Introduction. Opioid abuse is a national epidemic. Traditional pain management after total knee arthroplasty (TKA) relied heavily on opioids. The evidence that in-hospital multimodal pain management (MMPM) is more effective than opioid-only analgesia is overwhelming. There has been little focus on post-discharge pain management. The purpose of this study was to determine whether MMPM after TKA could reduce opioid consumption in the 30-day period after hospital discharge. Methods. This was a prospective, two-arm, comparative study with a provider cross-over design. The first arm utilized a standard opioid-only (OO) prn regimen. The second arm utilized a 30-day MMPM regimen (standing doses of acetaminophen, metaxalone, meloxicam, gabapentin) and opioid medications prn. Surgeons crossed over protocols every four weeks. The primary outcome measure was VAS pain score. Secondary outcome measures included morphine milligram equivalents (MME) consumed, failure of the protocol, and manipulation under anesthesia (MUA). A pre-hoc power analysis was performed for the primary outcome measure and an intent-to-treat analysis was done utilizing a longitudinal mixed model. Results. There were 43 patients in the OO cohort and 39 patients in the MMPM cohort. There was no difference in the baseline demographics or preoperative scores (p=0.94). There was no clinically meaningful difference in VAS score between the two groups at any time. The average opioid consumption at 30-days was 469 and 344 MME's for the OO and MMPM cohorts, respectively (p=0.026). 19/43 (44.2%) patients in the OO cohort failed vs. 4/39 (10.3%) in the MMPM cohort (p=0.002). There was 1 MUA in the OO and none in the MMPM cohort (p=0.338). Discussion. A 30-day post-discharge multimodal pain regimen reduced opioid use after TKA. Opioid-only regimens are at an increased risk of failure to control pain. As a result of this study, multimodal pain management after hospital discharge is standard at our institution

Abstract. Introduction. 30-day emergency readmission is an indicator of treatment related complication once discharged, resulting in readmission. A board-approved quality improvement pathway was introduced to reduce elective re-admissions. Method. The pathway involved telephone and email contact details provision to patients for any non-life threatening medical assistance, allowing for initial nurse led management of all issues. A new clinic room available 7 days, and same day ultrasound scanning for DVT studies were introduced. A capability, opportunity and behavior model of change was implemented. Readmission rates before and six months after implementation were collected from Model Hospital. A database used to document patient communications was interrogated for patient outcomes. Results. Prior to implementation, readmission rates following elective primary total knee replacement (TKR) at the 1st business quarter of 2021 (April – June 2021), was 8.7%, (benchmark 3.8%). Following implementation, readmission rates decreased to 4.1% (October – December 2021). 54% of patients making contact were managed with telephone advice. 15% of patients required face-to-face clinic. 32% of those required a same day scan to exclude DVT (1/4). 20 out of 684 TKRs performed following protocol introduction were re-admitted within 30 days. Readmissions were 41% surgical, 29% medical. 52% were unaware of the newly implemented protocol. Further improvements have been made to the protocol based on these findings. Implementation of a suitable pathway can significantly reduce re-admission rates in our center and could be used to reduce readmission rates in other national elective treatment centers

Introduction. We designed this study to compare 30-day complications and length of hospital stay between patients undergoing total knee arthroplasty (TKA) with general anesthesia, to those undergoing TKA with spinal, epidural anesthesia, or Monitored Anesthesia Care (MAC, a combination of local anesthesia with sedation and analgesia provided by an anesthesiologist) with or without regional nerve blocks. Methods. We identified patients ≥18 years undergoing TKA between the years of 2006 and 2017 from the American College of Surgeons National Surgical Quality Improvement Program (NSQIP). We collected patient demographics, anesthesia type, 30-day complications, length of operation and hospital stay from the database. We used multivariable regression to compare complications and length of stay (LOS) between anesthesia types, while adjusting for relevant covariables. Results. We identified 265,325 TKA patients. Of these, 91 (0.03%) underwent epidural anesthesia with a nerve block, while 1,855 (0.70%) underwent epidural anesthesia with no block, 12,800 (4.82%) underwent MAC with a block, 25,643 (9.66%) underwent MAC with no block, 13,575 (5.12%) underwent spinal anesthesia with a block, 80,803 (30.45%) underwent spinal anesthesia with no block, and 130,558 (49.21%) underwent general anesthesia. The rate of complications was not associated with the presence of a block, while the unadjusted LOS was significantly lower with the use of a block in patients treated with spinal anesthesia (2.54±2.07 vs. 2.84±2.25), epidural anesthesia (2.87±1.81 vs. 3.88± 4.67), and MAC (2.51±2.14 vs. 2.68±2.11), p<0.0001. The unadjusted rate of major complications was significantly lower in patients who underwent spinal anesthesia (2.10%), and MAC (1.91%) compared to general anesthesia (2.31%), p<0.0001. Similarly, the unadjusted rate of minor complications was significantly lower for patients treated with spinal anesthesia(1.86%) and MAC (1.78%) compared to general anesthesia (2.11%), p<0.0001. The unadjusted LOS was significantly longer in patients treated with epidural (3.83±4.58), compared to general (2.94±3.64) anesthesia, p<0.0001. In contrast, the unadjusted LOS was significantly lower for patients treated with spinal anesthesia (2.80±2.23), and MAC (2.62±2.12) compared to general anesthesia, p<0.0001. Following covariable adjustment, spinal anesthesia and MAC were associated with a 0.93 (0.87–0.98), and 0.84 (0.78–0.91), odds of major complications compared to general anesthesia. Similarly, spinal anesthesia and MAC were associated with a 0.92 (0.87–0.98) and 0.89 (0.82–0.97) odds of minor complications compared to general anesthesia. Following covariable adjustment, epidural anesthesia increased the LOS by 0.25 (0.27–0.28) days compared to general, while spinal anesthesia and MAC decreased the LOS by 0.04 (95%CI 0.05–0.04), and 0.10 (0.11–0.09) days, compared to general. In patients treated with spinal anesthesia, epidural anesthesia, and MAC, the use of a block was independently associated with a decreased LOS by 0.10 (0.12–0.90), 0.24 (0.39–0.09), and 0.07 (0.08–0.05). Conclusion. Patients who undergo TKA with spinal anesthetic, and MAC appear to have superior outcomes compared to those who undergo TKA with general anesthesia. In addition, the use of a regional nerve block appears to be independently associated with a shorter LOS in patients who undergo TKA with neuraxial (spinal and epidural) anesthetic, and MAC. Introduction. Social determinants of health (SDOH) may contribute markedly to the total cost of care (COC) for patients undergoing elective total knee arthroplasty (TKA). This study investigated the association between demographics, health status, and SDOH on lengths of stay (LOS) and 30-day COC. Methods. Patients who underwent TKA between January 2018 and December 2019 were identified. Those who had complete SDOH data were utilized, leaving 234 patients. Data elements were drawn from the Chesapeake Regional Information System, the Center for Disease Control social vulnerability index (SVI), the Food Access Research Atlas (FARA). The SVI identifies areas vulnerable to catastrophic events, with 4 themed scores including: (1) socioeconomic status; (2) household composition and disability; (3) minority status and language; and (4) housing and transportation. Food deserts were defined as neighborhoods located 1 or 10 miles from a grocery store in urban and rural areas, respectively. Multiple regression analyses were performed to determine an association with LOS and cost, after controlling for demographics. Results. Increased 30-day COC associated with SVI theme 3, (3.074 days; p=0.001) and patients who lived in a food desert ($53,205; p=0.001), as well as those who had anemia ($16,112; P = 0.038), chronic obstructive pulmonary disease ($32,570, P = 0.001), congestive heart failure ($30,927, P = 0.003), and dementia ($33,456, P = 0.008). Longer hospital lengths of stay were associated with SVI theme 3. In addition, patients who had anemia and congestive heart failure were at risk for longer hospital lengths of stay (P < 0.001, P = 0.001, respectively). Conclusion. Higher SVI theme 3 scores and living in food deserts were risk factors for increased LOS and costs, respectively. Identifying social factors including a patient's transportation options, living situation and access to healthy foods may prove to be both prognostic of outcomes and an opportunity for intervention

Aims. In the last decade, perioperative advancements have expanded the use of outpatient primary total knee arthroplasty (TKA). Despite this, there remains limited data on expedited discharge after revision TKA. This study compared 30-day readmissions and reoperations in patients undergoing revision TKA with a hospital stay greater or less than 24 hours. The authors hypothesized that expedited discharge in select patients would not be associated with increased 30-day readmissions and reoperations. Methods. Aseptic revision TKAs in the National Surgical Quality Improvement Program database were reviewed from 2013 to 2020. TKAs were stratified by length of hospital stay (greater or less than 24 hours). Patient demographic details, medical comorbidities, American Society of Anesthesiologists (ASA) grade, operating time, components revised, 30-day readmissions, and reoperations were compared. Multivariate analysis evaluated predictors of discharge prior to 24 hours, 30-day readmission, and reoperation. Results. Of 21,610 aseptic revision TKAs evaluated, 530 were discharged within 24 hours. Short-stay patients were younger (63.1 years (49 to 78) vs 65.1 years (18 to 94)), with lower BMI (32.3 kg/m. 2. (17 to 47) vs 33.6 kg/m. 2. (19 to 54) and lower ASA grades. Diabetes, chronic obstructive pulmonary disease, hypertension, and cancer were all associated with a hospital stay over 24 hours. Single component revisions (56.8% (n = 301) vs 32.4% (n = 6,823)), and shorter mean operating time (89.7 minutes (25 to 275) vs 130.2 minutes (30 to 517)) were associated with accelerated discharge. Accelerated discharge was not associated with 30-day readmission and reoperation. Conclusion. Accelerated discharge after revision TKA did not increase short-term complications, readmissions, or reoperations. Further efforts to decrease hospital stays in this setting should be evaluated. Abstract. Introduction. COMPOSE describes the demographics, fracture characteristics, management and associated outcomes of knee femoral periprosthetic fractures (KFPPF). Methods. Multicentre retrospective cohort study conducted 01/01/2018-31/12/2018. Data collected included: patient demographics, social and mobility characteristics, fracture characteristics, management strategy and post-treatment outcomes (length of stay, reoperation, readmission, 30-day and 12-month mortality). Results. 785 PPFs from 27 NHS sites were included in the COMPOSE cohort. Of these 162 (21%) were related to an isolated knee prosthesis (151 femur, 10 tibia and 1 patella). The KFPPF group had a mean age of 81.1 years, 127 (84%) female, 114 (76%) living in their own home, with 99 (63%) reliant on walking aids/bedbound. Most fractures were B (58%) or C (35%) type and occurred around a primary cemented replacement (141,94%) at a mean of 8.2 years after surgery. 116 (76.8%) KFPPFs were treated operatively. Mean time to surgery was 5 days and the commonest surgical strategy was fixation alone (64%) vs revision+/-fixation (36%). Mean operative time was 126 minutes and 11 (10%) patients required ITU/HDU admission after surgery. Mean LOS was 22 days and 47 patients (31%) experienced a complication prior to discharge. Overall, 5 patients (3%) had a further operation within 12 months, 7 (5%) were readmitted within 30 days and the 30-day and 12-month mortality were 6.6% and 23.2% respectively. Conclusions. KFPPF patients are elderly and frail and have mortality, re-operation and readmission rates comparable to hip fracture patients. However, they wait longer for surgery and surgical treatment is more complex

Aims. Spinal anaesthesia has seen increased use in contemporary primary total knee arthroplasties (TKAs). However, controversy exists about the benefits of spinal in comparison to general anaesthesia in primary TKAs. This study aimed to investigate the pain control, length of stay (LOS), and complications associated with spinal versus general anaesthesia in primary TKAs from a single, high-volume academic centre. Methods. We retrospectively identified 17,690 primary TKAs (13,297 patients) from 2001 to 2016 using our institutional total joint registry, where 52% had general anaesthesia and 48% had spinal anaesthesia. Baseline characteristics were similar between cohorts with a mean age of 68 years (SD 10), 58% female (n = 7,669), and mean BMI of 32 kg/m. 2. (SD 7). Pain was evaluated using oral morphine equivalents (OMEs) and numerical pain rating scale (NPRS) data. Complications including 30- and 90-day readmissions were studied. Data were analyzed using an inverse probability of treatment weighted model based on propensity score that included many patient and surgical factors. Mean follow-up was seven years (2 to 18). Results. Patients treated with spinal anaesthesia required fewer postoperative OMEs (p < 0.001) and had lower NPRS scores (p < 0.001). Spinal anaesthesia also had fewer cases of altered mental status (AMS; odds ratio (OR) 1.3; p = 0.044), as well as 30-day (OR 1.4; p < 0.001) and 90-day readmissions (OR 1.5; p < 0.001). General anaesthesia was associated with increased risk of any revision (OR 1.2; p = 0.021) and any reoperation (1.3; p < 0.001). Conclusion. In the largest single institutional report to date, we found that spinal anaesthesia was associated with significantly lower OME use, lower risk of AMS, and lower overall 30- and 90-day readmissions following primary TKAs. Additionally, spinal anaesthesia was associated with reduced risk of any revision and any reoperation after accounting for numerous patient and operative factors. When possible and safe, spinal anaesthesia should be considered in primary TKAs. Cite this article: Bone Joint J 2022;104-B(11):1209–1214

Background. The purpose of this study is to evaluate the early perioperative outcomes after primary knee arthroplasty with the use of a smartphone-based exercise and educational platform compared to a standard of care control group. Methods. A multicenter prospective randomized control trial was conducted evaluating the use of the mymobility smartphone-based care platform for primary total knee arthroplasty (TKA) and unicondylar knee arthroplasty (UKA). Patients in the control group (224 patients) received the respective institution's standard of care typically with formal physical therapy. Those randomized to mymobility treatment group (192 patients) were provided an Apple Watch and mymobility smartphone application. The treatment group was not initially prescribed physical therapy, but could if their surgeon determined it necessary. Early outcomes assessed included 90-day knee range of motion, KOOS Jr scores, 30-day single leg stance (SLS) time, Time up and Go (TUG) time and need for manipulation under anesthesia (MUA). There was no significant difference in age, BMI or gender between groups. Results. The 90-day knee flexion was not significantly different between controls (118.3±11.8) and mymobility (118.8 ±12) (p=0.7), nor was knee extension (1.6 ±3.5 vs. 1 ±3.1, p=0.16). KOOS Jr scores were not significantly different between control group (74 ±13.1) and mymobility group (71 ±13.3) (p=0.06). 30-day SLS was 22.3 ± 19.5 sec in controls and 24 ± 20.8 sec in mymobility (p=0.2). 30-day TUG times were 16 ± 44.3 sec in control and 15 ± 40.6 sec in mymobility (p=0.84). MUAs were performed in 4.02% of patients in the control group and 2.8% in the mymobility group (p=0.4%). Conclusion. The use of the mymobility care platform demonstrated similar early outcomes to traditional care models, while providing communication and insights into patient engagement with the care plan. There was no significant difference in 90-day range of motion or need for MUA

Aims. Infection complicating primary total knee arthroplasty (TKA) is a common reason for revision surgery, hospital readmission, patient morbidity, and mortality. Increasing incidence of methicillin-resistant Staphylococcus aureus (MRSA) is a particular concern. The use of vancomycin as prophylactic agent alone or in combination with cephalosporin has not demonstrated lower periprosthetic joint infection (PJI) rates, partly due to timing and dosing of intravenous (IV) vancomycin administration, which have proven important factors in effectiveness. This is a retrospective review of a consecutive series of primary TKAs examining incidence of PJI, adverse reactions, and complications using IV versus intraosseous (IO) vancomycin at 30-day, 90-day, and one-year follow-up. Methods. A retrospective review of 1,060 patients who underwent TKA between May 2016 to July 2020 was performed. There were 572 patients in the IV group and 488 in the IO group, with minimal 30 days of follow-up. Patients were followed up at regularly scheduled intervals (two, six, and 12 weeks). No differences between groups for age, sex, BMI, or baseline comorbidities existed. The IV group received an IV dose of 15 mg/kg vancomycin given over an hour preceding skin incision. The IO group received a 500 mg dose of vancomycin mixed in 150 ml of normal saline, injected into proximal tibia after tourniquet inflation, before skin incision. All patients received an additional dose of first generation cephalosporin. Evaluation included preoperative and postoperative serum creatinine values, tourniquet time, and adverse reactions attributable to vancomycin. Results. Incidence of PJI with minimum 90-day follow-up was 1.4% (eight knees) in the IV group and 0.22% (one knee) in IO group (p = 0.047). This preliminary report demonstrated an reduction in the incidence of infection in TKA using IO vancomycin combined with a first-generation cephalosporin. While the study suffers from limitations of a retrospective, multi-surgeon investigation, early findings are encouraging. Conclusion. IO delivery of vancomycin after tourniquet inflation is a safe and effective alternative to IV administration, eliminating the logistical challenges of timely dosing. Abstract. Introduction. Uni-compartmental knee replacement (UKR) has become popularised due to quicker recovery times, reduced postoperative pain, and blood loss. The desire to increase bed capacity and reduce costs, while preserving safety and patient satisfaction, has led to increased interest in day-case arthroplasty. This study observes the feasibility of UKR as a day-case procedure and whether this affects short and long-term postoperative outcomes. Methodology. Between 2018 and 2021, at a single institution and operated by a single orthopaedic surgeon, seventy-seven patients received a UKR on an elective basis. The patients were divided into two groups: ‘day-case’ for those discharged on the same day, and ‘non day-case’ group. Results. 31 patients were identified as day case procedures with the remaining 46 requiring between one to three days before discharge. Mean age, sex, modal ASA score, BMI, Charlson co-morbidity index, and pre-op oxford knee score showed no statistically significant difference between the two groups (p>0.05). No significant difference between the post-op oxford knee score at 1 year was found for patients treated and discharged as a day case procedure (37.8 +/− 6.88) and those who remained as an inpatient postoperatively (37.8 +/− 10.7); t(df) = −0.0007, p=0.994. No patients in either group suffered any complications beyond the peri-operative period. 30-day and 90-day readmission rates were equal. Conclusions. With no significant differences in post-op knee scores, complication, and readmission rates, we feel UKR can be performed as a viable day case procedure in a planned elective setting. This will result in significant cost savings

Aims. Social determinants of health (SDOHs) may contribute to the total cost of care (TCOC) for patients undergoing total knee arthroplasty (TKA). The aim of this study was to investigate the association between demographic data, health status, and SDOHs on 30-day length of stay (LOS) and TCOC after this procedure. Methods. Patients who underwent TKA between 1 January 2018 and 31 December 2019 were identified. A total of 234 patients with complete SDOH data were included. Data were drawn from the Chesapeake Regional Information System, the Centers for Disease Control social vulnerability index (SVI), the US Department of Agriculture, and institutional electronic medical records. The SVI identifies areas vulnerable to catastrophic events with four themed scores: socioeconomic status; household composition and disability; minority status and language; and housing and transportation. Food deserts were defined as neighbourhoods located one or ten miles from a grocery store in urban and rural areas, respectively. Multiple regression analyses were performed to determine associations with LOS and costs after controlling for various demographic parameters. Results. Divorced status was significantly associated with an increased LOS (p = 0.043). Comorbidities significantly associated with an increased LOS included chronic obstructive pulmonary disease/asthma and congestive heart failure (p = 0.043 and p = 0.001, respectively). Communities with a higher density of tobacco stores were significantly associated with an increased LOS (p = 0.017). Comorbidities significantly associated with an increased TCOC included chronic obstructive pulmonary disease (p = 0.004), dementia (p = 0.048), and heart failure (p = 0.007). Increased TCOCs were significantly associated with patients who lived in food deserts (p = 0.001) and in areas with an increased density of tobacco stores (p = 0.023). Conclusion. Divorced marital status was significantly associated with an increased LOS following TKA. Living in food deserts and in communities with more tobacco stores were significant risk factors for increased LOS and TCOC. Food access and ease of acquiring tobacco may both prove to be prognostic of outcome after TKA and an opportunity for intervention. Cite this article: Bone Joint J 2021;103-B(6 Supple A):113–118

Aims. The purpose of this study was to assess total knee arthroplasty (TKA) volume and rates of early complications in morbidly obese patients over the last decade, where the introduction of quality models influencing perioperative care pathways occurred. Methods. Patients undergoing TKA between 2011 to 2018 were identified in the American College of Surgeons National Surgical Quality Improvement Program database. Patients were stratified by BMI < 40 kg/m. 2. and ≥ 40 kg/m. 2. and evaluated by the number of cases per year. The 30-day rates of any complication, wound complications, readmissions, and reoperation were assessed. Trends in these endpoints over the study period were compared between groups using odds ratios (ORs) and multivariate analyses. Results. In total, 314,695 patients underwent TKA and 46,362 (15%) had BMI ≥ 40 kg/m. 2. The prevalence of morbid obesity among TKA patients did not change greatly, ranging between 14% and 16%. Reoperation rate decreased from 1.16% to 0.96% (odds ratio (OR) 0.81 (95% confidence interval (CI) 0.66 to 0.99)) for patients with BMI < 40 kg/m. 2. , as did rates of readmission (4.46% to 2.87%; OR 0.61 (0.55 to 0.69)). Patients with BMI ≥ 40 kg/m. 2. also had fewer readmissions over the study period (4.87% to 3.34%; OR 0.64 (0.49 to 0.83)); however, the rate of reoperation did not change (1.37% to 1.41%; OR 0.99 (0.62 to 1.56)). Significant improvements were not observed for infective complications over time for either group; patients with BMI ≥ 40 kg/m. 2. had increased risk of both deep infection and wound complications compared to non-morbidly obese patients. Rate of any complication decreased for all patients. Conclusion. The proportion of TKAs in morbidly obese patients has not significantly changed over the past decade. Although readmission rates improved for all patients, reductions in reoperation in non-morbidly obese patients were not experienced by the morbidly obese, resulting in a widening of the complication gap between these cohorts. Introduction. The Centers for Medicare and Medicaid Services (CMS) removed total knee arthroplasty (TKA) from inpatient-only status in 2018. Our goal was to measure the adoption of outpatient TKAs, the impact on re-treatment rates, and the economic implications for hospitals. Methods. We utilized 100% national Medicare Part A fee-for-service (FFS) patient-level claims data for 2017–2018. We excluded DRG 469 TKAs since they are unlikely to be outpatient candidates, which left 257,107 primary TKAs in 2017 and 264,393 in 2018. We examined the time trend in monthly case volume and 30-day retreatment rate (defined as percent of patients having a second TKA within 30 days of the first. We calculated the loss in revenue for a hospital by multiplying the decrease in payment rate between inpatient and outpatient by the outpatient and total 2018 TKA volume. Results. In 2017 0.2% of primary TKAs were performed outpatient. Following the rule change, 25% of cases were performed outpatient in Q1 2018. This stayed at 25% in Q2, increased to 27% in Q3, and then increased to 30% in Q4 2018. The 30-day re-treatment rate was 0.16% in 2017 and 0.15% in 2018. Across hospitals there was the following distribution in the decrease in payment rate from inpatient to outpatient TKAs in 2018: 10. th. percentile: $1,994, 25. th. : $2,612, 50. th. : $3,487, 75. th. : $4,918, 90. th. : $7,231. In 2018 outpatient TKA coding cost hospitals (saved CMS) $243M in Medicare FFS payments (an average of $89,000 per hospital). If all TKAs were performed outpatient hospital Medicare FFS payments would have been $965M lower ($353,000 per hospital) in 2018. Conclusion. Outpatient TKA volumes grew through 2018. This did not impact 30-day retreatment rates. Medicare FFS payment rates declined by a median of $3,487 per outpatient case. Introduction. Revision total knee arthroplasty (revision TKA) occurs for a wide variety of indications and along with revision total hip arthroplasty is billed for using Diagnosis-Related Groups (DRGs) 466, 467, and 468 in the United States. However, DRGs do not account for revision etiology, a potentially substantial driver of cost. This study investigates revision TKA costs and 30-day complications by indication, employing both local granular as well as national standardized databases. Methods. Hospitalization costs and complication rates for 1,422 aseptic revision TKAs performed at a high-volume center between 2009 and 2015 were retrospectively reviewed. Additionally, charges for 28,173 revision TKAs in the National Inpatient Sample (NIS) were converted to costs using the Healthcare Cost and Utilization Project cost-to-charge ratios. 30-day complication rates for 3,450 revision TKAs were obtained using the American College of Surgeons National Surgical Quality Improvement Project (ACS-NSQIP). Costs and complications were compared between revision TKAs performed for the indication of fracture, wear/loosening, and instability with use of simultaneous and pairwise comparisons and a multivariable model accounting for American Society of Anesthesiologists (ASA) score, age, and sex. Results. Local hospitalization costs for fracture (median, $30,643) were significantly higher than those for wear/loosening ($24,734; p < 0.001) or instability ($22,932; p < 0.001), with differences remaining significant even after adjusting for patient comorbidities (p < 0.001). Local fracture patients were admitted for an average of 1.9 days longer (mean length of stay [LOS]: 4.8 days; IQR: 3 – 6) than instability patients (mean LOS: 2.9; IQR: 2 – 3; p < 0.001) and 1.7 days longer than wear/loosening patients (mean LOS: 3.1, IQR: 2 – 3; p < 0.001) but had similar 30-day orthopedic-specific complication rates (10.8%) as compared to instability (11.2%; p > 0.999) and wear/loosening (15.9%; p = 0.333). Nationally, NIS costs for fracture (median, $31,207) were higher than those for other aseptic indications (wear/loosening: $21,747; instability: $16,456; p< 0.001). Combined medical and surgical complication rates for fracture (56.6% of patients with ≥1 complication) were significantly higher than those for wear/loosening (19.7%) and instability (15.5%) (p < 0.001), with 3.5 – 5.4 fold increased transfusion rates (45.7% fracture, 13.2% wear/loosening, 8.5% instability; p < 0.001) and 1.2 – 3.3 fold increased urinary tract infection rates (2.3% fracture, 0.7% wear/loosening, 1.9% instability; p = 0.004). Discussion and Conclusion. Hospitalization costs for revision TKA for fracture were 37% to 50% higher than for all other aseptic revision TKAs, both locally and nationally. This increased cost persisted even after multivariable comorbidity adjustment, the current approach for DRG assignment for both revision TKA and THA reimbursement. Indication-specific coding and reimbursement systems are necessary to maintain sustainable access to revision TKA for all patients. Aims. This study investigated the influence of body mass index (BMI) on the post-operative fall in the level of haemoglobin (Hb), length of hospital stay (LOS), 30-day re-admission rate, functional outcome and quality of life, two years after total knee arthroplasty (TKA). Patients and Methods. A total of 7733 patients who underwent unilateral primary TKA between 2001 and 2010 were included. The mean age was 67 years (30 to 90). There were 1421 males and 6312 females. The patients were categorised into three groups: BMI < 25.0 kg/m. 2. (normal); BMI between 25.0 and 39.9 kg/m. 2. (obese); and BMI ≥ 40.0 kg/m. 2. (morbidly obese). Results. Compared with the normal and obese groups, the mean LOS was longer by one day (95% confidence interval (CI) 0 to 2) in the morbidly obese group (p = 0.003 and p = 0.001 respectively). The 30-day re-admisison rate was also higher in the morbidly obese group compared to the obese group (OR 2.323, 95% CI 1.101 to 4.900, p = 0.024); and showed a higher trend compared to the normal group (OR 1.850, 95% CI 0.893 to 3.831, p = 0.100). However, the morbidly obese group had a smaller drop in post-operative Hb level by a mean of 0.5 g/dl (0.3 to 0.6) and 0.3 g/dl (0.1 to 0.5), when compared with the normal and obese groups respectively (both p < 0.001). Furthermore, the mean improvement in Oxford Knee Score (OKS) and Knee Society Knee Score (KSKS) at two years follow-up was three points (two to four) and five points (two to seven) more in the morbidly obese group than in the normal group (both p < 0.001). The mean improvement in Knee Society Function Score, and Physical and Mental Component Scores of Short Form-36 were comparable between the three BMI groups (p = 0.736, p = 0.739 and p = 0.731 respectively). The ten-year rate of survival was 98.8% (98.0 to 99.3), 98.9% (98.5 to 99.2) and 98.0% (95.8 to 100), for the normal, obese and morbidly obese groups, respectively (p = 0.703). Conclusion. Although morbidly obese patients have a longer LOS and higher 30-day re-admission rate after TKA, they have a smaller drop in post-operative Hb level and larger improvement in OKS and KSKS at two years follow-up. The ten-year rate of survival of TKA was also comparable with those with a normal BMI. . Take home message: Morbidly obese patients should not be excluded from the benefits of TKA. Aims. To evaluate the influence of discharge timing on 30-day complications following total knee arthroplasty (TKA). Patients and Methods. We identified patients aged 18 years or older who underwent TKA between 2005 and 2016 from the American College of Surgeons’ National Surgical Quality Improvement Program (NSQIP) database. We propensity score-matched length-of-stay (LOS) groups using all relevant covariables. We used multivariable regression to determine if the rate of complications and re-admissions differed depending on LOS. Results. Our matched cohort consisted of 76 246 TKA patients (mean age 67 years . (sd. 9)). Patients whose LOS was zero and four days had an increased risk of major complications by an odds ratio (OR) of 1.8 (95% confidence interval (CI) 1.0 to 3.2) and 1.5 (95% CI 1.2 to 1.7), respectively, compared with patients whose LOS was two days. Patients whose LOS was zero, three, and four days had an increased risk of minor complications (OR 1.8 (95% CI 1.3 to 2.7), 1.2 (95% CI 1.0 to 1.4), and 1.6 (95% CI 1.4 to 1.9), respectively), compared with patients whose LOS was two days. In addition, a LOS of three days increased the risk of re-admission by an OR of 1.2 (95% CI 1.0 to 1.3), and a LOS of four days increased the risk of re-admission by an OR of 1.5 (95% CI 1.3 to 1.6), compared with a LOS of two days. Conclusion. Patients discharged on days one to two postoperatively following TKA appear to have reduced major and minor complications compared with discharge on the day of surgery, or on days three to four. Aims. The purpose is to determine the non-inferiority of a smartphone-based exercise educational care management system after primary knee arthroplasty compared with a traditional in-person physiotherapy rehabilitation model. Methods. A multicentre prospective randomized controlled trial was conducted evaluating the use of a smartphone-based care management system for primary total knee arthroplasty (TKA) and partial knee arthroplasty (PKA). Patients in the control group (n = 244) received the respective institution’s standard of care with formal physiotherapy. The treatment group (n = 208) were provided a smartwatch and smartphone application. Early outcomes assessed included 90-day knee range of movement, EuroQoL five-dimension five-level score, Knee Injury and Osteoarthritis Outcome Score for Joint Replacement (KOOS JR) score, 30-day single leg stance (SLS) time, Time up and Go (TUG) time, and need for manipulation under anaesthesia (MUA). Results. Overall, 90-day mean flexion was not significantly different between the control (121° (SD 11.7°)) and treatment groups (121. o. ; p = 0.559); 90-day mean SLS was 22.7 seconds (SD 9.8) in controls and 24.3 seconds (SD 20.8) in treatment (p = 0.519); 90-day mean TUG times were 10.1 seconds (SD 4.8) in control and 9.3 seconds (SD 3.3) in treatment (p = 0.139). Mean KOOS JR scores were significantly different between control group (73.6 (SD 13.4)) and treatment group (70.4 (SD 12.6); p = 0.026). MUAs were performed in nine (3.7%) patients in the control group and four (1.9%) in the treatment group (p = 0.398). Physiotherapy was performed by 230 (94.4%) of control group and 123 (59.3%) of treatment group (p < 0.001). There were no significant differences between groups in postoperative urgent care visits, or readmissions within 90 days, with significantly fewer emergency department visits in the treatment group (16 (8.2%) vs five (2.5%), p = 0.014). Conclusion. The use of the smartwatch/smartphone care platform demonstrated non-inferiority of clinically significant outcomes to traditional care models, while requiring significantly less postoperative physiotherapy and fewer emergency department visits. Background. The evaluation and management of outcomes risk has become an essential element of a modern total joint replacement program. Our multidisciplinary team designed an evidence-based tool to address modifiable risk factors for adverse outcomes after primary hip and knee arthroplasty surgery. Methods. Our protocols were designed to identify, intervene, and mitigate risk through evidence-based patient optimization. Nurse navigators screened patients preoperatively, identified and treated risk factors, and followed patients for 90 days postoperatively. We compared patients participating in our optimization program (N=104) to both a historical cohort (N=193) and a contemporary cohort (N=166). Results. Risk factor identification and optimization resulted in lower hospital length of stay and post-operative emergency department visits. Patients in the optimization cohort had a statistically significant decrease in mean LOS as compared to both the historical cohort (2.55 vs 1.81 days, P<0.001) and contemporary cohort (2.56 vs 1.81 days, p<0.001). Patients in the optimization cohort had a statistically significant decrease in 30- and 90-day ED visits compared to the historical cohort (P. 30-day. =0.042, P. 90-day. =0.003). When compared with the contemporary cohort, the optimization cohort had a statistically significant decrease in 90-day ED visits (21.08% vs. 10.58%, P=0.025). The optimization cohort had a statistically significant increase in the percentage of patients discharged home. We noted nonsignificant reductions in readmission rate, transfusion rate, and surgical site infections. Conclusion. Optimization of patients prior to elective primary THA and TKA reduced average LOS, ED visits, and drove tele-rehabilitation use. Our results add to the limited body of literature supporting this patient-centered approach

Introduction. This study was designed to evaluate the effect of discharge timing on 30-day major and minor complications in patients undergoing total knee arthroplasty (TKA) while adjusting for other variables. Methods. Patients 18 years and older undergoing TKA between the years of 2005 and 2016 were identified from the American College of Surgeons National Surgical Quality Improvement Program (NSQIP). Patients whose length of stay (LOS) was >4 days were excluded. Patient demographics, anesthesia type, length of operation and hospital stay, as well as 30-day major and minor complications were collected from the database. Chi square tests were utilized to compare the unadjusted rates of complications between patients whose LOS was 0, 1, 2, and 3–4 days. Multivariable regression was utilized to evaluate the effect of LOS on complication rates, while adjusting for age, American Society of Anesthesiologist (ASA) class, type of anaesthesia, functional status, comorbidities, sex, steroid/immunosuppressant use, body mass index (BMI), diabetes, length of operation and smoking status. Results. A total of 198,191 TKA patients were identified (average age 66 ± 10 years). Average LOS was 2.5 ± 0.81 days. Of these patients, 1,667 (0.84%) were discharged the day of surgery, while 16,186 (8.17%) were discharged one-day post-surgery, 63,540 (32.06%) were discharged on day two, and 115,471 (58.25%) were discharged between 3 and 4 days post-surgery. Unadjusted rates of complications were significantly higher for patients who were discharged on the day of surgery (2.64%), or those whose LOS was 3–4 days (2.78%) compared to those whose LOS was 1 (1.40%) or 2 days (1.67%). After adjusting for all relevant covariables, discharge on the day of surgery increased the risk of major complications by 1.8 (95% Confidence interval [95%CI] 1.2–2.8), and minor complications by 1.6 (95%CI 1.2 – 2.2) compared to patients whose LOS was 2 days. A LOS of 1 day did not affect the risk of major complications compared to a LOS of 2 days, while a LOS of 3–4 days increased the risk of major complications by 1.4 (95%CI 1.3 – 1.6), and minor complications by 1.6 (95%CI 1.4 – 1.7). Regarding specific complications, same-day discharge remained an independent predictor of myocardial infarction (MI), cardiac arrest, pulmonary embolism (PE), unplanned reintubation, >48 hours on ventilator, pneumonia and renal insufficiency, while a LOS of 3–4 days remained an independent predictor of MI, cardiac arrest, sepsis, acute renal failure, PE, unplanned reintubation, >48 hours on ventilator, stroke, deep vein thrombosis, pneumonia, urinary tract infection, renal insufficiency, deep and superficial surgical site infection, and wound disruption compared to patients whose LOS was 2 days (p<0.01). Conclusion. Discharge on days 1–2 postoperatively following TKA appears to have superior outcomes compared to discharge on the day of surgery, or on days 3–4. Prospective clinical data is required to confirm the effect of discharge timing on postoperative outcomes following TKA

Introduction. Patient satisfaction is an important outcome measure after total knee arthroplasty (TKA) and is the ultimate goal of surgery. However, patient satisfaction reflects a complex construct of the patient's personal expectations and preferences in addition to subjective evaluation of outcome after TKA. Multiple studies have found a patient dissatisfaction rate of approximately 20% at 1 year after TKA. The purpose of this study is to determine whether there is an association between a single-item validated TKA satisfaction score and patient-reported outcome measures (PROMs) at 3 time points (1, 2, and 5 years after TKA) and to determine if dissatisfaction rate after TKA varies over time. Methods. A multi-center, prospective cohort of 12,952 patients (8,078 patients were assessed at 1-year, 702 patients at 2-year, and 4,172 patients at 5-year) undergoing primary TKA were enrolled by 230 surgeons in 28 states between 2012–2015. Surgeons practices varied in size, reimbursement models, and geographic setting ensuring that the cohort included diverse patient populations and delivery models. Surgeons agreed to invite all TKA patients to participate and sporadic audits of surgical logs validated that all patients were invited and > 90% of patients were included. Demographic and clinical data [age, gender, body mass index (BMI), and modified Charlson co-morbidity index (CCI)] were collected. Patient-reported outcome measures (PROMs) were collected pre-op and post-op at 1, 2, and 5 years using an internet-based platform including the KOOS (total score, and pain, ADL, QoL sub scores), KOOS Jr, SF-36 (PCS and MCS). We used the single-item satisfaction scale which was tested and validated by the Swedish Knee Arthroplasty Registry. The patients' responses were made on 5-point Likert scale (very satisfied, somewhat satisfied, neutral, somewhat dissatisfied, very dissatisfied). Patients were classified into 2 categories: satisfied group for patients who answered satisfied or very satisfied and dissatisfied group for patients answered neutral, somewhat dissatisfied, or very dissatisfied. Univariate analysis of the difference between the satisfied and dissatisfied patients' groups was performed using Mann-Whitney U test for continuous variables and chi-square test for categorical variables. Logistic regression model was performed to study the correlation between the satisfaction and PROMs with 95% confidence interval. Results. Mean age was 66.6 years, 62.7% were female, and mean BMI was 31.6. The CCI was 0 in 55.1%, 1 in 22%, 2–5 in 12.6% and ≥ 6 in 10.3%. Cumulative revision rate was 1.29% at 2 years. The 30-day adverse events incidence was 2.5% while the 90-day adverse events incidence was 4.7%. The dissatisfaction rate was 18.1% at 1-year, 11.5% at 2-year, and 8.5% at 5-year (P<0.001, Chi-square). Dissatisfaction significantly affects younger patients (<55 years) (P=0.04, Chi-square) and patients with high Charlson comorbidity index >1 (P=0.001, Chi-square). Low post-operative KOOS Pain, KOOS ADL, KOOS total score, KOOS JR, SF-36 PCS, and SP-36 MCS scores were significantly associated with dissatisfaction (P<0.001). At 5 years follow-up, in patients with KOOS scores greater than 70, 1.3% of patients were dissatisfied; with KOOS 50–70, 16.3% were dissatisfied and when KOOS < 50, 62.2% are dissatisfied. Logistic regression showed significant correlation of satisfaction with postoperative KOOS pain, KOOS ADL, KOOS QoL, KOOS total score, KOOS JR, and SF-36 PCS (P< 0.001) at 1-year, 2-year, and at 5-year. The MCS was correlated with patient satisfaction only at 1-year (P< 0.001). Conclusion. The patient dissatisfaction rate 5 years after TKA is 8.1% in FORCE-TJR patients which is significantly lower than 18.1% at 1-year. Less improvement of PROM scores after TKA are significantly associated with patient dissatisfaction. Postoperative PROM scores are associated with patient satisfaction at 1-year, 2-years, and 5-years. When the 5-year post- op KOOS total score is >70, 98.7% of patients are satisfied and only 1.3% are dissatisfied. Patient satisfaction is an important outcome measure after TKA that can be determined by asking a single question. However, in order to understand why a patient is dissatisfied, KOOS scores (KOOS pain, KOOS ADL, KOOS QoL) that assess specific postoperative outcomes can assist in determining the reason for patient dissatisfaction after TKA
