Instant messaging via WhatsApp is used within hospital teams. Group messaging can lead to efficient and non-hierarchical communication. Despite being end-to-end encrypted, WhatsApp is owned by Facebook, raising concerns regarding data security. The aims of this study were: 1) to record the prevalence of WhatsApp group instant messaging amongst clinical teams; 2) to ascertain clinician attitudes towards use of instant messaging, 3) to gauge clinicians’ awareness of best practice regarding mobile data protection and 4) to create a practical guideline based off available literature that can be used to by clinicians to improve data security practice. Over a two-week period, clinical nurse specialists in the Auckland District Health Board Department of Orthopaedics retrospectively completed a blind audit of all messaging activity across the five teams WhatsApp group message threads, recording quantity of messages sent and the nature of the messages. Concurrently individuals in these WhatsApp groups completed an anonymous survey of their use of WhatsApp and their awareness of local data security policies and practice. A guideline adapted from available literature was created to compare current practice to recommended standards and subsequently adopted into local policy. 1360 messages were sent via WhatsApp in a two-week period. 384 (28%) of the messages contained patient identifiable data. Thirty-six photos were shared. Participants rated use of WhatsApp at 9.1/10 – extremely beneficial. Sixty-five per cent of clinicians reported they had not read or were unaware of the ADHB policies regarding mobile devices and information privacy and security. WhatsApp use is widespread within the Orthopaedic department and is the preferred platform of communication with many perceived benefits. Data security is a risk and implementation of an appropriate guideline to assist clinicians in achieving best practice is crucial to ensure patient data remains protected

Retrieval analysis has been valuable in the assessment of in-vivo surface damage of orthopedic devices. Historically, subjective techniques were used to grade damage on the implant's surface. Microscopy improved the ability to localize and quantify damage, but cannot measure volumetric wear due to this damage. Laser scanning provides volumetric wear, but lacks image data. Recent techniques superimpose image data on laser scan data (photorendering) and combine the strengths of both methods. Our goal is to use such methods to improve our damage assessment and potentially correlate this assessment to volumetric wear. This project focused on two areas: image-stitching and photorendering. Image-stitching registers multiple images into large-scale high-resolution composites. Six total disc replacement components were imaged with a digital microscope (Moticam 2, Motic). Three sets were taken of each component: a single template at 10x zoom (1×1), a 4-image composite at 18x zoom (2×2), and a 9-image composite at 18x zoom (3×3). The 2×2 and 3×3 sets were image-stitched to resemble their template counterpart. Measurement error was defined using common pixels identified between the composite and template images for comparison with a semi-automated feature detection algorithm (Figure 1). For photorendering, a pilot study was performed on a single retrieved tibial bearing. The component was imaged with a digital microscope (VHX-2000, Keyence) under a 3D image-stitching setting, providing a high-resolution photo embedded with height values. MATLAB was used to convert the image into a photo-rendered point cloud approximating the surfaces. The component was then laser scanned, creating a 3D point cloud with resolution 0.127 mm. The photo-rendered point cloud data was registered to the laser scan data using an iterative closest point algorithm (Geomagic Studio, Geomagic). An analysis of all composite images showed a mean error of 0.221 mm. Figure 2 compares regions of images for the template, 2×2, and 3×3 composites. Zooming in shows the effect of the increased resolution contained in the composite. The 2×2 and 3×3 composites had mean errors of 0.231 mm and 0.209 mm, respectively; these were not significantly different. Comparisons among image types showed that components with less features exhibited larger errors during image-stitching. Figure 3 shows images resulting from each step of the photorendering process. The final image of the figure shows a qualitative result of our ability to photorender the tibial bearing surface of the component. While combining microscopy and laser scan data works anecdotally, further analyses must be performed to assure the robustness of the technique. The digital microscope's embedded image-stitching software is limited in its maximum field of view; we look to extend this by taking multiple scans and using in-house software to generate a composite of a whole implant. The improved resolution provided by microscopy offer an opportunity to automate damage assessment, yielding damage mapped images which can also be overlaid on laser scan data. This may provide a means to better quantify observed damage and yield meaningful correlations with volumetric loss due to wear

Background. The advent of value-based conscientiousness and rapid-recovery discharge pathways presents surgeons, hospitals, and payers with the challenge of providing the same total hip arthroplasty episode of care in the safest and most economic fashion for the same fee, despite patient differences. Various predictive analytic techniques have been applied to medical risk models, such as sepsis risk scores, but none have been applied or validated to the elective primary total hip arthroplasty (THA) setting for key payment-based metrics. The objective of this study was to develop and validate a predictive machine learning model using preoperative patient demographics for length of stay (LOS) after primary THA as the first step in identifying a patient-specific payment model (PSPM). Methods. Using 229,945 patients undergoing primary THA for osteoarthritis from an administrative database between 2009– 16, we created a naïve Bayesian model to forecast LOS after primary THA using a 3:2 split in which 60% of the available patient data “built” the algorithm and the remaining 40% of patients were used for “testing.” This process was iterated five times for algorithm refinement, and model performance was determined using the area under the receiver operating characteristic curve (AUC), percent accuracy, and positive predictive value. LOS was either grouped as 1–5 days or greater than 5 days. Results. The machine learning model algorithm required age, race, gender, and two comorbidity scores (“risk of illness” and “risk of morbidity”) to demonstrate excellent validity, reliability, and responsiveness with an AUC of 0.87 after five iterations. Hospital stays of greater than 5 days for THA were most associated with increased risk of illness and risk of comorbidity scores during admission compared to 1–5 days of stay. Conclusions. Our machine learning model derived from administrative big data demonstrated excellent validity, reliability, and responsiveness after primary THA while accurately predicting LOS and identifying two comorbidity scores as key value-based metrics. Predictive data has the potential to engender a risk-based PSPM prior to primary THA and other elective orthopaedic procedures

Primary malignant bone tumours are a scarce entity with limited population-based data from developing countries. The aim of the study is to investigate the frequency and anatomical distribution of primary malignant bone tumours in a local South African population. This will be an epidemiological retrospective study. Data will be used of patients that were diagnosed with primary malignant bone tumours over a period of nine years spanning from 1 January 2014 to 31 December 2022. This data will be received from private and government laboratories. Data to be considered are type of primary malignant bone tumours diagnosed, incidence of primary malignant bone tumours over a period of nine years and the most common anatomical sites of primary malignant bone tumours. The rationale behind our study is to assess the frequency of different primary malignant bone tumours in another geographic area of South Africa and to compare these findings to local and international literature. With a projected increase in diagnosis of primary malignant bone tumours in developing countries it is important to have more available data about primary malignant bone tumours from these areas to have a better understanding of these conditions and to understand the impact of the burden they impose on healthcare systems so that management of these conditions can also be improved. Preliminary results show that 23.83% of primary malignant bone tumours occurred in the age group 0–24 years of age, 49.22% in the 25–59 age group and 26.95% in the 60+ age group. The most common tumour that occurred was chondrosarcoma (49.21%) followed by osteosarcoma (41.80%) then Ewing's sarcoma (4,69%) and lastly chordoma (4.30%). From the 256 samples that met the inclusion criteria the five most common anatomical sites were distal femur (63), proximal tibia (41), proximal humerus (38), pelvis (34) and proximal femur (20)

Worldwide, most spine imaging is either “inappropriate” or “probably inappropriate”. The Choosing Wisely recommendation is “Do not perform imaging for lower back pain unless red flags are present.” There is currently no detailed breakdown of lower back pain diagnostic imaging performed in New Brunswick (NB) to inform future directions. A registry of spine imaging performed in NB from 2011-2019 inclusive (n=410,000) was transferred to the secure platform of the NB Institute for Data, Training and Research (NB-IRDT). The pseudonymized data included linkable institute identifiers derived from an obfuscated Medicare number, as well as information on type of imaging, location of imaging, and date of imaging. The transferred data did not include the radiology report or the test requisition. We included all lumbar, thoracic, and complete spine images. We excluded imaging related to the cervical spine, surgical or other procedures, out-of-province patients and imaging of patients under 19 years. We verified categories of X-ray, Computed Tomography (CT), and Magnetic Resonance Imaging (MRI). Red flags were identified by ICD-10 code-related criteria set out by the Canadian Institute for Health Information. We derived annual age- and sex-standardized rates of spine imaging per 100,000 population and examined regional variations in these rates in NB's two Regional Health Authorities (RHA-A and RHA-B). Age- and sex-standardized rates were derived for individuals with/without red flag conditions and by type of imaging. Healthcare utilization trends were reflected in hospital admissions and physician visits 2 years pre- and post-imaging. Rurality and socioeconomic status were derived using patients’ residences and income quintiles, respectively. Overall spine imaging rates in NB decreased between 2012 and 2019 by about 20% to 7,885 images per 100,000 people per year. This value may be higher than the Canadian average. Females had 23% higher average imaging rate than males. RHA-A had a 45% higher imaging rate than RHA-B. Imaging for red flag conditions accounted for about 20% of all imaging. X-rays imaging accounted for 67% and 75% of all imaging for RHA-A and RHA-B respectively. The proportions were 20% and 8% for CT and 13% and 17% for MRI. Two-year hospitalization rates and rates of physician visits were higher post-imaging. Females had higher age-standardized hospitalization and physician-visit rates, but the magnitude of increase was higher for males. Individuals with red flag conditions were associated with increased physician visits, regardless of the actual reason for the visit. Imaging rates were higher for rural than urban patients by about 26%. Individuals in the lowest income quintiles had higher imaging rates than those in the highest income quintiles. Physicians in RHA-A consistently ordered more images than their counterparts at RHA-B. We linked spine imaging data with population demographic data to look for variations in lumbar spine imaging patterns. In NB, as in other jurisdictions, imaging tests of the spine are occurring in large numbers. We determined that patterns of imaging far exceed the numbers expected for ‘red flag’ situations. Our findings will inform a focused approach in groups of interest. Implementing high value care recommendations pre-imaging ought to replace low-value routine imaging


Reverse Total shoulder arthroplasty (RTSA) was initially introduced to treat rotator cuff arthropathy. With proven successful long-term outcomes, it has gained a noteworthy surge in popularity with its indications consequently being extended to treating various traumatic glenohumeral diseases. Several countries holding national registries remain a guide to the use the prosthesis, however a notable lack of epidemiological data still exists. More so in South Africa where the spectrum of joint disease related to communicable diseases such as HIV and tuberculosis may influence indications and patient demographics. By analysing the epidemiology of patients who underwent RTSA at our institution, we aimed to outline the local disease spectrum, the patients afflicted and indications for surgery. A retrospective review of all patients operated within the sports unit between 1 January 2019 and 31 December 2022 was conducted. An analysis of the epidemiological data pertaining to patient demographics, diagnosis, indications for surgery and complications were recorded. Included in the review were 58 patients who underwent primary RTSA over the 4-year period. There were 41 females and 17 male patients, age <55 years (n= 14) >55 years (n=44). The indications included 23 rotator cuff arthropathy (40%), 12 primary glenohumeral osteoarthritis (OA) (20%), 10 avascular necrosis (AVN) humeral head (17%), 7 inflammatory OA (12%), 4 chronic shoulder dislocation (7%) and 2 sequalae of proximal humerus fractures (4%). The study revealed RTSA being performed in patients older than 55 years of age, the main pathologies included rotator cuff arthropathy and primary OA, however AVN and shoulder dislocations secondary to trauma contributed significantly to the total tally of surgeries undertaken. This highlights the disease burden of developing countries contributing to patients presenting for RTSA


The National Joint Registry (NJR) was set up by the Department of Health to collect information on all joint replacements. The NJR data is externally validated against nationally collated Hospital Episode Statistics (HES). Errors associated with the use of HES data have been widely documented. We sought to explore the accuracy of the NJR data, for a single surgeon, against a prospectively collected personal logbook. The NJR and logbook were compared over a 3-year period (01/07/2009 to 30/06/2012). Total procedure recorded in the personal logbook was 684 and in the NJR was 681. TKR in personal log book was 304 and in NJR 316, revision knee's in personal logbook 45 and in NJR 36, THR 274 in personal logbook and 271 in NJR, revision hip procedures in personal logbook 64 and 58 in NJR. Whilst the total number of procedures captured correlates closely (681 vs 684) there is more variation with the different individual procedures. This may be due to the addition of 11% of HES data used for this time period by the NJR as it is known to be inaccurate. This therefore demonstrates the importance of maintaining your own accurate records

Aboriginal and Torres Strait Islander (ATSI) people have higher elective wait times compared to non-ATSI population in Australia. The Murrumbidgee Local Health District (MLHD) in southern New South Wales services 125,242km. 2. and a population of 287,000 people, with 5.8% identifying as ATSI. The aim of this study is to investigate the arthroplasty waitlist time of ATSI, and the impact of rurality on joint replacement, within the MLHD and compared to the Australian national data. 1435 consecutive patients who underwent elective hip or knee arthroplasty from July 2018 to June 2021 were collated. Demographics, ATSI status, total wait time, readiness for care, and rurality were collected. Rurality was measured by distance from the arthroplasty hospital within MLHD. 1,151 patients were included after excluding patients with missing data or underwent emergent surgery. Within this cohort, 72 of 1,151 patients (6.2%) identified as ATSI. ATSI were younger than non-ATSI population (60.7y v 66.4y). There was no difference between Aboriginal status and ready for care wait time (368.0 v 349.9 days; p=0.116). The rurality of the groups was similar and increasing rurality did not affect total wait time (ATSI 103.1km v 98.6km; p=0.309). There was no difference in total or not-ready-for-care time between the groups (p: 0.68). Findings suggest equitable access to joint arthroplasty in the MLHD between ATSI and non-ATSI populations, which differs from the national experience. There is no significant difference between rurality and accessibility in the MLHD. This may be a result of the increased focus to ATSI and rural health within the district. A state or national study would be beneficial in identifying high performing regions and reviewing processes that enable equitable and accessible care. MLHD provides equitable access to arthroplasty surgery between ATSI and non-ATSI, as well as patients from rural areas within the LHD

The aim was to predict the number and incidence of distal radius fractures in Scotland over the next two decades according to age group, categorised into under 65yrs(<65) and 65yrs and older (≥65), and estimate the potential increased operative burden of this. The number of distal radius fracture in Scotland was isolated from the Global Burden of Disease database and this was used, in addition to historic population data and population estimates, to create a multivariable model allowing incorporation of age group, sex and time. A Negative Binomial distribution was used to predict incidence in 2030 and 2040 and calculate projected number of fractures according to the population at risk. A 20.4% operative intervention rate was assumed in the ≥65 group (local data). In terms of number of fractures, there was a projected 61% rise in the ≥65 group with an overall increase of 2099 fractures per year from 3417 in 2020 (95% confidence interval (CI) 2960 – 3463) to 5516 in 2040 (95% CI 4155–5675). This was associated with 428 additional operative interventions per year for those ≥65yrs. The projected increase between 2020 and 2040 was similar in both sexes (60% in females, 63% in males), however the absolute increase in fracture number was higher in females (2256 in 2020 [95% CI 1954–2287] to 3620 in 2040 [95% CI 2727–3721]) compared to males (1160 [95% CI 1005–1176] to 1895 [95% CI 1427–1950]). There was a 4% projected fall in the number of fractures in those <65. Incidence of distal radius fractures is expected to considerably increase over the next two decades due to a projected increase in the number of fractures in the elderly. This has implications for the associated morbidity and healthcare resource use


Introduction. The first VRAS TKA was performed in New Zealand in November 2020 using a Patient Specific Balanced Technique whereby VRAS enables very accurate collection of the bony anatomy and soft tissue envelope of the knee to plan and execute the optimal positioning for a balanced TKA. Method. The first 45 VRAS patients with idiopathic osteoarthritis of the knee was compared with 45 sequential patients who underwent the same TKA surgical technique using Brainlab 3 which the author has used exclusively in over 1500 patients. One and two year outcome data will be presented. Results. Aim. Aim was to compare revision rates when using single versus dual antibiotic loaded cement (ABLC) in hip fracture arthroplasty and aseptic revision hip or knee arthroplasty using data from the Dutch national joint registry (LROI). Methods. All primary cemented (hemi-)arthroplasties for acute hip fractures and cemented aseptic hip or knee revision arthroplasties, were incorporated in 3 datasets. All registered implants between 2007 and 2018 were included (minimum 2 years follow-up). Primary end-point was subsequent revision rates for infection and for any reason in the single and dual ABLC groups. Cumulative crude incidence of revision was calculated using competing risk analysis. Results. A total of 22,308 hip fracture arthroplasties, 2,529 hip revision and 7,124 knee revision arthroplasties were registered and analyzed in the study period. The majority of hip fracture patients (97.1%) was treated with single ABLC. For hip and knee revision arthroplasties dual ABLC was used in 33.8% and 25.7%. The revision rate for infection in the fracture arthroplasty group was not different between groups (0.5% versus 0.8%, p=0.27). The re-revision rate following hip or knee revision based on single versus dual ABLC was not different between groups (3.2% versus 2.8%, p=0.82 for hip revision and 1.8% versus 2.5%, p=0.36 for knee revision). In addition, the re-revision rate for any reason was not different in all three datasets. The crude cumulative revision and re-revision rates for any reason based on single ABLC versus dual ABLC showed no differences in all three datasets. The crude cumulative 7-year re-revision rate for any reason following revision THA with Gentamicin ABLC use was 11.8%, with Gentamicin + Clindamycin ABLC use 13.1% and with Erythromycin + Colistin ABLC use 14.8% (ns). The crude cumulative 9-year re-revision rate for any reason following revision TKA with Gentamicin ABLC use was 17.7% and with Gentamicin + Clindamycin ABLC use 16.5% (ns). Conclusions. In conclusion, we could not show a difference in revision rate for hip fracture arthroplasty or re-revision rates for revision hip- or knee arthroplasty with the use of dual ABLC compared to single ABLC bone cement, with 7and 9 year follow up. The low percentage of dual ABLC in hip fracture arthroplasties in our registry do not enable us to make a reliable estimation of the added value in this patient category. The results of this study do not confirm the potential benefit of dual ABLC use in revision cases

Introduction. The purpose of this study was to demonstrate the feasibility of passively collecting objective data from a commercially available smartphone-based care management platform (sbCMP) and robotic assisted total knee arthroplasty (raTKA). Methods. Secondary data analysis was performed using de-identified data from a commercial database that collected metrics from a sbCMP combined with intraoperative data collection from raTKA. Patients were included in this analysis if they underwent unilateral raTKA between July 2020 and February 2021, and were prescribed the sbCMP (n=131). The population consisted of 76 females and 55 males, with a mean age of 64 years (range, 43 – 81). Pre-operative through six-week post-operative data included step counts from the sbCMP, as well as administration of the KOOS JR. Intraoperative data included surgical times, the hip-knee-ankle angle (HKA), and medial and lateral laxity assessments from the robotic assessment. Data are presented using descriptive statistics. Comparisons were performed using a paired samples t-test, or Wilcoxon Signed-rank test, with significance assessed at p<0.05. A minimal detectable change (MDC) in the KOOS JR score was considered ½ standard deviation of the preoperative values. Results. KOOS JR scores improved from a preoperative mean of 51.5 ± 11.5 to a 6-week postoperative mean of 64 ± 10.04 (p<0.001). An MDC of 5.75 units was achieved. Step counts decreased initially and returned to preoperative values by week 6 (Figure 1, p=0.196). When evaluating time requirements from landmarking to completed surgical cuts, the median surgical time was 40.2 minutes (IQR, 29.4 – 52.0). The median absolute deformity for HKA preoperatively was 6.9 degrees (IQR, 4.1 – 10.1) and the final intraoperative median HKA was 0.9 degrees (IQR, 0.1 – 3, p<0.001). There was a difference in medial and lateral joint laxity in flexion and extension at the initial intraoperative evaluation (p<0.01). At the final evaluation there was no difference in medial and lateral joint laxity in extension (p=0.239); however, a slight difference in flexion was noted (p=0.001). Given the median values of 1.2mm (0.8 – 2.4) medially vs. 1.4mm (0.9 – 3) laterally, this difference is not likely clinically relevant. Patients who had <1 mm of medial laxity in flexion had significantly fewer step counts at week 6 post-operatively (p=0.035). There was no difference in KOOS JR scores associated with tightness (p>0.05). Discussion. The use of passively collected objective measures in a commercial database across the episode of care was feasible and demonstrated associations between intraoperative and post-operative metrics. To our knowledge, this is the first integrated data collection and reporting platform to report on these measures in a commercial population. Future research is needed in order to understand the benefit of displaying these metrics, as well as the role of variations in alignment and gap balance on function. Conclusions. Contemporary data platforms may be used to improve the understanding of individual recovery paths through real-time passive data collection throughout the episode of care. While clinically important improvements in Oxford Shoulder Scores have been defined for patients with general shoulder problems or those undergoing subacromial decompression, no threshold has been reported for classifying improvement after shoulder replacement surgery. This study aimed to establish the minimal clinically important change (MCIC) for the Oxford Shoulder Score in patients undergoing primary total shoulder replacement (TSR). Patient-reported outcomes data were sourced from the Australian Orthopaedic Association National Joint Replacement Registry Patient-Reported Outcome Measures Program. These included pre- and 6-month post-operative Oxford Shoulder Scores and a rating of patient-perceived change after surgery (5-point scale ranging from ‘much worse’ to ‘much better’). Two anchor-based methods (using patient-perceived improvement as the anchor) were used to calculate the MCIC: 1) mean change method; and 2) predictive modelling, with and without adjustment for the proportion of improved patients. The analysis included 612 patients undergoing primary TSR who provided pre- and post-operative data (58% female; mean (SD) age 70 (8) years). Most patients (93%) reported improvement after surgery. The MCIC derived from the mean change method was 6.8 points (95%CI 4.7 to 8.9). Predictive modelling produced an MCIC estimate of 11.6 points (95%CI 8.9 to 15.6), which reduced to 8.7 points (95%CI 6.0 to 12.7) after adjustment for the proportion of improved patients. For patient-reported outcome measures to provide valuable information that can support clinical care, we need to understand the magnitude of change that matters to patients. Using contemporary psychometric methods, this analysis has generated MCIC estimates for the Oxford Shoulder Score. These estimates can be used by clinicians and researchers to interpret important changes in pain and function after TSR from the patient's perspective. While clinically important improvements in Oxford Shoulder Scores have been defined for patients with general shoulder problems or those undergoing subacromial decompression, no threshold has been reported for classifying improvement after shoulder replacement surgery. This study aimed to establish the minimal clinically important change (MCIC) for the Oxford Shoulder Score in patients undergoing primary total shoulder replacement (TSR). Patient-reported outcomes data were sourced from the Australian Orthopaedic Association National Joint Replacement Registry Patient-Reported Outcome Measures Program. These included pre- and 6-month post-operative Oxford Shoulder Scores and a rating of patient-perceived change after surgery (5-point scale ranging from ‘much worse’ to ‘much better’). Two anchor-based methods (using patient-perceived improvement as the anchor) were used to calculate the MCIC: 1) mean change method; and 2) predictive modelling, with and without adjustment for the proportion of improved patients. The analysis included 612 patients undergoing primary TSR who provided pre- and post-operative data (58% female; mean (SD) age 70 (8) years). Most patients (93%) reported improvement after surgery. The MCIC derived from the mean change method was 6.8 points (95%CI 4.7 to 8.9). Predictive modelling produced an MCIC estimate of 11.6 points (95%CI 8.9 to 15.6), which reduced to 8.7 points (95%CI 6.0 to 12.7) after adjustment for the proportion of improved patients. For patient-reported outcome measures to provide valuable information that can support clinical care, we need to understand the magnitude of change that matters to patients. Using contemporary psychometric methods, this analysis has generated MCIC estimates for the Oxford Shoulder Score. These estimates can be used by clinicians and researchers to interpret important changes in pain and function after TSR from the patient's perspective. We conclude that an increase in Oxford Shoulder Scores of at least 9 points can be considered a meaningful improvement in shoulder-related pain and function after TSR

Aim. The aim of this investigation was to compare risk of infection in both cemented and cementless hemiarthroplasty (HA) as well as total hip arthroplasty (THA) following femoral neck fracture. Methods. Data collection was performed using the German Arthroplasty Registry (EPRD) In HA and THA following femoral neck fracture fixation method was divided into cemented and cementless protheses and paired according to age, sex, body mass index (BMI), and the Elixhauser score using Mahalanobis distance matching. Results. Overall in 13,612 cases of intracapsular femoral neck fracture, with 9,110 (66.9 %) HAs and 4502 (33.1 %) THAs were analyzed. Infection rate in HA was significantly reduced in cases with use of antibiotic-loaded cement compared to cementless fixated prosthesis (p=0.013). In patients with THA no statistical difference between cemented and cementless prothesis was registered, however after one year 2.4 % of infections were detected in cementless and 2.1 % in cemented THA. In the subpopulation of HA after one year 1.9 % of infections were registered in cemented and 2.8 % in cementless HA. BMI (p=0.001) and Elixhauser-Comorbidity-Score (p<0.003) were identified as risk factors of PJI, while in THA also cemented prosthesis demonstrated within the first 30 days an increased risk (HR=2.728; p=0.010). Conclusion. The rate of infection after intracapsular femoral neck fracture was significantly reduced in patients treated by antibiotic-loaded cemented hemiarthroplasty. In particular for patients with multiple risk factors for the development of a PJI the usage of antibiotic-loaded bone cement seems to be a reasonable procedure for prevention of infection

Introduction. Dual mobility (DM) total hip arthroplasty (THA) prostheses are designed to increase stability. In the setting of primary and revision THA, DM THA are used most frequently for dysplasia and instability diagnoses, respectively. As the use of DM THA continues to increase, with 8,031 cases logged in the American Joint Replacement Registry from 2012–2018, characterizing in vivo damage and clinical failure modes are important to report. Methods. Under IRB-approved implant retrieval protocol, 43 DM THA systems from 41 patients were included. Each DM THA component was macroscopically examined for standard damage modes. Clinically-relevant data, including patient demographics and surgical elements, were collected from medical records. Fretting and corrosion damage grading is planned, according to the Goldberg et al. classification system. Results. In this 43-retrieved implant series, there were 23 female and 17 male patients (n=1, unknown), with an average body mass index of 29 (range, 19–49), and average ages at index and revision of 63 years (range, 34–80) and 64 years (range, 38–88), respectively. The average duration of implantation was 12.9 months (range, 0.1–72.0). Reasons for revision included infection (n=11, 26%), mechanical complication (n=10, 23%), intraprosthetic dislocation (n=6, 14%), periprosthetic fracture (n=5, 12%), pain (n=4, 9%), acetabular-associated loosening (n=3, 7%), unknown (n=3, 7%), hematoma (n=2, 5%), leg length discrepancy (n=1, 2%), and inflammatory reaction (n=1, 2%); some cases included multiple reasons for revision. On articular surfaces, scratching was the most commonly observed damage mode on all components, with more than 40% of acetabular cup and femoral heads showing scratching damage (Figure 1A). Abrasion, burnishing, and pitting damage were also observed in more than 10% of acetabular cup and acetabular liner components; further, approximately 20% of polyethylene acetabular liners exhibited edge deformation damage. On backside surfaces, polyethylene acetabular liners showed the greatest damage, with more than 60% of components exhibiting abrasion, scratching, or pitting damage (Figure 1B). Conclusion. This series showed various reasons for revision as well as in vivo damage of retrieved DM systems following short-to-midterm implantation. Damage was observed on both articular and backside surfaces of the five components of DM THA. Modularity of DM THA prostheses may amplify rates of in vivo damage. Mortality following hip arthroplasty is affected by a large number of confounding variables each of which must be considered to enable valid interpretation. The aim of this study was to establish whether it is possible to determine a true cause-and-effect relationship between the risk of mortality and data that are routinely collected by the NJR and to establish the degree to which variation in the mortality rate could be explained by each variable. Relevant variables available from the 2011 NJR data setwere included in a Cox model. We carried out two analyses:. Firstly, we conducted an analysis of data collected from the NJR data set used in preparation of the NJR's 8th Annual Report (2011) looking for an association between the variables collected and the risk of mortality. Secondly, as social deprivation is also known to influence mortalityrates but is not routinely collected as part of the NJR data set, a further analysis was performed which included social deprivation data derived from partial postcodes. Mortality rates in hip arthroplasty patients were lower than in the age matched population across all hip types. Age at surgery, ASA grade, diagnosis, gender, provider type, hip type and lead surgeon grade all had a significant effect on mortality. Schemper's statistic showed that only 18.98% of the variation in mortality was explained by the variables available in the NJR data set. It is inappropriate to use Registry data to study an outcome affected by a multitude of confounding variables when these cannot be adequately accounted for in the available data set

Hospital Episode Statistics [HES] are often used by hospital managers and politicians as a reflection of departmental workload. The accuracy of these data is often questioned. We aimed to ascertain the reliability of this database for trauma admissions. Between 2002 and 2003, all admissions were recorded by doctors using a separate departmental database. Data were collected during the daily trauma meetings and compared with the HES returns for the same period. 2496 patients were recorded in the trauma admissions database. Overall, 36.4% of the patients were either not recorded by the HES database or wrongly coded in terms of type of admissions or diagnosis. HES data for all 2496 records was analysed by type of admissions and speciality.4.2% of trauma patients were incorrectly classified as elective or day cases. 2.9% of trauma patients admitted to hospital were not recorded in the HES data as orthopaedic admission. The accuracy of HES diagnosis coding was tested on 300 records randomly selected by a statistical package. HES recorded the wrong diagnosis in 29.3% of cases. A significant number of trauma cases were not counted in the HES data. This may have significant implications for trauma funding. HES data does not accurately record diagnosis and therefore can not be used as a research tool for specific injuries. Data recording practice should be changed to improve HES data accuracy

Up to 40% of patients are using opioids at the time of joint replacement surgery in the USA despite emerging evidence suggesting opioids are ineffective for chronic non-cancer pain. Our primary objective was to determine if preoperative opioid use among patient awaiting total knee arthroplasty (TKA) was associated with worse patient-reported outcomes (PRO) measures at one-year follow-up when compared to non-opioid users, after adjusting for age, gender, and comorbidities. The study linked Alberta's Pharmacy Information Network (PIN) data with prospectively collected Alberta Bone and Joint Health Repository administrative data (medical and PRO data) for patients who underwent primary TKA in Alberta from 2013–2015. The PIN contains prescribing information from physician offices and pharmacies across Alberta. Preoperative ‘opioid users’ were defined as having 90-days of consistent opioid use in the 180-days prior TKA, and ‘opioid-exposed’ subjects had recorded opioid prescriptions in the 180-days prior to TKA, but did not meet the definition of an opioid user. Those with no opioid-exposure in the 180-days pre-TKA were deemed a ‘non-opioid user’. We used multiple linear regression to examine how preoperative opioid use (opioid user, opioid-exposed, non-opioid user) impacted Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) pain and function scores at one year after TKA after adjusting for confounding variables. These included age, sex, preoperative WOMAC scores, comorbidities including depression, diabetes, obesity, stroke, pulmonary disease, renal disease, cardiac disease, liver disease, and overall comorbid burden. Of the 2182 unique cases identified, 151 (7%) were opioid users, 527 (24%) opioid-exposed and 1504 (69%) non-opioid user. Opioid users were more likely to be prescribed strong opioids (e.g., hydromorphone, oxycodone) compared to with opioid-exposed subjects (p < 0 .001) and had a median morphine equivalent dose of 30.7 mg/day compared with the opioid-exposed group (2.2 mg/day, p < 0 .001) in the 180-days prior to TKA. Opioid users, in the 180-days prior to TKA had an active opioid prescription for a mean duration of 153 days (95CI 149, 157) within the 180-days prior to TKA, compared to 34 days (95CI 32, 37) for opioid-exposed (p < 0 .001). In the parsimonious pain and function models, opioid use, lower preoperative WOMAC score, depression, and obesity were associated with worse one-year pain and function. Patients prescribed preoperative opioids had worse WOMAC scores one-year after TKA respectively when compared to non-opioid users, after adjusting for other factors (opioid user pain score: −9.5, function score: −9.4, opioid exposed pain score: −2.6, function score: −3.6, p < 0 .001 for all). Further, opioid users with a concomitant diagnosis of depression had significantly worse one-year postoperative WOMAC scores when compared to non-depressed non-opioid users (scores −14, p < 0 .001 for both pain and function). In Alberta, 31% of patients were prescribed opioids within 180-days before TKA, preoperative opioid use was associated with worse one-year postoperative WOMAC pain and function scores relative to non-opioid users. Our results suggest that strategies to reduce preoperative opioid use could improve patient outcomes after TKA, and support the most recent Canadian opioid prescribing guidelines that attempt to minimize opioid use for chronic conditions such as arthritis

Background and aim. Since the market withdrawal of the ASR hip resurfacing in August 2010 because of a higher than expected revision rate as reported in the Australian Joint Replacement Registry (AOAJRR), metal-on-metal hip resurfacing arthroplasty (MoMHRA) has become a controversial procedure for hip replacement. Failures related to destructive adverse local tissue reactions to metal wear debris have further discredited MoMHRA. Longer term series from experienced resurfacing specialists however, demonstrated good outcomes with excellent 10-to-15-year survivorship in young and active men. These results have recently been confirmed for some MoMHRA designs in the AOAJRR. Besides, all hip replacement registries report significantly worse survivorship of total hip arthroplasty (THA) in patients under 50 compared to older ages. The aim of this study was to review MoMHRA survivorship from the national registries reporting on hip resurfacing and determine the risk factors for revision in the different registries. Methods. The latest annual reports from the AOAJRR, the National Joint Registry of England and Wales (NJR), the Swedish Hip Registry (SHR), the Finnish Arthroplasty Registry, the New Zealand Joint Registry and the Arthroplasty Registry of the Emilia-Romagna Region in Italy (RIPO) were reviewed for 10-year survivorship of MoMHRA in general and specific designs in particular. Other registries did not have enough hip resurfacing data or long term data yet. The survivorship data were compared to conventional THA in comparable age groups and determinants for success/failure such as gender, age, diagnosis, implant design and size and surgical experience were reviewed. Results. All registries showed a significant decline of the use of MoMHRA. The AOAJRR reported a cumulative revision rate of 9.5% (95%CI: 8.9–10.1%) at 10 years for all hip resurfacings. Female gender, developmental dysplasia and femoral head sizes <49mm were significant risk factors with revision rates twice as high for head sizes <49mm compared to >55mm. In males, cumulative revision rate for all MoMHRA was 6.6% at 10 years and 7.8% at 13years with no difference in the age groups. ASR had significantly higher revision rates (23.9% at 7 years) compared to other designs. The Adept and the Mitch had the lowest revision rates at 7 years (3.6%). Cumulative revision rates for 10,750 BHR (males and females pooled) were 5.0% at 7 years, 6.9% at 10years and 8.4% at 13 years. Cumulative revision rates of THA in patients <55 years was 6.0% at 10 years and 9.4% at 13 years. Similar survivorship results were found in the Finnish, Swedish, New Zealand and RIPO registry. In the SHR, 10-year survivorship of THA in patients <50 was only around 87%. In the NJR, cumulative revision rates for all MoMHRA pooled were much less favourable (13% at 10 years – 22% in patients<50) but the revision probability of ASR was 30.4% compared to 9.0% for BHR. Surgical experience was also identified as an important determinant of success/failure. Discussion. Registries are now confirming good 10-year survivorship of certain MoMHRA designs with excellent results in males. Risk factors for revision are female gender, small head size, dysplasia and certain implant designs
