This retrospective study evaluated the midterm clinical and radiographic outcomes of a second-generation total knee replacement system. In a multicentre consecutive series of 1512 patients, 1970 knees were treated with the PFC Sigma knee system (Depuy, Warsaw, Indiana). The patients were reviewed for functional outcome, and underwent independent radiographic evaluation at a mean follow-up of 7.3 years (5 to 10). A total of 40 knees (2%) required revision, 17 (0.9%) for infection. The incidence of osteolysis was 2.2%. The ten-year survival with revision for any cause other than infection as the endpoint was 97.2% (95% CI 95.4 to 99.1). The PFC Sigma knee system appears to provide excellent results in the medium term

Purpose

Little information is available relating to patient demographics, reasons for failure and types of implants used at time of revision following failure of patellofemoral joint (PFJ) replacement.

Methods and Results

Using data extracted from the NJR a series of 128 PFJ revisions in whom the index primary procedure was also recorded in the NJR were identified. This cohort therefore represents early failures of PFJ replacements revised over a 2 year period which were implanted after April 2003 and included revisions of 11 different brands of PFJ replacement from 6 different manufacturers.

The median age at primary procedure was 59.0 (Range 21.1 to 83.2) of which 43 patients were <55 years old (31 males, 97 females). 19% of the revisions were performed in the first year after implantation, in the second year in 33 cases (26%), in the third year in 39 cases (31%) and between years 4 to 7 in 32 patients (25%).

The commonest reasons for revision were pain (35%), aseptic loosening (18%), subluxation, dislocation or instability (11%), PE wear (7%) and component malalignment (6%). No reason for revision was stated in 30% and only 2 cases were revised for infection. Reason for revision differed according to year of failure but was consistent with respect to age at primary surgery. PFJ revision reason differed from those stated for revisions of primary UKR and TKR from the same period with pain being more prevalent and aseptic loosening and infection being less prevalent in the PFJ group. Single stage revision was performed in 124 cases and 118 underwent cemented revision.

Abstract. INTRODUCTION. 10% of patients with knee osteoarthritis (OA) have disease confined to the patellofemoral joint (PFJ). The main surgical options are total knee replacement (TKR) and PFJ replacement (PFJR). PFJR has advantages over TKR, including being less invasive, bone preserving, allowing faster recovery and better function and more ‘straight forward’ revision surgery. We aim to compare the clinical results of revised PFJR with primary TKR taking into consideration the survival length of the PFJR. METHODOLOGY. Twenty-five patients (21 female) were retrospectively identified from our arthroplasty database who had undergone revision from PFJR to TKR (2006–2019). These patients were then matched with regards to their age at their primary procedure, sex and total arthroplasty life (primary PFJ survival + Revision PFJ time to follow up) up to point of follow-up with a group of primary TKRs implanted at the same point as the primary PFJR. RESULTS. Mean survival of the PFJs revised were 4.2 years. In the PFJR revision group (mean arthroplasty life 7.8 years) mean Oxford knee score (OKS) at latest follow up was 27.8. In the primary knee group (mean arthroplasty life 7.5 years) mean OKS was 32.4. This difference was not statistically significant. All PFJR revisions were performed using primary prostheses. CONCLUSION. PFJR provides comparable clinical outcome even after revision surgery to TKR as primary TKRs at midterm follow up and should be considered in all patients meeting the selection criteria. Given comparable proms and straight forward revisions, staged arthroplasty to preserve bone-stock is a reasonable choice

Abstract. Introduction. Derby technique for posterolateral corner (PLC) reconstruction uses a doubled gracilis autograft to reconstruct the popliteo-fibular ligament (PFL) and a split biceps tendon transfer to reconstruct the lateral collateral ligament (LCL). We report midterm outcomes of a case series who underwent PLC reconstruction. Methodology. A retrospective review of 27 patients who underwent PLC reconstruction from 2012 until 2018 was performed (6 females, 21 males, median age 26). Median time interval from injury to primary procedure was 10 months. Outcomes were recorded as per clinical assessment and outcome scores. Results. All patients had no varus laxity in full knee extension. Similarly, at 30° of knee flexion, 25 patients (92.5%) had no varus laxity and 2 patients (7.5%) had grade 1 laxity. No patients demonstrated abnormal external rotation at final follow up. Outcome scores were obtained from 17 patients who underwent primary ligament reconstruction at a mean follow up of 72 months postoperatively. The mean Lysholm score was 94 +/-5.5 (72–100) and mean subjective IKDC score was 78/87 (89.6%) (51–87). 90% of our patients had gained normal or nearly normal IKDC scores on returning to routine activities of daily living and sports following surgery compared to their pre-injury status. Conclusion. Our technique enables an anatomical LCL & PFL reconstruction in isolation or in combination with ACL/PCL reconstruction or to augment an acute repair without contralateral graft harvest or allograft. The functional knee scores demonstrated in this cohort are comparable and in fact superior to previously published knee scores after PLC reconstruction

Aims. Antibiotic-loaded bone cements (ALBCs) may offer early protection against the formation of bacterial biofilm after joint arthroplasty. Use in hip arthroplasty is widely accepted, but there is a lack of evidence in total knee arthroplasty (TKA). The objective of this study was to evaluate the use of ALBC in a large population of TKA patients. Materials and Methods. Data from the National Joint Registry (NJR) of England and Wales were obtained for all primary cemented TKAs between March 2003 and July 2016. Patient, implant, and surgical variables were analyzed. Cox proportional hazards models were used to assess the influence of ALBC on risk of revision. Body mass index (BMI) data were available in a subset of patients. Results. Of 731 214 TKAs, 15 295 (2.1%) were implanted with plain cement and 715 919 (97.9%) with ALBC. There were 13 391 revisions; 2391 were performed for infection. After adjusting for other variables, ALBC had a significantly lower risk of revision for any cause (hazard ratio (HR) 0.85, 95% confidence interval (CI) 0.77 to 0.93; p < 0.001). ALBC was associated with a lower risk of revision for all aseptic causes (HR 0.85, 95% CI 0.77 to 0.95; p < 0.001) and revisions for infection (HR 0.84, 95% CI 0.67 to 1.01; p = 0.06). The results were similar when BMI was added into the model, and in a subanalysis where surgeons using only ALBC over the entire study period were excluded. Prosthesis survival at ten years for TKAs implanted with ALBC was 96.3% (95% CI 96.3 to 96.4) compared with 95.5% (95% CI 95.0 to 95.9) in those implanted with plain cement. On a population level, where 100 000 TKAs are performed annually, this difference represents 870 fewer revisions at ten years in the ALBC group. Conclusion. After adjusting for a range of variables, ALBC was associated with a significantly lower risk of revision in this registry-based study of an entire nation of primary cemented knee arthroplasties. Using ALBC does not appear to increase midterm implant failure rates. Cite this article: Bone Joint J 2019;101-B:1331–1347

Introduction. Our objective is to describe early and midterm results with the use of a new knee prosthesis as an articulating spacer in planned two-stage management for infected total knee arthroplasty. As a second objective, we compared outcomes between the group with a retained first stage and those with a completed 2-stage revision. Methods. Forty-seven patients (48 knees) from January 2012 and November 2017 underwent a 2-stage exchange with an articulating spacer with new implants was used for a chronic knee periprosthetic joint infection with a mean follow-up of 3.7 years (2–6.5 years). The most frequently identified infecting organism was MSSA (31%), MRSA (21%) or MRSE (20%). At the first stage, a new PS femoral component and a new all-polyethylene posterior stabilized (PS) tibial component or a standard PS tibial liner were cemented with antibiotic-cement, typically 3.6 gm tobramycin and vancomycin 1.5 gm. IV antibiotics for six weeks were administered. The planned reimplantation was at 3 months, but ninetteen spacers (14 all poly tibias and 5 tibial liner) were retained for over 12 months. Postoperative assessment included knee range of motion (ROM), quality of life (QOL) scores (SF-12, WOMAC, KOOS, Oxford, and UCLA scores), and a satisfaction scale from 0–100%. Results. Of the 48 knees, 8 failed due to lack of infection control, and 2 died within the first year for medical reasons (4%), giving a failure rate of 17% (8/46). One of these knees was not revised due to poor medical status. Of the remaining 7, 2 had a repeat 2-stage exchange, one a single stage revision and 4 irrigation and debridement with retained implants. All 7 had a successful outcome with infection control after this, leaving a permanent failure rate of 2% (1/46). Five of the 19 knees with initially retained implants were revised to a second stage after 12 months for continued pain or instability (1/14 all poly tibia and 4/5 PS liner). There were no significant differences in final range of motion or QOL scores between patients with a retained first stage procedure and those who underwent a second stage operation. Also, there was no statistically significant difference in the initial failure rate due to infection between patients with a complete 2-stage revision and those with a retained first stage (19% (6/31) vs. 14% (2/14), respectively; p=0.613). Conclusions. When an all poly tibial implant is used a spacer in two-stage exchange arthroplasty, it may be retained for over 12 months, with no difference in infection control, range of motion, and quality of life assessment to patients with complete 2-stage revision surgery. With this technique, the second stage may be delayed in patients who are doing well, and may never need to be revised, hence we propose the new term “one-and-a-half stage exchange”

Aims

Micromotion of the polyethylene (PE) inlay may contribute to backside PE wear in addition to articulate wear of total knee arthroplasty (TKA). Using radiostereometric analysis (RSA) with tantalum beads in the PE inlay, we evaluated PE micromotion and its relationship to PE wear.

Methods

A total of 23 patients with a mean age of 83 years (77 to 91), were available from a RSA study on cemented TKA with Maxim tibial components (Zimmer Biomet). PE inlay migration, PE wear, tibial component migration, and the anatomical knee axis were evaluated on weightbearing stereoradiographs. PE inlay wear was measured as the deepest penetration of the femoral component into the PE inlay.

This retrospective study evaluates the midterm results of the Rotating Platform PFC Sigma Total Knee Arthroplasty System. We reviewed 161 cases in 147 patients with an average follow-up of 5 years (4 to 7 years) operated on at our institution between June 2001 and June 2004. Patient outcomes were measured using the Oxford and American Knee Society Scores and radiographs underwent independent radiological evaluation. Average pre-operative Oxford Scores improved from 43(29-55) to 21 (13 - 44) postoperative;y and average Knee Society Scores improved from 57 (43-70) preoperatively to 80 (58-90) at last follow up. Range of movement improved from 91 degrees (70 - 110) to 113 degrees (75 - 130). There were 5 complications reported; one superficial infection treated with antibiotics, one patient required manipulation under anaesthesia at 3 months for post operative stiffness, which improved. There was one arthrotomy for dislocation of the bearing immediate postoperatively. There was one case of DVT and one non-fatal PE. There were no deep infections. The PFC Sigma Knee Rotating Platform system offers excellent mid-term results in our review

Purpose. To assess the midterm functional results after Medial Patellofemoral Ligament (MPFL) reconstruction with single semitendinosis autograft in patients with patellofemoral instability. Methods - Results. Forty six knees (45 patients) with patellofemoral instability were treated with surgical stabilisation of the patella with a single semitendinosis autograft and followed up for a minimum of 24 months. The average follow-up was 33.5 months (range 24-54 months). Clinical evidence of patellar instability and radiological (MRI) evidence of MPFL rupture or deficiency was documented prior to surgery. The average post injury interval was 22.3 months. Ipsilateral joint co-morbidities included previous lateral release or distal realignment (n=10), patellofemoral joint (PFJ) dysplasia (n=14) and concurrent ACL, PCL and/or MCL rupture (n=6). Knee function was assessed preoperatively and postoperatively with the use of Kujala, Tegner, Lysholm and International Knee Documentation Committee (IKDC) scores. There were 34 male and 11 female patients with a mean age of 32.5 years (range 17 - 60 years). All measured knee function scores improved postoperatively. Kujala score improved from 58 to 77 (95%CI +/− 9.71). Tegner score improved from 3 to 5 (95%CI+/− 0.65). IKDC score improved from 51 to 75 (95%CI +/− 5.27) and Lysholm score improved from 59 to 79 (95%CI +/− 6.5). Conclusions. MPFL reconstruction with semitendinosis autograft is an effective treatment at a minimum of 24 months (midterm) follow-up in patients with patellofemoral instability even in the presence of heterogeneous knee co-morbidities such as PFJ dysplasia, previous lateral release and failed realignment surgery

Aims

Knee osteonecrosis in advanced stages may lead to joint degeneration. Total knee arthroplasty (TKA) for osteonecrosis has traditionally been associated with suboptimal results. We analyzed outcomes of contemporary TKAs for osteonecrosis, with particular emphasis on: survivorship free from aseptic loosening, any revision, and any reoperation plus the clinical outcomes, complications, and radiological results.

Patients and Methods

In total, 156 patients undergoing 167 primary TKAs performed for osteonecrosis between 2004 and 2014 at a single institution were reviewed. The mean age at index TKA was 61 years (14 to 93) and the mean body mass index (BMI) was 30 kg/m2 (18 to 51) The mean follow-up was six years (2 to 12). A total of 110 TKAs (66%) were performed for primary osteonecrosis and 57 TKAs (34%) for secondary osteonecrosis. Overall, 15 TKAs (9%) had tibial stems, while 12 TKAs (7%) had femoral stems. Posterior-stabilized designs were used in 147 TKAs (88%) of TKAs. Bivariate Cox regression analysis was conducted to identify risk factors for revision and reoperation.


Aims

Patient-specific instrumentation of total knee arthroplasty (TKA) is a technique permitting the targeting of individual kinematic alignment, but deviation from a neutral mechanical axis may have implications on implant fixation and therefore survivorship. The primary objective of this randomized controlled study was to compare the fixation of tibial components implanted with patient-specific instrumentation targeting kinematic alignment (KA+PSI) versus components placed using computer-assisted surgery targeting neutral mechanical alignment (MA+CAS). Tibial component migration measured by radiostereometric analysis was the primary outcome measure (compared longitudinally between groups and to published acceptable thresholds). Secondary outcome measures were inducible displacement after one year and patient-reported outcome measures (PROMS) over two years. The secondary objective was to assess the relationship between alignment and both tibial component migration and inducible displacement.

Patients and Methods

A total of 47 patients due to undergo TKA were randomized to KA+PSI (n = 24) or MA+CAS (n = 23). In the KA+PSI group, there were 16 female and eight male patients with a mean age of 64 years (sd 8). In the MA+CAS group, there were 17 female and six male patients with a mean age of 63 years (sd 7). Surgery was performed using cemented, cruciate-retaining Triathlon total knees with patellar resurfacing, and patients were followed up for two years. The effect of alignment on tibial component migration and inducible displacement was analyzed irrespective of study group.

Aims

Little is known about the risk factors that predispose to a rupture of the posterior cruciate ligament (PCL). Identifying risk factors is the first step in trying to prevent a rupture of the PCL from occurring. The morphology of the knee in patients who rupture their PCL may differ from that of control patients. The purpose of this study was to identify any variations in bone morphology that are related to a PCL.

Patients and Methods

We compared the anteroposterior (AP), lateral, and Rosenberg view radiographs of 94 patients with a ruptured PCL to a control group of 168 patients matched by age, sex, and body mass index (BMI), but with an intact PCL after a knee injury. Statistical shape modelling software was used to assess the shape of the knee and determine any difference in anatomical landmarks.

Aims

A retrospective study was conducted to measure short-term in vivo linear and volumetric wear of polyethylene (PE) inserts in 101 total knee arthroplasty (TKA) patients using model-based radiostereometric analysis (MBRSA).

Patients and Methods

Nonweightbearing supine RSA exams were performed postoperatively and at six, 12, and 24 months. Weightbearing standing RSA exams were performed on select patients at 12 and 24 months. Wear was measured both linearly (joint space) and volumetrically (digital model overlap) at each available follow-up. Precision of both methods was assessed by comparing double RSA exams. Patient age, sex, body mass index, and Oxford Knee Scores were analyzed for any association with PE wear.

Aims

The aim of this study was to assess the effect of posterior cruciate ligament (PCL) resection on flexion-extension gaps, mediolateral soft-tissue laxity, fixed flexion deformity (FFD), and limb alignment during posterior-stabilized (PS) total knee arthroplasty (TKA).

Patients and Methods

This prospective study included 110 patients with symptomatic osteoarthritis of the knee undergoing primary robot-assisted PS TKA. All operations were performed by a single surgeon using a standard medial parapatellar approach. Optical motion capture technology with fixed femoral and tibial registration pins was used to assess gaps before and after PCL resection in extension and 90° knee flexion. Measurements were made after excision of the anterior cruciate ligament and prior to bone resection. There were 54 men (49.1%) and 56 women (50.9%) with a mean age of 68 years (sd 6.2) at the time of surgery. The mean preoperative hip-knee-ankle deformity was 4.1° varus (sd 3.4).

The use of hinged implants in primary total knee replacement (TKR) should be restricted to selected indications and mainly for elderly patients. Potential indications for a rotating hinge or pure hinge implant in primary TKR include: collateral ligament insufficiency, severe varus or valgus deformity (> 20°) with necessary relevant soft-tissue release, relevant bone loss including insertions of collateral ligaments, gross flexion-extension gap imbalance, ankylosis, or hyperlaxity. Although data reported in the literature are inconsistent, clinical results depend on implant design, proper technical use, and adequate indications. We present our experience with a specific implant type that we have used for over 30 years and which has given our elderly patients good mid-term results. Because revision of implants with long cemented stems can be very challenging, an effort should be made in the future to use shorter stems in modular versions of hinged implants.

Structural allografts may be used to manage uncontained bone defects in revision total knee replacement (TKR). However, the availability of cadaver grafts is limited in some areas of Asia. The aim of this study was to evaluate the mid-term outcome of the use of femoral head allografts for the reconstruction of uncontained defects in revision TKR, focusing on complications related to the graft.

We retrospectively reviewed 28 patients (30 TKRs) with Anderson Orthopaedic Research Institute (AORI) type 3 bone defects, who underwent revision using femoral head allografts and stemmed components. The mean number of femoral heads used was 1.7 (1 to 3). The allograft–host junctions were packed with cancellous autograft.

At a mean follow-up of 76 months (38 to 136) the mean American Knee Society knee score improved from 37.2 (17 to 60) pre-operatively to 90 (83 to 100) (p < 0.001). The mean function score improved from 26.5 (0 to 50) pre-operatively to 81 (60 to 100) (p < 0.001). All the grafts healed to the host bone. The mean time to healing of the graft was 6.6 months (4 to 16). There have been no complications of collapse of the graft, nonunion, infection or implant loosening. No revision surgery was required.

The use of femoral head allografts in conjunction with a stemmed component and autogenous bone graft in revision TKR in patients with uncontained bone defects resulted in a high rate of healing of the graft with minimal complications and a satisfactory outcome. Longer follow-up is needed to observe the evolution of the graft.

This prospective study describes the complications and survival of the first 688 Phase 3 Oxford medial unicompartmental knee replacements implanted using a minimally-invasive technique by two surgeons and followed up independently. None was lost to follow-up. We had carried out 132 of the procedures more than five years ago. The clinical assessment of 101 of these which were available for review at five years is also presented.

Nine of the 688 knees were revised: four for infection, three for dislocation of the bearing and two for unexplained pain. A further seven knees (1%) required other procedures: four had a manipulation under anaesthesia, two an arthroscopy and one a debridement for superficial infection. The survival rate at seven years was 97.3% (95% confidence interval 5.3). At five years, 96% of the patients had a good or excellent American Knee Society score, the mean Oxford knee score was 39 and the mean flexion was 133°. This study demonstrates that the minimally-invasive Oxford unicompartmental knee replacement is a reliable and effective procedure.
