Aims. The aim of this study is to report the implant survival and factors associated with revision of total elbow arthroplasty (TEA) using data from the Dutch national registry. Methods. All TEAs recorded in the Dutch national registry between 2014 and 2020 were included. The Kaplan-Meier method was used for survival analysis, and a logistic regression model was used to assess the factors associated with revision. Results. A total of 514 TEAs were included, of which 35 were revised. The five-year implant survival was 91%. Male sex, a higher BMI, and previous surgery to the same elbow showed a statistically significant association with revision (p < 0.036). Of the 35 revised implants, ten (29%) underwent a second revision. Conclusion. This study reports a five-year implant survival of TEA of 91%. Patient factors associated with revision are defined and can be used to optimize informed consent and shared decision-making. There was a high rate of secondary revisions. Aims. Advances in treatment have extended the life expectancy of patients with metastatic bone disease (MBD). Patients could experience more skeletal-related events (SREs) as a result of this progress. Those who have already experienced a SRE could encounter another local management for a subsequent SRE, which is not part of the treatment for the initial SRE. However, there is a noted gap in research on the rate and characteristics of subsequent SREs requiring further localized treatment, obligating clinicians to extrapolate from experiences with initial SREs when confronting subsequent ones. This study aimed to investigate the proportion of MBD patients developing subsequent SREs requiring local treatment, examine if there are prognostic differences at the initial treatment between those with single versus subsequent SREs, and determine if clinical, oncological, and prognostic features differ between initial and subsequent SRE treatments. Methods. This retrospective study included 3,814 adult patients who received local treatment – surgery and/or radiotherapy – for bone metastasis between 1 January 2010 and 31 December 2019. All included patients had at least one SRE requiring local treatment. A subsequent SRE was defined as a second SRE requiring local treatment. Clinical, oncological, and prognostic features were compared between single SREs and subsequent SREs using Mann-Whitney U test, Fisher’s exact test, and Kaplan–Meier curve. Results. Of the 3,814 patients with SREs, 3,159 (83%) patients had a single SRE and 655 (17%) patients developed a subsequent SRE. Patients who developed subsequent SREs generally had characteristics that favoured longer survival, such as higher BMI, higher albumin levels, fewer comorbidities, or lower neutrophil count. Once the patient got to the point of subsequent SRE, their clinical and oncological characteristics and one-year survival (28%) were not as good as those with only a single SRE (35%; p < 0.001), indicating that clinicians’ experiences when treating the initial SRE are not similar when treating a subsequent SRE. Conclusion. This study found that 17% of patients required treatments for a second, subsequent SRE, and the current clinical guideline did not provide a specific approach to this clinical condition. We observed that referencing the initial treatment, patients in the subsequent SRE group had longer six-week, 90-day, and one-year median survival than patients in the single SRE group. Once patients develop a subsequent SRE, they have a worse one-year survival rate than those who receive treatment for a single SRE. Future research should identify prognostic factors and assess the applicability of existing survival prediction models for better management of subsequent SREs. Aims

This scoping review aims to identify patient-related factors associated with a poorer outcome following total ankle arthroplasty (TAA).

Methods

A scoping review was performed using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. A computer-based literature search was performed in PubMed, Embase, Cochrane trials, and Web of Science. Two reviewers independently performed title/abstract and full-text screening according to predetermined selection criteria. English-language original research studies reporting patient-related factors associated with a poorer outcome following TAA were included. Outcomes were defined as patient-reported outcome measures (PROMs), perioperative complications, and failure.

Aims. Although periacetabular osteotomies are widely used for the treatment of symptomatic dysplastic hips, long-term surgical outcomes and patient-reported outcome measures (PROMs) are still unclear. Accordingly, we assessed hip survival and PROMs at 20 years after transpositional osteotomy of the acetabulum (TOA). Methods. A total of 172 hips in 159 patients who underwent TOA were followed up at a mean of 21.02 years (16.6 to 24.6) postoperatively. Kaplan-Meier analysis was used to assess survivorship with an endpoint of total hip arthroplasty (THA). PROMs included the visual analogue scale (VAS) Satisfaction, VAS Pain, Oxford Hip Score (OHS), and Forgotten Joint Score-12 (FJS-12). Thresholds for favourable outcomes for OHS (≥ 42) and FJS-12 (≥ 51) were obtained using the receiver operating characteristic curve with VAS Satisfaction ≥ 50 and VAS Pain < 20 as anchors. Results. THA was performed on 37 hips (21.5%) by the latest follow-up. Kaplan-Meier analysis indicated that the hip survival rate at 20 years was 79.7% (95% confidence interval (CI) 73.7 to 86.3). Multivariate analysis showed that preoperative Tönnis grade significantly influenced hip survival. Tönnis grades 0, 1, and 2 were associated with 20-year survival rates of 93.3% (95% CI 84.8 to 100), 86.7% (95% CI 79.8 to 94.3), and 54.8% (95% CI 41.5 to 72.3), respectively. More than 60% of the patients exhibited favourable PROMs. An advanced Tönnis grade at the latest follow-up and a higher BMI were both significantly associated with unfavourable OHS, but not with other PROMs. Conclusion. This study demonstrated the durability of TOA for hips with Tönnis grades 0 to 1 at 20 years. While the presence of advanced osteoarthritis and higher BMI was associated with lower hip functions (OHS), it was not necessarily associated with worse patient satisfaction and joint awareness. Total hip arthroplasty (THA) has high rates of patient satisfaction; however patient expectations for recreational and sporting activities are not always met. Our study aimed to identify preoperative factors that predict whether patient expectations for sporting or recreational activity are met 12 months following THA. Patient reported outcome measures (PROMs) were collected prospectively from 2015-2018 at one private hospital in Sydney. Age, gender, postcode, weight, and height were recorded preoperatively. Included participants underwent primary THA by one of the investigating surgeons. Univariable and multivariable analyses were performed with an expectation fulfilment score used as the primary outcome variable. Preoperative predictor variables included: age, gender, BMI, Socio-economic Indexes for Areas (SEIFA), Oxford Hip Score, Hip Osteoarthritis Outcome Score, EQ-5D-5L and EQ Visual Analogue Scale (EQ VAS). 1019 participants were eligible and included. 13% reported that preoperative expectations of sport or recreation were not met at 12 months. Younger age, lower preoperative EQ VAS, and higher BMI were associated with failed expectations on multivariable analysis. Odds of failed expectations increased by 2% for every one year younger in age (OR= 0.98, 95% CI = 0.96 to 1.00, p=.048), by 2% for every one point lower on EQ VAS (OR=0.98, 95% CI = 0.98 to 0.99, p=.002), and by 4% for every one-point increase in BMI (OR = 1.04, 95% CI = 1.00 to 1.09, p=.042). Failure to have expectations met for sporting or recreational activity was associated with younger age, poorer general health, and high BMI. With a rise in younger patients who likely have higher physical demands, a tailored preoperative education is preferable to generic models to better manage patient expectations. Younger age, higher BMI, and poorer health may predict unmet expectations for sport and recreation after THA. Tailored education in these groups should be considered

Background. Obesity has been linked with increased rates of knee osteoarthritis. Limited information is available on the survival and functional outcome results of rTKR in the obese patients. This registry-based study aimed to identify whether BMI is an independent risk factor for poorer functional outcomes and /or implant survival in rTKA. Methods. New Zealand Joint Registry (NZJR) data of patients who underwent rTKA from 1st January 2010 to January 2023 was performed. Demographics, American Society of Anesthesiologists (ASA), BMI, Operative time, indications for revision and components revised of the patients undergoing rTKA was collected. Oxford knee score (OKS) at 6 months and rates of second revision (re- revision) were stratified based on standardised BMI categories. Results. Of the 2687 revisions, functional outcome scores were available for 1261 patients. Oxford knee scores following rTKA are significantly inferior in higher BMI patients (36.5 vs 31.5 p<0.001). This held true when adjusted for age (35.7 vs 30.9 p<0.001). Tibial component loosening was a more common indication for revision in patients with BMI >40 (31.1% vs 21% for BMI <25), whereas periprosthetic femoral fracture was significantly more commonly seen in patients with BMI <25. Re-revision rates displayed no significant differences between any pairs of BMI groups (2.18/100 component years) and adjusting for age and sex did not alter this (p= 0.462). Indications for re-revision were also not different between BMI categories. Over 50% of the rTKA patients were obese. Significantly more obese patients were ASA grade 3,4 and more were <75 years. Operative time was longer in the obese patients (p<0.001). Conclusions. Although overall re-revision rates are similar between all BMI categories, the functional outcomes favour those with lower BMI. Patients with higher BMI are younger, more comorbid and carry potentially higher perioperative risks. The registry data provides valuable information when providing counsel to patients undergoing rTKA and lends further support to optimising patients prior to pTKA

In health care, several studies have suggested worse outcomes in African-Americans. Our objective was to study the relationships of race with outcomes in a series of total hip arthroplasty (THA) cases. A consecutive series of 2,435 total hip/knee arthroplasties (primaries and revisions) performed in a single hospital by a single surgeon were studied. Revisions due to infections were excluded. Data on race was available for 718 THAs which were finally included. Cases of African-Americans (AA) (n=55) and Whites (n=663) were compared on baseline demographics, Charlson, ASA, preoperative-diagnosis; preoperative and postoperative pain intensity/frequency visual-analogue-scale, QWB-7, SF-36, WOMAC, Hip Harris, Postel-D'Aubigne scores; and on postoperative transfusion rates. T-tests, Chi-Square, MANCOVA (age, ethnicity, BMI, and preoperative-diagnosis adjustments) were used. Mean follow-up: 3 years. Alpha was set at 0.05. At baseline and compared to Whites, African-Americans were significantly younger (mean, 67 vs. 59 years), had fewer Hispanics (61% vs. 26%), and had higher BMI (28.5 vs. 30.6 Kg/m. 2. ) (all p≤0.048), respectively. Preoperative diagnoses were significantly different (p<0.001). There were no significant differences between the groups on preoperative scores after adjusting for confounders. Postoperatively, SF-36 bodily-pain (70 vs. 57), SF-36 mental-component-summary (56 vs. 53), WOMAC-total (5 vs. 12), and WOMAC-stiffness (0.14 vs. 0.57) were significantly worse in African-Americans (all p≤0.043), respectively. African-Americans underwent more transfusions (28% vs. 65%, p=0.001). Compared to Whites, African-Americans underwent THA earlier in life, with higher BMI, and different preoperative-diagnosis. They also had worse postoperative scores and more transfusions. Race seems to be strongly associated with outcomes

Although periacetabular osteotomies are widely used for the treatment of symptomatic acetabular dysplasia, the surgical outcomes after long term follow-up are still limited. Thus, we assessed hip survival and patient-reported outcomes (PROMs) at 20 years after the transposition osteotomy of acetabulum (TOA). Among 260 hips in 238 patients treated with TOA, 172 hips in 160 patients were evaluated at average 20.8 years, excluding patients who died or lost to follow-up. Kaplan-Meier analysis was used to assess survivorship with an end-point of THA. PROMs were evaluated using the VAS satisfaction, VAS pain, Oxford hip score (OHS), and Forgotten joint score (FJS). The thresholds of favorable outcomes of FJS and OHS were obtained using the receiver-operating characteristic curve with VAS satisfaction ≥ 50 and VAS pain < 20 as anchors. Thirty-three hips (19.2%) underwent THA at average 13.3 years after TOA. Kaplan-Meier analysis revealed hip survival rate at 20 years was 79.7%. Multivariate analysis showed the preoperative Tönnis grade significantly influenced hip survival. Survival rates with Tönnis grade 0, grade 1, and grade 2 were 93.3%, 86.7%, and 54.8% at 20 years, respectively. More than 60% of the patients showed favorable PROMs (VAS satisfaction ≥ 50, VAS pain < 20, OHS ≥ 42, FJS ≥ 51). Advanced Tönnis grade at the latest follow-up and higher BMI were significantly associated with unfavorable OHS, but not with other PROMs. This study demonstrated the durability of TOA for hips with Tönnis grade 0–1 and favorable satisfaction in majority of the patients at 20 years after surgery. Current presence of advanced osteoarthritis is associated with the lower hip function (OHS), but not necessarily associated with subjective pain and satisfaction. Higher BMI also showed a negative impact on postoperative function

Aims. Large bone defects resulting from osteolysis, fractures, osteomyelitis, or metastases pose significant challenges in acetabular reconstruction for total hip arthroplasty. This study aimed to evaluate the survival and radiological outcomes of an acetabular reconstruction technique in patients at high risk of reconstruction failure (i.e. periprosthetic joint infection (PJI), poor bone stock, immunosuppressed patients), referred to as Hip Reconstruction In Situ with Screws and Cement (HiRISC). This involves a polyethylene liner embedded in cement-filled bone defects reinforced with screws and/or plates for enhanced fixation. Methods. A retrospective chart review of 59 consecutive acetabular reconstructions was performed by four surgeons in a single institution from 18 October 2018 to 5 January 2023. Cases were classified based on the Paprosky classification, excluding type 1 cases (n = 26) and including types 2 or 3 for analysis (n = 33). Radiological loosening was evaluated by an orthopaedic surgeon who was not the operating surgeon, by comparing the immediate postoperative radiographs with the ones at latest follow-up. Mean follow-up was 557 days (SD 441; 31 to 1,707). Results. Out of the 33 cases analyzed, six (18.2%) constructs required revision, with four revisions due to uncontrolled infection, one for dislocation, and one for aseptic loosening. Among the 27 non-revised constructs, only one showed wider radiolucencies compared to immediate postoperative radiographs, indicating potential loosening. Patients who underwent revision (n = 6) were significantly younger and had a higher BMI compared to those with non-revised constructs (p = 0.016 and p = 0.026, respectively). Sex, race, ethnicity, American Society of Anesthesiologists grade, infection status (patients with postoperative PJI diagnosis (septic) vs patients without such diagnosis (aseptic)), and mean follow-up did not significantly differ between revised and non-revised groups. Conclusion. The HiRISC technique may serve as a feasible short-term (about one to two years) alternative in patients with large acetabular defects, particularly in cases of PJI. Non-surgical osteoarthritis management includes analgesia escalation to oral opiates; however, tolerance can occur. This study aims to assess analgesic effects of opiate use pre-operatively and whether this influences outcome 1-year post-operatively in patients undergoing total hip/knee arthroplasty (THA/TKA). This prospective study assessed 1487 patients undergoing primary THA (n=729) or TKA (n=758) for osteoarthritis, with 95 respectively reporting pre-operative opiate use >1 month. THA opiate users had significantly higher BMI (p=0.007) and more likely to suffer associated comorbidities. TKA opiate users were significantly younger (p<0.001), with higher BMI (p=0.019) and more likely to suffer associated comorbidities. Pre-operative quality of life (QoL) and joint specific function were significantly worse (Hip EQ-5D 0.17 vs 0.41, p<0.001, OHS 14.6 vs 21.2, p<0.001; Knee EQ-5D 0.27 vs 0.44, p<0.001, OKS 16.4 vs 21.4, p<0.001). Pre-operative pain was significantly worse in those taking opioids (Hip Pain VAS 42.73 vs 50.70, p<0.001; Knee Pain VAS 50.93 vs 53.36, p=0.30). Post-operatively the THA opiate group had significant improvement in EQ-5D (0.175, p<0.001) and OHS (6.5, p<0.001) but were significantly less improved than opiate naïve patients after adjusting for confounding (EQ-5D 0.10, p<0.001; OHS 3.2, p<0.001). TKA opiate group also had significant improvement in EQ-5D (p<0.001) and OKS (p<0.001) but were significantly less (EQ-5D 0.089, p<0.001; OKS 3.9, p<0.001) than opioid naïve patients. Pre-operative opiate use was associated with significantly worse pre-operative QoL, joint specific function and worse subjective pain. Post-operatively, the opiate group had significantly lower improvement in their QoL and joint specific function

Abstract. Introduction. The Oxford Unicompartmental Knee Replacement's (OUKR's) fully-congruent design minimises polyethylene wear. Consequently, wear is a rare failure mechanism. Phase-3 OUKR linear wear at 5 years was higher than previous OUKR phases, but very low compared to fixed-bearing UKRs. This study aimed to measure OUKR bearing wear at 10 years and investigate factors that may affect wear. Methodology. Bearing thickness for 39 OUKRs from a randomised study was calculated using radiostereometric analysis at regular intervals up to 10 years. Data for 39 and 29 OUKRs was available at 5 and 10 years, respectively. As creep occurs early, wear rate was calculated using linear regression between 6 months and 10 years. Relationships between wear and patient factors, fixation method, Oxford Knee Score (OKS), bearing position, and component position were analysed. Results. The mean wear rate was 0.06mm/year. Fixation method, age, OKS, component size, and bearing size had no correlation with wear. A higher BMI was associated with lower wear (p=0.01). Bearings more than 4mm from the wall had significantly more wear (p=0.04) than those less than 4mm from the wall. There was a linear correlation between the femoral component contact area on the bearing and wear (p=0.04). Conclusions. Phase-3 bearing wear rate is constant, significantly higher than previous OUKR phases at 10 years, and may increase the risk of long-term bearing failure. To minimise complications associated with wear, size 4 bearings should be used in young patients and manufacturing bearings from more durable, highly crosslinked polyethylene should be studied

Adult hip dysplasia AHD is a complex 3D pathology of lateral coverage, version and/or volume and is often associated with increased femoral anteversion. The Mckibbin index MI is the combination of acetabular version AV and femoral version FV and is used as a measure of anterior hip Stability/ Impingement(1). The Bernese Periacetabular osteotomy PAO is a powerful tool in treating AHD, but it does not address FV. De-rotational femoral osteotomies FO increases risk of complications, operative time and might condemn the patient to Secondary osteotomies to balance the gait. We aim to investigate the effect of MI and FV on PROMs in patients undergoing PAO only. 593 PAOs identified on the Local Hip preservation registry between 01/2013 and 7/2023. PAOs for retroversion, residual Perthes and those combined with FO were excluded. Patients with no available PROMS at 2 years were excluded. Independent variables were collected from E-notes and imaging including MI and FV. A multiple linear regression analysis was performed against preoperative iHot12, and iHot12 change at 2years. The mean FV was 18.86± 12.4 SD. Mean MI was 36.07, SD 15.36. Mean preop iHot12 score was 29.83 ± 17.38 SD. Mean change in iHot12 at 2 years was+36.47 ± 28.44 SD. Females and a higher BMI were statistically correlated to a lower preoperative iHot12. A lower preop iHot12 score and a higher preop AI were correlated to a bigger change in iHot12 at 2years with statistical significance. MI and FV were not found to have a statistically significant correlation with Outcome measures,. An increased Mckibbin index and femoral anteversion were not correlated with worse outcomes at two years. PAO alone in the presence of increased femoral anteversion avoids risks associated with FO which can be performed later if required

Introduction. Ankle fractures represent approximately 10% of the fracture workload and are projected to increase due to ageing population. We present our 5 years outcome review post-surgical management of ankle fractures in a large UK Trauma unit. Methods. A total of 111 consecutive patients treated for an unstable ankle fracture were entered into a database and prospectively followed up. Baseline patient characteristics, complications, further intervention including additional surgery, functional status were recorded during five-year follow-up. Pre-injury and post-fixation functional outcome measures at 2-years were assessed using Olerud-Molander Ankle Scores (OMAS) and Lower Extremity Functional Scales (LEFS). A p value < 0.05 was considered significant. Results. The mean age was 46 with a male:female ratio of 1:1.1. The distribution of comorbidities was BMI >30 (25%), diabetes (5%), alcohol consumption >20U/week (15%) and smoking (26%). Higher BMI was predictive of worse post-op LEFS score (p = 0.02). Between pre-injury and post fixation functional scores at 2 years, there was a mean reduction of 26.8 (OMAS) and 20.5(LEFS). Using very strict radiological criteria, 31 (28%) had less than anatomical reduction of fracture fragments intra-operatively. This was, however, not predictive of patients' functional outcome in this cohort. Within 5-year period, 22 (20%) patients had removal of metalwork from their ankle, with majority 13 (59%) requiring syndesmotic screw removal. Further interventions included: joint injection (3), deltoid reconstruction (1), arthroscopic debridement (1), superficial sinus excision (2), and conversion to hindfoot nail due to failure of fixation (1). Reduction in OMAS was predictive of patients' ongoing symptoms (p=0.01). Conclusion. There is a significant reduction in functional outcome after ankle fracture fixation and patients should be counselled appropriately. Need for removal of metalwork is higher in patients who require syndesmosis stabilisation with screw(s)

Aims. Few risk factors for rotator cuff disease (RCD) and corresponding treatment have been firmly established. The aim of this study was to evaluate the relationship between numerous risk factors and the incidence of surgery for RCD in a large cohort. Methods. A population-based cohort of people aged between 40 and 69 years in the UK (the UK Biobank) was studied. People who underwent surgery for RCD were identified through a link with NHS inpatient records covering a mean of eight years after enrolment. Multivariate Cox proportional hazards regression was used to calculate hazard ratios (HRs) as estimates of associations with surgery for RCD accounting for confounders. The risk factors which were considered included age, sex, race, education, Townsend deprivation index, body mass index (BMI), occupational demands, and exposure to smoking. Results. Of the 421,894 people who were included, 47% were male. The mean age at the time of enrolment was 56 years (40 to 69). A total of 2,156 people were identified who underwent surgery for RCD. Each decade increase in age was associated with a 55% increase in the incidence of RCD surgery (95% confidence interval (CI) 46% to 64%). Male sex, non-white race, lower deprivation score, and higher BMI were significantly associated with a higher risk of surgery for RCD (all p < 0.050). Greater occupational physical demands were significantly associated with higher rates of RCD surgery (HR = 2.1, 1.8, and 1.4 for ‘always’, ‘usually’, and ‘sometimes’ doing heavy manual labour vs ‘never’, all p < 0.001). Former smokers had significantly higher rates of RCD surgery than those who had never smoked (HR 1.23 (95% CI 1.12 to 1.35), p < 0.001), while current smokers had similar rates to those who had never smoked (HR 0.94 (95% CI 0.80 to 1.11)). Among those who had never smoked, the risk of surgery was higher among those with more than one household member who smoked (HR 1.78 (95% CI 1.08 to 2.92)). The risk of RCD surgery was not significantly related to other measurements of secondhand smoking. Conclusion. Many factors were independently associated with surgery for RCD, including older age, male sex, higher BMI, lower deprivation score, and higher occupational physical demands. Several of the risk factors which were identified are modifiable, suggesting that the healthcare burden of RCD might be reduced through the pursuit of public health goals, such as reducing obesity and modifying occupational demands. Open tibial fractures can be difficult to manage, with a range of factors that could affect treatment and outcome. We present a large cohort of patients, and analyse which factors have significant associations with infection outcome. Elucidation will allow clinicians to strive for treatment optimisation, and patients to be advised on likely complications. Open tibia fractures treated at a major trauma centre between 2015-2021 were included. Mean age at injury was 55.4 (range 13-102). Infection status was categorized into no infection, superficial infection, and osteomyelitis. Age, mode of injury, polytrauma, fibula status, Gustilo-Anderson (GA) classification, wound contamination, time from injury to: first procedure/definitive plastics procedure/definitive fixation, type of definitive fixation, smoking and diabetic status, and BMI, were collected. Multicollinearity was calculated, with highly correlated factors removed. Multinomial logistic regression was performed. Chi Squared testing, with Post Hoc Bonferroni correction was performed for complex categorical factors. Two hundred forty-four patients with open tibial fractures were included. Forty-five developed superficial infection (18.4%), and thirty-nine developed osteomyelitis (16.0%). Polytrauma, fibula status, and type of definitive fixation were excluded from the multivariate model due to strong multicollinearity with other variables. With reference to the non-infected outcome; superficial infection patients had higher BMI (p<0.01), higher GA grade (p<0.01), osteomyelitis patients had longer time to definitive fixation (p=0.049) and time to definitive plastics procedure (p=0.013), higher GA grade (p<0.01), and positive wound contamination(p=0.015). Poc hoc analysis showed “no infection” was positively associated with GA-I (p=0.029) and GA-II (p<0.01), and negatively associated with GA-IIIC (p<0.01). Osteomyelitis was positively associated with GA-IIIc (p<0.01). This study investigated the associations between the injury and presentation factors that may affect infection outcome. The variables highlighted are the factors clinicians should give extra consideration to when treating cases, and take preventative measures to optimize treatment and mitigate infection risk

Background. Post-operative acute kidney injury is significant complication following surgery. Patients who develop AKI have an increased risk for progression into chronic kidney disease, end-stage renal failure and increased mortality risk. The patient outcomes following total knee replacement (TKR), who develop AKI has been a topic of interest in recent years as it may have patient and medicolegal implications. Nevertheless, there are no studies looking at the incidence, risk factors and outcomes of AKI following bilateral TKRs at the same sitting. Objectives. To determine the incidence, risk factors and outcomes of post-operative AKI following bilateral TKRs surgery at the same sitting. Methods. This was a retrospective single-centre study performed at the Princess Royal Hospital, which performed a total of 25 BTKR. The incidence, Surgical and patient risk factors were recorded and analysed. Results. The incidence of AKI as defined by NICE guidelines following bilateral TKRs was 20%. 16% (4 patients) had stage 1 and 4% (1 patient) had stage 2 AKI. The mean change in Creatinine between pre- and post-operative blood tests was +19μmol/L. There was a strong significant correlation between CKD and AKI (r=0.75, P<0.05). Furthermore, a moderate correlation was found between higher BMI and pre-operative Charlson index and AKI. AKI did not have an effect on the length of inpatient stay with the mean inpatient length of stay for patients who had an AKI of 10 days compared to 11days for those who did not. All AKIs were resolved within 72 hours. There were no associated mortalities with AKI. Conclusion. The incidence of AKI following bilateral TKR was 20%. Pre-operative chronic kidney disease as well as having a higher BMI were identified as risk factors for developing AKI. Pre-operative CKD optimisation and careful adequate hydration intra-operatively should be considered in these patients. AKI was not associated with an increased length of stay or mortality in our study

Background. In recent years, ‘Get It Right First Time (GIRFT)’ have advocated cemented replacements in femoral part of Total hip arthroplasty (THA) especially in older patients. However, many studies were unable to show any difference in outcomes and although cemented prostheses may be associated with better short-term pain outcomes there is no clear advantage in the longer term. It is not clear when and why to do cemented instead of cementless. Aim. To assess differences in patient reported outcomes in uncemented THAs based on patient demographics in order to decide when cementless THA can be done safely. Method. Prospective data collection of consecutive 1079 uncemented THAs performed for 954 patients in single trust between 2010 and 2020. Oxford Hip Score (OHS) and complications were analysed against demographic variables (age, sex, BMI, ASA) and prosthesis features (femoral and acetabular size, offset and acetabular screws). Results. The mean pre-operative OHS was 14.6 which improved to 39.0 at 1 year follow up (P Value=0.000). There was no statistically significant difference between OHS outcome in patients aged over 70 versus younger groups. With a small number of revisable complications increase with age from 50s upwards. Male patients’ OHS score was on average 2.4 points higher than women. Men, however were 2.9 times more likely to experience fractures and high offset hips were 2.5 times more likely to experience dislocations. DAIR, intraoperative calcar fractures, post-operative fractures and dislocations were not associated with worse OHS. Patients with increased BMI had worse pre and post-operative hip functions yet, there was a significant multivariate association between increased BMI and increased improvement in OHS from pre-op to 1 year in women aged 55–80 and men under the age of 60. Femoral stem size increases with age but decreases in male patients over 80. There was no difference found in OHS between bilateral hip replacements and unilateral, nor was there any change found with laterality side of the replacement. Conclusions. This study suggests that ageing >70 is not associated with poorer outcomes despite small number of revisable complication rates that increase with age from 50 upwards. Men had marginally higher average OHS than women At 1 year. Higher BMI or ASA scores are associated with worse pre-operative hips and worse final outcome score. Despite this, the Delta OHS increases with increased BMI shown in the 55–80 year old female patients and male patients under 60. Key Words: THR, Uncemented, Oxford hip score, outcome

Aim. Prosthetic joint infection (PJI) is a devastating and costly complication of total joint arthroplasty (TJA). Use of extended oral antibiotic prophylaxis (EOAP) has become increasingly popular in the United States following a highly publicized study (Inabathula et al) from a single center demonstrating a significant protective effect (81% reduction) against PJI in ‘high-risk’ patients. However, these results have not been reproduced elsewhere and EOAP use directly conflicts with current antibiotic stewardship efforts. In order to study the role of EOAP in PJI prevention, consensus is needed for what defines ‘high-risk’ patients. The revision TJA (rTJA) population is an appropriate group to study due to having a higher incidence of PJI. The purpose of the current study was to rigorously determine which preoperative conditions described by Inabathula et al. (referred to as Inabathula criteria (IBC)) confer a higher rate of PJI in patients undergoing aseptic rTJA. Method. 2,256 patients that underwent aseptic rTJA at a single high-volume institution between 2016–2022 were retrospectively reviewed. Patient demographics and comorbidities were recorded to determine if they had 1 or more ‘IBC’, a long list of preoperative conditions including autoimmune diseases, active smoking, body mass index (BMI)>35, diabetes mellitus, and chronic kidney disease (CKD). Reoperation for PJI at 90-days and 1-year was recorded. Chi-squared or Fischer's exact tests were calculated to determine the association between preoperative presence/absence of IBC and PJI. Multivariable logistic regressions were conducted to determine if specific comorbidities within the IBC individually conferred an increased PJI risk. Results. 1223 patients (54.2%) had at least one IBC condition. IBC-positive patients were more likely to be female, have an increased ASA score, and higher BMI. IBC-positive patients had a significant increase in PJI risk at both 90-days (relative risk (RR)=2.32, p<0.0001) and 1-year (RR=2.14, p=0.002) versus IBC-negative patients. Within IBC-positive patients, every additional IBC condition conferred a 1.8× odds increase for 90-day PJI (p<0.0001), and 1.76× odds increase in 1-year PJI (p<0.0001). Multivariable logistic regression identified active smoking, BMI>35, CKD, and diabetes mellitus as being independently associated with PJI development (p<0.05). Conclusions. Over half of rTJA patients meet IBC and could be eligible to receive EOAP in the United States. However, the specific presence of active smoking, BMI>35, CKD, and diabetes mellitus appear to be responsible for the increased risk of PJI. Prospective studies investigating EOAP use for patients with these specific conditions are urgently needed to prevent unnecessary antibiotic use

Aims. The aim of this study was to determine satisfaction rates after hip and knee arthroplasty in patients who did not respond to postoperative patient-reported outcome measures (PROMs), characteristics of non-responders, and contact preferences to maximize response rates. Methods. A prospective cohort study of patients planned to undergo hip arthroplasty (n = 713) and knee arthroplasty (n = 737) at a UK university teaching hospital who had completed preoperative PROMs questionnaires, including the EuroQol five-dimension health-related quality of life score, and Oxford Hip Score (OHS) and Oxford Knee Score (OKS). Follow-up questionnaires were sent by post at one year, including satisfaction scoring. Attempts were made to contact patients who did not initially respond. Univariate, logistic regression, and receiver operator curve analysis was performed. Results. At one year, 667 hip patients (93.5%) and 685 knee patients (92.9%) had undergone surgery and were alive. No response was received from 151/667 hip patients (22.6%), 83 (55.0%) of whom were ultimately contacted); or from 108/685 knee patients (15.8%), 91 (84.3%) of whom were ultimately contacted. There was no difference in satisfaction after arthroplasty between initial non-responders and responders for hips (74/81 satisfied vs 476/516 satisfied; p = 0.847) or knees (81/93 satisfied vs 470/561 satisfied; p = 0.480). Initial non-response and persistent non-response was associated with younger age, higher BMIs, and worse preoperative PROMs for both hip and knee patients (p < 0.050). Being in employment was associated with persistent non-response for hip patients (p = 0.047). Multivariate analysis demonstrated that younger age (p < 0.038), higher BMI (p = 0.018), and poorer preoperative OHS (p = 0.031) were independently associated with persistent non-response to hip PROMs. No independent associations were identified for knees. Using a threshold of > 66.4 years predicted a preference for contact by post (area under the curve 0.723 (95% confidence interval (CI) 0.647 to 0.799; p < 0.001, though this CI crosses the 0.7 limit considered reliable). Conclusion. The majority of initial non-responders were ultimately contactable with effort. Abstract. Introduction. Cementless total knee arthroplasty (TKA) offers a number of conceptual benefits including osteointegration, bone preservation and reduced aseptic loosening from third body wear. Evidence of equivalence to cemented fixation exists, but the cam-post interaction of posterior-stabilised (PS)-TKA on implant osteointegration remains uncertain. This study aims to assess the survivorship of a single prosthesis PS-TKA. Methodology. All patients undergoing a PS-TKA using the Triathlon Total Knee System (Stryker Orthopaedics, USA) between 01/01/2010 and 08/04/2019, with exposure to at least 2 years’ risk of revision were identified from the hospital database. Results. 1001 TKAs were identified; 734 cementless and 267 cemented. The cementless group more frequently were male, younger, of higher BMI; and had larger components used (p<0.01). Mean exposure to risk of revision was longer in the cementless group (69.5 vs 57 months, p<0.05). Return to theatre rates were similar (4.6% vs 4.8% p = 0.88), a greater proportion in the cementless group underwent revision (2.9% vs 1.1% p=0.11). Multivariate analysis was only possible for reoperations, increased with cemented fixation (OR 2.34 CI 1.01-5.43) and decreased with age (OR 0.96 CI 0.92-1.00). Conclusion. Both cementless and cemented Triathlon PS-TKA demonstrate excellent survivorship, with revision rates of 2.9% and 1.1% at a mean of 5.25 years. This difference was not statistically significant, and variations in baseline characteristics may have influenced it. Further study is planned to assess radiographs of these patients for evidence of osteointegration, and we advocate further study with greater patient numbers

This study aims to determine satisfaction rates after hip and knee arthroplasty in patients who did not initially respond to PROMs, characteristics of non-responders, and contact preferences to maximise response rates. We performed a prospective cohort study of 709 patients undergoing THA and 737 patients undergoing TKA in a single centre in 2018. EQ-5D health related quality of life score and Oxford Hip/knee scores (OHS/OKS) were completed preoperatively and at 1year postoperatively via post when satisfaction was also assessed. Univariate, multivariate and receiver operator curve analysis were performed. 151/709 (21.2%) hip patients were non-responders, 83 (55.0%) of whom were contactable. 108/737 (14.6%) knee patients were non-responders, 91 (84.3%) of whom were contactable. There was no difference in satisfaction after arthroplasty between initial non-responders and responders for hips (74/81 vs 476/516, p=0.847) or knees (81/93 vs 470/561, p=0.480). Initial and persistent non-response was associated with younger age, higher BMIs and significantly worse preoperative PROMS for both hip and knee patients (p=0.05). Multivariate analysis demonstrated that younger age, higher BMI and poorer pre-operative OHS were independently associated with persistent non-response to hip PROMs (p<0.05). For the entire cohort (n=1352) patients <67 years were less likely to respond to postal PROMs with OR 0.63 (0.558 to 0.711). Using a threshold of >66.4 years predicted a preference for contact by post with 65.4% sensitivity and 68.1% specificity (AUC 0.723 (0.647-0.799 95%CI, p<0.001)). Most initial non-responders were ultimately contactable with effort. Satisfaction rates were not inferior in patients who did not initially respond to PROMs

Aims. The aim of this study was to radiologically evaluate the quality of cement mantle and alignment achieved with a polished tapered cemented femoral stem inserted through the anterior approach and compared with the posterior approach. Methods. A comparative retrospective study of 115 consecutive hybrid total hip arthroplasties or cemented hemiarthroplasties in 110 patients, performed through anterior (n = 58) or posterior approach (n = 57) using a collarless polished taper-slip femoral stem, was conducted. Cement mantle quality and thickness were assessed in both planes. Radiological outcomes were compared between groups. Results. No significant differences were identified between groups in Barrack grade on the anteroposterior (AP) (p = 0.640) or lateral views (p = 0.306), or for alignment on the AP (p = 0.603) or lateral views (p = 0.254). An adequate cement mantle (Barrack A or B) was achieved in 77.6% (anterior group, n = 45) and in 86% (posterior group, n = 49), respectively. Multivariate analysis revealed factors associated with unsatisfactory cement mantle (Barrack C or D) included higher BMI, left side, and Dorr Type C morphology. A mean cement mantle thickness of ≥ 2 mm was achieved in all Gruen zones for both approaches. The mean cement mantle was thicker in zone 7 (p < 0.001) and thinner in zone 9 for the anterior approach (p = 0.032). Incidence of cement mantle defects between groups was similar (6.9% (n = 4) vs 8.8% (n = 5), respectively; p = 0.489). Conclusion. An adequate cement mantle and good alignment can be achieved using a collarless polished tapered femoral component inserted through the anterior approach. Background. This study aimed to investigate the effect of body mass index (BMI) on functional outcome following hip preservation surgery using the U.K. Non-Arthroplasty Hip Registry (NAHR). Methods. Data on adult patients who underwent hip arthroscopy or periacetabular osteotomy (PAO) between January 2012 and December 2018 was extracted from the UK Non-Arthroplasty Hip Registry dataset allowing a minimum of 12 months follow-up. Data is collected via an online clinician and patient portal. Outcomes comprised EuroQol-5 Dimensions (EQ-5D) index and the International Hip Outcome Tool 12 (iHOT-12), preoperatively and at 6 and 12 months. Results. A total of 6,666 patients were identified with BMI data available in 52%, comprising 3,220 arthroscopies and 277 PAO. Patients were divided into WHO groups: <25kg/m. 2. (n=1,745 (49.8%)), 25–30kg/m. 2. (n=1,199 (34.2%)), and ≥30kg/m. 2. (n=562 (16.0%)). Patients with higher BMI tended to be older. Pre-operative, 6 and 12-month follow-up were available for 91%, 49% and 45% of cases respectively. Higher BMI was associated with significantly poorer baseline, 6- and 12-month outcomes (12-month mean iHOT-12 score: <25kg/m. 2. = 62.3 (95%CI 60.4 to 64.3), 25–30kg/m. 2. = 57.3 (95%CI 55.0 to 59.7), ≥30kg/m. 2. = 54.7 (95%CI 51.1 to 58.2)). However, all groups saw similar and statistically significant improvement in pre- vs post-op scores (mean 12-month iHOT-12 gain: <25kg/m. 2. = +27.1 (95%CI 25.1 to 29.0), 25–30kg/m. 2. = +26.5 (95%CI 24.0 to 29.0), ≥30kg/m. 2. = +26.8 (95%CI 23.2 to 30.4), between-group p = 0.9). EQ-5D outcomes followed the same trend. Modelling for age, sex and procedure we found no significant difference in 12-month iHOT-12 gain between BMI groups. Conclusion. Whilst obese patients started from, and achieved lower post-operative raw functional scores, all BMI groups saw similar and significant degrees of improvement in functional outcome post-operatively. Obesity should not be considered a contraindication to hip preservation surgery

Abstract. Objectives. Exploring the relationship of gait function pre and post total knee replacement (TKR) in two groups of patients. Methods. Three-dimensional gait analysis was performed at Cardiff University, UK, and Karolinska University Hospital, Sweden, on 29 and 25 non-pathological (NP) volunteers, and 39 and 28 patients with end-stage knee osteoarthritis (OA), respectively. Patients were assessed pre and one-year post-TKR. Data reduction was performed via Principal Component (PC) analysis on twenty-four kinematic and kinetic waveforms in both NP and pre/post-TKR. Cardiff's and Karolinska's cohorts were analysed separately. The Cardiff Classifier, a classification system based on the Dempster-Shafer theory, was trained with the first 3 PCs of each variable for each cohort. The Classifier classifies each participant by assigning them a belief in NP, belief in OA (BOA) and belief in uncertainty, based on their biomechanical features. The correlation between patient's BOA values (range: 0–1, 0 indicates null BOA and 1 high BOA) pre and post-TKR was tested through Spearman's correlation coefficient in each cohort. The related-samples Wilcoxon signed-rank test (α=0.05) determined the significant changes in BOA in each cohort of patients. The Mann-Whitney U test (α=0.05) was run to explore differences between the patients’ cohorts. Results. There were no significant differences between patients’ cohorts in median age (p=0.096), height (p=0.673), weight (p=0.064) or KOOS sub-scores pre or post-TKR (p-value ranged 0.069 to 0.955) but Cardiff's patients had a significantly higher BMI (p=0.047). There was a significant, median decrease of 0.12 and 0.19 in the BOA pre to post TKR (p<0.001) in Cardiff's and Karolinska's patients, respectively. There was a statistically significant, strong positive correlation between the BOA pre and post-TKR (Cardiff:r. s. =0.706, p<0.001; Karolinska:r. s. =0.669, p<0.001). Conclusions. In two distinct cohorts of patients, having a more compromised gait function in end-stage knee OA was correlated with poorer gait function post-TKR

Aims. Periprosthetic fungal infections are rare and account for 1–2% of all periprosthetic joint infections (PJI). This study aims at presenting treatment details, clinical and microbiological results in a large single centre cohort. Methods. We retrospectively identified 29 patients (9 total knee replacements (TKA) and 20 total hip replacements (THA) treated for a fungal infection between 2007 and 2019. Microbiological findings, patient demographics and complications were analysed. Statistical analysis was performed using descriptive statistics; non-parametric analysis were performed using the Mann-Whitney U-Test. Infection-free survival was determined using Kaplan-Meier analysis and differences in survival were analysed using the log-rank test. The p value was set at p<0.05 with 95% confidence intervals (95% CI) provided. Results. 28% (8/29) suffered from reinfection. The reinfection-free survival probability was 65% (95% CI 45–85) after a median follow- up period of 28 months (IQR 6 – 39). With the numbers we had, we were not able to detect a difference between THA and TKA re-infections (p=0.517). Four patients underwent amputation, 3 patients had a definitive girdlestone hip and eight patients died after a median of 5 months after first-stage surgery (IQR 1–7). All patients treated had positive synovial fluid or tissue cultures for Candida species. In 22 /29 patients C. albicans, in 3 patients C. parapsilosis, in 2 patients C. glabrata and in 1 patient each C. famata, C. dubliniensis and C. gulliermondii. Polymicrobial bacterial infection was found in 86% of patients with staphylococci in 20 patients, E. coli in 2 patients, vancomycin-resistant enterococci, pseudomonas, acinetobacter and achromobacter species in 1 patient each. When investigating risk factors for reinfection, with the numbers we had we were not able to find a significant difference for patients with polymicrobial infection (p=0.974), azole-resistant Candida (p=0.491), tobacco users (p=0.175), or diabetics (p=0.54). Furthermore, median age (73 vs. 72, p=0.756) and Charlson comorbidity score (6 (interquartile range (IQR) 4–8) vs. 8 (IQR 5–10), p=0.184) were not different between the groups while on the other hand there was a trend for a higher body mass index in patients with reinfection (34 (IQR 31–38) vs. 28 (IQR 25–33), p=0.075). Conclusions. Fungal PJI is associated with poor reinfection free survival, frequent revisions, and high mortality. All infections were caused by Candida spp. in which azole-resistance most be considered when planning treatment. While polymicrobial infection complicated treatment there was no difference in survival. A higher BMI and comorbidity score might be associated with higher risk for reinfections

The purpose of our study is to examine the outcome of patients undergoing outpatient total hip arthroplasty with a BMI >35. Case-control matching on age, gender (46% female;54%male), and ASA (mean 2.8) with 51 outpatients BMI≥35 kg/m. 2. (mean of 40 (35–55)), mean age of 61 (38–78) matched to 51 outpatients BMI<35 kg/m. 2. (mean of 27 (17–34)) mean age 61 (33–78). Subsequently 47 inpatients BMI≥35 kg/m. 2. (mean of 40 (35–55)) mean age 62 (34–77) were matched outpatients BMI≥35 kg/m. 2. For each cohort, adverse events, readmission in 90 days, reoperations were recorded. Rate of adverse events was significantly higher in BMI ≥35: 15.69% verus 1.96% (p=0.039) with 5 reoperations in the BMI≥35 cohort vs 0 in the BMI<35 kg/m. 2. (p= 0.063). Readmissions did not differ between groups (p=0.125). No significant difference for all studied outcomes between the outpatient and inpatients cohorts with BMI≥35 kg/m. 2. The most complications requiring surgery/medical intervention (3B) were in the inpatient cohort of patients >35. The prevalence of Diabetes and Obstructive Sleep apnea was 21.6% and 29.4% for BMI>35 compared to 9.8% and 11.8%, for BMI <35, respectively. Severely obese patients have an overall higher rate of adverse events and reoperations however it should not be used a sole variable for deciding if the patient should be admitted or not

The prevalence of gluteal tendinopathy (GT) associated with osteoarthritis of the hip is difficult to determine as it is frequently undiagnosed or misdiagnosed as trochanteric bursitis. Its relationship to total hip arthroplasty (THA) outcomes is currently unknown. The aim of this study was to determine the incidence of GT at the time of hip arthroplasty and examine the relationship between GT and patient reported outcomes (PROMS) before and after THA. Patients undergoing THA for primary osteoarthritis between August 2017 and August 2020 were recruited. Tendinopathy was assessed and graded at time of surgery. PROMS included the Oxford Hip Score (OHS), HOOS JR, EQ-5D, and were collected preoperatively and at one year after THA. Satisfaction with surgery was also assessed at 1 year. 797 patients met eligibility criteria and were graded as Grade 1: normal tendons (n =496, 62%), Grade 2: gluteal tendinopathy but no tear (n=222, 28%), Grade 3: partial/full thickness tears or bare trochanter (n=79, 10%). Patients with abnormal gluteal tendons were older (p=0.001), had a higher mean BMI (p=0.01), and were predominately female (p=0.001). Patients with higher grade tendinopathy had statistically significant inferior PROMS at one year, OHS score (44.1 v 42.9 v 41.3, p 0.001) HOOS JR (89.3 V 86.3 V 85.6 p 0.005). Increasing gluteal tendon grade was associated with a greater incidence of problems with mobility (p=0.001), usual activities (p=0.001) and pain (p=0.021) on EQ5D. There was a 3 times relative risk of overall dissatisfaction with THA in the presence of gluteal tears. This study demonstrated that gluteal tendinopathy was commonly observed and associated with inferior 1-year PROMS in patients undergoing THA for OA. Increasing degree of tendinopathy was a negative prognostic factor for worse functional outcomes and patient satisfaction

Aims. Thresholds for operative eligibility based on body mass index (BMI) alone may restrict patient access to the benefits of arthroplasty. The purpose of this study was to evaluate the relationship between BMI and improvements in patient-reported outcome measures (PROMs), and to determine how many patients would have been denied improvements in PROMs if BMI cut-offs were to be implemented. Methods. A prospective cohort of 3,449 primary total hip arthroplasties (THAs) performed between 2015 and 2018 were analyzed. The following one-year PROMs were evaluated: hip injury and osteoarthritis outcome score (HOOS) pain, HOOS Physical Function Shortform (PS), University of California, Los Angeles (UCLA) activity, Veterans Rand-12 Physical Component Score (VR-12 PCS), and VR-12 Mental Component Score (VR-12 MCS). Positive predictive values for failure to improve and the number of patients denied surgery in order to avoid a failed improvement were calculated for each PROM at different BMI cut-offs. Results. There was a trend to improved outcomes in terms of pain and function improvements with higher BMI. Patients with BMI ≥ 40 kg/m. 2. had median (Q1, Q3) HOOS pain improvements of 58 points (interquartile range (IQR) 41 to 70) and those with BMI 35 to 40 kg/m. 2. had median improvements of 55 (IQR 40 to 68). With a BMI cut-off of 30 kg/m. 2. , 21 patients would have been denied a meaningful improvement in HOOS pain score in order to avoid one failed improvement. At a 35 kg/m. 2. cut-off, 18 patients would be denied improvement, at a 40 kg/m. 2. cut-off 21 patients would be denied improvement, and at a 45 kg/m. 2. cut-off 21 patients would be denied improvement. Similar findings were observed for HOOS-PS, UCLA, and VR-12 scores. Conclusion. Patients with higher BMIs show greater improvements in PROMs. Using BMI alone to determine eligibility criteria did not improve the rate of clinically meaningful improvements. BMI thresholds prevent patients who may benefit the most from surgery from undergoing THA. Surgeons should consider PROMs improvements in determining eligibility for THA while balancing traditional metrics of preoperative risk stratification. Introduction. A common goal of total knee arthroplasty (TKA) is to restore normal knee kinematics. While substantial data is available on TKA kinematics, information regarding non-implanted knee kinematics is less well studied especially in larger patient populations. The objectives of this study were to determine normal femorotibial kinematics in a large number of non-implanted knees and to investigate parameters that yield higher knee flexion with weight-bearing activities. Methods. Femorotibial kinematics of 104 non-implanted healthy subjects performing a deep knee bend (DKB) activity were analyzed using 3D to 2D fluoroscopy. The average age and BMI were 38.1±18.2 years and 25.2±4.6, respectively. Pearson correlation analysis was used to determine statistical correlations. Results. On average, subjects experienced 21.5±7.2 mm, 13.8±8.9 mm, and 27.1°±12.1° of lateral rollback, medial rollback, and external femorotibial axial rotation, respectively (Figure 1). Most rollback occurred in early flexion, with 10.2±6.4 mm and 5.3±6.3 mm of rollback for the lateral and medial condyles, respectively. While the lateral condyle consistently moved posteriorly, the medial condyle experienced 1.8±4.8 mm of anterior sliding between 90° to 120° of flexion. There was a positive correlation between higher weight-bearing flexion and lateral condylar rollback (r=0.5480, p<.0001) (Figure 2), medial condylar rollback (r=0.3188, p=0.001) (Figure 3), and external axial rotation (r=0.5505, p<.0001) (Figure 4). There was an inverse correlation between advancing age and knee flexion (r=-0.7358, p<.0001) as well as higher BMI and flexion (r=-0.3332, p=0.0007), indicating that multiple factors contribute to postoperative range-of-motion. Conclusion. This represents one of the largest studies on normal knee femorotibial kinematics in non-implanted healthy subjects. These results indicate that increased condylar rollback and external axial rotation correlate with increased weight-bearing knee flexion, while increased age and BMI yield decreased flexion. Therefore, in order to achieve higher weight-bearing flexion following TKA, normal-like kinematics such as high rollback and external axial rotation should be incorporated into TKA design. Aims. Access to total knee arthroplasty (TKA) is sometimes restricted for patients with severe obesity (BMI ≥ 40 kg/m. 2. ). This study compares the cost per quality-adjusted life year (QALY) associated with TKA in patients with a BMI above and below 40 kg/m. 2. to examine whether this is supported. Methods. This single-centre study compared 169 consecutive patients with severe obesity (BMI ≥ 40 kg/m. 2. ) (mean age 65.2 years (40 to 87); mean BMI 44.2 kg/m. 2. (40 to 66); 129/169 female) undergoing unilateral TKA to a propensity score matched (age, sex, preoperative Oxford Knee Score (OKS)) cohort with a BMI < 40 kg/m. 2. in a 1:1 ratio. Demographic data, comorbidities, and complications to one year were recorded. Preoperative and one-year patient-reported outcome measures (PROMs) were completed: EuroQol five-dimension three-level questionnaire (EQ-5D-3L), OKS, pain, and satisfaction. Using national life expectancy data with obesity correction and the 2020 NHS National Tariff, QALYs (discounted at 3.5%), and direct medical costs accrued over a patient’s lifetime, were calculated. Probabilistic sensitivity analysis (PSA) was used to model variation in cost/QALY for each cohort across 1,000 simulations. Results. All PROMs improved significantly (p < 0.05) in both groups without differences between groups. Early complications were higher in BMI ≥ 40 kg/m. 2. : 34/169 versus 52/169 (p = 0.050). A total of 16 (9.5%) patients with a BMI ≥ 40 kg/m. 2. were readmitted within one year with six reoperations (3.6%) including three (1.2%) revisions for infection. Assuming reduced life expectancy in severe obesity and revision costs, TKA in patients with a BMI ≥ 40 kg/m. 2. costs a mean of £1,013/QALY (95% confidence interval £678 to 1,409) more over a lifetime than TKA in patients with BMI < 40 kg/m. 2. In PSA replicates, the maximum cost/QALY was £3,921 in patients with a BMI < 40 kg/m. 2. and £5,275 in patients with a BMI ≥ 40 kg/m. 2. . Conclusion. Higher complication rates following TKA in severely obese patients result in a lifetime cost/QALY that is £1,013 greater than that for patients with BMI < 40 kg/m. 2. , suggesting that TKA remains a cost-effective use of healthcare resources in severely obese patients where the surgeon considers it appropriate. Aims. Rotational acetabular osteotomy (RAO) is an effective joint-preserving surgical treatment for acetabular dysplasia. The purpose of this study was to investigate changes in muscle strength, gait speed, and clinical outcome in the operated hip after RAO over a one-year period using a standard protocol for rehabilitation. Patients and Methods. A total of 57 patients underwent RAO for acetabular dysplasia. Changes in muscle strength of the operated hip, 10 m gait speed, Japanese Orthopaedic Association (JOA) hip score, and factors correlated with hip muscle strength after RAO were retrospectively analyzed. Results. Three months postoperatively, the strength of the operated hip in flexion and abduction and gait speed had decreased from their preoperative levels. After six months, the strength of flexion and abduction had recovered to their preoperative level, as had gait speed. At one-year follow-up, significant improvements were seen in the strength of hip abduction and gait speed, but muscle strength in hip flexion remained at the preoperative level. The mean JOA score for hip function was 91.4 (51 to 100)) at one-year follow-up. Body mass index (BMI) showed a negative correlation with both strength of hip flexion (r = -0.4203) and abduction (r = -0.4589) one year after RAO. Although weak negative correlations were detected between strength of hip flexion one year after surgery and age (r = -0.2755) and centre-edge (CE) angle (r = -0.2989), no correlation was found between the strength of abduction and age and radiological evaluations of CE angle and acetabular roof obliquity (ARO). Conclusion. Hip muscle strength and gait speed had recovered to their preoperative levels six months after RAO. The clinical outcome at one year was excellent, although the strength of hip flexion did not improve to the same degree as that of hip abduction and gait speed. A higher BMI may result in poorer recovery of hip muscle strength after RAO. Radiologically, acetabular coverage did not affect the recovery of hip muscle strength at one year’s follow-up. Background. The impact of a patient's activity level following total hip arthroplasty (THA) remains controversial, with some concerned about increased polyethylene wear, aseptic loosening and revisions. The purpose of this study is to report on implant survivorship and outcomes of high activity patients compared to low activity patients after THA with current polyethylene. Methods. A retrospective review identified 2002 patients (2532 hip) that underwent a primary THA with vitamin E infused highly crosslinked polyethylene liner and 2-year minimum follow-up or revision. Patients were divided in two groups based on their University of California Los Angeles (UCLA) activity level: Low activity (LA) (UCLA ≤5) and high activity (HA) (UCLA ≥ 6). Outcomes included Harris Hip Score, UCLA activity score, complications and reoperations. Multivariate nominal regression analysis was performed to evaluate the significance of postoperative activity level on survivorship while controlling for age, gender, preoperative pain, HHS and body mass index (BMI). Results. Mean follow-up was 4.5 years (range, 0.3 to 9.9 years). The LA group had significantly more female patients, were older, higher BMI and lower HHS pre-operatively (all with p<0.001). HA group had significantly higher improvements in HHS (p<0.001) and UCLA activity score (p<0.001). Aseptic revisions were performed in 2.1% of the LA group and in 0.4% hips of the HA group (p<0.001). Multivariate regression analysis found that a higher postoperative activity level remained a significant factor for improved aseptic survivorship with an OR of 4.9 (95% CI, 1.1 to 21.2, p=0.03). The aseptic 5-year survivorship was 99.6% for the HA group and 98% for the LA group (p<0.001). Conclusions. Higher activity level after primary THA did not increase the risk for midterm aseptic or all cause failure. Patient activity level after THA may not need to be limited with modern polyethylene

Up to one-third of patients experience limited benefit following surgical intervention for LS-OA. Thus, identifying contributing factors to this is important. People with OA often have multijoint involvement, yet this has received limited attention in this population. We documented the occurrence and evaluated the influence of multijoint symptoms on outcome following surgery for LS-OA. 141 patients undergoing decompression surgery+/−fusion for LS-OA completed the Oswestry Disability Index (ODI) pre- and 12-months post-surgery. Also captured pre-surgery: age, sex, education, BMI, smoking, depressive symptoms and comorbidities. Any joints with “pain/stiffness/swelling most days of the month” were indicated on a homunculus. A symptomatic joint site count (e.g. one/both knees= one site), excluding the back, was derived (range zero to nine) and considered as a predictor of magnitude of ODI change, and likelihood of achieving minimally clinically important improvement in ODI (MCID=12.8) using multivariable adjusted linear and log-Poisson regression analyses. Mean age: 66 years (range:42–90), 46% female. 76% reported one+ joint site other than the back, 43% reported three+, and nearly 10% reported six+. (< MCID) for those with three sites, and four units for those with six+ sites. Associated with a greater likelihood of not achieving MCID were increasing joint count (11% increase per site (p=0.012)), higher BMI, current/former smoker, and worse baseline ODI tertile. Results suggest there is more than just the back to consider to understand patient-reported back outcomes. Multijoint symptoms directly contribute to disability, but there is potential they may contribute to systemic, largely inflammatory, effects in OA as well

Introduction. Implementing operative eligibility thresholds based on body mass index (BMI) alone risks restricting access to improved pain, function, and quality-of-life. The purpose of this study was to: 1) investigate the relationship between BMI and improvements in 1-year patient reported outcome measures (PROMs), and 2) determine how many patients would have been denied 1-year improvements with specific BMI cut-offs. Methods. Data were collected on a prospective cohort of 3,214 TKA patients from 2015–2018. Clinically meaningful 1-year improvements were defined as 15 points for Knee Injury and Osteoarthritis Outcome Scores (KOOS) pain and Physical Function Shortform (PS), and 14 points for Knee-Related Quality-of-Life (KRQOL). For specific BMI cut-offs, the positive predictive value for predicting a failure to improve and number of patients denied surgery to avoid one failed improvement was calculated. Results. PROMs improvements were greater with increasing BMI. Patients with BMI ≥40 kg/m. 2. had median (Q1, Q3) KOOS pain improvements of 47.2 points (33.3, 58.3) and those with BMI 18.5–24.9 kg/m. 2. had median improvements of 41.7 (27.8, 55.6). Similar findings were observed for KOOS PS (28.4 vs. 26.3) and KRQOL (50.0 vs. 43.8). With a BMI cut-off of 30 kg/m. 2. , 9 patients would have been denied improvements from surgery for each failed improvement avoided whereas with a BMI cut-off of 50 kg/m. 2. , 15 patients would have been denied improvements from surgery. Implementing BMI thresholds alone did not influence the rate of improvements in KOOS-PS or KRQOL. Conclusion. Patients with higher BMI were observed to have greater improvements in PROMs after primary TKA. Enforcing arbitrary BMI cut-offs would deprive pain and functional improvements from patients who would benefit the most. Moreover, there appears to be no increase in the rate of PROMs improvements by enforcing BMI thresholds. This study demonstrates that determining TKA eligibility should involve a holistic approach rather than limiting to BMI measurements alone

Introduction. Body Mass Index (BMI) is an essential tool for orthopaedic surgeons in regards to preoperative risk stratification as well as assessment of overall health and nutritional status. Patient's self-awareness of their height, weight and BMI is crucial in maintaining a healthy lifestyle. The purpose of our study was to determine the accuracy of orthopaedic patient's reported height and weight. We hypothesized that a patient's age, sex and/or BMI may affect the accuracy of these reported values. Methods. After IRB approval, we performed a prospective, observational study in the setting of our orthopaedic clinic. Patients were asked to report their predicted height and weight and then were weighed and measured using a single standardized scale. All values, including age and sex, were recorded. Differences were then calculated. Patients were categorized based on their age (younger than 65 vs older than 65), sex, and actual BMI (less than 30 vs greater than 30). Student t-test was used to calculate significance (p <0.05 conferred significance). Results. A total of 211 patients participated in our study (127 females, 84 males). Females had an average height discrepancy of 2.21cm, whereas males had an average height discrepancy of 1.56 cm (p=0.22). Females had an average weight discrepancy of 2.46 kg compared to 2.13 kg in males (p=0.58). The average height discrepancy in patients less than 65 years old was 2.09 cm compared to 1.76 cm in patients older than 65 (p=0.81). The average weight discrepancy in patients less than 65 years old was 2.50 kg compared to 2.12 kg in patients older than 65 (p=0.54). The average height discrepancy in the high BMI group was 2.29 cm compared to 1.42 in the low BMI group (p=0.11). The average weight discrepancy in the high BMI group was 2.71 kg compared to 1.72 kg in the low BMI group. This difference approached statistical significance (p=0.094). In regards to BMI changes based on values reported, 64 patients had a lower actual BMI than reported (range −0.015 to −5.29 kg/cm. 2. ), 6 patients had no change in BMI, and 141 had an increase in BMI (range 0.0006 to 16.6 kg/cm. 2. ). Average BMI of those patients with less than 1 kg/cm. 2. change in BMI was 30.9 kg/cm. 2. , whereas those with greater than 1 kg/cm. 2. change in BMI had an average of 35.18 kg/cm. 2. (p=2.27×10. −5. ). There were 9 patients' whose reported weights gave them a BMI of less than 40 kg/cm. 2. , whereas their actual weight put their BMI over 40 kg/cm. 2. Conclusion. There was a trend towards higher BMI patients (BMI >30 kg/cm. 2. ) being less accurate in predicting their height and weight compared to a lower BMI population. Patients who had a more likely chance of inaccurate weight estimation leading to a greater change in BMI had a statistically higher BMI. In 9 instances, patients' reported weights kept them below the recommended threshold for receiving a total joint replacement, whereas their actual weight put them over the recommended threshold

Background. The purpose of this study was to report functional outcome scores following arthroscopic acetabular chondral procedures using the U.K. Non-Arthroplasty Hip Registry (NAHR). Methods. Data on adult patients who underwent hip arthroscopy between January 2012 and December 2018 was extracted from the NAHR. Patients who underwent femoral sided chondral procedures were excluded. Patients who underwent osteophyte excision or a concurrent extra-articular procedure were also excluded. Cases were then classified according to the acetabular chondral procedure performed – ‘chondroplasty’, ‘microfracture’ or ‘none’ (no chondral procedure recorded). Outcomes comprised EuroQol-5 Dimensions (EQ-5D) index and the International Hip Outcome Tool 12 (iHOT-12), preoperatively and at 6 and 12 months. Results. A total of 4,978 arthroscopies were identified (chondroplasty 26.4%, microfracture 5.4%, none 68.2%). Patients who underwent microfracture were significantly older (37.9y vs 37.3y) and of higher BMI (26.4 vs 25.8 kg/m2) than patients who underwent chondroplasty. Pre-operative, 6 and 12-month follow-up were available for 77%, 42% and 38% of cases respectively. This registry study found significant clinical improvement in all groups based on arthroscopic chondral procedure following hip arthroscopy. Patients who have microfracture seem to take longer to improve and have lower peak scores as compared with chondroplasty. All groups saw significant (p <0.0001) EQ-5D and iHOT-12 gain compared to pre-operative baseline scores at 6 months which was maintained at 12 months (12 month iHOT-12 gain: chondroplasty = +27.6 (95%CI 25.0 to 30.2), microfracture = +22.6 (95%CI 17.6 to 27.6), none = +25.4 (23.8 to 27.0)). There were no statistically significant between group differences in iHOT-12 gain. Comparing <40y vs >40y age groups, all saw significant improvements in baseline scores maintained up to 12 months however patients over 40y were slower to improve, particularly in microfracture with significantly poorer iHOT-12 gain at 6 months in comparison to patients under 40y but significance was lost by 12 months. Conclusion. Whilst patient selection is crucial (and we acknowledge selection bias as a key limitation), all three chondral procedures were effective at improving patient outcomes for both younger and older patients

BACKGROUND. Some papers recently reported conflicting results on implant survivorship in all-poly tibial UKRs. Furthermore, the influence of BMI on this specific implant survivorship remains unclear, since existing reports are often based on small series of non-consecutive patients with different follow up durations, enabling to generate meaningful conclusions. PURPOSE. To determine the 10-years survival rate of an all-poly tibial UKR in a large series of consecutive patients and to investigate whether a correlation exists between a higher BMI and an increased risk of revision for any reason. METHODS. A retrospective evaluation of 273 patients at 6 to 13 years of follow-up was performed. Clinical evaluation was based on KSS and WOMAC scores. Subjective evaluation was based on a VAS for pain self-assessment. Radiographic evaluation was performed by 3 independent observers. A Kaplan-Meier survival analysis was performed assuming revision for any reason as primary endpoint. Reason of revision was determined basing on clinical and radiographic data. RESULTS. The 10-years implant survivorship was 90.8%. Twenty-five revisions (9.2%) were performed and aseptic loosening of the tibial component was the most common failure mode (11 cases, 4%). No significant correlation was identified between failure and patients'BMI. Mean post-operative results for KSS and WOMAC score were 87.0 (st.dev. 14.6) and 87.37 (st.dev. 11.48), respectively. VAS showed a significant improvement (p<0.0001) respect to pre-operative condition. CONCLUSIONS. Unlike some recent reports, this study demonstrated a satisfactory 10-years implant survivorship using an all-poly tibial UKR. A higher BMI does not reduce survival rate at 6 to 13 years of follow-up

Introduction. Revision Total Knee Arthroplasty (TKA) is becoming increasingly prevalent as the number of TKA procedures grow in a younger, higher-demand population. Factors associated with patients requiring multiple revision TKAs are not yet well understood. The purpose of this study is to investigate the epidemiology of re-revision TKA, and identify risk factors that are associated with failure of re-revision TKA. Methods. A retrospective analysis was performed on 358 patients who underwent revision TKA at a single institution between 1/2012 and 12/2013. Patients who underwent revision knee arthroplasty two or more times were included. Patients were excluded if their indication for the first revision was periprosthetic joint infection (PJI). Patient demographics, surgical indications, revision details, and available follow-up information were collected. Re-revision failure was defined as the need for any additional operative intervention. A logistic regression analysis was performed to assess for significant predictors of re-revision failure. Results. A total of 66 re-revision TKA patients were included in this study. Mean age at re-revision was 60 (±11 years). There were 48 (73%) females. Mean BMI was 31.8 (±6.9). Median ASA level was 2 (40/59; 68%). Average follow up was 2.1 (±1.0) years, with 68% (45/66) of patients having greater than 2 year follow up (Table 1). The median number of revisions was 2 (range 2–11). The most common indication for re-revision was arthrofibrosis (15; 23%), followed by PJI (14; 21%) and aseptic component loosening (13; 20%). Among re-revision patients, the most common indication of the first revision was aseptic component loosening (17; 30%), followed by arthrofibrosis (16; 28%) and instability (9; 16%) (Table 2). Among the top four indications for re-revision, both the re-revision and initial revision indication were the same. Additionally, 42% of patients possessed the same indication for re-revision as the initial revision. The proportion of patients that had a lateral release performed in either the index procedure or initial revision was higher in re-revisions performed for patellar maltracking (p=0.013). There was a significantly increased risk of re-revision failure if the patient had a higher BMI (OR=1.22; p=0.006). Re-revision survival at 30 days was 92% (60/65), at 1 year was 81% (52/64), and at 2 years 73% (33/45). The indication history of re-revision failure is shown on Table 3. Discussion. Arthrofibrosis and PJI were the most common indications for re-revision. There was an increased risk of re-revision failure in patients with a higher BMI. It was common to have a re-revision TKA for the same indication as the initial revision. A better understanding of the indications and patient factors that are associated with re-revision failures can help align surgeon and patient expectations in this challenging population. Introduction. Obesity is known to influence surgical risk in total hip replacement (THR), with increased Body Mass Index (BMI) leading to elevated risk of complications and poorer outcome scores. Using a multinational trial data of a single implant, we assess the impact of BMI and regional variations on Harris Hip scores (HHS). Method. We assessed BMI in 11 regional centres and associations with HHS at one year. Data were collected from 744 patients prospectively from 11 centres in the UK, Germany, Switzerland, Austria, New Zealand and Netherlands as part of a multicentre outcome trial. All Arthroplasties used RM Pressfit vitamys components (Mathys, Switzerland). Demographic, operative data and HHS were analysed with General Linear Model Anova, Minitab 16 (Minitab Inc, Pennsylvania). Results. 744 patients were included with mean age 70.0yrs (30.5–93.1, SD 10yrs) with 58.3% female distribution. Mean BMI was 27.7 (16.7–47.1 SD 4.7). The most frequent approach was Posterior in 48.5% cases, Anterior 18.1%, Anterolateral 16.9% and Lateral 15.7%. The study included patients from Austria 6.4%, Germany 20.2%, Netherlands 34.6%, NZ 6.9%, Switzerland 24.2% and UK 39.5%. The greatest proportion of high risk BMI >40 were performed in the UK 4.4% and NZ 5.9% with Germany and the Netherlands operating on high BMI patients <1% of the time. The UK had the highest proportion of overweight (BMI >30) patients at 39%. The greatest proportion of BMI <25 patients were found in the Netherlands, NZ and Switzerland at over one third. Higher BMI was associated with improvements in HHS (p=0.043). Age and Approach had no influence on outcome scores. Discussion. Higher BMI positively correlates with a greater improvement in Harris Hip score showing that obese patients benefit more from THR. No differences were observed with age, approach or gender. Conclusion. Although higher risk, this study shows obese patients benefit more from THR

Osteoarthritis (OA) is traditionally believed to affect the osteochondral unit by wear-and-tear from the superficial zone to the deep zone of cartilage and extended to subchondral plate. Obesity is commonly considered as a risk of OA development and hence total knee replacement (TKR), but the mechanism remains unclear. We hypothesized that obesity accelerated OA development by deteriorating tidemarks and increasing bone remodelling. 616,495 cases of TKR for OA from Australia and British joint replacement registries were collected, and data indicated that patients with higher BMI had TKR at earlier age. Specifically, patients with BMI ≤25kg/m. 2. showed 8 years younger than patients with BMI ≥40kg/m. 2. (P<0.0001) when they received TKR. We next examined tibia plateaus of 88 knee OA patients by micro-CT and histomorphometry. Linear regression showed that less cartilage degradation was associated with increased BMI in the load-bear compartment (p<0.05), while 58.3% of patients with BMI≥40kg/m. 2. demonstrated a clear anatomical separation close to tidemarks filled with fibrosis, erythrocytes and bone fragments (compared to BMI ≤25kg/m. 2. group: 7.7%, p<0.01). In subchondral bone, elevated bone formation was associated with increased BMI, as higher thickness of osteoid (p<0.01), percent osteoid volume (p<0.01), percent osteoid surface (p<0.01) were found in obese patients. However, no alteration of bone resorption and microstructural parameters was found to be associated with BMI. We suspected that the abnormal loading in knee joint due to high BMI led to the direct deterioration of binding site of osteochondral unit, which might be the mechanism of the rapid progression in obesity-related OA

The authors would like to present a retrospective study conducted on 178 patients having undergone a hip replacement. The aim of this study was to look at the immediate and short-term complications of hip replacement in relation to the body mass index (BMI). Patients ranging from age 49 to 90 were included in this study with an average age of 67.5. BMI ranged from 18 to 41. Length of stay ranged from 3 days to 76 days with an average of 11.5 days. Follow up of each patient included any wound complications, time to mobilisation and time to discharge. Operative blood loss and need to transfusion were also looked at. Blood loss was found to be from 150 mls to 2400 mls. A large number of orthopaedic surgeons use a BMI value of 35 as the upper cut off point to refuse elective surgery. Using the Null hypothesis the authors wanted to prove that a higher BMI resulted in higher complication rates. Statistical analysis of the data however did not show a significant relationship between BMI and early complications in hip replacement surgery. Parameters measured had a higher relationship to individual surgeons rather than the BMI. The authors would like to conclude that using the BMI as a predictor of a higher rate of short-term complications and refusing surgery to patients with a higher BMI is not justified

Introduction. Surgical site infection (SSI) remains a concern following total hip arthroplasty (THA). We aimed to identify risk factors for post-operative SSI in THA. Patients/Materials & Methods. All primary THAs performed in our institution during 2009–2010 were included, giving 1832 cases in 1716 patients. Cohort demographics were mean age 67.9 years (SD10.2), mean BMI 29.6 (SD5.3), 60% female and 90.2% primary indication of osteoarthritis. Post-operative SSI within one year was identified either through hospital infection control records or from Information Services Division (NHS Scotland). Demographic and peri-operative data for known or suspected risk factors for SSI were collected from clinical records. Groups were compared using independent t-tests and chi-squared tests as appropriate. Results. Within one year post-operatively, 27 patients (1.5%) developed SSI, with 23 (1.3%) being deep infections. The infected group had a higher mean BMI than the non-infected group (33.1 vs 30.0, p=0.001). 40.7% of the infected group had operative times ≥90 minutes compared to 22.7% in the non-infected group (p=0.026). Peri-operative blood transfusions were received by 25.9% of those developing infections but only 3.9% of those who did not (p<0.001). Discussion. The deep infection rate is similar to the national average and within the often quoted 2%. In keeping with other studies, these data have shown higher BMI, extended operative time and peri-operative blood transfusions to be risk factors for post-operative SSI. Obesity, along with other factors such as surgeon experience, may prolong surgical time and increase the risk of peri-operative blood transfusion, therefore leading to higher SSI rates. These results reinforce the importance of pre-operative weight loss for reducing SSIs. Conclusion. This study highlights that the rising incidence of obesity presents a serious ongoing challenge to surgeons performing THAs in terms of increasing risk of SSI. Reports of hospital infection rates should include the patient population BMI and be interpreted accordingly

Total knee replacement (TKA) is one of the most successful procedures in orthopaedic surgery. Although originally limited to more elderly and less active individuals, the inclusion criteria for TKA have changed, with ever younger, more active and heavier patients receiving TKA. This broadening of indications coincided with the widespread adoption of modular cemented and cementless TKA systems in the 1980's, and soon thereafter wear debris related osteolysis and associated prosthetic loosening became major modes of failure for TKA implants of all designs. Initially, tibial components were cemented all polyethylene monoblock constructs. Subsequent long-term follow-up studies of some of these implant designs have demonstrated excellent durability in survivorship studies out to twenty years. While aseptic loosening of these all polyethylene tibial components was a leading cause of failure in these implants, major polyethylene wear-related osteolysis around well-fixed implants was rarely (if ever) observed. Cemented metal-backed nonmodular tibial components were first introduced to allow for improved tibial load distribution and protection of the underlying (often osteoporotic) bone. Eventually, modularity between the polyethylene tibial component and the metal-backed tray was introduced in the mid-80s mainly to facilitate screw fixation for cementless implants. These designs also provided intraoperative versatility by allowing interchange of various polyethylene thicknesses, and also aided the addition of stems and wedges. Modular vs. All Polyethylene Tibial Components in Primary TKA: Kremers et al. reviewed 10,601 adult (>18 years) patients with 14,524 condylar type primary TKA procedures performed at our institution between 1/1/1988 and 12/31/2005 and examined factors effecting outcome. The mean age was 68.7 years and 55% were female. Over an average 9 years follow-up, a total of 865 revisions, including 252 tibia revisions were performed, corresponding to overall survival of 89% (Confidence intervals (CI): 88%, 90%) at 15 years. In comparison to metal modular designs, risk of tibial revision was significantly lower with all polyethylene tibias (HR 0.3, 95% CI: 0.2, 0.5). With any revision as the endpoint, there were no significant differences across the 18 designs examined. Similarly, there were no significant differences across the 18 designs when we considered revisions for aseptic loosening, wear, osteolysis. Among patient characteristics, male gender, younger age, higher BMI were all significantly associated with higher risk of revisions (p<0.008). In a more recent review from our institution of over 11, 600 primary TKA procedures, Houdek et al. again showed that all polyethylene tibial components had superior survivorship vs. metal backed designs, with a lower risk of revision for loosening, osteolysis or component fracture. Furthermore, results for all polyethylene designs were better for all BMI subgroups except for those <25 BMI where there was no difference. All polyethylene results were also better for all age groups except for those under age <55 where there again was no difference. Finally, in a recently published meta-analysis of 28 articles containing data on 95,847 primary TKA procedures, all polyethylene tibial components were associated with a lower risk of revision and adverse outcomes. The available current data support the use of all polyethylene tibial designs in TKA in all patients regardless of age and BMI. In all patients, (not just older individuals) use of an all polyethylene tibial component is an attractive and more cost effective alternative, and is associated with the better survivorship and lower risk of revision than seen with modular metal backed tibial components

Introduction. At a time when many surgeons are reluctant to perform a unilateral TKA in the obese patient, little is written on the safety and efficacy of bilateral simultaneous TKA in this same patient population. While these potential benefits are attractive to patients, surgeons may be hesitant to perform bilateral TKA due to the greater physical demand placed on the patient, and a potential increase in postoperative complication. The primary aim of this study was to analyze the impact of obesity on clinical outcomes and complication rates of patients undergoing bilateral TKA under one anesthetic. Materials and Methods. The clinical outcomes of 133 patients (266 knees) who underwent bilateral TKA between 2013 and 2016 were reviewed. The procedures were performed by three separate surgeons across three major academic institutions. ASA scores, tourniquet time, operative time, blood loss, length of stay, readmission, and postoperative complications were compared between different BMI categories of less than 30 kg/ m2, 30–34.99 kg/ m2, 35–39.99 kg/ m2and above 40 kg/ m2. Results. There were 83 females and 50 males who underwent bilateral TKA identified, with an average age of 60.17 years. The average LOS was 5 days and there was no significant impact of BMI on the length of stay or blood loss. 31 out of 133 patients experienced either a minor or major complication postoperatively (Table 1). Obese patients experienced more complications than non-obese patients. Specifically, patients identified as morbidly obese experienced a complication rate of 44.4%. This was significantly higher than the complication rate in the non-obese (less than 30 kg/ m2) patient cohort, 20.8% (p=.034). Of the 31 complications, 10 patients required a return to the operating room for a manipulation under anesthesia. There was no significant difference in the manipulation rate for the obese and non-obese patient. As BMI increased; postoperative ROM displayed a trend in the negative direction (Table 2). On average, obese patients had significantly higher ASA scores, with only 13.8% of patients with a BMI below 30 assigned an ASA score of 3 in comparison to 50% of patients with a BMI of 40 and above (p= .013) (Table 3). Higher BMI was significantly correlated with longer operative times (p=.002). Conclusion. Similar to numerous unilateral TKA studies in the obese patient, greater complication rates in the obese patient population were noted. The majority of complications that occurred within the time of this study were minor and did not affect the outcome of the procedure. Surgeons should carefully analyze the comorbidities of patients with a BMI above 40 kg/m2 such as cardiac history, diabetes mellitus, and smoking status when considering operating on morbidly obese patients and take steps to address these comorbidities and maximize the patient prior to surgery. Aims

Total hip arthroplasty (THA) is a common procedure to address pain and enhance function in hip disorders such as osteoarthritis. Despite its success, postoperative patient recovery exhibits considerable heterogeneity. This study aimed to investigate whether patients follow distinct pain trajectories following THA and identify the patient characteristics linked to suboptimal trajectories.

Methods

This retrospective cohort study analyzed THA patients at a large academic centre (NYU Langone Orthopedic Hospital, New York, USA) from January 2018 to January 2023, who completed the Patient-Reported Outcomes Measurement Information System (PROMIS) pain intensity questionnaires, collected preoperatively at one-, three-, six-, 12-, and 24-month follow-up times. Growth mixture modelling (GMM) was used to model the trajectories. Optimal model fit was determined by Bayesian information criterion (BIC), Vuong-Lo-Mendell-Rubin likelihood ratio test (VLMR-LRT), posterior probabilities, and entropy values. Association between trajectory groups and patient characteristics were measured by multinomial logistic regression using the three-step approach.

Aims

We aimed to assess the cumulative risk of total hip arthroplasty (THA) from in situ fixation for slipped capital femoral epiphysis (SCFE) after a follow-up of almost 50 years.

Methods

In this study, 138 patients with 172 affected hips treated with in situ fixation were evaluated retrospectively. A total of 97 patients (70%) were male and the mean age was 13.6 years (SD 2.1); 35 patients (25%) had a bilateral disease. The median follow-up time was 49 years (interquartile range 43 to 55). Basic demographic, stability, and surgical details were obtained from patient records. Preoperative radiographs (slip angle; SA) were measured, and data on THA was gathered from the Finnish National Arthroplasty Register.

Aims

Nonagenarians (aged 90 to 99 years) have experienced the fastest percent decile population growth in the USA recently, with a consequent increase in the prevalence of nonagenarians living with joint arthroplasties. As such, the number of revision total hip arthroplasties (THAs) and total knee arthroplasties (TKAs) in nonagenarians is expected to increase. We aimed to determine the mortality rate, implant survivorship, and complications of nonagenarians undergoing aseptic revision THAs and revision TKAs.

Methods

Our institutional total joint registry was used to identify 96 nonagenarians who underwent 97 aseptic revisions (78 hips and 19 knees) between 1997 and 2018. The most common indications were aseptic loosening and periprosthetic fracture for both revision THAs and revision TKAs. Mean age at revision was 92 years (90 to 98), mean BMI was 27 kg/m2 (16 to 47), and 67% (n = 65) were female. Mean time between primary and revision was 18 years (SD 9). Kaplan-Meier survival was used for patient mortality, and compared to age- and sex-matched control populations. Reoperation risk was assessed using cumulative incidence with death as a competing risk. Mean follow-up was five years.

Aims

The purpose of this study is to determine an individual’s age-specific prevalence of total knee arthroplasty (TKA) after cruciate ligament surgery, and to identify clinical and genetic risk factors associated with undergoing TKA.

Methods

This study was a retrospective case-control study using the UK Biobank to identify individuals reporting a history of cruciate ligament surgery. Data from verbal history and procedural codes recorded through the NHS were used to identify instances of TKA. Patient clinical and genetic data were used to identify risk factors for progression from cruciate ligament surgery to TKA. Individuals without a history of cruciate ligament reconstruction were used for comparison.

Aims

In the last decade, perioperative advancements have expanded the use of outpatient primary total knee arthroplasty (TKA). Despite this, there remains limited data on expedited discharge after revision TKA. This study compared 30-day readmissions and reoperations in patients undergoing revision TKA with a hospital stay greater or less than 24 hours. The authors hypothesized that expedited discharge in select patients would not be associated with increased 30-day readmissions and reoperations.

Methods

Aseptic revision TKAs in the National Surgical Quality Improvement Program database were reviewed from 2013 to 2020. TKAs were stratified by length of hospital stay (greater or less than 24 hours). Patient demographic details, medical comorbidities, American Society of Anesthesiologists (ASA) grade, operating time, components revised, 30-day readmissions, and reoperations were compared. Multivariate analysis evaluated predictors of discharge prior to 24 hours, 30-day readmission, and reoperation.

Aims

Histology is widely used for diagnosis of persistent infection during reimplantation in two-stage revision hip and knee arthroplasty, although data on its utility remain scarce. Therefore, this study aims to assess the predictive value of permanent sections at reimplantation in relation to reinfection risk, and to compare results of permanent and frozen sections.

Methods

We retrospectively collected data from 226 patients (90 hips, 136 knees) with periprosthetic joint infection who underwent two-stage revision between August 2011 and September 2021, with a minimum follow-up of one year. Histology was assessed via the SLIM classification. First, we analyzed whether patients with positive permanent sections at reimplantation had higher reinfection rates than patients with negative histology. Further, we compared permanent and frozen section results, and assessed the influence of anatomical regions (knee versus hip), low- versus high-grade infections, as well as first revision versus multiple prior revisions on the histological result at reimplantation. Sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), chi-squared tests, and Kaplan-Meier estimates were calculated.

Introduction. This study explores whether subjects with bicruciate retaining TKRs (BiCR) have more normal knee biomechanics during level walking and stair ascent than subjects with posterior cruciate retaining TKRs (PCR). Due to anterior cruciate ligament (ACL) preservation, we expect BiCR subjects will not show the reduced flexion and altered muscle activation patterns characteristic of persons with TKRs. Methods. Motion and electromyography (EMG) data were collected during level walking and stair climbing for 16 BiCR subjects (4/12 m/f, 65±3 years, 30.7±7.0 BMI, 8/8 R/L), 17 PCR subjects (2/15 m/f, 65±7 years, 30.4±5.1 BMI, 7/10 R/L), and 17 elderly healthy control subjects (8/9 m/f, 55±10 years, 25.8±4.0 BMI, 10/7 R/L), using the point cluster marker set. Surface EMG electrodes were placed on the vastus medialis obliquus (VMO), rectus femoris (RF), biceps femoris (BF), and semitendinosus (ST) muscles. EMG data are reported as percent relative voluntary contraction (%RVC), normalized to the average peak EMG signals during level walking. Statistical nonparametric mapping was used for waveform analysis. Results. Both TKR groups were older, and PCR subjects had higher BMI than control subjects (p≤0.020). The BiCR group walked slower and with shorter stride lengths than controls (p≤0.012). During level walking, BiCR subjects had less knee extension and posterior tibial displacement than controls (95–98%, 96–100% gait cycle, p=0.003, 0.001). PCR subjects showed higher flexion mid-stance than controls (36–44% gait cycle, p=0.001) and more external rotation (66–69% gait cycle, p=0.003). Both TKR groups had smaller extension moment peaks (PCR 5, 59–75, 96%, BiCR 61–78, 95–97% stance, p≤0.007, 0.003) than the control group. The BiCR group had smaller adduction and external rotation moment peaks (20–24%, 10–18% stance, p=0.003, 0.001) compared with controls. During stair climbing, BiCR subjects displayed more external tibial rotation (4–16% stance), more knee abduction (36–52% stance), and a lower adduction moment peak (24–34% stance) compared to healthy controls (p≤0.005). TKR subjects from both groups showed lower flexion moment peaks than controls (PCR 24–35%, BiCR 24–28% stance, p≤0.001, 0.004). For EMG, PCR subjects had more BF activity during stair ascent versus level walking than healthy subjects (56–74% stance, p≤0.001). Discussion. BiCR and PCR showed more similarities than expected. Both had altered kinematics and kinetics compared to controls, suggesting some intrinsic extensor mechanism weakness, possibly an aftereffect of osteoarthritis. The EMG results agreed accordingly, as both TKR groups showed (non-significant) decreased quadriceps activity during stair climbing. Interestingly, PCR subjects also had more BF activity during stair than healthy controls, a trend that is common for both TKR subjects and people with ACL deficiency. On the other hand, although BiCR subjects were significantly more externally rotated in early stance phase for stair climbing, their rotation patterns began to align more closely to those of the healthy control subjects at lower flexion angles where the ACL should come into play. In conclusion, ACL retention in TKRs does not correct the extensor mechanism deficits commonly found in TKR patients, although it has some effect on secondary knee kinematics and hamstring muscle activity

Aims

The risk of mechanical failure of modular revision hip stems is frequently mentioned in the literature, but little is currently known about the actual clinical failure rates of this type of prosthesis. The current retrospective long-term analysis examines the distal and modular failure patterns of the Prevision hip stem from 18 years of clinical use. A design improvement of the modular taper was introduced in 2008, and the data could also be used to compare the original and the current design of the modular connection.

Methods

We performed an analysis of the Prevision modular hip stem using the manufacturer’s vigilance database and investigated different mechanical failure patterns of the hip stem from January 2004 to December 2022.

Aims

This study evaluated the effect of treating clinician speciality on management of zone 2 fifth metatarsal fractures.

Methods

This was a retrospective cohort study of patients with acute zone 2 fifth metatarsal fractures who presented to a single large, urban, academic medical centre between December 2012 and April 2022. Zone 2 was the region of the fifth metatarsal base bordered by the fourth and fifth metatarsal articulation on the oblique radiograph. The proportion of patients allowed to bear weight as tolerated immediately after injury was compared between patients treated by orthopaedic surgeons and podiatrists. The effects of unrestricted weightbearing and foot and/or ankle immobilization on clinical healing were assessed. A total of 487 patients with zone 2 fractures were included (mean age 53.5 years (SD 16.9), mean BMI 27.2 kg/m2 (SD 6.0)) with a mean follow-up duration of 2.57 years (SD 2.64).

Aims

The Exeter V40 femoral stem is the most implanted stem in the National Joint Registry (NJR) for primary total hip arthroplasty (THA). In 2004, the 44/00/125 stem was released for use in ‘cement-in-cement’ revision cases. It has, however, been used ‘off-label’ as a primary stem when patient anatomy requires a smaller stem with a 44 mm offset. We aimed to investigate survival of this implant in comparison to others in the range when used in primary THAs recorded in the NJR.

Methods

We analyzed 328,737 primary THAs using the Exeter V40 stem, comprising 34.3% of the 958,869 from the start of the NJR to December 2018. Our exposure was the stem, and the outcome was all-cause construct revision. We stratified analyses into four groups: constructs using the 44/00/125 stem, those using the 44/0/150 stem, those including a 35.5/125 stem, and constructs using any other Exeter V40 stem.

Aims

Despite numerous studies focusing on periprosthetic joint infections (PJIs), there are no robust data on the risk factors and timing of metachronous infections. Metachronous PJIs are PJIs that can arise in the same or other artificial joints after a period of time, in patients who have previously had PJI.

Methods

Between January 2010 and December 2018, 661 patients with multiple joint prostheses in situ were treated for PJI at our institution. Of these, 73 patients (11%) developed a metachronous PJI (periprosthetic infection in patients who have previously had PJI in another joint, after a lag period) after a mean time interval of 49.5 months (SD 30.24; 7 to 82.9). To identify patient-related risk factors for a metachronous PJI, the following parameters were analyzed: sex; age; BMI; and pre-existing comorbidity. Metachronous infections were divided into three groups: Group 1, metachronous infections in ipsilateral joints; Group 2, metachronous infections of the contralateral lower limb; and Group 3, metachronous infections of the lower and upper limb.

Aims

This study compared patient-reported outcomes of three total knee arthroplasty (TKA) designs from one manufacturer: one cruciate-retaining (CR) design, and two cruciate-sacrificing designs, anterior-stabilized (AS) and posterior-stabilized (PS).

Methods

Patients scheduled for primary TKA were included in a single-centre, prospective, three-armed, blinded randomized trial (n = 216; 72 per group). After intraoperative confirmation of posterior cruciate ligament (PCL) integrity, patients were randomly allocated to receive a CR, AS, or PS design from the same TKA system. Insertion of an AS or PS design required PCL resection. The primary outcome was the mean score of all five subscales of the Knee injury and Osteoarthritis Outcome Score (KOOS) at two-year follow-up. Secondary outcomes included all KOOS subscales, Oxford Knee Score, EuroQol five-dimension health questionnaire, EuroQol visual analogue scale, range of motion (ROM), and willingness to undergo the operation again. Patient satisfaction was also assessed.

Aims

The aim of this study was to compare the early postoperative mortality and morbidity in older patients with a fracture of the femoral neck, between those who underwent total hip arthroplasty (THA) and those who underwent hemiarthroplasty.

Methods

This nationwide, retrospective cohort study used data from the Japanese Diagnosis Procedure Combination database. We included older patients (aged ≥ 60 years) who underwent THA or hemiarthroplasty after a femoral neck fracture, between July 2010 and March 2022. A total of 165,123 patients were included. The THA group was younger (mean age 72.6 (SD 8.0) vs 80.7 years (SD 8.1)) and had fewer comorbidities than the hemiarthroplasty group. Patients with dementia or malignancy were excluded because they seldom undergo THA. The primary outcome measures were mortality and complications while in hospital, and secondary outcomes were readmission and reoperation within one and two years after discharge, and the costs of hospitalization. We conducted an instrumental variable analysis (IVA) using differential distance as a variable.

Aims

The preventive effects of bisphosphonates on articular cartilage in non-arthritic joints are unclear. This study aimed to investigate the effects of oral bisphosphonates on the rate of joint space narrowing in the non-arthritic hip.

Methods

We retrospectively reviewed standing whole-leg radiographs from patients who underwent knee arthroplasties from 2012 to 2020 at our institute. Patients with previous hip surgery, Kellgren–Lawrence grade ≥ II hip osteoarthritis, hip dysplasia, or rheumatoid arthritis were excluded. The rate of hip joint space narrowing was measured in 398 patients (796 hips), and the effects of the use of bisphosphonates were examined using the multivariate regression model and the propensity score matching (1:2) model.

Aims

The purpose of this study was to determine the association between prior sleeve gastrectomy in patients undergoing primary total hip and knee arthroplasty, and 90-day complications, incidence of revision arthroplasty, and patient-reported outcome scores at final follow-up.

Methods

This is a retrospective, single-centre analysis. Patients undergoing primary hip or knee arthroplasty with a prior sleeve gastrectomy were eligible for inclusion (n = 80 patients). A morbidly obese control group was established from the same institutional registry using a 1:2 match, for cases:controls with arthroplasty based on propensity score using age, sex, pre-sleeve gastrectomy BMI, Current Procedural Terminology code to identify anatomical location, and presurgical haemoglobin A1C. Outcomes included 90-day complications, incidence of revision arthroplasty, and patient-reported outcome scores at final follow-up. Multivariable logistic regressions evaluated associations of underlying preoperative demographic and treatment characteristics with outcomes.

Aims

Periprosthetic fractures (PPFs) following hip arthroplasty are complex injuries. This study evaluates patient demographic characteristics, management, outcomes, and risk factors associated with PPF subtypes over a decade.

Methods

Using a multicentre collaborative study design, independent of registry data, we identified adults from 29 centres with PPFs around the hip between January 2010 and December 2019. Radiographs were assessed for the Unified Classification System (UCS) grade. Patient and injury characteristics, management, and outcomes were compared between UCS grades. A multinomial logistic regression was performed to estimate relative risk ratios (RRR) of variables on UCS grade.

Aims

Exosomes (exo) are involved in the progression of osteoarthritis (OA). This study aimed to investigate the function of dysfunctional chondrocyte-derived exo (DC-exo) on OA in rats and rat macrophages.

Methods

Rat-derived chondrocytes were isolated, and DCs induced with interleukin (IL)-1β were used for exo isolation. Rats with OA (n = 36) or macrophages were treated with DC-exo or phosphate-buffered saline (PBS). Macrophage polarization and autophagy, and degradation and chondrocyte activity of cartilage tissues, were examined. RNA sequencing was used to detect genes differentially expressed in DC-exo, followed by RNA pull-down and ribonucleoprotein immunoprecipitation (RIP). Long non-coding RNA osteoarthritis non-coding transcript (OANCT) and phosphoinositide-3-kinase regulatory subunit 5 (PIK3R5) were depleted in DC-exo-treated macrophages and OA rats, in order to observe macrophage polarization and cartilage degradation. The PI3K/AKT/mammalian target of rapamycin (mTOR) pathway activity in cells and tissues was measured using western blot.

Aims

The use of fluoroscopy in orthopaedic surgery creates risk of radiation exposure to surgeons. Appropriate personal protective equipment (PPE) can help mitigate this. The primary aim of this study was to assess if current radiation protection in orthopaedic trauma is safe. The secondary aims were to describe normative data of radiation exposure during common orthopaedic procedures, evaluate ways to improve any deficits in protection, and validate the use of electronic personal dosimeters (EPDs) in assessing radiation dose in orthopaedic surgery.

Methods

Radiation exposure to surgeons during common orthopaedic trauma operations was prospectively assessed using EPDs and thermoluminescent dosimeters (TLDs). Normative data for each operation type were calculated and compared to recommended guidelines.

Objectives. Electronic forms of data collection have gained interest in recent years. In orthopaedics, little is known about patient preference regarding pen-and-paper or electronic questionnaires. We aimed to determine whether patients undergoing total hip (THR) or total knee replacement (TKR) prefer pen-and-paper or electronic questionnaires and to identify variables that predict preference for electronic questionnaires. Methods. We asked patients who participated in a multi-centre cohort study investigating improvement in health-related quality of life (HRQoL) after THR and TKR using pen-and-paper questionnaires, which mode of questionnaire they preferred. Patient age, gender, highest completed level of schooling, body mass index (BMI), comorbidities, indication for joint replacement and pre-operative HRQoL were compared between the groups preferring different modes of questionnaire. We then performed logistic regression analyses to investigate which variables independently predicted preference of electronic questionnaires. Results. A total of 565 THR patients and 387 TKR patients completed the preference question. Of the THR patients, 81.8% (95% confidence interval (CI) 78.4 to 84.7) preferred pen-and-paper questionnaires to electronic questionnaires, as did 86.8% (95% CI 83.1 to 89.8) of TKR patients. Younger age, male gender, higher completed level of schooling and higher BMI independently predicted preference of electronic questionnaires in THR patients. Younger age and higher completed level of schooling independently predicted preference of electronic questionnaires in TKR patients. Conclusions. The majority of THR and TKR patients prefer pen-and-paper questionnaires. Patients who preferred electronic questionnaires differed from patients who preferred pen-and-paper questionnaires. Aims

Antibiotic prophylaxis involving timely administration of appropriately dosed antibiotic is considered effective to reduce the risk of surgical site infection (SSI) after total hip and total knee arthroplasty (THA/TKA). Cephalosporins provide effective prophylaxis, although evidence regarding the optimal timing and dosage of prophylactic antibiotics is inconclusive. The aim of this study is to examine the association between cephalosporin prophylaxis dose, timing, and duration, and the risk of SSI after THA/TKA.

Methods

A prospective multicentre cohort study was undertaken in consenting adults with osteoarthritis undergoing elective primary TKA/THA at one of 19 high-volume Australian public/private hospitals. Data were collected prior to and for one-year post surgery. Logistic regression was undertaken to explore associations between dose, timing, and duration of cephalosporin prophylaxis and SSI. Data were analyzed for 1,838 participants. There were 264 SSI comprising 63 deep SSI (defined as requiring intravenous antibiotics, readmission, or reoperation) and 161 superficial SSI (defined as requiring oral antibiotics) experienced by 249 (13.6%) participants within 365 days of surgery.

Aims

The aim of this study was to develop and internally validate a prognostic nomogram to predict the probability of gaining a functional range of motion (ROM ≥ 120°) after open arthrolysis of the elbow in patients with post-traumatic stiffness of the elbow.

Methods

We developed the Shanghai Prediction Model for Elbow Stiffness Surgical Outcome (SPESSO) based on a dataset of 551 patients who underwent open arthrolysis of the elbow in four institutions. Demographic and clinical characteristics were collected from medical records. The least absolute shrinkage and selection operator regression model was used to optimize the selection of relevant features. Multivariable logistic regression analysis was used to build the SPESSO. Its prediction performance was evaluated using the concordance index (C-index) and a calibration graph. Internal validation was conducted using bootstrapping validation.

Aims

Navigation devices are designed to improve a surgeon’s accuracy in positioning the acetabular and femoral components in total hip arthroplasty (THA). The purpose of this study was to both evaluate the accuracy of an optical computer-assisted surgery (CAS) navigation system and determine whether preoperative spinopelvic mobility (categorized as hypermobile, normal, or stiff) increased the risk of acetabular component placement error.

Methods

A total of 356 patients undergoing primary THA were prospectively enrolled from November 2016 to March 2018. Clinically relevant error using the CAS system was defined as a difference of > 5° between CAS and 3D radiological reconstruction measurements for acetabular component inclination and anteversion. Univariate and multiple logistic regression analyses were conducted to determine whether hypermobile (Δsacral slope(SS)stand-sit > 30°), or stiff (SSstand-sit < 10°) spinopelvic mobility contributed to increased error rates.

Aims

Thresholds of acceptable early migration of the components in total knee arthroplasty (TKA) have traditionally ignored the effects of patient and implant factors that may influence migration. The aim of this study was to determine which of these factors are associated with overall longitudinal migration of well-fixed tibial components following TKA.

Methods

Radiostereometric analysis (RSA) data over a two-year period were available for 419 successful primary TKAs (267 cemented and 152 uncemented in 257 female and 162 male patients). Longitudinal analysis of data using marginal models was performed to examine the associations of patient factors (age, sex, BMI, smoking status) and implant factors (cemented or uncemented, the size of the implant) with maximum total point motion (MTPM) migration. Analyses were also performed on subgroups based on sex and fixation.

Limited literature exists providing comprehensive assessment of complications following opening wedge high tibial osteotomy (OWHTO). We performed a retrospective study of local patients who underwent OWHTO for isolated medial compartment knee osteoarthritis from 1997–2013. One hundred and fifteen patients met inclusion criteria. Mean follow-up = 8.4years. Mean age = 47 (range 32–62). Mean BMI = 29.1 (range 20.3–40.2). Implants used included Tomofix (72%), Puddu plate (21%) and Orthofix (7%) (no significant differences in age/ sex/ BMI). Wedge defects were filled with autologous graft (30%), Chronos (35%) or left empty (35%). Five year survival rate (conversion to arthroplasty) = 80%. Overall complication rate = 31%. 25% of patients suffered 36 complications including minor wound infections (9.6%), major wound infections (3.5%), metalwork irritation necessitating plate removal (7%), non-union requiring revision (4.3%), vascular injury (1.7%), compartment syndrome (0.9%), and other minor complications (4%). No thromboembolic complications were observed. A higher BMI (mean 34.2) was apparent in those patients suffering complications than those not (mean 26.9). No significant differences existed in complication rates relative to implant type, type of bone graft used or patient age at surgery. Complications following OWHTO appear higher than previously reported in the literature; serious complications appear rare

Perthes’ disease is an uncommon hip disorder with limited data on the long-term outcomes in adulthood. We partnered with community-based foundations and utilized web-based survey methodology to develop the Adult Perthes Survey, which includes demographics, childhood and adult Perthes’ disease history, the University of California Los Angeles (UCLA) Activity Scale item, Short Form-36, the Hip disability and Osteoarthritis Outcome Score, and a body pain diagram. Here we investigate the following questions: 1) what is the feasibility of obtaining > 1,000 survey responses from adults who had Perthes’ disease using a web-based platform?; and 2) what are the baseline characteristics and demographic composition of our sample?

Methods

The survey link was available publicly for 15 months and advertised among support groups. Of 1,505 participants who attempted the Adult Perthes survey, 1,182 completed it with a median timeframe of 11 minutes (IQR 8.633 to 14.72). Participants who dropped out were similar to those who completed the survey on several fixed variables. Participants represented 45 countries including the USA (n = 570; 48%), UK (n = 295; 25%), Australia (n = 133; 11%), and Canada (n = 46; 4%). Of the 1,182 respondents, 58% were female and the mean age was 39 years (SD 12.6).

Aims

The aim of this study was to estimate time to arthroplasty among patients with hip and knee osteoarthritis (OA), and to identify factors at enrolment to first-line intervention that are prognostic for progression to surgery.

Methods

In this longitudinal register-based observational study, we identified 72,069 patients with hip and knee OA in the Better Management of Patients with Osteoarthritis Register (BOA), who were referred for first-line OA intervention, between May 2008 and December 2016. Patients were followed until the first primary arthroplasty surgery before 31 December 2016, stratified into a hip and a knee OA cohort. Data were analyzed with Kaplan-Meier and multivariable-adjusted Cox regression.

There is nothing going to ruin your day like a complication after shoulder arthroplasty, either hemiarthroplasty (HA) anatomical (TSA) or reverse arthroplasty (RTSA). While complications are fortunately uncommon with anatomical shoulder arthroplasty (approximately 8% but as high as 40%), the complication rate for RTSA has been reported as high as 70%. Most complications are multifactorial and cannot all be blamed on the patient. Basically you do not want to operate upon a young patient who is an insulin dependent diabetic, has an ASA of 3 or 4, who smokes, has HIV disease and has a BMI over 40. The most common predictors of failure are determined by the indication for surgery, the type of implant used and the skill of the surgeon. The major risk factors for HA are eccentric glenoids, young age and rotator cuff failure. The biggest risk factors for TSA are metal backed glenoid components, younger age of the patient, rotator cuff pathology and insufficient glenoid bone. The major risk factors for RTSA are the type of component used (Grammont type versus lateral center of rotation designs) and the indication for surgery. Infection risks include previous infected arthroplasty, previous joint infection, immunosuppression (e.g. steroid use, insulin dependent diabetes, HIV disease), ASA 3 or 4 and higher BMI (i.e. over 40). Not all of these risk factors can be prevented in patients but informed consent is critical to the patient understanding the potential outcomes of their surgery

Aims

Debridement, antibiotics, and implant retention (DAIR) is a widely accepted form of surgical treatment for patients with an early periprosthetic joint infection (PJI) after primary arthroplasty. The outcome of DAIR after revision arthroplasty, however, has not been reported. The aim of this study was to report the success rate of DAIR after revision arthroplasty with a follow-up of two years.

Methods

This retrospective study, conducted at the Sint Maartenskliniek, Nijmegen, the Netherlands, included 88 patients who underwent DAIR within 90 days of revision total hip or total knee arthroplasty between 2012 and 2019. Details of the surgical procedures and PJI were collected. Univariate analysis and a subgroup analysis of the culture-positive group were performed. Kaplan-Meier survivorship curves were constructed.

Total knee replacement (TKR) is one of the most successful procedures in orthopaedic surgery. Although originally limited to more elderly and less active individuals, the inclusion criteria for TKR have changed, with ever younger, more active and heavier patients receiving TKR. Currently, wear debris related osteolysis and associated prosthetic loosening are major modes of failure for TKR implants of all designs. Initially, tibial components were cemented all polyethylene monoblock constructs. Subsequent long-term follow-up studies of these implants have demonstrated excellent durability in survivorship studies out to twenty years. Aseptic loosening of the tibial component was one of the main causes of failure in these implants. Cemented metal-backed nonmodular tibial components were subsequently introduced to allow for improved tibial load distribution and to protect osteoporotic bone. Long-term studies have established that many one-piece nonmodular tibial components have maintained excellent durability. Eventually, modularity between the polyethylene tibial component and the metal-backed tray was introduced in the mid-80s mainly to facilitate screw fixation for cementless implants. These designs also provided intraoperative versatility by allowing interchange of various polyethylene thicknesses, and to also aid the addition of stems and wedges. Other advantages included the reduction of inventory, and the potential for isolated tibial polyethylene exchanges as a simpler revision procedure. However, several studies have documented the high failure rate of isolated polyethylene exchange procedures, probably because technical problems related to the original components are left uncorrected. Since the late 1980s, the phenomena of polyethylene wear and osteolysis have been observed much more frequently when compared with earlier eras. The reasons for this increased prevalence of synovitis, progressive osteolysis, and severe polyethylene wear remain unclear, but there is no question that it was associated with the widespread use of both cementless and cemented modular tibial designs. Mayo Data. The study population included 10,601 adult patients with 14,524 primary TKA procedures performed at our institution between 1/1/1988 and 12/31/2005. Mean age was 68.7 years and 55% were female. Overall revision rates and revisions for loosening, wear/osteolysis were compared across different designs. Over an average 9 year follow-up, a total of 865 revisions, including 252 tibia revisions were performed, corresponding to overall survival of 89% (Confidence intervals (CI): 88%, 90%) at 15 years. In comparison to metal modular designs, risk of tibial revision was significantly lower with all-poly tibias (HR 0.3, 95% CI: 0.2, 0.5). Overall, posterior cruciate-retaining designs performed better than the posterior-stabilised designs (p=0.002). With any revision as the endpoint, there were no significant differences across the 18 designs examined. Similarly, there were no significant differences across the 18 designs when we considered revisions for aseptic loosening, wear, and osteolysis. Among patient characteristics, male gender, younger age, and higher BMI were all significantly associated with higher risk of revisions (p<0.008). Summary. Available data support the use of nonmodular tibial designs in TKA in order to prevent or reduce the chance of backside wear, third body particles from resulting metallic debris and associated polyethylene induced osteolysis. In all patients, (not just older individuals) use of an all polyethylene tibial component is an attractive and more cost effective alternative, and is associated with the best survivorship and lowest risk of revision

Aims

The aim of this study was to explore the genetic correlation and causal relationship between blood plasma proteins and rheumatoid arthritis (RA).

Methods

Based on the genome-wide association studies (GWAS) summary statistics of RA from European descent and the GWAS summary datasets of 3,622 plasma proteins, we explored the relationship between RA and plasma proteins from three aspects. First, linkage disequilibrium score regression (LD score regression) was applied to detect the genetic correlation between RA and plasma proteins. Mendelian randomization (MR) analysis was then used to evaluate the causal association between RA and plasma proteins. Finally, GEO2R was used to screen the differentially expressed genes (DEGs) between patients with RA and healthy controls.

What are the data on obesity and THA risk? Which complications are elevated? If you decide on surgery, how can you minimise complications? These are timely questions because the rates of obesity are rising in the US and in many other parts of the world. Does obesity increase risk of THA complications? Answer: yes: at least for some complications. Complications which are increased: infection, wound healing, nerve injury; possibly: dislocation, periprosthetic fractures. The data are mixed on whether aseptic loosening and/or bearing surface wear problems are increased in the obese. Higher BMI may be offset by lower activity levels, particularly in a congruent joint such as the hip. Outcomes of THA in obese: Lower function scores and activity scores compared to nonobese. But good pain relief and the preoperative to postoperative change in functional scores is similar to non-obese. Is there a critical BMI threshold above which complications become unacceptable? Several studies show BMI ≥40 associated with strong risk of complications. One study from Mayo Clinic on patients with BMI ≥50 showed a 39% surgical complication rate, a 12% medical complication rate, and a high mortality rate in the several years after THA. Individualise operative decisions based on risk/benefit analysis for each patient. If you decide to operate, how can you minimise risk? Lose weight before surgery by diet: often ineffective, but worth trying. Lose weight before THA with bariatric surgery: effective in producing weight loss, but beware of the “malnourished” obese patient. In surgery: care with patient positioning, sufficient incision length, greater exposure, avoid sciatic nerve injury, fractures, care with acetabular component positioning, extra drains in subcutaneous tissue and wound compression. Engage patient in discussion of risks/benefits before surgery: shared decision making

Dislocation following total hip arthroplasty (THA) is a well-known and potentially devastating complication. Clinicians have used many strategies in attempts to prevent dislocation since the introduction of THA. While the importance of postoperative care cannot be ignored, particular emphasis has been placed on preoperative planning in the prevention of dislocation. The strategies have progressed from more traditional approaches, including modular implants, the size of the femoral head, and augmentation of the offset, to newer concepts, including patient-specific component positioning combined with computer navigation, robotics, and the use of dual-mobility implants. As clinicians continue to pursue improved outcomes and reduced complications, these concepts will lay the foundation for future innovation in THA and ultimately improved outcomes.

We undertook a retrospective cohort study to determine if body mass index (BMI) is an independent factor in determining patient outcome following total hip replacement (THR). Outcomes were determined using the Oxford Hip Score, a questionnaire designed to evaluate self-reported function and pain levels in people with hip pain. We looked at all THRs carried out on patients with a BMI of ≥30 over a 38 month period (220 operations) and compared these with 220 randomly selected THRs carried out on patients with a BMI of <30 within the same 38 month period. For each case we recorded patient BMI, P score and Oxford scores. We calculated the difference between pre-operative and one-year post-operative Oxford hip scores. We used Kendall's rank correlation to assess any potential correlation between BMI and functional change, and the Mann-Whitney U test to establish if functional change between the groups was statistically significant. Our study indicates that BMI is not an independent factor in determining relative subjective functional change following THR. Although it showed that patients with a BMI of ≥30 have worse function both before and after THR, there was no statistically significant difference between the two groups in terms of functional change. We therefore suggest that patients with a higher BMI be considered candidates for THR as they can expect the same degree of functional improvement. Given the trend for increasing numbers of obese patients in the population these findings could have a significant impact on the future of THR operations

Purpose. To determine how patient characteristics; health status and pre-operative knee function effect patient reported satisfaction and operative success following total knee replacement (TKR). We hypothesised that a number of patient factors would influence the reported levels of symptomatic improvement (success) and satisfaction and that these two outcomes would represent different aspects of the patient's perception of outcome. Design. Retrospective cohort analysis of patient reported outcome measures (PROMs) and National Joint Registry (NJR) data. Complete patient data was available for 9,874 TKRs performed for osteoarthritis between 1/8/08 and 31/12/10. The relationship of the background factors to patient perceived satisfaction and success was investigated using ordinal logistic regression and structural equation modelling (SEM). Results. Gender, pre-operative general health, depression, ASA grade and Euroqol index scores individually influenced satisfaction and/or success. When the interactions between variables were considered the size of the improvement in the Euroqol score and female gender were the most important predictors of lower levels of satisfaction/success. The effects of female gender were largely through its association with higher BMI, higher levels of depression and lower levels of general health. In total 8,286 (84%) of patients were satisfied and 8,789 (89%) rated their operation as a success. 712 (8%) of the patients who rated their operation as successful were dissatisfied. This is consistent with the premise that success relates to the patients perception of whether they have symptomatically improved whereas satisfaction relates to the extent to which they are happy with this improvement. Conclusions. Patient perceived satisfaction and success are influenced by a number of patient factors, the most important of which are the improvement in the Euroqol score and gender. This information can be used to counsel patients about the expected outcomes and guide patient selection in cases of borderline clinical need

Introduction. This project reports differences in outcome measures after total ankle arthroplasty (TAA) for patients with Osteoarthritis (OA), Rheumatoid Arthritis (RA) and Post-traumatic Osteoarthritis (PTOA). Materials and Methods. Patients who underwent TAA between March 2006 and May 2010 were included. Assessments including questionnaires (height, weight, Foot and Ankle Outcome Score, SF-36) and American Orthopaedic Foot and Ankle Score (AOFAS) were completed pre-operatively, 3, 6, and 12-months after surgery. Analyses of outcomes by diagnosis were adjusted for age, gender and BMI. Results. A total of 114 TAAs performed for OA (59), PTOA (35), and RA (20). OA patients were significantly older than PTOA (64 vs. 56, p=0.002) and more likely to be male than either RA or PTOA (78% vs. 35% and 54%, p=0.001). PTOA and OA patients had significantly higher BMI (30 and 29) than RA (25, p=0.002). There were significant improvements at 12-months for AOFAS scores for all groups 31 to 75 (OA), 29 to 77 (PTOA) and 24 to 80 (RA). No significant difference in AOFAS scores between the groups at any time. Despite no significant difference in preoperative patient-reported measures between groups, OA patients reported worse ankle-related pain and function outcomes compared to the RA and PTOA patients. This trend increases at each follow-up period and was significant at 1-year for pain (p=0.03) and function (p=0.018). PTOA patients had significantly better SF-36 scores at 1 year (p<0.05 for 5 domains). Conclusion. These early results comparing outcomes by diagnostic groups demonstrate that PTOA and RA patients have superior outcomes compared to patients with primary OA

Aims

Day-case arthroplasty is gaining popularity in Europe. We report outcomes from the first 12 months following implementation of a day-case pathway for unicompartmental knee arthroplasty (UKA) and total hip arthroplasty (THA) in an NHS hospital.

Methods

A total of 47 total hip arthroplasty (THA) and 24 unicompartmental knee arthroplasty (UKA) patients were selected for the day-case arthroplasty pathway, based on preoperative fitness and agreement to participate. Data were likewise collected for a matched control group (n = 58) who followed the standard pathway three months prior to the implementation of the day-case pathway. We report same-day discharge (SDD) success, reasons for delayed discharge, and patient-reported outcomes. Overall length of stay (LOS) for all lower limb arthroplasty was recorded to determine the wider impact of implementing a day-case pathway.

Aims

Wear of the polyethylene (PE) tibial insert of total knee arthroplasty (TKA) increases the risk of revision surgery with a significant cost burden on the healthcare system. This study quantifies wear performance of tibial inserts in a large and diverse series of retrieved TKAs to evaluate the effect of factors related to the patient, knee design, and bearing material on tibial insert wear performance.

Methods

An institutional review board-approved retrieval archive was surveyed for modular PE tibial inserts over a range of in vivo duration (mean 58 months (0 to 290)). Five knee designs, totalling 1,585 devices, were studied. Insert wear was estimated from measured thickness change using a previously published method. Linear regression statistical analyses were used to test association of 12 patient and implant design variables with calculated wear rate.

There is mixed evidence in the literature regarding increasing age, ASA and BMI as risk factors for surgical site infection in orthopaedic surgery. To investigate the matter further, we examined 1055 wounds in 1008 patients in the Department of Trauma and Orthopaedic Surgery at University College London Hospital between 2000 and 2006. All patients with a minimum two-night stay were included. Data was collected by four designated research nurses. The age, height, weight and ASA status of each patient was recorded. All wounds were classified using ASEPSIS. This is a quantitative wound scoring method which is a summation of scores calculated from visual wound characteristics and the clinical consequences of infection. Our results showed a strong linear association between age and ASEPSIS scores. Single variable regression analysis showed a t value of 3.32 and p value of 0.001. A similar linear association was seen between ASA grading and ASEPSIS scores. Single variable regression analysis showed a t value of 2.75 and p value of 0.006. The association between BMI and ASEPSIS scores was markedly different from that seen with age and ASA. The graph was U-shaped with patients with a BMI of 25-30 having the lowest average ASEPSIS scores. Patients with a lower and a higher BMI had higher average ASEPSIS scores. Single variable regression analysis was not significant since the relationship between BMI and ASEPSIS scores is not linear. In conclusion, there are clearly defined patient groups who are at increased risk of developing a surgical site infection: older patients, patients with a higher ASA, and patients with both a low and high BMI. These patients should be targeted to reduce overall infection rates. This can be achieved by ensuring adequate antibiotic prophylaxis, having a low threshold to treat suspected infection and arranging regular follow-up

Background. Cauda equina syndrome (CES) is a physical process. There is compression of the cauda equina resulting in arrest of the electrochemical signal from the central nervous system. Previous studies have demonstrated that anthropomorphic features influence nerve conduction properties. Aim. We therefore sought to if there was an association between biophysical parameters and CES. Setting and design. Single centre retrospective comparative study. Method and materials. We analysed consecutive patients who had elective lumbar discectomy. Demographic data-including age, gender, height, weight and BMI were recorded. Identical information was collected in consecutive patients who underwent emergency lumbar discectomy for MRI-proven CES. Results. There were 40 patients who underwent emergency surgery for CES. There were 22 women and 18 men with a mean age of 38.6 years. 92 patients underwent elective lumbar discectomy. There were 45 men and 47 women with a mean age of 44 years. Patient undergoing emergency discectomy for CES were significantly heavier (p=0.001) and had a significantly higher BMI (p< 0.0005) compared to the elective surgery cohort. The mean difference in weight and BMI were 11.2 kg (95% confidence interval: 3.8–18.7) and 4.6kg/m2 (95% confidence interval: 2.4–6.9) respectively. The CES-group was also slightly younger (mean difference 5.4 years 95% CI: 1.7–9.8 p=0.01). There was no statistically significant difference between the heights of the two groups or the gender ratio. Multivariate binary logistic regression showed increasing weight to be associated with the increasing odds of CES (P< 0.0005). In contrast increasing height was correlated with a reducing likelihood of CES (P< 0.01). Increasing BMI was associated with increased odds of CES (p< 0.0005). Conclusion. This is the first study to relate anthropometric features to CES. Our study observed that increasing BMI is linked with the increased odds of CES syndrome as was increasing weight and decreasing height

Aims

Single-stage revision total knee arthroplasty (rTKA) is gaining popularity in treating chronic periprosthetic joint infections (PJIs). We have introduced this approach to our clinical practice and sought to evaluate rates of reinfection and re-revision, along with predictors of failure of both single- and two-stage rTKA for chronic PJI.

Methods

A retrospective comparative cohort study of all rTKAs for chronic PJI between 1 April 2003 and 31 December 2018 was undertaken using prospective databases. Patients with acute infections were excluded; rTKAs were classified as single-stage, stage 1, or stage 2 of two-stage revision. The primary outcome measure was failure to eradicate or recurrent infection. Variables evaluated for failure by regression analysis included age, BMI, American Society of Anesthesiologists grade, infecting organisms, and the presence of a sinus. Patient survivorship was also compared between the groups.

One-stage revision hip arthroplasty for periprosthetic joint infection (PJI) has several advantages; however, resection of the proximal femur might be necessary to achieve higher success rates. We investigated the risk factors for resection and re-revisions, and assessed complications and subsequent re-revisions.

Methods

In this single-centre, case-control study, 57 patients who underwent one-stage revision arthroplasty for PJI of the hip and required resection of the proximal femur between 2009 and 2018 were identified. The control group consisted of 57 patients undergoing one-stage revision without bony resection. Logistic regression analysis was performed to identify any correlation with resection and the risk factors for re-revisions. Rates of all-causes re-revision, reinfection, and instability were compared between groups.

Unicompartmental knee replacement (UKR) is an established treatment for single compartment end-stage arthrosis with good recorded survivorship. UKRs are often implanted into more active younger patients, but patient selection remains controversial. A recent study, led by the Royal College of Surgeons Clinical Effectiveness Unit, demonstrated that prosthesis revision rates decrease strongly with age (Van Der Meulen et al 2008). It has therefore been suggested that UKR should only be considered in elderly patients. This contrasts our observed experience of early revision cases leading us to compare these patients with a control group. Between September 2002 and 2008, 812 Oxford Mobile Bearing Medial UKRs were implanted. We compared all patients who underwent UKR revision to Total Knee Replacement (TKR) against a control group of 50 consecutive UKR patients. 20 implants have required revision to TKR in 19 patients since 2002. Median age at index surgery was 68 (range 48-81), median BMI was 31 (range 25-41.5), 17 patients were female (85%), and median implant survival was 25 months (range 6-57). Control group median age at index surgery was 66 (range 46-81), median BMI was 30 (range 22-51), and 27 patients were female (54%). Median Oxford Knee Score recorded in September 2009 was 36 (range 14-54) for revision patients and 21 (range 14-39) for the control group (p=0.021). Our UKR patients with early failure requiring revision are far more likely to be female (p=0.015), as well as older and with a higher BMI than the control group. We feel this is a subset of patients at high risk of failure, despite meeting all criteria for UKR. The underlying causes are likely to be multifactorial, but a key factor may be that this group has varus tricompartment osteoarthritis rather than classical anteromedial osteoarthritis. Our data counters recent advice based on National Joint Registry data

Total knee replacement is one of the most successful procedures in orthopaedic surgery. Although originally limited to more elderly and less active individuals, the inclusion criteria have changed, with ever younger, more active and heavier patients receiving TKA. Currently, wear debris related osteolysis and associated prosthetic loosening are major modes of failure for TKA implants of all designs. Initially, tibial components were cemented all polyethylene monoblock constructs. Subsequent long-term follow-up studies of these implants have demonstrated excellent durability in survivorship studies out to twenty years. Aseptic loosening of the tibial component was one of the main causes of failure in these implants. Polyethylene wear with osteolysis around well fixed implants was rarely (if ever) observed. Cemented metal-backed nonmodular tibial components were subsequently introduced to allow for improved tibial load distribution and to protect osteoporotic bone. Long-term studies have established that many one-piece nonmodular tibial components have maintained excellent durability. Eventually, modularity between the polyethylene tibial component and the metal-backed tray was introduced in the mid-80s mainly to facilitate screw fixation for cementless implants. These designs also provided intraoperative versatility by allowing interchange of various polyethylene thicknesses, and also aided the addition of stems and wedges. Since the late 1980's, the phenomena of polyethylene wear and osteolysis have been observed much more frequently when compared with earlier eras. The reasons for this increased prevalence of synovitis, progressive osteolysis, and severe polyethylene wear remain unclear, but there is no question that it was associated with the widespread use of both cementless and cemented modular tibial designs. Mayo Data: Modular versus All Polyethylene Tibial Components in Primary TKA. The study population included 10,601 adult (>18 years) patients with 14,524 primary TKA procedures performed at our institution between 1/1/1988 and 12/31/2005. Mean age was 68.7 years and 55% were female. Overall revision rates and revisions for loosening, wear/osteolysis were compared across different designs using Cox proportional hazards regression models adjusting for age, sex, calendar year and body mass index (BMI). Over an average 9 years follow-up, a total of 865 revisions, including 252 tibia revisions were performed, corresponding to overall survival of 89% (Confidence intervals (CI): 88%, 90%) at 15 years. In comparison to metal modular designs, risk of tibial revision was significantly lower with all-poly tibias (HR 0.3, 95% CI: 0.2, 0.5). Overall, posterior cruciate-retaining (CR) designs performed better than the posterior-stabilised (PS) designs (p=0.002). With any revision as the endpoint, there were no significant differences across the 18 designs examined. Similarly, there were no significant differences across the 18 designs when we considered revisions for aseptic loosening, wear, osteolysis. Among patient characteristics, male gender, younger age, higher BMI were all significantly associated with higher risk of revisions (p<0.008). Summary: Available data support the use of nonmodular tibial designs in TKA in order to prevent or reduce the chance of backside wear, third body particles from resulting metallic debris and associated polyethylene induced osteolysis. In all patients, (not just older individuals) use of an all polyethylene tibial component is an attractive and more cost effective alternative, and is associated with the best survivorship and lowest risk of revision

It has been proposed that higher knee adduction moments and associated malalignment in subjects with severe medial knee joint osteoarthritis (OA) is due to anatomical deformities as a result of OA [1, 2]. The emergence of patient-matched implants should allow for correction of any existing malalignment. Currently the plans for such surgeries are often based on three dimensional supine computed tomography (CT) scans or magnetic resonance imaging (MRI), which may not be representative of malalignment during functional loading. We investigated differences in frontal plane alignment in control subjects and subjects with severe knee joint OA who had undergone both supine imaging and gait analysis. Fifteen subjects with severe knee OA, affecting either the medial or lateral compartment, and 18 control subjects were selected from a database established as part of a larger study. All subjects had undergone gait analysis using the Vicon motion capture system. OA subjects had undergone routine CT scans and were scheduled for knee joint replacement surgery. Control subjects had no known musculoskeletal conditions and had undergone MRI imaging of hip, knee and ankle joints. Frontal plane knee joint angles were measured from supine imaging (supine) and from motion capture during standing (static) and during gait at the first peak ground reaction force (gait). OA subjects had a significantly higher BMI (p < 0.01) and different gender composition (13 males and 2 females vs 4 males and 5 females; p = 0.03) compared with controls. Multiple linear regression analysis indicated no significant confounding effect of these differences on frontal plane angles measured in supine, static or gait conditions. For both OA and healthy subjects, frontal plane knee angles were significantly higher during gait compared with supine (p = 0.03 and 0.02, respectively). There were also significant differences in knee alignment between OA and healthy subjects for supine and static (p < 0.05) but not for gait, although this was approaching significance (p = 0.052). Overall there seemed to be higher variation in alignment in the OA subjects (Fig. 1). The significantly higher frontal plane knee joint angles measured in both control and OA subjects during gait compared with supine imaging indicate that functional alignment should be taken into consideration when planning patient-specific surgeries. Higher variation in OA patients may be due to alterations in gait patterns due to pain or degree of wear in their osteoarthritic joints, and requires further investigation. In addition, methodological considerations should be taken when comparing alignment from measurements taken with imaging and motion capture to avoid systematic errors in the data. In conclusion, we believe that both supine and loadbearing imaging are insufficient to gain a full representation of functional alignment, and analysis of functional alignment should be routinely performed for optimal surgical planning

Background:. The number of young patients undergoing total knee arthroplasty is rapidly increasing. Long-term follow-up of modern type implants is needed to provide a benchmark of implant longevity for these patients. Methods:. Between January 1995 and October 1997, 245 consecutive total knee arthroplasties were performed in 217 patients by a single surgeon. In 156 knees, the Genesis I implant was used, and in 89 knees the Genesis II implant was used. Mean age at surgery was 69.3 years for the Genesis I cohort and 66 years for the Genesis II (p = 0.016). At 15 to 17 years, cumulative survivorship was calculated using Kaplan-Meier statistics whilst outcomes were rated with the ‘Knee society score’ and with the ‘Knee Injury and Osteoarthritis Outcome Score’. Radiological assessment included coronal alignment measured on full leg standing X-rays, and analysis of radiolucent lines and polyethylene thickness on AP, Lateral and Axial X-rays, positioned under fluoroscopic control. Results:. At 15 to 17 years, 79 patients had died (56 with Genesis I) and 27 were lost to follow-up. Eleven TKA had been revised including 10 Genesis I for infection (1), recurrent dislocation (1), loosening (1), patellar maltracking (2) or polyethylene (PE) wear (5) and 1 Genesis II for PE wear. 139 knees in 129 patients were in situ. Overall, the cumulative survivorship with endpoint revision for any reason was 92.4% at 15.7 years (95%CI: 91.0–93.8). In the Genesis I cohort, cumulative survivorship was 90.1% at 15.1 years (95%CI: 88.0–92.2). In the Genesis II cohort, cumulative survivorship was 98.1% at 14.9 years (95%CI: 97.9–98.3). The difference in survivorship was not statistically significant (log rank 3.136; p = 0.077), but the odds ratio for failure was 7.6 for the Genesis I compared to Genesis II. Logistic regression further identified gamma-air sterilization of PE as a significant predictor of failure, with an odds ratio of 13.4 compared to ETO sterilization. There was no difference in survivorship between patients younger or older than 55 years at surgery or between genders. Patients who underwent a revision had a significantly higher BMI (p = 0.027). There was no statistically significant difference in clinical outcome, radiographic radiolucencies or PE thickness changes between the surviving Genesis I and II cohorts. Conclusions:. At 15 years, the overall 92.4% survivorship of the Genesis I TKA is good with excellent 98.1% survivorship for the Genesis II with the adapted femoral trochlear design. The results confirm the risk of PE wear when gamma-air sterilisation is used. Excellent longer-term outcome is obtained with a contemporary TKA implant using non cross-linked polyethylene

An analysis on prospective data collected on our hip database was carried out on all patients undergoing primary cemented unilateral total hip arthroplasty in the last 5 years comparing the short term outcomes between diabetics and non-diabetics. There were 1220 non-diabetics and 77 diabetics identified from the database with at least 3 years follow up. (average 3.6 years). We found no significant difference with respect to age, sex and diagnosis between the 2 groups. Diabetics did have a significantly higher BMI (30.2 versus 27.7, p< 0.001) and higher incidence of coronary artery disease (31% versus 15%, p< 0.001) We found no increase in the rate of deep periprosthetic infection, superficial wound complications, dislocation, blood loss and DVT between diabetics and non-diabetics even after adjusting for potential confounders of age, sex, diagnosis, BMI and the presence of coronary artery disease. The only factor that was found to be significantly different between the 2 groups was length of stay (10.73 versus 9.56 days, p< 0.05). Further analysis of the diabetic group only showed no difference with regard the same outcomes between insulin-dependent, diet-controlled and diet and oral hypoglycaemic-controlled patient subsets. From this study we conclude that at the time of taking informed consent from diabetic patients undergoing total hip arthroplasty the only potential difference from non-diabetics is that the length of stay may be longer
