The preparation of antibiotic-containing polymethyl methacrylate (PMMA), as spacers generates a high polymerization heat, which may affect their antibiotic activity; it is desirable to use bone cement with a low polymerization heat. Calcium phosphate cement (CPC) does not generate heat on polymerization, and comparative elution testings are reported that vancomycin (VCM)-containing CPC (VCM-CPC) exceeded the antibiotic elution volume and period of PMMA (VCM-PMMA). Although CPC alone is a weak of mechanical property spacer, the double-layered, PMMA-covered CPC spacer has been created and clinically used in our hospital. In this study, we prepared the double-layered spacers: CPC covered with PMMA and we evaluated its elution concentration, antimicrobial activity and antibacterial capability. Method. We prepared spherical, double-layered, PMMA-coated (CPC+PMMA; 24 g CPC coated with 16 g PMMA and 2 g VCM) and PMMA alone (40 g PMMA with 2 g VCM) spacers (5 each). In order to facilitate VCM elution from the central CPC, we drilled multiple holes into the CPC from the spacer surface. Each spacer was immersed in phosphate buffer (1.5 mL/g of the spacer), and the solvent was changed daily. VCM concentrations were measured on days 1, 3, 7, 14, 28, 56, and 84. Antimicrobial activity against MRSA and MSSA was evaluated by the broth microdilution method. After measuring all the concentration, the spacers were compressed at 5 mm/min and the maximum compressive load up to destruction was measured. Results. The VCM concentration of the CPC+PMMA spacer exceeded that of the PMMA spacer at all-time points; in particular, it was approximately 7.3 times (109.30 vs. 15.03 μg/mL) and approximately 9.1 times (54.47 vs. 6.50 μg/mL) greater on days 14 and 28, respectively. Using the broth microdilution method, we found that the CPC+PMMA spacer had higher antimicrobial activity than the PMMA model. On day 56, the PMMA spacer lost the capability to inhibit bacterial growth, but the CPC+PMMA spacer maintained this ability. The average maximum compressive load for the CPC+PMMA was 7.28 kN, and that of PMMA was 16.21 kN. Conclusions. The CPC+PMMA spacer was superior to PMMA alone in VCM elution volume and duration, so CP- C+PMMA may be effective for the treatment of MRSA and MSSA infection. The double-layered,

Between stages, however, the patients may be uncomfortable with limited mobility and activity because the joint function is severely restricted by the removal of the infected prosthesis and a thorough debridement. Introduction and Aims: The purpose of this study was to assess pre-operative bone stock and immediate postoperative cement-bone interface as factors affecting infection control and mechanical outcome after one stage revision THR for deep infection. Method: This study included 115 cases which satisfied the following conditions: 1) One stage revision THR for deep infection was the primary intervention for infected hip replacement by a single surgeon (BMW) unless the bone stock was too poor for fixing implants; 2) follow-up of more than five years; 3) A complete series of radiographs was available for radiological study including pre-operative and immediate post-operative ones. Pre-operative bone stock was classified into four grades (Grade 0: No bone loss, Grade 1: Demarcation, Grade 2: Localised cavitation, Grade 3: Extensive bone loss). The immediate post-operative cement-bone interface was also graded into four categories (Grade A: White-out, obscure interface, Grade B: Clear line, no measurable gap, Grade C: Gap>

1mm). These two factors were analysed with regard to infection control and the mechanical survival of implants after surgery. Results: Bone stock did not have significant influence on infection control, while it did affect mechanical outcome. All registered implants between 2007 and 2018 were included (minimum 2 years follow-up). Methods
Periprosthetic joint infection (PJI) remains the main cause of failure in primary and revision total knee arthroplasties (TKAs). Local delivery of antibiotics, mainly antibiotic-loaded bone cement (ALBC), is commonly employed to prevent PJI. Over the past decade, tantalum and porous titanium have been successfully utilized as metaphyseal fixation devices to address bone loss and improve biologic fixation during revision TKA. However, no study has examined the antimicrobial properties compared to bone cement. The purpose of this study was to compare the ability of tantalum, 3D porous titanium, antibiotic-loaded bone cement (ALBC) and smooth titanium alloy (STA) to inhibit Staphylococci bacterial agents in an in vitro medium environment, based on the evaluation of the zone of inhibition (ZOI) and the antibacterial activity duration. Our study hypothesis was that we will found no significant difference between groups to inhibit Methicillin-Sensitive or Methicillin-Resistant Staphylococcus aureus (MSSA/MRSA) agents. Thirty beads made of 3 different materials (tantalum/ 3D porous titanium/ STA) were bathed during 1hour inside of a solution made of 1g vancomycin with 20-mL of sterile water for injection (bath concentration: 50 mg/mL). Ten 1cm3 cylinders were also created mixing standard surgical cement with 1g of Vancomycin in standardized sterile molds (ALBC beads). Finally, thirty beads made of tantalum/ 3D porous titanium/ STA were bathed in phosphate buffered saline solution to act as a control group. Cylinders were then placed on agar plates inoculated with MSSA and MRSA. Inhibition zone diameters were measured each day and cylinders were transferred onto a new inoculated plate. Inhibition zones were measured with a manual Vernier caliper and with automated software. The mean inhibition zones between groups were compared using the Wilcoxon Test.Introduction
Methods
The management of infected total hip replacements is a challenging problem in orthopaedic surgery. Two-stage revision procedures usually involve the application of a temporary antibiotic-loaded polymethylmethacrylate spacer. A preformed spacer which allows weight-bearing and joint motion while ensuring a sustained antibiotic release was evaluated. From September 1996 to March 2002, 26 consecutive patients with an infected total hip arthroplasty were treated by the insertion of an industrially preformed temporary spacer (Spacer-G®). This device comprises a cylindrical stainless-steel rod coated with bone cement supplemented with gentamicin (1.9% w/w) and vancomycin (2.5% w/w). The spacer is currently available in three sizes of head diameter, each size with two stem lengths. Joint mobilisation and assisted weight-bearing were permitted when the bone stock provided adequate mechanical stability of the spacer. Patients’ evaluation included clinical assessment and standard X-ray and laboratory parameters. Reimplantation was performed when serological parameters had normalised. The spacer remained in situ for an average of 155 (70–272) days, allowing healing of the infection in 24 cases. Five patients required resection arthroplasty (two persistent infections, two inadequate local bone conditions and one acute recurrence of infection). A second spacer was implanted after 4 months in one subject. In four cases the spacer dislocated, because the head diameter was too small or because of a rotational instability of the stem in the femur. The successfully-reimplanted patients (21) were assessed with a mean 53 (22–88) months of follow-up, showing no clinical or bio-humoral signs of infection recurrence. Functional outcome was satisfactory with a mean value of Harris Hip Score of 79 (53–100), and no radiographic aspects of loosening were observed. The Spacer-G® used in the two-stage revision of infected total hip replacements permitted an effective local antibiotic release together with some range of joint motion, which improved the quality of life of the patients during treatment of infection and accelerated recovery of function after reimplantation.
Average interval between the two surgical stages was 5.5 months (range, 2 weeks to 13 months). Systemic antibiotics were administered to all patients for a minimum period of 6 weeks after removal of the infected implant. The revision stem was cemented in 5 patients and not cemented in 13 patients. All the acetabular components were uncemented. In one patient, the second stage procedure consisted exclusively in removal of the spacer and debridement, owing to persisting infection sustained by Staph. aureus + Staph. epidermidis.
Recurrence of infection (Staph. aureus) occurred in 1 patient and was treated by resection-arthroplasty. Aseptic loosening of the stem was observed in 1 patient, who was subsequently treated by stem revision. Average Harris Hip Score was 78 points (range, 65 to 90 points).
