The survival of humeral hemiarthroplasties in patients with relatively intact glenoid cartilage could theoretically be extended by minimizing the associated postoperative glenoid erosion. Ceramic has gained attention as an alternative to metal as a material for hemiarthroplasties because of its superior tribological properties. The aim of this study was to assess the in vitro wear performance of ceramic and metal humeral hemiarthroplasties on natural glenoids. Intact right cadaveric shoulders from donors aged between 50 and 65 years were assigned to a ceramic group (n = 8, four male cadavers) and a metal group (n = 9, four male cadavers). A dedicated shoulder wear simulator was used to simulate daily activity by replicating the relevant joint motion and loading profiles. During testing, the joint was kept lubricated with diluted calf serum at room temperature. Each test of wear was performed for 500,000 cycles at 1.2 Hz. At intervals of 125,000 cycles, micro-CT scans of each glenoid were taken to characterize and quantify glenoid wear by calculating the change in the thickness of its articular cartilage.Aims

Ceramic-on-ceramic (COC) bearing surfaces have consistently demonstrated reliable clinical results with when coupled with appropriated designed femoral stems and acetabular shells. Ceramic bearing surfaces are highly wettable and display both boundary and hydrodynamic fluid-film lubrication modes, which lead to extremely low wear rates. Furthermore, COC bearing couples have been shown to exhibit virtually no risk of adverse biologic reaction and have not been associated with corrosion-induced adverse tissue reactions that occur with metal taper junctions, particularly head-neck taper junctions. The relative brittleness of ceramics initially was thought to be a major disadvantage; however, four decades of improvement in the manufacture of ceramics and rigorous proof testing has led an extremely low risk of fracture, perhaps lower than that for cross-linked polyethylene. Aims. We present the ten-year data of a cohort of patients, aged between
18 and 65 years (mean age 52.7 years; 19 to 64), who underwent total
hip arthroplasty. Patients were randomised to be treated with a
cobalt-chrome (CoCr) femoral head with an ultra-high molecular weight
polyethylene (UHMWPE), highly cross-linked polyethylene (XLPE) or
ceramic-on-ceramic (CoC) bearing surface. Patients and Methods. A total of 102 hips (91 patients) were randomised into the three
groups. At ten years, 97 hips were available for radiological and
functional follow-up. Two hips (two patients) had been revised (one
with deep infection and one for periprosthetic fracture) and three
were lost to follow-up. Radiological analysis was performed using
a validated digital assessment programme to give linear, directional
and volumetric wear of the two polyethylene groups. Results. There was a significantly reduced rate of steady-state linear
wear with XLPE (0.07 mm/yr) compared with UHMWPE (0.37 mm/yr) (p
= 0.001). Volumetric wear was also significantly reduced in the
XLPE group (29.29 mm. 3. /yr) compared with the UHMWPE group
(100.75mm. 3. /yr) (p = 0.0001). There were six patients
with UHMWPE who had non-progressive osteolysis and none in the XLPE
group. All three bearing groups had significant improvements in
12-item short form health survey scores, Western Ontario and McMaster
Universities Osteoarthritis Index score and Harris Hip Score. However,
the improvement in HSS was significantly less in the UHMWPE group
(p = 0.0188) than in the other two groups. At ten years, the rates of volumetric and linear wear in the
XLPE group remain low and predominantly below the estimated threshold
for osteolysis (1 mm/yr). The rate of linear wear in the XLPE group
was three times less than in the UHMWPE group at five-year follow-up
and five times less at ten years. The rate of volumetric wear was also
three times less in the XLPE group at ten years. Conclusion. While CoC also performs well, XLPE at ten years remains a safe

The mean age of patients undergoing total knee arthroplasty (TKA) has reduced with time. Younger patients have increased expectations following TKA. Aseptic loosening of the tibial component is the most common cause of failure of TKA in the UK. Interest in cementless TKA has re-emerged due to its encouraging results in the younger patient population. We review a large series of tantalum trabecular metal cementless implants in patients who are at the highest risk of revision surgery. A total of 454 consecutive patients who underwent cementless TKA between August 2004 and December 2021 were reviewed. The mean follow-up was ten years. Plain radiographs were analyzed for radiolucent lines. Patients who underwent revision TKA were recorded, and the cause for revision was determined. We describe the clinical and radiological results of 38 total hip replacements (THR) using the JRI Furlong hydroxyapatite-ceramic (HAC)-coated femoral component in patients younger than 50 years. The mean age at the time of operation was 42 years (22 to 49) and the mean length of follow-up was ten years (63 to 170 months). All patients receiving a Furlong HAC THR were entered into the study regardless of the primary pathology including patients who had undergone previous hip surgery. The mean Harris hip score improved from 44 before operation to 92 at the last postoperative review. After 12 years the cumulative surivival for the stem was 100% (95% confidence interval 89 to 100). Total hip replacement using an alumina head and socket and a titanium alloy stem is evaluated in a series of patients under 50 years of age. Between April 1977 and December 1986, 86 such replacements were performed in 75 patients, but mainly because patients had difficulty travelling from Africa, only 71 hips were followed up adequately; of these, 56 were primary procedures and 15 revisions. Survivorship analysis showed that 98% of the prostheses were retained for 10 years. On clinical and radiological examination 51 of the 71 hips were stable and acceptable, 15 had radiological changes on the acetabular side, and one on the femoral side; four other cases had clinical and radiological changes suggesting impending failure, possibly because fixation of the socket was inadequate. We report the long-term outcome of 218 Charnley low-friction arthroplasties in 141 patients who were 40 years old or younger at the time of surgery. The minimum follow-up was ten years with a mean of 16 years. The probability of the femoral component surviving 20 years was 86% and of the acetabular component, 84%. The chance that both components would survive for this period was 75%. The pathological diagnosis significantly influenced implant survival. In rheumatoid patients the probability of both components surviving at 20 years was 96% compared with 51% in patients with osteoarthritis. Due to issues related to osteolysis which became increasingly evident in the 1990's, approaches to combat wear focused upon either improving ultra-high molecular grade polyethylene or to abandon it and employ alternative bearings: metal upon metal or ceramic upon ceramic (COC). Ceramics have played a role in hip bearings for decades with much of the experience coming from Europe. While there is consistent evidence of low wear rates in this bearing couple due to its surface hardness, wettability and resultant low friction, problems unique to this bearing couple were noted: a small but real incidence of fracture, surface damage due to metal transfer and stripe wear as well as the unique issue of squeaking. What we have learned is that these hard bearings (either COC or Metal on Metal) despite being able to use larger diameter heads, are exquisitely sensitive to component position and orientation. With the tremendous improvements in 2nd and now 3rd generation crosslinked polyethylenes demonstrating vastly reduced wear rates and having none of the issues of fracture, stripe wear, or squeaking, it remains unclear what role ceramic bearings have in modern use. Hip fractures in patients < 60 years old currently account for only 3% to 4% of all hip fractures in England, but this proportion is increasing. Little is known about the longer-term patient-reported outcomes in this potentially more active population. The primary aim is to examine patient-reported outcomes following isolated hip fracture in patients aged < 60 years. The secondary aim is to determine an association between outcomes and different types of fracture pattern and/or treatment implants. All hip fracture patients aged 18 to 60 years admitted to a single centre over a 15-year period were used to identify the study group. Fracture pattern (undisplaced intracapsular, displaced intracapsular, and extracapsular) and type of operation (multiple cannulated hip screws, angular stable fixation, hemiarthroplasty, and total hip replacement) were recorded. The primary outcome measures were the Oxford Hip Score (OHS), the EuroQol five-dimension questionnaire (EQ-5D-3L), and EQ-visual analogue scale (VAS) scores.
