Aims

This study aims to assess the relationship between history of pseudotumour formation secondary to metal-on-metal (MoM) implants and periprosthetic joint infection (PJI) rate, as well as establish ESR and CRP thresholds that are suggestive of infection in these patients. We hypothesized that patients with a pseudotumour were at increased risk of infection.

Methods

A total of 1,171 total hip arthroplasty (THA) patients with MoM articulations from August 2000 to March 2014 were retrospectively identified. Of those, 328 patients underwent metal artefact reduction sequence MRI and had minimum two years’ clinical follow-up, and met our inclusion criteria. Data collected included demographic details, surgical indication, laterality, implants used, history of pseudotumour, and their corresponding preoperative ESR (mm/hr) and CRP (mg/dl) levels. Multivariate logistic regression modelling was used to evaluate PJI and history of pseudotumour, and receiver operating characteristic curves were created to assess the diagnostic capabilities of ESR and CRP to determine the presence of infection in patients undergoing revision surgery.

Metal artefact reduction (MAR) MRI is now widely considered to be the standard for imaging metal-on-metal (MoM) hip implants. The Medicines and Healthcare Products Regulatory Agency (MHRA) has recommended cross-sectional imaging for all patients with symptomatic MoM bearings. This paper describes the natural history of MoM disease in a 28 mm MoM total hip replacement (THR) using MAR MRI. Inclusion criteria were patients with MoM THRs who had not been revised and had at least two serial MAR MRI scans. All examinations were reported by an experienced observer and classified as A (normal), B (infection) or C1–C3 (mild, moderate, severe MoM-related abnormalities). Between 2002 and 2011 a total of 239 MRIs were performed on 80 patients (two to four scans per THR); 63 initial MRIs (61%) were normal. On subsequent MRIs, six initially normal scans (9.5%) showed progression to a disease state; 15 (15%) of 103 THRs with sequential scans demonstrated worsening disease on subsequent imaging. Most patients with a MoM THR who do not undergo early revision have normal MRI scans. Late progression (from normal to abnormal, or from mild to more severe MoM disease) is not common and takes place over several years. Cite this article: Bone Joint J 2013;95-B:1035–9

Femoroacetabular impingement is a prearthritic deformity frequently associated with early chondral damage. Several techniques exist for restoring larger cartilage defects. While AMIC proved to be an effective treatment in knee and ankle, there are only short-term data available in hip. This study aimed to investigate the mid-term clinical outcome of patients with chondral lesions treated by AMIC and evaluate the quality of repair tissue via MRI. This retrospective, single center study includes 18 patients undergoing surgical hip dislocation for FAI between 2013 and 2016. Inclusion criteria were: cam or pincer-type FAI, femoral or acetabular chondral lesions > 1 cm. 2. , (IRCS III-IV). Due to exclusion criteria and loss-to-follow-up 9 patients (10 hips) could be included. Patient reported outcome measures included Oxford Hip Score (OHS) & Core Outcome Measure Index (COMI)). MRIs were evaluated using the Magnetic Resonance Observation of Cartilage Repair Tissue (MOCART) score. None of the patients underwent revision surgery except screw removals from the greater trochanter. Followup data indicate a satisfactory to good hip function at 5 years: PROMS improved from pre- to postop at 5 years: OHS from 38.1 to 43.4, COMI from to 1.8 and UCLA from 4 to 8.1 respectively. MOCART score was 67.5 postoperatively. Subgrouping showed slightly better results for acetabular defects (Ø 69.4) compared femoral defects (Ø 60). Based on the reported mid-term results, we consider AMIC as a valuable treatment option for larger chondral defects of the hip

Introduction. The direct anterior approach (DAA) and the posterior approach (PA) are 2 common total hip arthroplasty (THA) exposures. This prospective study quantitatively compared changes in periarticular muscle volume after DAA and PA THA. Materials. Nineteen patients undergoing THA were recruited from the practices of 3 fellowship-trained hip surgeons. Each surgeon performed a single approach, DAA or PA. Enrolled patients underwent a preoperative MRI of the affected hip and two subsequent postoperative MRIs, averaging 9.6 and 24.3 weeks after surgery. Clinical evaluations were done by Harris Hip Score at each follow-up interval. Results. MRIs for 10 DAA and 9 PA patients were analyzed. Groups did not differ significantly with regard to BMI, age, or pre-operative muscle volume. 1 DAA patient suffered a periprosthetic fracture and was excluded from the study. DAA hips showed significant atrophy in the obturator internus (−37.3%) muscle at early follow-up, with persistent atrophy of this muscle at the final follow-up. PA hips showed significant atrophy in the obturator internus (−46.8%) and externus (−16.0%), piriformis (−8.12%), and quadratus femoris muscles (−13.1%) at early follow-up, with persistent atrophy of these muscles at final follow-up. Loss of anterior capsular integrity was present at final follow-up in 2/10 DAA hips while loss of posterior capsular integrity was present in 5/9 PA hips. There was no difference in clinical outcomes. Discussion. This study provides objective evidence that, regardless of surgical approach, a muscle whose tendon is detached from its insertion is likely to demonstrate persistent atrophy 6 months following THA. Although the study was not powered to compare clinical outcomes, it should be noted that no significant difference in patient outcomes were observed. Introduction. Modular neck (MN) components in total hip arthroplasty (THA) offer intraoperative flexibility, but adverse local tissue reactions (ALTR) due to tribocorrosion at modular junctions are a potential complication of such designs. Serum ion levels and metal artifact reduction sequence (MARS) MRI are used to assess ALTR following modular THA. This study investigates serum ion levels and MARS MRI findings in a series of hips with MN components and differing articulating surfaces. Methods. We retrospectively evaluated a cohort of 184 primary THAs in 159 patients implanted with a dual modular femoral stem by one surgeon from 2005–2013. 121 THAs had a cobalt-chromium neck component and non-metal-on-metal articulation, while 63 THAs had a titanium neck component and metal-on-metal (MoM) articulation. Serum ion levels were recorded for all patients. MARS MRI scans were read by musculoskeletal-trained radiologists. Pseudotumor grade and location were measured. Results. Serum cobalt levels as a function of time post-operatively demonstrated no correlation. In THAs with non-MoM articulation, pseudotumors were visualized in 13 of 54 (24.1%) initial MARS MRIs performed 1–40 months post-operatively compared to 5 of 67 (7.5%) performed 41–120 months post-operatively (p=0.02), and findings typically remained consistent across multiple MARS MRI scans. Pseudotumors were generally located lateral to the hip joint in hips with non-MoM articulation compared to anteromedial following MoM arthroplasty. In the cobalt-chrome MN group, cobalt levels were elevated in 11 of 20 (55%) of patients with pseudotumor compared to 15 of 19 (79%) in the MoM group. Conclusion. Generation of ALTR leading to pseudotumor formation is a concern with cobalt-chrome MN designs. Psuedotumor characteristics differ between patients with cobalt-chrome designs versus MoM articulation. Normal serum ion levels did not exclude the presence of a pseudotumor and routine MARS MRI should be included in follow-up of patients with cobalt-chromium MN prostheses

Introduction. Occult hip fractures occur in 3% of cases. Delay in treatment results in significantly increased morbidity and mortality. NICE guidelines recommend cross-sectional imaging within 24 hours and surgery on the day of, or day after, admission. MRI was the standard imaging modality for suspected occult hip fractures in our institution, but since January 2013, we have switched to multi-detector CT (MDCT) scan. Our aims were to investigate whether MDCT has improved the times to diagnosis and surgery; and whether it resulted in missed hip fractures. Patients/Materials & Methods. Retrospective review of a consecutive series of patients between 01/01/2013 and 31/08/2014 who had MDCT scan for suspected occult hip fracture. Missed fracture was defined as a patient re-presenting with hip fracture within six weeks of a negative scan. A comparative group of consecutive MRI scans from 01/01/2011 to 31/12/2012 was used. Results. Seventy-three MDCTs and 70 MRIs were included. MDCT identified nine hip fractures and other fractures in 27 patients. Mean time to scan for MDCT was 13 hours 37 minutes compared with 53 hours 36 minutes for MRI scan (p<0.001). 88.5% of MDCTs were performed within 24 hours compared with 33.3% of MRIs. Nine and 16 patients required surgery in the MDCT and MRI groups, respectively. Mean time to surgery for MDCT was 50 hours 41 minutes compared with 223 hours 21 minutes for MRI scan (p = 0.25). There were no missed hip fractures in the patients with negative MDCT scan. Discussion. MDCT scan has led to a significant reduction in time to diagnosis, and a large reduction in time to surgery. MDCT did not miss any hip fractures. Conclusion. We advocate the use of MDCT over MRI in suspected occult hip fractures as it is cheaper, quicker, and more readily available; and does not result in missed hip fractures

Patients from a randomised trial on resurfacing hip arthroplasty (RHA) (n = 36, 19 males; median age 57 years, 24 to 65) comparing a conventional 28 mm metal-on-metal total hip arthroplasty (MoM THA) (n = 28, 17 males; median age 59 years, 37 to 65) and a matched control group of asymptomatic patients with a 32 mm ceramic-on-polyethylene (CoP) THA (n = 33, 18 males; median age 63 years, 38 to 71) were cross-sectionally screened with metal artefact reducing sequence-MRI (MARS-MRI) for pseudotumour formation at a median of 55 months (23 to 72) post-operatively. MRIs were scored by consensus according to three different classification systems for pseudotumour formation. . Clinical scores were available for all patients and metal ion levels for MoM bearing patients. . Periprosthetic lesions with a median volume of 16 mL (1.5 to 35.9) were diagnosed in six patients in the RHA group (17%), one in the MoM THA group (4%) and six in the CoP group (18%). The classification systems revealed no clear differences between the groups. Solid lesions (n = 3) were exclusively encountered in the RHA group. Two patients in the RHA group and one in the MoM THA group underwent a revision for pseudotumour formation. There was no statistically significant relationship between clinical scoring, metal ion levels and periprosthetic lesions in any of the groups. . Periprosthetic fluid collections are seen on MARS-MRI after conventional CoP THA and RHA and may reflect a soft-tissue collection or effusion. . Currently available MRI classification systems seem to score these collections as pseudotumours, causing an-overestimatation of the incidence of pseudotumours. . Aims

Eccentric reductions may become concentric through femoral head ‘docking’ (FHD) following closed reduction (CR) for developmental dysplasia of the hip (DDH). However, changes regarding position and morphology through FHD are not well understood. We aimed to assess these changes using serial MRI.

Methods

We reviewed 103 patients with DDH successfully treated by CR and spica casting in a single institution between January 2016 and December 2020. MRI was routinely performed immediately after CR and at the end of each cast. Using MRI, we described the labrum-acetabular cartilage complex (LACC) morphology, and measured the femoral head to triradiate cartilage distance (FTD) on the midcoronal section. A total of 13 hips with initial complete reduction (i.e. FTD < 1 mm) and ten hips with incomplete MRI follow-up were excluded. A total of 86 patients (92 hips) with a FTD > 1 mm were included in the analysis.

Introduction. This study aims to determine how the acetabular version changes during the key developmental stage of adolescence, and what contributes to this change. In addition, we examined whether patient factors (BMI, activity levels) or the femoral-sided anatomy contribute to any observed changes. Patients/Materials & Methods. This prospective longitudinal cohort study included 19 volunteers (38 healthy hips). The participants underwent clinical examination (BMI, range of movement assessment), MRIs of both hips at recruitment and at follow-up (6 ± 2 years) and HSS Paediatric Functional Activity Brief Scale (Pedi-FABS) questionnaire. MRI scans were assessed at both time points to determine change of the tri-radiate cartilage complex (TCC), the acetabular anteversion, the degree of anterior, posterior, and superior femoral head coverage by the acetabulum, and anterior and antero-superior alpha angles. We investigated if the change in anteversion and sector angles was influenced by the BMI, range of movement measurements, the Pedi-FABS or the alpha angle measurements. Results. At the baseline MRI, all hips had a Grade I (open) TCC; the TCC was Grade III (closed) by follow-up MRI in all of the hips. The acetabular anteversion increased moving caudally further away from the roof for both time-points. The mean anteversion increased from 7.4° ± 3.8 to 12.2° ± 4 (p < 0.001). The increase in version occurred universally on the acetabulum but was greatest at the rostral ¼ of the acetabulum. The change in version did not correlate with any of the patient factors tested (p = 0.1–0.6). Discussion. The native acetabulum orientation changes around adolescence, with the version significantly increasing as a result of a reduction of the femoral head coverage anteriorly. Disturbance of this process would lead to pathology contributing to pincer or retroversion FAI. Conclusion. Further study of greater power is needed to provide further insight into association between version and patient factors

Introduction. Failure of metal on metal (MOM) total hip arthroplasty (THA) and resurfacing arthroplasty (HRA) due to development of adverse local tissue reaction (ALTRs) is a significant problem. The prevalence of ALTRs in asymptomatic MOM arthroplasty patients is highly variable. The purpose of this prospective, longitudinal study was to: 1) determine MRI ALTR prevalence in patients with HRA; 2) determine if patients with HRA have a greater rate of MRI ALTRs compared to control patients with ceramic on poly (COP) THA; and 3) evaluate changes in patient reported outcomes between these implant designs. Methods. Following IRB approval with informed consent, self-reported asymptomatic primary COP and HRA patients greater than one year post arthroplasty were evaluated with 4 annual (TP1–4) MRIs using a standardized protocol and serum ion level testing. Morphologic and susceptibility reduced images were acquired for each hip and evaluated for synovial thickness, volume, capsule dehiscence and the presence of ALTR. Patient reported outcomes were evaluated by Hip Disability and Osteoarthritis Outcome Scores (HOOS). Analyses were performed to detect differences of synovial thickness and volume, and HOOS subgroups between and within bearing surfaces at each time point and over time, and to compare the time to and the risk of developing MRI ALTR. Analyses were adjusted for age, gender, and length of implantation. Results. 86 hips were evaluated at the initial time point (TP1): HRA, n=41, age=58±8 y.o., 34/7 (M/F); COP, n=45, age=65±9 y.o., 22/23 (M/F). The mean synovial volume of the subjects was similar at TP1, and the synovial volume in HRA subjects increased significantly from TP1 to TP4 (mean volume difference=9 cm3, p=0.031) while COP subjects displayed a minimal increase from TP1 to TP4 (mean volume difference =0.3 cm3, p=0.9). The synovial volume in HRA subjects tended to be larger and with greater variability than COP subjects at TP4 (HRA=15±58 cm3; COP=6 ±10 cm3, p=0.3). By TP4, MRI ALTRs developed in 13/41 HRA subjects (mean time to ALTR=1.5 yr) compared to 2/25 COP subjects (mean time to ALTR=2.0 yr), p=0.001. While risk of MRI ALTRs in HRA subjects was 9.9 times the risk in COP subjects (p=0.07), 68% of HRA subjects did not have an MRI ALTR present. Significant differences of [Co] and [Cr] in HRA subjects with MRI ALTR present were not detected at 2/4 time points and 1/4 time points, respectively, and there were no significant differences in ion levels in the COP group. The HOOS sub-scores of Pain and Activity of Daily Living scores of HRA subjects were similar to COP subjects, and HRA subjects had better Sporting Ability than COP subjects at all time points. HRA subjects had higher Symptoms at TP1 (p=0.016) but both cohorts had high scores (HRA=92.3/100; COP=87.7/100). Initially dehiscent posterior capsules of HRA subjects (9%) tended not to resolve (10/11, 91%), while dehiscent COP subjects (15%) had greater resolution (6/22, 27%). The presence of osteolysis was limited (HRA: 6/41; COP: 3/45; p=0.101). Conclusion. A greater synovial volume and higher rate of MRI ALTRs was found in HRA subjects, while the patient symptomatology via HOOS remained slightly better in HRA subjects than COP subjects, suggesting a poor association between MRI ALTRs and symptoms. The presence of posterior decompression of the posterior capsule into the bursa alone was not necessarily indicative of an ALTR, and is likely a normal postoperative finding. The inconsistent findings of the serum ion level testing further supports MRI as a non-invasive imaging modality capable of assessing peri-prosthetic soft tissue complications, and should be considered as part of the routine patient follow up to allow early detection and monitoring of ALTRs

Introduction. The prevalence of adverse reactions to metal debris around metal-on-metal (MOM) hip replacements has been reported to range from 7 to 69%. Little has been published on MRI scans with conventional total hip bearing surfaces. This study aimed to establish the prevalence of soft tissue lesions associated with metal-on-polyethylene (MOP) and ceramic-on-ceramic (COC) bearings, compared to MOM prostheses. Patients/Materials & Methods. All Metal Artefact Reduction Sequence (MARS) MRI scans for MOM THRs performed at our unit from January 2009 to present were reviewed, identifying those with contralateral primary MOP or COC THRs included on the scan. These were compared to a previously analysed cohort of 281 MOM THRs. Scans were classified using the Modified Oxford Classification as ‘Normal’, ‘Trochanteric Fluid’, ‘Effusions’ or Adverse Reactions to Debris. Results. 72 MOP and 17 COC prostheses were identified. More abnormalities were seen with scans performed within 12 months of surgery; 7 of 11 having findings consistent with post-operative seromas. These 11 were therefore excluded from further analysis. A higher proportion of normal MRIs were seen in the conventional prostheses group (78% v 34%, p<0.001). Adverse reactions to debris were significantly more common in MOM prostheses (39% v 4%, p<0.001). Effusions were more common with MOM bearings (13% v 4%, p=0.028). There was no difference in the rates of trochanteric fluid (both groups 14%). Discussion. A much higher rate of clinically significant fluid collections were seen with MOM prostheses as a result of increased toxicity of metal debris versus polyethylene or ceramic. There was little difference between MOP and COC scans. Trochanteric fluid collections are more likely to represent bursitis than any problem with the bearing surface. Conclusion. These results provide a useful baseline for comparison of MOM MARS MRI scans, adding to our understanding of the unique failure patterns of MOM prostheses

Objectives. The purpose of this study was to investigate whether the femoral head–neck contour, characterised by the alpha angle, varies with the stage of physeal maturation using MRI evaluation of an asymptomatic paediatric population. . Methods. Paediatric volunteers with asymptomatic hips were recruited to undergo MRI of both hips. Femoral head physes were graded from 1 (completely open) to 6 (completely fused). The femoral head–neck contour was evaluated using the alpha angle, measured at the 3:00 (anterior) and 1:30 (anterosuperior) positions and correlated with physeal grade, with gender sub-analysis performed. Results. A total of 43 asymptomatic paediatric volunteers (26 male, 17 female) with mean age 13.0 years (eight to 18) were included with review of bilateral hip MRIs. Correlation between the physeal grade and alpha angle was moderate in males at both the 3:00 (r = 0.477, p < 0.001) and 1:30 (r = 0.509, p < 0.001) positions, whereas there was no significant correlation in females. A significant difference was found between the alpha angles of all the physeal grades (3:00, p = 0.030, 1:30, p = 0.005), but only in males, with the angle increasing with higher grades. For physeal grading, the inter-reader reliability was substantial (intraclass correlation coefficient (ICC) = 0.694), and the intra-reader reliability was also substantial (ICC = 0.788). . Conclusion . The femoral head–neck contour varies and correlates with the stage of physeal development, but only in males, with the alpha angle increasing with progressive physeal maturation. This suggests that gender differences exist in the natural physiological growth, development or remodelling of femoral head–neck junction. In males, pre-physeal fusion may be a critical period of vulnerability for development of morphologic abnormalities of the femoral head–neck junction. Cite this article: Bone Joint Res 2015;4:17–22

Aims

Rotational acetabular osteotomy (RAO) has been reported to be effective in improving symptoms and preventing osteoarthritis (OA) progression in patients with mild to severe develomental dysplasia of the hip (DDH). However, some patients develop secondary OA even when the preoperative joint space is normal; determining who will progress to OA is difficult. We evaluated whether the preoperative cartilage condition may predict OA progression following surgery using T2 mapping MRI.

Methods

We reviewed 61 hips with early-stage OA in 61 patients who underwent RAO for DDH. They underwent preoperative and five-year postoperative radiological analysis of the hip. Those with a joint space narrowing of more than 1 mm were considered to have 'OA progression'. Preoperative assessment of articular cartilage was also performed using 3T MRI with the T2 mapping technique. The region of interest was defined as the weightbearing portion of the acetabulum and femoral head.

Objectives

Using a simple classification method, we aimed to estimate the collapse rate due to osteonecrosis of the femoral head (ONFH) in order to develop treatment guidelines for joint-preserving surgeries.

Methods

We retrospectively analyzed 505 hips from 310 patients (141 men, 169 women; mean age 45.5 years (sd 14.9; 15 to 86)) diagnosed with ONFH and classified them using the Japanese Investigation Committee (JIC) classification. The JIC system includes four visualized types based on the location and size of osteonecrotic lesions on weightbearing surfaces (types A, B, C1, and C2) and the stage of ONFH. The collapse rate due to ONFH was calculated using Kaplan–Meier survival analysis, with radiological collapse/arthroplasty as endpoints.

Aims

We investigated whether blood metal ion levels could effectively identify patients with bilateral Birmingham Hip Resurfacing (BHR) implants who have adverse reactions to metal debris (ARMD).

Patients and Methods

Metal ion levels in whole blood were measured in 185 patients with bilateral BHRs. Patients were divided into those with ARMD who either had undergone a revision for ARMD or had ARMD on imaging (n = 30), and those without ARMD (n = 155). Receiver operating characteristic analysis was used to determine the optimal thresholds of blood metal ion levels for identifying patients with ARMD.

Cartilage defects of the hip cause significant pain and may lead to arthritic changes that necessitate hip replacement. We propose the use of fresh osteochondral allografts as an option for the treatment of such defects in young patients. Here we present the results of fresh osteochondral allografts for cartilage defects in 17 patients in a prospective study. The underlying diagnoses for the cartilage defects were osteochondritis dissecans in eight and avascular necrosis in six. Two had Legg-Calve-Perthes and one a femoral head fracture. Pre-operatively, an MRI was used to determine the size of the cartilage defect and the femoral head diameter. All patients underwent surgical hip dislocation with a trochanteric slide osteotomy for placement of the allograft. The mean age at surgery was 25.9 years (17 to 44) and mean follow-up was 41.6 months (3 to 74). The mean Harris hip score was significantly better after surgery (p < 0.01) and 13 patients had fair to good outcomes. One patient required a repeat allograft, one patient underwent hip replacement and two patients are awaiting hip replacement. Fresh osteochondral allograft is a reasonable treatment option for hip cartilage defects in young patients.

The aim of this study was to establish the natural course of unrevised asymptomatic pseudotumours after metal-on-metal (MoM) hip resurfacing during a six- to 12-month follow-up period. We used repeated metal artefact reduction sequence (MARS)-magnetic resonance imaging (MRI), serum metal ion analysis and clinical examination to study 14 unrevised hips (mean patient age 52.7 years, 46 to 68, 5 female, 7 male) with a pseudotumour and 23 hips (mean patient age 52.8 years, 38 to 69, 7 female, 16 male) without a pseudotumour. The mean post-operative time to the first MARS-MRI scan was 4.3 years (2.2 to 8.3), and mean time between the first and second MARS-MRI scan was eight months (6 to 12). At the second MRI scan, the grade of severity of the pseudotumour had not changed in 35 hips. One new pseudotumour (Anderson C2 score, moderate) was observed, and one pseudotumour was downgraded from C2 (moderate) to C1 (mild). In general, the characteristics of the pseudotumours hardly changed.

Repeated MARS-MRI scans within one year in patients with asymptomatic pseudotumours after MoM hip resurfacing showed little or no variation. In 23 patients without pseudotumour, one new asymptomatic pseudotumour was detected.

This is the first longitudinal study on the natural history of pseudotumours using MARS-MRI scans in hip resurfacing, and mirrors recent results for 28 mm diameter MoM total hip replacement.

The direct anterior approach in total hip replacement anatomically offers the chance to minimise soft-tissue trauma because an intermuscular and internervous plane is explored. This motivated us to abandon our previously used transgluteal approach and to adopt the direct anterior approach for total hip replacement. Using MRI, we performed a retrospective comparative study of the direct anterior approach with the transgluteal approach. There were 25 patients in each group. At one year post-operatively all the patients underwent MRI of their replaced hips. A radiologist graded the changes in the soft-tissue signals in the abductor muscles. The groups were similar in terms of age, gender, body mass index, complexity of the reconstruction and absence of symptoms.

Detachment of the abductor insertion, partial tears and tendonitis of gluteus medius and minimus, the presence of peri-trochanteric bursal fluid and fatty atrophy of gluteus medius and minimus were significantly less pronounced and less frequent when the direct anterior approach was used. There was no significant difference in the findings regarding tensor fascia lata between the two approaches.

We conclude that use of the direct anterior approach results in a better soft-tissue response as assessed by MRI after total hip replacement. However, the impact on outcome needs to be evaluated further.
