Introduction. Between 2002–2009 we operatively treated 193 acetabular fracture. Among these 44 both-columns fractures according to the classification of Letournel and Judet have been reviewed in order to evaluate the results. These fractures are rare, difficult to treat and often have poor clinical results. Patients and Methods. 44 cases of ORIF of displaced both columns fractures have been studied at a mean 37 months follow-up (range, 13 to 76 months) after the injury. 40 hips were operated with the ilioinguinal approach alone, 1 with Kocker Langenbeck and Smith petersen combined. The 3 remaining hips were operated with a double Kocher-Langenbeck and ilioinguinal approach. Every case was evalueted on X-rays according to Matta criteria and clinically with the Harris hip score and the WOMAC score. Results. The mean Harris hip score has been 85,8% (range 30%–100%) while the mean WOMAC score has been 88,3 points (range 39,1–100). The main complications were 4 early post-traumatic arthritis operated by hip replacement. One patient instead developed a gastric carcinoma. Discussion. The rate of anatomical reduction decreased with the complexity of the fracture, the age, and the interval between the injury and the reduction. The clinical result was adversely affected by associated injuries of the femoral head, an older age of the patient. It was positively affected by an anatomical reduction and postoperative congruity between the femoral head and the acetabular roof. Conclusions. These findings indicate that in many patients who have a complex acetabular fracture the hip joint can be preserved and post-traumatic osteoarthritis can be avoided if an anatomical reduction is achieved. Both column acetabular fractures can be reduced with a combined surgical approaches or with ilioinguinal alone. We used the ilioinguinal approach alone in 91% of cases reaching good results, even comparing Letournel and Matta's results. Surgeon experience and a long learning curve can help in decision making and in obtaining an anatomical reduction with a low rate of post-operative complications

Introduction: Minimal invasive surgery (MIS) seems to be part of future orthopaedic solutions. Currently, most approaches for the Bernese periacetabular osteotomy (PAO) are characterized by relatively extensive incisions, dissection and detachment of muscles. We have developed a new MIS approach for the Bernese PAO. The purposes were to reduce patient morbidity and to improve the cosmetic result following surgery without negatively influencing the achieved reorientation of the acetabular articular surface. In this study we present the surgical technique, results and compare them to the ilioinguinal (II) approach. Methods: The new MIS technique is a trans-sartorial approach using a three inch skin incision. Previously the II approach was used. From 1999–2006 a total of 215 patients with acetabular dysplasia were operated by the same surgeon in two successive time periods with the II (97) and the trans-sartorial (118) approaches. No supplemental surgery was performed. The two approaches are retrospectively compared regarding perioperative measures, transfusion requirements, complications and the achieved reorientation of the acetabular articular surface. Data are compared by Kruskal-Wallis Test and are presented as median and interquartile range. Results: The trans-sartorial approach significantly reduced days of admission (8 days (7–9) vs. 10 days (8–13), p< 0.0001), duration of surgery (70 min (60–75) vs. 100 min (82.5–120), p< 0.0001), perioperative blood loss (200ml (150–350) vs. 450ml (325–700), p< 0.0001) and the percentage of patients receiving blood transfusion (18.6 % vs. 3.4%). Of severe neurovascular, infectious and technical complications none occurred in the trans-sartorial group and 3 cases of arterial thrombosis were seen in the II group. The achieved reorientation measured by the CE-angle postoperatively had median values of 31° (25–36) in the II group and 33° (29–36) in the trans-sartorial group, p=0.016. The postoperative AI-angles were 10° (2–14) and 3° (0–7) in the II and trans-sartorial groups respectively, p< 0.0001. Discussion: Our shift to the trans-sartorial approach was rewarding as the duration of surgery, perioperative blood loss and transfusion requirements were reduced. The new MIS technique is safe and improves the cosmetic result without negative influence on the achieved reorientation of the acetabular articular surface

This study compared the quality of reduction and complication rate when using a standard ilioinguinal approach and the new pararectus approach when treating acetabular fractures surgically. All acetabular fractures that underwent fixation using either approach between February 2005 and September 2014 were retrospectively reviewed and the demographics of the patients, the surgical details and complications were recorded. . A total of 100 patients (69 men, 31 women; mean age 57 years, 18 to 93) who were consecutively treated were included for analysis. The quality of reduction was assessed using standardised measurement of the gaps and steps in the articular surface on pre- and post-operative CT-scans. . There were no significant differences in the demographics of the patients, the surgical details or the complications between the two approaches. A significantly better reduction of the gap, however, was achieved with the pararectus approach (axial: p = 0.025, coronal: p = 0.013, sagittal: p = 0.001). . These data suggest that the pararectus approach is at least equal to, or in the case of reduction of the articular gap, superior to the ilioinguinal approach. . This approach allows direct buttressing of the dome of the acetabulum and the quadrilateral plate, which is particularly favourable in geriatric fracture patterns. Since 1989 we have treated most rotationally or vertically unstable pelvic fractures operatively. An anterior extra peritoneal approach has been used to achieve access to all parts of the anterior ring. This can be combined with the lateral approach on the iliac wing or with posterior approach for the SI and sacral lesions. The extra peritoneal midline approach is created through a 10–15 cm long midline incision beginning from the symphysis. The rectus muscles are not detached. Blunt preparation along the superior ramus gives more space laterally and reveals the obturator foramen. The corona mortis vessels are ligated. The iliac vessels, femoral nerve and the psoas muscle can be gentle elevated with a long hook. The eminential area, linea terminalis as well as the quadrilateral space are then visualised. All essential fragments can be reduced and fixed with plates and screws.

Our study of 101 patients with an unstable pelvic ring (68 rotationally and vertically unstable injuries, 21 lateral compression injuries and 12 open book injuries) showed excellent or good reduction in 88, fair in 11 and poor in 2 cases. The overall functional results were excellent or good in 83, fair in 13 and poor in 5 patients. The correlation between anatomical reduction and good functional result was clear.

Our experience and new data strongly support the use of ORIF in Type C pelvic ring injuries, in Type B- open book injuries, and in markedly displaced Type-B lateral compression injuries. Good reduction and a reliable stability can be achieved. Moreover, short postoperative morbidity and hospital stay as well as full weight bearing after 4 to 8 weeks resulted after adopting ORIF in pelvic fractures. External fixation is still used by us as a temporary bleeding control device before the final operative treatment when the bleeding is considered significant.

Operative approaches to the acetabulum are generally classified into anterior, posterior, extensile or combined approaches. The choice of approach depends upon the fracture pattern and the amount of relative displacement affecting the anterior and posterior bony structures. Occasionally, extensile or combined surgical approaches are indicated for the treatment of complex fracture patterns with extensive involvement of both the anterior and posterior acetabular anatomy. However, it is believed that these approaches may be associated with higher complication rates than more limited surgical approaches. The ilioinguinal approach described by Letournel is routinely employed in the treatment of anterior column, anterior wall, anterior column/posterior hemi- transverse and certain associated both-columns, transverse and T-type fractures. The utility of this approach is sometimes limited by difficulty in visualising, reducing and applying instrumentation to the quadrilateral plate and posterior column components of these fractures. A surgical approach described by Stoppa in 1989—and later extended to acetabular indications by Cole and Bolhofner—can be used, often in combination with the lateral window of the standard ilioinguinal approach, to effectively treat the same range of fractures as an ilioinguinal approach. Access to the quadrilateral plate and certain displaced posterior column fracture lines is enhanced by this approach—possibly eliminating the need for combined or extensile approaches in certain cases. A retrospective study undertaken at our institution demonstrated that anatomic articular reduction was achieved in 14 of 17 complex acetabular fractures treated via a Stoppa approach. All fractures in the study had at least 5 mm of posterior column displacement preoperatively

Isolated fractures of the anterior column and anterior wall are a relatively rare subgroup of acetabular fractures. We report our experience of 30 consecutive cases treated over ten years. Open reduction and internal fixation through an ilioinguinal approach was performed for most of these cases (76.7%) and percutaneous techniques were used for the remainder. At a mean follow-up of four years (2 to 6), 26 were available for review. The radiological and functional outcomes were good or excellent in 23 of 30 patients (76.7%) and 22 of 26 patients (84.6%) according to Matta’s radiological criteria and the modified Merlé d’Aubigné score, respectively. Complications of minor to moderate severity were seen in six of the 30 cases (20%) and none of the patients underwent secondary surgery or replacement of the hip

Purpose: Fractures of the anterior acetabular wall with preservation of the pelvic inlet are rare. These lesions were not noted or classiþed by Judet and Letournel in their classiþcation system Ð Ç In fractures of the anterior wall, the anterior part of the articular horseshoe breaks off with a major portion of the middle segment of the anterior column È. The ilio-inguinal approach was recommended for the surgical treatment of these fractures. Method : We have encountered two cases involving purely the anterior wall with preservation of the pelvic inlet, rather than the anterior wall fracture described by Judet and Letournel. We have identiþed only two other cases in the international literature. The recognition that these fractures were not as that described by Judet and Letournel was essential, as an alternative surgical approach was necessary for reconstruction. The ilioinguinal approach of Judet and Letournel is the technique of choice in anterior fractures, but provides only very limited potential for intra-articular manipulation through the line of fracture. The Smith-Petersen approach was practically the only feasible choice as intra-articular exposure is difþcult or impossible to obtain with the classical approaches used for anterior acetabular fractures. Results: Both of our cases were reconstructed via the Smith-Petersen approach. Postoperative review at 18 months demonstrated painless, fully mobile hips with evidence of radiological union of the fractures. Conclusion: We propose designating as ñfracture of the anterior columnñ all the anterior fractures described by Judet and Letournel (column and wall) that justify the ilioinguinal approach. We would reserve the term ñanterior wallñ for large anterior acetabular rim fractures, with the extension exclusively lateral to and below the pelvic brim. We recommend the use of a Smith-Petersen approach for reconstruction once this fracture pattern is identiþed

Background: Surgical approaches to the acetabular fracture present a challenge for most surgeons. The ilioinguinal approach by Letournel has fundamentally contributed to the successful treatment of such fractures. The current tendency is to minimize the approach while maintaining a proper visual field that allows inspection of fracture fragments and the ability to reduce them. The Stoppa approach is a less invasive anterior intra-pelvic approach that could be combined with a lateral window of the ilioinguinal approach, especially helpful in facilitating the approach to the quadrilateral plate. Aim: To evaluate the efficacy and safety of the Stoppa approach for the treatment of different types of acetabular fractures. Patients: Between the years 2004 and 2006, 45 patients with acetabular fractures were operated on using the Stoppa approach. Their age ranged between 14 and 73 (average 42). There were 33 males and 12 females whose fractures were classified as anterior column fractures (21) associated both columns (11) transverse (6), T-shape and other various types. They were consecutively operated in two level 1 trauma centers; in 13 the Stoppa approach was used solely and in the others a combination of the Stoppa approach with an iliac window was employed. Patients were followed for an average of 11.4 months (range 2–36 months), and assessed using the Merle d’Aubigné score. Results: Visualization of the fracture, reduction and fixation were all feasible through the Stoppa approach. The average length of surgery was 4 hours and an average of intraoperative bleeding was 1086 cc. We were able to achieve excellent reduction in 37 patients and a good reduction in the rest of the cases. Clinical outcomes were good to excellent in 90% of the patients. 3 patients underwent hip arthroplasty due to post traumatic osteoarthritis. Conclusions: The Stoppa approach in combination with an iliac window offers superb visualization of the quadrilateral plate, anterior column and in some cases the posterior column as well and enables excellent reduction. It is a reasonable replacement for the iliuinguinal approach offering less tissue dissection

Aim: We report results of surgical treatment of acetabular fractures and highlight the importance of single approach for complex fractures. Methods: 50 acetabular fractures referred to a specialist unit between 1994 and 1999 were treated surgically with anatomic reduction and internal þxation. Mean follow up was 32.3 months (14 to 67). Patients were regularly followed up in a special pelvic clinic for documentation of Harris hip score (pain, function, movement, activity), and radiological evidence of healing, avascular necrosis or other complications. Results: 18 patients were treated with the anterior ilioinguinal approach for 11 both- column, 3 anterior column, 3 transverse fractures and 1 central dislocation. 28 patients had posterior Kocher-Langenbeck approach for 17 posterior fracture dislocations, 2 both-column, 6 posterior wall and 1 each of transverse, posterior column and Tshaped fracture. 4 needed combined anterior- posterior or extensile triradiate approach due to comminution or delayed referral. Mean hospital stay was 24.7 days and mean injury surgery interval was 10.8 days. Of the 42 transfers from other units, 7 had surgery after 3 weeks from the injury. Outcome: 47 patients were followed up. Mean Harris hip score was 82.7 (31–100) for the posterior approach group, and 78.1 (27–99) for anterior approach group.3 patients needed total hip arthroplasty for secondary arthritis. Grade3–4 Brooker ectopic bone was noted in 3 posterior and 1 anterior approaches. There was no infection or avascular necrosis. Conclusion: Single approach surgery was possible in 46 patients and had a low rate of complications. Poor outcome occurred in highly comminuted fractures or with a delay in referral. Anterior ilioinguinal approach, although demanding, was the approach of choice for both column fractures. Early referral to a specialist unit is recommended

Introduction. The clinical and functional outcome following simultaneous fractures of the acetabulum and pelvic ring represent a challenge for the clinician and are currently obscure in the existing literature. Patients and Methods. Between January 2003 and July 2009 all patients with combined pelvic and acetabular fractures were included in this prospectively study. Demographic details, mechanism of injury, ISS, surgical procedures, post-operative course, length of hospital stay, and radiological (Matta's score) and functional outcomes (The Merle d' Aubigne hip score) were recorded. The minimum follow up was 12 months (12–72). Results. Out of 520 cases, 30 (21 males) patients met the inclusion criteria. The mean age was 32. 55% sustained lateral compression, 40% combined mechanism and 5% anterior/posterior type of injury pattern. 15 out of the 30 patients had sustained other injuries. The mean time from injury to definite reconstruction was 7 days (3–14). Ilioinguinal approach was used in 20 patients, Pfannestiel in 2 and Kocher-Langebeck in 4. A wheelchair was used initially in 22 cases. The median time to FWB was 3 months. One patient underwent a THR at 18 months. 19 patients scored the full 18 points of the Merle score. Matta radiological score was excellent in 16 patients, Good 11, Fair 2, Poor 1. Conclusion. The management of these injuries poses many difficulties to the trauma surgeon. With the available expertise in place good outcomes can be obtained as seen in this series of patients

Purpose: Open reduction and internal fixation (ORIF) is considered the treatment of choice in dislocated acetabular fractures. However ORIF has several drawbacks, such as intra operative blood loss, operative trauma and septic complications. To overcome these problems we applied percutaneous cannulated screw fixation in some cases. Methods: 198 acetabular fractures were treated between 1996 and 2003 in our department. According to the AO classification there were: 74 type A, 99 type B and 25 type C cases. The causes of the trauma were fall from high at 29, traffic accident at 112 and simple drop with osteoporosis in 57 patients. There were 29 polytraumatized and 46 multitraumatized patients. The 89 non-displaced fractures were treated conservatively: 8–12 weeks non-weight bearing were applied. The other cases were treated surgically: the simple wall fractures with screws, the column fractures with plates. The ilioinguinal approach was used in 11 and the Kocher-Langenbeck in 92 patients. Recently we have started to apply a percutaneous technique with cannulated screws. We used them at the fractures of the roof of the acetabulum and at elderly patients who had moderately dislocated anterior column fracture. We applied this technique in 6 cases. Results: In the cannulated screw group there was no intra- or postoperative complication, and the functional results have been excellent or good. In those cases, where the fracture involved the posterior wall or the posterior column, and percutaneous reduction could not have been achieved; we made open reduction, and ORIF. Conclusions: Percutaneous cannulated screw technique can be useful in the treatment of the fractures of the anterior column and the dome of the acetabulum

Introduction: The development of the ilioinguinal approach by the pioneering work of Letournel in 1965 has transformed the treatment of acetabular fractures. To date, this approach has been well established and few modifications have been described of the original approach. However, this approach is difficult, takes long time for exposure and closure of abdominal layers. The aim of this article is to report a modification of the approach which the authors have found particularly useful. Material and Method: Cadaveric dissection showed that it was easier to detach the inguinal ligament from the anterior superior iliac spine and reflect the anterior abdominal wall as one layer, than by the classical approach through layers of the anterior abdominal wall. Closure was also simpler, in the cadaver, with the entire anterior abdominal wall reattaching satisfactorily by a single transosseous suture. The rest of the approach, including division of iliopectineal fascia and developing the three windows remains the same as in the original approach. Results: To date, three acetabular fractures have been reduced and fixed using this modification. Besides substantially speeding up the exposure and closure, this approach allows superior distal visualization of the anterior column and wall, and the impression of rather less bleeding. No complications developed with the three cases treated through this modified approach, and specifically, there have been no hernias, nor has the lateral femoral cutaneous nerve of the thigh been damaged. Discussion: This small study demonstrates a modification to a classic approach that seems to be both safe and fast We hope that further experience will also show reduction in problems associated with wound healing. Further work in progress may also show that femoral venous flow is less impeded by this approach as retraction is not against the unyielding inguinal ligament

Purpose of the study: Misalignment after insufficient treatment of unstable fracture of the pelvis is often poorly tolerated, compromising quality-of-life due to limping, leg length discrepancy, posterior pain, uncomfortable sitting position and/or sexual disorders due to mechanical problems. Secondary surgical treatment can be proposed despite its invasive nature (generally three phase surgery). The purpose of this work was to present the technique and the results of standardized correction of pelvis misalignment using a two-phase procedure. Material and methods: Eight consecutive patients (May 2002–May 2004) with sequelae of Tile C fractures were treated on average eight years after the initial trauma. A double-approach was used. The series included four men and four women, aged 18–43 years. The first posterior approach in the ventral supine position was used for osteotomy of the sacroiliac callus and systematic debridement by section of the sacroiliac ligaments. The secondary ilioinguinal approach was performed in the dorsal supine position to achieve osteotomy of the symphyseal callus, reduction of the iliac wing, and symphyseal synthesis using a sacroiliac plate anteriorly and percutaneous screws. Results: The mean operative time was four hours 30 minutes. Blood loss required transfusion of 3.5 packed red cell units on average. Anatomic reduction was achieved in six cases, partial reduction in two. Despite one nosocomial infection and two partial popliteal external sciatic deficits, all patients wer satisfied with the operation at mean eight months follow-up. Bone healing was achieve din all cases. Discussion: Standard two-phase surgery is possible for a wide range of cases. The anatomic result is reliable with good clinical outcome. The duration of the operation and blood loss are reduced compared with classical techniques. Conclusion: Despite the advantage of this original operative strategy, surgery for correction of pelvis misalignment remains a difficult surgical procedure for selected and motivated patients informed of the operative risks

We present the early results of the Bernese osteotomy via an ilioinguinal approach performed at an U.K. district general hospital with no links to the Bernese group. Between 1997 and 2002, 19 such osteotomies have been performed at our institution. Average follow-up is 3.2 years (range: 1–5.5 years). The male: female ratio was 1: 8.5. Average age at time of operation was 32.3 years (range 18 – 48). 14 were classified as having Severin grade 2 dysplasia and 5 as having Severin grade 3 dysplasia. The average preoperative Merle d’Aubigne and Postel score was 12.7 points (range 6 – 15). 21.1% of patients were rated as good, as 57.8% fair and 21.1% poor. At most recent follow-up of preserved hip joints total mean score had increased to 16.3 (range 13 – 18). 26.3% of hips were graded as excellent, 58.9% as good and 14.8% as fair. The average postoperative values for the lateral centre-edge angle and acetabular index were 42.3° and 10.0° respectively which represents an average of 31.4° and 24.7° of improved lateral and loading zone coverage after osteotomy. The preoperative severity of osteoarthritis according to the criteria of Tonnis was grade 1 in 15.8% and grade 2 in 26.3%. Only one joint deteriorated sufficiently for it to be converted to a total hip arthroplasty. There was an overall complication rate of 59.9% comprised of 47.4% trivial, 10.4% moderate and 0.05% major complications. Our early results demonstrate the steep learning curve of this technically demanding operation and are encouraging

The Stoppa approach was originally conceived to deal with difficult abdominal hernia surgery. Its use has been modified to deal with Acetabular and Pelvic surgery. We report on our use of the Stoppa approach in 26 cases from 1998–2003 to fix Pelvic, Acetabular, and combined Pelvic/Acetabular fractures. The Stoppa approach was used in combination with other approaches to afford the best access for fixation. 11 of the cases were Acetabular fractures with no pelvic ring disruption (42.3%), 4 cases (15.3%) were pelvic ring disruptions without an Acetabular component. The other 11 cases (42.3%) were combined Pelvic and Ace-tabular fractures where this approach came into its own. In particular it is to be noted that the Corona Mortis was easily identifiable in 5 (19.2%) of the cases to allow its safe ligation. The anatomy of the approach and the access afforded are considered, along with the plating techniques that can be achieved because of its use. Patients were followed up for an average of 17.39 months with one lost to follow up. Clinical results were excellent in 20 cases, good in 2, fair in 2, and poor in 1. Complications were lateral femoral cutaneous nerve palsy in 11 patients, 1 bladder rupture, 2 superficial wound infections, one lateral incisional hernia related to an ilioinguinal approach, and 1 deep vein thrombosis. Heterotopic Ossification occurred in 3 patients in whom the Kocher-Langenbeck approach was used. One revision for screw proximity to the joint was undertaken. The Stoppa approach allows safe access and ease of reduction and fixation in these complex fractures, in combination with other approaches, particularly in combined pelvic and Acetabular fractures. We outline our recommendations for its use in this paper and outline a series of fracture patterns where it is most helpful

We assessed the long-term outcomes of a large cohort of patients who have undergone a periacetabular osteotomy (PAO), and sought to validate a patient satisfaction questionnaire for use in a PAO cohort.

Methods

All patients who had undergone a PAO from July 1998 to February 2013 were surveyed, with several patient-reported outcome measures (PROMs) and radiological measurements of preoperative acetabular dysplasia and postoperative correction also recorded. Patients were asked to rate their level of satisfaction with their operation in achieving pain relief, restoration of activities of daily living, ability to perform recreational activity, and their overall level of satisfaction with the procedure.

Improvement in coverage achieved by double or triple osteotomies is limited by the size of the acetabular fragment and the ligaments connected with the sacrum. Correction is achieved with the notable asymmetry of the pelvis. In periacetabular Ganz osteotomy (PAO) the acetabular fragment has no connection with the sacrum, which creates enormous possibilities for correction, leaving the pelvic ring untouched. The aim of the study is to present our experience and early results of using PAO in the treatment of hip dysplasia in adolescents and young adults who were previously treated operatively in childhood, and to find the technical and clinical impact of previous operations on our Results: In the years 1998–2005 262 periacetaubular osteotomies were performed in our hospital. All the patients were operated by one surgeon (JC). From this group 41 patients (43 hips) had previously been operated in childhood for the treatment of hip dysplasia. The previous treatment consisted of: open reduction in 10 hips, DVO in 14 hips, pelvic osteotomy (Salter, Dega, Chiari) in 8 hips, combined: open reduction+DVO+pelvic osteotomy in 10 hips, greater trochanter transfer in 3 hips, bone lengthening in 4 hips, acetabular cyst removal in 1 hip. The age at the primary operation ranged from 1–20.. The follow-up period ranged from 1–7,5 ys av. 2 ys. In 31 hips the Smith-Petersen, and in 12 hips ilioinguinal approach were performed. Methods. In clinical pre-op, and post-op examination the following factors were considered: pain, limping, Trendelenburg sign, range of motion, length discrepancy. Radiographic pre-op and post-op examination consisted of: AP view of the pelvis, false profile and AP view with the leg in abduction. Classic and anterior CE angle were measured. During the last examination Harris Hip Score was used. Results. Flexion slightly decreased from pre-op. 90–140° (av. 118°) to 80–130° (av. 104°) post-op. abduction remained unchanged 15–60° (av. 40°) and 15–60° (av. 40°) respectively, adduction slightly increased 15–40° (av. 31) and 20–50° (av. 33°). The range of rotation did not change after the operation. The sign of Trendelenburg was found in 27 hips before operation and in 8 hips post-op. Pain (acetabular rim syndrome) was found in 40 hips before operation and in 4 hips after the surgery. Either classic or anterior CE angle increased after the surgery in all cases from −14° to 34° and from −10° to 35° respectively. We had a rather low complication rate. But in cases previously operated and in males we strongly recommend Smith-Petersen approach extended into the frontal part of the hip for a better exposure in the scarred and hardened tissues. We find the technique of PAO as a safe, and effective tool for treating hip joint pathology increasing treatment possibilities for hip joint preservation

Aims

Posterior column plating through the single anterior approach reduces the morbidity in acetabular fractures that require stabilization of both the columns. The aim of this study is to assess the effectiveness of posterior column plating through the anterior intrapelvic approach (AIP) in the management of acetabular fractures.

Methods

We retrospectively reviewed the data from R G Kar Medical College, Kolkata, India, from June 2018 to April 2023. Overall, there were 34 acetabulum fractures involving both columns managed by medial buttress plating of posterior column. The posterior column of the acetabular fracture was fixed through the AIP approach with buttress plate on medial surface of posterior column. Mean follow-up was 25 months (13 to 58). Accuracy of reduction and effectiveness of this technique were measured by assessing the Merle d’Aubigné score and Matta’s radiological grading at one year and at latest follow-up.

Aims

Hyaline cartilage has a low capacity for regeneration. Untreated osteochondral lesions of the femoral head can lead to progressive and symptomatic osteoarthritis of the hip. The purpose of this study is to analyze the clinical and radiological long-term outcome of patients treated with osteochondral autograft transfer. To our knowledge, this study represents a series of osteochondral autograft transfer of the hip with the longest follow-up.

Methods

We retrospectively evaluated 11 hips in 11 patients who underwent osteochondral autograft transfer in our institution between 1996 and 2012. The mean age at the time of surgery was 28.6 years (8 to 45). Outcome measurement included standardized scores and conventional radiographs. Kaplan-Meier survival curve was used to determine the failure of the procedures, with conversion to total hip arthroplasty (THA) defined as the endpoint.

Aims

Psychological status may be an important predictor of outcome after periacetabular osteotomy (PAO). The aim of this study was to investigate the influence of psychological distress on postoperative health-related quality of life, joint function, self-assessed pain, and sports ability in patients undergoing PAO.

Methods

In all, 202 consecutive patients who underwent PAO for developmental dysplasia of the hip (DDH) at our institution from 2015 to 2017 were included and followed up at 63 months (SD 10) postoperatively. Of these, 101 with complete data sets entered final analysis. Patients were assessed by questionnaire. Psychological status was measured by Brief Symptom Inventory (BSI-18), health-related quality of life was raised with 36-Item Short Form Survey (SF-36), hip functionality was measured by the short version 0f the International Hip Outcome Tool (iHOT-12), Subjective Hip Value (SHV), and Hip Disability and Outcome Score (HOS). Surgery satisfaction and pain were assessed. Dependent variables (endpoints) were postoperative quality of life (SF-36, HOS quality of life (QoL)), joint function (iHOT-12, SHV, HOS), patient satisfaction, and pain. Psychological distress was assessed by the Global Severity Index (GSI), somatization (BSI Soma), depression (BSI Depr), and anxiety (BSI Anx). Influence of psychological status was assessed by means of univariate and multiple multivariate regression analysis.
