Background

Intracapsular neck of femur fractures are one of the most common injuries seen in Orthopaedics. When the fracture is amenable to internal fixation there are 2 main treatment options, namely multiple cannulated hip screws (MCS) and 2-hole sliding hip screws (SHS). In this retrospective study we examine the outcomes associated with these two methods of internal fixation. At present there is little consensus regarding which treatment should be used

Methods

161 patients were found to have suffered intracapsular neck of femur fracture treated with either SHS or MCS fixation over a 5 year period from April 2009 to April 2014, allowing at least 1 year follow up following injury. The patients imaging and clinical notes were then reviewed to ascertain the outcome of their treatment and any complications.

Statement of purpose. Cement fixation of total knee replacement (TKR) is commonly cited as being the gold standard, with better long-term survival rates when compared to uncemented fixation so the authors set out to analyse the longterm survivorship without aseptic loosening in a series of 471 uncemented TKR.

Methods

A consecutive single surgeon series of patients undergoing routine follow up after a hydroxyapatite coated, uncemented and cruciate retaining TKR performed from 1992 to 1995 were analysed. All patients were invited for clinical review and radiological assessment. Revision of the TKR for aseptic loosening was the primary outcome. Secondary outcomes included Knee Society Score (0–200), range of movement, secondary surgical interventions and the presence of polyethylene wear or osteolysis on plain radiography.

Results

471 TKRs were performed in 356 patients (115 bilateral). 432 TKRs were accounted for through follow up. 39 TKRs in 31 pts were lost to follow-up representing 8% who had a mean KSS of 176 at 10 yr f/u. Mean f/u time period was 16.4 yrs (range 15.1–18.5 yrs). Average age at f/u was 81 yrs. 11 TKR had been revised for aseptic loosening. 19 TKRs in 19 patients had had revision of femoral/tibial components for any reason. A further 7 TKRs had undergone polyethylene insert exchange leaving an overall revision rate of 9% or 91% survival without revision. Survivorship without aseptic loosening was 96% (95%CI of 91.9–98.1%) at up to 18 years. A competing risks analysis was undertaken in order to avoid overestimation of survivorship adjusted for the competing risk of death within the study group. This analysis estimated a cumulative risk of revision for aseptic loosening at 18 years of 4.5%. Mean KSS was 176 (SD 21.5). Mean range of movement was 113 degrees of flexion.

Objective

The aim of this study was to determine the correlation between body weight and fracture union for displaced intracapsular fracture neck of femur treated by closed reduction and internal fixation.

Patients and methods

A total of 197 patients with displaced intracapsular fracture of neck of femur, all of whom have been treated with closed reduction and internal fixation, were studied. The mean age was 71 years and 79% were female. Patients were followed up until fracture endpoint (union or non-union) with minimum follow up of 200 days.

Introduction and aim There are different methods of internal fixation of intracapsular fractures of the hip of which three AO screws is one of the more popular. There have been no evidence-based publications describing the optimal position for screws. The aim was to establish the relationship between screw position and angle, and subsequent failure of union.

Method Using computer software we studied the position of AO screws in 395 consecutive patients inserted between 1989 and 2003. Follow-up was prospective and for a minimum of 100 days. The diagnosis of non-union was made clinically and confirmed radiographically.

Results The mean age of our population was 73.9 years (range 22–96). Eighty-six (21.8%) were male. Three hundred and twenty seven (82.8%) came from their own home. The mean time between fall and surgery was 37.0 hours and between admission and surgery 20.9 hours. The mean length of radiographic follow-up for those fractures that did not develop non-union was 454 days (range 94–1898). Of the 395 patients 242 (61%) fractures united and 153 (39%) fractures suffered non-union.

Radiographic analysis suggests that the position of the screws on the AP view (superior, middle, inferior or spread) did not alter the outcome significantly. However three factors were related to lower risk of non-union on the lateral view: the closer the middle screw to the center of the head (p< 0.04), the more anterior the anterior screw (p< 0.008), and the greater the ‘spread’ between the anterior and posterior screws (p< 0.005).

Conclusions We conclude that to reduce the risk of non-union with screw fixation of intracapsular fractures of the hip, in the lateral view the middle screw must be positioned as close to the centre of the femoral head as possible, and the anterior and posterior screws achieve maximal spread.

Study Design: A retrospective study of the clinical outcome of patients with lumbar discogenic pain with Modic changes on MRI prior to intradiscal steroid injection.

Objectives: To determine whether the clinical outcome of patients with discogenic back pain who underwent intradiscal steroid injection could be predicted from MRI Modic changes.

Methods: The pre-operative scans were studied by two senior spinal surgeons. The lumbar vertebral end-plate changes were then classified according to the method described by Modic. The intra- and inter-observer ratings were satisfactory.

Subjects: 40 patients with discogenic back pain were recruited in this study. The mean age was 43.6 years (23 to 72 years). The male to female ratio was 1 to 1.

Outcome Measures: The clinical outcomes at six months post-intradiscal steroid injection were correlated with the Modic changes. The clinical outcomes were assessed using visual analogue scores for back pain as well as Oswestry disability index (ODI). At least a 2-point improvement in visual analogue score and a 20-point improvement in ODI were required to indicate significant symptomatic relief.

Results: We found that in those patients without Modic changes there was improvement of the low back pain in 9% (1/11). In those with Modic I changes there were significant relief in 64% (9/14), moderate relief in 29% (4/14) and no relief in 7% (1/14). In those with Modic II changes there were significant relief in 27% (4/15), moderate relief in 27% (4/15) and no relief in 47% (7/15). There were no cases with Modic III changes.

Conclusions: Previous studies on intradiscal steroid injections have shown variable results. Two prospective double-blind clinical trials, using intradiscal steroids, identified no significant benefit or improvement in the clinical outcome. Our results however suggest that patients with Modic I changes on MRI are most likely to benefit from intradiscal steroid injection in the short term.
