal distance between the tibial tubercle and the midpoint of the femoral trochlea (TTD) patella engagement – the percentage of the patella height that is captured in the trochlea groove in full extension. The Wilk’s Lambda test for multi-variate analysis was used to establish whether any relationship was present between the degree of patello-femoral instability and bony or soft tissue anatomical variables. Non-parametric statistical tests were applied across the groups and within the groups to assess their relative significance. The following variables showed a significant relationship with patellofemoral subluxation; distance of the VMO from the patella (<
0.001), TTD (<
0.001), patella engagement (0.001), sulcus angles (0.004) and patella alta (0.005). This study agrees with previous work showing a significant correlation between subluxation and trochlea sulcus angle and TTD. This is the first study to establish a significant correlation between patella engagement and radiological instability. The lower the percentage engagement of the patella in the trochlea, the greater the degree of patello-femoral instability. Patella engagement showed a more significant relationship with subluxation than patella alta. We report a new method of predicting patello-femoral instability by measuring the overlap of the patella in the trochlea groove.

This is the first study to establish that patella engagement is related to PFJ subluxation showing that the lower the percentage engagement of the patella in the trochlea, the greater the severity/magnitude of patellofemoral subluxation. The finding provides greater insight into the aetiology and understanding of the mechanism of symptomatic PFJ subluxation.
Metal-on-metal hip resurfacing is commonly performed for osteoarthritis in young active patients. We have observed cystic or solid masses, which we have called inflammatory pseudotumours, arising around these devices. They may cause soft-tissue destruction with severe symptoms and a poor outcome after revision surgery. The aim of this study was to determine the incidence of and risk factors for pseudotumours that are serious enough to require revision surgery. Since 1999, 1419 metal-on-metal hip resurfacings have been implanted by our group in 1224 patients; 1.8% of the patients had a revision for pseudotumour. In this series the Kaplan-Meier cumulative revision rate for pseudotumour increased progressively with time. At eight years, in all patients, it was 4% (95% confidence interval (CI) 2.2 to 5.8). Factors significantly associated with an increase in revision rate were female gender (p <
0.001), age under 40 (p = 0.003), small components (p = 0.003), and dysplasia (p = 0.019), whereas implant type was not (p = 0.156). These factors were inter-related, however, and on fitting a Cox proportional hazard model only gender (p = 0.002) and age (p = 0.024) had a significant independent influence on revision rate; size nearly reached significance (p = 0.08). Subdividing the cohort according to significant factors, we found that the revision rate for pseudotumours in men was 0.5% (95% CI 0 to 1.1) at eight years wheras in women over 40 years old it was 6% (95% CI 2.3 to 10.1) at eight years and in women under 40 years it was 13.1% at six years (95% CI 0 to 27) (p <
0.001). We recommend that resurfacings are undertaken with caution in women, particularly those under 40 years of age but they remain a good option in young men. Further work is required to understand the aetiology of pseudotumours so that this complication can be avoided.
We developed a questionnaire to assess patient-reported outcome after surgery of the elbow from interviews with patients. Initially, 17 possible items with five response options were included. A prospective study of 104 patients (107 elbow operations) was carried out to analyse the underlying factor structure, dimensionality, internal and test-retest reliability, construct validity and responsiveness of the questionnaire items. This was compared with the Mayo Elbow performance score clinical scale, the Disabilities of the Arm, Shoulder and Hand questionnaire, and the Short-Form (SF-36) General Health Survey. In total, five questions were considered inappropriate, which resulted in the final 12-item questionnaire, which has been referred to as the Oxford elbow score. This comprises three unidimensional domains, ‘elbow function’, ‘pain’ and ‘social-psychological’; with each domain comprising four items with good measurement properties. This new 12-item Oxford elbow score is a valid measure of the outcome of surgery of the elbow.
