Aims

The ulna is an extremely rare location for primary bone tumours of the elbow in paediatrics. Although several reconstruction options are available, the optimal reconstruction method is still unknown due to the rarity of proximal ulna tumours. In this study, we report the outcomes of osteoarticular ulna allograft for the reconstruction of proximal ulna tumours.

Methods

Medical profiles of 13 patients, who between March 2004 and November 2021 underwent osteoarticular ulna allograft reconstruction after the resection of the proximal ulna tumour, were retrospectively reviewed. The outcomes were measured clinically by the assessment of elbow range of motion (ROM), stability, and function, and radiologically by the assessment of allograft-host junction union, recurrence, and joint degeneration. The elbow function was assessed objectively by the Musculoskeletal Tumor Society (MSTS) score and subjectively by the Toronto Extremity Salvage Score (TESS) and Mayo Elbow Performance Score (MEPS) questionnaire.

Open debridement and Outerbridge and Kashiwagi debridement arthroplasty (OK procedure) are common surgical treatments for elbow arthritis. However, the literature contains little information on the long-term survivorship of these procedures. The purpose of this study was to determine the survivorship after elbow debridement techniques until conversion to total elbow arthroplasty and revision surgery. We performed a retrospective chart review of patients who underwent open elbow surgical debridement (open debridement, OK procedure) between 2000 and 2015. Patients were diagnosed with primary elbow osteoarthritis, post-traumatic arthritis, or inflammatory arthritis. A total of 320 patients had primary surgery including open debridement (n=142) and OK procedure (n=178), and of these 33 patients required a secondary revision surgery (open debridement, n=14 and OK procedure, n=19). The average follow-up time was 11.5 years (5.5 - 21.5 years). Survivorship was analyzed with Kaplan-Meier curves and Log Rank test. A Cox proportional hazards model was used assess the likelihood of conversion to total elbow arthroplasty or revision surgery while adjusting for covariates (age, gender, diagnosis). Significance was set p<0.05. Kaplan-Meier survival curves showed open debridement was 100.00% at 1 year, 99.25% at 5 years, and 98.49% at 10 years and for OK procedure 100.00% at 1 year, 98.80% at 5 years, 97.97% at 10 years (p=0.87) for conversion to total elbow arthroplasty. There was no difference in survivorship between procedures after adjusting for significant covariates with the cox proportional hazard model. The rate of revision for open debridement and OK procedure was similar at 11.31% rand 11.48% after 10 years respectively. There were higher rates of revision surgery in patients with open debridement (hazard ratio, 4.84 CI 1.29 – 18.17, p = 0.019) compared to OK procedure after adjusting for covariates. We also performed a stratified analysis with radiographic severity as an effect modifier and showed grade 3 arthritis did better with the OK procedure compared to open debridement for survivorship until revision surgery (p=0.05). However, this difference was not found for grade 1 or grade 2 arthritis. This may suggest that performing the OK procedure for more severe grade 3 arthritis could decrease reoperation rates. Further investigations are needed to better understand the indications for each surgical technique. This study is the largest cohort of open debridement and OK procedure with long term follow-up. We showed that open elbow debridement and the OK procedure have excellent survivorship until conversion to total elbow arthroplasty and are viable options in the treatment of primary elbow osteoarthritis and post traumatic cases. The OK procedure also has lower rates of revision surgery than open debridement, especially with more severe radiographic arthritis

Used routinely in maxillofacial reconstructive surgery, the chondrocostal graft is also applied to hand surgery in traumatic or pathologic indications. The purpose of this overview was to analyze at long-term follow-up the radiological and histological evolution of this autograft, in hand and wrist surgery. We extrapolated this autograft technique to the elbow by using perichondrium. Since 1992, 148 patients have undergone chondrocostal autograft: 116 osteoarthritis of the thumb carpometacarpal joint, 18 radioscaphoid arthritis, 6 articular malunions of the distal radius, 4 kienbock's disease, and 4 traumatic loss of cartilage of the proximal interphalangeal (PIP) joint. Perichondrium autografts were used in 3 patients with elbow osteoarthritis. Magnetic Resonance Imaging (MRI) was performed in 19 patients with a mean follow-up of 68 months (4–159). Histological studies were performed on: i) perioperative chondrocostal grafts (n=3), ii) chondrocostal grafts explanted between 2 and 48 months after surgery (n=10), and iii) perioperative perichondrium grafts (n=2). Whatever the indication, the reconstruction by a chondrocostal/ostochondrocostal or perichondrium graft yielded satisfactory clinical results at long-term follow-up. The main question was the viability of the graft. -. For rib cartilage grafting: The radiological study indicated the non-wear of the graft and a certain degree of ossification. The MRI and histology confirmed a very small degree of osseous metaplasia and graft viability. The biopsies showed neo-vascularization of the cartilage that had undergone morphological, constitutional and architectural changes. Comparison of these structural modifications with perioperative chondrocostal graft histology is in progress. -. For perichondrium grafting: The first cases gave satisfactory clinical results but must be confirmed on a larger number of patients. Histological results highlighted a tissue composed of one fibrous layer and one cartilage-like layer, a common composition of supporting tissue. Despite the strong mechanical strain in the hand and wrist, chondrocostal graft is a biological arthroplasty that is trustworthy and secure over the long term, although it can cause infrequent complications inherent to this type of surgery. Despite the inevitable histological modification, the cartilage remains alive and is of satisfactory quality at long term follow-up and fulfills the requirements for interposition and reconstruction of an articular surface. The perichondrium graft constitutes a new arsenal to cure cartilage resurfacing. The importance of perichondrium for the survival of the grafted cartilage, as previously reported, as well as its role in resurfacing, is being investigated

To prevent insufficiency of the triceps after total elbow arthroplasty, we have, since 2008, used a triceps-sparing ulnar approach. This study evaluates the clinical results and post-operative alignment of the prosthesis using this approach. We reviewed 25 elbows in 23 patients. There were five men and 18 women with a mean age of 69 years (54 to 83). There were 18 elbows with rheumatoid arthritis, six with a fracture or pseudoarthrosis and one elbow with osteoarthritis. Post-operative complications included one intra-operative fracture, one elbow with heterotopic ossification, one transient ulnar nerve palsy, and one elbow with skin necrosis, but no elbow was affected by insufficiency of the triceps. Patients were followed for a mean of 42 months (24 to 77). The mean post-operative Japanese Orthopaedic Association Elbow Score was 90.8 (51 to 100) and the mean Mayo Elbow Performance score 93.8 (65 to 100). The mean post-operative flexion/extension of the elbow was 135°/-8°. The Manual Muscle Testing score of the triceps was 5 in 23 elbows and 2 in two elbows (one patient). The mean alignment of the implants examined by 3D-CT was 2.8° pronation (standard deviation (. sd). 5.5), 0.3° valgus (. sd. 2.7), and 0.7° extension (. sd. 3.2) for the humeral component, and 9.3° pronation (. sd. 9.7), 0.3° valgus (. sd. 4.0), and 8.6° extension (. sd. 3.1) for the ulnar component. There was no radiolucent line or loosening of the implants on the final radiographs. . The triceps-sparing ulnar approach allows satisfactory alignment of the implants, is effective in preventing post-operative triceps insufficiency, and gives satisfactory short-term results. . Background. The optimal treatment for symptomatic elbow osteoarthritis remains debatable especially in patients still involved in heavy manual work. The Outerbridge-Kashiwagi (OK) procedure has been used when simple measures fail. The aim of this study is to analyse the results of the OK procedure in patients with symptomatic osteoarthritis. Methods. Twenty-two patients were included in the study. The male:female ratio was 18:4. The mean age was 60 years with mean follow-up of 38 months (24–60 months). 17 were manual workers, 3 involved in sports activities and 2 non-manual workers. All patients were assessed using Mayo Elbow Performance Index Score system. Preoperative radiological assessment showed osteophytes around olecranon and coronoid process and joint space narrowing in radio-humeral articulationin all cases. Results. There was a significant improvement (p<0.05) in movement in the flexion-extension axis from 78.2° to 107.3°. There was a significant reduction in pain post-operatively (p<0.001). Mean MEPI score improved from 50 to 87.4 post surgery which was significant (p<0.05). One patient had ulnar nerve palsy which resolved in six months with conservative management. The results were excellent in six patients (27%), good in fifteen (68%), fair in one (5%) and one (5%) had poor result. Discussion and conclusion. The present study indicates that the OK procedure provides significant pain relief and a functionally useful range of movement of more than 100°. The procedure can be used in high demand patients, wherein total elbow replacement is not indicated. It significantly reduces the disability in patients with significant elbow arthritis

We retrospectively assessed the value of identifying impinging osteophytes using dynamic computer simulation of CT scans of the elbow in assisting their arthroscopic removal in patients with osteoarthritis of the elbow. A total of 20 patients were treated (19 men and one woman, mean age 38 years (19 to 55)) and followed for a mean of 25 months (24 to 29). We located the impinging osteophytes dynamically using computerised three-dimensional models of the elbow based on CT data in three positions of flexion of the elbow. These were then removed arthroscopically and a capsular release was performed.

The mean loss of extension improved from 23° (10° to 45°) pre-operatively to 9° (0° to 25°) post-operatively, and the mean flexion improved from 121° (80° to 140°) pre-operatively to 130° (110° to 145°) post-operatively. The mean Mayo Elbow Performance Score improved from 62 (30 to 85) to 95 (70 to 100) post-operatively. All patients had pain in the elbow pre-operatively which disappeared or decreased post-operatively. According to their Mayo scores, 14 patients had an excellent clinical outcome and six a good outcome; 15 were very satisfied and five were satisfied with their post-operative outcome.

We recommend this technique in the surgical management of patients with osteoarthritis of the elbow.

Purpose: Elbow osteoarthritis (OA) is characterized by a loss of elbow motion secondary to joint capsular hypertrophy and osteophyte formation. Previous work on joint capsules in post-traumatic (PT) elbow joint contractures has shown that alterations in cell populations (increased number of alpha-SMA positive myofibroblasts), matrix molecule and enzyme, and growth factor mRNA profiles are associated with loss of elbow motion in this condition. The objective of this study was to determine whether alterations in joint capsule parameters were similar or different in two etiologies of human elbow contractures, primary OA and PT. Method: Posterior elbow joint capsules were obtained from eight male patients with primary elbow OA (age 52±12 yr ), five male patients with chronic (> 1 year) PT (age 47±12 yr ) and four male organ donors free of OA and contractures (age 43±10 yr ). RNA was extracted for subsequent real-time PCR for alpha-SMA, interleu-kin-1beta, MMP-1, MMP-3, collagen type III, biglycan, versican, tenascin C, TIMP-1, MMP-2, iNOS, COX-2, glyceraldehyde – 3 phosphate dehydrogenase (GAPDH) and 18S. 18S was used to normalize gene expression. Statistical comparisons used a oneway ANOVA followed by posthoc Tukey test. Significance was p < 0.05. Results: The mRNA levels in the OA and PT capsules were increased compared to controls in most cases. This includes the major matrix molecule collagen I and the myofibroblast marker alpha-SMA, the growth factors TGF-beta1 and CTGF plus decorin, the injury response elements (collagen III, biglycan, versican, tenascin C) as well as TIMP-1 and MMP-2. The housekeeping gene GAPDH was similar in all 3 groups as was COX-2, while iNOS was elevated in both groups characterized by contractures. When comparing the two contracture groups, the mRNA levels were similar for some molecules while differences were evident in other instances. In PT, alpha-SMA and collagen I were greater than in OA. Conversely, in the OA group, the growth factors and matrix enzyme systems exhibited higher levels than PT. Conclusion: In this study of human elbow joint capsules, we have shown that relative mRNA levels for markers of myofibroblasts, major matrix components, injury response elements and selected growth factors are significantly elevated in elbow OA and post-traumatic contractures when compared to age matched organ donor controls free of contractures. When comparing the OA and PT groups, the injury response molecules were elevated to similar relative levels. The OA group had greater increases in the growth factors and many of the matrix enzymes / inhibitors measured, while the PT group had greater increases in the myofibroblast marker alpha-SMA and the major matrix molecule collagen I. Thus in general matrix, growth factor and cellular properties appear to be preferentially altered in the two conditions studied when compared to control tissues, strengthened by the fact that the housekeeping gene GAPDH had similar relative levels in all 3 groups

Purpose : The purpose of this study is to assess the effectiveness of the arthroscopic method in the stiffness of elbow after osteoarthritis or posttraumatic arthritis. Materials and methods : In the time period January 1999 to December 2001, 11 patients with primary osteoarthritis and posttraumatic arthritis of the elbow were treated in our clinic with the arthroscopic method. All patients had stiffness and pain. Nine of them were men and two women with a mean age of 46 years (range 28–56 years). Average preoperative flexion was to 1150 (range 90 – 1400), and average extension loss was −250 (range 15 – 350). Mean follow-up was 30,3 months. Results : The range of motion showed a progressive increase until 1 year after surgery. However, after 1 year, the range of motion showed little additional increase, especially in laborers. The range of motion acquired during surgery usually was the same range that patients achieved during the rehabilitation period. Average postoperative flexion was to 1380 (range 120 – 1430) and average extension was to 50 (range 0 – 120). The range of motion showed more improvement in patients whose duration of symptoms was less than 1 year than in those whose duration of symptoms was longer than 1 year. All patients had a significant decrease in pain and 5 of them complete relief of pain. Conclusions : The arthroscopic surgery in the stiffness of elbow after osteoarthritis and posttraumatic arthritis is a minimally invasive procedure with significant results in range of motion and pain relief

To ascertain the effect of the site and number of loose bodies on the functional outcome of Outerbridge-Kashiwagi (O-K) procedure in management of osteoarthritis of the elbow. 12 patients were reviewed after having O-K procedure, and assessed using Mayo Elbow Performance score, and radiographs assessed using Derby Elbow Osteoarthritis Radiography score. There were 10 male and 2 female patients with mean age of 47 years. The mean follow up was 24 months. In 8 (66%) patients the diagnosis was primary osteoarthritis, and 4 (34%) had post-traumatic arthritis. Nine (75%) patients had osteoarthritis of the dominant elbow, and three were non-dominant. Nine patients had locking and catching symptoms. 7 patients had < 2 loose bodies, and 5 had > 2 loose bodies. 7 had anterior loose bodies alone and 5 had both anterior and posterior loose bodies. The Mayo Elbow Performance score improved from a mean preoperative value of 51 to 85 points postoperatively (p< 0.0001). There were 3 excellent, 7 good, 2 fair, and no poor results. Visual analogue pain score improved from a mean of 7.4 to 2.6 postoperatively (p< 0.001). The Derby Elbow Osteoarthritis Radiography score improved from preoperative mean of 6.5 to 5.3 postoperatively (p< 0.013). There was no significant difference between functional outcome of primary osteoarthritis and post-traumatic arthritis (p> 0.42). Number of loose bodies had no significance on the functional outcome (p> 0.39), neither did the site of the loose bodies (p> 0.44). There was no significant difference of the number of loose bodies on the overall total score of Derby Osteoarthritis Elbow Score (p> 0.2). In two patients revision had to be undertaken due to persistent locking that improved postoperatively. The number and site of loose bodies, the type of osteoarthritis and the duration of symptoms have no significant prognostic value in predicting functional outcome

Since described by Kashiwagi in 1978, the Outerbridge - Kashiwagi procedure (OK procedure) has been used to treat osteoarthritis of the elbow when simple measures have failed. Despite being used for 20 years, there have been surprisingly few series. The aim of this study was to assess the medium to long term results of the OK procedure and to analyse the preoperative and operative findings to identify features which predict a good outcome.

A consecutive series of 43 patients (44 elbows) underwent ulnohumeral debridement (OK procedure) for osteoarthritis of the elbow. Their mean age was 57 years (range 24 – 85 years) with one female patient. 35 patients (36 elbows) were reviewed after a mean follow up of 39 months (range 12 – 71 months). Overall, 81% of patients were satisfied with 12 good, 19 fair and 5 poor outcomes. The mean arc of flexion/extension (p=0.001), pain score (p=0.002) and locking (p=0.003) were significantly improved but a significant number of patients developed rest pain (p< 0.0001). There was a complication rate of 17% (2 ulna nerve entrapment, 1 ulna nerve neuropraxia which resolved completely, 1 superficial wound infection, 1 wound haematoma and 1 myocardial infarction). The reoperation rate was 8% (2 revision OK procedures and 1 ulna nerve decompression).

Patients with symptoms for less than 2 years, considerable preoperative pain or cubital tunnel syndrome had a significantly increased chance of a good outcome. The absence of preoperative locking was associated with a significantly increased chance of a poor outcome. Joint space narrowing on radiographs or presence of posterior loose bodies at operation was associated with an increased chance of a good outcome but these were not statistically significant. A history of trauma, the preoperative range of movement and Xray score did not predict outcome.
