Summary. Revision TKA using CCK prosthesis showed comparable outcome to PS prosthesis in clinical and radiological results. Introduction. In revision total knee arthroplasty (TKA), the goal should be to obtain good motion, function and most importantly stability. The stability depends on remaining soft tissue and implant design. The more the ligaments retain function, the less the implant constraint is needed to achieve stability. With increased constraint, the transfer of joint reaction forces to implant-bone interface may lead to mechanical loosening of the implant. Constrained condylar knee (CCK) prosthesis provides more constraint compared with posterior stabilized (PS) prosthesis. The purpose of this study was to compare the clinical, radiological outcome and survivorship of CCK and PS prosthesis in revision TKA. Materials and Methods. One hundred and twenty-one consecutive revision TKAs using CCK (79 knees) and PS (42 knees) were included. The mean follow-up period was 63.2 months for CCK and 64.8 months for PS. The mean age was 69.5 years and 70.6 years in CCK and PS, respectively. Range of motion (ROM), American Knee Society (AKS) score, Hospital for Special Surgery (HSS) score, complications and failure rate were assessed. Radiographic measurements included tibiofemoral angle and radiolucent lines. The Kaplan-Meier survivorship analysis was performed with an end point of re-revision surgery for any reason and compared between CCK and PS using the Log-rank test. Results. The mean range of motion improved from 97.5° to 115.9° in PS, from 89.5° to 110.1° in CCK. The mean Knee Society knee and functional scores improved from 47.6 and 36.4 to 89.7 and 66.1 with PS, from 53.5 and 41.7 to 79.2 and 66.8 with CCK. The mean Hospital for Special Surgery knee score also improved from 57 to 76.9 and 59.7 to 77.6, respectively. The complication rate was 7.5% (4 cases; 3 recurred infections, 1 instability) in PS and 10.3% (7 cases; 4 recurred infections, 1 periprosthetic fracture, 2 stem tip pains) in CCK. The Kaplan–Meier survivorship analysis revealed that ten year survival of the components was 85.4% for PS and 80.0% for CCK. In all aspects, there were no statistical difference (a p-value of more than 0.05) between PS and CCK. Conclusion. Revision TKA using CCK prosthesis showed comparable outcome to PS prosthesis in clinical and radiological results. CCK prosthesis is a reliable and successful option for prosthesis selection in revision TKA when PS prosthesis is not enough for management of instability

Prosthetic joint infections provide complex management, due to often-difficult diagnosis, need for multiple surgeries and increased technical and financial requirements. “2 in 1” single stage approaches have been advocated due to reduction in risks, costs and complications. This study aimed to investigate the results of single stage revision using metaphyseal sleeves for infected primary Total Knee Replacement (TKR).

Prospective data was collected on all patients presenting with an infected primary TKR over an 8-year period (2009–17). All revision procedures were undertaken in a single stage using metaphyseal sleeves.

26 patients were included, 2 of which had previously failed 2 stage revision and 3 failed DAIR procedures. Mean age was 72.5. Mean BMI was 33.4. Median ASA 2. Mean time to revision was 3.5 years range 3 months to 12 years. Six patients had actively discharging sinuses at the time of surgery. Only 4 of the 26 patients had no positive microbiological cultures from deep tissue samples or joint aspirates.

Only one patient has a recurrence of infection. This patient did not require further surgery and is treated on long term antibiotic suppression and is systemically well.

There were statistically significant improvements in both the pain and function component of AKSS scores. There was no significant improvement in flexion, however mean extension and total range of movement both showed statistically significant improvements.

Using Metaphyseal sleeves in single stage revision for infected TKR are safe and lead to an improvement in pain, function and have excellent efficacy for eradication of infection.

Revision Total Knee Arthroplasty (rTKA) is predicted to increase by more than 600% between 2005 and 2030. The survivorship of primary TKA has been extensively investigated, however more granular information on the risks of rTKA is needed. The aim of the study was to investigate the incidence of re-revision TKA, with explanatory variables of time from primary to revision, and indication (aseptic vs septic). Secondary aim was to investigate mortality. This is an analysis of the Scottish Arthroplasty Project data set, a national audit prospectively recording data on all joint replacements performed in Scotland. The period from 2000 to 2019 was studied. 4723 patients underwent revision TKA. The relationship between time from primary to revision TKA and 2nd revision was significant (p<0.001), with increasing time lowering probability of re-revision (OR 0.99 95% CI 0.987 to 0.993). There was no significant association in time to first revision on time from 1st revision to re-revision (p>0.05). Overall mortality for all patients was 32% at 10 years (95% CI 31-34), Time from primary TKA to revision TKA had a significant effect on mortality: p=0.004 OR 1.03 (1.01-1.05). Septic revisions had a reduced mortality compared to aseptic, OR 0.95 (0.71-1.25) however this was not significant (p=0.69). This is the first study to demonstrate time from primary TKA to revision TKA having a significant effect on probability of re-revision TKA. Furthermore the study suggests mortality is increased with increasing time from primary procedure to revision, however decreased if the indication is septic rather than aseptic

Aims

Femoral periprosthetic fractures are rising in incidence. Their management is complex and carries a high associated mortality. Unlike native hip fractures, there are no guidelines advising on time to theatre in this group. We aim to determine whether delaying surgical intervention influences morbidity or mortality in femoral periprosthetic fractures.

Methods

We identified all periprosthetic fractures around a hip or knee arthroplasty from our prospectively collated database between 2012 and 2021. Patients were categorized into early or delayed intervention based on time from admission to surgery (early = ≤ 36 hours, delayed > 36 hours). Patient demographics, existing implants, Unified Classification System fracture subtype, acute medical issues on admission, preoperative haemoglobin, blood transfusion requirement, and length of hospital stay were identified for all patients. Complication and mortality rates were compared between groups.

Aims

The aim of this study was to describe services available to patients with periprosthetic femoral fracture (PPFF) in England and Wales, with focus on variation between centres and areas for care improvement.

Methods

This work used data freely available from the National Hip Fracture Database (NHFD) facilities survey in 2021, which asked 21 questions about the care of patients with PPFFs, and nine relating to clinical decision-making around a hypothetical case.

Radiological evidence of post-traumatic osteoarthritis (PTOA) after fracture of the tibial plateau is common but end-stage arthritis which requires total knee arthroplasty is much rarer. The aim of this study was to examine the indications for, and outcomes of, total knee arthroplasty after fracture of the tibial plateau and to compare this with an age and gender-matched cohort of TKAs carried out for primary osteoarthritis. Between 1997 and 2011, 31 consecutive patients (23 women, eight men) with a mean age of 65 years (40 to 89) underwent TKA at a mean of 24 months (2 to 124) after a fracture of the tibial plateau. Of these, 24 had undergone ORIF and seven had been treated non-operatively. Patients were assessed pre-operatively and at 6, 12 and > 60 months using the Short Form-12, Oxford Knee Score and a patient satisfaction score. Patients with instability or nonunion needed total knee arthroplasty earlier (14 and 13.3 months post-injury) than those with intra-articular malunion (50 months, p < 0.001). Primary cruciate-retaining implants were used in 27 (87%) patients. Complication rates were higher in the PTOA cohort and included wound complications (13% vs 1% p = 0.014) and persistent stiffness (10% vs 0%, p = 0.014). Two (6%) PTOA patients required revision total knee arthroplasty at 57 and 114 months. The mean Oxford knee score was worse pre-operatively in the cohort with primary osteoarthritis (18 vs 30, p < 0.001) but there were no significant differences in post-operative Oxford knee score or patient satisfaction (primary osteoarthritis 86%, PTOA 78%, p = 0.437). Total knee arthroplasty undertaken after fracture of the tibial plateau has a higher rate of complications than that undertaken for primary osteoarthritis, but patient-reported outcomes and satisfaction are comparable. Introduction. There has been renewed interest in the unicompartmental knee arthroplasty with reports of good long term outcomes. Advantages over a more extensive knee replacement include: preservation of bone stock, retention of both cruciate ligaments, preservation of other compartments and better knee kinematics. However, a number of authors have commented on the problem of osseous defects requiring technically difficult revision surgery. Furthermore, a number of recent national register studies have shown inferior survivorship when compared to total knee arthroplasty. The purpose of this study was to review the cases of our patients who had a revision total knee arthroplasty for failed unicompartmental knee arthroplasty. To determine the reason for failure, describe the technical difficulties during revision surgery and record the clinical outcomes of the revision arthroplasties. Methods. Between 2003 and 2009 our institute performed thirty three revisions of a unicompartmental knee arthroplasty on thirty two patients. The time to revision surgery ranged from 2 months to 159 months with a median of 19 months. Details of the operations and complications were taken form case notes. Patient assessment included range of motion, need for walking aids and the functional status of the affected knee in the form of the Oxford knee score questionnaire. Results. The reasons for failure were aseptic loosening of tibial component, persistent pain, dislocated meniscus, mal-alignment and osteoarthritis in another compartment. Of the 33 revision knee arthroplasties 18 required additional intra-operative constructs. 11 knees required a long tibial stem while 1 required a long femoral stem. 10 knees required medial wedge augmentation and bone graft was used in 6. Mean 1 year Oxford knee scores for failed unicompartmental knee replacements was 29 compared to 39 for primary total knee replacements performed at the same institute. Of the revision knee replacements 2 required further revision due to infection and loosening. Conclusion. From the evidence of our group of failed unicompartmental knee replacements, revision surgery is technically difficult and often requires intra-operative constructs. Clinical outcome of revision total knee arthroplasty following failed unicompartmental knee arthroplasty is not comparable to primary total knee arthroplasty

Understanding the cause of failure of total knee arthroplasties (TKA) is essential in guiding clinical decision making and adjusting treatment concepts for revision surgery. The purpose of the study was to determine current mechanisms of failure of TKA and to describe changes and trends in revision surgery over the last 10 years. A retrospective review was done on all patients who had revision total knee arthroplasty during a 10-year period (2000–2009) at one institution. The preoperative evaluation in conjunction with the intraoperative findings was used to determine causes of failure. All procedures were categorizes as Sharkey et al. described previously. The data was analyzed regarding the cause of failure and displaying the incidence and trends over the last 10 years. 1225 surgeries were done in the time period with a steady increase of procedures per year (34 procedures in 2000 to 196 in 2009). The most common cause of revision TKA was aseptic failure in 65% and septic failure in 31% of the reviewed cases. However, we could observe a steady proportional increase of the septic classified revisions over the time. Both categories could be subdivided to specific causes of failure including aseptic loosening (24%), anterior knee pain (20%), instability (6,4%), arthrofibrosis (4,9%), PE wear (3,6%), malpositioning/malrotation (2,7%) periprosthetic fracture (2,0%) and other (4,6%), or in early (12,9%), late (15,4%) or low-grade infection (3,3%), respectively. Complementary to the classification Sharkey et al. described in 2002 we identified new subcategories of failure: malrotation (since 2003), Low-Grade-Infection (since 2006), allergic failure/loosening (since 2006), Mid-Flexion-Instability (since 2007), soft tissue impingement (since 2009). The incidence of the classic aseptic loosening due to PE wear shows a clear decrease in the last 10 years whereas we could observe an increase of the new diagnosis of instability, malrotation or low-grade-infection as determined cause of failure. The detailed analysis of the failure mechanism in total knee arthroplasty is important to understand the clinical problem and to adjust treatment strategies. We were able to complement present classifications and give a first overview on the incidence for specific causes of failure. Our data shows changes in the indication for surgery over the time and compared to the collective of Sharkey et al. from 1997–2000. This might be due to new diagnostic methods and better implant materials as well as to a generally increased awareness of the specific mechanism of TKA failure

Bicondylar tibial plateau fractures result from high-energy injuries. Fractures of the tibial plateau can involve the tibial tubercle, which represents a disruption to the extensor mechanism and logically must be stabilised. The purpose of this study was to identify the incidence of an independent tibial tubercle fracture in bicondylar tibial plateau fractures, and to report management strategies and potential complications. We retrospectively reviewed a prospectively collected orthopaedic trauma database for the period January 2003 to December 2008, and identified 392 bicondylar fractures of the tibial plateau, in which 85 tibial tubercle fractures (21.6%) were identified in 84 patients. There were 60 men and 24 women in our study group, with a mean age of 45.4 years (18 to 71). In 84 fractures open reduction and internal fixation was undertaken, either with screws alone (23 patients) or with a plate and screws (61 patients). The remaining patient was treated non-operatively. In all, 52 fractures were available for clinical and radiological assessment at a mean follow-up of 58.5 weeks (24 to 94). All fractures of the tibial tubercle united, but 24 of 54 fractures (46%) required a secondary procedure for their tibial plateau fracture. Four patients reported pain arising from prominent tubercle plates and screws, which in one patient required removal. Tibial tubercle fractures occurred in over one-fifth of the bicondylar tibial plateau fractures in our series. Fixation is necessary and can be reliably performed with screws alone or with a screw and plate, which restores the extensor mechanism and facilitates early knee flexion.
