Aim. One of the most common patient-reported complaints following intramedullary nailing (IMN) of tibial shaft fractures is anterior knee pain reported by 10% to 80% of patients. The present study aimed to compare the 12-month Knee injury and Osteoarthritis Outcome Score (KOOS) sport and recreation activities subscale (sport/rec) scores after IMN with external ring fixation (RF) to adult patients with tibial shaft fractures. Methods. This study was a pragmatic multicentre randomized, non-blinded trial, with two-group parallel design. Included were adult patients (aged ≥ 18 years) presenting with an acute tibial shaft fracture deemed operable with an intramedullary nail. The primary outcome was the KOOS sport/rec, ranging from 0 (worst score) to 100 (best score) at 12-month follow-up. Secondary outcomes included the Foot and Ankle Outcome Score (FAOS), health-related quality of life assessed by EuroQol five-dimension five-level health questionnaire, and pain scores. Results. A total of 67 patients were included in the study. In all, 33 patients were randomized to standard IMN and 34 patients to RF. The mean age of the patients was 47.7 years (SD 19.2; 18 to 84) and 34% were female (n = 23). The primary analysis revealed no statistically significant difference in KOOS sport/rec between the IMN and RF groups at the 12-month follow-up (adjusted mean difference -18.1 (95 % CI -43.4 to 7.2); favouring RF). Conclusion. No statistically significant differences in the KOOS sport/rec were observed between RF and IMN at 12-month follow-up. The rate of arterial injury in trauma patients with pelvic ring fractures has been cited as high as 15%. Addressing this source of hemorrhage is essential in the management of these patients as mortality rates are reported as 50%. Percutaneous techniques to control arterial bleeding, such as embolization and REBOA, are being employed with increasing frequency due to their assumed lower morbidity and invasiveness than open exploration or cross clamping of the aorta. There are promising results with regards to the mortality benefits of angioembolization. However, there are concerns with regards to morbidity associated with embolization of the internal iliac vessels and its branches including surgical wound infection, gluteal muscle necrosis, nerve injury, bowel infarction, and thigh / buttock claudication. The primary aim of this study is to determine whether pelvic arterial embolization is associated with surgical site infection (SSI) in trauma patients undergoing pelvic ring fixation. This observational cohort study was conducted using US trauma registry data from the American College of Surgeons (ACS) National Trauma Database for the year of 2018. Patients over the age of 18 who were transported through emergency health services to an ACS Level 1 or 2 trauma hospital and sustained a pelvic ring fracture treated with surgical fixation were included. Patients who were transferred between facilities, presented to the emergency department with no signs of life, presented with isolated penetrating trauma, and pregnant patients were excluded from the study. The primary study outcome was surgical site infection. Multivariable logistic regression was performed to estimate treatment effects of angioembolization of pelvic vessels on surgical site infection, adjusting for known risk factors for infection. Study analysis included 6562 trauma patients, of which 508 (7.7%) of patients underwent pelvic angioembolization. Overall, 148 (2.2%) of patients had a surgical site infection, with a higher risk (7.1%) in patients undergoing angioembolization (unadjusted odds ratio (OR) 4.0; 95% CI 2.7, 6.0; p < 0 .0001). Controlling for potential confounding, including patient demographics, vitals on hospital arrival, open fracture, ISS, and select patient comorbidities, pelvic angioembolization was still significantly associated with increased odds for surgical site infection (adjusted OR 2.0; 95% CI 1.3, 3.2; p=0.003). This study demonstrates that trauma patients who undergo pelvic angioembolization and operative fixation of pelvic ring injuries have a higher surgical site infection risk. As the use of percutaneous hemorrhage control techniques increase, it is important to remain judicious in patient selection

Objectives. Osteosynthesis of anterior pubic ramus fractures using one large-diameter screw can be challenging in terms of both surgical procedure and fixation stability. Small-fragment screws have the advantage of following the pelvic cortex and being more flexible. The aim of the present study was to biomechanically compare retrograde intramedullary fixation of the superior pubic ramus using either one large- or two small-diameter screws. Materials and Methods. A total of 12 human cadaveric hemipelvises were analysed in a matched pair study design. Bone mineral density of the specimens was 68 mgHA/cm. 3. (standard deviation (. sd). 52). The anterior pelvic ring fracture was fixed with either one 7.3 mm cannulated screw (Group 1) or two 3.5 mm pelvic cortex screws (Group 2). Progressively increasing cyclic axial loading was applied through the acetabulum. Relative movements in terms of interfragmentary displacement and gap angle at the fracture site were evaluated by means of optical movement tracking. The Wilcoxon signed-rank test was applied to identify significant differences between the groups. Results. Initial axial construct stiffness was not significantly different between the groups (p = 0.463). Interfragmentary displacement and gap angle at the fracture site were also not statistically significantly different between the groups throughout the evaluated cycles (p ⩾ 0.249). Similarly, cycles to failure were not statistically different between Group 1 (8438, . sd. 6968) and Group 2 (10 213, . sd. 10 334), p = 0.379. Failure mode in both groups was characterised by screw cutting through the cancellous bone. Conclusion. From a biomechanical point of view, pubic ramus stabilisation with either one large or two small fragment screw osteosynthesis is comparable in osteoporotic bone. Aims

Infected and deformed neuropathic feet and ankles are serious challenges for surgical management. In this study we present our experience in performing ankle arthrodesis in a closed manner, without surgical preparation of the joint surfaces by cartilaginous debridement, but instead using an Ilizarov ring fixator (IRF) for deformity correction and facilitating fusion, in arthritic neuropathic ankles with associated osteomyelitis.

Methods

We retrospectively reviewed all the patients who underwent closed ankle arthrodesis (CAA) in Ilizarov Scientific Centre from 2013 to 2018 (Group A) and compared them with a similar group of patients (Group B) who underwent open ankle arthrodesis (OAA). We then divided the neuropathic patients into three arthritic subgroups: Charcot joint, Charcot-Maire-Tooth disease, and post-traumatic arthritis. All arthrodeses were performed by using an Ilizarov ring fixator. All patients were followed up clinically and radiologically for a minimum of 12 months to assess union and function.

Fine-wire accepted as a minimally external fixation is invasive technique, which can provide better outcomes than traditional open methods in the management of complex fractures of the tibial plateau. Available fixators vary in their biomechanical stability, and we believe that a stable beam-loading system is essential for consistently good outcomes. We assessed, prospectively, the clinical, radiological and general health status of 20 of 21 consecutive patients with complex fractures of the tibial plateau who had been treated using a standard protocol, with percutaneous screw fixation and a neutralisation concept with a fine wire beam-loading fixator allowing early weight-bearing.

Bony union was achieved in all patients, with 85% having good or excellent results. Full weight-bearing started during the first six weeks in 60% of patients. The general health status assessment correlated well with the knee scores and reflected a satisfactory outcome.

Introduction. Temporary spanning fixation aims to provide bony stability whilst allowing access and resuscitation of traumatised soft-tissues. Conventional monolateral fixators are prone to half-pin morbidity in feet, variation in construct stability and limited weight-bearing potential. This study compares traditional delta-frame fixators to a circular trauma frame; a virtual tibial ring block spanned onto a fine-wire foot ring fixation. Materials and Methods. The two cohorts were compared for demographics and fracture patterns. The quality of initial reduction and the maintenance of reduction until definitive surgery was assessed by two authors and categorised into four domains. Secondary measures included fixator costs, time to definitive surgery and complications. Results. Fifty-six delta-frames and 48 circular fixators were statistically matched for demographics and fracture pattern. Good or excellent initial reduction was achieved in 51 (91%) delta-frames and 48 (100%) circular fixators (p=0.022). Loss of reduction was observed in 15 (27%) delta-frames and 3 (6%) circular fixators (p<0.001). Post-fixator dislocation occurred in five (9%) delta-frames and one (2%) circular fixator (p=0.147). Duration in spanned fixation was equivalent (11.5 and 11.6 days respectively, p=0.211). Three (5%) delta-frames and 12 (25%) circular fixators were used as definitive fixation. The mean hardware cost was £3,116 for delta-frames and £2,712 for circular fixators. Conclusions. Temporary circular fixation offers statistically superior intra-operative reduction and maintenance of reduction, facilitates weight-bearing and provides more opportunity as the definitive fixation. Circular fixation hardware proved to be less expensive and protected against further scheduled and unscheduled operations

Background. Charcot neuroarthropathy is a destructive disease characterized by progressive bony fragmentation as a result of the isolated or accumulative trauma in patients with decreased sensation that manifests as dislocation, periarticular fractures and instability. Although amputation can be a reasonable cost economic solution, many patients are willing to avoid that if possible. We explored here one of the salvage procedures. Methods. 23 patients with infected ulcerated unstable Charcot neuroarthropathy of the ankle were treated between 2012 and 2017. The mean age was 63.5 ±7.9 years; 16 males and 7 females. Aggressive open debridement of ulcers and joint surfaces, with talectomy in some cases, were performed followed by external fixation with an Ilizarov frame. The primary outcome was a stable plantigrade infection free foot and ankle that allows weight bearing in accommodative foot wear. Results. Limb salvage was achieved in 91.3% of cases at the end of a mean follow up time of 25 months (range: 19–32). Fifteen (71.4%) solid bony unions were evident clinically and radiographically, while 6 (28.5%) patients developed stable painless pseudoarthrosis. Two patients had below knee amputations due to uncontrolled infection. Conclusion. Aggressive debridement and arthrodesis with ring external fixation can be used successfully to salvage severely infected Charcot arthropathy of the ankle. Pin tract infection, delayed wound healing and stress fracture may complicate the procedure but can be easily managed. Amputation may be the last resort in uncontrolled infection

Standard fixation for intra-articular distal humerus fracture is open reduction and internal fixation (ORIF). However, high energy fractures of the distal humerus are often accompanied with soft tissue injuries and or vascular injuries which limits the use of internal fixation. In our report, we describe a highly complex distal humerus fracture that showed promising healing via a ring external fixator. A 26-year-old man sustained a Gustillo Anderson Grade IIIB intra-articular distal humerus fracture of the non-dominant limb with bone loss at the lateral column. The injury was managed with aggressive wound debridement and cross elbow stabilization via a hinged ring external fixator. Post operative wound managed with foam dressing. Post-operatively, early controlled mobilization of elbow commenced. Fracture union achieved by 9 weeks and frame removed once fracture united. No surgical site infection or non-union observed throughout follow up. At 2 years follow up, flexion - extension of elbow is 20°- 100°, forearm supination 65°, forearm pronation 60° with no significant valgus or varus deformity. The extent of normal anatomic restoration in elbow fracture fixation determines the quality of elbow function with most common complication being elbow stiffness. Ring fixator is a non-invasive external device which provides firm stabilization of fracture while allowing for adequate soft tissue management. It provides continuous axial micro-movements in the frame which promotes callus formation while avoiding translation or angulation between the fragments. In appropriate frame design, they allow for early rehabilitation of joint where normal range of motion can be allowed in controlled manner immediately post-fixation. Functional outcome of elbow fracture from ring external fixation is comparable to ORIF due to better rehabilitation and lower complications. Ring external fixator in our patient achieved acceptable functional outcome and fracture alignment meanwhile the fracture was not complicated with common complications seen in ORIF. In conclusion, ring external fixator is as effective as ORIF in treating complex distal humeral fractures and should be considered for definitive fixation in such fractures

Background:. Lower limb reconstruction is performed in trauma centres where uplifted tariffs support the treatment of severely injured patients. Calculation of Healthcare Resource Groups (HRG4) codes is affected by the accuracy of clinical coding, determining the financial viability of this service in a district general hospital (DGH). Methods:. A prospective review of coding was performed for 17 sequential patients treated using ring fixation. Relevant clinical codes and HRG4 tariffs were obtained, allowing comparison with operation notes (including pertinent diagnostic information) and implant costs. Hexapod and paediatric cases were excluded. Results:. Mean implant costs were £3,300 and mean tariffs were £9,300. However, the tariffs for the care episodes which did not attract a code for ‘reconstruction’ were lower, averaging £4,300. Clinical outcomes were comparable to published literature. No uplift was received. Discussion:. Despite attempts to avoid mis-coding for the fledgling service and factoring ancillary costs of patient care, the ring fixation procedures which resulted in HRG4 codes other than those for limb reconstruction cost the trust more than it received. Conclusion:. Lower limb reconstruction is barely financially viable even when coding is carefully performed. It is financially easier for this Trust to transfer patients to a trauma centre

Introduction. Fractures of the distal tibia can be challenging to manage. Numerous surgical techniques have been utilised in managing these however there remains debate as to the optimum method of fixation. This study aims to assess the surgical outcomes and PROMs of patients with distal tibial fractures managed with intramedullary-nails or ring fixation. Methods. This is a retrospective study of patients with closed distal tibial fractures managed between 01/01/2013–31/12/2016. Adult patients admitted with closed fracture of the distal tibia fixed with an intramedullary-nail or circular-frame were included in the study. Primary outcomes were time of union, alignment of tibia post-operatively and the results of two validated PROMs (Kujala knee score and Olerud and Molander Ankle Score). Results. 12 patients had circular-frame and 14 patients underwent intramedullary-nailing. PROMS were completed in 9 (75%) of the frame group and 7 (50%) of the nail group. There was no statistically significant difference in age (p=0.095); no statistically significant difference in time to union (medians = frame 29.7 weeks, IM nail 24 weeks, p=0.212); no statistically significant difference in the coronal angulation difference from neutral (medians = frame 1.9 degrees, IM nail 2.0 degrees, p=0.940). There was statistically significant difference in sagittal angulation difference from neutral (Medians = frame 3 degrees, IM nail 0.6 degrees, p=0.041); the proportion of males in the frames groups was statistically significantly higher (p=0.033). There was no statistically significant difference in outcome of ankle scores (medians = frame 92.5, IM nail 75, p=0.132); there was a statistically significant difference in the knee score favouring the frame group (medians = frame 99, IM nail 74.5, p=0.041). Discussion. Our results show distal tibia fractures can be treated with circular-frames or IM-nails. Patients at high-risk of soft tissue complication or to minimise the risk of knee symptoms should be considered for a circular-frame

Introduction. Distal tibial fractures are notoriously difficult to treat and a lack of consensus remains on the best approach. This study examined clinical and functional outcomes in such patients treated definitively by circular external fixation (Ilizarov). Patients and Methods: Between July 2011 and May 2016, patients with fractures extending to within 1 muller square of the ankle were identified from our prospective Ilizarov database. Existing data was supplemented by review of clinical records. Fractures were classified according to the AO/OTA classification. Functional outcome data, including general measures of health related quality of life (SF-12 and Euroqol) and limb specific scores (Olerud and Molander Score and Lysholm scores) had been routinely collected for part of the study period. Patients in whom this had not been collected were asked to complete these by post. Adverse events were documented according to Paley's classification of: problems, obstacles and complications. Results. 142 patients with 143 fractures were identified, 40 (28%) were open, 94 (66%) were intra-articular, 85 (59%) were tertiary referrals. 32% were type 1, 28%, type 2 and 40% type 3 AO/OTA severity. 139 (97%) of the fractures united (2 non-unions, 1 amputation and 2 delayed unions who remain in frames), at a median of 165 days (range 104 to 429, IQR 136 to 201). 62% united by 6 months, 87% by 9 months and 94% by 1 year. Both non-unions have united with further treatment. Closed fractures united more rapidly than open (median 157 vs 185 days; p=0.003) and true Pilon (43C3) fractures took longer to unite other fractures (median 156 vs 190 days; p<0.001). 34% of patients encountered a problem, 12% an obstacle and 10% a complication. Of the complications, 6 (4%) were minor, 5 (3.5%) major not interfering with the goals of treatment and 4 (3%) major interfering with treatment goals (including the 2 patients with non-union and 1 who underwent amputation as well as 1 significant mal-union). This will increase to 4% if the 2 delayed unions fail to unite. Overall 56% reported good or excellent ankle scores at last report, 28% fair and 16% poor. Closed, extra-articular and non-43C3 fractures had better functional outcome scores than open, intra-articular and 43C3 fractures respectively. Conclusions. This study demonstrates a high union and low serious complication rate, suggesting that external ring fixation is a safe and effective treatment for these injuries. *Judged best paper*

Aims

This study aimed to evaluate the effectiveness of the induced membrane technique for treating infected bone defects, and to explore the factors that might affect patient outcomes.

Methods

A comprehensive search was performed in PubMed, Embase, and the Cochrane Central Register of Controlled Trials databases between 1 January 2000 and 31 October 2021. Studies with a minimum sample size of five patients with infected bone defects treated with the induced membrane technique were included. Factors associated with nonunion, infection recurrence, and additional procedures were identified using logistic regression analysis on individual patient data.

Introduction. The Taylor Spatial Frame (TSF) is an hexapod external ring fixation system that can move with six degrees of freedom to correct complex limb deformities. The lengths of the struts between the rings are independently adjusted to correct the deformity. The struts form an acute and obtuse “ring-strut” angle with the ring with the sum of these angles totalling 180°. In the course of a correction schedule a strut may need to be exchanged for one of longer or shorter length. The manufacturer's instructions direct that a temporary seventh strut can be placed in any orientation to ensure stability during the exchange. We have noted several episodes of temporary frame instability during this procedure resulting in discomfort. The aim of this study was to investigate which temporary strut positions gave maximal stability. Methods. A TSF frame was constructed in a neutral alignment with a neutral strut height of 130mm. Strut 1 (red) was identified for exchange. There were 169 theoretical placement options for the temporary strut that were sequentially tested. Fast-FX™ struts were used. Strut 1 was released and the shortening that occurred was recorded using the strut length gauge. Shortening of over 10mm was considered grossly unstable. Results. There were 21 stable configurations and 11 unstable configurations. There were 68 mechanically impossible positions. The most important factors for frame stability were the avoidance of temporary strut crossing the strut being exchanged, and matching acute/obtuse “ring-strut” angle orientation of the temporary strut. Conclusions. This study identifies a positioning rule that ensures maximum stability of a neutral frame during strut changes. It further identifies grossly unstable positions that may result in pain during a change

To review the results of the treatment of pilon fracture with percutaneous internal fixation and extrarticular ring fixation in neutralization, twenty-two fractures in twenty-one patients were included in the study. The mean follow-up time was 5.3 years. Five fractures were classified Ruedi-Algower type I, six were Ruedi-Algower type II and eleven Ruedi-Algower type III. Six were open fractures (3 Gustilo type III) and there were 19 associated fibular fractures (five were internally fIxed). Thirteen fractures (60%) were associated with metaphysealdiaphyseal dissociation (MDD). The majority of fractures were high energy (18 out of 22). General health outcome was assessed with the use of the SF-36 and functional outcome was evaluated with AOFAS score and Bone’s criteria. The average AOFAS score for the study population was 79.4. The AOFAS scores decreased as the severity of the fracture increases and these differences were statistically significant between the Ruedi-Algower types I and III. The pilon fractures population scored lower in all SF-36 categories but mental health and energy and vitality when compared to an age matched population but statistically significant differences were only found in the categories of physical function and limitation due to health problems. 65% achieved excellent or good results according to Bone’s criteria. No significant differences were found in the union times in the MDD group (253 days) when compared to the fractures with no MDD (224 days), but this can be due to the high incidence of autograft in the MDD group (7 out of 13). All patients achieved full weight bearing at 6 weeks. Fourteen patients had superficial pin site infections (one needed screw removal) that settled with oral antibiotics. There was one case of non-union and two varus heels. Conclusion: Good results are achieved treating pilon fractures with minimal internal fixation and the Sheffield ring fixator in neutralization but quality of life and functional scores significantly worsen in the most severe Ruedi-Algower type III fractures

Introduction: Rabbits are a well-established animal model for orthopaedic research. and the tibia is commonly used for investigations of fracture repair with different implant materials. Occurring forces in the animal model are of fundamental interest for the development of degradable bone implants to prevent implant failure. Therefore, a new method for the direct measurement of forces in the rabbit tibia was developed. The aim of this study was to determine maximal forces during weight bearing in the rabbit for future implementation into FEM-simulation. Animals and Methods: An external ring fixation was attached to the left tibiae of 5 rabbits and an ostectomy followed. Force sensors were included into the collateral rods to incur the emerging forces completely. On each side, a measurement amplifier was applied to transfer the collected data telemetrically. During the study, the animals were weighted and x-rays were taken regularly. Measurements started 8 days postoperatively and were repeated 8 times until day 50 post-op. The rabbits were placed in a run and animated to move while the forces were registered. Force peaks were filtered from the collected data of each measurement as absolute values and relative to the animals’ weight (force-weight ratio/FWR). Results: All included animals tolerated the external fixa-tion well and no clinical intolerances occurred. Beginning of callus formation was detected radiographically about 3 weeks post-op and all fixations could be removed 12–14 weeks after application without any permanent detriments. The maximal force amounted to 6950 g and 172 % FWR in animal 4 during the first recording. Means of the 5 maximal values for each measurement were located between 55 % FWR and 152 % FWR for the first measurement, converged to approx. 80 % FWR during the second recording 3 days later and descended to 20–40 % FWR until the end of the experiment. Discussion: Aim of this study was to determine maximal forces during weight bearing in a rabbit model. Our model for in-vivo monitoring of these forces was practicable and provided profound data. The highest values occurred during the first or second recording. That coincides with the radiographic detection of callus after 3 weeks. Therefore, reliable measurements have to be carried out during the first 2 weeks postoperatively. Detected values show that the rabbit tibia is strained with up to 170 % of the body weight, which is the compressive force an implant in a weight bearing bone has to be able to bear. Future research will focus on the in-vivo monitoring of bending and torsion forces and the implementation of these data into FEM-simulation

The Taylor Spatial Frame is a new external ring fixation system for correction of multi-planar deformities of the extremities. We report the first 100 consecutive cases treated with this system at the Bristol Limb Reconstruction Unit from November 1999. The Taylor Spatial Frame incorporates the technology of a virtual hinge and a Stewart Gough Platform. With the use of computer software it is capable of adjustments to within 1 degree and 0.5mm accuracy. Deformities are measured on plain radiographs. Required corrections over any period of time are calculated and a printed prescriptiion of daily adjustments is given to the patients to perform themselves at home. The following conditions were treated: Non-union (44), malunion (16), Leg length discrepancy (14), limb deformity (13), and acute fractures (13). The aims of frame treatment were non-union treatment (28), bone transport (12), acute fracture healing (12), correction of deformity (28), leg lengthening (15), and arthrodesis (5). Most cases involved the tibia (77) but the frame was also used on the femur (13), knee (3), ankle (4), humerus (2), and forearm (1). Complete correction of deformity was achieved in all but 7 patients. Union was achieved in 99 cases. All non unions united without bone graft. Mean transport of 46mm and lengthening of 38mm was achieved. 1 arthrodesis failed to unite. All fractures united without any residual deformity. Mean treatment time was 169 days (range 43 to 401). There was 100% compliance with patients performing adjustments themselves. Minor pin site problems were common (34 patients) but only 3 required debridement. Other problems included wire breakage (10), pain (3), peroneal nerve palsy (1) and DVT (1). 15 knees and 11 ankles developed stiffness which resolved. This study demonstrates the role of the Taylor Spatial Frame as an extremely versatile, accurate and safe new tool at the orthopaedic surgeon’s disposal in limb reconstruction and trauma surgery

In five teaching hospitals, seventy-two patients with seventy-three bicondylar tibial plateau fractures were prospectively randomized by envelope to treatment either by AO methods (group A) or ring fixator methods (group B). Outcome measures included clinical and radiographic parameters, & HSS knee scores. Results: Re-intervention was necessary within six months in ten group A & seven Group B patients. Forty-three procedures were performed (thirty-three Group A-ten Group B) I& D 12- 3: STSG 3-0: Quadricepsplasty 1-0; Manipulation 2–4; Muscle flap 2–0; Above knee amputation 1–0; Revision ORIF 5–1; Revision Rings 0–1; Bone graft 2–1; Bead pouch 3-0; Synovectomy 1-0; Sequestrectomy 1-0. More patients had more septic and wound complications resulting in more need for re-intervention following ORIF. Conclusion. For bi-condylar tibial plateau fractures (OTA 41.C) six-month HSS scores are significantly higher after treatment with Ring Fixator methods. Reintervention rates for deep sepsis/wound problems are higher with AO methods. Wound and infection complications occurring after AO treatment are more severe and require multiple procedures for control. We have conducted a prospective randomized trial to determine the outcomes of treatment by. Open reduction and internal fixation or. Closed reduction and ring fixation for the treatment of bi-condylar tibial plateau fractures (OTA 41.C). We report our early findings on re-intervention rates for complications. Aims

Minimally invasive fixation of pelvic fragility fractures is recommended to reduce pain and allow early mobilization. The purpose of this study was to evaluate the outcome of two different stabilization techniques in bilateral fragility fractures of the sacrum (BFFS).

Methods

A non-randomized, prospective study was carried out in a level 1 trauma centre. BFFS in 61 patients (mean age 80 years (SD 10); four male, 57 female) were treated surgically with bisegmental transsacral stablization (BTS; n = 41) versus spinopelvic fixation (SP; n = 20). Postoperative full weightbearing was allowed. The outcome was evaluated at two timepoints: discharge from inpatient treatment (TP1; Fitbit tracking, Zebris stance analysis), and ≥ six months (TP2; Fitbit tracking, Zebris analysis, based on modified Oswestry Disability Index (ODI), Majeed Score (MS), and the 12-Item Short Form Survey 12 (SF-12). Fracture healing was assessed by CT. The primary outcome parameter of functional recovery was the per-day step count; the secondary parameter was the subjective outcome assessed by questionnaires.

Aims

The aim of this study was to describe the use of 3D-printed sacral endoprostheses to reconstruct the pelvic ring and re-establish spinopelvic stability after total en bloc sacrectomy (TES) and to review its outcome.

Patients and Methods

We retrospectively reviewed 32 patients who underwent TES in our hospital between January 2015 and December 2017. We divided the patients into three groups on the basis of the method of reconstruction: an endoprosthesis group (n = 10); a combined reconstruction group (n = 14), who underwent non-endoprosthetic combined reconstruction, including anterior spinal column fixation; and a spinopelvic fixation (SPF) group (n = 8), who underwent only SPF. Spinopelvic stability, implant survival (IS), intraoperative haemorrhage rate, and perioperative complication rate in the endoprosthesis group were documented and compared with those of other two groups.
