Introduction. Genu valgum is a common presentation in paediatric patients with congenital limb deformities. The aim of this study is to assess the outcome of guided growth surgery in paediatric patients referred via our physiotherapy pathway with isolated genu valgum and associated patellar instability. Materials & Methods. Patients were identified from our prospective patellar instability database. Inclusion criteria was acquired or congenital genu valgum associated with patellar instability in skeletally immature patients. The mechanical lateral-distal femoral angle was assessed on long leg alignment radiographs (mLDFA <85 degrees). Surgical treatment was the placement of a guided growth plate (PediPlate, OrthoPediatrics, USA) on the medial distal femoral physis (hemi-epiphysiodesis). KOOS-child scores were collected pre-operatively and post-operatively (minimum at 6 months). Results. Eleven patients (seven female) with mean age of 12(range 5–15) were identified. Five patients had congenital talipes equinovarus(CTEV), one fibular hemimelia, one di-George syndrome, one septic growth arrest and three had idiopathic genu valgum. Pre- and post-operative KOOS-child scores showed overall improvement: 58(range 36–68) to 88(65–99) and knee symptoms subscores: 64(43–71) to 96(68–100) p<0.01, t-test. Mean follow-up was 10 months (range 3–23). No subsequent dislocations/subluxations occurred during follow-up. Conclusions. Guided growth surgery is an effective way of treating symptomatic patellar instability in skeletally immature patients with genu valgum in the absence of other structural pathology. It was most common in our cohort in patients with unilateral CTEV. We would recommend to screen syndromic and congenital limb deformity patients for patellar instability symptoms in the presence of genu valgum

We report the results of medial physeal stapling in 16 knees with primary genu valgum and 27 with secondary genu valgum. In the primary group, stapling was undertaken at a mean chronological age of 12 years in girls and 13 years in boys. The medial femoral physis was stapled in ten knees and the medial femoral and tibial physes in six knees. At skeletal maturity, all patients had excellent or good leg alignment. Secondary genu valgum is due to skeletal dysplasia, haematological or endocrine disorders, or to juvenile chronic arthritis. Stapling was at a mean chronological age of 11 years in girls and 14 years in boys. The medial femoral physis was stapled in 13 knees, the medial tibial physis in three and both in 11 knees. At skeletal maturity, 85% had excellent or good leg alignment, and correction had occurred within one year. Two of the poor results were due to staple extrusion from osteoporotic bone, and two to overcorrection. Rebound growth was minimal and unpredictable after the removal of staples. Medial physeal stapling is a suitable method of treatment for both primary and secondary genu valgum in late childhood and in adolescence. At least one year of knee growth is required to achieve correction, and care is needed to avoid overcorrection of the secondary genu valgum

Forty-nine patients treated by epiphysial stapling for idiopathic genu valgum are reviewed. The children were aged from eleven to fourteen and a half years. The severity of the condition and subsequent correction were assessed by measuring the inter-malleolar distance. Results show that femoral stapling is the treatment of choice for most patients with inter-malleolar separation of up to 12-5 centimetres. The optimum age for stapling for both boys and girls is eleven and a half years. Stapling of both epiphyses is advised for those patients presenting late, for example, between thirteen and fourteen and a half years, or where the deformity is greater than 12-5 centimetres of separation. Correction rarely takes more than one year, and was cosmetically satisfactory in all cases. The operation is virtually free from complication except for a tendency for the scars to be broad and conspicuous. Stapling has proved to be a safe, effective and predicatable operation for idiopathic genu valgum

Osteotomies for valgus deformity are much less frequent than those for varus deformity as evidenced by published series which are, on one hand, less numerous and on the other hand, based on far fewer cases. For genu varum deformity, it has been proved that navigation allows to reach easier the preoperative correction goal. Our hypothesis was that navigation for genu valgum could be as accurate as for genu varum deformity. The aim of this paper was to present the mid-term results of 29 computer-assisted osteotomies for genu valgum deformity performed between September 2001 and March 2013. The series was composed of 27 patients (29 knees), 20 females and 7 males, aged from 15 to 63 years (mean age: 42.4+/−14.3 years). The preoperative functional status was evaluated according to the Lyshölm-Tegner score. The mean score was of 64+/−20.5 points (18–100). The stages of osteoarthritis were evaluated according to modified Ahlbäck's criteria. We operated on 12 stage 1, 9 stage 2, 5 stage 3 and 1 stage 4. 2 female patients had no osteoarthritis but a particularly unesthetic deformity (of which one was related to an overcorrected tibial osteotomy). The pre and postoperative HKA angle was measured according to Ramadier's protocol. We measured also the medial tibial mechanical angle (MTMA) and the medial femoral mechanical angle (MFMA). The mean preoperative HKA angle was 189.3°+/−3.9° (181° to 198°); the mean MFMA was 97.2° +/− 2.6° (93° to 105°) and the mean MTMA was 90.1° +/− 2.8° (86° to 95°). The goal of the osteotomies was to obtain an HKA angle of 179° +/− 2° and a MTMA of 90°+/2° in order to avoid an oblique joint line. We performed 24 femoral osteotomies (14 medial opening wedge and 10 lateral closing wedge) and 5 double osteotomies (medial tibial closing wedge + lateral opening wedge osteotomy). The functional results were evaluated according to Lyshölm-Tegner, IKS and KOO Scores, which were obtained after revision or telephone call. We did not find any complication except a transient paralysis of the common fibular nerve. 23 patients (4 lost to follow-up) were reviewed at a mean follow-up of 50.9+/−38.8 months (6–144). The mean Lyshölm-Tegner score was 92.9+/−4 points (86–100), the mean KOO score 89.7+/−9.3 (68–100), the mean IKS ≪knee≫ score 88.7 +/−11.4 points (60 à 100) and the mean ≪function≫ score 90.6 +/−13.3 points (55–100). 22 of the 23 reviewed patients (25 knees) were very satisfied or satisfied of the result. Regarding the radiological results, the mean HKA angle was of 180.1°+/−1.9° (176° to 185°), the mean MFMA of 90.7°+/−2.5° (86°-95°) and the mean MTMA of 89.1°+/−1.9° (86°-92°). The preoperative goal was reached in 86.2% (25/29) of the cases for HKA angle and in 100% of the cases for MTMA when performing double level osteotomy (5 cases). At this follow-up, no patient was revised to TKA. Computer-assisted osteotomies for genu valgum deformity lead to excellent results a mid-term follow-up. Navigation is very useful to reach the preoperative goal

We evaluated the use of percutaneous screw epiphysiodesis to treat genu valgum deformity in adolescents, and the possibilities of extending its use to younger patients with different causes of angular deformities or leg length discrepancies. To date, the surgical options for adolescent idiopathic genu valgum have been medial physeal retardation by stapling, growth arrest by epiphysiodesis of the distal femur and/or tibia, or osteotomy. From September 1999, we prospectively studied 16 patients, 11 of whom had angular knee deformities (20 legs) and five limb length inequality. From a preoperative mean of 12.25( the tibiofemoral angle reduced to 6.4° at the latest assessment. Percutaneous epiphysiodesis using transphyseal screws proved to be a reliable method with few complications and the advantages of simplicity, short operating times, rapid postoperative rehabilitation and reversibility

The purpose of this study was to review the outcomes of four children with genu valgum secondary to Hurler Syndrome treated with circular external fixators (frames) for angular correction. We retrospectively reviewed the medical and radiographic records of four children with Hurler Syndrome and genu valgum treated with frames. Three children had simultaneous bilateral tibial corrections. The fourth child had unilateral femoral correction. The mean age of the children was 14 years at application of frame. Mean duration of frame was 113 days for the tibial frames, and the femoral frame remained in-situ for 150 days. Correction was assessed clinically, and radiologically with x-rays and CT scannograms, with excellent results in all four cases. The complexities of each individual case necessitated specific and individualised treatment for each child. Complications included further deformities arising in treated and un-treated long-bones both during and after application of frame. Prior to the introduction of bone marrow transplantation, the average life expectancy of children with Hurler Syndrome was seven years. With bone marrow transplantation, affected children are now living much longer, and many develop characteristic long bone deformities in their lower limbs. These deformities are progressive and can be multifocal and polyostotic. Managament can be extremely challenging, and prior reports of management with hemiepiphysiodesis with staples and 8-plates have been mixed. We believe that this is the first series of circular frame lower limb reconstruction in children with Hurler Syndrome. The flexibility and adaptability of frames confers a unique advantage in the management of these complex deformities

Purpose of the study: TKA on genu valgum raises serious problems for the ligament balance. Excessive release of the lateral retracted ligaments exposes the knee to potential instability in the frontal plane. To resolve this problem and avoid implantation of a constrained TKA, we opted for osteotomy of the lateral condyle removing the insertion of the lateral collateral ligament and the popliteal muscle after release of the fascia lata. The purpose of our study was to evaluate the functional and radiographic outcomes of these patients. Material and methods: This was a retrospective study from 2002 to 2006. All patients with degenerative joint disease of the knee with severe and/or fixed genu valgum were included. These patients were implanted with a navigated posterostabilised Wallaby TKA (Navitrack) associated with osteotomy of the lateral condyle fixed with screws after acquisition of the ligament balance. The diagnosis and surgical history were noted. The preoperative alignement was determined on the full limb x-ray and from navigation data. The following variables were reviewed: polyethylene height, lowering of the lateral condyle, blood loss, operative time. The postoperative alignment was established at least one year after surgery. Intraoperative, postoperative and late complications were noted. The Knee Society function scores were used. Results: Fifteen patients, mean age 70 years were reviewed at mean 35 months. The mean duration of the operative time was 136 min with mean blood loss of 620 ml. The mean PE height was 13 mm. All operated knees were corrected with mean alignment improving from 17.71 to 1.5 valgus postoperatively. The function score improved from 35 preoperatively to 79 at last follow-up. There were no cases of patellar instability or secondary laxity. Two patients developed late reflex dystrophy. The only case of revision concerned one non-union of the lateral condyle (screw removed at four months) but had a function score of 85 at last follow-up. Discussion: Performing an osteotomy of the lateral condyle in complement with the navigated posterostabilised TKA for fixed genu valgum enabled good relaxation and satisfactory functional results so that totally constrained implants can be avoided

Introduction: Lateral insufficiency fractures following total hip replacements have been reported with the femoral stems positioned in varus, together with osteopenia of the lateral femoral cortex. Any abnormal alignment of the lower limbs, such as genu valgum, will alter the load distribution across the femoral cortices, and repetitive loading during walking will predispose the bones to stress fractures at any stress riser point, such as the tip of a femoral component. Bilateral femoral stress fractures post total hip replacements have not been previously described. Materials and Methods: We present a 55 yr old lady, diagnosed with juvenile idiopathic arthritis aged 5 years, and had undergone bilateral total hip replacements at the age of 29 and 30 years and bilateral knee replacements aged 37 and 42 years. The right hip required revision of the cup 15 years later. The knees were in valgus and the left knee was extremely stiff flexing to just 5 degrees. She presented to us as an emergency with bilateral thigh pain with plain radiographs confirming bilateral peri-prosthetic fractures of the femur at the tip of well fixed femoral components. There had been no history of injury and her hips were functioning well up to this time. Results: The patient required revision of both hips to long stem un-cemented components, bypassing the fractures, and revision of both knees to stemed seni-constrained implants, thereby correcting the alignment of both lower limbs. Both fractures healed and the patient is currently pain free and mobile with walking aids. Discussion: Surgeons must remain aware that when implants are in situ, abnormal alignments will lead to abnormal forces, and stress fractures are likely to occur at any stress riser around the implant. Avoiding mal-alignment will avoid this complication. Bilateral peri-prosthetic stress fractures following total hip replacements have not been previously reported

A total of 25 children (37 legs and 51 segments) with coronal plane deformities around the knee were treated with the extraperiosteal application of a flexible two-hole plate and screws. The mean age was 11.6 years (5.5 to 14.9), the median angle of deformity treated was 8.3° and mean time for correction was 16.1 months (7 to 37.3). There was a mean rate of correction of 0.7° per month in the femur (0.3° to 1.5°), 0.5° per month in the tibia (0.1° to 0.9°) and 1.2° per month (0.1° to 2.2°) if femur and tibia were treated concurrently. Correction was faster if the child was under 10 years of age (p = 0.05). The patients were reviewed between six and 32 months after plate removal. One child had a rebound deformity but no permanent physeal tethers were encountered.

The guided growth technique, as performed using a flexible titanium plate, is simple and safe for treating periarticular deformities of the leg.

Varus osteotomy of the distal femur is recommended for osteoarthritis of the knee with significant valgus deformity, but the operation is difficult to plan and perform. A simple technique involving a jig referenced to the line of the tibia is described. This allows accurate overcorrection by a few degrees, with impaction and secure fixation at the osteotomy. It has been successful, with no complications, in 12 consecutive knees.

Osteoarticular infections in paediatric population are primarily hematogenous in origin, although cases secondary to penetrating trauma, surgery or contiguous site are also reported.

Despite being rare, numerous studies report infection relapse rates around 5 %. Osteomyelitis complications in children include septic arthritis, osteonecrosis of the bone segment, impaired growth.

7 years old male patient presented with history of traffic injury in January 2004. He sustained closed diaphyseal fracture of the right femur initially treated by elastic osteosynthesis.

Four years after traffic injury he was diagnosed at our Institution of chronic femoral Osteomyelitis with positive cultures for methicillin sensible Staphylococcus aureus, requiring multiple surgical debridements and systemic antibiotic therapy.

Five years follow- up the patient developed valgus deformity of his right knee (mechanical axis 11° genu valgum) with limb length discrepancy of 15 mm, intermalleolar distance of 15 cm and bone edema in external compartment of the knee (MRI). At this time the patient did not present any recurrence of septic process with normalization of laboratory parameters (ESR and CRP) and clinically asymptomatic.

In February 2014, at the end of growth, a distal femoral varus osteotomy was used to treat valgus knee malalignment. Medial closing wedge osteotomy was performed satisfactorily using Tomofix® Osteotomy System (DePuySynthes).

18 months follow- up after varus osteotomy the patient progressed satisfactorily without pain and a normal function of his right knee. Correction limb length discrepancy was achieved (5 mm) with a normal alignment of his right limb (mechanical axis 3° genu valgum).

Although Osteomyelitis is not very frequent in children population, its treatment requires not only prolonged antibiotic therapy but also multiple surgical debridements.

We recommend monitoring over a long period of time children affected with Osteomyelitis in order to prevent and treat correctly impaired growth.

Aims. Our aim was to investigate the predictive factors for the development of a rebound phenomenon after temporary hemiepiphysiodesis in children with genu valgum. Patients and Methods. We studied 37 limbs with idiopathic genu valgum who were treated with hemiepiphyseal stapling, and with more than six months remaining growth at removal of the staples. All children were followed until skeletal maturity or for more than two years after removal of the staples. Results. On multivariate logistic regression analysis, the rate of correction, body mass index (BMI), age, and initial valgus angle were significantly associated with a rebound phenomenon. With those characteristics, a predictive model for rebound was generated using recursive partitioning analysis. Children with a rapid rate of correction had the most frequent and severe rebound phenomenon (incidence 79%, mean 4°), whereas those with a slow rate of correction had less rebound when they had low BMI (43%, 2°) and none when the BMI was ≥ 21 kg/m. 2. . Conclusion. This is the first study to evaluate a predictive model for a rebound phenomenon after temporary hemiepiphysiodesis in children with idiopathic genu valgum. Patellofemoral instability (PFI) is a common cause of knee pain and disability in the paediatric population. Patella alta, lateralised tibial tubercle, medial patellofemoral ligament (MPFL) deficiency, genu valgum and trochlear dysplasia are well known risk factors. A prospective database was created including patients referred through our physiotherapy pathway following first-time patella dislocation. Patella alta and lateralisation of the tibial tuberosity was treated with a Fulkerson-type tibial tubercle osteotomy(TTO). Medial patellofemoral ligament was reconstructed using quads tendon autograft pull-down technique. A modified Sheffield protocol was used postoperatively allowing weightbearing in a hinged knee brace. Forty patients were identified with 8 patients having bilateral presentations. Male to female ratio was 12:28 with an age range of 4–17 years. Eight patients had congenital PFI, five patients acquired PFI through traumatic patella dislocation and twenty-seven patients developed PFI from recurrent dislocations. Structural abnormalities were found in 38(95%) of patients. Patella alta (Caton-Deschamps index >1.2) was identified in 19(47%) patients, genu valgum in 12(30%) patients, increased tibial tubercle-trochlear groove distance(TT-TG>20mm) was present in 9(22.5%) patients and persistent femoral anteversion(> 20 deg) in 7(17%) patients. Eight patients were treated with TTO and MPFL reconstruction, three patients with MPFL reconstruction alone and five patients had guided growth for genu valgum correction. Ten patients are awaiting surgery. No postoperative patients had recurrence of PFI at their latest follow up. PFI is a common problem in the adolescent paediatric population with identifiable structural abnormalities. Correcting structural pathology with surgery leads to predictable and safe outcomes

Purpose: Over recent years, several authors have estimated that the distal femur presents an epiphyseal torsion which can be measured intraoperatively or on the preoperative scan. This measurement does not however take into account the dynamic mechanical axis, particularly the mechanical axis at 90° flexion when walking. We used a computer-assisted navigation system (Ortho-pilot®) to attempt to measure femoral rotation by dynamic gonometry in extension and 90° flexion before and after implantation of a total knee arthroplasty. Material and methods: We recorded the preoperative (Rx) and intraoperative (Orthopilot) HKA in extension and in 90° flexion before and after implantation of 50 total knee prostheses (Search®, Aesculup, AG, Tuttlingen) and again postoperatively (Rx). The series included 19 knees with genu valgum (mean HKA 187.36±5.4°, range 181°–203°), 30 knees with genu varum (HKA 169.2±4.11, range 160°–176°), and one normal axis knee. Results: The radiographic values obtained preoperatively were confirmed by Orthopilot, respectively 186.68±5.25° and 169.76±3.84° in extension. At 90° flexion, HKA was 178.63±5.7° before implantation for genu valgum giving a significant varus due to lateral opening during flexion,and 171.6±4.15° for genu varum, showing persistence of varus. After implantation of the total knee prosthesis, the values were as follows. For the genu varum: HKA in extension 180.57±0.82° and HKA in 90° flexion 176.86±2.55° giving a mean residual varus of 3.16±2.86° (from 4–8° varus) without external rotation of the femoral implant. For genu valgum, HKA in extension was 179.60±0.92° and HKA in 90° flexion was 176.1±3.23°, giving a mean residual varus of 3.26±2.86° (0–10° varus), recalling that in the event of genu valgum we impose external rotation due to the frequent hypoplasia of the lateral condyle. Discussion: Orthopilot-assisted implantation of total knee prostheses provides new information concerning dynamic gonometry, particularly the varus or valgus in flexion, which corresponds to measuring natural external or internal rotation. Measuring epiphyseal torsion of the distal femur with classical methods does not take into account the global rotation of the femur which is often an external rotation (up to 8° for genu varum). Systematic implantation of the femoral component in external rotation raises the risk of increasing considerably the varus forced to the implant during flexion

Navigation of Uni knee arthroplasty (UKA) is not common. Usually the software includes navigation of the tibial as well as the femoral implant. In order to simplify the surgical procedure we thought that navigation of the tibial plateau alone could be a good option. Since 2005 we have been using a mobile bearing UKA of which the ancillary is based on dependent bone cuts. The tibial cut is made first and the femoral cut is automatically performed using cutting blocks inserted between the tibial cut and the distal end of the femur. Although we are satisfied with this procedure, it is not rare we have some difficulties getting the right under correction needed to get a good long-term result. The aim of this paper was to present our computer-assisted UKA technique and our preliminary radiological results in genu varum (17 cases) as well as genu valgum (6 cases) deformities. The series was composed of 23 patients, 10 females and 13 males, aged from 63 to 88 years old (mean age: 75 +/− 8). The mean preoperative HKA (Hip-Knee-Ankle) angle was: 172.35° +/− 2.31° (167° to 176°) for the genu vara and 186.33° +/− 2.87° (182° to 189°) for the genu valga. The goal of the navigation was to get an HKA angle of 177° +/− 2° for genu varum deformity and 183° +/− 2° for genu valgum. We used the SURGETICS® device (PRAXIM, GRENOBLE, FRANCE) in the first six cases and the ORTHOPILOT® device (B-BRAUN-AESCULAP, TUTTLINGEN, GERMANY) in the other cases. The principles are the same for both devices. The 1rst step consists in inserting percutaneously the rigid-bodies on the distal end of the femur and on the proximal end of the tibia. Then, we locate the center of the hip by a movement of circumduction, the center of the ankle by palpating the malleoli and the center of the knee by palpating intra articular anatomic landmarks to get the HKA angle in real time. This step is probably the most important because it allows checking the reducibility of the deformity in order to avoid an over correction when inserting a mobile bearing prosthesis. The 3. rd. step consists in navigation of the tibial cut such as the height of the resection, the tibial slope (3 to 5° posterior tibial slope) and the varus of the implant (2 to 3°). Once the tibial cut was done, we must use the conventional ancillary to perform the femoral bone cuts (distal and chamfer). The last step consists in inserting the trial implants and checking the HKA angle and the laxity of the medial or lateral side. We used postoperative long leg X-Rays to evaluate the accuracy of navigation and plain radiographs to evaluate the right position of the implant. As far as genu varum deformity was concerned, the mean postoperative HKA angle was 177.23° +/− 1.64° (173°–179°). The preoperative goal was reached in 94% of the cases. Moreover, this angle could be superimposed on the peroperative computer-assisted angle, which was 177° +/− 1.43° (p>0.05). For genu valgum, the mean postoperative HKA angle was 181° +/− 1.41° (179°–183°). The preoperative goal was reached in 66% of the cases but the series is too short to give any conclusion. The navigation of tibial plateau alone can be used with accuracy, provided one has the right ancillary to use dependent bone cuts. The procedure is quick and needs only one tibial cutting guide equipped with a rigid-body. Our results, especially in genu varum deformity, are quite remarkable. Regarding genu valgum, the results seem to be less accurate, but the software was designed for medial UKA and the series is short, so, it is too soon to extrapolate any conclusion. The main interest in this navigation is to avoid too much under correction and even better to avoid over correction when the deformity is over reducible. Indeed, when one uses a mobile bearing plateau, the risk is to have a dislocation of the meniscus. So, when tightening the collateral ligaments, checking the lower limb axis may persuade not to use a mobile bearing plateau but rather a fixed plateau

Objectives

The development of tibiofemoral angle in children has shown ethnic variations. However this data is unavailable for our population.

Methods

We measured the tibiofemoral angle (TFA) and intercondylar and intermalleolar distances in 360 children aged between two and 18 years, dividing them into six interrupted age group intervals: two to three years; five to six years; eight to nine years; 11 to 12 years; 14 to 15Â years; and 17 to 18 years. Each age group comprised 30 boys and 30 girls. Other variables recorded included standing height, sitting height, weight, thigh length, leg length and length of the lower limb.

Aims. Temporary hemiepiphysiodesis (HED) is applied to children and adolescents to correct angular deformities (ADs) in long bones through guided growth. Traditional Blount staples or two-hole plates are mainly used for this indication. Despite precise surgical techniques and attentive postoperative follow-up, implant-associated complications are frequently described. To address these pitfalls, a flexible staple was developed to combine the advantages of the established implants. This study provides the first results of guided growth using the new implant and compares these with the established two-hole plates and Blount staples. Methods. Between January 2013 and December 2016, 138 patients (22 children, 116 adolescents) with genu valgum or genu varum were treated with 285 flexible staples. The minimum follow-up was 24 months. These results were compared with 98 patients treated with 205 two-hole plates and 92 patients treated with 535 Blount staples. In long-standing anteroposterior radiographs, mechanical axis deviations (MADs) were measured before and during treatment to analyze treatment efficiency. The evaluation of the new flexible staple was performed according to the idea, development, evaluation, assessment, long-term (IDEAL) study framework (Stage 2a). Results. Overall, 79% (109/138) of patients treated with flexible staples achieved sufficient deformity correction. The median treatment duration was 16 months (interquartile range (IQR) 8 to 21). The flexible staples achieved a median MAD correction of 1.2 mm/month/HED site (IQR 0.6 to 2.0) in valgus deformities and 0.6 mm/month/HED site (IQR 0.2 to 1.5) in varus deformities. Wound infections occurred in 1%, haematomas and joint effusions in 4%, and implant-associated complications in 1% of patients treated with flexible staples. Valgus AD were corrected faster using flexible staples than two-hole plates and Blount staples. Furthermore, the median MAD after treatment was lower in varus and valgus AD, fewer implant-associated complications were detected, and reduced implantation times were recorded using flexible staples. Conclusion. The flexible staple seems to be a viable option for guided growth, showing comparable or possibly better results regarding correction speed and reducing implant-associated complications. Introduction. Angular deformity in the lower extremities can result in pain, gait disturbance, deformity and joint degeneration. Guided growth modulation uses the tension band principle with the goal of treatment being to normalise the mechanical axis. To assess the success of this procedure we reviewed our results in an attempt to identify patients who may not benefit from this simple and elegant procedure. Materials and Methods. We reviewed the surgical records and imaging in our tertiary children's hospital to identify all patients who had guided growth surgery since 2007. We noted the patient demographics, diagnosis, peri-operative experience and outcome. All patients were followed until skeletal maturity or until metalwork was removed. Results. 173 patients with 192 legs were assessed for eligibility. Six were excluded due to inadequate follow-up or loss of records. Of the 186 treated legs meeting criteria for final assessment 19.8% were unsuccessful, the other 80.2% were deemed successful at final follow up. Complications included infection and metal-work failure. Those with a pre-treatment diagnosis of idiopathic genu valgum/ varum had a success rate of 83.6%. Conclusions. In our hands, guided growth had an 80-percent success rate when all diagnosis were considered. Those procedures that were unlikely to be successful included growth disturbances due to mucopolysaccharide storage disease, Blounts disease and achondroplasia. Excluding those three diagnoses, success rate was 85.4%. We continue to advocate the use of guided growth as a successful treatment option for skeletally immature patients with limb deformity

Introduction. Distal femoral and proximal tibial osteotomies are effective procedures to treat degenerative disease of the knee joint. Previously described techniques advocate the use of bone graft to promote healing at the osteotomy site. In this present study a novel technique which utilises the osteogenic potential of the cambial periosteal layer to promote healing “from the outside in” is described. Materials and Methods. A retrospective analysis of a consecutive single-surgeon series of 23 open wedge osteotomies around the knee was performed. The median age of the patients was 37 years (range 17–51 years). The aetiology of the deformities included primary genu valgum (8/23), fracture malunion (4/23), multiple epiphyseal dysplasia (4/23), genu varum (2/23), hypophosphataemic rickets (1/23), primary osteoarthritis (1/23), inflammatory arthropathy (1/23), post-polio syndrome (1/23), and pseudoachondroplasia (1/23). Results. There were two cases lost to follow-up with a median follow-up period 17 months (range 1–32 months). Union was achieved in all cases, with 1/23 requiring revision for early fixation failure for technical reasons. The median time to radiographic union 3.2 months (95% Confidence Interval (CI) 2.5–3.8 95% CI). CT scans demonstrated early periosteal callus, beneath the osteoperiosteal flap, bridging the opening wedge cortex. Clinical union occurred at 4.1 months (95% CI 3.9–4.2 months). Complications included superficial surgical site infection (1/23), deep vein thrombosis (1/23), and symptomatic metalwork requiring removal (7/23). Conclusions. The osteoperiosteal flap technique was a safe and effective technique for opening wedge osteotomies around the knee with a reliable rate of union
