Elevated blood cobalt secondary to metal-on-metal (MoM) hip arthroplasties has been shown to be a risk factor for developing cardiovascular complications including cardiomyopathy. Published case reports document cardiomyopathy in patients with blood cobalt levels as low as 13µg/l. Clinical studies have found conflicting evidence of cobalt-induced cardiomyopathy in patients with MoM hips. The extent of cardiovascular injury, measured by global longitudinal strain (GLS), in patients with elevated blood cobalt levels has not previously been examined.

Sixteen patients with prospectively collected blood cobalt ion levels above 13µg/l were identified and matched with eight patients awaiting hip arthroplasty with no history of cobalt implants. Patients underwent echocardiogram assessment including GLS.

Patients with MoM hip arthroplasties had a mean blood cobalt level of 29µg/l compared to 0.01µg/l in the control group. There was no difference or correlation in EF, left ventricular (LV) end systolic dimension, LV end diastolic dimension, fractional shortening, ventricular wall thickness or E/e’ ratio. However, GLS was significantly reduced in patients with MoM hip arthroplasties compared to those without (−15.2% v −18%, (MoM v control) p= 0.0125). Pearson correlation demonstrated that GLS is significantly correlated with blood cobalt level (r= 0.8742, p=0.0009).

For the first time, this study has demonstrated reduced cardiac function in the presence of normal EF as assessed by GLS in patients with elevated cobalt above 13µg/l. As GLS is a more sensitive measure of systolic function than EF, routine echocardiogram assessment including GLS should be performed in all patients with MoM hip arthroplasties and elevated blood cobalt.

Clinical studies have found conflicting evidence of cobalt-induced cardiomyopathy in patients with MoM hips. Global longitudinal strain (GLS) is an echocardiography measurement known to be more sensitive than ejection fraction at diagnosing early cardiomyopathies. The extent of cardiovascular injury, as measured by GLS, in patients with elevated blood cobalt levels has not previously been examined.

Sixteen patients with documented blood cobalt ion levels above 13µg/l were identified from a regional arthroplasty database. They were matched with eight patients awaiting hip arthroplasty with no history of cobalt implants. All patients underwent electrocardiogram and echocardiogram assessment for signs of cardiomyopathy including GLS.

Patients with MoM hip arthroplasties had a mean blood cobalt level of 29µg/l (495nmol/l) compared to 0.01µg/l (0.2nmol/l) in the control group. There was no difference or correlation in ejection fraction (EF), left ventricular (LV) end systolic dimension, LV end diastolic dimension, fractional shortening, ventricular wall thickness or E/e’ ratio. However, GLS was significantly reduced in patients with MoM hip arthroplasties compared to those without (−15.2% v −18%, (MoM v control) p= 0.0125). Pearson correlation demonstrated that GLS is significantly correlated with blood cobalt level (r= 0.8742, p=0.0009).

Acetabular retroversion is a recognised cause of hip impingement. Pelvic tilt influences acetabular orientation and is known to change in different functional positions. While previously reported in patients with developmental dysplasia of the hip, positional changes in pelvic tilt have not been studied in patients with acetabular retroversion.

We retrospectively analysed supine and standing AP pelvic radiographs in 22 patients with preoperative radiographs and 47 with post-operative radiographs treated for symptomatic acetabular retroversion. Measurements were made for acetabular index (AI), lateral centre-edge angle (LCEA), crossover index, ischial spine sign, and posterior wall sign. The change in pelvic tilt angle was measured both by the Sacro-Femoral-Pubic (SFP) angle and the Pubic Symphysis to Sacro-iliac (PS-SI) Index.

In the supine position, the mean calculated pelvic tilt angle (by SFP) was 1.05° which changed on standing to a pelvic tilt of 8.64°. A significant increase in posterior pelvic tilt angle from supine to standing of 7.59° (SFP angle) and 5.89° (PS –SI index) was calculated (p<0.001;paired t-test). There was a good correlation in pelvic tilt change between measurements using SFP angle and PS-SI index (rho .901 in pre-op group, rho .815 in post-op group). Signs of retroversion were significantly reduced in standing x-rays compared to supine: Crossover index (0.16 vs 0.38; p<0.001) crossover sign (19/28 vs 28/28 hips; p<0.001), ischial spine sign (10/28 hips vs 26/28 hips; p<0.001) and posterior wall sign (12/28 vs 24/28 hips; p<0.001).

Posterior pelvic tilt increased from supine to standing in patients with symptomatic acetabular retroversion, in keeping with previous studies of pelvic tilt change in patients with hip dysplasia. The features of acetabular retroversion were much less evident on standing radiographs. The low pelvic tilt angle in the supine position is implicated in the appearance of acetabular retroversion in the supine position. Patients presenting with symptoms of hip impingement should be assessed by supine and standing pelvic radiographs so as not to miss signs of retroversion and to assist with optimising acetabular correction at the time of surgery.

National guidelines encourage the use of total hip arthroplasty (THA) to treat intracapsular neck of femur fractures. There have been no population based studies appraising the surgical outcomes for this indication across an entire population. This study aims to calculate the complication rates for THA when performed for a fractured neck of femur and compare them to THA performed for primary osteoarthritis in the same population.

The Scottish Arthroplasty Project identified all THAs performed in Scotland for neck of femur fracture and osteoarthritis between 1st of January 2009 and 31st December 2014. Dislocation, periprosthetic infection and revision rates at 1 year were calculated.

The rate of dislocation, periprosthetic infection and revision at 1 year were all significantly increased among the fracture neck of femur cohort. In total 44046 THAs were performed, 38316 for OA and 2715 for a neck of femur fracture. 2.1% of patients (n=57) who underwent a THA for a neck of femur fracture suffered a dislocation in the 1st year postoperatively, compared to 0.9% (n=337) when the THA was performed for osteoarthritis. Relative Risk of dislocation: 2.4 (95% C.I. 1.8077–3.1252, p value <0.0001). Relative Risk of infection: 1.5 (95% C.I. 1.0496–2.0200, p value 0.0245) Relative Risk of revision: 1.5 (95% C.I. 1.0308–2.1268, p value 0.0336).

This is the first time a dislocation rate for THA performed for a neck of femur fracture has been calculated for an entire population. As the number of THAs for neck of femur fracture increases this dislocation rate will have clinical implications.

Since the introduction of national guidelines in 2009 encouraging the use of total hip arthroplasty (THA) to treat intracapsular neck of femur fractures there has been no population-based studies into the surgical outcomes for this indication. This study aims to calculate the complication rates for THA when performed for a fractured neck of femur and compare them to THA performed for primary osteoarthritis in the same population.

Purpose: To compare the postoperative cerebrospinal fluid (CSF) leak rate in two consecutive cohorts of patients undergoing intradural spinal tumour excision with 2 different dural closure techniques.

Methods and Result: Data for this study was collected retrospectively from case notes. Between January 1994 and December 2001 forty seven intradural (thirty two extramedullary and fifteen intramedullary) spinal tumour excisions were performed. The dural incision was closed using vicryl 6.0. Operations of nine patients in this group (19 %) were complicated by CSF leak. From 2002 onwards the closure method for dural incisions was changed to single layer continuous prolene 6.0 suture in conjunction with a check valsalva manoeuvre prior to closure of wound incision in an attempt to reduce the incidence of CSF leak. Fifty three (thirty three extramedullary and twenty intramedullary) patients underwent intradural spinal tumour excision between January 2002 and October 2008. Three (5.7%) patients developed cerebrospinal fluid leak and one (1.9%) patient developed a pseudomeningocele post operatively. All four patients were subsequently managed with a lumbar drain. In both groups of patients good exposure of the proximal and distal aspect was achieved prior to dural closure. Statistical analysis comparing the outcome of both groups was performed using Fisher’s exact test – p values calculated were 0.0381 (one-tailed) and 0.0618 (two-tailed).

Conclusion: Single layer continuous prolene suture in conjunction with a check valsalva manoeuvre is superior to vicryl alone for the closure of the dura post intradural spinal tumour excision. The reduction in the leak rate may be due to the valsalva manoeuvre itself rather than the different suture material. Along with good exposure to the proximal and distal aspect of the incision we feel that this can reduce post operative cerebrospinal fluid leak in intradural spinal tumour excision operations.

Study Design: Retrospective case review.

Objective: To assess factors influencing functional outcome, recurrence and survival following surgery for intradural spinal tumours

Subjects: Between January 1994 and December 2001, 115 patients had surgery for intradural spinal tumours: 76 extramedullary (48 male, mean age 54 years): 39 intramedullary (22 male, mean age 44 years).

Outcome measures and analysis: Functional outcome: Frankel scale. Recurrence: new symptoms with tumour growth. Death: post-operative or disease progression. Univariate and multivariate analysis was performed to identify features predicting post-operative functionally useful Frankel scale (4–5), recurrence and survival.

Results: 64 extramedullary tumours were excised, 12 debulked. 21 intramedullary tumours were excised, 13 debulked, 5 biopsied. Commonest tumours: meningioma, schwannoma, ependymoma, astrocytoma. 14 intramedullary tumours received radiotherapy. Mean follow-up was 45 months (range 4–117 months). There were 12 recurrent tumours. There were 8 deaths (2 post-operative, 6 disease-progression). 23% had complications (CSF leak, meningitis, wound infection/dehiscence). Functionally 96% of extramedullary tumours were unchanged/improved, 82% of intramedullary tumours were unchanged/improved. Multivariate analysis demonstrated that recurrence (Odds Ratio 28.2; 95% Confidence Interval 2.3–342.4) was the only significant factor influencing survival for intramedullary tumours. No factors investigated predicted functional outcome or recurrence in intramedullary tumours. No factors predicted any outcome in extramedullary tumours.

Conclusions: Our results were comparable to other studies. The two cases of MRSA meningitis (one death, one paralysis) reflect the growing problem of MRSA in neurosurgical units. Recurrence predicted poor survival in intramedullary tumours.
