Avulsion of the proximal hamstring tendon from the ischial tuberosity is an uncommon but significant injury. Recent literature has highlighted that functional results are superior with surgical repair over non-surgical treatment. Limited data exists regarding the optimal rehabilitation regime in post-operative patients. The aim of this study was to investigate the early interim patient outcomes following repair of proximal hamstring tendon avulsions between a traditionally conservative versus an accelerated rehabilitation regimen. In this prospective randomised controlled trial (RCT) 50 patients underwent proximal hamstring tendon avulsion repair, and were randomised to either a braced, partial weight-bearing (PWB) rehabilitation regime (CR = 25) or an accelerated, unbraced, immediate full weight-bearing (FWB) regime (AR group; n = 25). Patients were evaluated preoperatively and at 3 months after surgery, using the Lower Extremity Functional Scale (LEFS), Perth Hamstring Assessment Tool (PHAT), visual analog pain scale (VASP), Tegner score, and 12-item Short Survey Form (SF-12). Patients also filled in a diary questioning postoperative pain at rest from Day 2, until week 6 after surgery. Primary analysis was by per protocol and based on linear mixed models. Both groups, with respect to patient and characteristics were matched at baseline. Over three months, five complications were reported (AR = 3, CR = 2). At 3 months post-surgery, significant improvements (p<0.001) were observed in both groups for all outcomes except the SF-12 MCS (P = 0.623) and the Tegner (P = 0.119). There were no significant between-group differences from baseline to 3 months for any outcomes, except for the SF-12 PCS, which showed significant effects favouring the AR regime (effect size [ES], 0.76; 95% CI, 1.2-13.2; P = .02). Early outcomes in an accelerated rehabilitation regimen following surgical repair of proximal hamstring tendon avulsions, was comparable to a traditionally conservative rehabilitation pathway, and resulted in better physical health-related quality of life scores at 3 months post-surgery. Further long term follow up and functional assessment planned as part of this study

Hamstring tendons are commonly used for Anterior Cruciate Ligament (ACL) reconstruction. In our series of 100 consecutive Hamstring ACL reconstructions, a four-strand graft was less than 7.0 mm in 5 patients. The aim of this study was to develop a screening test to assess the size of the hamstring tendon and so aid in the pre operative planning and patient counselling especially if hamstring size was deemed to be inadequate. A retrospective study of 100 consecutive hamstring ACL reconstructions analysed the correlation of the tendon size to height, weight and body mass index. A prospective double blind study was also set up involving independent observations by a musculoskeletal radiologist and the lead Orthopaedic surgeon. Result. There was no correlation between the anthropometric measures and hamstring size in the retrospective study. A total of 27 limbs were assessed sonographically, in 26 patients. Spearman's rank correlation coefficient was found to Pearson's r = 0.0786; p = 0.715. Conclusion. Pre-operative anthropometric measures and Ultrasound could not be used as a screening test to detect inadequate hamstrings in the clinical setting

This prospective longitudinal study compared the results of isolated endoscopic ACL reconstruction utilising 4-strand hamstring tendon (HT) or patellar tendon (PT) autograft over 15-years with respect to reinjury, clinical outcomes and osteoarthritis. Ninty consecutive patients with isolated ACL rupture were reconstructed with a PT autograft and 90 patients received HT autograft, with an identical surgical technique. Patients were assessed at 2, 5, 7, 10 and 15 years. Assessment included the IKDC Knee Ligament Evaluation, radiographic evaluation, KT1000, Lysholm Knee Score, kneeling pain, and clinical outcomes. Subjects who received the PT graft had significantly worse outcomes compared to those who received the HT graft at 15 years for the variables of radiologically detectable osteoarthritis (p=0.03), motion loss (p=0.01), single leg hop test (p=0.002), participation in strenuous activity (p=0.05), and kneeling pain (p=0.04). There was no significant difference between the HT and PT groups in overall IKDC grade (p=0.31). ACL graft rupture occurred in 16% of HT group and 8% of the PT group (p=0.07). ACL graft rupture was associated with non-ideal tunnel position (odds ratio 5.0) and males (odds ratio 3.2). Contralateral ACL rupture occurred in significantly more PT patients (24%) than HT patients (12%) (p=0.03), and was associated with age < 18 years (odds ratio 4.1) and the patellar tendon graft (odds ratio 2.6). Radiologically detectable osteoarthritis at 15 years was associated with the PT graft (odds ratio 2.3). Significant differences have developed in the groups at 15 years after surgery which were not seen at earlier reviews. Compared to the HT Group, the PT group had significantly worse outcomes with respect to radiological osteoarthritis, extension loss and functional tests but no significant difference in laxity was identified. There was a high incidence of ACL injury after reconstruction, to both the reconstructed and the contralateral knee

Hamstring grafts have been associated with reduced strength, donor site pain and muscle strains following Anterior Cruciate Ligament Reconstruction (ACLR). Traditional graft fixation methods required both semitendinosus and gracilis tendons to achieve a graft of sufficient length and diameter, but newer techniques allow for shorter, broad single tendon grafts.

This study seeks to compare the outcomes between Single Tendon (ST) and Dual Tendon (DT) ACLR, given there is no prospective randomised controlled trial (RCT) in the literature comparing outcomes between these options.

In this ongoing RCT: (ANZ Clinical Trials Registry ACTRN126200000927921) patients were recruited and randomised into either ST or DT groups. All anaesthetic and surgical techniques were uniform aside from graft technique and tibial fixation. 13 patients were excluded at surgery as their ST graft did not achieve a minimum 8mm diameter. 70 patients (34 ST, 36DT) have been assessed at 6 months, using PROMS including IKDC2000, Lysholm and Modified Cincinnati Knee, visual analog scale for pain frequency (VAS-F) and severity (VAS-S), dedicated donor site morbidity score, KT-1000 assessment, and isokinetic strength.

Graft diameters were significantly lesser in the ST group compared to the DT group (8.44mm/9.11mm mean difference [MD],-0.67mm; P<0.001). There was a significant and moderate effect in lower donor site morbidity in the ST group compared to the DT group (effect size [ES], 0.649; P = .01). No differences between groups were observed for knee laxity in the ACLR limb (P=0.362) or any of the patient-reported outcome measures (P>0.05). Between-group differences were observed for hamstrings strength LSI favouring the ST group, though these were small-to-moderate and non-significant (ES, 0.351; P = .147).

ST (versus DT) harvest results in significantly less donor site morbidity and this is the first prospective RCT to determine this. There were no differences between ST and DT hamstring ACLR were observed in PROMs, knee laxity and hamstring strength. Younger female patients tend to have inadequate single tendon size to produce a graft of sufficient diameter, and alternative techniques should be considered. Further endpoints include radiological analysis, longer term donor site morbidity, revision rates and return to sport and will continue to be presented in the future.

The optimal method of tibial fixation when using a hamstring tendon autograft in anterior cruciate ligament (ACL) reconstruction is unclear. This study aimed to compare the risk of revision ACL reconstruction between suspensory and interference devices on the tibial side.

Prospective data on primary ACL reconstructions recorded in the New Zealand ACL Registry between April 2014 and December 2019 were analyzed. Only patients with a hamstring tendon autograft fixed with a suspensory device on the femoral side were included. The rate of revision ACL reconstruction was compared between suspensory and interference devices on the tibial side. Univariate Chi-Square test and multivariate Cox regression was performed to compute hazard ratios (HR) and 95% confidence intervals (CI) with adjustment for age, gender, time-to-surgery, activity at the time of injury, number of graft strands and graft diameter.

6145 cases were analyzed, of which 59.6% were fixed with a suspensory device on the tibial side (n = 3662), 17.6% fixed with an interference screw with a sheath (n = 1079) and 22.8% fixed with an interference screw without a sheath (n = 1404). When compared to suspensory devices (revision rate = 3.4%), a higher risk of revision was observed when using an interference screw with a sheath (revision rate = 6.2%, adjusted HR = 2.05, 95% CI 1.20 – 3.52, p = 0.009) and without a sheath (revision rate = 4.6%, adjusted HR = 1.81, 95% CI 1.02 – 3.23, p = 0.044). The number of graft strands and a graft diameter of ≥8 mm did not influence the risk of revision.

When reconstructing the ACL with a hamstring tendon autograft, the use of an interference screw, with or without a sheath, on the tibial side has a higher risk of revision when compared to a suspensory device.


Purpose

The Purpose of this study was to evaluate hamstring strength after autogenous hamstring anterior cruciate ligament(ACL) reconstruction with emphasis on deeper knee flexion angles. A comparison of hamstring strength between patients undergoing ACL reconstruction using semitendinosus(ST), and those with semitendinosus and Gracilis(STG) tendons was conducted.

Method

Sixty patients were prospectively followed after undergoing ACL reconstruction surgery. Forty patients received a ST graft, and 20 patients had a STG graft. All patients had standard IKDC subjective knee evaluation completed, and had bilateral hamstring strengths tested using isokinetic testing with a Cybex Orthotron machine. In addition, a hand held Microfet dynamometer was used to measure hamstring strengths at deeper knee flexion angles at six, 12, and 24 months.


Most previous studies investigating autograft options (quadriceps, hamstring, bone-patella-tendon-bone) in primary anterior cruciate ligament (ACL) reconstruction are confounded by concomitant knee injuries. This study aims to investigate the differences in patient reported outcome measures and revision rates for quadriceps tendon in comparison with hamstring tendon and bone-patella-tendon-bone autografts. We use a cohort of patients who have had primary ACL reconstruction without concomitant knee injuries. All patients from the New Zealand ACL Registry who underwent a primary arthroscopic ACL reconstruction with minimum 2 year follow-up were considered for the study. Patients who had associated ipsilateral knee injuries, previous knee surgery, or open procedures were excluded. The primary outcome was Knee Injury and Osteoarthritis Outcome Score (KOOS) and MARX scores at 2 years post-surgery. Secondary outcomes were all-cause revision and time to revision with a total follow-up period of 8 years (time since inception of the registry). 2581 patients were included in the study; 1917 hamstring tendon, 557 bone-patella-tendon-bone, and 107 quadriceps tendon. At 2 years, no significant difference in MARX scores were found between the three groups (2y mean score; 7.36 hamstring, 7.85 bone-patella-tendon-bone, 8.05 quadriceps, P = 0.195). Further, no significant difference in KOOS scores were found between the three groups; with the exception of hamstring performing better than bone-patella-tendon-bone in the KOOS sports and recreation sub-score (2y mean score; 79.2 hamstring, 73.9 bone-patella-tendon-bone, P < 0.001). Similar revision rates were reported between all autograft groups (mean revision rate per 100 component years; 1.05 hamstring, 0.80 bone-patella-tendon-bone, 1.68 quadriceps, P = 0.083). Autograft revision rates were independent of age and gender variables. Quadriceps tendon is a comparable autograft choice to the status quo for primary ACL reconstruction without concomitant knee injury. Further research is required to quantify the long-term outcomes for quadriceps tendon use

Extensor mechanism complications after or during total knee arthroplasty are problematic. The prevalence ranges from 1–12% in TKR patients. Treatment results for these problems are inferior to the results of similar problems in non-TKR patients. Furthermore, the treatment algorithm is fundamentally different from that of non-TKR patients. The surgeon's first question does not focus on primary fixation; rather the surgeon must ask if the patient needs surgery and if so am I prepared to augment the repair? Quadriceps tendon rupture, periprosthetic patellar fracture, and patellar tendon rupture have similar treatment algorithms. Patients who are able to perform a straight leg raise and have less than a 20-degree extensor lag are generally treated non-operatively with extension bracing. The remaining patients will need surgical reconstruction of the extensor mechanism. Loose patellar components are removed. Primary repair alone is associated with poor results. Whole extensor mechanism allograft, Achilles tendon allograft, and synthetic mesh reconstruction are the current techniques for augmentation. In the acute setting if these are not available hamstring tendon harvest and augmentation is an option. Achilles tendons and synthetic mesh are easier to obtain than and entire extensor mechanism but are limited to patients that have an intact patella and the patella that can be mobilised to within 2–3 cm of the joint line. No matter which technique is used the principles are: rigid distal/tubercle fixation, coverage of allograft/mesh with host tissue to decrease infection, tensioning the augment material in extension, no flexion testing of reconstruction and post-operative extension bracing

Background. Surgical reconstruction of the anterior cruciate ligament is a common practice to treat the disability or chronic instability of the knee. Several factors associated with success or failure of the ACL reconstruction, including surgical technique and graft material and graft tension. We aimed to show how we can optimize the graft properties and achieve better post surgical outcomes during ACL reconstruction using 3-dimensional computational finite element simulation. Methods. In this paper, 3-dimensional model of the knee was constructed to investigate the effect of graft tensioning on the knee joint biomechanics. Four different grafts were compared: 1) bone-patellar tendon-bone graft (BPTB) 2) Hamstring tendon 3) BPTB and a band of gracilis 4) Hamstring and a band of gracilis. The initial graft tension was set as “0, 20, 40, or 60N”. The anterior loading was set to 134 N. Findings. Our study shows that the use of the discarded gracilis tendon, which usually excised after graft fixation, could be associated with a host of merits. Our results show that preserving this excess part of gracilis would decrease the required pretention load and, subsequently, could optimize biomechanical properties of the knee. Conclusion. Required pretension during surgery will have decreased significantly by adding a band of gracilis to the proper graft. Therefore, in addition to achieving normal stability of the knee, we can have lower risk of degradation

Complete or partial disruption of the collateral ligament during total knee arthroplasty in severe deformities is a devastating complication which sometimes remains unnoticed at the time of surgery. We analysed 280 knees in 150 patients with over 30° varus with flexion (196 knees), flexion deformity alone (60 knees) & valgus deformity (24 knees) treated with primary total knee arthroplasty from January 2002 to January 2008. The mean age of patient was 65 years (ranging from 54 years to 78 years). Problems encountered were avulsion of MCL from tibial attachment in 26 knees, femoral attachment avulsion with a bony fragment in 9 knees. Lateral collateral ligament was injured in 5 knees. In 12 knees with varus of >30° with element of tibial rotation medial release alone was not enough to balance the joint. Lateral collateral ligament tightening was needed in these cases using lateral epicondylar osteotomy. In 4 cases the medial collateral ligament was reconstructed using semi-tendinosus graft. Direct repair was not done in any case. Constrained knee prosthesis was used in only 22 knees to provide added stability. The mean duration of follow-up was 42 months. Stability of the knee was assessed clinically and on fluoro-radiography in full extension, 30°flexion and at 90° flexion of the knee. Seventy three percent (38/52) patients regained stability within 2 mm, 25% (13/52) had instability upto 4 mm and only one patient had gross instability needing hinge knee prosthesis. On the side of ligament reconstruction the ROM remained 10 to 15° less than the opposite side. Range of motion knee-brace was needed for 3 weeks in all cases. In cases with ligament reconstruction using Hamstring tendon, brace was used up to 12 weeks. In conclusion identification of ligamentous insufficiency and its management will not only prevent unstable joint but also minimize the risk of revision in future. Lateral collateral ligament tightening is needed in certain select extreme varus deformities in addition to medial release

Chronic patello-femoral joint instability leads to recurrent subluxation and dislocation affecting knee function and preventing participation at sport. Traumatic dislocation of the patella results in rupture of the medial patello-femoral ligament (MPFL) in the majority of cases with a high incidence of chronic instability after a second dislocation. MPFL reconstruction can prevent recurrent dislocation and improve knee function. We report on our experience in MPFL reconstruction using hamstring tendons and its effect on the knee function in patients with chronic patello-femoral joint instability. In this prospective study from 2005 to 2008, 68 patients (69 knees) with chronic patello-femoral instability were treated with MPFL reconstruction through a minimally invasive and arthroscopically assisted approach. In this procedure the semintendonosis with or without the gracilis tendon is routed from the pes anserinus to the most distal portion of the medial intramuscular septum before being secured to the superomedial border of the patella. All patients were evaluated pre-operatively and then post-operatively for a mean of 25 months (range 12-48 months). Knee function was assessed by the Tegner, Kujala and Lysholm scores. There were 44 (65%) women and 24 (35%) men. Average age was 27 years. There were 2.7 mean pre-operative patellar dislocations (range 2-7). Mean follow-up was 19.6 months (range 12 to 56 months). IKDC score improved from 48.2 to 74.2, Kujala scores improved from 55.4 to 85.3. Lysholm scores improved from 52.4 to 77.3. Tegner activity scores improved from 2.5 to 5.4. There were overall 87% good to excellent results. No recurrent dislocations or wound related complications. Medial patello-femoral reconstruction can provides excellent results in the treatment of patello-femoral joint instability with poor knee function

Aims

Despite the increase in the surgical repair of proximal hamstring tears, there exists a lack of consensus in the optimal timing for surgery. There is also disagreement on how partial tears managed surgically compare with complete tears repaired surgically. This study aims to compare the mid-term functional outcomes in, and operating time required for, complete and partial proximal hamstring avulsions, that are repaired both acutely and chronically.

Methods

This is a prospective series of 156 proximal hamstring surgical repairs, with a mean age of 48.9 years (21.5 to 78). Functional outcomes were assessed preinjury, preoperatively, and postoperatively (six months and minimum three years) using the Sydney Hamstring Origin Rupture Evaluation (SHORE) score. Operating time was recorded for every patient.

Objectives

Surgical marking during tendon surgery is often used for technical and teaching purposes. This study investigates the effect of a gentian violet ink marker pen, a common surgical marker, on the viability of the tissue and cells of tendon.

Methods

In vitro cell and tissue methods were used to test the viability of human hamstring explants and the migrating tenocytes in the presence of the gentian violet ink.
