Aims

Proximal humeral fractures (PHFs) are common. There is increasing evidence that most of these fractures should be treated conservatively. However, recent studies have shown an increase in use of operative treatment. The aim of this study was to identify the trends in the incidence and methods of treatment of PHFs in Finland.

Methods

The study included all Finnish inhabitants aged ≥ 16 years between 1997 and 2019. All records, including diagnostic codes for PHFs and all surgical procedure codes for these fractures, were identified from two national registers. Data exclusion criteria were implemented in order to identify only acute PHFs, and the operations performed to treat them.

The aim of this study was to quantify the stability of fracture-implant complex in fractures after fixation. A total of 15 patients with an undisplaced fracture of the femoral neck, treated with either a dynamic hip screw or three cannulated hip screws, and 16 patients with an AO31-A2 trochanteric fracture treated with a dynamic hip screw or a Gamma Nail, were included. Radiostereometric analysis was used at six weeks, four months and 12 months post-operatively to evaluate shortening and rotation.

Migration could be assessed in ten patients with a fracture of the femoral neck and seven with a trochanteric fracture. By four months post-operatively, a mean shortening of 5.4 mm (-0.04 to 16.1) had occurred in the fracture of the femoral neck group and 5.0 mm (-0.13 to 12.9) in the trochanteric fracture group. A wide range of rotation occurred in both types of fracture. Right-sided trochanteric fractures seem more rotationally stable than left-sided fractures.

This prospective study shows that migration at the fracture site occurs continuously during the first four post-operative months, after which stabilisation occurs. This information may allow the early recognition of patients at risk of failure of fixation.

Over the last decade stemless shoulder arthroplasty has become increasingly popular. However, stability of metaphyseal loading humeral components remains a concern. This study aimed to assess the stability of the Affinis stemless humeral component using Radiostereometric analysis (RSA). Patients underwent total shoulder arthroplasty via a standardised technique with a press-fit stemless humeral component and a cemented pegged glenoid. Tantalum beads were inserted into the humerus at the time of operation. RSA of the relaxed shoulder was completed at weeks 1, 6, 13, 26, 52 and 104 post-operatively. Stressed RSA with 12 newtons of abduction force was completed from week 13 onwards. ABRSA 5.0 software (Downing Imaging Limited, Aberdeen) was used to calculate humeral component migration and induced movement. 15 patients were recruited. Precision was: 0.041, 0.034, 0.086 and 0.101 mm for Superior, Medial, Posterior and Total Point Motion (TPM) respectively. The mean TPM over 2 years was 0.24 (0.30) mm, (Mean (Standard deviation)). The mean rate of migration per 3 month time period decreased from 0.45 (0.31) to 0.02 (0.01) mm over 2 years. Mean inducible movement TPM peaked at 26 weeks at 0.1 (0.08) mm, which reduced to 0.07 (0.06) mm by 104 weeks when only 3 patients had measurable inducible motion. There was no clear trend in direction of induced movement. There were no adverse events or revisions required. We conclude migration of the humeral component was low with little inducible movement in the majority of patients implying initial and 2 year stability of the stemless humeral component

Aims

Despite limited clinical scientific backing, an additional trochanteric stabilizing plate (TSP) has been advocated when treating unstable trochanteric fractures with a sliding hip screw (SHS). We aimed to explore whether the TSP would result in less post operative fracture motion, compared to SHS alone.

Methods

Overall, 31 patients with AO/OTA 31-A2 trochanteric fractures were randomized to either a SHS alone or a SHS with an additional TSP. To compare postoperative fracture motion, radiostereometric analysis (RSA) was performed before and after weightbearing, and then at four, eight, 12, 26, and 52 weeks. With the “after weightbearing” images as baseline, we calculated translations and rotations, including shortening and medialization of the femoral shaft.

Introduction. Total hip replacements using highly cross-linked polyethylene show excellent clinical outcomes, low wear, and minimal lysis at 5 years follow-up. A recent RSA study reports a significant increase in femoral head penetration between 5 and 7 years. This study is a multi-center radiographic analysis to determine whether the RSA observation is present in a large patient cohort. Methods. Six centers were enrolled for radiographic analysis of primary total hip arthroplasty for standard head sizes (26mm, 28mm, or 32mm). Radiographic inclusion criteria required a minimum of four films per patient at the following time points: 1 year; 2–4.5 years; 4.5–5.5 years; and 5.5–11 years. The Martell Hip Analysis Suite was used to analyze pelvic radiographs resulting in head penetration values. Wear rates were determined in two ways: the longest follow-up radiograph compared to the 1 year film, and individual linear regressions for the early and late periods. For both methods, average wear rates from the early period (1 to 5.5 years) and late period (>5.5 years) were compared using t-tests. Results. We present the completed analysis of 235 of the 250 hips under active analysis with 3160 film comparisons. Average follow-up was 7.2±0.99 years (range 5.4–10.1). Based on latest follow-up, the average wear rate was not significantly different during the early and late periods (10.1±95μm/year and −3.8±235μm/year respectively, p=0.518). Based on the group regression, the average wear rate was not significantly different during the early and late periods (7.5μm/year and −36μm/year respectively, p=0.13). Conclusion. In this large multicenter radiographic study, we found no late increase in femoral head penetration into highly cross-linked polyethylene as suggested by the RSA report. Additional centers and patients are being recruited in order to reduce the variation in the late period

Introduction. When introducing new joint replacement designs, it is difficult to predict with any certainty the clinical performance of the new designs. Using roentgen stereophotogrammetric analysis (RSA) to evaluate the first two years of follow-up can serve as a predictor of late mechanical loosening for hip and knee prostheses. This prospectively randomized study was designed to evaluate the clinical performance of the Triathlon total knee system and compare the results between the two versions; posterior stabilized (PS) and cruciate retaining (CR). Methods. Sixty patients were consecutively randomized (two patients left the study prior to surgery) to receive either the Triathlon total knee PS (30 patients) or the Triathlon total knee CR (28 patients). All components were cemented. The study was approved by the Ethical Committee for Lund University. All patients met the inclusion criteria. There were no statistically significant differences between the demographics for PS and CR. RSA was set to be the principal evaluation parameter. Patient outcome was assessed by the KSS and KOOS questionnaires. Results. There were no statistically significant differences in rotation or translation around or along the three coordinal axes. Neither were there any differences in the maximal total point motion (MTPM) during the 2-year follow-up (p<0.05). There were no statistically significant differences between the clinical results of both groups when using the KSS and the KOOS scoring system (p<0.05). Discussion. There were no clinically significant differences in the RSA 2-year follow-up data, nor in the clinical scores of KSS and KOOS. The results of this study suggest that there is no major differences between the posterior stabilized system and the cruciate retaining system for the Triathlon total knee neither in clinical nor in predictable long term mechanical performance

Due to its popularity of intramedullary nails (IMN) high success rate, newer design (titanium) IMN system have been introduced to replace stainless steel system. However the stability provided by the titanium IMN. may not be adequate, there by influencing the union rate. We aimed to compare the results of both IMN systems via prospective clinical study and biomechanical testing using RSA. Biomechanical study. This study was done in an experimental set-up which consisted of a physically simulated femoral shaft fractures models fixed with a stainless steel (Russell Taylor) or Titanium (Trigen) IM nailing system. Two common fracture configurations with stimulated weight bearing conditions were used and the axis of fragment movements recorded. Clinical study. The data on two groups of patients were collected as part of a prospective cohort study. Details of the implant, such as size of nail, cross screw lengths, screw thickness, etc. was collected. Patients were followed up for a minimum of 4 months and details of clinical complications recorded. Biomechanical study. The degree of translation movement in comminuted fracture, using titanium IMN system, was 6 times more compared to stainless steel IMN system. Clinical study. The results show that there is a 5.7% of non union and 14% hardware problems with titanium based IMN system when compared to 2.2% non union in the stainless steel IMN system. Titanium based IM nailing system have a potential to inherent mechanical instability when used to treat comminuted fractures. This may explain some of the clinically observed delayed or non-union of femoral fractures

Purpose. to analyze the survivorship of the RSA with a minimum 10 years follow up. Patients and Methods. Between 1992 and 1999, 145 Delta (DePuy) RSAs have been implanted in 138 patients. It was a mulicentric study. Initial etiologies were gathered as following: group A (92 cases) Cuff tear arthropaties (CTA), osteoarthritis (OA) with at least 2 involved cuff tendons, and massive cuff tear with pseudoparalysis (MCT); group B (39 cases) -failed hemiarthroplasties (HA), failed total shoulder arthroplasties (TSA), and fracture sequelae; and group C (14 cases) rheumatoid arthritis, fractures, tumor, and instability. Survival curves were established with the Kaplan-Meier technique. Two end-points were retained: -implant revision, defined by glenoid or humeral replacement or removal, or conversion to HA; - a poor clinical outcome defined by an absolute Constant score of less than 30. Results. At the time of review, 47 patients had died with their prosthesis in place and 30 were lost to follow-up. There were 12 revisions, 6 for infections, 3 for glenoid loosening, 1 for dislocation, 1 for glenoid dissociation (by unscrewing), and 1 for humeral loosening. The survival curve to prosthetic removal showed an overall survivorship of 92% at 10 years. Segmentation according to etiology showed a 97% survivorship for group A and 88% for group B This difference was not significant. No patients of group C had a minimum follow up of ten years because there were died or lost to follow up. The survival curve to a Constant score of less than 30 showed an overall survivorship of 90% at 10 years. Segmentation according to etiology showed a significant difference at 10 years in favor of group A (92%) compared to group B (86%) with a break of the curve after 9 years for group B. Discussion. Our results show that the overall survivorship of the reverse shoulder prosthesis to removal is good even 10 years after implantation, in particular if it had been implanted for CTA, OA, or MCT. However, functional results did deteriorate progressively after 9 years in particular if it had been implanted for revision (HA or TSA). Therefore, extreme caution must be observed in relation to the indications for reverse shoulder arthroplasty, especially in younger patients

Introduction. Several short femoral stems have been introduced in primary total hip arthroplasty, supposedly in order to save proximal bone stock. We intended to analyse primary stability, changes in periprosthetic bone mineral density (BMD), and clinical outcome after insertion of the uncemented collum femoris preserving (CFP)-femoral device. Methods. A prospective cohort study on 30 patients scheduled for receiving the CFP-stem combined with an uncemented cup was carried out. Stem migration was analysed by radiostereometry (RSA). Preoperative total hip BMD and postoperative periprosthetic BMD in Gruen zones 1–7 was investigated by DXA, and the Harris hips score (HHS) was determined. The patients were followed up to 12 months. Results. 2 patients were intraoperatively excluded because their proximal femur was found to be unsuitable for insertion of the studied implant, 1 patient was later revised due to a deep infection. This left 27 patients for final analysis. RSA showed that only very little migration of the implant occurred, with the largest amplitude found in rotation around the y-axis (1.8°, SD 0.6, after 12 mths), representing minimal stem retroversion. DXA after 12 mths demonstrated substantial BMD loss in Gruen zones 7 (−30.8%), 6 (−19.1%) and 2 (−13.3%, p-values for all described changes <0.001 when comparing with baseline BMD determined immediately postoperatively). There was a moderate correlation of low preoperative total hip BMD with a higher amount of bone loss in Gruen zones 2 (Pearson correlation coefficient r = 0.6, p = 0.001), 6 (r = 0.5, p = 0.005) and 7 (r = 0.6, p = 0.003). In contrast, we found no correlation of periprosthetic bone loss in any of the Gruen zones 1–7 with logarithmically transformed maximal total point translation (MTPT) of the stem (p > 0.05 for all regions), neither after 3 nor after 12 mths. The mean HHS increased from 49 (SD 15) preoperatively to 99 (SD 2) after 12 mths. Interpretation. Based on these short-term data, we conclude that i) the studied implant seems to be stable within the first year, ii) substantial loss in periprosthetic BMD - with a predominance in the calcar region - occurs, iii) low preoperative total hip BMD predisposes towards greater loss of periprosthetic BMD after 12 months, iv) postoperative loss in periprosthetic BMD does not correlate with increased stem migration. Clinical results are excellent so far. Continuing follow-up will reveal whether this novel stem remains stable in the medium and long term, and whether the loss in BMD in the regions mentioned above can be recovered with time or whether it continues

We investigated the stability of seven Schatzker type II fractures of the lateral tibial plateau treated by subchondral screws and a buttress plate followed by immediate partial weight-bearing. In order to assess the stability of the fracture, weight-bearing inducible displacements of the fracture fragments and their migration over a one-year period were measured by differentially loaded radiostereometric analysis and standard radiostereometric analysis, respectively. The mean inducible craniocaudal fracture fragment displacements measured −0.30 mm (−0.73 to 0.02) at two weeks and 0.00 mm (−0.12 to 0.15) at 52 weeks. All inducible displacements were elastic in nature under all loads at each examination during follow-up. At one year, the mean craniocaudal migration of the fracture fragments was −0.34 mm (−1.64 to 1.51).

Using radiostereometric methods, this case series has shown that in the Schatzker type II fractures investigated, internal fixation with subchondral screws and a buttress plate provided adequate stability to allow immediate post-operative partial weight-bearing, without harmful consequences.

Our aim in this pilot study was to evaluate the fixation of, the bone remodelling around, and the clinical outcome after surgery of a new, uncemented, fully hydroxyapatite-coated, collared and tapered femoral component, designed specifically for elderly patients with a fracture of the femoral neck.

We enrolled 50 patients, of at least 70 years of age, with an acute displaced fracture of the femoral neck in this prospective single-series study. They received a total hip replacement using the new component and were followed up regularly for two years.

Fixation was evaluated by radiostereometric analysis and bone remodelling by dual-energy x-ray absorptiometry. Hip function and the health-related quality of life were assessed using the Harris hip score and the EuroQol-5D.

Up to six weeks post-operatively there was a mean subsidence of 0.2 mm (−2.1 to +0.5) and a retroversion of a mean of 1.2° (−8.2° to +1.5°). No component migrated after three months. The patients had a continuous loss of peri-prosthetic bone which amounted to a mean of 16% (−49% to +10%) at two years. The mean Harris hip score was 82 (51 to 100) after two years.

The two-year results from this pilot study indicate that this new, uncemented femoral component can be used for elderly patients with osteoporotic fractures of the femoral neck.

Patients with transfemoral amputation (TFA) often experience problems related to the use of socket-suspended prostheses. The clinical development of osseointegrated percutaneous prostheses for patients with a TFA started in 1990, based on the long-term successful results of osseointegrated dental implants.

Between1999 and 2007, 51 patients with 55 TFAs were consecutively enrolled in a prospective, single-centre non-randomised study and followed for two years. The indication for amputation was trauma in 33 patients (65%) and tumour in 12 (24%). A two-stage surgical procedure was used to introduce a percutaneous implant to which an external amputation prosthesis was attached. The assessment of outcome included the use of two self-report questionnaires, the Questionnaire for Persons with a Transfemoral Amputation (Q-TFA) and the Short-Form (SF)-36.

The cumulative survival at two years’ follow-up was 92%. The Q-TFA showed improved prosthetic use, mobility, global situation and fewer problems (all p < 0.001). The physical function SF-36 scores were also improved (p < 0.001). Superficial infection was the most frequent complication, occurring 41 times in 28 patients (rate of infection 54.9%). Most were treated effectively with oral antibiotics. The implant was removed in four patients because of loosening (three aseptic, one infection).

Osseointegrated percutaneous implants constitute a novel form of treatment for patients with TFA. The high cumulative survival rate at two years (92%) combined with enhanced prosthetic use and mobility, fewer problems and improved quality of life, supports the ‘revolutionary change’ that patients with TFA have reported following treatment with osseointegrated percutaneous prostheses.
