Aims. This paper aims to review the evidence for patient-related factors associated with less favourable outcomes following hip arthroscopy. Methods. Literature reporting on preoperative patient-related risk factors and outcomes following hip arthroscopy were systematically identified from a computer-assisted literature search of Pubmed (Medline), Embase, and Cochrane Library using Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA) guidelines and a scoping review. Results. Assessment of these texts yielded 101 final articles involving 90,315 hips for qualitative analysis. The most frequently reported risk factor related to a less favourable outcome after hip arthroscopy was older age and preoperative osteoarthritis of the hip. This was followed by female sex and patients who have low preoperative clinical scores, severe hip dysplasia, altered hip morphology (excess acetabular retroversion or excess femoral anteversion or retroversion), or a large cam deformity. Patients receiving workers’ compensation or with rheumatoid arthritis were also more likely to have a less favourable outcome after hip arthroscopy. There is evidence that obesity, smoking, drinking alcohol, and a history of mental illness may be associated with marginally less favourable outcomes after hip arthroscopy. Athletes (except for ice hockey players) enjoy a more rapid recovery after hip arthroscopy than non-athletes. Finally, patients who have a favourable response to local anaesthetic are more likely to have a favourable outcome after hip arthroscopy. Conclusion. Certain patient-related risk factors are associated with less favourable outcomes following hip arthroscopy. Understanding these risk factors will allow the appropriate surgical indications for hip arthroscopy to be further refined and help patients to comprehend their individual risk profile. Objective. Hip arthroscopy in the setting of hip dysplasia is controversial in the orthopaedic community, as the outcome literature has been variable and inconclusive. We hypothesise that outcomes of hip arthroscopy may be diminished in the setting of hip dysplasia, but outcomes may be acceptable in milder or borderline cases of hip dysplasia. Methods. A systematic search was performed in duplicate for studies investigating the outcome of hip arthroscopy in the setting of hip dysplasia up to July 2015. Study parameters including sample size, definition of dysplasia, outcomes measures, and re-operation rates were obtained. Furthermore, the levels of evidence of studies were collected and quality assessment was performed. Results. The systematic review identified 18 studies investigating hip arthroscopy in the setting of hip dysplasia, with 889 included patients. Criteria used by the studies to diagnose hip dysplasia and borderline hip dysplasia included centre edge angle in 72% of studies but the range of angles were quite variable. Although 89% of studies reported improved post-operative outcome scores in the setting of hip dysplasia, revision rates were considerable (14.1%), with 9.6% requiring conversion to total hip arthroplasty. Conclusion. The available orthopaedic literature suggests that although improved outcomes are seen in hip arthroscopy in the setting of hip dysplasia, there is a high rate of re-operation and conversion to total hip arthroplasty. Aims. Periacetabular osteotomy (PAO) is the preferred treatment for symptomatic acetabular dysplasia in adolescents and young adults. There remains a lack of consensus regarding whether intra-articular procedures such as labral repair or improvement of femoral offset should be performed at the time of PAO or addressed subsequent to PAO if symptoms warrant. The purpose was to determine the rate of subsequent hip arthroscopy (HA) in a contemporary cohort of patients, who underwent PAO in isolation without any intra-articular procedures. Methods. From June 2012 to March 2022, 349 rectus-sparing PAOs were performed and followed for a minimum of one year (mean 6.2 years (1 to 11)). The mean age was 24 years (14 to 46) and 88.8% were female (n = 310). Patients were evaluated at final follow-up for patient-reported outcome measures (PROMs). Clinical records were reviewed for complications or subsequent surgery. Radiographs were reviewed for the following acetabular parameters: lateral centre-edge angle, anterior centre-edge angle, acetabular index, and the alpha-angle (AA). Patients were cross-referenced from the two largest hospital systems in our area to determine if subsequent HA was performed. Descriptive statistics were used to analyze risk factors for HA. Results. A total of 16 hips (15 patients; 4.6%) underwent subsequent HA with labral repair and femoral osteochondroplasty, the most common interventions. For those with a minimum of two years of follow-up, 5.3% (n = 14) underwent subsequent HA. No hips underwent total hip arthroplasty and one revision PAO was performed. Overall, 17 hips (4.9%) experienced a complication and 99 (26.9%) underwent hardware removal. All PROMs improved significantly postoperatively. Radiologically, 80% of hips (n = 279) reached the goal for acetabular correction (77% for acetbular index and 93% for LCEA), with no significant differences between those who underwent subsequent HA and those who did not. Conclusion. Rectus-sparing PAO is associated with a low rate of subsequent HA for intra-articular pathology at a mean of 6.2 years’ follow-up (1 to 11). Acetabular correction alone may be sufficient as the primary intervention for the majority of patients with symptomatic acetabular dysplasia. Aims. Hip arthroscopy has gained prominence as a primary surgical intervention for symptomatic femoroacetabular impingement (FAI). This study aimed to identify radiological features, and their combinations, that predict the outcome of hip arthroscopy for FAI. Methods. A prognostic cross-sectional cohort study was conducted involving patients from a single centre who underwent hip arthroscopy between January 2013 and April 2021. Radiological metrics measured on conventional radiographs and magnetic resonance arthrography were systematically assessed. The study analyzed the relationship between these metrics and complication rates, revision rates, and patient-reported outcomes. Results. Out of 810 identified hip arthroscopies, 359 hips were included in the study. Radiological risk factors associated with unsatisfactory outcomes after cam resection included a dysplastic posterior wall, Tönnis grade 2 or higher, and over-correction of the α angle. The presence of acetabular retroversion and dysplasia were also significant predictors for worse surgical outcomes. Notably, over-correction of both cam and pincer deformities resulted in poorer outcomes than under-correction. Conclusion. We recommend caution in performing hip arthroscopy in patients who have three positive acetabular retroversion signs. Acetabular dysplasia with a lateral centre-edge angle of less than 20° should not be treated with isolated hip arthroscopy. Acetabular rim-trimming should be avoided in patients with borderline dysplasia, and care should be taken to avoid over-correction of a cam deformity and/or pincer deformity. Arthroscopic hip procedures have increased dramatically over the last decade as equipment and techniques have improved. Patients who require hip arthroscopy for femoroacetabular impingement on occasion require surgery on the contralateral hip. Previous studies have found that younger age of presentation and lower Charlson comorbidity index have higher risk for requiring surgery on the contralateral hip but have not found correlation to anatomic variables. The purpose of this study is to evaluate the factors that predispose a patient to requiring subsequent hip arthroscopy on the contralateral hip. This is an IRB-approved, single surgeon retrospective cohort study from an academic, tertiary referral centre. A chart review was conducted on 310 primary hip arthroscopy procedures from 2009-2020. We identified 62 cases that went on to have a hip arthroscopy on the contralateral side. The bilateral hip arthroscopy cohort was compared to unilateral cohort for sex, age, BMI, pre-op alpha angle and centre edge angle measured on AP pelvis XRay, femoral torsion, traction time, skin to skin time, Tonnis grade, intra-op labral or chondral defect. A p-value <0.05 was deemed significant. Of the 62 patients that required contralateral hip arthroscopy, the average age was 32.7 compared with 37.8 in the unilateral cohort (p = 0.01) and BMI was lower in the bilateral cohort (26.2) compared to the unilateral cohort (27.6) (p=0.04). The average alpha angle was 76.3. 0. in the bilateral compared to 66. 0. in the unilateral cohort (p = 0.01). Skin to skin time was longer in cases in which a contralateral surgery was performed (106.3 mins vs 86.4 mins) (p=0.01). Interestingly, 50 male patients required contralateral hip arthroscopy compared to 12 female patients (p=0.01). No other variables were statistically significant. In conclusion, this study does re-enforce existing literature by stating that younger patients are more likely to require contralateral hip arthroscopy. This may be due to the fact that these patients require increased range of motion from the hip joint to perform activities such as sports where as older patients may not need the same amount of range of motion to perform their activities. Significantly higher alpha angles were noted in patients requiring contralateral hip arthroscopy, which has not been shown in previous literature. This helps to explain that larger CAM deformities will likely require contralateral hip arthroscopy because these patients likely impinge more during simple activities of daily living. Contralateral hip arthroscopy is also more common in male patients who typically have a larger CAM deformity. In summary, this study will help to risk stratify patients who will likely require contralateral hip arthroscopy and should be a discussion point during pre-operative counseling. That offering early subsequent or simultaneous hip arthroscopy in young male patients with large CAMs should be offered when symptoms are mild

There has been a marked increase in the number of hip arthroscopies performed over the past 16 years, primarily in the management of femoroacetabular impingement (FAI). Insights into the pathoanatomy of FAI, and high-level evidence supporting the clinical effectiveness of arthroscopy in the management of FAI, have fuelled this trend. Arthroscopic management of labral tears with repair may have superior results compared with debridement, and there is now emerging evidence to support reconstructive options where repair is not possible. In situations where an interportal capsulotomy is performed to facilitate access, data now support closure of the capsule in selective cases where there is an increased risk of postoperative instability. Preoperative planning is an integral component of bony corrective surgery in FAI, and this has evolved to include computer-planned resection. However, the benefit of this remains controversial. Hip instability is now widely accepted, and diagnostic criteria and treatment are becoming increasingly refined. Instability can also be present with FAI or develop as a result of FAI treatment. In this annotation, we outline major current controversies relating to decision-making in hip arthroscopy for FAI. Cite this article: Bone Joint J 2022;104-B(5):532–540

This study aimed to analyse the incidence, the operative findings and outcomes of hip arthroscopy after periacetabular osteotomy (PAO). We conducted a retrospective study of prospective collected data to report the operative findings and outcomes of hip arthroscopy for recurrent pain following PAO for acetabular dysplasia. Demographic data, radiographic and arthroscopic findings were analysed. Pre- and post-operative patient reported outcome measures (PROMs) were collected. Of 184 PAO patients, 15 hips in 15 patients (8.2%) underwent post-PAO hip arthroscopy with 2 males and 13 females at a mean time of 3.9 ±2.9(0.3–10) years. Hip arthroscopy findings included labral tears (15 hips, 100%), chondral damage (11 hips, 73%) with one Beck 1, two Beck 2, four Beck 4, and four Beck 5 damage. There were 8 (53%) isolated labral debridement, 7 (47%) labral repair, and 2 (13%) adhesiolysis. A femoral osteochondroplasty was performed in 4 (27%) hips. Four hips (27%) were finally converted to total hip replacement at a mean delay of 2.3 ±1.4 (0.7–3.2) years post hip arthroscopy and these 4 patients were significantly older than those who were not (p=0.02). There were no significant differences in PROMs pre and post hip scope. 8.2% of patients undergoing a PAO required a hip scope for persistent hip pain. Common post-PAO hip arthroscopy findings included labral tears, chondral changes and femoroacetabular impingement. 27% of patients finally underwent conversion to total hip replacement at a mean follow-up of 4.5 years post hip scope

As the field of hip arthroscopy continues to develop, functional measures and testing become increasingly important in patient selection, managing patient expectations prior to surgery, and physical readiness for return to athletic participation. The Hip Sport Test (HST) was developed to assess strength, coordination, agility, and range of motion prior to and following hip arthroscopy as a functional assessment. However, the relationship between HST and hip strength, range of motion, and hip-specific patient reported outcome (PRO) measures have not been investigated. The purpose of this study was to evaluate the correlation between the HST scores and measurements of hip strength and range of motion prior to undergoing hip arthroscopy. Between September 2009 and January 2017, patients aged 18-40 who underwent primary hip arthroscopy for the treatment of femoroacetabular impingement with available pre-operative HST, dynamometry, range of motion, and functional scores (mHHS, WOMAC, HOS-SSS) were identified. Patients were excluded if they were 40 years old, had a Tegner activity score < 7, or did not have HST and dynamometry evaluations within one week of each other. Muscle strength scores were compared between affected and unaffected side to establish a percent difference with a positive score indicating a weaker affected limb and a negative score indicating a stronger affected limb. Correlations were made between HST and strength testing, range of motion, and PROs. A total of 350 patients met inclusion criteria. The average age was 26.9 ± 6.5 years, with 34% females and 36% professional athletes. Total and component HST scores were significantly associated with measure of strength most strongly for flexion (rs = −0.20, p < 0 .001), extension (rs = −0.24, p<.001) and external rotation (rs = −0.20, p < 0 .001). Lateral and diagonal agility, components of HST, were also significantly associated with muscle strength imbalances between internal rotation versus external rotation (rs = −0.18, p=0.01) and flexion versus extension (rs = 0.12, p=0.03). In terms of range of motion, a significant correlation was detected between HST and internal rotation (rs = −0.19, p < 0 .001). Both the total and component HST scores were positively correlated with pre-operative mHHS, WOMAC, and HOS-SSS (p<.001 for all rs). The Hip Sport Test correlates with strength, range of motion, and PROs in the preoperative setting of hip arthroscopy. This test alone and in combination with other diagnostic examinations can provide valuable information about initial hip function and patient prognosis

Introduction. With resumption of non-urgent surgery in May 2020, standard anesthesia for hip arthroscopy switched from general with endotracheal intubation (GA) to spinal (SA) in response to COVID-19 implications; reducing potential aerosolized exposure for patient and staff and reducing consumption of personal protective equipment (PPE). There are no studies that compare the attributes for these two anesthesia methods for hip arthroscopy; and thus, this was viewed as an opportunity to perform a comparative observational study on SA to a recent matched group of GA. Methods. Beginning in May 2020, SA became the standard for hip arthroscopy. GA was used if the patient refused SA or had a history of previous lumbar spinal surgery, or body mass index (BMI) greater than 35. SA patients were carefully matched for age, gender and procedure to a recent previous GA population and compared for recovery room (RR) length of stay, entry and discharge visual analog scores (VAS), morphine mg equivalent (MME) usage, and untoward events. Additionally, SA and GA cases performed since May 2020 were compared for the length of time from entry to the operating room (OR) until the surgeon was able to perform an examination under anesthesia (EUA). Results. Statistical analysis determined that these groups are too small (46 in each group) to establish significant differences, but the authors felt that an opportunity to explore this, based on a recent change out of necessity (COVID-!9), was worth presenting as a novel study to compare two accepted methods of anesthesia for hip arthroscopy. SA patients required fewer regional blocks (7 vs 1) and needed less narcotics (99 vs 153). As a potential advantage of SA, continued investigation to see if this reaches statistical significance is meaningful. SA patients did spend more time in the PACU (136 vs 133); and had more problems with urinary retention, requiring catheterization (5 vs 0); but most of these occurred early in the experience and was corrected by having the patient void immediately prior to transfer to the OR and avoiding anticholinergic medications. SA seemed to add only slightly to the length of time until the surgeon could perform an EUA and begin positioning for the procedure (9 vs 8). Conclusion. Hip arthroscopy can be effectively performed with either GA or SA. Of particular interest with further studies will be whether choice of anesthesia affects early postoperative rehabilitation

This study aims to describe trends in self-reported reported sexual function before and after HA. Methods. Adult (≥18 years) patients who underwent HA between 1/01/2012 and 31/10/2020 were extracted from the UK Non-Arthroplasty Hip Registry. International Hip Outcome Tool 12 (iHOT-12) questionnaires were collected pre-operatively and at 6 and 12 months. The iHOT-12 then asks asking patients to quantify ‘how much trouble they experience with sexual activity because of their hip?’ with responses converted to a continuous scale (0–100) to measure function. Results. Of 7639 procedures (59% female, mean age 36.5 years (SD 11)), 91% (5616 of 6151 respondents) indicated pre-operatively that questions about sexual activity were relevant to them (male 93%, female 90%, p < 0.001). Overall, mean pre-operative sexual function increased from 42.0 (95%CI 41.2 to 42.8, n=5267) to 61.8 (60.6 to 63.1, n=2393) at 6 months, and 62.1 (60.8 to 63.5, n=2246) at 12 months post-operatively. At 12 months, both sexes saw significant improvement in their pre-operative sexual function scores (p<0.0001). Males started from significantly higher baseline sexual function (53.3 vs 34.2) and achieved higher scores by 12 months (68.8 vs 58.0) compared to female patients. There was no significant difference in pre- or post-operative scores when comparing younger (<40 years) and older (>40 years). Conclusions. Most patients can expect to experience improvement in their sexual function following hip arthroscopy, regardless of sex or age group

In the young and highly active population of military patients, femoroacetabular impingement can be a source of serious disability as well as a threat to their career. This morbidity can be treated with hip arthroscopy with debridement of cam lesion, and excision or repair of a corresponding labral tear. We report on the long term outcomes (>1 year) of 26 military cases who underwent hip arthroscopy for femoroacetabular impingement, in a single surgeon's series. Twenty two patients (four bilateral cases) underwent hip arthroscopy as a day case procedure during the period February 2013 to October 2014. Non-Arthritic Hip Scores (NAHS) were obtained from patients pre-operatively, at two months, four months and at least one year post-operatively. There was a significant improvement in NAHS at two months (75.6)(p<0.05), four months (85.1)(p<0.001) and one year (84.8)(p<0.001), compared to pre-operative NAHS (65.8). There was no significant change in NAHS beyond four months. Only three out of the 22 patients were medically discharged secondary to persistent hip symptoms. Hip arthroscopy for femoroacetabular impingement is an effective, viable procedure for military patients. Mobility, pain and function is significantly improved after surgery. This improvement is sustained in the long term up to and beyond one year

Aims. Responsiveness and ceiling effects are key properties of an outcome score. No such data have been reported for the original English version of the International Hip Outcome Tool 12 (iHOT-12) at a follow-up of more than four months. The aim of this study was to identify the responsiveness and ceiling effects of the English version iHOT-12 in a series of patients undergoing hip arthroscopy for intra-articular hip pathology at a minimum of one year postoperatively. Methods. A total of 171 consecutive patients undergoing hip arthroscopy with a diagnosis of femoroacetabular impingement (FAI) under the care of a single surgeon between January 2013 and March 2017 were included. iHOT-12 and EuroQol 5D-5L (EQ-5D-5L) scores were available pre- and postoperatively. Effect size and ceiling effects for the iHOT-12 were calculated with subgroup analysis. Results. A total of 122 patients (71.3%) completed postoperative PROMs scores with median follow-up of 24.3 months (interquartile range (IQR) 17.2 to 33.5). The median total cohort iHOT-12 score improved significantly from 31.0 (IQR 20 to 58) preoperatively to 72.5 (IQR 47 to 90) postoperatively (p < 0.001). The effect size (Cohen’s d) was 1.59. In all, 33 patients (27%) scored within ten points (10%) of the maximum score and 38 patients (31.1%) scored within the previously reported minimal clinically important difference (MCID) of the maximum score. Furthermore, nine (47%) male patients aged < 30 years scored within 10% of the maximum score and ten (53%) scored within the previously reported MCID of the maximum score. Conclusion. There is a previously unreported ceiling effect of the iHOT-12 at a minimum one-year follow-up which is particularly marked in young, male patients following hip arthroscopy for FAI. This tool may not have the maximum measurement required to capture the true outcome following this procedure. Cite this article: Bone Joint J 2020;102-B(8):1010–1015

The gold standard treatment of hip dysplasia is a peri-acetabular osteotomy (PAO). Labral tears are seen in the majority of patients presenting with hip dysplasia and diagnosed using Magnetic Resonance Imaging (MRI). The goal was to (1) evaluate utility/value of MRI in patients undergoing hip arthroscopy at time of PAO, and (2) determine whether MRI findings of labral pathology can predict outcome. A prospective randomized controlled trial was conducted at tertiary institutions, comparing patients with hip dysplasia treated with isolated PAO versus PAO with adjunct hip arthroscopy. This study was a subgroup analysis on 74 patients allocated to PAO and adjunct hip arthroscopy (age 26±8 years; 89.2% females). All patients underwent radiographic and MRI assessment using a 1.5-Tesla with or 3-Tesla MRI without arthrography to detect labral or cartilage pathology. Clinical outcome was assessed using international Hip Outcome Tool-33 (iHOT). 74% of patients (55/74) were pre-operatively diagnosed with a labral tear on MRI. Among these, 41 underwent labral treatment (74%); whilst among those without a labral tear on MRI, 42% underwent labral treatment (8/19). MRI had a high sensitivity (84%), but a low specificity (56%) for labral pathology (p=0.053). There was no difference in pre-operative (31.3±16.0 vs. 37.3±14.9; p=0.123) and post-operative iHOT (77.7±22.2 vs. 75.2±23.5; p=0.676) between patients with and without labral pathology on MRI. Value of MRI in the diagnostic work-up of a patient with hip dysplasia is limited. MRI had a high sensitivity (84%), but low specificity (44%) to identify labral pathology in patients with hip dysplasia. Consequently, standard clinical MRI had little value as a predictor of outcome with no differences in PROM scores between patients with and without a labral tear on MRI. Treatment of labral pathology in patients with hip dysplasia remains controversial. The results of this subgroup analysis of a prospective, multi-centre RCT do not show improved outcome among patients with dysplasia treated with labral repair

Purpose: Hip arthroscopy has been used at an increasing frequency over the last few years. Majority of patients undergoing hip arthroscopy are young and active individuals who seek definitive therapy for a painful hip condition and wish to avoid undergoing a hip replacement. Although relatively successful, complications following hip arthroscopy occur. This multi-institutional study presents a worrisome and previously unrecognized complication of hip arthroscopy, namely chondrolysis that lead to accelerated development of end-stage arthritis. Method: Using the computerized database in each institution, patients undergoing total hip arthroplasty between 1999–2008 who had received hip arthroscopy prior to arthroplasty were identified. 39 patients were identified to have undergone at least one hip arthroscopy on the affected hip prior to arthroplasty. There were 21 female and 18 male patients. The mean age of patients at the time of hip arthroscopy was 45.9 years. Data regarding demographics, comorbidities, preoperative diagnosis, number of previous procedures, and the details of the surgical procedure were compiled. Radiographs were evaluated. Results: The median time from arthroscopy to arthroplasty was 14.8 months (range 2.2 months to 7 years). Fourteen patients (35%) underwent THA within 12 months of a previous hip arthroscopy and in nine of these patients the indication for hip arthroscopy was labral tear debridement. These patients despite having none to minimal arthritis at the time of arthroscopy developed accelerated arthritis within a year that necessitated hip replacement. Conclusion: Hip arthroscopy can cause accelerated degenerative arthritis of the hip in some patients. We believe the subset of our patients who developed arthritis so early after hip arthroscopy may have suffered chondrolysis and/or chondral injury during the arthroscopy that resulted in progressive and aggressive arthritis of the hip within 12 months. Hip arthroscopy should be reserved for a select group of patients. All measures to minimize the possibility of chondrolysis and/or chondral injury should be exercised

Background. Hip arthroscopy is well established as a diagnostic and therapeutic tool in the native hip joint. However, its application in the symptomatic post-hip arthroplasty patient is still being explored. Aims and Methods. We have described the use of hip arthroscopy in symptomatic patients following total hip replacement, resurfacing hip arthroplasty and partial resurfacing hip arthroplasty in 24 patients (study group), and compared it with arthroscopy of the native hip of 24 patients. Results. The diagnostic yield of hip arthroscopy in symptomatic post-arthroplasty patients was 95.8% (23 / 24) and a therapeutic arthroscopic intervention resulted in relief of symptoms in 41.6% (10 / 24) of the patients. It led to revision hip replacement in a further 29.1% (7 / 24). In contrast, hip arthroscopy of the native hip (control group) had a 100% diagnostic yield and an arthroscopic therapeutic intervention was carried out in all the patients resulting in symptomatic relief in 87.5% (21 / 24). The mean operative time in the study group (59.7 mins, SD 21.1) was less than the control group (71 mins, SD 17.1, p < 0.05) but the arthroscopic approach was more difficult. Conclusion. Hip arthroscopy is a rapidly expanding technique that has a steep learning curve. Simulation may have a role in helping trainees overcome this. However there is as yet no validated hip arthroscopy simulator. This study aimed to test the construct validity of a virtual reality hip arthroscopy simulator. Nineteen orthopaedic surgeons performed a simulated arthroscopic examination of a healthy hip joint in the supine position. Surgeons were categorized as either expert (those who had performed 250 hip arthroscopies or more) or novice (those who had performed fewer than this). Twenty-one targets were visualized within joint; nine via the anterior portal, nine via the anterolateral and three via the posterolateral. This was followed by a task testing basic probe examination of the joint in which a series of eight targets were probed via the anterolateral portal. Each surgeon's performance was evaluated by the simulator using a set of pre-defined metrics including task duration, number of soft tissue & bone collisions, and distance travelled by instruments. No repeat attempts at the tasks were permitted. Construct validity was then evaluated by comparing novice and expert group performance metrics over the two tasks using the Mann–Whitney test, with a p value of less than 0.05 considered significant. On the visualization task, the expert group outperformed the novice group on time taken (P=0.0003), number of collisions with soft tissue (P=0.001), number of collisions with bone (P=0.002) and distance travelled by the arthroscope (P=0.02). On the probe examination, the two groups differed only in the time taken to complete the task (P=0.025). Increased experience in hip arthroscopy was reflected by significantly better performance on the VR simulator across two tasks, supporting its construct validity. This study validates a virtual reality hip arthroscopy simulator and supports its potential for developing basic arthroscopic skills

Aim. The aim of this study was to assess the role of hip arthroscopy in the management of hip pain in elite athletes. Patients and methods. It is a retrospective study of prospectively collected data. 31 hip arthroscopies were performed on 27 elite athletes. All the patients were assessed pre-operatively with a thorough clinical examination, radiographs and MRI scans. The modified HHS was also recorded for all the patients' pre and post-operatively. All operations were performed by the senior author, and the patients were assessed at 6 weeks, 3 months, 6 months and a year following the operation. Results. The M:F ratio was 25:2 with an average age of 38 years (Range: 18-58) in the study group. 65% of the patients were professional football players followed by cricket and tennis. The patients were referred at an average of 506 days. Two of the 27 patients demonstrated minimal acetabular dysplasia on their pre-operative radiographs. Of the 31 arthroscopies, a primary diagnosis of a labral tear was made in 20, a chondral flap and delamination in 5, early osteoarthritis in 4 and a ligamentum teres tear in 2. Along with the primary diagnosis of a labral tear, twelve of the 20 patients had a secondary diagnosis of chondral injury and or Femeroacetabular Impingement as well. There were no complications reported. Conclusions. Hip Injuries in athletes are reported late and Chondral Flaps and Ligamentum teres tears are under-diagnosed. Most athletes have a good chance to return back to the original level of sport within 4 months following hip arthroscopy. However, about 40% do continue to have some form of pain. Hip arthroscopy is a safe and effective method for diagnosis and treatment of intra-articular disorders in elite athletes

Introduction. Hypothermia is the drop in body temperature under 35°C (95°F), It has implications in immunological function and healing process, increasing the infection and the cardiovascular risk. During hip arthroscopy patients are exposed to several risk factors that may lead to hypothermia. Objective. to determinate if there are hypothermia and which are the factors contributing to hypothermia during hip arthroscopy. Methods and materials. we developed an prospective observational analytical study in a cohort group of patients that went to a hip arthroscopy, all operated by a single surgeon for treatment of femoroacetabular impingement, all were done under general anesthesia in supine position, with vair hugher warmer body air convection system always in 43.3°C; and we registered central body temperature (esophagical) during the entire procedure, we also recorded operation room temperature, volume (liters) and temperature of the saline solution used for the arthroscopy, age of the patient, body mass index (BMI), arterial tension, pump pressure, and surgical time. We analyzed the results with STATA 10.0 statistical software. Results. we performed 12 cases, 30 years old in average (14 – 46), BMI 24.0 in average (18.7 – 30.4), and hypothermia was founded in 16% (2/12) of cases. There are a direct assosiation between hypothermia and operating time over 120 minutes (p <0,001) (fig.1). Despite the rest of the parameters we didn't find any other correlation. Conclusion. Hypothermia is present in 16,67% of the patients, during hip arthroscopy for the treatmemt of femoroacetabular impingement, are group is small but we demonstrated a direct correlation between operating time and hypothermia, further studys with larger groups should be performer to identify the correlations and risk factors for hypothermia in these patients

We present a prospective two-year follow-up study of 1038 hip arthroscopies performed at a high volume tertiary referral centre for hip preservation. We feel that this manuscript is both pertinent and timely due to the advances in the field of hip preservation. We used four validated patient-reported outcome (PRO) scores along with the visual analog scale (VAS) and patient satisfaction scores to assess preoperative and postoperative outcomes in all patients undergoing hip arthroscopy. We divided the entire cohort into patients undergoing primary and revision hip arthroscopies. We found a statistically significant improvement from preoperative to two-year postoperative PRO scores in the two subgroups. We also found a significant difference in the PRO scores at three months, one year, and two years postoperatively between the primary and revision subgroups. The revision subgroup had inferior VAS and patient satisfaction compared to the primary subgroup, however these results were not significant. The conversion to total hip arthroplasty/hip resurfacing (THA/HR) was 5.6% and 11.2% in the primary and revision subgroups, respectively. This resulted in a relative risk of 2.0 for conversion to THA/HR in the revision subgroup. We had a complication rate of 5.3 (only 0.5% of which were considered major) which was similar to that reported in the literature for hip arthroscopy. The primary purpose was to perform a survival analysis in a large mixed cohort of patients undergoing hip arthroscopy at a high volume tertiary referral centre for hip preservation with minimum two-year follow-up. The secondary purpose was to compare clinical outcomes of primary versus revision hip arthroscopy. From February 2008 to June 2012, data were prospectively collected on all patients undergoing primary or revision hip arthroscopy. Patients were assessed pre- and post-operatively with modified Harris Hip Score (mHHS), Non-Arthritic Hip Score (NAHS), Hip Outcome Score-Activities of Daily Living (HOS-ADL), and Hip Outcome Score-Sport Specific Subscales (HOS-SSS). Pain was estimated on the visual analog scale (VAS). Patient satisfaction was measured on a scale from 0 to 10. There were a total of 1155 arthroscopies performed, including 1040 primary arthroscopies (926 patients) and 115 revision arthroscopies (106 patients). Of these, 931 primary arthroscopies (89.5%) in 824 patients (89.0%) and 107 revision arthroscopies (93.0%) in 97 patients (91.5%), were available for follow-up and included in our study. The mean change in patient reported outcome (PRO) scores at two-year follow-up in the primary subgroup was 17.4 for mHHS, 19.7 for HOS-ADL, 23.8 for HOS-SSS, 21.3 for NAHS, and −3.0 for VAS. The mean change in PRO scores at two-year follow-up in the revision subgroup was 13.4, 10.9, 16.1, 15.4, and −2.7, respectively. All scores improved significantly compared to pre-operatively (p<0.001). PRO scores were higher at all time points for the primary subgroup compared to the revision subgroup (p<0.05). Satisfaction was 7.7 and 7.2 for primary and revision subgroups, respectively. Of 931 primary arthroscopies, 52 (5.6%) underwent THA/HR. Of 107 revision arthroscopies, 12 (11.2%) underwent THA/HR. The relative risk of a THA/HR was 2.0 after revision procedures compared to primary procedures. The overall complication rate was 5.3%. Hip arthroscopy showed significant improvement in all PRO, VAS, and satisfaction scores at two years postoperatively. Primary arthroscopy patients showed greater PRO scores and a trend towards greater VAS compared to the revision subgroup. The relative risk of a THA/HR was 2.0 after revision procedures compared to primary procedures

Background: Developmental dysplasia of the hip (DDH) is a common paediatric orthopaedic problem. Open reduction and debridement of the hip joint in neonates is necessary to ensure a congruent reduction in some patients. Despite advances in the treatment of DDH, the various surgical approaches are not without limitations and risks. The development of hip arthroscopy is a new science, which we believe could be applied to the treatment of DDH. Aims: To date there have been no reports in the literature of the use of hip arthroscopy in either the neonatal hip or in infantile hips with DDH. The purpose of this study was: (a) to design a suitable animal model of DDH for the purpose of designing and evaluating hip arthroscopy, (b) to document the pathoanatomy of the dysplastic hip arthroscopically and (c) to define the methodology of performing hip arthroscopy in neonates with DDH. Method: A novel model of producing hip dysplasia in large white cross piglets has been created. 4-week-old piglets undergo surgical fixation of the knee by retrograde passage of a 3.5mm diameter steinmann pin. After free ambulation, progressive hip dysplasia is produced. We have monitored the development of hip dysplasia at 4 and 6 weeks post fixation by plain radiographs, MRI and Hip Arthroscopy using a 2.7mm diameter arthro-scope. Results: We have successfully produced hip dysplasia in an animal model of comparable size and anatomy to that seen in infants. Hip arthroscopy was performed in 20 animals. Documentation of a lax capsule, elongated ligamentum teres and pulvinar has been made. In addition arthroscopic debridement of the joint has been performed. We believe that arthroscopic debridement of the impediments to reduction in DDH is possible using the techniques learned from this model

There is a known association between femoroacetabular impingement and osteoarthritis of the hip. What is not known is whether arthroscopic excision of an impingement lesion can significantly improve a patient’s symptoms. This study compares the results of hip arthroscopy for cam-type femoracetabular impingement in two groups of patients at one year. The study group comprised 24 patients (24 hips) with cam-type femoroacetabular impingement who underwent arthroscopic debridement with excision of their impingement lesion (osteoplasty). The control group comprised 47 patients (47 hips) who had arthroscopic debridement without excision of the impingement lesion. In both groups, the presence of femoroacetabular impingement was confirmed on pre-operative plain radiographs. The modified Harris hip score was used for evaluation pre-operatively and at one-year. Non-parametric tests were used for statistical analysis. A tendency towards a higher median post-operative modified Harris hip score was observed in the study group compared with the control group (83 vs 77, p = 0.11). There was a significantly higher proportion of patients in the osteoplasty group with excellent/good results compared with the controls (83% vs 60%, p = 0.043). Additional symptomatic improvement may be obtained after hip arthroscopy for femoroacetabular impingement by the inclusion of femoral osteoplasty

Aim: The purpose of this study is to explore the experience of a consultant orthopaedic surgeon, and to quantitatively describe the learning curve for hip arthroscopy. Introduction: Arthroscopic surgery in orthopaedics is a well established procedure for both diagnostic and therapeutic purposes. Unlike many other joint arthroscopies, hip arthroscopy has been delayed in its development. It was first pioneered by Burman in 1931, who under-took a study on cadavers, stating that ‘it is manifestly impossible to insert a needle between the head of the femur and the acetabulum’. Over several decades, this technique has developed considerably, but still remains a technically demanding and difficult procedure. The learning curve for hip arthroscopy has not previously been objectively quantified. Method: We prospectively reviewed the first 100 hip arthroscopies performed in the supine position between 1999 and 2004. Surgery was performed by a single experienced hip and knee consultant orthopaedic surgeon (FH). We assessed the operative time (traction time), surgeon comfort, patient satisfaction at 6 months and operative complications. This was analysed for consecutive blocks of 10 cases. Results of the first 10 and the remaining 90 cases, subsequently the first 20 and remaining 80 cases, and finally the first 30 and remaining 70 cases were compared for a difference. Results: The mean traction time was 55 minutes (range: 36–94 minutes). Mean surgeon comfort was 73% (range: 52–89%). 49% of patients reported an excellent outcome at 6 months follow – up. Only 8% of patients reported an unsatisfactory outcome. The main complications noted were chondral damage (6 cases) and perineal injuries (4 cases). There was a remarkable decrease in complications from the first 30 cases compared to the remaining 70 operations. 5 cases of chondral damage was noted in the first 30 cases, compared to 1 (1.4%) in the remaining 70 cases. The number of perineal injuries was noted to decrease from 3 cases in the first 30 operations to 1 (1.4%) in the subsequent 70 operations. There is an overall decrease in operative time over the 100 cases, representing a gradual learning process throughout. However, the fall from an average time of 75 minutes for the first 30 cases, to the average operative time of 30 minutes for the remaining 70 cases, is a significant learning process (40% fall in operative time). We thus, believe the learning curve to be 30 operations. Conclusion: We have demonstrated that there is a considerable fall in operative time when comparing the first 30 cases with the remaining 70 cases. This quantitative decrease is indicative of a rapid learning curve. This is further suggested by the remarkable fall in complications during this learning phase

Introduction: The popularity of hip arthroscopy has lagged behind that of other joints. However surgeons are increasingly using hip arthroscopy to investigate and treat disorders such as early osteoarthritis, inflammatory arthritis, labral tears, loose bodies and paediatric hip disease. We present the indications, intra operative findings and outcomes of patients undergoing hip arthroscopy. Methods: Prospective study of 43 patients from 2000 to 2005 undergone hip arthroscopy performed by the senior author. Pre & post operative visual analogue scores and oxford hip scores were recorded. CT or MRI was performed were clinically indicated. Mean follow up 4 months, range(2– 10). Results: 45 hips. 20 right. 21 left. 2 bilateral. 22 females. 21 males. Mean age 39.6yrs, Range 20–65yrs. Symptoms: Pain, Clicking, Giving way. History: Idiopathic pain 30, DDH 5, Perthes 3, Trauma acetabulum fracture 2, RT A Dislocation 1, SUFE 1, Non union 1. 6 patients had pre-op CT scans and 22 had MRI. MRI Findings: 3 loose bodies, 14 labral tears, 2 large filling defects, 3 normal MRI. 42 Arthroscopic debridement and wash outs, 3 failed scopes. Intra operative findings: 5 loose bodies, 4 degenerate labrum’s, 10 labral tears, 14 Grade 3–4 Osteoarthritic changes, 7 Torn ligament Teres,1 normal joint. 3 normal MRI findings had labral tears and articular cartilage defects. Mean Pre-op VAS- 7.9 Range(5– 10). Mean Post-op VAS- 4.7 Range(1– 10). Mean Pre-op Oxford Hip score – 39.4 Range(27–53). Mean Post-op Oxford Hip Score – 25.2 Range(12–51). Patient Satisfaction score – 7.3 Range(1–10). 1 Superficial wound infection, settled with antibiotics. Discussion: Hip arthroscopy is of value in assessing and treating patients with hip pain of uncertain cause. Our results indicated good patient satisfaction and outcomes with improved pain and Oxford hip scores. However patient selection and diagnostic expertise are critical to successful outcomes

Hip arthroscopy is becoming more popular. A literature review demonstrated paucity of published papers reporting the outcome of hip arthroscopy in teenagers without developmental dysplasia of the hip. Our aim was to record the type of lesions found and report the outcome and level of satisfaction following hip arthroscopy in teenagers. From 2002 to 2008, 96 hip arthroscopies were undertaken in 76 patients. Pre-operative and two-week, six-week and current post-operative assessments were performed using the modified Harris hip score (HHS) and the Non Arthritic Hip Score (NAHS). In addition, a satisfaction survey was completed at their most recent review. Patients enrolled in the study were under the age of 20. Patients with a history of developmental dysplasia of the hip, Perthes disease and arthritis were excluded from the study. Patients had at least a six-month follow-up from their surgery. Our study cohort comprised 53 males and 43 females with an average age of 17 years old (range 13 to 19 years). The average duration of follow up was 19 months (range 3 to 75 months). There were 41 left and 54 right-sided hip arthroscopies. There were five re-operations. The average duration of hip traction was 19 minutes (range 6 to 47 minutes). We found pathology in all hips that underwent arthroscopy. We report a significant improvement in MHHS and NAHS at six weeks and current review (p-value <0.01). Sixty-two percent of patients had returned to sport at the previous level of competition, 32% of patients returned to sport at a lower level of competition and 5% patients did not return to sport. Overall, 84% of patients were satisfied following their hip arthroscopy and 91% would have the surgery again if they had to. There were five re-operations. Our study has revealed a range of intra articular hip pathologies amenable to surgical treatment using hip arthroscopy. We have observed a significant improvement in hip scores; with up to 94% of patients returning to sport in the short term with high satisfaction levels. Long term follow up of this group is ongoing

Hip arthroscopy is a relative newcomer to the arthroscopic repertoire, yet even in its current infancy has shown the hip joint in a different light. One poorly understood area is the ligamentum teres, thought by many to do little more than carry a blood vessel in the developing hip. Ligamentum injuries have only rarely been described, though in recent years have been arthroscopically classified. We present a large series of ligamentum teres abnormalities treated by hip arthroscopy. Of 925 consecutive hip arthroscopies, 68 patients (73 hips) had either a complete ligamentum tear (Type I), partial ligamentum tear (Type II), or a degenerate ligamentum (Type III). Only 4 patients had a correct diagnosis given prior to hip arthroscopy. Hyperadduction was the commonest cause of Type I tears, though not all tears had a traumatic aetiology. Complete tears have a shorter history and a higher chance of associated intra-articular damage. Partial tears typically have a long history of ill-defined hip pain. The degenerate ligamentum generally presents with the features of underlying osteoarthritis. At three-year review, Type I and II tears improved significantly following hip arthroscopy, Type III abnormalities less so. This paper thus presents the largest series of ligamentum tears to-date reported

Periacetabular osteotomy (PAO) is the preferred treatment for symptomatic acetabular dysplasia in adolescents and young adults. There remains a lack of consensus regarding whether intra-articular work such as labral repair or improvement of femoral offset should be performed at the time of PAO or addressed subsequent to PAO if symptoms warrant. The purpose was to determine the rate of subsequent hip arthroscopy (HA) in a contemporary PAO cohort with no intra-articular work performed at the time of PAO. From June 2012 to March 2022, 368 rectus sparing PAOs were performed and followed for a minimum of one year (mean 5.9 years). The average age was 24 (range 14–46) and 89% were female. Patients were evaluated at last follow-up for patient-reported outcomes (PROMs). Clinical records were reviewed for complications or subsequent surgery. Radiographs were reviewed for the following acetabular parameters: LCEA, ACEA, AI, and the alpha-angle (AA). Patients were cross-referenced from the two largest hospital systems in our area to determine if subsequent HA was performed. Descriptive statistics were used to analyze risk factors for HA. 16 hips (15 patients) (4.4%) underwent subsequent HA with labral repair and femoral osteochondroplasty most common. For those with a minimum of two years of follow-up, 5.3% underwent subsequent HA. No hips underwent THA; one revision PAO was performed. 14 hips experienced a complication and 99 underwent hardware removal. All PROMs improved significantly post-operatively. Radiographically 80% of hips were in goal for acetabular correction parameters with no significant differences between those who underwent subsequent HA and those who did not. Rectus sparing PAO is associated with a low rate of subsequent HA for intra-articular pathology at 5-year follow-up. Acetabular correction alone may be sufficient as the primary intervention for the majority of patients with symptomatic acetabular dysplasia

We have investigated whether the pattern of subchondral acetabular cyst formation reflects hip pathology and may provide a prognostic indicator for treatment. A single surgeon series of sequential hip arthroscopies was reviewed to identify the most recent 200 cases undertaken on a previously un-operated joint with pre-operative plain radiographs and computed tomography or magnetic resonance scan available for review. Also, serial “non-arthritic hip scores” (NAHS) recorded pre-operatively, at 6 weeks and 3 months post-surgery. The acetabular Lateral Centre Edge Angle, the Acetabular Index, the FEAR index and the Kallgren and Lawrence grade were determined. All images were reviewed by two independent assessors and divided into four groups according to acetabular subchondral cyst distribution. NC - No cysts - 74 cases (mean: 40.1 years), SPC - a Single Peripheral Cyst – 58 cases (mean: 43.1 years), SDC - a Solitary Dome Cyst – 32 cases (mean: 33 years) and MC - Multiple Cysts – 36 cases (mean: 42.7 years). No association was identified between gender and patient reported outcomes. SDC patients were significantly younger than the other three groups (p <0.001). At three months after surgery, the average increase of the NAHS in the four groups was 25.3, 23.5, 4.2 and 4.9 respectively. Acetabular dysplasia was identified in 72% of the SDC group compared to 18%, 16% and 33% in NC, SPC and MC groups. Degenerative change was identified in 86% of the MC group compared to 18%, 40% and 41% of the NC, SPC and SDC groups. The early patient reported outcome following hip arthroscopy was significantly affected by the location and number of acetabular subchondral cysts. A Solitary Dome Cyst is indicative of underling hip dysplasia. Multiple Cysts are indicative of degenerative disease. The majority of patients in these groups have poor three-month outcomes after hip arthroscopy

Hip arthroscopy (HA) and pelvic osteotomy (PO) are surgical procedures used to treat a variety of hip pathology affecting young adults, including femoroacetabular impingement and hip dysplasia respectively. This study aimed to investigate the trends and regional variation in the provision of HA and PO across England from 2010 to 2023 to inform healthcare resource allocation. We analysed the National Hospital Episode Statistics database for all HA and PO procedures in NHS England using specific OPCS-4 codes: HA: ‘W83+Z843’ or ‘W84+Z843’; PO: ‘X222+Z75’. We collected patient demographics, age, sex, and region of treatment. We performed descriptive and regression analyses to evaluate temporal trends in PO volume, age, sex and regional variation. 22,401 HAs and 1,348 POs were recorded between 2010 and 2023. The annual number of HAs declined by 28.4%, whilst the number of POs increased by 64% (p<0.001). Significantly more females underwent PO vs HA (90% vs 61.3%) and were older than males undergoing the same procedure (PO: 29.0±8.7 vs 25.8±9.2 years; HA: 36.8±12.0 years vs 35.8±11.2 years, p<0.001). For HA, the mean age of both sexes decreased by 3.3 and 2.9 years respectively (p<0.001), whereas the age of PO patients did not change significantly over the study period. There were significant regional variations with a mean incidence of 1.60/100,00 for HA (ranging from 0.70–2.66 per 100,000) and 0.43/100,000 for PO (ranging from 0.08–2.07 per 100,000). We have observed a decline in HA volume in England, likely due to improved patient selection and the impact of COVID-19, whilst PO volume has significantly increased, with regional variation persisting for both procedures. These trends highlight the need for equitable HA and PO access to improve patient outcomes and call for strategic healthcare planning and resource allocation to reduce disparities and improve training opportunities

Arthroscopic procedures are increasingly performed for femoroacetabular impingement (FAI). The Non-Arthroplasty Hip Register (NAHR) collects data including the iHOT12 and EQ5D. However there is currently little evidence of its usefulness in assessing hip arthroscopy outcomes. This study aims to assess minimum 1-year outcomes of hip arthroscopy for FAI using the minimum data set (MDS) of the NAHR by comparing these to a patient satisfaction questionnaire. Pre-operative scores for 78 consecutive hips in 76 patients (43F/33M, mean age at surgery 31.76±10.02 years) undergoing hip arthroscopy for FAI at our institution between February 2013-June 2015 were entered into the NAHR. Insufficient post-operative data was available from the registry. Therefore we collected iHOT12, EQ5D and satisfaction data by postal survey. Preoperative mean iHOT-12 score was 32.67±14.23, median EQ5D Index score 0.653 (IQR 0.277) and median EQ5D Visual Analogue Scale 70 (IQR 25). Postoperative scores were available for 56 cases (55 patients, 71.8%) at median 18.9 months (IQR 13.77). There was a significant postoperative improvement in self-reported outcome as measured by iHOT-12 (mean improvement 35.7 points, p<0.001) and EQ5DIndex scores (median improvement 0.127, p=0.001). 24 patients were very satisfied, 19 satisfied, 6 neutral, 4 dissatisfied and 1 very dissatisfied. Satisfied patients exhibited greater improvement in iHOT-12 (mean 41.64±19.29 vs 2.8±24.08, p<0.001) and EQ5D index scores (p=0.013) but no difference in EQ5D VAS (p=0.15). Improvement in iHOT12 correlated with improvement in EQ5DIndex (r=0.676, p<0.001) and EQ5DVAS(r=0.552, p<0.001). Hip arthroscopy for FAI yielded significant improvements in iHOT-12 and EQ5D index scores and 80% of responders were satisfied/very satisfied at a minimum one year postop. iHOT12 and EQ5DIndex correlated with patient satisfaction and improvements in iHOT12 correlated with improvements in general quality of life. Our findings suggest that the MDS of the NAHR is useful for assessing the outcome in these patients

Hip arthroscopy is a well-established technique becoming more and more an indispensable tool in institutions specialized in hip diseases. Several surgeons around the world have developed and refined the proper instruments and the surgical technique for this operation. By now, the indications have been well formulated for both diagnostic and interventional purposes. My personal experience is of 98 hip arthroscopies performed in the last 6 years. Most common preoperative indication has been chronic hip pain after failure of conservative treatment. Other indications or arthroscopic findings have been: labral pathology, hip dysplasia, synovial chondromatosis, initial osteoarthritis, calcium pyrophosphate disease, ligamentum teres damage, chondral damage, post-traumatic loose bodies, avascular necrosis, sepsis, villonodular synovitis. More recent, indications for hip arthroscopy are staging of avascular necrosis of the femoral head and shaving of polyethylene debris after total hip replacement. Contraindications to arthroscopy include recent fracture of the pelvis osteoarthritis with osteophytosis, AVN with head collapse. Hip arthroscopy can facilitate both comprehensive access to and treatment of an evolving series of conditions that affect the hip joint. Purpose of this presentation is to show the surgical technique and present the results obtained. New indications and potential future evolutions are also discussed

Introduction. The role for hip arthroscopy (HS) in combination with periacetabular osteotomy (PAO) for the treatment of symptomatic acetabular dysplasia has not been clearly defined. In this study, we assessed midterm outcomes of concurrent hip arthroscopy and PAO (HS/PAO) for the treatment of symptomatic acetabular dysplasia with associated intra-articular pathology. Methods. We performed a retrospective review of concurrent HS/PAO cases in a prospectively collected institutional hip preservation database. From November 2005 to December 2012, 85 hips in 80 patients underwent combined HS/PAO. Two hips had a diagnosis other than acetabular dysplasia and 12 hips were lost to follow up, leaving 71 hips for analysis at mean 6.6-year follow-up (range 4–11 years). Hips were evaluated using the University of California, Los Angeles (UCLA) Activity Score, modified Harris Hip Score (mHHS), and the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) pain subscore. Hips were stratified into preserved or failed groups, with failure defined as conversion to THA or a symptomatic hip (mHHS <70 or WOMAC pain subscore > 10). Complications were recorded and graded by the Clavien-Dindo classification. Regression analyses were performed to identify the interaction between patient factors, radiographic measures, and surgical details with outcome. Results. At final follow-up, two hips had undergone THA and an additional 15 hips were deemed symptomatic. 54 hips (76.1%) did not undergo conversion to THA or meet symptom thresholds, and this group had mean mHHS 92.1 ± 8.8, UCLA activity score 7.8 ± 2.1, and WOMAC pain subscore 1.8 ± 2.4. In univariate analysis, no patient factors, pre- or post-operative radiographic metrics, or intra-operative findings or procedures were associated with failure. There were 3 (4.2%) major complications (Clavien-Dindo Grade III or IV), and 5 patients underwent repeat surgery for recurrent symptoms (4 hip arthroscopy, 1 open psoas release with ramus osteoplasty) during the follow up period with resolution of symptoms following repeat surgery. Conclusion. This study demonstrates that concurrent hip arthroscopy and periacetabular osteotomy is associated with good clinical outcomes and an acceptable complication profile at midterm follow up

Orthopaedic surgeons frequently use opioids for peri-operative pain management and there is considerable variability in the amount prescribed between surgeons. As such, the appropriate number of opioids to prescribe for specific procedures is often unknown. Leftover prescription opioids are at risk for diversion to family and friends for nonmedical use. The aim of this study was to determine the optimal amount of narcotics to prescribe postoperatively for patients undergoing hip arthroscopy. 23 consecutive patients were enrolled. All patients were prescribed 60 tablets of hydrocodone/acetaminophen 10/325 postoperatively as part of a multimodal pain management strategy. Patients were called at 14 and 21 days post-operatively to tabulate the number of pills used and knowledge of how to properly dispose of pills. The median number of narcotic pain pills required was 6 (IQR: 3,15). Over half of patients (56.5%) required ≤10 narcotic pain pills postoperatively. A large number of narcotic tablets (1071/1380, 77.6%) were unused and a small percentage of patients (8/23, 34.8%) were aware of proper disposal techniques. Knowledge of how to properly dispose of unused narcotics was protective against a prolonged duration of narcotic use postoperatively (Parameter estimate −5.7, 95% CI: −11.3, −0.1, p = 0.045). Reducing the number of prescribed narcotic tablets to 25 would meet the post-operative pain demands of over 85% of hip arthroscopy patients. More judicious post-operative prescribing patterns and patient education regarding disposal may help minimize physician contribution to opioid misuse, overuse and diversion


Background. Hip arthroscopy is a rapidly growing, evolving area within arthroscopic Orthopaedic Surgery, with annual rates increasing as much as 25-fold each year. Despite improvements in equipment and training, it remains a challenging procedure. Rates of revision surgery have been reported as 6.3% to 16.9%. Objectives. The primary objective was to determine the success of joint preservation after hip arthroscopy. The secondary objective was to determine whether patient characteristics or PROM functional score trends could predict revision hip arthroscopy or Total Hip Arthroplasty (THA). Study Design & Methods. We reviewed 1363 hip arthroscopies performed from January 2010 to December 2016 by a single high-volume surgeon at a single institution. Data was prospectively collected and retrospectively reviewed with a minimum 2-year follow-up. Functional outcomes were assessed with the International Hip Outcome Tool (IHOT-33). Hip arthroscopy failures were defined as Total Hip Arthroplasty or revision hip arthroscopy after index hip arthroscopy. Results. There were751 females and 612 males with an average age of 34.63 years (19 – 58 years). There were 199 cases (14.6%) of labrum repairs only, 286 (20.9%) cam and labrum repairs, 319 (23.4%) cam and pincer surgeries and 193 (14.1%) cam only surgeries. All pre-operative IHOT-33 patient-reported outcomes scores (27.42 ± 6.2) improved significantly at the 6 week-, 3 month- and 6-month follow-up visits (p<0.05). The best improvements were seen in symptoms and functional improvements (IHOT-SFL) (p<0.05) while the poorest amelioration of job-related concerns (IHOT-JRC) existed at all time points (p<0.05). There were 223 failures, 131 patients (9.61%) underwent revision hip arthroscopy and 92 required THA (6.75%) at 18.45 months ± 7.34 months. The 2-year survival rate was 89.2%. The THA conversion rate was 8.4% for patients > 50 years old and 3.72% for patients < 50 years old. Age (>50 years) and female sex were associated with increased risk of conversion to THA (p<0.05) while young (<25 years old) and female patients were most likely to undergo hip revision arthroscopy. Surgery involving repair of the labrum only are more likely to result in revision arthroscopy surgery and THA (70.7% at 10.6 years). CAM & pincer surgery have best longevity (90.9% at 10.6 years). Conclusions. Joint preservation and no subsequent surgery at 10.6 years is 83.64%. This study showed that predictors of revision hip arthroscopy or THA included poor pre-morbid functional score, female gender, age > 45, sudden functional score decrease at 3 months follow-up and cases in which only the labrum is surgically repaired

Aims: Developmental dysplasia of the hip (DDH) is a common paediatric orthopaedic problem. Open reduction and debridement of the hip joint in neonates is necessary to ensure a congruent reduction in some patients. Despite advances in the treatment of DDH, the various surgical approaches are not without limitations and risks. The purpose of this study was: (a) to design a suitable animal model of DDH for the purpose of designing and evaluating hip arthroscopy, (b) to document the pathoanatomy of the dysplastic hip arthroscopically and (c) to deþne the methodology of performing hip arthroscopy in neonates with DDH. Methods: A novel model of producing hip dysplasia in large white cross piglets has been created. 4-week-old piglets undergo surgical þxation of the knee by retrograde passage of a 3.5mm diameter steinmann pin. After free ambulation, progressive hip dysplasia is produced. We have monitored the development of hip dysplasia at 4 and 6 weeks post þxation by plain radiographs, MRI and Hip Arthroscopy using a 2.7mm diameter arthroscope. Results: We have successfully produced hip dysplasia in an animal model of comparable size and anatomy to that seen in infants. Hip arthroscopy was performed in 20 animals. Documentation of a lax capsule, elongated ligamentum teres and pulvinar has been made. In addition arthroscopic debridement of the joint has been performed. We believe that arthroscopic debridement of the impediments to reduction in DDH is possible using the techniques learned from this model

This study used a national registry to assess the outcomes of hip arthroscopy (HA) for the treatment femoroacetabular impingement (FAI). All HAs for FAI recorded in the UK Non-Arthroplasty Hip Registry (NAHR) between January 2012 and September 2023 were identified. Cases were grouped according to the index procedure performed for FAI (cam, pincer, or mixed). Patient outcomes captured included the International Hip Outcome Tool (iHOT)-12. 7,511 HAs were identified; 4,583 cam (61%), 675 pincer (9%), 2,253 mixed (30%). Mean age (34.8) was similar between groups. There was a greater proportion of females in the pincer group (75%) compared to cam (52%) and mixed (50%). A higher proportion of patients had a recorded cartilage injury in association with a cam lesion compared to pincer. The pincer group had poorer mean pre-op iHOT-12 scores (31.6 \[95%CI 29.9 to 33.3\]; n=364) compared to cam (33.7 \[95%CI 32.1 to 34.4\]; n=3,941) and achieved significantly lower scores at 12 months (pincer = 52.6 (50.2 to 55); n=249, cam = 58.3 (57.1 to 59.5); n=1,679). Overall, significant (p < 0.0001) iHOT-12 and EQ-5D improvement vs baseline pre-operative scores were achieved for all FAI subtypes at 6 months (overall mean iHOT-12 improvement +26.0 \[95%CI 25.0 to 26.9\]; n=2,983) and maintained out to 12 months (+26.2 \[25.1 to 27.2\]; n=2,760) at which point 67% and 48% of patients continued to demonstrate a score improvement greater than or equal to the minimum clinically important difference (>/=13 points) and substantial clinical benefit (>/=28 points) for iHOT-12 respectively. This study demonstrates excellent early functional outcomes following HA undertaken for FAI in a large national registry

Labral tears can lead to disabling hip pain however underlying structural (femoroacetabular impingement) and developmental abnormalities predisposing to labral pathology may be left untreated if the peripheral compartment is ignored during hip arthroscopy. Femoroace-tabular impingement (FAI) can be secondary to abnormal morphologic features involving either the proximal femur and/or the acetabulum. Both acetabular labral tears and FAI lead to premature osteoarthritis of the hip. Early diagnosis and treatment of these hip pathologic abnormalities is important, not only to provide pain relief but also to prevent the development of osteoarthritis. Purpose: To describe the technique for arthroscopy of the peripheral compartment of the hip joint without traction. Methods: We performed 9 hip arthroscopies without traction from a lateral and and anterolateral portals in the supine position. After a traditional central arthroscopy with traction, 60 degrees of flexion at the hip joint without traction allowed relaxation of the anterior capsule and increased the intra-articular volume of the peripheral compartment. Results: Inspection of the peripheral compartment was obtained from the anterolateral portal. The anterior neck area, medial neck area, medial head area, anterior head area, lateral head area and lateral neck area were viewed. In 3 patients, loose bodies were removed. In 1 patient with PVNS synovial biopsy was taken and synovectomy was performed. Osteochondroplasty was performed in 5 patients for femoroacetabular impingement. No complications were observed. Conclusions: Hip arthroscopy without traction is mandatory to complete assessment and adequate treatment of the painful hip

Surgical trainees are finding it increasingly more challenging to meet operative requirements and coupled with the effects of COVID-19, we face a future of insufficiently trained surgeons. As a result, virtual reality (VR) simulator training has become more prevalent and whilst more readily accepted in certain arthroscopic fields, its use in hip arthroscopy (HA) remains novel. This project aimed to validate VR high-fidelity HA simulation and assess its functional use in arthroscopic training. Seventy-two participants were recruited to perform two basic arthroscopic tasks on a VR HA simulator, testing hip anatomy, scope manipulation and triangulation skills. They were stratified into novice (39) and experienced (33) groups based on previous arthroscopy experience. Metric parameters recorded from the simulator were used to assess construct validity. Face validity was evaluated using a Likert-style questionnaire. All recordings were reviewed by 2 HA experts for blinded ASSET score assessment. Experienced participants were significantly faster in completing both tasks compared with novice participants (p<0.001). Experienced participants damaged the acetabular and femoral cartilage significantly less than novice participants (p=0.011) and were found to have significantly reduced path length of both camera and instrument across both tasks (p=0.001, p=0.007), demonstrating significantly greater movement economy. Total ASSET scores were significantly greater in experienced participants compared to novice participants (p=0.041) with excellent correlation between task time, cartilage damage, camera and instrument path length and corresponding ASSET score constituents. 62.5% of experienced participants reported a high degree of realism in all facets of external, technical and haptic experience with 94.4% advising further practice would improve their arthroscopic skills. There was a relative improvement of 43% in skill amongst all participants between task 1 and 2 (p<0.001). This is the largest study to date validating the use of simulation in HA training. These results confirm significant construct and face validity, excellent agreement between objective measures and ASSET scores, significant improvement in skill with continued use and recommend VR simulation to be a valuable asset in HA training for all grades

Over an eight-month period we prospectively enrolled 122 patients who underwent arthroscopic surgery of the hip for femoroacetabular impingement and met the inclusion criteria for this study. Patients with bilateral hip arthroscopy, avascular necrosis and previous hip surgery were excluded. Ten patients refused to participate leaving 112 in the study. There were 62 women and 50 men. The mean age of the patients was 40.6 yrs (95% confidence interval (CI) 37.7 to 43.5). At arthroscopy, 23 patients underwent osteoplasty only for cam impingement, three underwent rim trimming only for pincer impingement, and 86 underwent both procedures for mixed-type impingement. The mean follow-up was 2.3 years (2.0 to 2.9). The mean modified Harris hip score (HHS) improved from 58 to 84 (mean difference = 24 (95% CI 19 to 28)) and the median patient satisfaction was 9 (1 to 10). Ten patients underwent total hip replacement at a mean of 16 months (8 to 26) after arthroscopy. The predictors of a better outcome were the pre-operative modified HHS (p = 0.018), joint space narrowing ≥ 2 mm (p = 0.005), and repair of labral pathology instead of debridement (p = 0.032). Hip arthroscopy for femoroacetabular impingement, accompanied by suitable rehabilitation, gives a good short-term outcome and high patient satisfaction

Introduction. With the evolution of hip arthroscopy, it has been used as joint preserving surgery for osteoarthritis among various other indications. The purpose of this study was to assess the factors that affect the subgroup of patients with osteoarthritis that have had total hip replacements following hip arthroscopy. Methods. Data was retrieved retrospectively from 556 osteoarthritic patients' files that have had hip arthroscopy between the years 2002 to 2009 (mean follow up time 3.2 – range 1 to 6.4 years). Eighty-three (15%) of them have had eventually total hip replacement. The data analysis included different variables (i.e. age, arthritic stage, repeated procedures) that may have influenced the time elapsed between the hip arthroscopy and consecutive replacement procedures. Results. The time gap from the first arthroscopy to a hip replacement was longer in patients younger than 55 years (mean 1.9 – range 0.2 to 5.1 years) and in a milder osteoarthritic stage (mean 2.2 – range 0.2 to 5 years). Patients that had several hip scopes (2 to 4) had a double time gap (mean 2.8 – range 1.4 to 4.7 years) than those with only a single procedure. Conclusion. Osteoarthritic patients that are treated with hip arthroscopy have 15% chance of having a hip replacement within the period of seven years. The time difference for a hip replacement after an arthroscopy is longer in younger age and in milder osteoarthritis. Repeated arthroscopies may delay the need for arthroplasty

Previous authors have suggested that the analgesic effects of intra-articular morphine may be beneficial. Clonidine has been found to potentiate the analgesic effect of morphine. Following knee arthroscopy, morphine has demonstrated equivocal effect in comparison to bupivicaine for analgesia while circumventing the issue of chondrotoxicity. There have been no studies evaluating the effect of intra-articular morphine following hip arthroscopy. The purpose of this study was to evaluate the efficacy of intra-articular morphine in combination with clonidine on pain and narcotic consumption following hip arthroscopy surgery for femoroacetabular impingement. A retrospective review was performed on 43 patients that underwent hip arthroscopy between September 2014 and May 2015 at our institution for femoroacetabular impingement. All patients received preoperative Celebrex and Tylenol per our anesthesia protocol, and 22 patients received an additional intra-articular injection of 10 mg morphine and 100 mcg of clonidine at the conclusion of the procedure. Narcotic consumption, duration of anesthesia recovery, and perioperative pain scores were compared between the two groups. We found that patients who received intra-articular morphine and clonidine used significantly less opioid analgesic in the PACU, with 23 mEq of morphine equivalents required in the intra-articular morphine and clonidine group compared to 40 mEq of opiod equivalents in the non-injection group (p=0.0259). There were no statistically significant differences in time spent in recovery prior to discharge or in VAS pain scores recorded immediately post-operatively and at one hour following surgery. In conclusion, we found that an intraoperative intra-articular injection of morphine and clonidine significantly reduced the amount of narcotic requirement following hip arthroscopy. We do believe that there may be significant benefits to this, including less systemic effects from overall narcotic usage in the perioperative period. Our study demonstrated a beneficial effect of intra-articular morphine that may help with overall pain improvement, less narcotic consumption, and improved patient satisfaction following outpatient hip arthroscopy. This study provides the foundation for future research currently being conducted in a randomised-control setting

Purpose: There is a paucity of literature describing clinical outcomes following hip arthroscopy. Variables associated with short or prolonged recovery are undefined. This presents a challenge to surgeons in preoperatively communicating with patients about expectations after surgery. The goals of this study are to identify predictors of recovery and to develop models which will facilitate the proper counseling of patients prior to hip arthroscopy. In this study, we define a normal recovery after hip arthroscopy, determine the predictive values of preoperative and intraoperative variables for recovery and for progression to total hip arthroplasty (THA) after hip arthroscopy. Method: A retrospective review of 216 individuals treated with hip arthroscopy at a tertiary medical center was conducted by a single reviewer. Univariate analysis was used to identify independent variables that correlated with prolonged or short recovery following hip arthroscopy and also on variables correlated with progression to THA. Binary logistic regression analysis was used to develop and test multivariate models for predicting prolonged recovery and progression to THA. Results: Univariate analyses revealed multiple variables (spanning demographics, past medical history, radiographic findings, physical examination findings, and intraoperative findings) which were significantly (p≤0.05) correlated with prolonged recovery (13 significant predictors) and also with progression to THA (14 significant predictors). A multivariate predictive algorithm was generated using 5 significant predictors of prolonged recovery, which included Workman’s compensation involvement, female gender, use of pain medications, presence of a limp, and presence of a lateral labral tear. This algorithm was tested successfully using an independent sample of 25 individuals. Three multivariate predictors of progression to THA after hip arthroscopy were identified, including radiographic presence of arthritis, female gender and the presence of grade 4 chondral lesions, and a predictive algorithm was generated. Conclusion: We generated and initially validated a multivariate algorithm to predict prolonged recovery following hip arthroscopy. If validated in larger sample, this model may allow a surgeon to appropriately counsel patients regarding expectations for recovery after hip arthroscopy

Introduction. Decreases in trainees' working hours, coupled with evidence of worse outcomes when hip arthroscopies are performed by inexperienced surgeons, mandate the development of additional means of arthroscopic training. Though virtual reality simulation training has been adopted by other surgical specialities, its slow uptake in arthroscopic training is due to a lack of evidence as to its benefits. These benefits can be demonstrated through learning curves associated with simulator training – with practice reflecting measurable increases in validated performance metrics. Materials & Methods. Twenty-five medical students completed seven simulated arthroscopies of a healthy virtual hip joint in the supine position on a simulator previously shown to have construct validity. Twelve targets had to be visualised within the central compartment; six via the anterior portal, three via the anterolateral portal and three via the posterolateral portal. Eight students proceeded to complete seven probe examinations of a healthy virtual hip joint. Eight targets were probed via the anterolateral portal. Task duration, number of collisions with soft tissue and bone, and distance travelled by arthroscope were measured by the simulator for every session. Results. A learning curve was demonstrated by the students, with significant improvements in time taken (P<0.01), number of collisions (P<0.01), collision severity (P<0.01), and efficiency of movement (P<0.01). The largest difference between consecutive sessions was seen between sessions 1 and 2, with sessions thereafter showing only minimal rates of improvement. Similar improvements were found in the probe examination with students showing significant improvements in time taken (P<0.01), number of collisions (P<0.01), collision severity (P<0.01) and distance travelled by arthroscope (P<0.01). Conclusion. The results of this study demonstrate a learning curve for a previously validated hip arthroscopy simulator, confirming improved performance with repeated use. These results support the use of virtual reality as a potential means of developing basic hip arthroscopic skills

Purpose: This purpose of this study was to report 2 year outcomes following hip arthroscopy for the treatment of femoroacetabular impingement and chondrolabral dysfunction and determine factors associated with outcomes. Method: Between 3/2005 and 10/2005, 122 patients underwent hip arthroscopy by a single surgeon. Patients were included if they underwent arthroscopic treatment for FAI and chondrolabral dysfunction. All patients were prospectively enrolled in an IRB approved follow-up study. Ten patients refused to participate following enrollment. Results: At arthroscopy, for treatment of impingement, 23 patients underwent only osteoplasty for CAM impingement, 3 patients underwent only rim reduction for pincer impingement and 86 patients underwent osteoplasty and rim reduction for mixed type impingement. Ten patients underwent total hip arthroplasty at an average of 16 months (range: 8–26) after arthroscopy. Average follow-up was obtained on 90%(92/102) at 2.3 years (range: 2.0–2.9). The modified Harris Hip score(MHHS) improved from 57 to 84. HOS ADL improved from 69 to 88. HOS Sport improved from 41 to 69. NAHS improved from 64 to 82. All scores significantly improved (p< 0.05). The average patient satisfaction was 8.4 (range: 1–10). All patients returned to work (15% returned within 1 week, 53% returned in 1–5 weeks, 18% returned in 6–8 weeks, 13% returned in 2 to 6 months). Ten patients underwent THA at an average of 16 months. These patients were significantly older at arthroscopy (58 vs. 39;p=0.0001), had significantly less joint space at all 3 weight bearing surfaces (p=0.001), and patients with microfracture on both femoral head and acetabulum were more likely to undergo THA (p=0.001). The predictors of higher post-operative MHHS were preoperative modified Harris Hip score (p=0.018), joint space of 2mm or greater(p=0.005), and repair of labral pathology instead of debridement (p=0. 032)(r2=0.32;p=0.001). Patients with labral repair had higher MHHS compared to patients with labral debridement (87vs81). Independent predictors of patient satisfaction were postoperative MHHS(p=0.001) and joint space of 2mm or greater(p=0.040) (r2=0.67; p=0.001). Conclusion: Hip arthroscopy for femoroacetabular impingement accompanied by the prescribed rehabilitation led to return to excellent function and high patient satisfaction. Factors associated with higher outcomes included joint space of greater than 2mm, and labral repair for treatment of labral pathology

Hip arthroscopy (HA) is an effective treatment for various hip conditions but has a steep learning curve and its effect on long-term joint preservation is unclear. This study uses population-level data to assess (1) the 90-day complication rate, and (2) the frequency and timing of revision HA, total hip replacement (THR), and pelvic osteotomy (PO) following primary HA. We performed a retrospective analysis of the National Hospital Episode Statistics database, examining all patients who underwent primary HA in NHS hospitals in England from 2010 to 2023 using relevant OPCS-4 codes. We evaluated patient demographics, 90-day complications, and reoperation rates for revision HA, THR, and PO. Descriptive statistical analyses were performed to calculate frequencies and average time to reoperations. We included 22,401 HA procedures in the study. The mean LOS was 0.82±2.04 days. The 90-day readmission rate was 0.17% at a mean of 54.4±8.1 days. The most common reasons for readmission were reoperation (0.071%), followed by infection (0.031%), pulmonary embolism (0.027%), pain (0.022%), bleeding (0.018%), and deep vein thrombosis (0.004%). One patient died within 90 days. Overall, 4942 patients (22.1%) required further surgery at a mean of 2.71±2.27 years. The rates of revision HA, conversion to THR, and PO were 6.94%, 14.6%, and 0.50% at a mean of 2.39±1.79, 2.87±2.46, and 2.26±1.80 years respectively. Female patients had higher rates of reoperation than males for conversion to THR (9.99% vs 4.63%), revision HA (4.92% vs 2.02%), and subsequent PO (0.43% vs. 0.06%) (p<0.001). This study demonstrates a low short-term complication rate after primary HA, supporting existing literature. However, a large proportion of patients required further surgery, especially females. These findings highlight the need for careful patient selection and counselling before HA to optimise outcomes, as well as further research on factors influencing longer-term outcomes and cost-effectiveness

The risk that hip preserving surgery may negatively influence the performance and outcome of subsequent total hip replacement (THR) remains a concern. The aim of this study was to identify any negative impact of previous hip arthroscopy on THR. Out of 1271 consecutive patients who underwent primary THR between 2005 and 2009, eighteen had previously undergone ipsilateral hip arthroscopy. This study group (STG) was compared with two control groups (CG: same approach, identical implants; MCG: paired group matched for age, BMI and Charnley categories). Operative time, blood loss, evidence of heterotopic bone and implant loosening at follow-up were compared between the SG and the MCG. Follow-up WOMAC were compared between the three groups. Blood loss was not found to be significantly different between the SG and MCG. The operative time was significantly less (p>0.001) in the SG. There was no significant difference in follow-up WOMAC between the groups. No implant related complications were noted on follow-up radiographs. Two minor complications were documented for the SG and three for the MCG. We have found no evidence that previous hip arthroscopy negatively influences the performance or short-term outcome of THR

Background. Femoroacetabular impingement (FAI) may be a predisposing factor in progression of osteoarthritis. The use of hip arthroscopy is in its infancy with very few studies currently reported. Early reports show favourable results for treatment of young patients with FAI. This prospective study over a larger age spectrum represents a significant addition to this expanding field of minimally invasive surgery. Methods. Over a twenty-two month period all patients undergoing interventional hip arthroscopy were recorded on a prospective database. Patient demographics, diagnosis, operative intervention and complications were noted. Patients were scored pre-operatively and postoperatively at 6 months and 1 year using the McCarthy score. Results. 94 patients met the criteria for inclusion in this study. Median age was 39 (15–66) years with 57.4% female, 16 patients were over the age of 50 years. At operation, 50 patients had a labral tear, 70 had cam impingement and 62 had chondral damage, with 21 of these deemed severe (grade 3 or 4). For all patients a mean increase in the McCarthy score of 14.6 (p=<0.0001) was seen at 6 months and 19.1 (p=0.0002) at 1 year postoperatively. For those over 50 years, at 6 months an increase of 11.9 (p=0.08) was seen, improving to 33.8 (p=<0.0001) at 1 year. Eight patients underwent THR, of these, 2 were over 50 years of age. All of this group of patients had chondral damage (50% judged as severe at arthroscopy). At 6 months postoperatively these patients had severe pain and their mean score worsened by −16.3 (p=0.2). Few complications were seen, 7 patients had tight access (5 males), 3 of which received chondral scuffing and 1 had the procedure abandoned, 1 patient had groin labral bruising and 2 patients had transient parasthesia. Conclusions. Patients see a gradual and significant improvement post hip arthroscopy, with symptoms continuing to improve until 1 year. This is a well-tolerated procedure with a low complication rate and the opportunity for treatment of a range of hip pathology. Patients over the age of 50 gain a significant improvement from this procedure. Two groups of patients who gain no improvement are those with inflammatory arthropathy and those with dysplastic acetabulae


Arthroscopic management of femoroacetabular impingement (FAI) has become the mainstay of treatment. However, chondral lesions are frequently encountered and have become a determinant of less favourable outcomes following arthroscopic intervention. The aim of this systematic review and meta-analysis was to assess the outcomes of hip arthroscopy (HA) in patients with FAI and concomitant chondral lesions classified as per Outerbridge. A systematic search was performed using the PRISMA guidelines on four databases including MEDLINE, EMBASE, Cochrane Library and Web of Science. Studies which included HA as the primary intervention for management of FAI and classified chondral lesions according to the Outerbridge classification were included. Patients treated with open procedures, for osteonecrosis, Legg-Calve-Perthes disease, and previous ipsilateral hip fractures were excluded. From a total of 863 articles, twenty-four were included for final analysis. Demographic data, PROMs, and radiological outcomes and rates of conversion to total hip arthroplasty (THA) were collected. Risk of bias was assessed using ROBINS-I. Improved post-operative PROMs included mHHS (mean difference:-2.42; 95%CI:-2.99 to −1.85; p<0.001), NAHS (mean difference:-1.73; 95%CI: −2.23 to −1.23; p<0.001), VAS (mean difference: 2.03; 95%CI: 0.93-3.13; p<0.001). Pooled rate of revision surgery was 10% (95%CI: 7%-14%). Most of this included conversion to THA, with a 7% pooled rate (95%CI: 4%-11%). Patients had worse PROMs if they underwent HA with labral debridement (p=0.015), had Outerbridge 3 and 4 lesions (p=0.012), concomitant lesions of the femoral head and acetabulum lesions (p=0.029). Reconstructive cartilage techniques were superior to microfracture (p=0.042). Even in concomitant lesions of the femoral head and acetabulum, employing either microfracture or cartilage repair/reconstruction provided a benefit in PROMs (p=0.027). Acceptable post-operative outcomes following HA with labral repair/reconstruction and cartilage repair in patients with FAI and concomitant moderate-to-severe chondral lesions, can be achieved. Patients suffering from Outerbridge 3 and 4 lesions, concomitant acetabular rim and femoral head chondral lesions that underwent HA with labral debridement, had worse PROMs. Reconstructive cartilage techniques were superior to microfracture. Even in concomitant acetabular and femoral head chondral lesions, employing either microfracture or cartilage repair/reconstruction was deemed to provide a benefit in PROMs

We present our data on a cohort of 25 patients who had an arthroscopy of their hip between the ages of 12 and 17 out of over 1100 hip arthroscopies performed. All patients presented with pain and marked restriction of activities. Either a CT (before MRI was available) or MRI scan was done pre-operatively. 10 patients presented with a history of either Perthes disease, DDH or a defined injury causing their symptoms. In the remaining the onset of symptoms was spontaneous. The intra-operative diagnosis varied: normal (6), labral tear (6), loose bodies (2), debris and/or chondromalacia (8), synovitis (2) and damaged ligamentum teres (1). None of the patients developed a complication. The pre-operative Harris Hip Score ranged from 0 to 40 (mean 21) for pain and 0 to 47 (mean 35) for function. The follow-up ranged from 6 weeks to 9years (mean 3 years). Harris Hip Score at latest follow-up ranged from 10 to 44 (mean 33) for pain and from 31 to 47 (mean 43) for function. Conclusion: Hip arthroscopy is a very useful tool as part of the investigation and treatment of hip problems in carefully selected children and adolescents

The purpose of this study is to audit the early clinical outcomes of a consecutive series of hip arthroscopies performed by one surgeon. Pre-operative WOMAC and Non Arthritic Hip scores were performed on the patients. All patients had MRI arthrograms. These were correlated with the clinical findings at the time of surgery. Postoperatively all complications were recorded and at one year post surgery the pre-operative scores were repeated. There are 37 consecutive cases. 15 males and 22 females. Average age 39 years. 76% of cases were accident related. The average length of symptoms pre-operatively was 21 months. There were 2 false negative MRI arthrograms and one false positive. The surgical treatment included debridement of 18 labral tears and 10 labral tears associated with an articular cartilage defect. The average traction time was 39 minutes. The complications were minor and all have resolved. There are many indications for hip arthroscopy however the outcomes of labral debridement are not clearly defined. This paper attempts to measure the success of arthroscopic labral debridement in a consecutive series of patients. The clinical symptoms and MRI arthrograms seem to have a good correlation with the arthroscopic findings. The short term results of this treatment are encouraging as is the low morbidity associated with the surgery itself

Introduction: Periacetabular rotational osteotomy is a joint preserving reconstructive surgery indicated in hip dysplasia. However hip dysplasia is frequently accompanied by intraarticular pathology like labral tears. This study was undertaken to ascertain whether:. concomitant hip arthroscopy during osteotomy could identify intra-articular pathology associated with hip dysplasia;. hip arthroscopy combined with our technique of acetabular osteotomy was effective in treatment of intraarticular pathology;. this technique was associated with a favorable outcome over a mid term followup period. Materials: Between July 2001 and February 2005, 43 dysplastic hips with 40 patients consecutive of periac-etabular rotational osteotomy using modified Ollier’s approach and concomitant hip arthroscopy could be followed up at average 52 months (range, 42–67 months). On arthroscopic examination labral lesion and cartilage condition was evaluated and torn labrum was treated. Two patients underwent simultaneous femoral procedures. Mean age at the time of surgery was 40 years (range, 20–67 years). Pre and postoperative hip function was compared using Harris hip score. Radiographic evaluation consisted of pre and postoperative measurement of CE angle, Sharp angle, acetabular head index, head lateralization index and medialization of femoral head. The degree of osteoarthritis was evaluated and compared by radiographic appearance using Tönnis classification. Results: Intraoperative arthroscopic examination revealed labral lesion in 38 hips. Twenty-eight (65%) out of 43 cases showed cartilage degeneration on arthros-copy. Mean HHS changed from 72.4 preoperatively to 94 postoperatively. On radiographic evaluation CE angle changed from 6.3o to 32.3o, Sharp angle changed from 47o to 39o, acetabular head index changed from 57% to 92% thus indicating the increase in contact area. Head lateralization index changed from 61.2 to 54.9. At the latest review, the severity of osteoarthritis improved in 12 (28%), remained unchanged in 29 (67.4%), and worsened in 2 cases (4.7%). Complications include intraarticular osteotomy, disruption of posterior column and deep vein thrombosis in 1 case each. No other complications like non-union, nerve injury, avascular necrosis, or infection were seen. No patients underwent a conversion THA indicating a 100% survival for the minimum followup period of 42 months and beyond. Conclusion: Periacetabular rotational osteotomy showed good results in midterm and in 88% of cases we observed and treated labral lesion through a concomitant hip arthroscopy. Thus we recommend that evaluation and treatment of labral lesion should be considered when the joint-preserving acetabular osteotomy is performing due to dysplastic hip

Problems with chondral toxicity caused by prolonged exposure to local anaesthetics have been increasingly recognised. However, day-case hip arthroscopic surgery is frequently carried out using an intraarticular depot of local anaesthetic as post-operative analgesia plus additional opiate or oral analgesia as required. We aimed to evaluate the efficacy of three different post-operative analgesic regimes at hip arthroscopy, in particular examining whether intraarticular local anaesthetics gave any benefit. We investigated 71 consecutive patients undergoing day-case hip arthroscopy and prospectively audited their post-operative analgesic requirements. Each patient was given one of three alternative post-operative analgesic regimes. Group A (n=29) received bupivicaine 0.25% 10ml intraarticular and 20ml peri-portal skin infiltration, group B (n=23) had bupivicaine 0.25% 20ml peri-portal skin infiltration only, and group C (n=19) had no infiltration. Outcome measures were visual analogue scores (VAS) at time-points T1 (immediate post-operatively), T2 (one hour post-operatively), T3 (two hours post-operatively), and T4 (four hours post-operatively). Total opiate consumption was also recorded. There was significantly less post-operative pain in group A, compared with group C at T1 (p=0.03) and T2 (p=0.004), and compared with group B at T3 (p=0.02) and T4 (p=0.03). There were no significant differences in VAS between groups B and C at any time-points. Group A used significantly less opiates post-operatively compared with group B (p=0.008) or C (p< 0.001) but there was no significant difference in opiate use between groups B and C. There are no previous studies relating to hip arthros-copy post-operative analgesic requirements. Intraarticular local anaesthetic significantly reduces post-operative pain, but at what cost to the chondral surface? Local skin infiltration of the arthroscopy portals does not significantly alter pain levels or opiate requirements. Avoidance of intararticular local anaesthetic raises opiate requirements. We require improved alternative analgesic regimes

Introduction. Periacetabular osteotomy (PAO) is the preferred treatment for symptomatic acetabular dysplasia in adolescents and young adults. There remains a lack of consensus regarding whether intra-articular work such as labral repair or improvement of femoral offset should be performed at the time of PAO or addressed subsequent to PAO if symptoms warrant. The purpose of this review was to determine the rate of subsequent hip arthroscopy (HA) in a contemporary PAO cohort with no intra-articular work performed at the time of PAO. Methods. From June 2012 to September 2019, 272 Rectus Sparing PAOs were performed and followed for a minimum of one year (mean 4.6 years). The average age was 24 (range 14–44) and 87% were female. The average BMI was 25 and average length of hospital stay was 2.9 days. Patients were evaluated at last follow-up with PROMIS PF-CAT, pain and mental health scores. Clinical records were reviewed for complications or subsequent surgery. Pre and post-operative radiographs were reviewed for change in the following acetabular parameters: LCEA, ACEA, AI, and the alpha-angle was obtained from preoperative radiographs. Patients were cross-referenced from the two largest hospital systems in our area to determine if subsequent HA was performed. Descriptive statistics and logistic regression were used to analyze risk factors for HA. Results. 13 hips (12 patients) (4.8%) underwent subsequent HA with labral repair and femoral-osteochondroplasty most common. No hips underwent THA and one revision PAO was performed. 13 hips experienced a complication and 90 hips underwent hardware removal. All PROM improved significantly post-operatively. Radiographically 80% of hips were in goal for acetabular correction parameters and amongst the LCEA, ACEA, AI and alpha angle (AA), only the AA was significant risk factor for subsequent arthroscopy. Conclusion. Rectus sparing PAO is associated with a low rate of subsequent HA for intra-articular pathology at 5-year follow-up. Acetabular correction alone may be sufficient for the majority of patients with symptomatic acetabular dysplasia

Reconstructive acetabular osteotomy is a well established and effective procedure in the treatment of acetabular dysplasia. However, the dysplasia is frequently accompanied by intra-articular pathology such as labral tears. We intended to determine whether a concomitant hip arthroscopy with peri-acetabular rotational osteotomy could identify and treat intra-articular pathology associated with dysplasia and thereby produce a favourable outcome. We prospectively evaluated 43 consecutive hips treated by combined arthroscopy and acetabular osteotomy. Intra-operative arthroscopic examination revealed labral lesions in 38 hips. At a mean follow-up of 74 months (60 to 97) the mean Harris hip score improved from 72.4 to 94.0 (p < 0.001), as did all the radiological parameters (p < 0.001). Complications included penetration of the joint by the osteotome in one patient, a fracture of the posterior column in another and deep-vein thrombosis in one further patient. This combined surgical treatment gave good results in the medium term. We suggest that arthroscopy of the hip can be performed in conjunction with peri-acetabular osteotomy to provide good results in patients with symptomatic dysplasia of the hip, and the arthroscopic treatment of intra-articular pathology may alter the progression of osteoarthritis

We would like to present our experience with hip arthroscopy done in a dorsal decubitus traction table with three standart portals. In a period of 24 months we operated 60 hips (55 patients) with a multitude of patologies. There were 30 men and 25 women with a mean age of 33,4 years old. The mean follow-up was 6 months. Labral tears were found in 20 scopes, associated with arthrosis in 4 cases and ligamentus teres tear in 2 cases. We did hip scopes in 10 total joint replacements to evaluate infection ( 5 cases), to treat acute infection ( 2 cases) and to evaluate pain with no apparent etiology (3 cases). Eigth cases were done after a hip dislocation, to evaluate and compare the arthroscopy with CT scans and MRI. In three of these cases we extracted bone fragments from within the joint. We had two cases of gunshot projectiles that were extractes successfully. Three cases were done in acute hip pyoartrosis. We had a 16% complication rate, mainly in the 25 first cases (pudendal nerve apraxia in 4 cases, condral flap tears in 4 cases, broken instrumental in 3 cases, worsening of syntoms in 2 cases, genital aedema in 2 cases, and others in 4 cases). The preliminary results showed an improving in Harris hip score in 57% of the patients. The results are encouraging and we must stress that there is a learning curve that is long, but hip arthroscopy is going to be an important and standart procedure in the very near future for hip surgeons

Health care facilities are major contributors of waste to landfills, with operating rooms estimated to assume 20–70% of this waste. With hip arthroscopy for femoroacetabular impingement (FAI) on the rise, it is important to understand its environmental impact and identify areas for greening practices. Given its minimally-invasive nature, we hypothesise overall arthroscopic waste per FAI patient case to be approximately 5 kg, with minimal biohazard waste. The purpose of this study was to determine the amount of waste produced in FAI procedures and understand the environmental impact of the procedure to aid us in developing greening practices. A single surgeon waste audit (with descriptive statistics) of five FAI hip arthroscopy procedures – categorised by: 1) normal/landfill waste; 2) recyclable cardboards and plastics; 3) biohazard waste; 4) sharp items; 5) linens; and 6) sterile wrapping – was performed in April 2015. The surgical waste (except laundered linens) from the five FAI surgeries totalled 47.4 kg, of which 21.7 kg (45.7%) was biohazard waste, 11.7 kg (24.6%) was sterile wrap, 6.4 kg (13.5%) was normal/landfill waste, 6.4 kg (13.5%) was recyclable plastics, and 1.2 kg (2.6%) was sharp items. There was an average of 9.4 kg (excluding laundered linens) of waste produced per procedure. Considerable waste, specifically biohazard waste, is produced in FAI procedures with an average of 9.4 kg of waste produced per procedure, including 4.3 kg of biohazard waste. In Canada (population 35.7 million), approximately 18 800 kg of waste (8600 kg of biohazard waste) is produced from an estimated 2000 FAI procedures performed every year. Additional recycling programs, reducing surgical overage, and continued adherence to proper waste segregation will be helpful in reducing waste production and its environmental burden. An emphasis on “green outcomes” is also required to demonstrate environmental responsibility and effectively manage and allocate finite resources

Aim. The current literature indicates that hip arthroscopy is safe in both adolescents and adults with good early outcomes but there are no available studies that directly compare the outcomes of this procedure for mixed indications between these two groups. Method. This was a prospective cohort study of 102 consecutive supine hip arthroscopies in 96 patients (48 adolescent (<18) hips, mean age 13.9 years and 54 adult (>18 hips), mean age 33.8 years) at a tertiary referral paediatric and adult university hospital followed-up for a minimum of two years. Patient demographics, indications for surgery, Modified Harris Hip (MHHS) and Non-Arthritic Hip (NAHS) scores, operative interventions and complications were recorded. Results. At two-year follow-up, the MHHS improved from a mean of 55.3 to 87.5 (p<0.05) in the adolescent group and from 58.7 to 86.9 (p<0.05) in the adult group while the NAHS improved from 57.1 to 86.7 (p<0.05) in the adolescent group and from 61.4 to 87.4 (p<0.05) in the adult group. There was no statistical significance difference in outcome between the two groups at 2 years. With respect to complications, there were 2 cases of transient lateral femoral cutaneous nerve palsy (1 adolescent and 1 adult) that resolved spontaneously. There were 3 transient pudendal nerve palsies that resolved spontaneously in the adolescent group by 4 weeks. There was no difference in mean operative traction time in both groups (47.6 minutes in adolescents versus 48.3 minutes in adults; p<0.05). No cases of proximal femoral physeal growth disturbance or osteonecrosis were seen at final follow-up. Conclusion. This study confirms that hip arthroscopy for mixed indications leads to good early outcomes with low complication rates in adolescent and adult patients. The incidence of pudendal nerve palsy in the adolescent group is however noted and is of concern

Purpose. Femoroacetabular impingement syndrome (FAI) is a common cause of hip and groin pain in young adults. Physiotherapy and surgery have both been used to treat FAI syndrome, but there is no robust evidence of comparative effectiveness. UK FASHIoN compared the clinical and cost-effectiveness of arthroscopic hip surgery (HA) versus best conservative care in patients with FAI syndrome. Methods. UK FASHIoN was a pragmatic, multicentre, 2 parallel arm, superiority, randomised controlled trial in patients with FAI syndrome. Eligible patients were over 16 without radiographic signs of osteoarthritis, deemed suitable for arthroscopic FAI surgery. Participants were randomly allocated to HA or Personalised Hip Therapy (PHT - a physiotherapist-led programme comprising 6 to 10 sessions). The primary outcome measure was hip-related quality of life using the patient-reported International Hip Outcome Tool (iHOT-33) at 12 months. Secondary outcomes included EQ5D5L, SF12, adverse events, and cost-effectiveness. Primary analysis compared differences in iHOT-33 scores at 12 months by intention to treat. Results. 348 patients were randomised. Time to surgery was 132 days (SD71) versus 47 days (SD52) to PHT. 92.5% were followed-up at 12 months. Baseline mean iHOT-33 scores were 39.2 (SD21) and 35.6 (SD18) in the surgery and PHT groups, and at 12 months 58.8 (SD27) and 49.7 (SD25) respectively. Mean scores in both groups improved over 12 months, but the mean iHOT-33 score increased more in those allocated to HA than to PHT, with an adjusted mean difference of 6.8 points (95% CI 1.7,12.0 p=0.009). One HA patient developed an infection requiring further surgery. Mean overall costs were £3713 for HA and £1283 for PHT. Conclusions. Hip arthroscopy and best conservative care both led to improved hip-related quality of life in patients with FAI syndrome. At 12-month follow-up, improvement was greater in those allocated to hip arthroscopy

The cause of unremitting hip joint pain often remains undiagnosed. In the knee, MRI often accurately detects soft tissue pathology, but there is a less than 5% chance that MRI will show an abnormality such as a labral tear. Because conventional surgery can lead to such complications as avascular necrosis, muscle weakness, trochanteric nonunion, heterotopic bone formation, neurovascular injuries and DVT, diagnostic open arthrotomy is seldom performed. Further, prolonged hospitalisation is costly and commits the patient to an extended rehabilitation program. Arthroscopic hip surgery, a relatively new but well-established procedure, permits the identification of previously unrecognised disorders and in some pathologies offers definitive treatment. In young selected patients with OA it serves as a temporary palliative procedure. This presentation is based on the first 50 hip arthroscopies performed by the author. Indications for operation were chronic hip pain without radiologically-discernible cause, the presence of loose bodies following trauma, chondrocalcinosis, labral lesions, chondral injuries, ruptured ligamentum teres, avascular necrosis, early OA, biopsy for suspected RA, and non-specific synovial disease. All patients were in hospital for only a day. Mobilisation was encouraged from day one. Dramatic improvement in previous symptoms was noted postoperatively. In three patients with degenerative changes, pain returned after a mean of 6 months and THR was subsequently performed. Hip arthroscopy is a minimally invasive diagnostic and treatment technique, especially in young patients presenting with hip pain

What is not known is whether arthroscopic excision of an impingement lesion can significantly improve a patient’s symptoms. This study compares the one-year results of hip arthroscopy for cam-type FAI in two groups of patients. The study (osteoplasty) group comprised 24 patients (24 hips) with cam-type FAI who underwent arthroscopic debridement with excision of their impingement lesion. The control (no osteoplasty) group comprised 47 patients (47 hips) who underwent arthroscopic debridement without excision of their impingement lesion. In both groups, the presence of FAI was confirmed on pre-operative plain radiographs. The modified Harris hip score (MHHS) was used for evaluation pre-operatively and at one year’s follow-up. Non-parametric tests were used for statistical analysis. A tendency towards higher median post-operative MHHS scores was observed in the study than in the control group (83 vs. 77, p = 0.11). This was supported by a significantly higher portion of patients in the osteoplasty group with excellent/good results (83% vs. 60%, p = 0.043). It appears that even further symptomatic improvement may be obtained after hip arthroscopy for FAI by means of the femoral osteoplasty. When treating cam impingement arthroscopically, both central and peripheral compartments of the hip should always be accessed

The purpose of this study was to analyze whether Tonnis grading is a reliable predictor of OA in patients with hip dysplasia. Methods. Thirty patients were identified who had undergone hip arthroscopy surgery to assess their suitability for periacetabular osteotomy. Radiographs were assessed for anterior centre edge angle, lateral centre edge angle, Tonnis angle and Tonnis grade for OA changes. The radiographic grading of OA was compared with arthroscopic findings. Results. The average age at the time of arthroscopy was 35 [16–53 yrs] (28 females). Tonnis grade did not correlate with arthroscopic findings (p=0.082). There was a trend for patients with a higher Tonnis grade to have more OA changes. Of the 30 patients, all 3 with grade 0 were fit for periacetabular osteotomy, while only 8 out of 24 with grade I, and 1 out of 3 with grade II were fit for periacetabular osteotomy. Discussion. This study reports that even when radiographic grading showed minimal OA changes, arthroscopy findings indicated significant OA changes. Hence radiographic grading is a poor indicator of OA and other diagnostic modality should be sought before proceeding with joint preserving surgery in this highly selected subgroup of dysplastic hips

Purpose: The purpose of this study is to investigate the relationships of traction force, traction time, and hip distraction to the development of nerve conduction abnormalities during hip arthroscopy. Method: Thirteen patients with hip pathology underwent hip arthroscopy. Traction forces applied to the operative leg were measured using a load-cell force transducer. Distraction of the hip joint was assessed using fluoroscopy. Nerve conduction studies of the tibial nerve was performed measuring the latency of the Hoffmann reflex (Hlat reflex). Measurements of the traction force, distraction of the hip, and nerve conduction studies were performed at routine intervals during the procedure, and compared to pre- and post-op values. Results: Nerve Conduction Studies- The mean baseline Hlat reflex was 30.4+/−2.2 milliseconds for all patients. Three patients lost the Hlat reflex immediately (t=0), and an additional three patients lost the Hlat reflex during the procedure (t=30, t=30, t=60). The remaining seven patients all had delayed conduction of the Hlat reflex over time. At one-hour post-op, the Hlat reflex was documented in all patients (mean 31.9+/−2.9 ms) and remained significantly different from baseline (p< 0.01). Clinically, one patient in the lost Hlat reflex group had an associated neuropraxia post-operatively. Traction Forces- The mean initial traction force at time of application for all patients was 97+/−28 lbs. The mean initial traction force of the lost Hlat reflex group and retained Hlat reflex group was 104+/−32.6 lbs and 91+/−24.1 lbs, respectively (p=0.44). Hip Distraction- The mean initial hip distraction at time of application of traction for all patients was 8.8+/−2 mm. The mean initial distraction of the lost Hlat reflex group and retained Hlat reflex group was 9.6+/−1.4 mm and 8+/−2.2 mm, respectively (p=0.15). Conclusion: Traction during hip arthroscopy is associated with significant nerve conduction abnormalities in the immediate post-operative period. Six of thirteen patients had complete loss of the Hlat reflex of the tibial nerve, one of these patients exhibiting clinical neuropraxia. Although length of time in traction may be a factor for the development of nerve conduction abnormalities, a more significant factor may be the change in length over time of the surrounding peripheral nerves

Introduction. Successful joint preservation surgery requires the ability to accurately assess the health of the articular cartilage pre-operatively. Traditional radiological methods allow morphological assessment of the cartilage and therefore only identify those with established degeneration. Biophysical properties of cartilage are now being used to identify these changes occurring earlier in the disease processes. Prior literature states that healthy cartilage has a transverse relaxation time of between 15–60 ms (16). Our study aims to establish the correlation and accuracy of MRI with T2 cartilage mapping with observed intra-operative chondral defects. Methods. We routinely request MRI with T2 mapping on all patients with suspected or confirmed femoroacetabular impingement (FAI). A review was performed on all patients who underwent both pre-operative imaging and subsequent hip arthroscopy for FAI over a 24-month period. Using linear regression we correlated intra-operatively observed chondral defects of the femoral head and acetabulum (Outerbridge classification scores) with the pre-operative transverse relaxation times. Statistical analysis of 66 chondral points was undertaken. Results. Results show that there is a significant association between an increase in transverse relaxation time and higher acetabular Outerbridge classification (p = 0.0141). Discussion. This study has identified that MRI with T2 cartilage mapping is an accurate predictor of acetabular cartilage health. Our findings suggest that 3T MRI with T2 cartilage mapping is a useful tool in joint preservation surgery and provides accurate information allowing hip arthroscopists to identify patients who may benefit most from conservative operative intervention

Background. Labral tears are now recognised as a common pathology especially in young adults. With advancement of arthroscopic techniques, most recent published literature is focused on short- or mid-term results of labral repair or re-fixation. There is limited data regarding long-term results of labral debridement and effect of co-existing pathology on outcomes. We investigated long-term results after arthroscopic labral debridement, the predictors of outcomes and correlation with any co-existing hip pathology. Materials and Methods. Between 1996 and 2003, 50 patients who underwent hip arthroscopy and labral debridement with mean follow-up of 8.4±1.7 years (range 7-13.6 years) were included in our study. Patients' pre-operative Harris Hip Score and co-existing pathologies such as FAI, dysplasia or arthritis were recorded as variables. Further, patients' post-operative HHS and satisfaction at the time of follow-up were recorded as outcomes. Spearman's rho correlation coefficient and regression analysis were calculated between these variables and outcomes. Results. Good or excellent results were noted in 80% of patients. The mean pre-operative HHS was 79.3±8.2 (range 58-86) which was improved to 92.2±12.1 (range 46-100) post-operatively. Patients without co-existing pathology had significantly higher satisfaction and HSS. Arthritis had a significantly negative correlation with post-op HHS (rho= -0.27, P<0.05) and satisfaction (rho= -0.32, P<0.05). Eighty-four percent of patients were satisfied and 16% were dissatisfied. All dissatisfied patients (eight cases) had arthritis; two patients also had FAI that did not have decompression of the lesion since it was not recognised at the time. In regression analysis, arthritis was the strongest independent predictors of post-op HHS and satisfaction. Conclusion. Arthroscopic labral debridement of symptomatic tears without co-existing pathology can result in favorable long-term results. Arthritis is the strongest independent predictors of outcomes

Purpose: Synovial chondromatosis is a cartilaginous metaplasia that can result in multiple intracapsular and extracapsular loose bodies. Open arthrotomy has been the conventional treatment for this condition in the hip, but is associated with neurovascular embarrassment and femoral head osteonecrosis. Hip arthroscopy avoids these problems, and is a minimally invasive approach to diagnosis and treatment. Our aim was to evaluate the role of arthroscopy in the management of synovial chondromatosis of the hip at the early-to-intermediate stages of disease. Method: Twenty-nine patients had arthroscopic treatment for synovial chondromatosis of the hip. All lesions were intracapsular and smaller than 10mm. Radiographs of the painful hip, computed tomography, and gadolinium-enhanced magnetic resonance imaging were obtained pre-operatively. During arthroscopy, loose bodies were removed via a cannula, and partial synovectomy, partial labrectomy, chondroplasty, and microfracture were done as needed. Results: Fourteen patients were women and 15 were men; mean age was 41 years old; mean duration of symptoms was 52 months. All patients had hip pain. Mechanical hip symptoms were reported by 63% of patients. Twenty-three patients had a follow-up of at least 12 months (12 to 184 months of follow-up). Loose bodies could be seen in the imaging studies of 58% (15 of 29) of patients: 8 on radiographs and 7 with computed tomography or gadolinium-enhanced magnetic resonance imaging. At surgery, 23 of the 29 patients had torn or frayed labra. There was an average of 35 loose bodies per patient. Twenty-three of the 29 patients had femoral head changes, four of which had Grade III or IV lesions. Twenty-five of the 29 had acetabular chondral findings, ten of which had Grade III or IV lesions. Five of the 29 patients eventually underwent total hip reconstruction surgery at an average of 52 months follow-up; four of these 5 patients had at least a Grade III lesion at the time of arthroscopy. Eleven of the 23 patients who had a minimum of 12 months follow-up (12 to 184 months) had good to excellent outcomes at an average of 60 months follow-up. Complications included a case of perineal numbness and another with tingling of the foot. Conclusion: Our patients with synovial chondromatosis of the hip benefited from hip arthroscopy. Imaging studies alone, including gadolinium-enhanced MRI, failed to establish the diagnosis in half of the patients. Diagnosis was eventually made by direct visualization of the loose bodies via arthroscopy. None of our patients had subsequent femoral head osteonecrosis or infection. For patients who had Grade I or II cartilage lesions, early diagnosis and treatment via arthroscopy helped prevent or delay the onset of secondary osteoarthritis. Arthros-copy also avoided an open surgical exposure with its associated prolonged rehabilitation. It is a valid and effective treatment for early-to-intermediate stages of synovial chondromatosis

Aims. Femoroacetabular impingement (FAI) patients report exacerbation of hip pain in deep flexion. However, the exact impingement location in deep flexion is unknown. The aim was to investigate impingement-free maximal flexion, impingement location, and if cam deformity causes hip impingement in flexion in FAI patients. Methods. A retrospective study involving 24 patients (37 hips) with FAI and femoral retroversion (femoral version (FV) < 5° per Murphy method) was performed. All patients were symptomatic (mean age 28 years (SD 9)) and had anterior hip/groin pain and a positive anterior impingement test. Cam- and pincer-type subgroups were analyzed. Patients were compared to an asymptomatic control group (26 hips). All patients underwent pelvic CT scans to generate personalized CT-based 3D models and validated software for patient-specific impingement simulation (equidistant method). Results. Mean impingement-free flexion of patients with mixed-type FAI (110° (SD 8°)) and patients with pincer-type FAI (112° (SD 8°)) was significantly (p < 0.001) lower compared to the control group (125° (SD 13°)). The frequency of extra-articular subspine impingement was significantly (p < 0.001) increased in patients with pincer-type FAI (57%) compared to cam-type FAI (22%) in 125° flexion. Bony impingement in maximal flexion was located anterior-inferior at femoral four and five o’clock position in patients with cam-type FAI (63% (10 of 16 hips) and 37% (6 of 10 hips)), and did not involve the cam deformity. The cam deformity did not cause impingement in maximal flexion. Conclusion. Femoral impingement in maximal flexion was located anterior-inferior distal to the cam deformity. This differs to previous studies, a finding which could be important for FAI patients in order to avoid exacerbation of hip pain in deep flexion (e.g. during squats) and for hip arthroscopy (hip-preservation surgery) for planning of bone resection. Purpose: The surgical correction of FAI deformity is a well accepted treatment in patients presenting with hip pain with associated labral-chondral damage. The anterior approach with assisted hip arthroscopy provides access to the anterior head-neck junction with potentially quicker recovery for patients. The purpose of this study is to present the safety and efficacy of this approach in performing impingement surgery. Method: Forty-eight Hueter procedures were performed in 45 patients (13 males and 32 females). Mean age was 42.09 years (range, 21–65 years), and mean BMI was 24.31(range, 21–33). The scope was performed first to deal with intra-articular damage. All patients were diagnosed with CAM type FAI with labral pathology based on MRI arthrogram with an alpha angle > 50.5 degrees. Results: At a mean follow-up of 21.8 months (range 12–30 months), Harris Hip scores improved from 64.66 (range, 42.0–93.0) to 79.97 (range, 47.0–96.0). There were 5 re-operations at a mean time of 15.2 months (range, 4–22). One had a repeat hip scope for intra-articular adhesions, and another for recurrent traumatic tear of the labrum. Three cases with residual hip dysplasia had corrective surgery with a peri-acetabular osteotomy at an average of 16.67 months (range, 15–18 months). Conclusion: Overall, we have found this to be a reliable, safe and reproducible approach to the treatment of FAI. This is a day care procedure as compared to the classic open procedure. Uncorrected hip dysplasia in the presence of a CAM deformity is a risk factor for early failure

Aims. Research on hip biomechanics has analyzed femoroacetabular contact pressures and forces in distinct hip conditions, with different procedures, and used diverse loading and testing conditions. The aim of this scoping review was to identify and summarize the available evidence in the literature for hip contact pressures and force in cadaver and in vivo studies, and how joint loading, labral status, and femoral and acetabular morphology can affect these biomechanical parameters. Methods. We used the PRISMA extension for scoping reviews for this literature search in three databases. After screening, 16 studies were included for the final analysis. Results. The studies assessed different hip conditions like labrum status, the biomechanical effect of the cam, femoral version, acetabular coverage, and the effect of rim trimming. The testing and loading conditions were also quite diverse, and this disparity limits direct comparisons between the different researches. With normal anatomy the mean contact pressures ranged from 1.54 to 4.4 MPa, and the average peak contact pressures ranged from 2 to 9.3 MPa. Labral tear or resection showed an increase in contact pressures that diminished after repair or reconstruction of the labrum. Complete cam resection also decreased the contact pressure, and acetabular rim resection of 6 mm increased the contact pressure at the acetabular base. Conclusion. To date there is no standardized methodology to access hip contact biomechanics in hip arthroscopy, or with the preservation of the periarticular soft-tissues. A tendency towards improved biomechanics (lower contact pressures) was seen with labral repair and reconstruction techniques as well as with cam correction. Aims. The frequency of severe femoral retroversion is unclear in patients with femoroacetabular impingement (FAI). This study aimed to investigate mean femoral version (FV), the frequency of absolute femoral retroversion, and the combination of decreased FV and acetabular retroversion (AR) in symptomatic patients with FAI subtypes. Methods. A retrospective institutional review board-approved observational study was performed with 333 symptomatic patients (384 hips) with hip pain due to FAI evaluated for hip preservation surgery. Overall, 142 patients (165 hips) had cam-type FAI, while 118 patients (137 hips) had mixed-type FAI. The allocation to each subgroup was based on reference values calculated on anteroposterior radiographs. CT/MRI-based measurement of FV (Murphy method) and AV were retrospectively compared among five FAI subgroups. Frequency of decreased FV < 10°, severely decreased FV < 5°, and absolute femoral retroversion (FV < 0°) was analyzed. Results. A significantly (p < 0.001) lower mean FV was found in patients with cam-type FAI (15° (SD 10°)), and in patients with mixed-type FAI (17° (SD 11°)) compared to severe over-coverage (20° (SD 12°). Frequency of decreased FV < 10° was significantly (p < 0.001) higher in patients with cam-type FAI (28%, 46 hips) and in patients with over-coverage (29%, 11 hips) compared to severe over-coverage (12%, 5 hips). Absolute femoral retroversion (FV < 0°) was found in 13% (5 hips) of patients with over-coverage, 6% (10 hips) of patients with cam-type FAI, and 5% (7 hips) of patients with mixed-type FAI. The frequency of decreased FV< 10° combined with acetabular retroversion (AV < 10°) was 6% (8 hips) in patients with mixed-type FAI and 5% (20 hips) in all FAI patients. Of patients with over-coverage, 11% (4 hips) had decreased FV < 10° combined with acetabular retroversion (AV < 10°). Conclusion. Patients with cam-type FAI had a considerable proportion (28%) of decreased FV < 10° and 6% had absolute femoral retroversion (FV < 0°), even more for patients with pincer-type FAI due to over-coverage (29% and 13%). This could be important for patients evaluated for open hip preservation surgery or hip arthroscopy, and each patient requires careful personalized evaluation. Aims. Hip arthroscopy (HA) has become the treatment of choice for femoroacetabular impingement (FAI). However, less favourable outcomes following arthroscopic surgery are expected in patients with severe chondral lesions. The aim of this study was to assess the outcomes of HA in patients with FAI and associated chondral lesions, classified according to the Outerbridge system. Methods. A systematic search was performed on four databases. Studies which involved HA as the primary management of FAI and reported on chondral lesions as classified according to the Outerbridge classification were included. The study was registered on PROSPERO. Demographic data, patient-reported outcome measures (PROMs), complications, and rates of conversion to total hip arthroplasty (THA) were collected. Results. A total of 24 studies were included with a total of 3,198 patients (3,233 hips). Patients had significantly less improvement in PROMs if they had Outerbridge grade III and IV lesions (p = 0.012). Compared with microfracture, autologous matrix-induced chondrogenesis (AMIC) resulted in significantly reduced rates of conversion to THA (p = 0.042) and of revision arthroscopy (p = 0.038). Chondral repair procedures in these patients also did not significantly reduce the rates of conversion to THA (p = 0.931), or of revision arthroscopy (p = 0.218). However, compared with microfracture, AMIC significantly reduced the rates of conversion to THA (p = 0.001) and of revision arthroscopy (p = 0.011) in these patients. Those with Outerbridge grade III and IV lesions also had significantly increased rates of conversion to THA (p = 0.029) and of revision arthroscopy (p = 0.023) if they had associated lesions of the acetabulum and femoral head. Those who underwent labral debridement had a significantly increased rate of conversion to THA compared with those who underwent labral repair (p = 0.015). Conclusion. There is universal improvement in PROMs following HA in patients with FAI and associated chondral lesions. However, those with Outerbridge grade III and IV lesions had significantly less improvement in PROMs and a significantly increased rate of conversion to THA than those with Outerbridge grade I and II. Idiopathic calcium pyrophosphate deposition disease (pseudogout) has a variable presentation. Many joints are usually affected; single joint disease is uncommon. We present a case report of primary monoarticular pseudogout affecting the hip. The diagnosis was made on the appearance and analysis of specimens obtained at arthroscopy. Monoarticular pseudogout is rare, but should be considered in the differential diagnosis of any presentation of joint pain.

To assess the current literature on suture anchor placement for the purpose of identifying factors that lead to suture anchor perforation and techniques that reduce the likelihood of complications.

Three databases (PubMed, Ovid MEDLINE, EMBASE) were searched, and two reviewers independently screened the resulting literature. Methodological quality of all included papers was assessed using Methodological Index for Non-Randomized Studies criteria and the Cochrane Risk of Bias Assessment tool. Results are presented in a narrative summary fashion using descriptive statistics.

Fourteen studies were included in this review. Four case series (491 patients, 56.6% female, mean age 33.9 years), nine controlled cadaveric/laboratory studies (111 cadaveric hips and 12 sawbones, 42.2% female, mean age 60.0 years), and one randomized controlled trial (37 hips, 55.6% female, mean age 34.2 years) were included. Anterior cortical perforation by suture anchors led to pain and impingement of pelvic neurovascular structures. The anterior acetabular positions (three to four o'clock) had the thinnest bone, smallest rim angles, and highest incidence of articular perforation. Drilling angles from 10° to 20° measured off the coronal plane were acceptable. The mid-anterior (MA) and distal anterolateral (DALA) portals were used successfully, with some studies reporting difficulty placing anchors at anterior locations via the DALA portal. Small-diameter (< 1 .8-mm) suture anchors had a lower in vivo incidence of articular perforation with similar stability and pull-out strength in biomechanical studies.

Suture anchors at anterior acetabular rim positions (3–4 o'clock) should be inserted with caution. Large-diameter (>2.3-mm) suture anchors increase the likelihood of articular perforation without increasing labral stability. Inserting small-diameter (< 1 .8-mm) all-suture suture anchors (ASAs) from 10° to 20° using curved suture anchor drill guides, may increase safe insertion angles from all cutaneous portals. Direct arthroscopic visualization, use of fluoroscopy, distal-proximal insertion, and the use of nitinol wire can help prevent articular violation.

Aims. Abnormal femoral torsion (FT) is increasingly recognized as an additional cause for femoroacetabular impingement (FAI). It is unknown if in-toeing of the foot is a specific diagnostic sign for increased FT in patients with symptomatic FAI. The aims of this study were to determine: 1) the prevalence and diagnostic accuracy of in-toeing to detect increased FT; 2) if foot progression angle (FPA) and tibial torsion (TT) are different among patients with abnormal FT; and 3) if FPA correlates with FT. Patients and Methods. A retrospective, institutional review board (IRB)-approved, controlled study of 85 symptomatic patients (148 hips) with FAI or hip dysplasia was performed in the gait laboratory. All patients had a measurement of FT (pelvic CT scan), TT (CT scan), and FPA (optical motion capture system). We allocated all patients to three groups with decreased FT (< 10°, 37 hips), increased FT (> 25°, 61 hips), and normal FT (10° to 25°, 50 hips). Cluster analysis was performed. Results. We found a specificity of 99%, positive predictive value (PPV) of 93%, and sensitivity of 23% for in-toeing (FPA < 0°) to detect increased FT > 25°. Most of the hips with normal or decreased FT had no in-toeing (false-positive rate of 1%). Patients with increased FT had significantly (p < 0.001) more in-toeing than patients with decreased FT. The majority of the patients (77%) with increased FT walk with a normal foot position. The correlation between FPA and FT was significant (r = 0.404, p < 0.001). Five cluster groups were identified. Conclusion. In-toeing has a high specificity and high PPV to detect increased FT, but increased FT can be missed because of the low sensitivity and high false-negative rate. These results can be used for diagnosis of abnormal FT in patients with FAI or hip dysplasia undergoing hip arthroscopy or femoral derotation osteotomy. However, most of the patients with increased FT walk with a normal foot position. This can lead to underestimation or misdiagnosis of abnormal FT. For the hip, published literature reports approximately 50% successful outcome of operative arthroscopy in the presence of arthritis. These results are not very promising but some patients respond remarkably well. Careful patient selection is paramount to a potentially successful outcome. General parameters include: younger patients; mechanical joint symptoms; partial joint space preservation; adequate rotational motion; reasonable expectations; and failure of conservative treatment.

Arthroscopy has helped to understand the pathological process associated with various forms of osteoarthritis. We are now capable of recognizing these at earlier stages in the disease. However, it remains to be seen whether we can change the natural history of the disease process. Despite increasingly sophisticated technology with MRI, MRA, etc., radiographs remain the most important tool in assessing arthritic changes. Radiographs have traditionally been poor at detecting early degenerative disease, but with the information from arthroscopy, we are now more capable of recognizing the clinical importance associated with subtle radiographic findings.

Femoroacetabular impingement (FAI) causes pain and chondrolabral damage via mechanical overload during movement of the hip. It is caused by many different types of pathoanatomy, including the cam ‘bump’, decreased head–neck offset, acetabular retroversion, global acetabular overcoverage, prominent anterior–inferior iliac spine, slipped capital femoral epiphysis, and the sequelae of childhood Perthes’ disease. Both evolutionary and developmental factors may cause FAI. Prevalence studies show that anatomic variations that cause FAI are common in the asymptomatic population. Young athletes may be predisposed to FAI because of the stress on the physis during development. Other factors, including the soft tissues, may also influence symptoms and chondrolabral damage. FAI and the resultant chondrolabral pathology are often treated arthroscopically. Although the results are favourable, morphologies can be complex, patient expectations are high and the surgery is challenging. The long-term outcomes of hip arthroscopy are still forthcoming and it is unknown if treatment of FAI will prevent arthrosis

Young adults with hip pain secondary to femoroacetabular impingement (FAI) are rapidly being recognised as an important cohort of orthopaedic patients. Interest in FAI has intensified over the last decade since its recognition as a precursor to arthritis of the hip and the number of publications related to the topic has increased exponentially in the last decade. Although not all patients with abnormal hip morphology develop osteoarthritis (OA), those with FAI-related joint damage rapidly develop premature OA. There are no explicit diagnostic criteria or definitive indications for surgical intervention in FAI. Surgery for symptomatic FAI appears to be most effective in younger individuals who have not yet developed irreversible OA. The difficulty in predicting prognosis in FAI means that avoiding unnecessary surgery in asymptomatic individuals, while undertaking intervention in those that are likely to develop premature OA poses a considerable dilemma. FAI treatment in the past has focused on open procedures that carry a potential risk of complications. Recent developments in hip arthroscopy have facilitated a minimally invasive approach to the management of FAI with few complications in expert hands. Acetabular labral preservation and repair appears to provide superior results when compared with debridement alone. Arthroscopic correction of structural abnormalities is increasingly becoming the standard treatment for FAI, however there is a paucity of high-level evidence comparing open and arthroscopic techniques in patients with similar FAI morphology and degree of associated articular cartilage damage. Further research is needed to develop an understanding of the natural course of FAI, the definitive indications for surgery and the long-term outcomes. Aims

Pelvic tilt is believed to affect the symptomology of osteoarthritis (OA) of the hip by alterations in joint movement, dysplasia of the hip by modification of acetabular cover, and femoroacetabular impingement by influencing the impingement-free range of motion. While the apparent role of pelvic tilt in hip pathology has been reported, the exact effects of many forms of treatment on pelvic tilt are unknown. The primary aim of this study was to investigate the effects of surgery on pelvic tilt in these three groups of patients.

Methods

The demographic, radiological, and outcome data for all patients operated on by the senior author between October 2016 and January 2020 were identified from a prospective registry, and all those who underwent surgery with a primary diagnosis of OA, dysplasia, or femoroacetabular impingement were considered for inclusion. Pelvic tilt was assessed on anteroposterior (AP) standing radiographs using the pre- and postoperative pubic symphysis to sacroiliac joint (PS-SI) distance, and the outcomes were assessed with the Hip Outcome Score (HOS), International Hip Outcome Tool (iHOT-12), and Harris Hip Score (HHS).

Aims

The prevalence of combined abnormalities of femoral torsion (FT) and tibial torsion (TT) is unknown in patients with femoroacetabular impingement (FAI) and hip dysplasia. This study aimed to determine the prevalence of combined abnormalities of FT and TT, and which subgroups are associated with combined abnormalities of FT and TT.

Methods

We retrospectively evaluated symptomatic patients with FAI or hip dysplasia with CT scans performed between September 2011 and September 2016. A total of 261 hips (174 patients) had a measurement of FT and TT. Their mean age was 31 years (SD 9), and 63% were female (165 hips). Patients were compared to an asymptomatic control group (48 hips, 27 patients) who had CT scans including femur and tibia available for analysis, which had been acquired for nonorthopaedic reasons. Comparisons were conducted using analysis of variance with Bonferroni correction.


The repair of chondral lesions associated with femoroacetabular impingement requires specific treatment in addition to that of the impingement. In this single-centre retrospective analysis of a consecutive series of patients we compared treatment with microfracture (MFx) with a technique of enhanced microfracture autologous matrix-induced chondrogenesis (AMIC).

Acetabular grade III and IV chondral lesions measuring between 2 cm2 and 8 cm2 in 147 patients were treated by MFx in 77 and AMIC in 70. The outcome was assessed using the modified Harris hip score at six months and one, two, three, four and five years post-operatively. The outcome in both groups was significantly improved at six months and one year post-operatively. During the subsequent four years the outcome in the MFx group slowly deteriorated, whereas that in the AMIC group remained stable. Six patients in the MFx group subsequently required total hip arthroplasty, compared with none in the AMIC group

We conclude that the short-term clinical outcome improves in patients with acetabular chondral damage following both MFx and AMIC. However, the AMIC group had better and more durable improvement, particularly in patients with large (≥ 4 cm2) lesions.

The aim of this study was to determine the accuracy of registration and the precision of the resection volume in navigated hip arthroscopy for cam-type femoroacetabular impingement, using imageless and image-based registration. A virtual cam lesion was defined in 12 paired cadaver hips and randomly assigned to either imageless or image-based (three-dimensional (3D) fluoroscopy) navigated arthroscopic head–neck osteochondroplasty. The accuracy of patient–image registration for both protocols was evaluated and post-operative imaging was performed to evaluate the accuracy of the surgical resection. We found that the estimated accuracy of imageless registration in the arthroscopic setting was poor, with a mean error of 5.6 mm (standard deviation (sd) 4.08; 95% confidence interval (CI) 4.14 to 7.19). Because of the significant mismatch between the actual position of the probe during surgery and the position of that probe as displayed on the navigation platform screen, navigated femoral osteochondroplasty was physically impossible. The estimated accuracy of image-based registration by means of 3D fluoroscopy had a mean error of 0.8 mm (sd 0.51; 95% CI 0.56 to 0.94). In terms of the volume of bony resection, a mean of 17% (sd 11; -6% to 28%) more bone was resected than with the virtual plan (p = 0.02). The resection was a mean of 1 mm deeper (sd 0.7; -0.3 to 1.6) larger than on the original virtual plan (p = 0.02).

In conclusion, given the limited femoral surface that can be reached and digitised during arthroscopy of the hip, imageless registration is inaccurate and does not allow for reliable surgical navigation. However, image-based registration does acceptably allow for guided femoral osteochondroplasty in the arthroscopic management of femoroacetabular impingement.

The benefit of arthroscopy of the hip in the treatment of femoroacetabular impingement (FAI) in terms of quality of life (QoL) has not been reported. We prospectively collected data on 612 patients (257 women (42%) and 355 men (58%)) with a mean age at the time of surgery of 36.7 years (14 to 75) who underwent arthroscopy of the hip for FAI under the care of a single surgeon. The minimum follow-up was one year (mean 3.2 years (1 to 7)). The responses to the modified Harris hip score were translated using the Rosser Index Matrix in order to provide a QoL score. The mean QoL score increased from 0.946 (-1.486 to 0.995) to 0.974 (0.7 to 1) at one year after surgery (p < 0.001). The mean QoL score in men was significantly higher than in women, both before and one year after surgery (both p < 0.001). However, the mean change in the QoL score was not statistically different between men and women (0.02 (-0.21 to 0.27) and 0.04 (-0.16 to 0.87), respectively; p = 0.12). Linear regression analysis revealed that the significant predictors of a change in QoL score were pre-operative QoL score (p < 0.001) and gender (p = 0.04). The lower the pre-operative score, the higher the gain in QoL post-operatively (ρ = -0.66; p < 0.001). One year after surgery the QoL scores in the 612 patients had improved in 469 (76.6%), remained unchanged in 88 (14.4%) and had deteriorated in 55 (9.0%).

We studied 16 hips (eight cadaver specimens) using arthrography, arthroscopy and anatomical dissection, under incremental traction of up to a maximum of 64 kg, to determine the relationship of the portals to nearby neurovascular structures.

The distance of each arthroscopic portal (anterior, anterolateral, and posterolateral) to the associated neurovascular structures was measured after the application of 23 kg of traction.

Traction of up to 64 kg on the lower limb failed to produce evidence of labral or capsular injury. Furthermore, traction of 23 kg resulted in little change in the position of adjacent neurovascular structures relative to the standard arthroscopic portals.

We have reviewed the current literature to compare the results of surgery aimed to repair or debride a damaged acetabular labrum. We identified 28 studies to be included in the review containing a total of 1631 hips in 1609 patients. Of these studies 12 reported a mean rate of good results of 82% (from 67% to 100%) for labral debridement. Of the 16 studies that reported a combination of debridement and re-attachment, five reported a comparative outcome for the two methods, four reported better results with re-attachment and one study did not find any significant difference in outcomes. Due to the heterogeneity of the studies it was not possible to perform a meta-analysis or draw accurate conclusions. Confounding factors in the studies include selection bias, use of historical controls and high rates of loss of follow-up.

It seems logical to repair an unstable tear in a good quality labrum with good potential to heal in order potentially to preserve its physiological function. A degenerative labrum on the other hand may be the source of discomfort and its preservation may result in persistent pain and the added risk of failure of re-attachment. The results of the present study do not support routine refixation for all labral tears.

Although good to excellent short-term results of arthroscopic partial limbectomy of the hip have been reported, there is little information on the long-term outcome of the procedure. We report a case with a 16-year follow-up.

A 34-year old woman presented with a three-month history of severe pain and limitation of movement of the hip. Plain radiographs and arthrography showed no abnormality. MRI showed an effusion and a deformity of the anterosuperior part of the labrum. This was confirmed as an inverted labral tear that was debrided arthroscopically.

At 16 years post-operatively, she remained symptom-free. Plain radiographs showed no abnormality and MRI suggested that the sealing effect of the scar tissue prevented articular damage and degeneration.

We present our early experience of arthroscopic reduction of the dislocated hip in very young infants with developmental dysplasia of the hip (DDH).

Eight dislocated hips, which had failed attempts at closed reduction, were treated by arthroscopy of the hip in five children with a mean age of 5.8 months (4 to 7). A two-portal technique was used, with a medial sub-adductor portal for a 2.7 mm cannulated system with a 70° arthroscope and an anterolateral portal for the instruments. Following evaluation of the key intra-articular structures, the hypertrophic ligamentum teres and acetabular pulvinar were resected, and a limited release of the capsule was performed prior to reduction of the hip. All hips were reduced by a single arthroscopic procedure, the reduction being confirmed on MRI scan. None of the hips had an inverted labrum. The greatest obstacle to reduction was a constriction of the capsule. At a mean follow-up of 13.2 months (9 to 24), all eight hips remained stable. Three developed avascular necrosis. The mean acetabular index decreased from 35.5° (30° to 40°) pre-operatively to 23.3° (17° to 28°).

This study demonstrates that arthroscopic reduction is feasible using two standardised portals. Longer follow-up studies are necessary to evaluate the functional results.

Arthroscopic hip surgery is increasingly common in Australia. Hip arthroscopy is indicated for a range of diagnostic and therapeutic purposes, including labral tears, capsular laxity and femoral-acetabular impingement (FAI). Despite this, previous cohort studies aiming to characterise hip pathology seen on arthroscopic examination are mostly limited to patients with known diagnoses of FAI. Therefore, little is known of the native articular wear patterns encountered in other disease states. Therefore, we aimed to define common osteochondral wear patterns for a cohort of patients managed via hip arthroscopy. We retrospectively analysed intraoperative data for 1127 patients managed via hip arthroscopy between 2008 and 2013, for either therapeutic or diagnostic purposes. Intraoperative data was categorized by location (A-E as defined by Fontana et al. 2016) and chondral damage (0-4 scale as defined by Beck et al. 2005) with respect to both acetabulum and femoral head. Data for 1127 patients were included. Location of acetabular chondral pathology was variable with locations C. 1. and D. 1. representing the most common regions of damage. Labral tears predominated in locations C and D. Femoral chondral pathology was evenly distributed. The degree of femoral chondral injury was predominantly grade 1, whilst acetabular wear was evenly distributed. Large proportions of wear were observed at the peripheral superior and anterior regions of the lunate surface of the acetabulum in keeping with prior works. However, we observed higher rates of central wear and lower rates of grade 4 acetabular damage extending into superior/posterior zones, in our cohort. Our work characterises common articular wear patterns encountered at the time of hip arthroscopy. Further inquiry into the natural history of osteochondral lesions is needed to better understand and manage these conditions

Since its creation, labral repair has become the preferred method among surgeons for the arthroscopic treatment of acetabular labral tears resulting in pain and dysfunction for patients. Labral reconstruction is performed mainly in revision hip arthroscopy but can be used in the primary setting when the labrum cannot be repaired or is calcified. The purpose of this study was to compare the survival between primary labral repair and labral reconstruction with survival defined as no further surgery (revision or total hip replacement). Patients who underwent labral repair or reconstruction between January 2005 and December 2018 in the primary setting were included in the study. Patients were included if they had primary hip arthroscopy with the senior author for femoroacetabular impingement (FAI), involving either labral reconstruction or labral repair, and were within the ages of 18 and 65 at the time of surgery. Exclusion criteria included confounding injuries (Leggs Calves Perthes, avascular necrosis, femoral head fracture, etc.), history of unilateral or bilateral hip surgeries, or Tönnis grades of 2 or 3 at the time of surgery. Labral repairs were performed when adequate tissue was available for repair and labral reconstruction was performed when tissue was absent, ossified or torn beyond repair. A total of 501 labral repairs and 114 labral reconstructions performed in the primary setting were included in the study. Labral reconstruction patients were older (37±10) compared to labral repair (34±11).(p=0.021). Second surgeries were required in 19/114 (17%) of labral reconstruction and 40/501(8%) [odds ratio: 2.3; 95% CI 1.3 to 4.2] (p=0.008). Revision hip arthroscopy were required in 6/114(5%) labral reconstructions and 33/501(6.5%) labral repair (p=0.496). Total hip replacement was required in 13/114 labral reconstructions and 7/501 labral repairs [odds ratio:9.1 95%CI 3.5 to 23] (p=< 0.01). The mean survival for the labral repair group was 10.2 years (95%CI:10 to 10.5) and 11.9 years (98%CI:10.9 to 12.8) in the labral reconstruction group. Conversion to total hip was required more often following primary labral reconstruction. Revision hip arthroscopy rates were similar between groups as was the mean survival, with both over 10 years. Similar survival was seen in labral repair and reconstruction when strict patient selection criteria are followed


MCID and PASS are thresholds driven from PROMS to reflect clinical effectiveness. Statistical significance can be derived from a change in PROMS, whereas MCID and PASS reflect clinical significance. Its role has been increasingly used in the world of young adult hip surgery with several publications determining the thresholds for Femoro-acetabular impingement FAI. To our knowledge MCID and PASS for patient undergoing PAO for dysplasia has not been reported. 593 PAOs between 1/2013 and 7/2023 were extracted from the Northumbria Hip Preservation Registry. Patients with available PROMS at 1year and/or 2years were included. PAOs for retroversion, residual Perthes and those combined with FO were excluded. MCID was calculated using the distribution method 0.5SD of baseline score(1). PASS was calculated using anchor method, ROC analysis performed, and value picked maximizing Youden index. A Logistic Regression analysis was performed to determine which independent variables correlated with achieving MCID and PASS. The MCID threshold for iHOt12 was 8.6 with 83.4 and 86.3 % of patients achieved it at 1 and 2 years respectively. The PASS score at 1 and 2 year follow up was 43 and 44 respectively, with 72.6 and 75.2% achieving it at 1 and 2 year postop. At 2 years a Higher preop iHOT 12 was associated with not achieving MCID and PASS (p<0.05). Preop acetabular version was negatively correlated with achieving MCID and previous hip arthroscopy was negatively correlated with PASS. The % of patients achieving MCID and PASS mimics that of FAI surgery (2). The negative correlation with preop iHOT12 reaffirms the importance of patient selection. The negative correlation of hip arthroscopy highlights the importance of having a high index of suspicion for dysplasia prior to hip arthroscopy and poorer outcomes of patients with mixed CAM and dysplasia pathology

Removal of bullets retained within joints is indicated to prevent mechanical blockade, 3rd body wear and resultant arthritis, plus lead arthropathy and systemic lead poisoning. The literature is sparse on this subject, with mostly sporadic case reports utilizing hip arthroscopy. We report on the largest series of removal of bullets from the hip joints using open surgical. We reviewed prospectively collected data of patients who presented to a single institution with civilian gunshot injuries that breached the hip joint between 01 January 2009 and 31 December 2022. We included all cases where the bullet was retained within the hip joint area. Exclusion criteria: cases where the hip joint was not breached, bullets were not retained around the hip area or cases with isolated acetabulum involvement. One hundred and eighteen (118) patients were identified. One patient was excluded as the bullet embedded in the femur neck was sustained 10 years earlier. Of the remaining 117 patients, 70 had retained bullets around the hip joint. In 44 patients we undertook bullet removal using the followingsurgical hip dislocation (n = 18), hip arthrotomy (n = 18), removal at site of fracture fixation/replacement (n = 2), posterior wall osteotomy (n = 1), direct removal without capsulotomy (tractotomy) (n = 5). In 26 patients we did not remove bullets for the following reasons: final location was extra-capsular embedded in the soft tissues (n=17), clinical decision to not remove (n=4), patients’ clinical condition did not allow for further surgery (n= 4) and patient refusal (n=1). No patients underwent hip arthroscopy. With adequate pre-operative imaging and surgical planning, safe surgical removal of retained bullets in the hip joint can be achieved without the use of hip arthroscopy; using the traditional open surgical approaches of arthrotomy, tractotomy and surgical hip dislocation

It has been reported that 60-85% of patients who undergo PAO have concomitant intraarticular pathology that cannot be addressed with PAO alone. Currently, there are limited diagnostic tools to determine which patients would benefit from hip arthroscopy at the time of PAO to address intra-articular pathology. This study aims to see if preoperative PROMs scores measured by IHOT-33 scores have predictive value in whether intra-articular pathology is addressed during PAO + scope. The secondary aim is to see how often surgeons at high-volume hip preservation centers address intra-articular pathology if a scope is performed during the same anesthesia event. A randomized, prospective Multicenter trial was performed on patients who underwent PAO and hip arthroscopy to treat hip dysplasia from 2019 to 2020. Preoperative PROMs and intraoperative findings and procedures were recorded and analyzed. A total of 75 patients, 84% Female, and 16% male, with an average age of 27 years old, were included in the study. Patients were randomized to have PAO alone 34 patients vs. PAO + arthroscopy 41 patients during the same anesthesia event. The procedures performed, including types of labral procedures and chondroplasty procedures, were recorded. Additionally, a two-sided student T-test was used to evaluate the difference in means of preoperative IHOT score among patients for whom a labral procedure was performed versus no labral procedure. A total of 82% of patients had an intra-articular procedure performed at the time of hip arthroscopy. 68% of patients who had PAO + arthroscopy had a labral procedure performed. The most common labral procedure was a labral refixation which was performed in 78% of patients who had a labral procedure performed. Femoral head-neck junction chondroplasty was performed in 51% of patients who had an intra-articular procedure performed. The mean IHOT score was 29.3 in patients who had a labral procedure performed and 33.63 in those who did not have a labral procedure performed P- value=0.24. Our findings demonstrate preoperative IHOT-33 scores were not predictive in determining whether intra-articular labral pathology was addressed at the time of surgery. Additionally, we found that if labral pathology was addressed, labral refixation was the most common repair performed. This study also provides valuable information on what procedures high-volume hip preservation centers are performing when performing PAO + arthroscopy

The postoperative pain after hip arthroscopy remains a major challenge in the immediate postoperative period. Adequate postoperative analgesia has been associated with increased patient satisfaction and decreased consumption of opioids. We evaluated the efficacy of pericapsular nerve group block (PENG) versus fascia iliaca block (FIB) in reducing post-operative pain and analgesic consumption within the first 24 hours following arthroscopic management of femoroacetabular impingement (FAI). Thirty-nine patients (17 females and 25 males, ages 18–42 years, mean ± SD (27.9 ± 6.2), and mean BMI of 25.13±5.08 kg/m2 were scheduled for primary arthroscopic management of FAI. Included patients were randomized into two groups according to the block used in each. Group (A) 19 patients were included and had FIB and group (B) 20 patients were included and received PENG block. The efficacy of both techniques was clinically and statistically valuated using VAS score and quadriceps muscle power. There was a statically significance difference in the mean at rest between the two groups at all measured time points following surgery (6, 12, 18 and 24 h). Also, in dynamic pain scores (with hip flexion) scores were statistically significant at 24 hours post-operative (P = .001). Total opioid consumption in the first 24 hours postoperative was lower in the PENG group with significant difference of mean 16.5 ±9.9 mg for PENG group versus 27.5±9.6 mg for FIB group (P < .005). Five patients (26.31%) in FIB group had weaker quadriceps muscle power while none in PENG group patients had quadriceps weakness. PENG block might be considered as an ideal regional anesthesia modality for hip arthroscopy. As an alternative to more conventional regional nerve blocks such as a fascia iliaca block. PENG block is easily performed in the preoperative setting, and appears to spare motor function while providing a prolonged sensory pain relief

Young patients undergoing THA or hip used. HOS and iHOT33 have demonstrated to be useful in hip preservation surgery but never used in THA. The aim of the study was to evaluate long-term clinical outcomes with HOS and iHOT33 in hip arthroscopy and THA. We conducted a retrospective study with prospective data collection of 118 consecutive young patients (<65 y.o.) between 2008 to 2012 who underwent hip arthroscopy or THA. The mean follow-up was 12.05 years. All surgeries were done by the same senior surgeon. PROMs used were iHOT-33 and HOS preoperatively, at one year and 10 years. SPSS Statistics Grad Pack 28.0 software was used for statistical analysis. Arthroscopy Group (57 patients): Mean age 35.36 years. 55.93% were male. iHOT33 mean differential improvement was 24.43 at 1 year, 70,17% exceed MCID and 49,1% SCB. HOS mean differential improvement was 16.26 at one year. 54,38 % patients exceed MCID and 49,36% SCB. At 10 years, iHOT33 mean differential improvement was 14,36 and 12.56 for HOS. Regarding complications, 3 patients underwent THA (5.26%) and 10 (17.54%) continued with groin pain. THA Group (61 patients): Mean age 52,54 years. 55.73% were male. Cementless THA was used in all patients with ceramic on ceramic used in 77%. iHOT33 mean differential improvement was 41,57 at 1 year. 95,08% patients exceed MCID and 85,25% SCB. HOS mean differential improvement was 16.57 points at one year. 85,25% patients exceed MCID and 81,97% SCB. At 10 years, iHOT33 mean differential improvement was 20,15 and 14.12 for HOS. Regarding complications, 1 patient underwent DAIR for infection (1.64%) and 1 dislocation with close reduction (1.64%). iHOT33 or HOS scores should be considered to be used in young active patients after hip preserving surgery or THA, with more predictable results at long-term in THA group
