The purpose of this study was to describe the radiological characteristics of a previously unreported finding: posterior iliac offset at the sacroiliac joint and to assess its association with pelvic instability as measured by initial displacement and early implant loosening or failure. Radiographs from 42 consecutive patients with a mean age of 42 years (18 to 77; 38 men, four women) and mean follow-up of 38 months (3 to 96) with Anteroposterior Compression II injuries, were retrospectively reviewed. Standardised measurements were recorded for the extent of any diastasis of the pubic symphysis, widening of the sacroiliac joint, static vertical ramus offset and a novel measurement (posterior offset of the ilium at the sacroiliac joint identified on axial CT scan). Pelvic fractures with posterior iliac offset exhibited greater levels of initial displacement of the anterior pelvis (anterior sacroiliac widening, pubic symphysis diastasis and static vertical ramus offset, p < 0.001,0.034 and 0.028, respectively). Pelvic fractures with posterior ilium offset also demonstrated higher rates of implant loosening regardless of fixation method (p = 0.05). Posterior offset of the ilium was found to be a reliable and reproducible measurement with substantial inter-observer agreement (kappa = 0.70). Posterior offset of the ilium on axial CT scan is associated with greater levels of initial pelvic displacement and early implant loosening. Cite this article: Bone Joint J 2014;96-B:1535–9

The purpose of this study is to analyze what kind of pattern of change in each posterior femoral condyle allows for a greater degree of flexion after total knee arthroplasty (TKA). The flexion angle was assessed pre-operatively, and at 12 months after the surgery in 98 patients (106 knees) who underwent consecutive TKA. We used a quantitative 3 dimensional technique using computed tomography for the assessment of changes in both the medial and lateral femoral condylar offset. There were no significant correlation between changes of each posterior condylar offset and post flexion angle (medial condyle; R=−0.038, p=0.70, lateral condyle; R=−0.090, p=0.36). There were no significant differences between changing patterns and increase rate of flexion (p=0.443). Additionally there were no significant differences between changing patterns and increase of flexion angle (p=0.593). Changes of each posterior condylar offset were no correlation to knee flexion after TKA in the current design prosthesis

The Bernese Periacetabular Osteotomy (PAO) has become the established method for treating developmental dysplasia of the hip. In the 1990s, the surgical technique was modified to avoid postoperative cam impingement due to uncorrected head neck offset or pincer impingement due to acetabular retroversion after reorientation. The goal of the study was to compare the survivorship of two series of PAOs with and without the modifications of the surgical technique and to calculate predictive factors for a poor outcome. A retrospective, comparative study of two consecutive series of PAOs with a minimum follow-up of 10 years was carried out. Series A included 75 PAOs performed between 1984 and 1987 and represent the first cases of PAO. Series B included 90 hips that underwent PAO between 1997 and 2000. In this series, emphasis was put on an optimal acetabular version next to the correction of the lateral coverage. Additionally, a concomitant arthrotomy was performed in every hip to check impingement-free range of motion after reorientation and in 50 hips (56%) an additional offset correction was performed. Survivorship analyses according to Kaplan and Meier were carried out and the endpoint was defined as conversion to a total hip arthroplasty, progression of osteoarthritis, or a Merle d'Aubign score 14. Predictive factors for poor outcome were calculated using the Cox-regression analysis. The cumulative 10-year survivorship of Series A was significantly decreased (77%; 95%-confidence interval [CI] 72–82%) compared to Series B (86%; 95%-CI 82–89%, p=0.005). Hips with an aspherical head showed a significantly increased survivorship if a concomitant offset correction was performed intraoperatively (90% [95%-CI 86–94%] versus 77% [95%-CI 71–82%], p=0.003). Preoperative factors predicting poor outcome included a high age at surgery, a Merle d'Aubign score 14, a positive impingement test, a positive Trendelenburg sign, limp, an increased grade of osteoarthritis according to Tönnis, and (sub-) luxation of the femoral head (Severin > 3). In addition, predictive factors related to the three dimensional orientation of the acetabular fragment were identified. These included total, anterior, and posterior acetabular over-coverage or under-coverage, acetabular retroversion or excessive anteversion, a lateral center edge angle < 22 °, an acetabular index > 14 °, and no offset correction in aspherical femoral heads. A good long term result after PAO mainly depends on optimal three-dimensional orientation of the acetabulum and impingement-free range of motion with correction of an aspherical head neck junction if necessary

Aims

To evaluate if, for orthopaedic trainees, additional cadaveric simulation training or standard training alone yields superior radiological and clinical outcomes in patients undergoing dynamic hip screw (DHS) fixation or hemiarthroplasty for hip fracture.

Methods

This was a preliminary, pragmatic, multicentre, parallel group randomized controlled trial in nine secondary and tertiary NHS hospitals in England. Researchers were blinded to group allocation. Overall, 40 trainees in the West Midlands were eligible: 33 agreed to take part and were randomized, five withdrew after randomization, 13 were allocated cadaveric training, and 15 were allocated standard training. The intervention was an additional two-day cadaveric simulation course. The control group received standard on-the-job training. Primary outcome was implant position on the postoperative radiograph: tip-apex distance (mm) (DHS) and leg length discrepancy (mm) (hemiarthroplasty). Secondary clinical outcomes were procedure time, length of hospital stay, acute postoperative complication rate, and 12-month mortality. Procedure-specific secondary outcomes were intraoperative radiation dose (for DHS) and postoperative blood transfusion requirement (hemiarthroplasty).

Background. Femoroacetabular impingement due to a reduced femoral head-neck-offset or excessive acetabular coverage will lead to early cartilage lesions of the hip joint. The purpose of the present study was to analyze the relationship between the extent of bony deformity and the presence and extent of cartilage lesions in femoroacetabular impingement. Methods. On the radiographs of 92 hip joints in 86 patients with a mean age of 36.5 ± 9.2 years who were operated on for with femoroacetabular impingement by surgical hip dislocation, the acetabular index of the weight-bearing zone, the center-edge-angle, the inclination of the acetabulum, the lateral-head-extrusion-index, retroversion signs of the acetabulum, the neck-shaft-angle, asphericity, superior and anterior alpha angles, and superior and anterior offset and offset ratios were assessed and correlated to the presence and extent of chondromalazia. Results. Acetabular chondromalazia was found in 60 hip joints. Full-thickness delamination of the acetabular cartilage was present in 42 of these hips with an average size of 1.1 ± 0.5 cm2. Femoral-sided chondromalazia was found in 9 hip joints. Hip joints with chondromalazia had a lower center-edge-angle (p = 0.014) and anterior offset ratio (p = 0.028) and a higher lateral-head-extrusion-index (p = 0.029). Hip joints with acetabular cartilage delamination had a significantly lower center-edge-angle (p = 0.012) and anterior offset (p = 0.03) and a significantly higher lateral-head-extrusion-index (p = 0.001) and superior (p = 0.006) and anterior alpha angles (p = 0.003). Conclusions. Marked cam-type femoral deformity and lesser coverage of the femoral head are associated with the acetabular cartilage lesions, arousing suspicion to the assumption that a large lateral aperture of the acetabulum abets proximal femora with cam-type deformity to inflict cartilage damage in femoroacetabular impingement

Aims

Patients with A1 and A2 trochanteric hip fractures represent a substantial proportion of trauma caseload, and national guidelines recommend that sliding hip screws (SHS) should be used for these injuries. Despite this, intramedullary nails (IMNs) are routinely implanted in many hospitals, at extra cost and with unproven patient outcome benefit. We have used data from the National Hip Fracture Database (NHFD) to examine the use of SHS and IMN for A1 and A2 hip fractures at a national level, and to define the cost implications of management decisions that run counter to national guidelines.

Methods

We used the NHFD to identify all operations for fixation of trochanteric fractures in England and Wales between 1 January 2021 and 31 December 2021. A uniform price band from each of three hip fracture implant manufacturers was used to set cost implications alongside variation in implant use.

Aims

Surgery is often delayed in patients who sustain a hip fracture and are treated with a total hip arthroplasty (THA), in order to await appropriate surgical expertise. There are established links between delay and poorer outcomes in all patients with a hip fracture, but there is little information about the impact of delay in the less frail patients who undergo THA. The aim of this study was to investigate the influence of delayed surgery on outcomes in these patients.

Methods

A retrospective cohort study was undertaken using data from the Scottish Hip Fracture Audit between May 2016 and December 2020. Only patients undergoing THA were included, with categorization according to surgical treatment within 36 hours of admission (≤ 36 hours = ‘acute group’ vs > 36 hours = ‘delayed’ group). Those with delays due to being “medically unfit” were excluded. The primary outcome measure was 30-day survival. Costs were estimated in relation to the differences in the lengths of stay.

Aims

Despite multiple trials and case series on hip hemiarthroplasty designs, guidance is still lacking on which implant to use. One particularly deficient area is long-term outcomes. We present over 1,000 consecutive cemented Thompson’s hemiarthroplasties over a ten-year period, recording all accessible patient and implant outcomes.

Methods

Patient identifiers for a consecutive cohort treated between 1 January 2003 and 31 December 2011 were linked to radiographs, surgical notes, clinic letters, and mortality data from a national dataset. This allowed charting of their postoperative course, complications, readmissions, returns to theatre, revisions, and deaths. We also identified all postoperative attendances at the Emergency and Outpatient Departments, and recorded any subsequent skeletal injuries.

Aims

Current levels of hip fracture morbidity contribute greatly to the overall burden on health and social care services. Given the anticipated ageing of the population over the coming decade, there is potential for this burden to increase further, although the exact scale of impact has not been identified in contemporary literature. We therefore set out to predict the future incidence of hip fracture and help inform appropriate service provision to maintain an adequate standard of care.

Methods

Historical data from the Scottish Hip Fracture Audit (2017 to 2021) were used to identify monthly incidence rates. Established time series forecasting techniques (Exponential Smoothing and Autoregressive Integrated Moving Average) were then used to predict the annual number of hip fractures from 2022 to 2029, including adjustment for predicted changes in national population demographics. Predicted differences in service-level outcomes (length of stay and discharge destination) were analyzed, including the associated financial cost of any changes.

Aims

The aims of this study were to identify means to quantify coronal plane displacement associated with distal radius fractures (DRFs), and to understand their relationship to radial inclination (RI).

Methods

From posteroanterior digital radiographs of healed DRFs in 398 female patients aged 70 years or older, and 32 unfractured control wrists, the relationships of RI, quantifiably, to four linear measurements made perpendicular to reference distal radial shaft (DRS) and ulnar shaft (DUS) axes were analyzed: 1) DRS to radial aspect of ulnar head (DRS-U); 2) DUS to volar-ulnar corner of distal radius (DUS-R); 3) DRS to proximal capitate (DRS-PC); and 4) DRS to DUS (interaxis distance, IAD); and, qualitatively, to the distal ulnar fracture, and its intersection with the DUS axis.

Aims

The aim of this study was to describe the management and associated outcomes of patients sustaining a femoral hip periprosthetic fracture (PPF) in the UK population.

Methods

This was a multicentre retrospective cohort study including adult patients who presented to 27 NHS hospitals with 539 new PPFs between 1 January 2018 and 31 December 2018. Data collected included: management strategy (operative and nonoperative), length of stay, discharge destination, and details of post-treatment outcomes (reoperation, readmission, and 30-day and 12-month mortality). Descriptive analysis by fracture type was performed, and predictors of PPF management and outcomes were assessed using mixed-effects logistic regression.

Anterior knee pain (AKP) is the commonest complication of total knee arthroplasty (TKA). This study aims to assess whether sagittal femoral component position is an independent predictor of AKP after cruciate retaining single radius TKA without primary patellofemoral resurfacing. From a prospective cohort of 297 consecutive TKAs, 73 (25%) patients reported AKP and 89 (30%) reported no pain at 10 years. Patients were assessed pre-operatively and at 1, 5 and 10 years using the short form 12 and Oxford Knee Score (OKS). Variables assessed included demographic data, indication, reoperation, patella resurfacing, and radiographic criteria. Patients with AKP (mean age 67.0 (38–82), 48 (66%) female) had mean Visual Analogue Scale (VAS) Pain scores of 34.3 (range 5–100). VAS scores were 0 in patients with no pain (mean age 66.5 (41–82), 60 (67%) female). Femoral component flexion (FCF), anterior femoral offset ratio, and medial proximal tibial angle all differed significantly between patients with AKP and no pain (p<0.001), p=0.007, p=0.009, respectively). All PROMs were worse in the AKP group at 10 years (p<0.05). OKSs were worse from 1 year (p<0.05). Multivariate analysis confirmed FCF and Insall ratio <0.8 as independent predictors of AKP (R. 2. = 0.263). Extension of ≥0.5° predicted AKP with 87% sensitivity. AKP affects 25% of patients following single radius cruciate retaining TKA, resulting in inferior patient-reported outcome measures at 10 years. Sagittal plane positioning and alignment of the femoral component are important determinants of long-term AKP with femoral component extension being a major risk factor

We evaluated the results of Cementless Surface Replacement Arthroplasty (CSRA) of the shoulder in 67 patients with advanced glenohumeral destruction who have an intact rotator cuff. Between november 2002 and december 2008, 70 CSRA (32 Copeland/ Biomet and 38 SMRR/ Lima) were implanted in 67 patients. A deltopectoral approach was used in 34 cases and an anterosuperior approach in 36 cases. Patients were assessed using Constant score, a patient satisfaction score and a detailed radiographic analysis. The mean follow up was 3.4 years (range 1 to 7.5 years). The mean Constant score improved preoperatively from 17.6 points (range 2–55) to an average postoperative score of 66.1 (range 13–91). The pain score improved from 1.13 points (range 0–6) to 12.3 points (range 3–15). The forward flexion and external rotation improved from 71° (range 20 to 140) and 0° (range −40 to +45) to 143° (range 60 to180) and 34.4° (range −20 to +60) respectively. Complications included: 1 subscapularis detachment, 5 secondary rotator cuff tear, 1sepsis, 3 patients with shoulder stiffness. No shift in implant position was observed. 11 humeral components developed radiolucencies at the prosthesis-bone interface. The radiographic analysis involved a system of dividing the prosthesis/bone interface into 5 zones. The best clinical results were significantly achieved in patients with necrosis compared with osteoarthritis (Constant Score, ant. elevation, ext. Rotation). Using regression analysis we found that changes in the head-shaft angle position of the implant (valgus/varus placement of the CSRA) significantly predicted the age and sex adjusted Constant score. When the inclination angle of the humeral head decreases, the adjusted Constant score increases. In the same model, we also found that the lateral offset of the humerus significantly predicted the adjusted Constant score. When the lateral offset of the humerus decreases, the adjusted Constant score increases. The medialization of the glenoid significantly and negatively predicted the Constant score. Conclusion. CSRA of the shoulder outcomes have been comparable with those of stemmed arthroplasties. Radiolucent lines occur with follow up and most of the time located in the S1 area. Glenoid wear and humeral head lateralization negatively impact the clinical score. Cementless Shoulder resurfacing is a viable alternative to conventional shoulder arthroplasty

Aims

Patients who sustain neck of femur fractures are at high risk of malnutrition. Our intention was to assess to what extent malnutrition was associated with worse patient outcomes.

Methods

A total of 1,199 patients with femoral neck fractures presented to a large UK teaching hospital over a three-year period. All patients had nutritional assessments performed using the Malnutrition Universal Screening Tool (MUST). Malnutrition risk was compared to mortality, length of hospital stay, and discharge destination using logistic regression. Adjustments were made for covariates to identify whether malnutrition risk independently affected these outcomes.

Aims

Complex joint fractures of the lower extremity are often accompanied by soft-tissue swelling and are associated with prolonged hospitalization and soft-tissue complications. The aim of the study was to evaluate the effect of vascular impulse technology (VIT) on soft-tissue conditioning in comparison with conventional elevation.

Methods

A total of 100 patients were included in this prospective, randomized, controlled monocentre study allocated to the three subgroups of dislocated ankle fracture (n = 40), pilon fracture (n = 20), and intra-articular calcaneal fracture (n = 40). Patients were randomized to the two study groups in a 1:1 ratio. The effectiveness of VIT (intervention) compared with elevation (control) was analyzed separately for the whole study population and for the three subgroups. The primary endpoint was the time from admission until operability (in days).

Background. Symptomatic flexion deformity of proximal interpahalangeal joint (PIPJ) is one of the most common foot deformities and usually treated with arthrodesis. In general, percutaneous K-wires are used to stabilize the joint after excision of cartilage. K-wires projecting out of the toe need special care and can occasionally be dislodged accidentally. Furthermore issues such as cellulitis, pin tract infections, rarely osteomyelitis and need for removal make alternative fixation methods desirable. Smart toe is an intra-osseous titanium memory implant, which is stored frozen. It expands on insertion and does not require removal. Methods. 30 consecutive K-wire PIPJ arthrodesis were compared with 30 Smart toe PIP fusions with a mean follow up of 6 months. Post operative forefoot scores and complications were documented. Results. Patient satisfaction was higher and complications were lower with Smart toe fusions than with K-wire arthrodesis. Conclusions. Fusion of PIP joints with smart toe is an effective and safer alternative to using K-wires. Although more expensive, higher patient satisfaction and lower complication rate may offset the extra cost of the implant

Aims

Complex displaced osteoporotic acetabular fractures in the elderly are associated with high levels of morbidity and mortality. Surgical options include either open reduction and internal fixation alone, or combined with total hip arthroplasty (THA). There remains a cohort of severely comorbid patients who are deemed unfit for extensive surgical reconstruction and are treated conservatively. We describe the results of a coned hemipelvis reconstruction and THA inserted via a posterior approach to the hip as the primary treatment for this severely high-risk cohort.

Methods

We have prospectively monitored a series of 22 cases (21 patients) with a mean follow-up of 32 months (13 to 59).

Introduction. It was the purpose to evaluate the biomechanical changes that occur after optimal and non-optimal component placement of a hip resurfacing (SRA) by using a subject specific musculoskeletal model based on CT-scan data. Materials and Methods. Nineteen hips from 11 cadavers were resurfaced with a BHR using a femoral navigation system. CT images were acquired before and after surgery. Grey-value segmentation in Mimics produced contours representing the bone geometry and identifying the outlines of the 3 parts of the gluteus medius. The anatomical changes induced by the procedure were characterised by the translation of the hip joint center (HJCR) with respect to the pelvic and femoral bone. The contact forces during normal gait with ‘optimal’ component placement were calculated for a cement mantle of 3 mm, a socket inclination of 45° and anteversion of 15°. The biomechanical effect of ‘non-optimal placement’ was simulated by varying the positioning of the components. Results. There was a significant (p<0.01) shortening of the muscle length with the ‘optimal’ component placement for all parts of the gluteus medius with the largest shortening of the posterior part by 6mm. This was caused by a significant shortening of the femoral offset by 2.3mm (p<0.01). Because of a significant (p<0.01) medialisation of the HJCR by 4 mm, there was no significant increase in contact force. The hip joint contact forces increased by 0.5% per mm HJCR displacement. Each millimeter of cranial and lateral displacement of the femoral HJCR increased the contact force by 0.5% and 1%, respectively. The contact stresses changed significantly by 0.8% and 0.2% per degree of socket inclination and anteversion. The contact force increased 1% per mm lateral displacement of the acetabular HJCR. Discussion. Optimal placement of the SRA components did not completely restore the biomechanics of the native hip joint. The contact forces were not increased due to the compensatory effect of the medialisation of the acetabular HJCR. This suggests that reaming to the acetabular floor should be conducted in SRA. Femoral component displacement in the cranial and lateral direction significantly increased the hip joint loading. Errors of socket placement in the coronal and sagital plane significantly increased the contact stresses. Accumulative errors of both component displacements could lead to increased contact stresses of 18% to 23% with socket inclinations of 50° and 55°. Surgeons should reconsider continuing the SRA procedure if a neck length loss and lateralisation of the HCJR by >5 mm is anticipated as this would increase the contact stresses by >12%

Objectives. Osteochondral ankle defects (OCDs) mainly occur in a young, active population. In 63% of cases the defect is located on the medial talar dome. Arthroscopic debridement and microfracture is considered the primary treatment for defects up to 15 mm. To treat patients with a secondary OCD of the medial talar dome, a 15-mm diameter metal implant (HemiCAP ®) was developed. The set of 15 offset sizes was designed to correspond with the anatomy of various talar dome curvatures. Recently, two independent biomechanical cadaver studies were published, providing rationale for clinical use. The present study was undertaken to evaluate the clinical effectiveness and safety of the metal implantation technique for osteochondral lesions of the medial talar dome in a prospective study. Material and methods. Since October 2007, twenty patients have been treated with the implant. Four patients who did not meet the inclusion criteria and four patients who had less than one-year follow-up at the time of writing were left out of this analysis. Twelve patients are reported with one year (n=8) or two years (n=4) follow-up. All patients had had one or two earlier operations without success. On preoperative CT-scanning, the mean lesion size was 16 × 11 (range, 9–26 × 8–14) mm. Outcome measures were: Numeric Rating Scale pain (NRS) at rest and when walking, Foot Ankle Outcome Score (FAOS), American Orthopaedic Foot and Ankle Society (AOFAS) Ankle-Hindfoot score, and clinical and radiographic complications. Data are presented as median and range. The Wilcoxon signed ranks test was used to calculate p-values. Results. All patients recovered well from surgery. The NRS at rest improved from 3 (0–7) preoperatively, to 0.5 (0–4) after 1 year and 1 (0–1) after 2 years follow-up (p < 0.05). The NRS when walking was 6.5 (4–8) preoperatively, improving to 1.5 (0–5) at 1 year and 1 (0–2) at 2 years follow-up (p < 0.05). The five subscales of the FAOS improved from 14–64 preoperatively, to 53–91 after 1 year and 63–100 after 2 years (p < 0.05). The AOFAS improved from 70 (42–75) before surgery to 86 (58–100) at 1 year, and 89 (82–90) at 2 years follow-up (p < 0.05). There were no clinical or radiographic complications. Conclusion. The metallic implantation technique appears to be a promising treatment for secondary OCDs of the talus, but more patients and longer follow-up are necessary to draw firm conclusions

Background/Aims. The development of extendable prostheses has permitted limb salvage surgery in paediatric patients with bone tumours in proximity to the physis. Prostheses are extended to offset limb length discrepancy as the child grows. Aseptic loosening (AL) is a recognised complication. The implant stem must fit the narrow paediatric medullary canal and remain fixed while withstanding growth and increasing physical demands. Novel designs incorporate a hydroxyapatite (HA) coated collar that manufacturers claim improves bony ongrowth and stability, providing even stress distribution in stem and shoulder regions and providing a bone-implant seal, resulting in decreased AL and prolonged survival. This study aims to assess whether there is a relationship between bony ongrowth onto a HA collar and AL. Hypothesis: Bone ongrowth onto the HA collar of extendable prostheses is associated with more stable fixation and less AL despite patient growth. Methods. Retrospective review of 51 primary partial femoral extendable prostheses implanted over 12 years from 1994–2006 (followed up to death at a mean of 2.5±2.2 years or last clinical encounter at a mean of 8.6 years) and 24 subsequent revisions, to ascertain failure rate and mode, together with a cohort study reviewing bony ongrowth onto the HA coated collar in 10 loose and 13 well fixed partial femoral, humeral and tibial implants. Patient growth was measured as a change in bone:implant-width ratio. Results. 21 (41.2%) primary femoral implants failed at a mean 42.8 months, 5 through AL. 1 secondary implant was revised for AL. 2 implants displayed evidence of progressive AL but had not failed at last follow-up. 5 of 11 tibial component revisions in distal femoral replacement were due to AL. 1 major complication occurred after revision surgery for AL in a primary implant: deep infection requiring 2 stage revision. Bony collar ongrowth was significantly higher in all 4 quadrants (anterior, posterior, medial and lateral) in the well-fixed as opposed to loose group, demonstrating a strong negative relationship in each quadrant between bony ongrowth and AL (p0.001) in the presence of patient growth as shown by increased bone:implant width ratio. In both groups, collar ongrowth was greatest in the posterior quadrant. Summary and Conclusions: AL has been confirmed as a common cause of failure in massive extendable endoprostheses. Revision surgery is difficult and may cause serious complications. For the first time, a significant relationship between a well fixed implant stem and bony ongrowth onto a HA coated collar in the context of massive implants used in tumour surgery has been demonstrated. This newly-proven relationship may result in longer-term implant survival and thus a reduced need for revision surgery. It is hoped that this study will provide the basis for further study of this relationship
