The reintroduction of hip resurfacing has raised the possibility of whether it could offer a better outcome in sporting and work activity after surgery compared to total hip replacement. Questionnaires were analysed from 125 total hip replacement (THR) and 108 hip resurfacing (HR) patients regarding participation in sport in the year after their surgery and returning to work they were employed at prior to their surgery. The two groups had similar average age (61 vs 60) and pre-operative Oxford hip scores (41 vs 38). Seventy-one of one-hundred and twenty-five THR patients compared to 76/108 HR patients participated in sporting activity in the year after their surgery. When including only patients that played sport before their surgery 54/75 THR and 66/83 HR patients returned to same sporting activity level after their surgery. Of the patients that worked before surgery 35/44 THR patients compared to 70/74 HR patients returned work to after their surgery. There is significant difference quantity of patients participating in sporting activity after HR and THR (fishers exact test, p value=0.04). However there is no significant difference in quantity of patients returning to sporting activity after HR or THR including only patients that played sport before their surgery (fishers exact test, p value=0.35). There is a significant difference in the quantity of patients returning to work activity after HR and THR (fishers exact test, p value= 0.02). HR has a better outcome in patients participating in sport after surgery compared to THR however there is no benefit for patients that are active pre-operatively. More patients return to work after HR than THR.

Background. Hip resurfacing arthroplasty (HRA) and total hip arthroplasty (THA) are treatments of end-stage hip disease. Gait analysis studies comparing HRA and THA have demonstrated HRA results in a more normal gait than THA. The reasons may include the larger, more anatomic head diameter, the preservation of the femoral neck with restoration of the anatomical hip centre position and normal proprioception. Total Hip Arthroplasty (THA) is a well-established, cost-effective treatment for improving function and alleviating pain in patients who have disabling hip disease with excellent long-term results. Based on the excellent results, there is an ongoing trend for THA to be performed in younger and more active patients, having higher physical demands on their new total joints. Polyethylene (PE) wear and its biological consequences are one of the main causes of implant failure in THA. Macrophages phagocytise PE wear particles and this will result in osteolysis and loss of periprosthetic bone. The risk of these complications can be estimated in relation to the amount of volumetric wear based on two assumptions: that the number of PE particles dispersed in the peri-prosthetic tissues is controlled by the amount of PE wear; and that the development of osteolysis and the resulting aseptic loosening is triggered by these PE particles. Based on these assumptions, a model was developed to estimate the osteolysis-free life of a THA, depending on the Linear Wear Rate (LWR) and femoral head size of the PE bearing. A review of the literature was conducted to provide an estimate of the radiologic osteolysis threshold based on the volumetric wear of the PE bearing. This review demonstrates that this radiologic osteolysis threshold is approximated 670 mm3 for conventional PE. The osteolysis-free life of the THA was estimated by simply dividing this threshold volume by the annual Volumetric Wear Rate (VWR) of the bearing. The annual VWR is basically controlled by two parameters: (1) annual LWR and (2) head size, and was calculated by using published formulae. For 28 mm heads, following osteolysis-free life was determined in function of the annual LWR. LWR: 10 µm/y => 116.6 years / LWR: 25 µm/y => 46.6 years / LWR: 50 µm/y => 23.3 years / LWR: 100 µm/y => 11.6 years. For 40 mm heads, following osteolysis-free life was determined in function of the annual LWR. LWR: 10 µm/y => 57.1 years / LWR: 25 µm/y => 22.9 years / LWR: 50 µm/y => 11.4 years / LWR: 100 µm/y => 5.7 years. This study reports the mid-term results of a large bearing hybrid metal on metal total hip replacement (MOMHTHR) in 199 hips (185 patients) with mean follow up of 62 months. Clinical, radiological outcome, metal ion levels and retrieval analysis were performed. Seventeen patients (8.6%) had undergone revision, and a further fourteen are awaiting surgery (defined in combination as failures). Twenty one (68%) failures were females. All revisions and ten (71%) of those awaiting revision were symptomatic. Twenty four failures (86%) showed progressive radiological changes. Fourteen revision cases showed evidence of adverse reactions to metal debris (ARMD). The failure cohort had significantly higher whole blood cobalt ion levels (p=0.001), but no significant difference in cup size (p=0.77), inclination (p=0.38) or cup version (p=0.12) in comparison to the non revised cohort. Female gender was associated with an increased risk of failure (chi squared p=0.04). Multifactorial analysis demonstrated isolated raised Co levels in the absence of either symptoms or XR changes was not predictive of failure (p=0.675). However both the presence of pain (p<0.001) and XR changes (p<0.001) in isolation were both significant predictors of failure. Wear analysis (n=5) demonstrated increased wear at the trunnion/head interface (mean out of roundness measurements of 34.5 microns +/−13.3 (+/−2SD, normal range 8-10 microns) with normal levels of wear at the articulating surfaces. There was evidence of corrosion at the proximal and distal stem surfaces. The cumulative survival rate, with revision for any reason was 92.4% (95%CI: 87.4-95.4) at 5 years. Including those awaiting surgery, the revision rate would be 15.1% with cumulative survival at 5 years of 89.6% (95% CI: 83.9-93.4). This MOMHTHR series has demonstrated unacceptable high failure rates with evidence of high wear at the head/trunnion interface and passive corrosion to the stem surface. Second-generation metal-on-metal (MoM) articulations in total hip arthroplasty (THA) were introduced in order to reduce wear-related complications. The current study reports on the serum cobalt levels and the clinical outcome at a minimum of 20 years following THA with a MoM (Metasul) or a ceramic-on-polyethylene (CoP) bearing. The present study provides an update of a previously published prospective randomized controlled study, evaluating the serum cobalt levels of a consecutive cohort of 100 patients following THA with a MoM or a CoP articulation. A total of 31 patients were available for clinical and radiological follow-up examination. After exclusion of 11 patients because of other cobalt-containing implants, 20 patients (MoM (n = 11); CoP (n = 9)) with a mean age of 69 years (42 to 97) were analyzed.
