Abstract. Introduction. A spotlight has been placed upon virtual assessment of patients during the coronavirus pandemic. This has been particularly prevalent in the assessment of acute knee injuries. In this study we aim to assess the accuracy of telephone triage, confirmed by Magnetic Resonance Imaging (MRI) in the diagnosis of acute knee injuries. Methods. Case records of patients triaged by telephone in the acute knee clinic at Leeds General Infirmary were analysed. Provisional diagnoses made following telephone triage were compared to radiological diagnoses made on subsequent MRI scans. Diagnostic accuracy was compared between those patients assessed virtually and those assessed in face-to-face clinics. Results. 1160 patients were referred to the acute knee injury clinic during the study period. 587 of these were triaged telephonically. MRI scans were requested for 107 (18%) virtually reviewed patients. Of these patients, 92 (79%) had an MRI scan requested after making a provisional diagnosis over the phone. Of the MRI requests made after virtual consultation, there was a 75% diagnostic accuracy of the pre-imaging diagnosis. Of the patients seen in face-to-face appointments, a diagnostic accuracy of 73% was observed. Conclusion. Virtual assessment can provide an efficient and cost-effective establishment of diagnosis of acute knee injuries whilst reducing hospital attendance. A combination of virtual and in-person clinics may allow quicker access to specialist opinion and therefore reduce patient waiting times

In current practice in the UK there are three main approaches to investigating suspected scaphoid fractures not seen on initial plain film x-rays. Early MRI of all cases. Review all cases in clinic at two weeks with repeat x-rays. Hybrid model. Virtual Fracture Clinic (VFC) triage to reduce those who are seen in clinic at two weeks by:. ∘. Organising early MRI for those with high-risk presentation. ∘. Discharging those with an alternative more likely diagnosis. Our unit uses the VFC model. We aimed to evaluate its efficiency, safety, clinical outcomes and economic viability. All patients attending the emergency department with either a confirmed or suspected scaphoid fracture between March and December 2020 were included (n=305). Of these 297 were referred to the VFC: 33 had a confirmed fracture on x-ray and 264 had a suspected fracture. Of the suspected fractures reviewed in VFC 14% had an MRI organised directly owing to a high-risk presentation, 79% were brought for fracture clinic review and 17% discharged with an alternative diagnosis such as osteoarthritis. Of those subsequently reviewed in fracture clinic at two weeks: 9% were treated as scaphoid fractures (based on clinical suspicion and repeat x-rays), 17% had MRI or CT imaging organised, 5% did not attend and 69% were discharged. Overall, 17% of cases initially triaged, had further imaging – 41 MRIs and 5 CTs. MRI detected: 5% scaphoid fracture, 17% other fracture, 24% bone contusion, complete ligament tear 10%, partial ligament tear 39% and normal study 10%. The results of MRI minimally affected management. 3 patients were taken out of plaster early, 1 patient was immobilized who was not previously and no patients underwent operative management. In the following 12-month period one patient re-presented with a hand or wrist issue. This approach avoided 218 MRIs, equating to £24000 and 109 hours of scanner time. VFC triage and selective use of MRI scanning is a safe, efficient and cost-effective method for the management suspected scaphoid fractures. This can be implemented in units without the resource to MRI all suspected scaphoid fractures from the emergency department

Three developments in the last 10 to 15 years have made it necessary to review how we ensure rapid access to treatment of patients with disabling low back pain. Firstly, there would appear to be an increase in the numbers of patients seeking medical help for low back pain, whether due to increased patient expectation, or better reporting, or a true increase associated with the increasing sedentary nature of life, is uncertain. Secondly, there is the realisation that amongst the factors that encourage acute back pain to become chronic is being off work, and the sooner a diagnosis and treatment is started the more likely that chronicity will be prevented, so a long waiting time to be seen in a clinic is productive of disability. Thirdly, reorganisation of consultant services has created the spinal surgeon, who in return for being allowed to practice spinal surgery almost exclusively undertakes the load of patients referred with back pain, amongst whom lurk those with a surgically remediable problem. The effect has been that although waiting times for general orthopaedic patients have dropped, as general orthopaedic, or other specialist orthopaedic surgeons no longer see spinal cases, the specialist spinal surgeon is overwhelmed by a large group of patients with back pain for whom there is not a surgical solution. Unfortunately, there is a shortage of spinal surgeons, which is likely in the UK at any rate to get worse. Being overwhelmed with non-surgical back pain interferes with their ability to deal with surgical problems. It also does create a recruitment problem, as back pain is not seen as a rewarding or satisfying problem to treat. Triage is a method of screening patients into groups at an early stage, identifying those who might benefit from surgery, and fast tracking them, identifying those who will benefit from other management and tracking them accordingly. Pioneered in general orthopaedics by Robin Ling in Exeter, it has been developed in the hospital setting somewhat randomly, by dedicated enthusiasts, many of whom will be speaking today. The purpose of this meeting today is to hear about the various systems, their funding and organisation and location, the triage staff used, the investigations used in primary triage and the effect on hospital specialist waiting times, the safety and patient satisfaction. Is a multiplicity of systems best, is one better than another, why have some units achieved no waiting times for MRI, and others six months etc?. In 1994, the Clinical Standards Advisory Group produced two books, An epidemiological Review, largely the work of Gordon Waddell, and a second book on Back Pain, chaired by Professor Michael Rosen with, I suspect, considerable input from Professor Waddell and others. Many of us met them when they toured the country collecting facts about the treatment and management of back pain. It discusses back pain triage, and suggests that it can be done within the average GP Consultation time of 9 minutes. It deals with simple back ache, “red flags”, (we now have “yellow flags” denoting the psycho-social factors) nerve root pain, cauda equina and inflammatory disorders. It is to be noted that chronic back pain, is not alluded to in the diagnostic triage, but it is stated that 90% of simple back ache recovers in six weeks. Their management guidelines emphasise the value of physical therapy (manipulation and active exercises), but it will be noted that they are addressed to a group of patients, 90% of whom will recover in six weeks. Sadly, therefore, the effect of this very sensible document, in ignoring in the triage system the chronic patient, has in many cases directed therapy in general practice towards open access for patients who in any event will improve spontaneously. We must therefore address where triage should be, hospital, or GP level. Certainly a GP gatekeeper will remove the acute back pains that are going to get better soon anyway from attending hospital and in certain units, a separate fast track is provided for acute radicular problems (Acute Sciatic Clinics). Any successful triage system involving chronic back pain must be associated with treatment possibilities, and I shall briefly discuss these, although the main thrust of the afternoon will be the triage organisation itself. The session is designed to allow considerable audience participation, as it is hoped that information, comments and criticisms from the audience will allow us to subsequently produce a booklet, hopefully with support from our Professional Societies, describing what we feel is Best Practice in Back Pain Triage, which we hope, after appropriate circulation, will encourage Trusts and Community Health Groups to develop such units and ensure that back pain patients get a better deal

Purpose of the study and background. This audit report outlines a physiotherapy lead spinal triage program for inpatients in a private hospital. The reason to trial this model was proposed as a way to expedite the assessment of inpatients presenting with back pain and implement an appropriate and prompt management plan. Summary of the methods used and the results. An audit tool was developed to record the patient details and outcomes of referrals to the spinal triage program. Length of time between referral and triage was recorded as was the number of patients that went on to require a spinal surgeon consult and spinal intervention. A total of 75 patients were referred into Spinal Triage over a 7 month period. The majority of in patients referred for spinal consult within this private hospital setting were triaged solely by physiotherapy (56%) and a management plan decided on in conjunction with the spinal surgeon on call who could access any imaging remotely. Six patients (8%) involved in the triage program went on to have spinal surgical intervention. Conclusion. Results indicate that the majority of spinal consults at inpatient level do not proceed to require surgical intervention. These results support the model of using Physiotherapists to triage this patient group. I can confirm that this abstract has not previously been published in whole or substantial part nor has it been presented previously at a national meeting. Referral patterns in spine clinic of young patients with suspected scoliosis is suboptimal with 19% of late referrals and 42% of inappropriate referrals. Patients' triage and prioritisation in spine clinic is a strategy to ensure that health care allocation is done according to the level of health needs, favoring effective management and efficient use of health care resources use. The objective of the study is to elaborate a model for triage and prioritisation of young patients in spine clinic based on expert consensus and literature on best practices. This projects was structured in three parts: 1)We documented best evidence. We conducted a review of empirical studies evaluating triage and prioritisation initiatives in order to identify key components for intervention success. 2)We elaborate a model of health care delivery with the professionals of a local paediatric spine clinic. In this model, the triage and prioritisation algorithm was developed from list of potential factors (demographics, signs and perceived symptoms, provisional diagnoses and known co-morbidities, results of preliminary physical examination and radiological findings) that was submitted to five paediatric orthopaedic surgeons for rating according to their potential relevance to orient prioritisation decisions. 3) We compared the professionals' model of health care delivery to the literature synthesis in order to propose the best model. Seven key components of triage and prioritisation systems were identified: centralised review of referral requests, list of consensual objectives criteria for triage, fast track evaluation of urgent cases, selection of cases for management at point of triage, cases prioritisation to main consultant, multidisciplinary evaluation and alternatives pathways. The consensual decision algorithm confirmed that cases who should be seen in priority are immature patients presenting with a significant trunk deformity. In addition, presence of persisting neurological symptoms, severe incapacitating pain or night pain, as well as abnormal scan or MRI findings were considered as urgent/PI priority. Cases characteristics for evaluation by nurse practitioners as well as alternative pathways of management were defined. Acceptability, compatibility, clinical relevance and discriminant capacity of the new model of health care delivery were satisfactorily demonstrated. Consensus was easily reached between the five respondents on factors supporting decisions to prioritise patients in spine clinic for suspected spinal deformity. Refinements to the initially proposed model according the identified key features from the literature, led to a final model of health care delivery that is evidence-base, feasible and coherent with the local context. Future implementation of this model should facilitate timely and appropriate health care delivery and best use of health care resources according to patients' needs

The Trauma Triage clinic (TTC) is a Virtual Fracture clinic which permits the direct discharge of simple, isolated fractures from the Emergency Department (ED), with consultant review of the clinical notes and radiographs. This study details the outcomes of patients with such injuries over a four-year period. All TTC records between January 2014 and December 2017 were collated from a prospective database. Fractures of the radial head, little finger metacarpal, fifth metatarsal, toe phalanges and soft tissue mallet finger injuries were included. Application of the direct discharge protocol, and any deviations were noted. All records were then re-assessed at a minimum of three years after TTC triage (mean 4.5 years) to ascertain which injuries re-attended the trauma clinic, reasons for re-attendance, source of referral and any subsequent surgical procedures. 6709 patients with fractures of the radial head (1882), little finger metacarpal (1621), fifth metatarsal (1916), toe phalanges (920) and soft tissue mallet finger injures (370) were identified. 963 (14%) patients were offered in-person review after TTC, of which 45 (0.6%) underwent a surgical intervention. 299 (4%) re-attended after TTC direct discharge at a mean time after injury of 11.9 weeks and 12 (0.2%) underwent surgical intervention. Serious interventions, defined as those in which a surgical procedure may have been avoided if the patient had not undergone direct discharge, occurred in 1 patient (0.01%). Re-intervention after direct discharge of simple injuries of the elbow, hand and foot is low. Unnecessary deviations from protocol offer avenues to optimise consumption of service resources

In Britain 8 million people consult their general practitioner annually with musculoskeletal conditions leading to referral of 1.5 million patients to Orthopaedics/ Rheumatology. Northern Ireland has the highest waiting lists for outpatients in Britain. The demand on orthopaedics continues to rise despite past attempts to reduce waiting lists. Trauma and orthopaedics accounted for 14% of the excess waiters for outpatients at June 2002. (DHSSPS Sept 2002) Roland et al 1991, etc. demonstrated that 43% of all orthopaedic referrals were inappropriate. In Belfast, G.P.s and Physiotherapists in partnership with the Regional Orthopaedic Service decided to pilot a Primary Care Orthopaedic Triage Service. The vast majority of orthopaedic referrals relate to three main body parts: lumbar spine 28%, knees 34% and hips 25% and these were chosen to be triaged for the pilot. Approval was sought and granted from Queen’s University Belfast Ethics Committee. Phase 1 involved the training of 2 GPs and 2 physiotherapists at the Musgrave Park and Royal Victoria Hospital with the full cooperation of the orthopaedic surgeons. Phase 2 tested independently the diagnostic capability of the trained professionals and assessed the appropriateness and management of orthopaedic referrals against the consultants decision as ‘gold standard’. 95 patients participated in the study. 55.8% of referrals were deemed appropriate by the consultants, compared to 44.6% by the GP/physio team. The Kappa statistical score was 0.79 reflecting a good level of agreement and is comparable to other clinical specialties (Sackett 1991). The sensitivity of the trained professionals on orthopaedic referrals was 83% and the specificity was 97%. Kappa value for management of inappropriate referrals was 0.83. Orthopaedic referral can be acceptably triaged by primary care professionals reducing the number of onward referrals to outpatients by 40% and increasing the appropriate referrals from 56% to 97%

Introduction: Mismatch between patient load and out-patient clinic resources in South Australian public sector has resulted in a long waiting list for initial appointment of new patients with shoulder complaints. There is no validated triage tool to help prioritize the patient referrals appropriately, to avoid inappropriate delays. Methods: We devised a self evaluation questionnaire to appropriately triage new referrals with shoulder complaints. Ethics committee approval was obtained. Forty eight new referrals to shoulder clinic filled in this questionnaire and the self evaluation part of American Shoulder and Elbow Society form. The patients were assessed in chronological order by two surgeons. Both were blinded to the GP referral and questionnaire findings at the time of patient assessment. GP referrals and the questionnaires were subsequently assessed and triaged. These findings were compared to assessment in the clinic. Results: Whilst the questionnaire provided significantly more information when compared to GP referrals, both were inadequate on their own as reliable triage tool. However in combination, majority of the patients could be triaged appropriately. Based on the presumptive diagnosis, basic investigations and therapy can be correctly instituted whilst awaiting formal orthopaedic review. None of the patients would have been allocated a lower priority than appropriate. The self evaluation part of ASES form on its own was not a satisfactory tool for triaging. Discussion: The proposed questionnaire and initial referral letter can be used for safe triaging and ensuring most basic relevant clinical workup is done prior to presentation. Further refinement of the questionnaire and additional studies are required to fully define its role in routine public hospital use

Background. A retrospective review of all patients transferred by helicopter ambulance to the Great Western Hospital over a 20-month period between January 2003 and September 2004 was undertaken to establish the case-mix of patients (trauma and non-trauma) transferred and the outcome of their admission and length of hospital stay. Methods. Details of all Helicopter Emergency Ambulance Service (HEAS) transfers to this unit in the study time period were obtained from the three HEAS providers in the area and case notes for all patients (where available) were reviewed. Results. 156 trauma patients were transferred (total patients 193). 111 cases were identified for analysis (45 transfers had inadequate information to identify them) with mean age of 33 years (range 1-92 years). Mean Injury Severity Score on admission was 12 (range 1-36). 45 (or 41%) patients were discharged home from the Accident and Emergency Department. 24 cases had operations, 9 patients required ICU care and 2 were pronounced dead on arrival in the A&E Department. Average hospital stay following HEAS transfer was 2.97 in-hospital days (range 0-18). Conclusions. Pre-hospital Helicopter Ambulance transfer in the acute setting is of debatable value. There is no doubt that it can be an extremely valuable service but triage criteria are at fault if as many as 41% of patients transferred are being discharged home from casualty having incurred the financial cost of helicopter transfer (estimated costs per transfer range from £4,500 to £6,000). We present the data collected in this study. We propose that, if 41 percent of all trauma admissions transferred by helicopter ambulance are discharged home directly from the emergency department, the triage criteria for helicopter emergency ambulance transfer are at fault. We suggest that the triage criteria should be reviewed locally and nationally and the appropriate changes should then be implemented

Integrated Regional Orthopaedic (MSK) Assessment clinics (ROAC) are now mandated in many provinces for the assessment and triage of patients referred for total joint arthroplasty (TJA). Their introduction underscores the lack of means for Primary Care Physicians (PCP) to appropriately refer patients for surgical consideration. Thus, problems arise when patients who are clear candidates for surgery are subject to a significant extra step in the care pathway by attending a ROAC while those who have insufficient problems are also seen, contributing to costs and crowding the access portal. We postulated that a patient reported outcome measure, decision aid combined with a validated grading of a weight bearing knee X-ray would provide an inexpensive yet effective tool to significantly improve the referral process for Knee OA (compared with the current mechanism). To date we have enrolled two hundred and forty-five consenting patients to the study, all referred by their PCP to the ROAC with a diagnosis of symptomatic Knee Osteoarthritis. All patients were evaluated as per the current ROAC protocol which included a medical history, physical examination and an X-ray (standing AP, lateral and patella-femoral skyline). Prior to the visit, subjects were sent a copy of a patient decision aid, Oxford Knee Score (OKS) and requested to answer whether their current clinical status described as Patient Acceptable Symptom State (PASS2) was acceptable. All radiographs were analyzed and scored for OA severity using the validated grading from 0 – 13. Of the 245 cases, 200 completed OKS and PASS2 uestionnaires and had standing X-rays for evaluation (only 120 completed the decision aid and these were left out of this report). Of the 200 included cases, 104 were referred from the ROAC to see a surgeon. In analysis, we found that a self-reported PASS 2 answer NO and an AP X-ray graded at 6 or above predicted over 75% of those patients that were referred. This represents a 3.4 greater likelihood of referral using this simple analysis. The OKS did not modify this prediction. Thus, use of a validated grading of a standing AP X-ray along with a response, ‘readiness for surgery’ indicated 75% of patients appropriate for surgical consideration. Patients with less severe gradings are likely being unnecessarily referred to ROAC leading to overuse of scarce resources, crowding the access and adding to costs, others, who score higher, are being needlessly delayed. The ability to discreetly screen for the best possible candidates should be a continued focus of ROAC and will lead to improved use of expensive resources, overall patient care and satisfaction and the provision of tools to the PCP for appropriate referral

Background Changes in professional boundaries have allowed many of the tasks traditionally undertaken by doctors to be delegated to Nurses and Allied Health professionals. The employment of an appropriately experienced specialist physiotherapist in orthopaedic clinics in the NHS to triage patients is well established. This paper examines the background, establishment and outcomes of the use of a Physiotherapist Extended Scope Practitioner (ESP) in the Orthopaedic Department at the Royal Hospital Haslar, Gosport. Method The post at RH Haslar has evolved in the five years since its implementation. All military patients referred to RH Haslar Orthopaedic Department with spinal, predominantly low back, pain are assessed in the Military Spinal Triage Clinic. The Physiotherapist ESP case-manages patients with access to radiological and haematological investigations and onward referral to other specialities as appropriate. Results In eighteen months 235 new patients have been assessed. Only 25 patients required review by the consultant spinal surgeon, while 5 were referred to non-spinal orthopaedic consultants with shoulder/hip pathology. A total of 18 patients were referred to Pain Clinic and 3 patients to rheumatology. The results indicate that nearly 90% (n=210) of patients who would previously been reviewed by a consultant spinal surgeon could be managed by a Physiotherapist ESP. The waiting time to spinal surgery has reduced from approximately 8 months to between 6 and 16 weeks. Conclusions It is concluded that an appropriately trained specialist physiotherapist is clinically and economically appropriate to manage patients in an Orthopaedic Department. This has important implications for optimising patient management and additionally supports the wider clinical employment of senior military physiotherapists

Purposes and Background. Musculoskeletal disorders including as back and neck pain are leading causes of work disability. Effective interventions exist (i.e. functional restoration, multidisciplinary biopsychosocial rehabilitation, workplace-based interventions, etc.), but it is difficult to select the optimal intervention for specific patients. The Work Assessment Triage Tool (WATT) is a clinical decision support tool developed using machine learning to help select interventions. The WATT algorithm categorizes patients based on individual, occupational, and clinical characteristics according to likelihood of successful return-to-work following rehabilitation. Internal validation showed acceptable classification accuracy, but WATT has not been tested beyond the original development sample. Our purpose was to externally validate the WATT. Methods and Results. A population-based cohort design was used, with administrative and clinical data extracted from a Canadian provincial compensation database. Data were available on workers being considered for rehabilitation between January 2013 and December 2016. Data was obtained on patient characteristics (ie. age, sex, education level), clinical factors (ie. diagnosis, part of body affected, pain and disability ratings), occupational factors (ie. occupation, employment status, modified work availability), type of rehabilitation program undertaken, and return-to-work outcomes (receipt of wage replacement benefits 30 days after assessment). Analysis included classification accuracy statistics of WATT recommendations for selecting interventions that lead to successful RTW outcomes. The sample included 5296 workers of which 33% had spinal conditions. Sensitivity of the WATT was 0.35 while specificity was 0.83. Overall accuracy was 73%. Conclusion. Accuracy of the WATT for selecting successful rehabilitation programs was modest. Algorithm revision and further validation is needed. No conflicts of interest. Purpose: Uncertainty around back pain management results in large volumes of patients with back related complaints being referred to orthopaedic surgeons for direction. The vast majority of these referrals are non surgical leading to unacceptable wait times (T1) across Canada. This reservoir delays not only those who are disabled with problems requiring a surgical remedy but also those who only require direction to appropriate conservative care. Physiotherapists with advanced training in orthopaedics possess skills in musculoskeletal interview, exam and Orthopaedic residents on the other hand must acquire spine specific skills in interview and exam, interpretation of radiographic exams, surgical decision making as well as surgical technique in a 2–3 month residency rotation. Our question was „Can an Experienced Physiotherapist Become Proficient in Triaging for Surgically Appropriate Patients After a 2–3 month „Residency „. Method: Following a 3 month clinical residency an experienced physiotherapist and a spine surgeon independently interviewed, physically examined and reviewed diagnostic imaging of 31 patients. It was then independently concluded whether the patients were candidates for surgical treatment, required conservative management or whether further investigations were necessary to make the final determination. The level of agreement was calculated using Chance Corrected Agreement or Kappa values. Operational definitions were reviewed and a second group of 29 patients were assessed. Results: The initial Kappa score was .68 (considered good clinical agreement) and the final Kappa score was 0.84 (considered virtually interchangeable). Conclusion: A 3 month period can prepare an experienced orthopaedic physiotherapist to triage a waiting list for surgical candidates. The therapist can add value through being better prepared to direct conservative options. Expediting triage will facilitate the right person getting to the right intervention within a reasonable time frame. Addressing the backlog of referrals will also help identify the magnitude of surgical need

Virtual encounters have experienced an exponential rise amid the current COVID-19 crisis. This abrupt change, seen in response to unprecedented medical and environmental challenges, has been forced upon the orthopaedic community. However, such changes to adopting virtual care and technology were already in the evolution forecast, albeit in an unpredictable timetable impeded by regulatory and financial barriers. This adoption is not meant to replace, but rather augment established, traditional models of care while ensuring patient/provider safety, especially during the pandemic. While our department, like those of other institutions, has performed virtual care for several years, it represented a small fraction of daily care. The pandemic required an accelerated and comprehensive approach to the new reality. Contemporary literature has already shown equivalent safety and patient satisfaction, as well as superior efficiency and reduced expenses with musculoskeletal virtual care (MSKVC) versus traditional models. Nevertheless, current literature detailing operational models of MSKVC is scarce. The current review describes our pre-pandemic MSKVC model and the shift to a MSKVC pandemic workflow that enumerates the conceptual workflow organization (patient triage, from timely care provision based on symptom acuity/severity to a continuum that includes future follow-up). Furthermore, specific setup requirements (both resource/personnel requirements such as hardware, software, and network connectivity requirements, and patient/provider characteristics respectively), and professional expectations are outlined. MSKVC has already become a pivotal element of musculoskeletal care, due to COVID-19, and these changes are confidently here to stay. Readiness to adapt and evolve will be required of individual musculoskeletal clinical teams as well as organizations, as established paradigms evolve. Study Design: Prospective cohort study. Objective: To assess the ‘red flag’ symptom of night pain as an indicator for serious disease in patients attending a back pain triage clinic (BPTC). Summary of Background Data: Although common in patients with known serious pathology, the prevalence of night pain in this population is not known. Methods: 482 consecutive patients attending BPTC were assessed (including history of frequency and duration of night pain). Clinical examination was performed and demographic data obtained. MRI was performed if indicated according to local guidelines. Outcome measures: Oswestry (ODI), Visual Analogue Scales (for pain, distress and coping) and Hospital Anxiety Depression (HAD) patient based outcome scores were obtained. Serious pathology was defined as infection or tumour as per AHCPR Guidelines (1994) which state that these symptoms are associated with severe night time pain. Results: 213 patients had night pain with 90 having pain every night. No serious pathology was identified. Patients with night pain had 4.95 hours continuous sleep (2–7) and were woken 2.5 times/night (0–6). Patients with pain every night exhibited higher ODI and HAD scores than those that did not. Conclusions: Although it is a significant and disruptive symptom for patients, these results challenge the sensitivity of the presence of night pain per se as a useful diagnostic indicator for serious spinal pathology in a back pain triage clinic

Background: Patients assessed at the clinic are classified and managed according to the guidelines from The Royal College of General Practitioners. The purpose of this study was to evaluate the differences in initial assessment findings between patients with ‘simple’ LBP and those with probable ‘Nerve Root Pain’ (NRP). Methods: All 1949 new patients attending over two years were assessed using a range of valid and reliable questionnaires to establish generic health status (Short-Form 36; SF36), self reported disability (Oswestry Disability Index; ODI) and psychological status (Hospital Anxiety and Depression Scale; HADS). The spinal examination was carried out by a Senior Physiotherapist and patients were triaged into the various categories of back pain. Differences between groups were assessed for the questionnaire scores and physical examination findings (SLR and lumbar flexion) using Chi-Square Analysis and unrelated T-Tests. Results: 908 patients were classified as having ‘Simple’ LBP and 302 were classified as having probable ‘Nerve Root Pain’. A significant difference was detected between the two groups for the mean ODI scores (mean difference: −8.73; 95% CI –11.3 to –6.2; P< 0.001). (mean ODI of 36.73 % (SD 18.88%) for ‘Simple LBP’ and 45.46% (SD 22%) for NRP group. Significant differences were also detected for the SF36-Physical Component scores, lumbar flexion and SLR. Conclusion: The ODI was found to be the strongest discriminator between the two groups. These findings support the inclusion of this condition specific outcome measure in the triage of back pain patients, as it appears to be sensitive to those patients with ‘NRP’

Background: This study tested the null hypothesis that computed tomography (CT) and magnetic resonance imaging (MRI) have the same diagnostic performance characteristics for triage of suspected scaphoid fractures. Methods: Thirty-four consecutive patients with a suspected scaphoid fracture (post-injury tenderness of the scaphoid and normal radiographs) underwent CT and MRI within ten days after trauma. CT-reconstructions were made in planes defined by the long axis of the scaphoid. The reference standard for a true fracture of the scaphoid was 6-week follow-up radiographs in four views, based on current literature. A panel including surgeons and radiologists came to a consensus diagnosis for each type of imaging considered in a randomized and blinded fashion, independent of the other types of imaging. We calculated sensitivity, specificity and accuracy as well as positive (PPV) and negative predictive values (NPV) for both imaging modalities. Results: According to the reference standard there were six true fractures of the scaphoid (prevalence 18%). CT diagnosed fracture of the scaphoid in five patients (15%), with one false positive, two false negative and four true positive results. MRI diagnosed a fracture in seven patients (21%), with three false positive, two false negative and four true positive results. Sensitivity, specificity and accuracy for CT were 67%, 96% and 91%; and for MRI 67%, 89% and 85% respectively. According to the McNemar test for paired binary data for each imaging modality these differences were not significant. The positive predictive values using Bayes’ formula were 76% for CT and 54% for MRI. Negative predictive values were 94% for CT and 93% for MRI. Conclusions: CT and MRI had comparable diagnostic characteristics. Both were subject to both false positive and false negative interpretations. They were better to rule out a fracture than to rule one in. The best reference standard for a true fracture is debatable

To evaluate the patient experience of patients referred to the ESP Orthopaedic Triage Service. To identify the demographic data of the patients. To evaluate patients' expectations and satisfaction of the service. A prospective audit of 50 new patients to the ESP service in Mountain Ash General Hospital and Prince Charles Hospital. The audit was carried out over a 3 month period between December 2008 and February 2009. Patients were asked to anonymously complete a survey following their appointment. Data was collated independently and analysed with descriptive statistics. Patients referred to ESP service. Self administered satisfaction survey. Mean age range 40-59 yrs (range 20 -70yrs). 50% >1 year duration of symptoms. 94% of patient's surveyed rated the service provided as good - excellent. 88% of patient's reported that they were happy to be assessed by the ESP. 96% of patients surveyed agreed they were able to discuss their treatment openly with only 10% preferring to see a Doctor. The ESP service in Cwm Taff Health board achieves a high level of patient satisfaction

Knee pain is common, representing a significant socioeconomic burden. Caused by a variety of pathologies, its evaluation in primary-care is challenging. Subsequently, an over-reliance on magnetic resonance imaging (MRI) exists. Prior to orthopaedic surgeon referral, many patients receive no, or incorrect, imaging. Electronic-triage (e-triage) tools represent an innovative solution to address this problem. The primary aim of this study was to ascertain whether an e-triage tool is capable of outperforming existing clinical pathways to determine the correct pre-hospital imaging based on knee pain diagnosis.

Patients ≥18 years with a new presentation of knee pain were retrospectively identified. The timing and appropriateness of imaging was assessed. A symptom-based e-triage tool was developed, using the Amazon LEXbotplatform, and piloted to predict five common knee pathologies and suggest appropriate imaging.

1462 patients were identified. 17% of arthroplasty patients received an ‘unnecessary MRI’, whilst 28% of arthroscopy patients did not have a ‘necessary MRI’, thus requiring a follow-up appointment, with a mean delay of three months (SD 2.6, range 0.2-20.2). Using NHS tariffs, a wasted cost through unnecessary/necessary MRIs and subsequent follow-up appointments was estimated at £45,816. The e-triage pilot was trialled with 41 patients (mean age:58.4 years, 58.5% female). Preliminary diagnoses were available for 34 patients. Using the highest proportion of reported symptoms in the corresponding group, the e-triage tool correctly identified three of the four knee pathologies. The e-triage tool did not correctly identify anterior cruciate ligament injuries (n=3). 79.2% of participants would use the tool again.

A significant number of knee pathology patients received incorrect imaging prior to their initial hospital appointment, incurring delays and unnecessary costs. A symptom-based e-triage tool was developed, with promising pilot data and user feedback. With refinement, this tool has the potential to improve wait-times and referral quality, whilst reducing costs.
