Introduction. Despite several studies, controversies prevailed about the rate of complications following one-stage and two-stage bilateral total hip arthroplasty (THA). In current prospective study, we compared the complications and functional outcomes of one-stage and two-stage procedures. Methods. One hundred and eighty patients (ASA class I or II) with bilateral hip osteoarthritis were assigned randomly to two equal groups. Two groups were matched in term of age and sex. All of the surgeries were performed through the Harding approach using uncemented implants. In two-stage procedures, surgeries were performed with 6 months to one year interval. All patients were evaluated one year postoperatively. Results. The Harris hip score averaged 84.1±12.6 and 82.6±15.3 in one-stage and two-stage groups, respectively (p=0.528). The hospital stay was significantly longer in two-stage group (9.8±1.1 versus 4.9±0.8 days). The cumulative hemoglobin drop and number of transfused blood units were the same. One patient in each group developed symptomatic deep venous thrombosis and managed successfully. There was no patient with perioperative death, pulmonary embolism, infection, dislocation, periprosthetic fracture or heterotrophic ossification. No patient required reoperation. Corrosion in modular taper connections of total joint replacement has become a hot topic in the orthopaedic community and failures of modular systems have been reported. The objective of the present study was to determine in vivo titanium ion levels following cementless total hip arthroplasty (THA) using a modular neck system. A consecutive series of 173 patients who underwent cementless modular neck THA and a ceramic on polyethylene bearing was evaluated retrospectively. Introduction. Higher concentrations of metal ion levels after Metal-on-metal (MoM) THA are a cause for concern. Elevated cobalt (Co) and chromium (Cr) ion levels in the blood indicate metal wear, and may predict secondary soft-tissue damage (adverse reaction to metal debris; ARMD). Although, it is well known that concentrations of metal ion levels are elevated in the short term after MoM, the long-term consequences in ion concentration and risk factors for increased ion levels are not clarified. BACKGROUND:. Modern total hip arthroplasty (THA) has shown a great improvement in pain, function and range of motion of patients but data on patients' quality of life after this surgery as assessed by validated tools are lacking. METHODS:. In this cohort study we evaluated the quality of life and functionality of 250 patients an average of 16 years (11–23) after THA using the SF-36 questionnaire, the Harris Hip Score, the WOMAC score, the Functional Comorbidity Index, and a study specific questionnaire. Models of multiple stepwise linear and logistic regression analysis were constructed to evaluate the relationships between the explanatory variables and the functional outcomes. RESULTS:. The SF-36 physical indexes of these patients compared negatively with the normative values but positively with the results obtained in untreated subjects with severe hip osteoarthritis. Introduction. Readmission after Total Hip Arthroplasty (THA) or Total Knee Arthroplasty (TKA) places a great burden on the health care system. As reimbursement systems place increased emphasis on quality measures such as readmission rates, understanding the causes for readmission becomes increasingly important. Methods. We queried an electronic database for all patients who underwent THA or TKA at our institution from 2006 through 2010. We identified those who had been readmitted within 90 days of discharge from the initial admission. We then collected clinical and demographic data as well as readmission diagnoses by ICD-9 code. INTRODUCTION. Total hip arthroplasty (THA) is not commonly performed in young patients. However, markedly advanced hip disease can cause disabling end-stage arthritis, and THA may be the only available option for pain relief and restoration of function. We report our experience with modern alternative bearing THA in patients younger than 21 years. METHODS. Twenty-one consecutive primary THAs were performed in 18 patients. Pre- and post-operative Harris Hip Scores (HHS) and any postoperative complications were recorded. Radiographs were reviewed for evidence of premature component loosening. Mean patient age at operation was 18 years (range, 13-20). There were 14 females (78%) and 4 males (22%). Nine patients (50%) were Caucasian, 8 (44%) were African-American, and 1 (6%) was Asian. Average follow-up was 45 months (range, 16-85). All patients failed conservative treatment; 15 patients had prior core decompression and bone grafting. Underlying etiology was chemotherapy induced osteonecrosis (7, 33%), steroid induced osteonecrosis (6, 29%), sickle cell disease (5, 24%), and chronic dislocation (3, 14%); 1 patient underwent THA for fracture of a previous ceramic bearing, 1 patient had a slipped capital femoral epiphysis, and 1 patient had idiopathic joint destruction. Components implanted were ceramic/ceramic (14, 67%), metal/highly cross-linked polyethylene (6, 29%), and metal/metal surface replacement (1, 5%). RESULTS. HHS scores improved from 43.6 pre-operatively to 83.6 post-operatively (p<0.001). Background. Total hip arthroplasty (THA) is a highly successful procedure, yet access to arthroplasty is limited in many developing nations. In response, organizations around the world have conducted service trips to provide international arthroplasty care to underserved populations. Little outcomes data are currently available related to these trips. We present a 1-year follow up. Methods. We completed an arthroplasty service trip to Brazil in 2017 where we performed 46 THAs on 38 patients. Patient demographic data, comorbidity profile, complication data, and pre- and postoperative Modified Harris Hip Score (mHHS), PROMIS Short Form Pain (SF-Pain), PROMIS Short Form Physical Function (SF-Function), and HOOS Jr scores were collected. Outcomes were collected postoperatively at 2, 6, and 12 weeks and 1 year. A multivariate regression analysis was performed to identify associations between patient factors and 12-week outcomes.
