Aims. Reimers migration percentage (MP) is a key measure to inform decision-making around the management of hip displacement in cerebral palsy (CP). The aim of this study is to assess validity and inter- and intra-rater reliability of a novel method of measuring MP using a smart phone app (HipScreen (HS) app). Methods. A total of 20 pelvis radiographs (40 hips) were used to measure MP by using the HS app. Measurements were performed by five different members of the multidisciplinary team, with varying levels of expertise in MP measurement. The same measurements were repeated two weeks later. A senior orthopaedic surgeon measured the MP on picture archiving and communication system (PACS) as the gold standard and repeated the measurements using HS app. Pearson’s correlation coefficient (r) was used to compare PACS measurements and all HS app measurements and assess validity. Intraclass correlation coefficient (ICC) was used to assess intra- and inter-rater reliability. Results. All HS app measurements (from 5 raters at week 0 and week 2 and PACS rater) showed highly significant correlation with the PACS measurements (p < 0.001). Pearson’s correlation coefficient (r) was constantly over 0.9, suggesting high validity. Correlation of all HS app measures from different raters to each other was significant with r > 0.874 and p < 0.001, which also confirms high validity. Both inter- and intra-rater reliability were excellent with ICC > 0.9. In a 95% confidence interval for repeated measurements, the deviation of each specific measurement was less than 4% MP for single measurer and 5% for different measurers. Conclusion. Fixation techniques used in the treatment of slipped capital femoral epiphysis (SCFE) that allow continued growth of the femoral neck, rather than inducing epiphyseal fusion in situ, have the advantage of allowing remodelling of the deformity. The aims of this study were threefold: to assess whether the Free-Gliding (FG) SCFE screw prevents further slip; to establish whether, in practice, it enables lengthening and gliding; and to determine whether the age of the patient influences the extent of glide. Methods. All patients with SCFE who underwent fixation using FG SCFE screws after its introduction at our institution, with minimum three years’ follow-up, were reviewed retrospectively as part of ongoing governance. All pre- and postoperative radiographs were evaluated. The demographics of the patients, the grade of slip, the extent of lengthening of the barrel of the screw and the restoration of Klein’s line were recorded. Most centres in the United Kingdom adopt a selective
screening programme for developmental dysplasia of the hip (DDH)
based on repeated clinical examination and selective ultrasound
examination. The Newborn Infant Physical Examination protocol implemented
in 2008 recommends a first examination at birth and then a second
and final examination at six to ten weeks of age. Due to concerns
over an increase in late presentations we performed a retrospective
review of our 15-year results to establish if late presentation
increases treatment requirements. Of children presenting before
six weeks of age, 84% were treated successfully with abduction bracing,
whereas 86% of children presenting after ten months eventually required
open reduction surgery. This equates to a 12-fold increase in relative
risk of requiring open reduction following late presentation. Increasing
age at presentation was associated with an increase in the number
of surgical procedures, which are inevitably more extensive and complex,
with a consequent increased in cost per patient. The implementation
of an opportunistic examination at three to five months could help
to reduce the unintended consequences of the Newborn Infant Physical Examination
