Background. There is a 20% dissatisfaction rate with knee replacements. Calls for tools that can pre-operatively identify patients at risk of being dissatisfied postoperatively have been widespread. However, it is unclear what sort of information patients would want from such a tool, how it would affect their decision making process, and at what part of the pathway such a tool should be used. Methods. Using focus groups involving 12 participants and in-depth interviews with 10 participants, we examined the effect outcome prediction has by providing fictitious predictions to patients at different stages of treatment. Background. Hip fractures cause significant morbidity and mortality, affecting 70,000 people in the UK each year. The dynamic hip screw (DHS) is used for the osteosynthesis of extracapsular neck of femur fractures, a procedure that requires complex psychomotor skills to achieve optimal lag screw positioning. The tip-apex distance (TAD) is a measure of the position of the lag screw from the apex of the femoral head, and is the most comprehensive predictor of cut-out (failure of the DHS construct). To develop these skills, trainees need exposure to the procedure, however with the European Working Time Directive, this is becoming harder to achieve. Simulation can be used as an adjunct to theatre learning, however it is limited. FluoroSim is a digital fluoroscopy simulator that can be used in conjunction with workshop bones to simulate the first step of the DHS procedure (guide-wire insertion) using image guidance. This study assessed the construct validity of FluoroSim. The null hypothesis stated that there would be no difference in the objective metrics recorded from FluoroSim between users with different exposure to the DHS procedure. Methods. This multicentre study recruited twenty-six orthopaedic doctors. They were categorised into three groups based on the number of DHS procedures they had completed as the primary surgeon (novice <10, intermediate 10≤x<40 and experienced ≥40). Twenty-six participants completed a single DHS guide-wire attempt into a workshop bone using FluoroSim. The TAD, procedural time, number of radiographs, number of guide-wire retires and cut-out rate (COR) were recorded for each attempt. Results. A significant construct effect was seen for TAD and COR between novice and other users (p < 0.05). The intermediate and experienced users were not significantly different for these metrics. Introduction. Following primary total knee arthroplasty (TKA), patients experience pain relief and report improved physical function and activity. However, there is paucity of evidence that patients are truly more active in daily life after TKA. The aims of this study were: 1) to prospectively measure physical activity with a wearable motion sensor before and after TKA; 2) to compare patient-reported levels of physical activity with objectively assessed levels of physical activity before and after TKA; 3) to investigate whether differences in physical activity after TKA are related to levels of physical function. Methods. 22 patients (age=66.6 ±9.3yrs; m/f= 12/11; BMI= 30.6 ±6.1) undergoing primary TKA (Vanguard, ZimmerBiomet), were measured preoperatively and 1–3 years postoperatively. Patient-reported outcome measures (PROMs) included KOOS-PS and SQUASH for assessment of perceived physical function and activity resp. Physical activity was assessed during 4 consecutive days in patients” home environments while wearing an accelerometer-based activity monitor (AM) at the thigh. All data were analysed using semi-automated algorithms in Matlab. AM-derived parameters included walking time (s), sitting time (s) standing time (s), sit-to-stand transfers, step count, walking bouts and walking cadence (steps/min). Objective physical function was assessed by motion analysis of gait, sit-to-stand (STS) transfers and block step-up (BS) transfers using a single inertial measurement unit (IMU) worn at the pelvis. IMU-based motion analysis was only performed postoperatively. Statistical comparisons were performed with SPSS and a per-protocol analysis was applied to present the results at follow-up. Results. Data were available for 17 of 22 patients at follow-up. PROMs demonstrated significant improvement of perceived physical function (KOOS-PS=68±21 vs. 34±26; p<0.001) and physical activity (SQUASH=2584 ±1945 vs. 3038 ±2228; p<0.001) following TKA. AM-based parameters of physical activity demonstrated no significant differences between pre- and postoperative quantitative outcomes. Only the qualitative outcome of walking cadence significantly changed after TKA (81.41 ±10.86 (steps/min) vs. 94.24 ±7.20 resp.; p<0.001). There were moderate correlations between self-reported and objectively assessed levels of physical activity after TKA (Pearson”s r=0.36–0.43; p<0.05). Outcomes of physical activity after TKA were moderately correlated to IMU-based functional outcome measures (Pearson”s r = 0.31 – 0.48; p<0.05). Conclusion. 1–3 years after TKA, patients demonstrate improved function. However, the self-perceived higher activity level (+18%) after TKA is not supported by any objective data obtained by wearable motion sensors such as steps, transfers or time-on-feet. This may have implications for general health and requires further investigation into patient communication,
Background. Knee replacement surgery is currently facing three dilemmas: a high dissatisfaction rate; increasing demand with financial constraints; and variation in utilisation. A patient centred approach, usually achieved through shared decision-making, has the potential to help address these dilemmas. However, such an approach requires an understanding of the factors involved in patient decision-making. This is the first study examining decision-making in knee replacements that includes patients at different stages of decision-making – this is critical when considering decision-making as a process. We base our findings in a theoretical model, proposed by Elwyn et al, that highlights the distinction between deliberation and decision-making, and propose modifications to this model specific to knee replacement decision-making. Methods. This study used two focus groups of six patients each and in-depth interviews with 10 patients to examine the factors that affect patient decision-making and their interaction at different points in the decision-making process. The renewed interest in the clinically proven low wear of the metal-on-metal bearing combined with the capacity of inserting a thin walled cementless acetabular component has fostered the reintroduction of hip resurfacing. As in other forms of conservative hip surgery, i.e. pelvic osteotomies and impingement surgery, patient selection will help minimize complications and the need for early reoperation. Patient Selection and Hip Resurfacing. Although hip resurfacing was initially plagued with high failure rates, the introduction of metal on metal bearings as well as hybrid fixation has shown excellent survivorships of 97 to 99% at 4 to 5 years follow-up. However, it is important to critically look at the initial published results. In all of these series there was some form of patient selection. For example, in the Daniel and associates publications, only patients with osteoarthritis with an age less than 55 were included with 79% of patients being male. Thermal stability is a key property in determining the suitability of an antibiotic agent for local application in the treatment of orthopaedic infections. Despite the fact that long-term therapy is a stated goal of novel local delivery carriers, data describing thermal stability over a long period are scarce, and studies that avoid interference from specific carrier materials are absent from the orthopaedic literature. In this study, a total of 38 frequently used antibiotic agents were maintained at 37°C in saline solution, and degradation and antibacterial activity assessed over six weeks. The impact of an initial supplementary heat exposure mimicking exothermically curing bone cement was also tested as this material is commonly used as a local delivery vehicle. Antibiotic degradation was assessed by liquid chromatography coupled to mass spectrometry, or by immunoassays, as appropriate. Antibacterial activity over time was determined by the Kirby-Bauer disk diffusion assay.Objectives

An experimental rabbit model was used to test the null hypothesis,
that there is no difference in new bone formation around uncoated
titanium discs compared with coated titanium discs when implanted
into the muscles of rabbits. A total of three titanium discs with different surface and coating
(1, porous coating; 2, porous coating + Bonemaster (Biomet); and
3, porous coating + plasma-sprayed hydroxyapatite) were implanted
in 12 female rabbits. Six animals were killed after six weeks and
the remaining six were killed after 12 weeks. The implants with
surrounding tissues were embedded in methyl methacrylate and grinded
sections were stained with Masson-Goldners trichrome and examined
by light microscopy of coded sections.Objectives

We assessed the predictive value of the macroscopic and detailed microscopic appearance of the coracoacromial ligament, subacromial bursa and rotator-cuff tendon in 20 patients undergoing subacromial decompression for impingement in the absence of full-thickness tears of the rotator cuff. Histologically, all specimens had features of degenerative change and oedema in the extracellular matrix. Inflammatory cells were seen, but there was no evidence of chronic inflammation. However, the outcome was not related to cell counts. At three months the mean Oxford shoulder score had improved from 29.2 (20 to 40) to 39.4 (28 to 48) (p <
0.0001) and at six months to 45.5 (36 to 48) (p <
0.0001). At six months, although all patients had improved, the seven patients with a hooked acromion had done so to a less extent than those with a flat or curved acromion judged by their mean Oxford shoulder scores of 43.5 and 46.5 respectively (p = 0.046). All five patients with partial-thickness tears were within this group and demonstrated less improvement than the patients with no tear (mean Oxford shoulder scores 43.2 and 46.4, respectively, p = 0.04). These findings imply that in the presence of a partial-thickness tear subacromial decompression may require additional specific treatment to the rotator cuff if the outcome is to be improved further.
There are many methods for analysing wear volume in failed polyethylene acetabular components. We compared a radiological technique with three recognised We tested 18 ultra-high-molecular-weight polyethylene acetabular components revised for wear and aseptic loosening, of which 13 had pre-revision radiographs, from which the wear volume was calculated based upon the linear wear. We used a shadowgraph technique on silicone casts of all of the retrievals and a coordinate measuring method on the components directly. For these techniques, the wear vector was calculated for each component and the wear volume extrapolated using mathematical equations. The volumetric wear was also measured directly using a fluid-displacement method. The results of each technique were compared. The series had high wear volumes (mean 1385 mm3; 730 to 1850) and high wear rates (mean 205 mm3/year; 92 to 363). There were wide variations in the measurements of wear volume between the radiological and the other techniques. Radiograph-derived wear volume correlated poorly with that of the fluid-displacement method, co-ordinate measuring method and shadowgraph methods, becoming less accurate as the wear increased. The mean overestimation in radiological wear volume was 47.7% of the fluid-displacement method wear volume. Fluid-displacement method, coordinate measuring method and shadowgraph determinations of wear volume were all better than that of the radiograph-derived linear measurements since they took into account the direction of wear. However, only radiological techniques can be used Interpretation of radiological measurements of acetabular wear must be done judiciously in the clinical setting.
We produced large full-thickness articular cartilage defects in 33 rabbits in order to evaluate the effect of joint distraction and autologous culture-expanded bone-marrow-derived mesenchymal cell transplantation (ACBMT) at 12 weeks. After fixing the knee on a hinged external fixator, we resected the entire surface of the tibial plateau. We studied three groups: 1) with and without joint distraction; 2) with joint distraction and collagen gel, and 3) with joint distraction and ACBMT and collagen gel. The histological scores were significantly higher in the groups with ACBMT collagen gel (p <
0.05). The area of regenerated soft tissue was smaller in the group allowed to bear weight (p <
0.05). These findings suggest that the repair of large defects of cartilage can be enhanced by joint distraction, collagen gel and ACBMT.
Post-mortem retrieval of canine, cemented femoral components was analysed to assess the performance of these implants in the dog as a model for human total hip replacement (THR). Mechanical testing and radiological analysis were performed to determine the stability of the implant and the quality of the cement. Thirty-eight implants from 29 dogs were retrieved after time intervals ranging from 0.67 to 11.67 years. The incidence of aseptic loosening was 63.2%, much higher than in human patients (6% in post-mortem studies). Failure of the femoral implants began with debonding at the cement-metal interface, similar to that in implants in man. The incidence of aseptic loosening was much lower in bilateral than in unilateral implants. Significant differences were observed for three different designs of implant. While the dog remains the animal model of choice for THR, results from this study provide insight into interspecies differences in the performance of implants. For example, the performance of THR in dogs should be compared with that in young rather than in elderly human patients.
