This study aimed to verify the hypothesis that an antibiotic loaded hydrogel, defensive antimicrobial coating (DAC), reduces overall complication and infection rates when used for high-risk primary and revision total hip arthroplasty (THA).

This was a retrospective study matched cohort study of 238 patients, treated with cementless implants with and without DAC. A sub-group analysis of patients undergoing 2nd stage revision THA for prosthetic joint infection (PJI) was also conducted. Re-infection rates within 2 years, complications necessitating surgical intervention and radiographic analysis for aseptic loosening was assessed.

The mean age was 68.3±11.5 years, with 39 (32.8%) Macpherson class A, 64 (53.8%) class B and 16 (13.4%) class C patients. 4 (3.4%) patients in the DAC group developed complications including 1 PJI and 1 delayed wound healing, while 13 (10.9%) patients in the control group developed complications including 5 PJIs and 3 delayed wound healing (p=0.032). PJI rates (p=0.136) and delayed wound healing rates (p=0.337) were not statistically significant. For 2nd stage revision THA for PJI there were 86 patients in the DAC group and 45 in the control group. 1 (1.2%) patient in the DAC group developed complications with no recurrences of infection or delayed wound healing, while 10 (22.2%) patients in the control group developed complications including 4 recurrent PJI and 1 delayed wound healing (p=0.003). Recurrent PJI rates were statistically significant (p=0.005) while delayed wound healing rates were not (p=0.165). Patients treated with DAC also had lower rates of aseptic loosening (0% vs 6.7%; p=0.015).

Antibiotic impregnated hydrogel coatings on cementless implants showed decreased complication rates after complex primary or revision THA. In 2nd stage revision THA for PJI, it was associated with reduced risk of re-infection and aseptic loosening.


In severe cases of total knee & hip arthroplasty, where off-the-shelf implants are not suitable (i.e., in cases with extended bone defects or periprosthetic fractures), 3D-printed custom-made knee & hip revision implants out of titanium or cobalt-chromium alloy represent one of the few remaining clinical treatment options. Design verification and validation of such custom-made implants is very challenging. Therefore, a methodology was developed to support surgeons and engineers in their decision on whether a developed design is suitable for the specific case. A novel method for the pre-clinical testing of 3D-printed custom-made knee implants has been established, which relies on the biomechanical test and finite element analysis (FEA) of a comparable clinically established reference implant. The method comprises different steps, such as identification of the main potential failure mechanism, reproduction of the biomechanical test of the reference implant via FEA, identification of the maximum value of the corresponding FEA quantity of interest at the required load level, definition of this value as the acceptance criterion for the FEA of the custom-made implant, reproduction of the biomechanical test with the custom-made implant via FEA, decision making for realization or re-design based on the acceptance criterion is fulfilled or not. Exemplary cases of custom-made knee & hip implants were evaluated with this new methodology. The FEA acceptance criterion derived from the reference implants was fulfilled in both custom-made implants and subsequent biomechanical tests verified the FEA results. The suggested method allows a quantitative evaluation of the biomechanical properties of custom-made knee & hip implant without performing physical bench testing. This represents an important contribution to achieve a sustainable patient treatment in complex revision total knee & hip arthroplasty with custom-made 3D printed implants in a safe and timely manner.

Aims

The aim of this study was to assess the incidence the microbiological spectrum and clinical outcome of hip and knee revision arthroplasties with unexpected-positive-intraoperative-cultures (UPIC) at a single center with minimum follow up of 2 years.

Methods

We retrospectively analyzed our prospectively maintained institutional arthroplasty registry. Between 2011 and 2020 we performed presumably aseptic rTHA (n=939) and rTKA (n= 1,058). Clinical outcome, re-revision rates and causes as well as the microbiological spectrum were evaluated.

Periprosthetic femoral fractures are increasing in incidence, and typically occur in frail elderly patients. They are similar to pathological fractures in many ways. The aims of treatment are the same, including 'getting it right first time' with a single operation, which allows immediate unrestricted weightbearing, with a low risk of complications, and one that avoids the creation of stress risers locally that may predispose to further peri-implant fracture. The surgical approach to these fractures, the associated soft-tissue handling, and exposure of the fracture are key elements in minimizing the high rate of complications. This annotation describes the approaches to the femur that can be used to facilitate the surgical management of peri- and interprosthetic fractures of the femur at all levels using either modern methods of fixation or revision arthroplasty.

Introduction

Dual mobility (DM) total hip arthroplasty (THA) prostheses are designed to increase stability. In the setting of primary and revision THA, DM THA are used most frequently for dysplasia and instability diagnoses, respectively. As the use of DM THA continues to increase, with 8,031 cases logged in the American Joint Replacement Registry from 2012–2018, characterizing in vivo damage and clinical failure modes are important to report.

Methods

Under IRB-approved implant retrieval protocol, 43 DM THA systems from 41 patients were included. Each DM THA component was macroscopically examined for standard damage modes. Clinically-relevant data, including patient demographics and surgical elements, were collected from medical records. Fretting and corrosion damage grading is planned, according to the Goldberg et al. classification system.

The unparalleled events of the year 2020 continue to evolve and challenge the worldwide community on a daily basis. The COVID-19 pandemic has had a major impact on all aspects of our lives, and has caused major morbidity and mortality around the globe. The impact of COVID-19 on the practice of orthopedic surgery has been substantial with practice shutdowns, elective surgery restrictions, heightened utilization of telemedicine platforms and implementation of precautionary measures for in-person clinic visits. During this transition period the scholarly and educational pursuits of academic surgeons have been de-emphasized as the more immediate demands of clinical practice survivorship have been the priority. This unavoidable focus on clinical practice has heightened the importance of orthopedic subspecialty societies in maintaining an appropriate level of attention on research and educational activities. Under the outstanding presidential leadership of Robert Barrack, MD, The Hip Society adapted to the profound challenges of 2020, and maintained strong leadership in the realms of education and research. The recent 2020 summer meeting of the Hip Society was a testimonial to the resilience and dedication of the Society members to ongoing innovation in research and education. Due to travel and social distancing restrictions the 2020 summer meeting was transitioned from an in-person to a virtual meeting format. Dr Barrack and Program Chair Dr John Clohisy assisted with oversight of the meeting, while Olga Foley and Cynthia Garcia ensured the success of the meeting with remarkable planning and organization. These collaborative efforts resulted in an organized, well-attended, high level scientific meeting with engaging discussion and a remarkable virtual conference environment. The Bone & Joint Journal is very pleased to partner with The Hip Society to publish the proceedings of this very unique virtual meeting. The Hip Society is based in the United States and membership is granted to select individuals for leadership accomplishments in education and research related to hip disease. The Society is focused on the mission of advancing the knowledge and treatment of hip disorders to improve the lives of patients. The vision of the Hip Society is to lead in the discovery and dissemination of knowledge related to disorders of the hip. The annual closed meeting is one of the most important events of the society as this gathering highlights timely, controversial and novel research contributions from the membership. The top research papers from The Hip Society meeting will be published and made available to the wider orthopedic community in The Bone & Joint Journal. This partnership with The Bone & Joint Journal enhances the mission and vision of The Hip Society by international dissemination of the meeting proceedings. Given the far-reaching circulation of The Bone & Joint Journal the highest quality work is available to an expanding body of surgeons, associated healthcare providers and patients. Ultimately, this facilitates the overarching Hip Society goal of improving the lives of our patients. The 2020 virtual Hip Society meeting was characterized by outstanding member attendance, high quality paper presentations and robust discussion sessions. The meeting was held over two days and encompassed 58 open paper presentations divided into ten sessions with moderated discussions after each session. All papers will be presented in this issue in abstract form, while selected full papers passing our rigorous peer review process will be available online and in The Bone & Joint Journal in a dedicated supplement in 2021. The first session of the meeting focused on issues related to complex primary THA and osteonecrosis of the femoral head. Dr Gross presented on the conversion of hip fusion to THA in 28 patents at a mean 7 years. He reported a high clinical success rate, yet complications of heterotopic ossification and neurologic injury were relatively common. Consideration of heterotopic ossification prophylaxis and the selective use of a constrained liner were recommended. Dr Pagnano summarized the use of various contemporary porous acetabular components in 38 hips in the setting of prior pelvic radiation. The mean follow-up was 5 years and 10 year survivorship was 100% with all implants radiographically fixed. Dr Bolognesi's study demonstrated that THA in solid organ transplant patients is associated with higher risk for facility placement, transfusions and readmissions. This patient population also has increased mortality risk (4.3% risk at 1 year) especially lung transplant patients. The second group of papers focused on femoral head osteonecrosis. Dr Iorio presented single center data demonstrating that CT scan was a useful adjunct for diagnosis in the staging work-up for cancer, yet was not useful for ARCO staging and treatment decision-making. On the basic science side, Dr Goodman utilized a rabbit model of steroid-induced femoral head osteonecrosis to determine that immunomodulation with IL-4 has the potential to improve bone healing after core decompression. The session was concluded by Dr Nelson's study of ceramic-on-ceramic THA in 108 osteonecrosis patients. The median 12 year results were outstanding with marked increases in PROs, maintenance of high activity levels, and a 3.7% revision rate. In the second session attention was directed to THA instability and spinopelvic mobility. Dr Sierra presented a machine learning algorithm for THA dislocation risk. Two modifiable variables (anterior/lateral approach, elevated liner) were most influential in minimizing dislocation risk. Dr Taunton's study demonstrated a deep learning artificial intelligence model derived from postoperative radiographs to predict THA dislocation risk. High sensitivity and negative predictive value suggest that this model may be helpful in assessing postoperative dislocation risk. In reviewing a large single-center, multiple surgeon cohort of 2,831 DAA procedures, Dr Moskal noted a very low dislocation rate (0.45%) at minimum 2 years. Importantly, spinopelvic pathology or prior spinal instrumentation was not associated with an increased dislocation risk (0.30%). Dr Huo and colleagues analyzed pelvic tilt during functional gait in patients with acetabular dysplasia. They detected variable pelvic tilt on different surfaces with the data suggesting that patients with more anterior pelvic tilt while standing tend to have greater compensatory posterior pelvic tilt during gait. Dr Lamontagne reported on the sagittal and axial spinomobility in patients with hip OA, and highlighted reductions in pelvic tilt, pelvic-femoral-angle, lumbar lordosis and seated maximal trunk rotation when compared to controls. Dr Dennis showed that differences in spinopelvic mobility may explain the variable accuracy of acetabular version measurements on the cross-table lateral radiographs. Dr Gwo-Chin presented on a comprehensive functional analysis of 1,592 patients undergoing THA and observed that spinopelvic abnormalities are not infrequent (14%) in THA patients. Consistent with these findings Dr Murphy and collaborators identified a low prevalence of previous spinal instrumentation (1.5%), yet a high prevalence of spine stiffness (27.6%) in 149 patients undergoing THA. Session three highlighted various aspects of treating hip disease in young patients. Dr Peters investigated the need for subsequent hip arthroscopy in 272 patients treated with an isolated PAO. Only 4.8% of these patients required subsequent arthroscopy calling into question the routine use of combined arthroscopy and PAO. Three papers addressed questions related to THA in young patients. Dr Berend's study of 2532 hips demonstrated that high activity level was not associated with an increased risk of midterm aseptic or all cause failure. Dr Nunley presented on 43 young patients with an average age of 52 years treated with a cementless stem and modular dual mobility articulation. Stress shielding was minimal and no concerning metal ion release detected. Dr Garvin summarized minimum 15 year data of THA with highly cross-linked polyethylene in patient less than 50 years. These hips performed exceptionally well with no mechanical loosening or radiographic osteolysis. Dr Engh examined 10 year results of the Birmingham Hip Resurfacing implant and reported a 92.9 % overall survivorship, with males less than 55 years achieving a 98.3% survivorship. The session was concluded by long-term data on the Conserve Plus hip resurfacing arthroplasty. Dr Amstutz presented an impressive dataset depicting an 83.1% 20 year survivorship for this early resurfacing cohort. Direct anterior approach total hip arthroplasty was the focus of session four. Dr Meneghini reported on the anesthesia and surgical times of direct anterior and posterior approaches from a large healthcare system database. These data suggested longer OR and surgical times for the DAA both in the inpatient and ASC environments. Dr Clohisy introduced the technique and early outcomes of lateral decubitus position DAA. In a learning curve experience of 257 hips. 96% of acetabular components were in the Lewinneck safe zone, the aseptic revision rate was 0.9% and there were no dislocations. Dr Beaule analyzed femoral stem cement mantle with the DAA and posterior approaches by comparing two matched cohorts. Stem alignment and cement mantle quality were equivalent with both approaches. Similarly, Dr Emerson demonstrated technical feasibility and fewer cemented femoral stem failures when compared to cementless stems in a series of 360 DAAs THAs. The final paper of the session presented by Dr Hamilton examined the impact of surgical approach on dislocation after isolated head and liner exchange. Neither the posterior nor the anterior approach was superior in reducing the dislocation rate for these high dislocation risk procedures. The fifth session explored contemporary topics related to anesthesia and pain management. Dr Byrd opened the session with a comparative study evaluating general versus spinal anesthesia for hip arthroscopy. This preliminary study was provoked by the desire to minimize aerosolized exposure early in the COVID-19 pandemic by transitioning to spinal anesthesia. Both anesthetic methods were effective. Dr Austin presented a randomized, double-blind controlled trial comparing spinal anesthetic with mepivacaine, hyperbaric bupivacaine and isobaric bupivacaine. Mepivacaine patients ambulated earlier and were more likely to be discharged the same day. Dr Mont provided a very timely study on the effects of “cannabis use disorder” and THA outcomes. This administrative database study of 44,154 patients revealed this disorder to be associated with longer hospital stays, increased complications rates and higher costs. Dr Bedair investigated whether a highly porous acetabular component submerged in an analgesic solution could enhance perioperative pain management. Interestingly, this novel strategy was associated with a reduction of postoperative pain scores and opioid consumption in 100 experimental patients compared to 100 controls. The concluding paper of the session by Dr Della Valle examined whether decreased discharge opioids led to increased postoperative opioid refills. A large single-center study of 19,428 patients detected a slight increase (5%) in opioid refills but a reduction in total refill morphine milligram equivalents. The final, sixth session of day one considered various challenging aspects of revision hip arthroplasty. Dr Nam started the session with review of preliminary results from a randomized control trial comparing closed incision negative-pressure therapy with a silver-impregnated dressing for wound management in 113 hips undergoing revision arthroplasty. Unlike previous reports, the negative pressure therapy was associated with a higher reoperation rate for wound-related complications. Dr Bostrom highlighted the potential clinical impact of basic biological interventions by establishing the presence of Neutrophil Extracellular Traps (NETS) in fibrotic tissue from human aseptic loosening specimens and in a murine model of unstable tibial implantation. NET inhibition in the murine model prevented the expected tibial implant osseointegration failure. Dr Lombardi presented early 3.3 year clinical results of a highly porous Ti6al4v acetabular component in complex primary and revision arthroplasty. Survivorship for aseptic loosening was 96.6 % and 95.3% for the primary and revision cases, respectively. Dr Schwarzkopf and colleagues explored the impact of time to revision arthroplasty on clinical outcomes. Analysis of 188 revision cases revealed early revisions (less than 2 years from primary) were associated with worse outcomes, longer hospitalizations and higher reoperation rates. Mid-term results for modular dual mobility implants in revision arthroplasty were reviewed by Dr Lachiewicz who reported on 126 hips at a mean 5.5 years. 11% of hips dislocated and the 6 year survival was 91%. An outer head diameter of 48mm or greater was associated with a lower risk of dislocation. Dr Berry concluded the session by discussing the outcomes of treating the challenging problem of interprosthetic femur fractures. A single-center study of 77 cases treated over 32 years demonstrated a 79% success rate free of reoperation at 2 years with 95% of patients being ambulatory. The second day commenced with the seventh session evaluating recent strategies to improve short-term THA outcomes. Dr Bozic and colleagues investigated the association of quality measure public reporting with hip/knee replacement outcomes. Annual trend data from 2010–2011 and 2016–2017 indicate that hospital-level complication and readmission rates decease after the start of public reporting, yet it is difficult to prove a direct effect. Dr Slover reviewed his institutions experience with the Comprehensive Care for Joint Replacement (CJR) model and emphasized that lower CJR target prices make it increasingly difficult for programs to meet target price thresholds. Cost saving strategies including same day discharge and reduction of home health services may result in smaller losses of positive margins. Dr Barsoum reported on the influence of patient and procedure-related risk factors of length of stay after THA. Patient-related risk factors provided substantial predictive value yet procedure-related risk factors (hospital site and surgical approach) remain the main drivers of predicting length of stay. Dr Hozack reviewed an impressive, single surgeon cohort of 3,977 DAA THAs and analyzed adverse events and 90 day perioperative outcomes. Simultaneous bilateral DAA THA was comparable with unilateral or staged bilateral procedures in regards to complications, readmission rate and home discharge rate but with an increased risk of transfusion. To examine the risk of complications with outpatient joint arthroplasty, Dr Della Valle performed a single-surgeon matched cohort analysis comparing outpatient and inpatient hip and knee arthroplasties. Outpatient procedures were not associated with an increased risk of any postoperative complications and actually experienced fewer emergency department visits. The eighth session covered various contemporary challenges in hip arthroplasty care. Dr Griffin began the session with an analysis of the timing of complications associated with two-stage exchange procedures for periprosthetic joint infection (PJI). Of the 189 hips included, 41.6% had a complication and the mortality was 14.1% at 2.5 years, highlighting the morbidity of this treatment method. Dr Fehring provided data assessing the fate of two-stage reimplantation after failed debridement, antibiotics and implant retention (DAIR) for a prosthetic hip infection. This analysis of 114 hips yielded concerning results demonstrating a 42.9% treatment failure of patients treated with a previous DAIR compared to a 12.3% failure rate in patients treated with an initial 2-stage procedure. Dr Jacobs reviewed the analysis of 106 femoral heads with severe corrosion and identified a chemically dominated etching process termed “column damage” to be a detrimental damage mode within CoCr femoral heads that is directly linked to banding within its microstructure. These data indicate that implant alloy microstructure must be optimized to minimize the release of fretting-corrosion products. Simon Mears presented retrospective data from 184 THAs with a dual modular femoral stem. A subgroup of hips with a modular, cobalt chromium femoral neck had a pseudotumor visualized in 15% with only 55% of these having elevated CoCr levels. These findings may support the use of routine follow-up MARS MRI for modular CoCr femoral neck prostheses. The final two studies explored timely issues related to viral illness and hip surgery. Dr Browne analyzed three large administrative databases to elucidate whether patients are at increased risk for viral illnesses following total joint replacement. The incidence of postoperative influenza after total joint replacement was not increased compared to patients not undergoing total joint replacement surgery suggesting that arthroplasty procedures may not heighten the risk of viral illness. In the final paper of the session Dr Haddad presented important data regarding perioperative complications in coronavirus positive patients undergoing surgical treatment of femoral neck fractures. In this multicenter cohort study from the United Kingdom 82 coronavirus positive patients were shown to have longer hospital stays, more critical care unit admissions, higher risk of perioperative complications and an increased mortality compared to 340 coronavirus negative patients. The eighth session had two papers on alternative femoral stem designs and three presentations more focused on femoral fracture treatments. Dr Mihalko focused on the European and US experiences with the Metha femoral neck retaining stem. The US experience mirrored the encouraging results from Europe with a 94% all cause femoral survivorship and a 99.1% femoral aseptic loosening survivorship at 5 years. Dr Kraay summarized dual energy x-ray absorptiometry (DEXA) evaluation of 16 low modulus composite femoral components at long-term follow-up of a mean 22 years. The bone mineral density associated with the implant increased in Gruen zones 2–6 and showed limited decreases in zones 1 and 7. These data support the concept that a low modulus femoral stem may more effectively load the proximal femur. Dr Springer provided data from the American Joint Replacement Registry (AJRR) and by evaluating outcomes of exact matched cohorts of 17,138 patients treated with cementless or cemented femoral implants for femoral neck fractures. Cemented implants were associated with marked reduction in early revision and periprosthetic fractures. However, cemented fixation was associated with an increased mortality at 90 days and 1 year. Additional data from the AJRR was presented by Dr Huddleston who investigated the risk factors for revision surgery after arthroplasty in a cohort of 75,333 femoral neck fractures. THA when compared to hemiarthroplasty was associated with higher early and overall revision rates. Cementless femoral fixation and increased age were also associated with higher rates of any revision. Both of these studies from the AJRR suggest that further consideration should be given to femoral fixation preferences in the femoral neck fracture population. Dr Vail summarized his institution's experience with an interdisciplinary hip fracture protocol for patients undergoing arthroplasty for acute femoral neck fractures. His study compared 157 cases prior to protocol implementation with 114 patients treated after the protocol was established. The impact of the interdisciplinary protocol was impressive as evidenced by a reduced time to operative treatment, length of stay, complication rate and one-year mortality. All being achieved without an increase in readmissions or facility discharges. The final session of the meeting addressed innovations in perioperative care of THA patients. Dr Barrack started the session with an interesting study examining the feasibility and patient preferences regarding telemedicine. A cross-sectional telephone survey of 163 arthroplasty patients indicated that 88% of patients use the internet and 94% own a device capable of videoconferencing. Nevertheless, only 18% of patients preferred a video visit over an in-person clinic visit due to concerns of inferior care. Dr Barnes quantified preoperative optimization work in 100 arthroplasty patients by using EMR activity logs and determined the surgical team spends an average 75 minutes per case on preoperative work activities. Dr Duwelius reported the early outcomes of primary THA with a smartphone-based exercise and educational platform compared to standard of care controls. A randomized control trial design with 365 patients demonstrated similar outcomes and non-inferiority of the smartphone platform relative to complications, readmissions, emergency room/urgent care visits. The association of controlled substance use with THA outcomes was assessed by Dr Higuera Rueda. A quantitative assessment using the NarxCare score identified 300 and above as a score associated with adverse outcomes after THA. Dr Macaulay reviewed data from a large retrospective study of 1,825 THAs indicating that discontinuation of intermittent pneumatic compression devices does not increase the risk of venous thromboembolism in standard risk patients being treated with 81mg ASA BID as prophylaxis. Dr Antoniou presented the final paper of the meeting investigating potential changes in patient health status as an indication for surgery over time. Data from this large systematic review of the literature found patients undergoing THA at similar health status to the past with no influence form patient age, gender, year of enrollment or geographic region. As summarized above, the 2020 virtual Hip Society Summer Meeting was rich in scientific content, productive discussion and a collaborative spirit. This collective body of work will result in impactful scientific contributions and will serve as a foundation for future innovation and advancements in the treatment of hip disease

Aim. In the diagnosis of prosthetic joint infection (PJI), many biomarkers have shown a sound performance in terms of accuracy, sensitivity and specificity. In this study we aimed to test the frequently used serum biomarkers C-reactive Protein (CRP), Fibrinogen, Leukocytes, Interleukin-6 (IL-6), Interferon alpha (IF-alpha) and Procalcitonin (PCT) regarding these qualities. Following that, the optimal multi-biomarker combination was calculated to further improve the diagnostic performance. Method. 124 knee or hip revision arthroplasty procedures were prospectively investigated focusing on preoperative serum blood levels of CRP, Fibrinogen, Leukocytes, IL-6, IF-alpha and PCT. The presence of PJI was determined by a blinded researcher. Logistic regression with lasso-regularization was used for the biomarkers and all their ratios. Following cross-validation on a training sample set to get optimal performance estimates, we performed the final model on a test set (25% of all samples). Results. Out of all evaluated biomarkers, CRP (AUC 0.91, p-value 0.03) and Fibrinogen (AUC 0.93, p-value 0.02) had the best performances. The optimal combination when testing multiple biomarkers in 32 cross-validation runs was calculated including Fibrinogen, CRP, the ratio of Fibrinogen to CRP and the ratio of serum Thrombocytes to CRP (AUC 0.92, accuracy 0.77, specificity 0.92, sensitivity 0.68, cut-off 0.63, p-value 0.04). Conclusions. It was not possible to increase the diagnostic performance by combining multiple biomarkers using sophisticated statistical methods. The calculated Multi-biomarker models did not improve the AUC, accuracy, sensitivity and specificity when compared to single biomarkers


Aims

To evaluate the hypothesis that failed osteosynthesis of periprosthetic Vancouver type B1 fractures can be treated successfully with stem revision using a transfemoral approach and a cementless, modular, tapered revision stem with reproducible rates of fracture healing, stability of the revision stem, and clinically good results.

Patients and Methods

A total of 14 patients (11 women, three men) with a mean age of 72.4 years (65 to 90) undergoing revision hip arthroplasty after failed osteosynthesis of periprosthetic fractures of Vancouver type B1 were treated using a transfemoral approach to remove the well-fixed stem before insertion of a modular, fluted titanium stem which obtained distal fixation. These patients were clinically and radiologically followed up for a mean 52.2 months (24 to 144).

We compared the length of hospitalisation, rate of infection, dislocation of the hip and revision, and mortality following primary hip and knee arthroplasty for osteoarthritis in patients with Alzheimer’s disease (n = 1064) and a matched control group (n = 3192). The data were collected from nationwide Finnish health registers. Patients with Alzheimer’s disease had a longer peri-operative hospitalisation (median 13 days vs eight days, p < 0.001) and an increased risk for hip revision with a hazard ratio (HR) of 1.76 (95% confidence interval (CI) 1.03 to 3.00). Dislocation was the leading indication for revision. There was no difference in the rates of infection, dislocation of the hip, knee revision and short-term mortality. In long-term follow-up, patients with Alzheimer’s disease had a higher mortality (HR 1.43; 95% CI 1.22 to 1.70), and only one third survived ten years post-operatively. Increased age and comorbidity were associated with longer peri-operative hospitalisation in patients with Alzheimer’s disease.

We report our experience of revision total hip replacement (THR) using the Revitan curved modular titanium fluted revision stem in patients with a full spectrum of proximal femoral defects. A total of 112 patients (116 revisions) with a mean age of 73.4 years (39 to 90) were included in the study. The mean follow-up was 7.5 years (5.3 to 9.1). A total of 12 patients (12 hips) died but their data were included in the survival analysis, and four patients (4 hips) were lost to follow-up. The clinical outcome, proximal bone regeneration and subsidence were assessed for 101 hips.

The mean Harris Hip Score was 88.2 (45.8 to 100) after five years and there was an increase of the mean Barnett and Nordin-Score, a measure of the proximal bone regeneration, of 20.8 (-3.1 to 52.7). Five stems had to be revised (4.3%), three (2.9%) showed subsidence, five (4.3%) a dislocation and two of 85 aseptic revisions (2.3%) a periprosthetic infection.

At the latest follow-up, the survival with revision of the stem as the endpoint was 95.7% (95% confidence interval 91.9% to 99.4%) and with aseptic loosening as the endpoint, was 100%. Peri-prosthetic fractures were not observed.

We report excellent results with respect to subsidence, the risk of fracture, and loosening after femoral revision using a modular curved revision stem with distal cone-in-cone fixation. A successful outcome depends on careful pre-operative planning and the use of a transfemoral approach when the anatomy is distorted or a fracture is imminent, or residual cement or a partially-secured existing stem cannot be removed. The shortest appropriate stem should, in our opinion, be used and secured with > 3 cm fixation at the femoral isthmus, and distal interlocking screws should be used for additional stability when this goal cannot be realised.

Aim. The saddle prosthesis was originally developed for reconstruction of large acetabular defects in hip revision arthroplasty. Later on the saddle prosthesis was also used for hip reconstruction after resection of peri-acetabular tumours. In case of patient survival a long-term good hip function is required of the saddle prosthesis. The goal of this study is the measurement of long-term clinical results of saddle prosthesis after reconstruction of peri-acetabular tumours. Method. Between 1987 and 2003 a total of 17 patients were treated in the Leiden University Medical Center with saddle prosthesis after resection of peri-acetabular tumours (12 chondrosarcoma, 3 osteosarcoma, 1 malignant fibrous histiocytoma, 1 metastasis). 11 of the 17 patients died, the mean survival was 37 months (range 2-59 months), and 6 patients were still alive (follow-up 12.1 year, range 8.3–16.8 year). The outcome was measured with the SF-36 questionnaire, the Toronto Extrimity Salvage Score (TESS) and the Musculo Skeletal Tumour Society (MSTS) score. In 1 patient the saddle prosthesis was removed already after 3 months because of luxation and infection. Results. In 13 of the 17 patients the use of walking devices was known: 54% required two crutches, 38% one crutch, and 8% no walking aids. The mean hip flexion was 60 degrees (range 40-100 degrees). Local complications were seen in 14 of the 17 patients: 9 wound infections, 7 luxations, and 2 leg length discrepancies requiring additional surgery. In the 5 surviving patients the MSTS score at long-term follow up was 47%, the mean TESS score was 53%, the mean composite SF-36 physical and mental scores were 43.92 and 50.64 respectively. Conclusion. Reconstruction with saddle prosthesis after peri-acetabular tumour surgery has a high risk of complications, and poor long term functional outcome with limited flexion of the hip

We wanted to evaluate the clinical and radiological results of acetabular revision using the acetabular reinforcement ring and allograft impaction in patients with severe acetabular bony defect. 41 hips revision arthroplasty using reinforcement ring were performed between April 1997 and October 2005 and were followed up for more than two years. The cause of primary arthroplasty was AVN in 18 cases, secondary osteoarthritis (OA) in 17 cases, fracture in cases and primary OA in 1 case. The cause of revision arthroplasty was acetabular cup loosening in 20 cases, massive osteolysis in 14 cases, infection in 4 cases, liner dissociation in 2 cases, and recurrent dislocation in 1 case. The average period between primary and revision arthroplasty was 11.4 years (range 0.6 to 29.1 years). Acetabular defects were classified based on the AAOS classification and Paprosky classification system. All were treated with autografts or allografts. Muller ring was used in 18 cases, Burch-Schneider ring was used in 14 cases, and Ganz ring in 9 cases. Clinical evaluations were performed according to the Harris hip score (HHS), and the radiographic results were evaluated by progression of acetabular component loosening, union of bone grafts, periacetabular osteolysis, and migration of the hip center. The mean preoperative Harris hip score of 64.9 was improved to 91.8 points at the latest follow-up. There were 39 cases of type 3 defect, 2 cases of type 4 defect according to the AAOS classification and 8 cases of type 2B defect, 3 cases of type 2C defect, 28 cases of type 3A defect, and 2 cases of type 3B defect according to Paprosky classification. Radiographically, the bone grafts were well united except one case. The mean preoperative hip center of rotation which was vertically 32.3mm, horizontally 33.2 mm migrated to vertically 26 mm, horizontally 33.2 mm postoperatively and it was statistically significant. The mean preoperative abductor lever arm of 41.7 mm changed to 45 mm postoperatively which was statistically insignificant. However the mean preoperatiave body lever arm of 89.4 mm changed to 96.9 mm postoperatively which was statistically significant. Postoperative complications were cup loosening in 1 case, dislocation in 2 cases, and recurrence of deep infection in 1 case. Clinically and radiographically, acetabular reconstruction using reinforcement ring showed very promising short term result. We conclude that reinforcement ring can provide stable support for grafted bone in severe bone defect. But meticulous surgical technique to get initial firm stability of ring and optimal indication in mandactory for the successful result

The use of neck modular adapter is a relative new solution for hip revision arthroplasty. This device assure a lot of advantages for the orthopaedic surgeon because Bioball can be use in different situations in order to solve different complications: hip prosthesis dislocation, correction of length (up to +21mm), save an old stem not mobilized, reduction of operation length. The hip prosthesis dislocation, in spite of the continuous progress of implants’ materials and design, is still an actual event in the orthopaedic clinical practice, both after total hip replacement or a endoprosthesis. Furthermore, dislocation has an important social-economic impact because of a protracted hospitalization and rehabilitation and elevated costs of an eventual revision. Although using heads with a diameter larger than 28 mm we obtain virtually a greater range of motion, with a contemporary increase of degree necessary to cause the head-neck impingement, the risck of dislocation hasn’t a significant increase using head with a diameter of 22 mm. Neck modular adapters (Bioball) allow to correct easily the biomechanics parameters of the dislocated prosthesic joint, avoiding the revision of the stem. Other indications for the use of the neck modular adapter are total hip replacement and intraoperatory correction of the limb length. Vantages are the possibilty to obtain a great range of motion through a small thickness of the 12/14 adapter, the possibility to extend the limb length up to 21 mm and to use ceramic heads during revisions, because the combination head/neck has a tribological unweared surface. In fact, in normal conditions, if the stem is not mobilizated, the use of ceramic head is rash; the Bioball adapter, instead, can be used with a old stem, so we can set a ceramic head. Every stem with a Biolox cone can be combined with a metal or ceramic head up to the 5XL size (+21 mm) through a Bioball adapter; in this way the cup is not removed. We have two kinds of neck modular adapters: 12/14 allow both to extend the neck and to correct the offset, and 14/16 that allow to extend only the neck, because of the largest diameter of the prosthesic neck and the small thickness of the adapter. For these neck modular adapters exist different sizes, from M to 5XL (+21mm). Recently to these two types of Bioball were introduced also solutions for special stems (like for Exeter, ABG I, ABGII, PCA and others) We have also proving heads and necks. The proving and definitive heads have to be of the Bioball system because these are inserted on a modular neck with a no-standard diameter. In the common practice the use of these adaptors has not to be considered as a routinary procedure, but have to be taken in consideration as a valid aid for orthopaedic surgeon to quickly and less invasively, solve technically demanding procedures with a real benefit for high-surgical risk patients

Bone impaction grafting (BIG) is a surgical technique for the restoration of bone stock loss with impaction of autograft or allograft bone particles (BoP). The goal of a series in-vitro and in-vivo experiments was to assess the suitability of deformable pure Ti (titanium) particles (TiP, FONDEL MEDICAL BV, Rotterdam, The Netherlands) for application as a full bone graft substitute in cemented revision total hip arthroplasty. TiP are highly porous (interconnective porosity before impaction 85 to 90%). In-vitro acetabular reconstructions were made in Sawbones (SAWBONES EUROPE, Malmö, Sweden) to evaluate migration by roentgen stereo photogrammetric analysis and shear force resistance by a lever out experiment. In-vitro femoral TiP reconstructions (SAWBONES, Malmö, Sweden) were used to evaluate micro-particle release and subsidence. Mature Dutch milk goats were used for two in-vivo experiments.

A non-loaded femoral defect model was used to compare osteoconduction of bioceramic coated TiP with BoP and ceramic particles (CeP).

Acetabular defects (AAOS type 3) were reconstructed in 10 goats using a metal mesh with impacted TiP acting as a full bone graft substitute in combination with a cemented polyethylene cup and a downsized cemented Exeter femoral stem (STRYKER BENOIST, Girard, France).

Blood samples were taken for toxicological analysis.

In-vitro: TiP were as deformable as BoP and created an entangled graft layer (porosity after impaction 70 to 75%). Acetabular TiP reconstructions were more stable and resistant to subsidence and shear force than BoP reconstructions (lever-out moment 56 ± 12 Nm respectively 12 ± 4 Nm, p < 0.001). After initial setting, femoral subsidence rates were smaller than seen in femoral bone impaction grafting (0.45 ± 0.04 mm after 300 000 loading cycles). Impaction generated 1.3 mg particles/g TiP (particle Ø 0.7–2 000 μm, tri-modal size distribution). In-vivo: Bioceramic coated (10 −40 μm) TiP showed bone ingrowth rates comparable to BoP and CeP. Reconstructed acetabular defects showed rapid bone ingrowth into the layer of TiP. Serum titanium concentrations slowly increased from 0.60 ± 0.28 parts per billion (ppb) preoperatively to 1.06 ± 0.70 ppb at fifteen weeks postoperatively (p = 0.04).

Mechanical studies showed very good initial mechanical properties of TiP reconstructed defects. The in-vitro study showed micro-particle generation, but in the short-term goat studies, histology showed very few particles and no negative biological effects were found. The in-vivo acetabular study showed very favorable bone ingrowth characteristics into the TiP layer and a much thinner interface with the cement layer compared to similar defects reconstructed with BoP or mixtures of BoP with CeP. Further analysis in a human pilot study should proof that TiP is an attractive and safe alternative for allograft bone in impaction grafting revision arthroplasty.

Introduction: The histology of prosthetic tissue is a gold standard for the diagnosis of prosthetic joint infection. However, the specificity and sensitivity of histology has never been 100% and this could be due to several causes. A possible cause for inconsistencies in histological results could be the type of specimen submitted to laboratory. The majority of authors obtain specimens from pseudocapsule, interface membrane and any tissue area suspicious of infection. Aim: The objective of our study was to elucidate which is the most accurate specimen for histological diagnosis of prosthetic joint infection. Methods: Prospective study including all revision arthroplasties performed in Hospital Clinic of Barcelona (Spain) from January 2007 to June of 2009. Specimens from pseudocapsule and from interface membrane were obtained from each patient. Definitive diagnosis of infection was considered when ≥2 cultures were positive for the same microorganism or the presence of pus around the prosthesis. Patients were classified in two groups:. patients submitted to hip revision arthroplasty due to an aseptic loosening in whom cultures (at least 5) obtained during surgery were negative and. patients submitted to hip revision arthroplasty due to a septic loosening confirmed by the presence of pus or ≥2 positive culture for the same microorganism. Results: A total of 69 revisions were included in the study; 57 were classified in the group A and 12 were classified in the group B. The percentage of positive interface membrane histology in patients with prosthetic joint infection (group B) was significantly higher than the percentage of positive pseudocapsule histology (83.3% vs 41.6%, p=0.04, Fisher exact test). Conclusion: The results suggest that the best specimen of periprosthetic soft tissue for histological study to diagnose the chronic periprosthetic infection in a revision total hip arthroplasty is the periprosthetic interface membrane

Introduction: There has been a proliferation of newer fluted tapered grit-blasted titanium stems in hip revision arthroplasty. However, only a limited number of clinical series have so far been reported in the literature. Moreover, all reports have only a short-term clinical and radiographical follow-up (< 5 years). Medium-term and long-term follow-up studies are lacking. Therefore, the aim of this study was to review a series of a cementless modular tapered revision femoral component (MP Link hip reconstruction prosthesis) with a minimum 5 year follow-up. Patients and Methods: This study includes 90 consecutive cases (87 patients) with the MP stem. We documented the Harris hip score (HHS) and the Visual analogue scale (VAS) was used to assess pain at rest and movement. Survivorship was calculated using Kaplan-Meier survival analysis. The 95% confidence intervals (CI) for the cumulative 5 year survival were calculated. Radiographs made immediately after the index operation were compared with those at follow-up examination in order to classify the restoration of femoral bone and vertical migration of the implant. Results: Of the original 90 cases, 24 (27%) died prior to clinical and radiographical review, 2 (2%) had a stem revision and 1 (1%) was lost to follow-up. Patients who died and were lost to follow-up were included in the survival analysis. For those patients, all data concerning complications and revisions were extracted from journal files and the Swedish Hip Register which collects all information on reoperations after hip revision surgery. The median follow-up time was 6 (5–11) years. The median VAS for pain for the affected hip was 0 (0–5) at rest and 0 (0–9) at movement. The median HHS at follow-up was 78 (16–100) points. 17 (19%) patients dislocated their hips during follow-up. A prosthesis head size of 22 mm was present in 6/17 (35%) patients with dislocation and in 11/73 (15%) patients without dislocation (P = 0.055). The cumulative 5 year survival rate was 98% (95% CI: 94–100%) with stem removal and 90% (95% CI: 85–96%) with any reoperation as the endpoint. At follow-up, we noted subjectively that 17% of the cases had evidence of proximal bone restoration, whereas 44% had constant defects. In 39% the quality of the proximal bone appeared to be declining. If present, this was mostly seen around the lesser trochanter. The median vertical stem migration was 2.7 (0–30) mm. Discussion: This is the first report with a clinical and radiographical medium-term follow-up of patients with a cementless modular tapered distally fixated hip revision stem. In our study, we found a discrepancy between a high implant survivorship and good pain relief on one hand and a high dislocation rate on the other hand

Introduction: Total hip arthroplasty is one of the most frequently performed surgical procedures, with implants usually giving over 90% survival at 10 years. The failure rate is primarily due to aseptic loosening often associated with progressive bone stock loss. Impaction of cancellous morselized allografts with cement can be used for revision total hip arthroplasty in such cases. There is increasing interest in the use of synthetic bone graft substitutes as extenders to allograft due to the shortage and variable quality of allograft. A chemically-pure synthetic calcium phosphate (CaP) allograft extender is compared with allograft alone for acetabular and/or femoral revisions using the Impaction Grafting technique. Methods: 96 hips in 94 patients underwent revision hip arthroplasty using impaction grafting at 4 sites. Hip revision using impaction grafting was carried out using the Exeter X-Change Instrumentation system, using an Exeter Stem and/or a cemented polyethylene cup. Patients were randomized to receive allograft or CaP plus allograft in a 50:50 volume. Clinical and radiographic assessment was conducted pre-operatively and immediately post-operatively and at 6, 12 and 24 months. Clinical assessments included Harris Hip, Oxford and Charnley modified Merle d’Aubigne scores. Clinical complications were also recorded. Radiographs were assessed for the graft quality, radiolucent lines, lyses or migration, and incorporation of graft. The X-rays were also independently reviewed by an experienced author. Results: The mean age was 70.2 years (range 41–89 years) with 58 males and 36 females. A total of 40 femoral revisions and 88 acetabular revisions were conducted. There were 46 patients in the ApaPore group, 49 patients in the allograft group and 1 patient who received CaP/allograft and allograft for an acetabular and femoral revision respectively. No deep infections or adverse events due to the CaP were reported, with no significant difference in complication rates including revision and re-operations. No significant difference in acetabular migration, femoral subsidence, radiolucencies and lyses between the groups was observed. The independent review found no difference between the groups in terms of migration. The bone density was apparently greater for the ApaPore group at 12 months (p=0.001) and 24 months (p=0.012) although the significance of this is unclear. No significant difference in the clinical measures was observed between the groups. Conclusion: CaP is comparable with allograft in terms of performance and safety when used as an allograft extender for total hip revision arthroplasty using impaction grafting

Introduction: The senior author undertakes single stage revision hip arthroplasty for cases with no preoperative evidence of infection based on history and examination, ESR and CRP results and negative results from selective aspirations. Despite this a large proportion of intra-operative samples are positive for infection. The purpose of our study is to look at the results of intra-operative histology and microbiology samples in these cases and to assess the subsequent incidence of infection. Methods: Retrospective case study comprising of 230 single stage revision total hip arthroplasties carried out by a single surgeon over 5 year period (2003–2008). Intra-operatively tissue samples were taken from multiple sites and sent for both histology and microbiology. Microbiology results were reported at 24hrs, 48hrs, 7 days and 21 days and correlated with histology reports. Results: From a total number of 230 patients, we had 98 left and 132 right hip revision arthroplasties. There were 95 men and 135 women with a mean age of 73 years (range 40–93). Intra-operative microbiology was negative in 108 patients (46.95%), of which 3 patients’ histology samples were consistent with infection. Of the 122 microbiology positive patients (53.04%), there were 8 histology samples consistent with infection. The most frequent growths were of Coagulase negative Staphylococcus (64 cases) and Propionibacterium (18 cases). 3 cases subsequently developed deep infection. Discussion: This study highlights a significant discrepancy between intra-operative microbiology and intra-operative histology results. There is also a very high discrepancy between pre-operative assessment and intra-operative microbiological findings although the majority of cases subsequently did not show any clinical evidence of infection

Modular femoral stem provides significant flexibility in total hip revision arthroplasty. There have been few clinical studies that have dealt with modular stem. We have evaluated the clinical and radiographic performance of 59 patients with distal fix modular Link MP stem. The average follow-up period was 6.4 years. The average Harris hip score was improved from 47 to 87.6. Of 19 patients with trochanteric osteotomy, greater trochanter was displaced in four patients. Re-revision was done to five patients. Three were for subsidence, one of them showed dissociation of the coupling part and the other two were for a nonunion of osteotomy site. There was no statistical relation (p=0.40) between stem subsidence and bone deficiency; the subsidence may have been too small for the canal. As a result of last follow-up, survival rate was 91.5 %(CI 95%, 89–101), but there was no case of recurrent dislocation or femoral stem fracture

Background: The aim of this prospective multi-center study was to evaluate the clinical and radiological results of a total of 314 uncemented femoral stem revisions using the modular MRP-titanium system. Methods: 305 patients (111 males, 194 females, mean age 67.7) with 314 MRP-titanium systems were followed-up for a meantime of 3.2 years (1 to 9 years). Pre- and post-operatively all patients were clinically documented using the Harris hip score. Radiologically, the preoperative bony defects were assessed by the Paprosky classification. Post-operatively, periprosthetic bone remodeling was evaluated on the basis of radiographic evidence of bone apposition or resorption. The modular MRP-titanium system has proven to be valuable in quite problematic cases of hip revision arthroplasty with extensive femoral defects. The system allows intraoperative adaptation of implant length and antetorsion angle to the actual situation, a feature not provided by non-modular femoral revision implants
