Current undergraduate trauma and life-support training inadequately equips medical students with the knowledge, practical skills and confidence to manage trauma patients. Often first to the scene of medical emergencies, it is imperative junior doctors feel confident and competent from day one. No UK university currently includes advanced trauma and life support (ATLS) in their curriculum. This study piloted an ATLS course for Cardiff final-year medical students to improve confidence and knowledge in management of the trauma patient. Anatomic all-inside ACL reconstruction using TransLateral technique is a relatively new technique that reduces surgical invasion and pain leading to early recovery. We evaluated clinical outcomes of patients undergoing primary anatomic all-inside ACL reconstruction using TransLateral technique. Retrospective case-series evaluating patients undergoing surgery from June 2013 – December 2017. Patients were followed up clinically and using PROMS including EQ-5D, KOOS, IKDC and Tegner scores. Paired two-tailed student t-tests were used to assess clinical significance. 138 patients were included (115 males, 23 females). Mean age was 30 years (range 16.0 – 60.2). Graft choice included isolated semitendinosus (n=115) or both semitendinosus and gracilis (n=26). Mean graft length and diameter were 62.1mm and 8.7mm. Sixteen cases (11.3%) returned to theatre; MUA for arthrofibrosis (n=4), infection (n=2), haemarthrosis (n=1) and metalwork failure (n=1). Incidence of graft re-rupture was 5.7% (n=8); 7 cases were in the mid-bundle femoral tunnel placement. 52.5% (n=74) had complete peri-operative PROMS scores. Mean peri-operative EQ-5D VAS scores were 69.8 and 78.2 (p=0.02). Mean peri-operative KOOS scores for all domains demonstrated significant improvements (p<0.001). Mean peri-operative IKDC scores were 46.1 and 72.5 (p<0.05) and peri-operative Tegner activity scores were 3.3 and 5.3 (p<0.001). Anatomic all-inside ACL reconstruction using TransLateral technique demonstrates favourable clinical and biomechanical advantages including independent anatomic femoral tunnel placement, bone preservation and use of single tendon graft. Patients report significant improvements in pain, functional outcome, quality of life and return to sports. Mid-bundle femoral tunnel placement has been abandoned due to higher failure rate.
Freiberg's Infraction; osteonecrosis of the metatarsal head, is the fourth most common intra-articular osteonecrosis in the body. Surgical intervention is usually reserved for late stage of the disease process (III-V) or failure of conservative management. We evaluated the outcomes of patients treated with primary Interpositional Arthroplasty technique using periosteum and fat for adequate surfacing and as a spacer for Freiberg's Disease. Twenty-three cases (21 patients) were performed from February 2009 – March 2016 (18 women, 5 men). Mean age at surgery was 51.1 years (range 19 – 70.5 years) with 91% affecting the second metatarsal. Twenty-one cases were primary and two cases were revision. Five cases were stage III, 10 were in stage IV and 8 were stage V. All patients underwent Interpositional Arthroplasty using periosteum and fat graft from affected metatarsal inserted as joint spacer and secured with sutures. Patients were followed up by postal questionnaires using two validated questionnaires; MOXFQ and AOFAS. Mean follow-up was 3.7 years (0.6 – 7.6 years). Paired two-tailed student t tests were used to assess clinical significance. The left and right foot was affected in 12 and 11 cases respectively. There were no postoperative infections, non-unions or transfer metatarsalgia. Surgery allowed 8 patients to wear normal footwear, 9 wearing fashion shoes, 5 wearing dress shoes and 5 patients returned to sporting activities. Mean pre-operative and post-operative VAS pain scores were 6.7 (range 4–10) and 3.2 (range 0–0) (p< 0.05). Mean peri-operative AOFAS scores were 43.8 (range 14–14) and 71.3 (range 10–10) (p< 0.05). Mean peri-operative MOXFQ scores were 62.9 (range 23–23) and 31.8 (range 0–0) (p< 0.05). We recommend our novel Interpositional Arthroplasty using periosteum and fat spacer for late stage Freiberg's disease as it can result in significant improvement in pain, prevents donor site morbidity and produces significant functional improvement and patient satisfaction.
Osteolysis has been reported following ACJ reconstruction with a synthetic graft. We present the first study into its prevalence and pattern, and its effect on patient outcome. Patients who underwent treatment of an unstable ACJ injury using the Surgilig/LockDown implant were identified via our database. Patients were invited to attend a dedicated outpatient clinic for clinical examination, radiographic evaluation, and completion of outcome scoring. Patients who were unable to attend were contacted by telephone. 49 patients were identified. We assessed 21 clinically at a mean of 7 years post-procedure (range 3–11 years). All had radiographic evidence of distal clavicle and coracoid osteolysis. We did not observe progression of osteolysis from the final post-operative radiographs. A further 13 were contacted by phone. The mean Oxford Shoulder Score was 43 (range 31–48) and mean DASH score was 8.5 (range 3–71). The average Patient Global Impression of Change score was 6 (range 2–7). Six patients underwent removal of a prominent screw at a mean of 2 years after surgery; the pattern of osteolysis was no different in this group. All patients had comparable abduction, forward flexion and internal rotation to their uninjured shoulder. We did not observe any relationship between patient demographics, position of implant or etiology and the pattern of osteolysis. Osteolysis of the distal clavicle and/or coracoid is always seen following synthetic reconstruction of the ACJ using this implant, but is non-progressive. Range of shoulder movement is largely unaffected and patient outcomes remain high.
Dislocation and instability remain leading cause of failure following THA. We present a single-surgeon 10-year experience with use of Dual Mobility (DM) bearings in Primary and Revision THA using posterior approach. 127 DM bearings were implanted between September 2006 – September 2016; 102 in high-risk primary THA's and 25 revision THA's for either treatment or prevention of instability. Selection for DM bearing followed individual patient risk assessment. Criteria for use of DM bearing were presence of multiple risk factors. Mean age was 72.9 years. 100 Mono-block DM implants, 22 Modular DM implants and 5 custom-made DM devices were implanted. Revision cohort included those used in conjunction with a cage or porous metal augments. 2 dislocations (1.6%) were observed, both in the Revision group, 1 was recurrent requiring revision to constrained liner. Primary group had 2 revisions; 1 peri-prosthetic fracture and 1 deep infection. No DM bearing specific complications were observed. A constructed life table calculated survival function with endpoint set as revision for any reason demonstrated a cumulative survival of 94% at 7.4 years. In high-risk patients, DM bearings are successful at preventing and treating dislocation in THA. Primary cohort in this study all had multiple risk factors for instability but no dislocations or bearing specific complications were observed. Dislocations observed in Revision group were associated with major soft tissue deficiency. This study adds to the promising results already reported with DM THA articulations and should be considered for patients at risk of dislocation or instability. Runner Up – Best Paper Award
Patients undergoing hip fracture surgery have a high peri-operative mortality rate. We performed a retrospective study to ascertain if there is any relation between postoperative haemoglobin (Hb) decrease and cardiac related events following the surgery. We carried out a retrospective study in this University Hospital's trauma unit. All patients operated for fracture neck of femur (hemiarthroplasty and DHS – Dynamic Hip Screw) between July 2006 and August 2008 were included in the study. Electronic records from the trauma unit, pathology portal, operating theatre and blood bank were obtained to identify the pre-operative and post-operative Hb levels, amount of blood transfused and Troponin T (TnT) level.Background and Objective
