Introduction. Patients with hip abductor tendon tears amenable to endoscopic repair tend to be severely disabled and older. However, low preop baseline patient reported outcome (PRO) and advancing age are each often reported to be a harbinger of poor result with hip arthroscopy. Thus, the purpose of this study is to report the demographic makeup of this population and how these patients faired in terms of preop scores and reaching both Minimal Clinically Important Difference (MCID) and Substantial Clinical Benefit (SCB). Methods. Sixty-six consecutive hips in 64 patients (2 bilateral) undergoing endoscopic abductor tendon repair with a hollow core bioabsorbable suture anchor and having achieved two-year follow-up were prospectively assessed with modified Harris Hip Score (mHHS) and international Hip Outcome Tool (iHOT) scores. The MCID for patients undergoing hip arthroscopy has previously been determined as 8 for the mHHS and 13 for the iHOT. SCB has been determined as 20 for the mHHS and 28 for the iHOT. Subgroups were compared using the independent samples t-test. Results. The average age was 57 years (range 22–83 years) with 59 females and 5 males. Post-operative follow-up averaged 28 months (range 24–60). There were 33 full-thickness and 33 partial-thickness tears; 39 gluteus medius tears, 25 medius and minimus tears, and 2 isolated minimus tears. Among the 66 hips, the average preop mHHS was 48.8 with 98.5% achieving MCID and 93.8% SCB. Among 60 hips that had complete iHOT data, the average preop score was 30.0 with 98.3% achieving MCID and 88.3% SCB. There were no complications. One patient underwent repeat arthroscopy for joint debridement at 12 months following abductor repair, and one subsequently underwent total hip replacement at 11 months following repair. There was no statistically significant difference between subgroups of full thickness/partial thickness tears, or single/two tendon tears. Conclusion. This report of endoscopic abductor tendon repair represents a heterogeneous group of single and two tendon involvement with partial and full thickness tears. Collectively these patients can respond exceptionally well in terms of MCID (98.5% mHHS; 98.3% iHOT) and SCB (93.8% mHHS; 88.3% iHOT), even in the presence of low preop baseline scores (average 48.8 mHHS; 30 iHOT) and older age (average 57 years). Partial thickness abductor tendon tears are a significant source of recalcitrant laterally based hip pain. For those that fail conservative treatment, the results of endoscopic repair are highly successful with minimal morbidity. The principal burden is the protracted rehabilitation that is necessary as part of the recovery process. There is a wide gap between failed conservative treatment and successful surgical repair. It is hypothesized that a non-repair surgical strategy, such as a bioinducitve patch, could significantly reduce the burden associated recovery from a formal repair. Thus, the purpose of this study is to report the preliminary results of this treatment strategy. Symptomatic partial thickness abductor tendon tears are treated conservatively, including activity modification, supervised physical therapy and ultrasound guided corticosteroid injections. Beginning in January 2022, patients undergoing hip arthroscopy for intraarticular pathology who also had persistently symptomatic partial thickness abductor tendon tears, were treated with adjunct placement of a bioinducitve (Regeneten) patch over the tendon lesion from the peritrochanteric space. The postop rehab protocol is dictated by the intraarticular procedure performed. All patients are prospectively assessed with a modified Harris Hip Score (mHHS) and iHOT and the tendon healing response examined by ultrasound. Early outcomes will be presented on nine consecutive cases. Conclusions - Will be summarized based on the preliminary outcomes to be reported

The 2020 London International Hamstring Consensus meeting was convened to improve our understanding and treatment of hamstring injuries. The multidisciplinary consensus panel included 14 International specialists on the management of hamstring injuries. The Delphi consensus process consisted of two rounds of surveys which were completed by 19 surgeons from a total of 106 participants. Consensus on individual statements was regarded as over 70% agreement between panel members. The consensus group agreed that the indications for operative intervention included the following: gapping at the zone of injury (86.9%); high functional demands of the patient (86.7%); symptomatic displaced bony avulsions (74.7%); and proximal free tendon injuries with functional compromise refractory to non-operative treatment (71.4%). Panel members agreed that surgical intervention had the capacity to restore anatomy and function, while reducing the risk of injury recurrence (86.7%). The consensus group did not support the use of corticosteroids or endoscopic surgery without further evidence. These guidelines will help to further standardise the treatment of hamstring injuries and facilitate decision-making in the surgical treatment of these injuries

The aim of this review is to evaluate the current available literature evidencing on peri-articular hip endoscopy (the third compartment). A comprehensive approach has been set on reports dealing with endoscopic surgery for recalcitrant trochanteric bursitis, snapping hip (or coxa-saltans; external and internal), gluteus medius and minimus tears and endoscopy (or arthroscopy) after total hip arthroplasty. This information can be used to trigger further research, innovation and education in extra-articular hip endoscopy

We report on a cadaveric study and early experience using patient specific drill guides to prevent cortex perforations and reduce the need for a trochanteric osteotomy in revision THA. Mimic software (Materialise) was used for 3D analysis of the cement mantle and cement plug. Based on the CT findings a Cannulated drill guide with the shape of the femoral stem was printed in medical graded nylon intraoperative findings and complications were recorded on videotape using a standard 5mm laparoscope for medullary inspection. Surgical Technique was to attain a pre-operative CT scan with MARS protocol of the proximal femur to evaluate the femoral stem positioning, the 3D anatomy of the cement mantle, the length of the cement plug and the quality of the surrounding bone. Subsequent a 3D printing of patient specific cannulated drill guide with the shape of the removed femoral component but an eccentric cannulation was made. Endoscopic inspection was performed of the inside of the cement mantle, then insertion of the autoclaved cannulated drill guide in the existing cement mantle. After perforation of the distal plug through the PSI drill guide using either a long drill or an ultrasound plug perforation tool (Zimmer Biomet, Warsaw) the excessive cement was removed with standard available flexible femoral shaft reamers (Zimmer Biomet). Further laproscopic examination of the femoral canal performed to verify completeness of the cement removal. Results. CT scans with 3D reconstruction of the existing cement mantle is possible using Modern CT with MARS protocols. After the training on sawbones and cadaveric bones a predictable plug perforation was obtained in all clinical cases. There were no intraoperative cortex perforations and no intraoperative femoral fractures. Conclusion. CT scan analysis of femoral cement mantles together with patient specific drill guides are promising tools to reduce the risk of femoral perforation in revision total hip arthroplasty

Aims

In order to release the contracture band completely without damaging normal tissues (such as the sciatic nerve) in the surgical treatment of gluteal muscle contracture (GMC), we tried to display the relationship between normal tissue and contracture bands by magnetic resonance neurography (MRN) images, and to predesign a minimally invasive surgery based on the MRN images in advance.

Methods

A total of 30 patients (60 hips) were included in this study. MRN scans of the pelvis were performed before surgery. The contracture band shape and external rotation angle (ERA) of the proximal femur were also analyzed. Then, the minimally invasive GMC releasing surgery was performed based on the images and measurements, and during the operation, incision lengths, surgery duration, intraoperative bleeding, and complications were recorded; the time of the first postoperative off-bed activity was also recorded. Furthermore, the patients’ clinical functions were evaluated by means of Hip Outcome Score (HOS) and Ye et al’s objective assessments, respectively.

Aims

The direct anterior approach (DAA) for total hip arthroplasty (THA) has potential advantages over other approaches and is most commonly performed with the patient in the supine position. We describe a technique for DAA THA with the patient in the lateral decubitus position and report the early clinical and radiological outcomes, the characteristics of the learning curve, and perioperative complications.

Methods

All primary DAA THAs performed in the lateral position by a single surgeon over a four-year period from the surgeon’s first case using the technique were identified from a prospectively collected database. Modified Harris Hip Scores (mHHS) were collected to assess clinical outcome, and routine radiological analysis was performed. Retrospective review of the medical records identified perioperative complications, the characteristics of the learning curve, and revisions.

Aims

Arthroplasty skills need to be acquired safely during training, yet operative experience is increasingly hard to acquire by trainees. Virtual reality (VR) training using headsets and motion-tracked controllers can simulate complex open procedures in a fully immersive operating theatre. The present study aimed to determine if trainees trained using VR perform better than those using conventional preparation for performing total hip arthroplasty (THA).

Patients and Methods

A total of 24 surgical trainees (seven female, 17 male; mean age 29 years (28 to 31)) volunteered to participate in this observer-blinded 1:1 randomized controlled trial. They had no prior experience of anterior approach THA. Of these 24 trainees, 12 completed a six-week VR training programme in a simulation laboratory, while the other 12 received only conventional preparatory materials for learning THA. All trainees then performed a cadaveric THA, assessed independently by two hip surgeons. The primary outcome was technical and non-technical surgical performance measured by a THA-specific procedure-based assessment (PBA). Secondary outcomes were step completion measured by a task-specific checklist, error in acetabular component orientation, and procedure duration.

Over recent years hip arthroscopic surgery has evolved into one of the most rapidly expanding fields in orthopaedic surgery. Complications are largely transient and incidences between 0.5% and 6.4% have been reported. However, major complications can and do occur. This article analyses the reported complications and makes recommendations based on the literature review and personal experience on how to minimise them.
