A consecutive series of patients with a hydroxyapatite-coated uncemented total knee replacement (TKR) performed by a single surgeon between 1992 and 1995 was analysed. All patients were invited for clinical review and radiological assessment. Revision for aseptic loosening was the primary outcome. Assessment was based on the Knee Society clinical score (KSS) and an independent radiological analysis. Of 471 TKRs performed in 356 patients, 432 TKRs in 325 patients were followed for a mean of 16.4 years (15 to 18). The 39 TKRs in 31 patients lost to follow-up had a mean KSS of 176 (148 to 198) at a mean of ten years. There were revisions in 26 TKRs (5.5%), of which 11 (2.3%) were for aseptic loosening. Other further surgery was carried out on 49 TKRs (10.4%) including patellar resurfacing in 20, arthrolysis in 19, manipulation under anaesthetic in nine and extensor mechanism reconstruction in one.

Survivorship at up to 18 years without aseptic loosening was 96% (95% confidence interval 91.9 to 98.1), at which point the mean KSS was 176 (134 to 200). Of 110 knees that underwent radiological evaluation, osteolysis was observed in five (4.5%), one of which was revised.

These data indicate that uncemented hydroxyapatite-coated TKR can achieve favourable long-term survivorship, at least as good as that of cemented designs.

The optimal management of the tibial slope in achieving a high flexion angle in posterior-stabilised (PS) total knee replacement (TKR) is not well understood, and most studies evaluating the posterior tibial slope have been conducted on cruciate-retaining TKRs. We analysed pre- and post-operative tibial slope differences, pre- and post-operative coronal knee alignment and post-operative maximum flexion angle in 167 patients undergoing 209 TKRs. The mean pre-operative posterior tibial slope was 8.6° (1.3° to 17°) and post-operatively it was 8.0° (0.1° to 16.7°). Multiple linear regression analysis showed that the absolute difference between pre- and post-operative tibial slope (p < 0.001), post-operative coronal alignment (p = 0.02) and pre-operative range of movement (p < 0.001) predicted post-operative flexion. The variance of change in tibial slope became larger as the post-operative maximum flexion angle decreased. The odds ratio of having a post-operative flexion angle < 100° was 17.6 if the slope change was > 2°. Our data suggest that recreation of the anatomical tibial slope appears to improve maximum flexion after posterior-stabilised TKR, provided coronal alignment has been restored.

Aims. The primary objective of this study was to compare migration of the cemented ATTUNE fixed bearing cruciate retaining tibial component with the cemented Press-Fit Condylar (PFC)-sigma fixed bearing cruciate retaining tibial component. The secondary objectives included comparing clinical and radiological outcomes and Patient Reported Outcome Measures (PROMs). Methods. A single blinded randomized, non-inferiority study was conducted including 74 patients. Radiostereometry examinations were made after weight bearing, but before hospital discharge, and at three, six, 12, and 24 months postoperatively. PROMS were collected preoperatively and at three, six, 12, and 24 months postoperatively. Radiographs for measuring radiolucencies were collected at two weeks and two years postoperatively. Results. The overall migration (mean maximum total point motion (MPTM)) at two years was comparable: mean 1.13 mm (95% confidence interval (CI), 0.97 to 1.30) for the ATTUNE and 1.16 mm (95% CI, 0.99 to 1.35) for the PFC-sigma. At two years, the mean backward tilting was -0.43° (95% CI, -0.65 to -0.21) for the ATTUNE and 0.08° (95% CI -0.16 to 0.31), for the PFC-sigma. Overall migration between the first and second postoperative year was negligible for both components. The clinical outcomes and PROMs improved compared with preoperative scores and were not different between groups. Radiolucencies at the implant-cement interface were mainly seen below the medial baseplate: 17% in the ATTUNE and 3% in the PFC-sigma at two weeks, and at two years 42% and 9% respectively (p = 0.001). Conclusion. In the first two postoperative years the initial version of the ATTUNE tibial component was not inferior with respect to overall migration, although it showed relatively more backwards tilting and radiolucent lines at the implant-cement interface than the PFC-sigma. The version of the ATTUNE tibial component examined in this study has subsequently undergone modification by the manufacturer. Level of Evidence: 1 (randomized controlled clinical trial). Abstract. Introduction. Total knee replacement (TKR) in patients with skeletal dysplasia is technically challenging surgery due to deformity, joint contracture, and associated co-morbidities. The aim of this study is to follow up patients with skeletal dysplasia following a TKR. Methodology. We retrospectively reviewed 22 patients with skeletal dysplasia who underwent 31 TKRs at our institution between 2006 and 2022. Clinical notes, operative records and radiographic data were reviewed. Results. Achondroplasia was the most common skeletal dysplasia (8), followed by Chondrodysplasia punctata (7) and Spondyloepiphyseal dysplasia (5). There were fourteen men and eight women with mean age of 51 years (28 to 73). The average height of patients was 1.4 metres (1.16–1.75) and the mean weight was 64.8 Kg (34.3–100). The mean follow up duration was 68.32 months (1–161). Three patients died during follow up. Custom implants were required in twelve patients (38.71%). Custom jigs were utilised in six patients and two patients underwent robotic assisted surgery. Hinged TKR was used in seventeen patients (54.84%), posterior stabilised TKR in nine patients (29.03%), and cruciate retaining TKR in five patients (16.13%). One patient underwent a patella resurfacing for persistent anterior knee pain and another had an intra-operative medial tibial plateau fracture which was managed with fixation. No revisions occurred during the follow up period. Conclusion. Despite the technical challenges and complexity of TKR within this unique patient group, we demonstrate good implant survivorship during the study period. Cross sectional imaging is recommended preoperatively for precise planning and templating

Aims. Conflicting clinical results are reported for the ATTUNE Total Knee Arthroplasty (TKA). This randomized controlled trial (RCT) evaluated five-year follow-up results comparing cemented ATTUNE and PFC-Sigma cruciate retaining TKAs, analyzing component migration as measured by radiostereometric analysis (RSA), clinical outcomes, patient-reported outcome measures (PROMs), and radiological outcomes. Methods. A total of 74 primary TKAs were included in this single-blind RCT. RSA examinations were performed, and PROMs and clinical outcomes were collected immediate postoperatively, and at three, six, 12, 24, and 60 months’ follow-up. Radiolucent lines (RLLs) were measured in standard anteroposterior radiographs at six weeks, and 12 and 60 months postoperatively. Results. At five-year follow-up, RSA data from 61 patients were available and the mean maximum total point motion (MTPM) of the femoral components were: ATTUNE: 0.96 mm (95% confidence interval (CI) 0.79 to 1.14) and PFC-Sigma 1.37 mm (95% CI 1.18 to 1.59) (p < 0.001). The PFC-Sigma femoral component migrated more in the first postoperative year, but stabilized thereafter. MPTM of the tibial components were comparable at five-year follow-up: ATTUNE 1.12 mm (95% CI 0.95 to 1.31) and PFC-Sigma 1.25 mm (95% CI 1.07 to 1.44) (p = 0.438). RLL at the medial tibial implant-cement interface remained more prevalent for the ATTUNE at five-year follow-up compared to the PFC-Sigma (20% vs 3%). RLL did not progress over time, and varied between patients at different timepoints for both TKA systems. Clinical outcomes and PROMs improved compared with preoperative scores, and were not different between groups. Conclusion. MTPM migration at five-year follow-up of the femoral and tibial component of the ATTUNE were similar and as low as that of the PFC-Sigma. MTPM migration of both knee implants did not significantly change from one year post-surgery, indicating stable fixation. Long-term ATTUNE performance may be expected to be comparable to the clinically well-performing PFC-Sigma. We have not found evidence of increased tibial component migration as measured by RSA to support concerns about cement debonding and a higher risk of aseptic loosening with the ATTUNE TKA. Aims. Total knee arthroplasty is an established treatment for knee osteoarthritis with excellent long-term results, but there remains controversy about the role of uncemented prostheses. We present the long-term results of a randomized trial comparing an uncemented tantalum metal tibial component with a conventional cemented component of the same implant design. Methods. Patients under the age of 70 years with symptomatic osteoarthritis of the knee were randomized to receive either an uncemented tantalum metal tibial monoblock component or a standard cemented modular component. The mean age at time of recruitment to the study was 63 years (50 to 70), 46 (51.1%) knees were in male patients, and the mean body mass index was 30.4 kg/m. 2. (21 to 36). The same cruciate retaining total knee system was used in both groups. All patients received an uncemented femoral component and no patients had their patella resurfaced. Patient outcomes were assessed preoperatively and postoperatively using the modified Oxford Knee Score, Knee Society Score, and 12-Item Short-Form Health Survey questionnaire (SF-12) score. Radiographs were analyzed using the American Knee Society Radiograph Evaluation score. Operative complications, reoperations, or revision surgery were recorded. A total of 90 knees were randomized and at last review 77 knees were assessed. In all, 11 patients had died and two were lost to follow-up. Results. At final review all patients were between 11 and 15 years following surgery. In total, 41 of the knees were cemented and 36 uncemented. There were no revisions in the cemented group and one revision in the uncemented group for fracture. The uncemented group reported better outcomes with both statistically and clinically significant (p = 0.001) improvements in knee-specific Oxford and Knee Society scores compared with the cemented group. The global SF-12 scores demonstrated no statistical difference (p = 0.812). Uncemented knees had better radiological analysis compared with the cemented group (p < 0.001). Conclusion. Use of an uncemented trabecular metal tibial implant can afford better long-term clinical outcomes when compared to cemented tibial components of a matched design. Summary. We report a large study of 331 patients at two years post operation who were prospectively randomised to receive either a rotating platform or a fixed bearing knee replacement of an otherwise identical design. Introduction. The mobile bearing total knee replacement was developed as there are theoretical benefits in that it may allow a better range of motion, better patella tracking and lower wear rates. This study was designed to see if these potential advantages are borne out in practice when using a cruciate retaining design. Methods. 331 patients undergoing TKR surgery were randomised to receive either a fixed bearing (170 patients) or a mobile bearing (161 patients). The femoral implant design was identical in the two groups. The tibial polyethylene bearing was either fixed in the metallic tibial tray (FB) or of rotating platform design (RP). All patients were assessed pre-operatively and at two years post-operatively using standard tools (Oxford, AKSS, Patellar Score) by independent nurse specialists. Results. The groups who received the FB and the RP implants have been assessed and their pre-operative to two year outcomes analysed with regard to the improvement in the range of motion (9.1 v. 10.2 degrees), Oxford Knee Score (-19.2 v. -17.6) and American Knee Society Knee and Function scores (51.3 v. 49.5 and 25.3 v. 23.6) at two years follow up. Conclusion. This large study shows that there is no statistical difference between a FB and a RP cruciate retaining TKR at two years post operation

Background. In recent years, the use of modern cementless implants in total knee arthroplasty has been increasing in popularity. These implants take advantage of new technologies such as additive manufacturing and potentially provide a promising alternative to cemented implant designs. The purpose of this study was to compare implant migration and tibiofemoral contact kinematics of a cementless primary total knee arthroplasty (TKA) implanted using either a gap balancing (GB) or measured resection (MR) surgical technique. Methods. Thirty-nine patients undergoing unilateral TKA were recruited and assigned based on surgeon referral to an arthroplasty surgeon who utilizes either a GB (n = 19) or a MR (n = 20) surgical technique. All patients received an identical fixed-bearing, cruciate-retaining beaded peri-apatite coated cementless femoral component and a pegged highly porous cementless tibial baseplate with a condylar stabilizing tibial insert. Patients underwent a baseline radiostereometric analysis (RSA) exam at two weeks post-operation, with follow-up visits at six weeks, three months, six months, and one year post-operation. Migration including maximum total point motion (MTPM) of the femoral and tibial components was calculated over time. At the one year visit patients also underwent a kinematic exam using the RSA system. Results. Mean MTPM of the tibial component at one year post-operation was not different (mean difference = 0.09 mm, p = 0.980) between the GB group (0.85 ± 0.37 mm) and the MR group (0.94 ± 0.41 mm). Femoral component MTPM at one year post-operation was also not different (mean difference = 0.27 mm, p = 0.463) between the GB group (0.62 ± 0.34 mm) and the MR group (0.89 ± 0.44 mm). Both groups displayed a lateral pivot pattern with similar frequencies of condylar separation. Conclusion. There was no difference in implant migration and kinematics of a single-radius, cruciate retaining cementless TKA performed using a GB or MR surgical technique. The magnitude of migration suggests there is no risk of early loosening. The results provide support for the use of a cementless TKA as a viable alternative to cemented fixation

Introduction. Inability to reproduce 6-degrees of freedom (6DOF) kinematics, abnormal “paradoxical” anterior femoral translation and loss of normal medial pivot rotation are challenges associated with contemporary posterior cruciate retaining and posterior stabilized total knee arthroplasty (TKA). The removal of the anterior and/or both cruciate ligaments in CR/PS TKA, leading to significant kinematic alteration of the knee joint, has been suggested as one of the potential contributory factors in patients remaining dissatisfied after TKA. Bi-cruciate retaining (BCR) TKA designs allow preservation of both anterior and posterior cruciate ligaments with the potential to replicate normal knee joint kinematics. Physically demanding tasks such as sit-to-stand (STS), and deep lunging may be more sensitive tools for investigating preserved kinematic abnormalities following TKA. This study aims to compare in-vivo kinematics between the operated and the contralateral non-operated knee in patients with contemporary BCR TKA design. Methods. Twenty-nine patients (14 male; 15 female, 65.7±7.7 years) unilaterally implanted with a contemporary BCR TKA design featuring an asymmetric femoral component and independently designed medial and lateral bearings were evaluated. Mean follow-up time after BCR TKA was 12.7±5.1 months. All patients received a computer tomography (CT) scan from the pelvis to the ankles for the creation of 3D surface models of both knees (BCR TKA and non-operated). Patients performed single leg deep lunges and sit-to-stand under a validated dual fluoroscopic imaging system (DFIS) surveillance. Each patient's 2D dynamic fluoroscopic images, corresponding 3D surface bone models (for contralateral non-operated knee) and computer aided design (CAD) implant models (for the BCR TKA implanted knee) were imported into a virtual DFIS environment in MATLAB. An optimization procedure was utilized to perform matching between the 3D surface bone models and the 2D fluoroscopic image outlines. In-vivo 6DOF kinematics of the BCR TKA knees and contralateral non-operated side were quantified and analyzed. Results. When performing the high-flexion lunge, BCR TKA knees demonstrated less average femoral posterior translation (13±4mm) during terminal flexion when compared to the contralateral non-operated knees (16.6±3.7mm) (p=0.001). Similarly, during STS, less femoral rollback was observed (11.6±4.5mm vs 14.4±4.6mm, p<0.04) in BCR TKA knees. Overall, BCR TKA knees partially reproduced a normal “screw-home” motion, demonstrating reduced internal rotation during several intervals of the cycles for strenuous flexion activities. BCR TKA knees demonstrated less internal rotation during high-flexion lunge (4±5.6° vs 6.5±6.1°, p=0.05). Similarly, during STS, less internal rotation was observed (4.5±6° vs 6.9±6.3°, p=0.04, p=0.02, p=0.01, p=0.02) in BCR TKA knees. Conclusion. The BCR TKA design demonstrated asymmetries in flexion-extension and internal-external rotation, suggesting that in-vivo tibiofemoral kinematic parameters are not fully restored in BCR patients during functionally strenuous activities such as single leg deep lunges and sit-to-stand. Further studies are required to elucidate the importance of patient factors, surgical component orientation and implant designs in optimizing in vivo kinematics in patients with BCR TKA. Introduction. Implant malalignment is an important predictor of prosthetic failure following total knee arthroplasty (TKA). The purpose of this study was to determine the incidence of outliers for common alignment targets and the impact of surgeon volume and experience on the accuracy of implant alignment with current generation manual instrumentation. Methods. This study was a retrospective, multi-center, radiographic analysis of 1675 consecutive primary uncomplicated TKAs from seven surgeons at three academic and state-funded centers in the US and UK. Surgeons were categorized as “high-volume” (≥50 TKAs/year) and “high-experience” (≥5 years post-fellowship). Femorotibial, tibial varus/valgus, and posterior tibial slope angles were digitally measured using postoperative radiographs. Femorotibial (<2° or >8° valgus), tibial (> ±3° deviation from the neutral axis), and tibial slope (<0° or >7° of flexion for cruciate retaining, <0° or >5° of flexion for posterior stabilized) angle outliers were identified. The proportion of outliers among surgeons in each subgroup was compared. Results. When comparing high-and low-volume surgeons, the proportion of femorotibial (12% vs. 19%, p <0.0001), posterior slope (17% vs. 28%, p <0.0001), and total outliers (12% vs. 19%, p <0.0001) was significantly lower in the high-volume group. Furthermore, the proportion of knees with well-aligned implants in all three measurements (69% vs. 53%, p <0.0001) was significantly higher in the high-volume group. When comparing high-and low-experience surgeons, the proportion of femorotibial (14% vs. 17%, p = 0.046), tibial (9% vs. 6%, p = 0.030), posterior slope (19% vs. 26%, p <0.0001), and total outliers (14% vs. 17%, p = 0.006) was higher in the low-volume group. Furthermore, the proportion of knees with well-aligned implants in all three measurements (64% vs. 58%, p = 0.008) was significantly higher in the high-experience group. Conclusions. Low surgeon volume and experience predispose to implant malalignment following TKA, with surgical volume bearing a greater influence on alignment accuracy. Even among high volume, high experience surgeons, outliers in at least one standard alignment target occur in over 30% of cases with current standard instrumentation

Introduction. Modern implant systems offer a variety of options to address the posterior cruciate ligament (PCL) and afford stability in primary total knee arthroplasty (TKA). One system has three bearing options for cruciate retaining (CR) TKA: standard CR bearing (CR-S) with 3° posterior slope and no posterior lip; CR lipped bearing (CR-L) with no slope and small posterior lip, and deep-dished anterior stabilized bearing (CR-AS) with large anterior build-up to compensates for deficient or lax PCL. We previously reported on 1940 patients (2449 CR-TKA) at early follow-up comparing these three CR bearings and found CR-AS bearings had greater improvement in ROM and lower rate of manipulation despite their use in patients with greater preoperative deformity and lower ROM. The purpose of this study was to review a broader series by a single surgeon-designer with minimum 2-year follow-up to determine if there were differences between bearing types in terms of clinical outcomes and survival. Methods. From 2003 to 2014, 3348 patients (4500 knees) underwent primary CR-TKA and were available for review with minimum 2-year follow-up. An algorithmic approach to constraint was employed, choosing the least amount necessary to afford stability. Bearings used were 3883 CR-S (86%), 256 CR-L (6%), and 361 CR-AS (8%). CR-S bearings were used in patients with less preoperative tibiofemoral deformity (p=0.0316), less flexion contracture (p<0.0001), and better ROM (p<0.0001), but more mediolateral instability (p<0.0001) than CR-AS bearings. Results. Mean follow-up overall was 5.7 years (range, 2–14). Overall 106 TKA (2.4%) have been revised (46 fully, 56 partial, 4 unknown). Indications for revision were instability in 40 (all CR-S), 28 infection, 10 aseptic loosening, 8 arthrofibrosis, 7 periprosthetic fracture, 3 extensor related, 2 heterotopic ossification, 3 pain, and 5 other or unknown. Chi-squared analysis showed no difference between bearing groups for overall revision (p=0.1724). However, revision for instability was greater for CR-S (1.0%) versus CR-AS bearings (p=0.0454, Fisher's exact). Frequency of manipulation was no different between bearing groups (p=0.8512). CR-AS patients had greater improvement than CR-S patients in ROM (4.4° versus 1.6°, p=0.0003), and Knee Society pain (39.3 versus 35.9, p=0.0002), clinical (53.5 versus 47.0, p<0.0001), and function (18.9 versus 16.0, p=16.0). CR-L patients had greater improvement than CR-S in pain (38.2, p=0.0367) and clinical score (53.6, p<0.0001). Discussion. In this large single surgeon series, Kaplan-Meier survival free of any revision was similar between bearing groups and excellent overall with 96.0% (±0.49%) at 14.1 years. No revisions for instability were observed with CR-L or CR-AS bearings, but were the most frequent cause of revision with CR-S bearings. Manipulation rates were similar between bearing groups. ROM, pain, clinical, and functional improvements were greater with a deep-dished, anterior-stabilized bearing

Introduction. Cementless total knees were historically associated with early failure. These failures, likely associated with implant design, made cemented total knee arthroplasty (TKA) the “gold standard”. Manufacturers have introduced newer uncemented technologies that provide good initial stability and utilize a highly-porous substrates for bony in-growth. Outcome data on these implants has been limited. In addition, these implants typically have a price premium which makes them difficult to use in the setting of cost containment and in at risk 90-day bundles. Our purpose was to compare 90-day outcomes of a new uncemented implant with those of a comparable cemented implant from the same manufacturer. We hypothesized that the implants would have equivalent 90-day clinical and economic outcomes. Methods. Ninety-day clinical and economic outcomes for 252 patients with prospectively collected data from the Michigan Arthroplasty Registry Collaborative Quality Initiative (MARCQI) database were reviewed. Ninety-day outcomes were compared between uncemented knees and an age-matched group of cemented knees (Triathlon cemented vs uncemented Triathalon-tritanium, Stryker Orthopedics, Mahwah, NJ, USA). Both cruciate retaining and posterior stabilized designs were included. MARCQI data: demographics, co-morbidities, length of stay, complications, emergency department visits, discharge disposition, and readmissions were reviewed. Financial data provided by the hospital's finance department was used for economic comparisons. Fischer's test was done to assess categorical data and a student's t-test was used to compare numerical data. Results. Uncemented knees had shorter length of stay (1.58 vs. 1.87, p<0.0001), were more frequently discharged home (90.48% vs. 68.75%; p<0.0001) and used less home care or extended care facilities (6.35% vs. 19.14%, p<0.0001; 2.78% vs. 11.72%, p=0.0001). More uncemented knees had “no complications”. Moreover, there were no reoperations in uncemented knees, compared to 19 reoperations in cemented knees most being manipulations (14 vs. 0, p=0.0028). Uncemented knees scored better than age matched counterparts Knee Injury and Osteoarthritis Outcomes Score (63.69 vs. 47.10, n=85 and 43, p<0.0001), and Patient Reported Outcomes Measurement Information System T-physical and T-mental (44.12 vs. 39.45, n=95 and 59, p<0.0001; 51.84 vs 47.82, n=97 and 59, p=0.0018). Cemented cases were more expensive overall, the surgical costs were higher ($6806.43 vs. $5710.78 p<0.01) and the total hospital costs were higher ($8347.65 vs. $7016.11 p<0.01). The 90-day readmission and hospital outpatient costs were not significantly different between the designs. Conclusion. The use of a modern uncemented TKA implants has increased, but data on outcomes and the economic impact has been limited particularly in regard to 90-day at risk global periods. Our study suggests that patients receiving an new uncemented TKA have a shorter length of stay, higher rate of discharge to home, better patient reported outcome measures, fewer complications and fewer reoperations than an age-matched group of patients receiving a similar, cemented design during the 90-day global period. Importantly, the uncemented knees had $1,095 less surgical episode costs (p< 0.001) and a 90-day cost savings of over $1,300 (p< 0.001). Uncemented TKA, when utilizing modern technologies, is successful and economically viable for an at-risk bundle. The results of this study should alleviate fears increased cost, early failure, complications or poor outcomes with the use of a modern uncemented TKA

Introduction. This study explores whether subjects with bicruciate retaining TKRs (BiCR) have more normal knee biomechanics during level walking and stair ascent than subjects with posterior cruciate retaining TKRs (PCR). Due to anterior cruciate ligament (ACL) preservation, we expect BiCR subjects will not show the reduced flexion and altered muscle activation patterns characteristic of persons with TKRs. Methods. Motion and electromyography (EMG) data were collected during level walking and stair climbing for 16 BiCR subjects (4/12 m/f, 65±3 years, 30.7±7.0 BMI, 8/8 R/L), 17 PCR subjects (2/15 m/f, 65±7 years, 30.4±5.1 BMI, 7/10 R/L), and 17 elderly healthy control subjects (8/9 m/f, 55±10 years, 25.8±4.0 BMI, 10/7 R/L), using the point cluster marker set. Surface EMG electrodes were placed on the vastus medialis obliquus (VMO), rectus femoris (RF), biceps femoris (BF), and semitendinosus (ST) muscles. EMG data are reported as percent relative voluntary contraction (%RVC), normalized to the average peak EMG signals during level walking. Statistical nonparametric mapping was used for waveform analysis. Results. Both TKR groups were older, and PCR subjects had higher BMI than control subjects (p≤0.020). The BiCR group walked slower and with shorter stride lengths than controls (p≤0.012). During level walking, BiCR subjects had less knee extension and posterior tibial displacement than controls (95–98%, 96–100% gait cycle, p=0.003, 0.001). PCR subjects showed higher flexion mid-stance than controls (36–44% gait cycle, p=0.001) and more external rotation (66–69% gait cycle, p=0.003). Both TKR groups had smaller extension moment peaks (PCR 5, 59–75, 96%, BiCR 61–78, 95–97% stance, p≤0.007, 0.003) than the control group. The BiCR group had smaller adduction and external rotation moment peaks (20–24%, 10–18% stance, p=0.003, 0.001) compared with controls. During stair climbing, BiCR subjects displayed more external tibial rotation (4–16% stance), more knee abduction (36–52% stance), and a lower adduction moment peak (24–34% stance) compared to healthy controls (p≤0.005). TKR subjects from both groups showed lower flexion moment peaks than controls (PCR 24–35%, BiCR 24–28% stance, p≤0.001, 0.004). For EMG, PCR subjects had more BF activity during stair ascent versus level walking than healthy subjects (56–74% stance, p≤0.001). Discussion. BiCR and PCR showed more similarities than expected. Both had altered kinematics and kinetics compared to controls, suggesting some intrinsic extensor mechanism weakness, possibly an aftereffect of osteoarthritis. The EMG results agreed accordingly, as both TKR groups showed (non-significant) decreased quadriceps activity during stair climbing. Interestingly, PCR subjects also had more BF activity during stair than healthy controls, a trend that is common for both TKR subjects and people with ACL deficiency. On the other hand, although BiCR subjects were significantly more externally rotated in early stance phase for stair climbing, their rotation patterns began to align more closely to those of the healthy control subjects at lower flexion angles where the ACL should come into play. In conclusion, ACL retention in TKRs does not correct the extensor mechanism deficits commonly found in TKR patients, although it has some effect on secondary knee kinematics and hamstring muscle activity

The aim of this study was to compare the long-term outcome from total knee replacement (TKR) in young versus old patients in terms of pain and functional outcome. We used our arthroplasty database which recorded prospectively pain and American Knee Society scores at regular intervals over ten years after TKR. The procedures used a modern, cemented resurfacing type cruciate retaining prosthesis. A cohort of young patients (≤55 years) were identified. A control group of patients > 56 was identified, matching for ASA, body mass index and underlying condition. Change over time was analysed using a factorial repeated measures ANOVA test, which allowed for investigation of difference between groups. 40 Knees in 26 patients were identified. 2 patients died prior to follow up, 2 were revised within the study period. (1 for infection at 2 years and one for change of poly at 7 years) and a further 4 were lost to follow up. 7 knees could not be matched and were excluded. This left a study group of 24 young and 24 older knees. Pain scores (p=0.025) and American Knee Society “Knee” (p<0.001) and “Function” (p<0.001) scores changed significantly over time. There were however no statistical differences over the 10 year period in pain (p=0.436) and knee performance (0.618) but overall function was higher throughout the period in the younger group (=0.004). Knee replacement in younger patients produces similar outcomes in terms of pain and function compared with older patients and TKR should not be withheld purely on account of age

High flexion designs are intended to provide a greater range of knee flexion and possibly improve flexion in stiff knees. This study assessed the effects of two implant designs. A posterior stabilised high flexion mobile bearing (MB) design vs a cruciate retaining standard fixed bearing (FB) design. The aim of this study was to assess whether implant design has an effect on the functional outcome one year after total knee arthroplasty (TKA). Methods. Ninety patients with knee osteoarthritis on the waiting list for unilateral TKA were recruited and randomly allocated to either the MB or FB group. Patients were assessed between one and four weeks before, and one year after TKA. Primary outcome was knee flexion during high flexion activities of daily living such as stair ascending and descending and squatting as measured using gait analysis. Knee flexion in long sitting using a manual goniometer and the WOMAC were also recorded. Two sample t-tests were used to investigate statistical differences between the two groups pre- and postoperatively. Results. Average age was 69 years. Thirty-three received the MB design and 39 the FB design. Age, gender balance and pre-operative flexion (112 and 113 degrees in the FB and MB groups respectively) were the same in both groups. There were no statistically significant differences in post-operative knee flexion during functional activities. Knee flexion in sitting and the stiffness and function components of the WOMAC were also similar between the two groups (p>0.05). However, post-operatively the WOMAC pain component was slightly higher in the MB group (4.2 vs 2.4 points, p<0.05). Conclusion. In our patient group with a mean pre-operative flexion of 112.7 degrees, the high flexion mobile bearing design did not improve knee flexion during high flexion functional activities of daily living

We studied a series of Endo-Modell(r) rotating hinge knee replacements (RHKRs) to determine indications, implant survival and complication rates. Case notes were audited for a consecutive series of 129 implants performed between 08/12/2002 and 30/01/2009. Indication for use of RHKR was complex primary arthroplasty in 37.8% and revision in 62.2%. For primary arthroplasty with hinge prosthesis, commonest indications were: collateral ligament insufficiency (44.4%); advanced RA (13.9%); supracondylar fracture (5.6%). Indications for revision RHK arthroplasty were: aseptic loosening (40.4%); ligamentous or soft tissue failure (14.0%); periprosthetic fracture (7%); infection (51%). Infection was proven in 21% with 54% of patients requiring a one stage and 46% two stage revision. For revision cases, 74% of primary prostheses were cruciate retaining PFC (Depuy) and in 5%, the primary was itself an Endo-Modell(r) RHKR. Mean time from index to revision procedures was 6.7 years (range 1 year – 23 years). Complications were: deep infection (6.1%) and non-fatal PE (1%). None developed clinically detectable DVT. Transfusion was required in 29 cases (for such cases, an average 3 units was given). 30-day mortality was 1%. For the revision cases, the average length of hospital admission was 11 days. Mean duration of follow up was 45 months (with a minimum of 21 days and maximum of 92 months). During this time 2 RHKRs failed. A total of 7 patients died during the period from complications unrelated to their surgery. 31 cases were lost to follow up. We conclude that in this series of Endo-Modell(r) rotating hinge knee arthroplasties, results are comparable with similar revision procedures. There was a low rate of prosthesis failure, DVT and PE

Introduction. We wished to quantify the influence of constraint and various principal indications upon functional outcome following aseptic first time revision knee arthroplasty. Methods. In this single centre, prospective study we have looked into the outcome of 175 total revision knee replacement performed between 2003 and 2008 at a minimum follow-up of 1 year. Data was refined to allow for examination exclusively of those cases where the level of constraint was increased from cruciate retaining or cruciate sacrificing knee replacement to either non linked constrained implant (condylar constrained) or linked constrained (rotating hinge) prosthesis. Results. At one year significant improvement (p<0.001) of WOMAC pain, function and pain score was seen. Significant improvement (p<0.05) was noted in physical functioning, role physical, bodily pain and social functioning components of SF-36 score. 69% were satisfied with the overall procedure. WOMAC function, pain and stiffness score was significantly worse for patients revised for instability (27%) as compared to that for aseptic loosening (46%). A significantly higher proportion of patients were satisfied with the procedure (p<0.001); had a better quality of life (p=0.004) and would have the surgery (p=0.005) again in the aseptic loosening group as compared to the instability group. Level of constraint did not make any statistically significant difference in the outcome within either group. The postoperative outcome scores for patients receiving two different levels of constraint was statistically not different. We found that increasing the level of constraint to linked constrained implant significantly (p<0.001) deteriorated the ability to descend from stairs (Figure 4). Conversely it had a positive influence (p=0.01) on ability to walk on flat. Conclusion. Patients undergoing revision surgery for instability exhibited poorer functional knee scores at 1 year. The degree of constraint within the revision construct did not significantly alter this change irrespective of aetiology

Introduction. Previous studies comparing cruciate retaining (CR) and cruciate sacrificing - posterior stabilised (PS) TKRs have failed to demonstrate a difference in outcomes based on numbers of patients recruited. This large study compares clinical outcomes in groups having PS and CR TKR and reports the results at 1 and 2 years post-operatively. Methods. A total of 683 patients undergoing TKR were consecutively enrolled in a prospective multi-centre study with 2 arms. In the first arm patients receiving a PS component were randomised to receive either a mobile bearing (176 patients) or fixed bearing (176 patients) implant. In the second arm, patients receiving a CR component were randomised to receive either a mobile bearing (161 patients) or fixed bearing (170 patients) implant. All patients were assessed preoperatively and at one and two years postoperatively using standard tools (Oxford, AKSS, Patellar Score) by independent nurse specialists. The data from the 2 arms of the trial were then analysed to compare differences between PS and CR implants. Results. Patients with a PS component showed a greater improvement in the pain component of the AKSS at 1 year (p=0.0003) and at 2 years (p=0.0085) post-op. Patients with a PS also showed a greater improvement in the AKSS knee score at 1 (p=0.0001) and 2 (p=0.001) years. Range of motion (ROM) improvement was also greater in the PS group at 1 (p=0.0001) and 2 (p=0.0170) years. PS knees also achieved better outcomes in these variables in both mobile and fixed subgroups. There were no other significant differences in the scores between the two groups at any stage. Conclusion. Improvements in pain and knee components of the AKSS score and ROM at both 1 and 2 years follow-up were greater in PS knees. Although this difference was statistically significant, differences in real terms were relatively small

Aims

The mid-term results of kinematic alignment (KA) for total knee arthroplasty (TKA) using image derived instrumentation (IDI) have not been reported in detail, and questions remain regarding ligamentous stability and revisions. This paper aims to address the following: 1) what is the distribution of alignment of KA TKAs using IDI; 2) is a TKA alignment category associated with increased risk of failure or poor patient outcomes; 3) does extending limb alignment lead to changes in soft-tissue laxity; and 4) what is the five-year survivorship and outcomes of KA TKA using IDI?

Methods

A prospective, multicentre, trial enrolled 100 patients undergoing KA TKA using IDI, with follow-up to five years. Alignment measures were conducted pre- and postoperatively to assess constitutional alignment and final implant position. Patient-reported outcome measures (PROMs) of pain and function were also included. The Australian Orthopaedic Association National Joint Arthroplasty Registry was used to assess survivorship.

Aims

Postoperative range of movement (ROM) is an important measure of successful and satisfying total knee arthroplasty (TKA). Reduced postoperative ROM may be evident in up to 20% of all TKAs and negatively affects satisfaction. To improve ROM, manipulation under anaesthesia (MUA) may be performed. Historically, a limited ROM preoperatively was used as the key harbinger of the postoperative ROM. However, comorbidities may also be useful in predicting postoperative stiffness. The goal was to assess preoperative comorbidities in patients undergoing TKA relative to incidence of postoperative MUA. The hope is to forecast those who may be at increased risk and determine if MUA is an effective form of treatment.

Methods

Prospectively collected data of TKAs performed at our institution’s two hospitals from August 2014 to August 2018 were evaluated for incidence of MUA. Comorbid conditions, risk factors, implant component design and fixation method (cemented vs cementless), and discharge disposition were analyzed. Overall, 3,556 TKAs met the inclusion criteria. Of those, 164 underwent MUA.
