This study investigated the effects on friction of changing the dimensions of a ball-and-socket Total Disc Arthroplasty (TDA). A generic ball-and-socket model was designed and manufactured based on the dimensions and geometry of a metal-on-metal Maverick (Medtronic, Minneapolis, USA) device. Keeping the radial clearance similar to the Maverick, the ball and socket dimensions varied between 10 to 16 mm and 10.015 to 16.015 mm, respectively, in order to enable the comparison between different dimensions. The implants were made out of Cobalt Chrome Molybdenum alloy, with a surface roughness of 0.05 μm. A Bose spine simulator (Bose Corporation, ElectroForce Systems Group, Minnesota, USA) was used to apply an axial compressive force to the TDA. Axial rotation of ±2° was then applied at various frequencies and the resulting frictional torque measured. The tests were performed under an axial load of 50, 600 and 1200 N and frequencies of 0.5, 1.0, 1.5 and 2.0 Hz, for four different samples of radii 10, 12, 14 and 16 mm (48 combinations in total). The results showed variation of frictional torque in different frequencies for all four samples under constant axial load. It was observed that the frictional torque had the lowest value for the implant with ball radius of 16mm. It might be concluded that the implant with larger ball radius may create less friction and hence offer a longer life

Introduction:. Due to absence of fusion in guided-growth devices for EOS (growing rods, Shilla, LSZ) movement of the rods against their attachment is possible resulting in wear debris formation. It is important to understand the wear resistance of materials used in these devices under appropriate conditions. Aim:. The aim of our work was to investigate wear resistance of titanium alloy Ti-6Al-4V and superelastic Nitinol. Nitinol has been used recently for correcting scoliosis and may provide a better and more gradual correction than other materials. Methodology:. Wear tests were conducted using pin-on-disc configuration in diluted calf serum, as required by ISO 18192 for spinal devices. Pins were made of titanium alloy Ti-6Al-4V and Nitinol (simulation of rods material). All discs were made of Ti6-Al-4V alloy. Results and Discussion:. Superelastic Nitinol has better wear resistance since its volume wear loss is 100 times less than that of Ti-6Al-4V. However volumetric loss from Nitinol/Ti-6Al-4V friction pair (sum of both components) was found to be just about 3 times less in comparison with Ti-6Al-4V/Ti-6Al-4V pair due to wear of the titanium component. Deposition of ceramic coatings of titanium nitride (TiN) and diamond like carbon (DLC) significantly improves the wear resistance of Nitinol/Ti-6Al-4V friction pair due to effective protection of Ti-6Al-4V alloy component from the wear damage. Wear of Nitinol used for spinal rods is not expected to be as great as the wear of titanium, additionally it is possible to reduce the wear of Nitinol/titanium combination further by using coatings on the titanium alloy. Aims. The aim of this study was to determine whether chilled irrigation saline decreases the incidence of clinical upper limb palsy (ULP; a reduction of one grade or more on manual muscle testing; MMT), based on the idea that ULP results from thermal damage to the nerve roots by heat generated by friction during bone drilling. Methods. Irrigation saline for drilling was used at room temperature (RT, 25.6°C) in open-door laminoplasty in 400 patients (RT group) and chilled to a mean temperature of 12.1°C during operations for 400 patients (low-temperature (LT) group). We assessed deltoid, biceps, and triceps brachii muscle strength by MMT. ULP occurring within two days post-operatively was categorised as early-onset palsy. Results. The incidence of ULP (4.0% vs 9.5%, p = 0.003), especially early-onset palsy (1.0% vs 5.5%, p < 0.001), was significantly lower for the LT group than for the RT group. Multivariate analysis indicated that RT irrigation saline use, concomitant foraminotomy, and opened side were significant predictors for ULP. Discussion. Using chilled irrigation saline during bone drilling significantly decreased the ULP incidence, particularly the early-onset type, and shortened the recovery period for ULP. Chilled irrigation saline can thus be recommended as a simple method for preventing ULP. Take home message: Chilled irrigation during laminoplasty reduces C5 palsy. The purpose of this study was to evaluate whether concerns about the release of metal ions in metal-on-metal total hip replacements (THR) should be extended to patients with metal-bearing total disc replacements (TDR).

Cobalt and chromium levels in whole blood and serum were measured in ten patients with a single-level TDR after a mean follow-up of 34.5 months (13 to 61) using inductively-coupled plasma mass spectrometry. These metal ion levels were compared with pre-operative control levels in 81 patients and with metal ion levels 12 months after metal-on-metal THR (n = 21) and resurfacing hip replacement (n = 36). Flexion-extension radiographs were used to verify movement of the TDR.

Cobalt levels in whole blood and serum were significantly lower in the TDR group than in either the THR (p = 0.007) or the resurfacing group (p < 0.001). Both chromium levels were also significantly lower after TDR versus hip resurfacing (p < 0.001), whereas compared with THR this difference was only significant for serum levels (p = 0.008). All metal ion levels in the THR and resurfacing groups were significantly higher than in the control group (p < 0.001). In the TDR group only cobalt in whole blood appeared to be significantly higher (p < 0.001). The median range of movement of the TDR was 15.5° (10° to 22°).

These results suggest that there is minimal cause for concern about high metal ion concentrations after TDR, as the levels appear to be only moderately elevated. However, spinal surgeons using a metal-on-metal TDR should still be aware of concerns expressed in the hip replacement literature about toxicity from elevated metal ion levels, and inform their patients appropriately.

This prospective study of 136 children with progressive infantile scoliosis treated under the age of four years, and followed up for nine years, shows that the scoliosis can be reversed by harnessing the vigorous growth of the infant to early treatment by serial corrective plaster jackets.

In 94 children (group 1), who were referred and treated in the early stages of progression, at a mean age of one year seven months (6 to 48 months) and with a mean Cobb angle of 32° (11° to 65°), the scoliosis resolved by a mean age of three years and six months. They needed no further treatment and went on to lead a normal life. At the last follow-up, their mean age was 11 years and two months (1 year 10 months to 25 years 2 months), 23 (24.5%) were at Risser stages 4 and 5 and 13 girls were post-menarchal.

In 42 children (group 2), who were referred late at a mean age of two years and six months (11 to 48 months) and with a mean Cobb angle of 52° (23° to 92°), treatment could only reduce but not reverse the deformity. At the last follow-up, at a mean age of ten years and four months (1 year 9 months to 22 years 1 month), eight children (19%) were at Risser stages 4 and 5 and five girls were post-menarchal. Fifteen children (35.7%) had undergone spinal fusion, as may all the rest eventually.
