Introduction. Failure of metal on metal (MOM) total hip arthroplasty (THA) and resurfacing arthroplasty (HRA) due to development of adverse local tissue reaction (ALTRs) is a significant problem. The prevalence of ALTRs in asymptomatic MOM arthroplasty patients is highly variable. The purpose of this prospective, longitudinal study was to: 1) determine MRI ALTR prevalence in patients with HRA; 2) determine if patients with HRA have a greater rate of MRI ALTRs compared to control patients with ceramic on poly (COP) THA; and 3) evaluate changes in patient reported outcomes between these implant designs. Methods. Following IRB approval with informed consent, self-reported asymptomatic primary COP and HRA patients greater than one year post arthroplasty were evaluated with 4 annual (TP1–4) MRIs using a standardized protocol and serum ion level testing. Morphologic and susceptibility reduced images were acquired for each hip and evaluated for synovial thickness, volume, capsule dehiscence and the presence of ALTR. Patient reported outcomes were evaluated by Hip Disability and Osteoarthritis Outcome Scores (HOOS). Analyses were performed to detect differences of synovial thickness and volume, and HOOS subgroups between and within bearing surfaces at each time point and over time, and to compare the time to and the risk of developing MRI ALTR. Analyses were adjusted for age, gender, and length of implantation. Results. 86 hips were evaluated at the initial time point (TP1): HRA, n=41, age=58±8 y.o., 34/7 (M/F); COP, n=45, age=65±9 y.o., 22/23 (M/F). The mean synovial volume of the subjects was similar at TP1, and the synovial volume in HRA subjects increased significantly from TP1 to TP4 (mean volume difference=9 cm3, p=0.031) while COP subjects displayed a minimal increase from TP1 to TP4 (mean volume difference =0.3 cm3, p=0.9). The synovial volume in HRA subjects tended to be larger and with greater variability than COP subjects at TP4 (HRA=15±58 cm3; COP=6 ±10 cm3, p=0.3). By TP4, MRI ALTRs developed in 13/41 HRA subjects (mean time to ALTR=1.5 yr) compared to 2/25 COP subjects (mean time to ALTR=2.0 yr), p=0.001. While risk of MRI ALTRs in HRA subjects was 9.9 times the risk in COP subjects (p=0.07), 68% of HRA subjects did not have an MRI ALTR present. Significant differences of [Co] and [Cr] in HRA subjects with MRI ALTR present were not detected at 2/4 time points and 1/4 time points, respectively, and there were no significant differences in ion levels in the COP group. The HOOS sub-scores of Pain and Activity of Daily Living scores of HRA subjects were similar to COP subjects, and HRA subjects had better Sporting Ability than COP subjects at all time points. HRA subjects had higher Symptoms at TP1 (p=0.016) but both cohorts had high scores (HRA=92.3/100; COP=87.7/100). Initially dehiscent posterior capsules of HRA subjects (9%) tended not to resolve (10/11, 91%), while dehiscent COP subjects (15%) had greater resolution (6/22, 27%). The presence of osteolysis was limited (HRA: 6/41; COP: 3/45; p=0.101). Conclusion. A greater synovial volume and higher rate of MRI ALTRs was found in HRA subjects, while the patient symptomatology via HOOS remained slightly better in HRA subjects than COP subjects, suggesting a poor association between MRI ALTRs and symptoms. The presence of posterior decompression of the posterior capsule into the bursa alone was not necessarily indicative of an ALTR, and is likely a normal postoperative finding. Introduction. Magnetic resonance arthrogram (MRA) is the investigation of choice in young adults with symptoms of femero acetabular impingement. The aim of the study is to assess the accuracy of MRA when correlated with surgical findings on hip arthroscopy. Materials and Method. Between June 2007 and January 2014, 136 patients had MRA followed by subsequent arthroscopy at our institution. The radiology information system was used to gather MRA data. All scans were reported by a consultant radiologist with a specialist interest in musculoskeletal radiology. Patient records were reviewed to gather surgical data. Assessment was made of labral injury, Camshaft (CAM) or Pincer lesion and degeneration of the cartilage. Results. Mean age at MRA was 34.5 years with a 2.5:1 female to male ratio. For labral pathology sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) were 89, 100, 100 and 22% respectively. For CAM (62, 80, 79 and 64% respectively) and Pincer lesions (15, 84, 81 and 18% respectively). MRA was specific for picking up Grade 3/4 degenerate changes in the articular cartilage (92.2%) but was not as sensitive (60%), PPV 55 and NPV 94. Conclusion. The primary aim of this study was to determine the ten-year outcome following surgical treatment for femoroacetabular impingement (FAI). We assessed whether the evolution of practice from open to arthroscopic techniques influenced outcomes and tested whether any patient, radiological, or surgical factors were associated with outcome. Prospectively collected data of a consecutive single-surgeon cohort, operated for FAI between January 2005 and January 2015, were retrospectively studied. The cohort comprised 393 hips (365 patients; 71% male (n = 278)), with a mean age of 34.5 years (SD 10.0). Over the study period, techniques evolved from open surgical dislocation (n = 94) to a combined arthroscopy-Hueter technique (HA + Hueter; n = 61) to a pure arthroscopic technique (HA; n = 238). Outcome measures of interest included modes of failures, complications, reoperation, and patient-reported outcome measures (PROMs). Leg length discrepancy (LLD) is a common pre- and postoperative issue in total hip arthroplasty (THA) patients. The conventional technique for measuring LLD has historically been on a non-weightbearing anteroposterior pelvic radiograph; however, this does not capture many potential sources of LLD. The aim of this study was to determine if long-limb EOS radiology can provide a more reproducible and holistic measurement of LLD. In all, 93 patients who underwent a THA received a standardized preoperative EOS scan, anteroposterior (AP) radiograph, and clinical LLD assessment. Overall, 13 measurements were taken along both anatomical and functional axes and measured twice by an orthopaedic fellow and surgical planning engineer to calculate intraoperator reproducibility and correlations between measurements.Aims

In order to release the contracture band completely without damaging normal tissues (such as the sciatic nerve) in the surgical treatment of gluteal muscle contracture (GMC), we tried to display the relationship between normal tissue and contracture bands by magnetic resonance neurography (MRN) images, and to predesign a minimally invasive surgery based on the MRN images in advance. A total of 30 patients (60 hips) were included in this study. MRN scans of the pelvis were performed before surgery. The contracture band shape and external rotation angle (ERA) of the proximal femur were also analyzed. Then, the minimally invasive GMC releasing surgery was performed based on the images and measurements, and during the operation, incision lengths, surgery duration, intraoperative bleeding, and complications were recorded; the time of the first postoperative off-bed activity was also recorded. Furthermore, the patients’ clinical functions were evaluated by means of Hip Outcome Score (HOS) and Ye et al’s objective assessments, respectively.Aims

A borderline dysplastic hip can behave as either stable or unstable and this makes surgical decision making challenging. While an unstable hip may be best treated by acetabular reorientation, stable hips can be treated arthroscopically. Several imaging parameters can help to identify the appropriate treatment, including the Femoro-Epiphyseal Acetabular Roof (FEAR) index, measured on plain radiographs. The aim of this study was to assess the reliability and the sensitivity of FEAR index on MRI compared with its radiological measurement. The technique of measuring the FEAR index on MRI was defined and its reliability validated. A retrospective study assessed three groups of 20 patients: an unstable group of ‘borderline dysplastic hips’ with lateral centre edge angle (LCEA) less than 25° treated successfully by periacetabular osteotomy; a stable group of ‘borderline dysplastic hips’ with LCEA less than 25° treated successfully by impingement surgery; and an asymptomatic control group with LCEA between 25° and 35°. Cardiac magnetic resonance (CMR) was used to assess whether cardiac function or tissue composition was affected in patients with well-functioning metal-on-metal hip resurfacing arthroplasties (MoMHRA) when compared with a group of controls, and to assess if metal ion levels correlated with any of the functional or structural parameters studied. In all, 30 participants with no significant cardiac history were enrolled: 20 patients with well-functioning MoMHRA at mean follow-up of 8.3 years post-procedure (ten unilateral, ten bilateral; 17 men, three women) and a case-matched control group of ten non-MoM total hip arthroplasty patients (six men, four women). The mean age of the whole cohort (study group and controls) at the time of surgery was 50.6 years (41.0 to 64.0). Previous studies have suggested that metal-on-metal (MoM) Pinnacle (DePuy Synthes, Warsaw, Indiana) hip arthroplasties implanted after 2006 exhibit higher failure rates. This was attributed to the production of implants with reduced diametrical clearances between their bearing surfaces, which, it was speculated, were outside manufacturing tolerances. This study aimed to better understand the performance of Pinnacle Systems manufactured before and after this event. A total of 92 retrieved MoM Pinnacle hips were analyzed, of which 45 were implanted before 2007, and 47 from 2007 onwards. The ‘pre-2007’ group contained 45 implants retrieved from 21 male and 24 female patients, with a median age of 61.3 years (interquartile range (IQR) 57.1 to 65.5); the ‘2007 onwards’ group contained 47 implants retrieved from 19 male and 28 female patients, with a median age of 61.8 years (IQR 58.5 to 67.8). The volume of material lost from their bearing and taper surfaces was measured using coordinate and roundness measuring machines. We investigated the changes seen on serial metal
artefact reduction magnetic resonance imaging scans (MARS-MRI) of
metal-on-metal total hip arthroplasties (MoM THAs). In total 155
THAs, in 35 male and 100 female patients (mean age 70.4 years, 42
to 91), underwent at least two MRI scans at a mean interval of 14.6
months (2.6 to 57.1), at a mean of 48.2 months (3.5 to 93.3) after
primary hip surgery. Scans were graded using a modification of the
Oxford classification. Progression of disease was defined as an
increase in grade or a minimum 10% increase in fluid lesion volume
at second scan. A total of 16 hips (30%) initially classified as
‘normal’ developed an abnormality on the second scan. Of those with
‘isolated trochanteric fluid’ 9 (47%) underwent disease progression,
as did 7 (58%) of ‘effusions’. A total of 54 (77%) of hips initially
classified as showing adverse reactions to metal debris (ARMD) progressed,
with higher rates of progression in higher grades. Disease progression
was associated with high blood cobalt levels or an irregular pseudocapsule
lining at the initial scan. There was no association with changes
in functional scores. Adverse reactions to metal debris in MoM THAs
may not be as benign as previous reports have suggested. Close radiological

The aim of this cadaver study was to identify
the change in position of the sciatic nerve during arthroplasty
using the posterior surgical approach to the hip. We investigated
the position of the nerve during this procedure by dissecting 11
formalin-treated cadavers (22 hips: 12 male, ten female). The distance
between the sciatic nerve and the femoral neck was measured before
and after dislocation of the hip, and in positions used during the
preparation of the femur. The nerve moves closer to the femoral
neck when the hip is flexed to >
30° and internally rotated to 90° (90°

In this meta-analysis we included 32 English-language articles published between January 1975 and June 2004 on the diagnostic performance of plain radiography, subtraction arthrography, nuclear arthrography and bone scintigraphy in detecting aseptic loosening of the femoral component, using criteria based on the Cochrane systematic review of screening and diagnostic tests. The mean sensitivity and specificity were, respectively, 82% (95% confidence interval (CI) 76 to 87) and 81% (95% CI 73 to 87) for plain radiography and 85% (95% CI 75 to 91) and 83% (95% CI 75 to 89) for nuclear arthrography. Pooled sensitivity and specificity were, respectively, 86% (95% CI 74 to 93) and 85% (95% CI 77 to 91) for subtraction arthrography and 85% (95% CI 79 to 89) and 72% (95% CI 64 to 79) for bone scintigraphy. Although the diagnostic performance of the imaging techniques was not significantly different, plain radiography and bone scintigraphy are preferred for the assessment of a femoral component because of their efficacy and lower risk of patient morbidity.
Femoroacetabular Junction Impingement (FAI) describes abnormalities
in the shape of the femoral head–neck junction, or abnormalities
in the orientation of the acetabulum. In the short term, FAI can
give rise to pain and disability, and in the long-term it significantly increases
the risk of developing osteoarthritis. The Femoroacetabular Impingement
Trial (FAIT) aims to determine whether operative or non-operative
intervention is more effective at improving symptoms and preventing
the development and progression of osteoarthritis. FAIT is a multicentre superiority parallel two-arm randomised
controlled trial comparing physiotherapy and activity modification
with arthroscopic surgery for the treatment of symptomatic FAI.
Patients aged 18 to 60 with clinical and radiological evidence of
FAI are eligible. Principal exclusion criteria include previous
surgery to the index hip, established osteoarthritis (Kellgren–Lawrence
≥ 2), hip dysplasia (centre-edge angle <
20°), and completion
of a physiotherapy programme targeting FAI within the previous 12
months. Recruitment will take place over 24 months and 120 patients
will be randomised in a 1:1 ratio and followed up for three years.
The two primary outcome measures are change in hip outcome score
eight months post-randomisation (approximately six-months post-intervention
initiation) and change in radiographic minimum joint space width
