Purpose. The success of total knee replacement (TKR) surgery can be attributed to improvements in TKR design, instrumentation, and surgical technique. Over a decade ago oxidized zirconium (OxZr) femoral components were introduced as an alternative bearing surface to cobalt-chromium (CoCr), based on strong in-vitro evidence, to improve the longevity of TKR implants. Early reports have demonstrated the clinical success of this material however no long-term comparative studies have demonstrated the superiority of OxZr implants compared to a more traditional CoCr implant. This study aims to compare long-term survivorship and outcomes in OxZr and CoCr femoral components in a single total knee design. Methods. We reviewed our institutional database to identify all patients whom underwent a TKA with a posterior stabilized OxZr femoral component with a minimum of 10 years of follow-up. These were then matched to patients whom underwent a TKA with the identical design posterior stabilized CoCr femoral component during the same time period by gender, age and BMI. All patients had their patella resurfaced. All patients were prospectively evaluated preoperatively and postoperatively at 6 weeks, 3 months, 12 months, 2 years and every 1 to 2 years thereafter. Prospectively collected clinical outcome measures included, Western Ontario and McMaster Universities osteoarthritis index (WOMAC), Short-Form 12 (SF-12) and Knee Society clinical rating scores (KSCRS). Charts and radiographs were reviewed to determine the revision rates and survivorship (both all cause and aseptic) at 10 years allowing comparison between the two cohorts. Paired analysis was performed to determine if differences existed in patient reported outcomes. Results. There were 194 OxZr TKAs identified and matched to 194 CoCr TKAs. There was no difference in average age (OxZr, 54.2 years; CoCr, 54.4 years), Gender (OxZr, 61.9% female; CoCr, 61.9% female) and average body mass index (OxZr, 35.9 kg/m2; CoCr, 36.4 kg/m2) between the two cohorts. There were no significant differences preoperatively in any of the outcome scores between the two groups (WOMAC (p=0.361), SF-12 (p=0.771) and KSCRS (p=0.161)). Both the SF12 (p=0.787) and WOMAC (p=0.454) were similar between the two groups, however the OxZr TKA cohort had superior KSCRS compared to the CoCr TKA cohort at a minimum of 10 years (173.5 vs. 159.1, p=0.002). With revision for any reason as the end point, there was no significant difference in 10-year survivorship between the two groups (OxZr and CoCr, 96.4%, p=0.898). Similarly, aseptic revisions demonstrated comparable survivorship rates at 10-year between the OxZr (98.9%) and CoCr groups (97.9%) (p=0.404). Conclusion. In this matched cohort study, both groups demonstrated similar improvements in patient reported outcomes, although the OxZr cohort had greater KSCRS scores compared to the CoCr cohort. The reason for this difference is not clear but may represent selection bias, where OxZr implants were perhaps used in more active patients. Implant survivorship, based on revision rates for all causes and/or aseptic reasons, was excellent and similar for both the OxZr and CoCr femoral components at 10 years. Therefore, with respect to implant longevity at the end of the first decade, there appears to be no clear advantage of OxZr compared to CoCr for patients with similar demographics with this specific posterior stabilized TKA design. Further follow-up into the second and third decade may be required to demonstrate if a difference does exist

Introduction. The aim of this study was to investigate whether methylene blue dye, commonly used in sterile surgical marker pens, would have an effect on human chondrocyte viability, when cultured on a collagen membrane in-vitro. Methods. Bilayered collagen membranes were seeded in duplicate with 12 million human chondrocytes per ml and cultured for 24 hours under standard conditions. Group A consisted of a membrane marked with methylene blue ink on its smooth side, group B marked on its porous side, and group C acting as an unmarked control. At the end of the culture period the membranes were qualitatively analysed for cell survival by live/dead fluorescent staining under confocal microscopy. Results. The control membranes of group C showed normal ‘live’ staining of cells. A marked reduction in chondrocyte viability was seen in groups A and B, with a significant reduction in viability seen opposite to the ink mark on the smooth side in A, and practically no cell viability in the same position on the porous side in group B. Conclusions. Methylene blue dye in surgical marker pen ink has been shown to be cytotoxic to chondrocytes cultured on bilayered collagen membranes. Sterile surgical marker pens are commonly used in cartilage repair surgery to aid in the placement of periosteal patches or collagen membranes in autologous chondrocyte transplantation. Surgeons need to be aware of the potential harmful effects of methylene blue dye when using surgical marker pens in cell based regenerative therapies

Introduction. Differing descriptions of patellar motion relative to the femur have resulted from many in-vitro and in-vivo studies. The aim of this study was to examine the tracking behaviour of the patella. We hypothesized that patellar kinematics would correlate to the trochlear geometry. Method. Seven normal fresh-frozen knees were CT scanned and their kinematics with quadriceps loading was measured by an optical tracker system and calculated in relation to the previously-established femoral axes. CT scans were used to reliably define frames of reference for the femur, tibia and the patella. A novel trochlear axis was defined, between the centres of best-fit medial and lateral trochlear articular surfaces spheres. Results. The path of the centre of the patella was circular and uniplanar (RMS error 0.3mm) above 16°±3° knee flexion. The distal end of the median ridge of the patella entered the groove at 6° knee flexion, and the midpoint at 22°. This circle was aligned 6°±2° (mean± SD) from the femoral anatomical axis, 91°±3° from the epicondylar axis, and 88°±3° from the trochlear axis, in the coronal plane. In the transverse plane it was 91°±3° and 88°±3° from the epicondylar and trochlear axes. Manipulation of the data using a transformation matrix to the previously reported anatomic axis of the femur showed an initial medial and then lateral translation. Discussion. The motion path of the patella is dominantly circular in normal knees, when observed using a trochlea specific frame of reference. There is lateral translation in the last 16 degrees of extension, but in the rest of the range of motion, the patella motion path is on a single circular path with an RMS of less than 0.5mm. The two plane motion paths previously described can be replicated by a geometric transformation. Patella kinematics and patho-kinematics are more easily understood using this method

Purpose. The purpose of this study was to determine whether intra-operative identification of osseous ridge anatomy (lateral intercondylar “residents” ridge and lateral bifurcate ridge) could be used to reliably define and reconstruct individuals' native femoral ACL attachments in both single-bundle (SB) and double-bundle (DB) cases. Methods. Pre-and Post-operative 3D, surface rendered, CT reconstructions of the lateral intercondylar notch were obtained for 15 patients undergoing ACL reconstruction (11 Single bundle, 4 Double-bundle or Isolated bundle augmentations). Morphology of native ACL femoral attachment was defined from ridge anatomy on the pre-operative scans. Centre's of the ACL attachment, AM and PL bundles were recorded using the Bernard grid and Amis' circle methods. During reconstruction soft tissue was carefully removed from the lateral notch wall with RF coblation to preserve and visualise osseous ridge anatomy. For SB reconstructions the femoral tunnel was sited centrally on the lateral bifurcate ridge, equidistant between the lateral intercondylar ridge and posterior cartilage margin. For DB reconstructions tunnels were located either side of the bifurcate ridge, leaving a 2mm bony bridge. Post-operative 3D CTs were obtained within 6 weeks post-op to correlate tunnel positions with pre-op native morphology. Results. Pre-op native ACL attachment site morphology was very similar to previous in-vitro studies: the mean centre was found at 27% along Blumensaat's line (range 19-33%) and 38% the width of the lateral femoral condyle (range 31-43%). Despite the variability between individuals there was close correlation between pre-operative localization of the femoral attachment centre and position of single bundle ACL reconstructions tunnels on the post-op CT (R=0.92). Similar results were observed for double-bundle and isolated bundle augment reconstructions. Conclusion. ACL attachment site morphology varies between individuals. Intra-operative localization of the osseous landmarks (lateral intercondylar and bifurcate ridges) appears to lead to accurate, individualised anatomical tunnel placement whether using single or double-bundle reconstruction techniques
