A: - determine the incidence of intraoperative periprosthetic femoral fracture (PFF)

B: - determine the incidence of, and the reoperation rate for postoperative PFF

When using either CC or PTS femoral components.

Retrospective review of a consecutive series of 11,018 THAs over a ten-year period. All PFFs were identified using regional radiograph archiving and electronic care systems.

Of the 11,018 THAs 4,952 were CC and 6,066 were PTS. Between groups, age, sex, and BMI did not differ.

A: - 55 (0.5%) had an intraoperative PFF. 44 CC and 11 PTS (p<0.001). 3 patients in each group had a femoral shaft fracture, remaining fractures were either the calcar (20 CC and 2 group) or the greater trochanter (11 CC and 6 PTS).

B: - 91(0.8%) sustained a postoperative PFF. Of those 15 were managed conservatively, 15 were revised and 61 (80.3%) had an ORIF. The CC group had both a lower overall rate of postoperative PFFs (0.7% (36/4,952) vs 0.9% (55/6,066); p = 0.341), and a lower rate of return to theatre (0.4% (22/4,952) vs 0.9% (54/6,066); p = 0.005). 1.3% of male PTS (36/2,674) had a reoperation compared to 0.3% of male CC (7/2,121) (p<0.001).

With regard to stem fracture there were none in the Corail group and 5 in the Exeter group. Of these 2 were sub trunnion and 3 were basal neck.

A: - There were significantly more intraoperative PFFs with CC 44 (0.8%) than PTS 11 (0.2%). However, the majority of fractures were either of the calcar or greater trochanter with no impact on early recovery or one year Oxford scores.

B: - Male PTS were five times more likely to have a reoperation for postoperative PFF. Females had the same incidence of reoperation with either component type.

There were 5 stem fractures in the Exeter group and none in the Corail.

These results represent robust estimates, which are likely to be more accurate than revision only studies typically generated from registry data.

Standard practice in revision total hip replacement (THR) for periprosthetic fracture (PPF) is to remove all cement from the femoral canal prior to implantation of a new component. This can make the procedure time consuming and complex.

Since 1991 it has been our practice to preserve the old femoral cement where it remains well fixed to bone, even if the cement mantle is fractured, and to cement a new component into the old mantle.

We have reviewed the data of 48 consecutive patients, treated at our unit between 1991 and 2009, with a first PPF around a cemented primary THR stem where a cement in cement revision was performed. 8 hips were revised to a standard length stem, 39 hips to a long stem & 1 patient had the same stem reinserted. All fractures were reduced and held with cerclage wires or cables and four had supplementary plate fixation.

Full clinical and radiographic follow up was available in 38 patients & clinical or radiographic follow up in a further 6 patients. The other 4 patients. without follow up but whose outcome is known, have suffered no complications and are pain free. Of the remaining 44 patients, forty-two went on to union of the fracture and two have required further surgery for non-union. One patient has ongoing undiagnosed hip pain.

Our long term experience with cement in cement revision for periprosthetic femoral fractures shows that this is a viable technique with a low complication rate and high rate of union (95%) in what is generally regarded as a very difficult condition to treat.

Introduction

Varus positioning of cemented ‘composite beam’ stems is associated with increased risks of aseptic loosening and stem fracture. We investigated whether the incidence of varus malalignment of the Exeter polished, double taper design in a multicentre prospective study adversely affected outcome after total hip replacement (THR).

Materials & Methods

A multicentre prospective study of 1189 THR was undertaken to investigate whether there is an association between surgical outcome and femoral stem malalignment. The primary outcome measure was the change in the Oxford hip score (OHS) at five years. Secondary outcomes included the rate of dislocation and revision, stem subsidence, quality of cementing. 938 (79.89%) were followed-up at five years.

Purpose

One of the drawbacks of cemented total hip arthroplasty (THA) is aseptic loosening after long period, major reason for which is bioinertness of PMMA bone cement. To improve longevity of THA, interface bioactive bone cement (IBBC) technique which is characterized with smearing hydroxyapatite (HA) granules just before cementation has been used in our institute.

Objective

Smooth-surfaced triple-tapered Titanium-alloy stem (T) and Exeter stem (E) have been used consecutively in the different period. Objective of the present study was thoroughly comparing two stems clinically and radiologically.

Aims. This study evaluates risk factors influencing fracture characteristics for postoperative periprosthetic femoral fractures (PFFs) around cemented stems in total hip arthroplasty. Methods. Data were collected for PFF patients admitted to eight UK centres between 25 May 2006 and 1 March 2020. Radiographs were assessed for Unified Classification System (UCS) grade and AO/OTA type. Statistical comparisons investigated relationships by age, gender, and stem fixation philosophy (polished taper-slip (PTS) vs composite beam (CB)). The effect of multiple variables was estimated using multinomial logistic regression to estimate odds ratios (ORs) with 95% confidence intervals (CIs). Surgical treatment (revision vs fixation) was compared by UCS grade and AO/OTA type. Results. A total of 584 cases were included. Median age was 79.1 years (interquartile range 72.0 to 86.0), 312 (53.6%) patients were female, and 495 (85.1%) stems were PTS. The commonest UCS grade was type B1 (278, 47.6%). The most common AO/OTA type was spiral (352, 60.3%). Metaphyseal split fractures occurred only with PTS stems with an incidence of 10.1%. Male sex was associated with a five-fold reduction in odds of a type C fracture (OR 0.22 (95% CI 0.12 to 0.41); p < 0.001) compared to a type B fracture. CB stems were associated with significantly increased odds of transverse fracture (OR 9.51 (95% CI 3.72 to 24.34); p < 0.001) and wedge fracture (OR 3.72 (95% CI 1.16 to 11.95); p = 0.027) compared to PTS stems. Both UCS grade and AO/OTA type differed significantly (p < 0.001 and p = 0.001, respectively) between the revision and fixation groups but a similar proportion of B1 fractures underwent revision compared to fixation (45.3% vs 50.6%). Conclusion. The commonest fracture types are B1 and spiral fractures. PTS stems are exclusively associated with metaphyseal split fractures, but their incidence is low. Males have lower odds of UCS grade C fractures compared to females. CB stems have higher odds of bending type fractures (transverse and wedge) compared to PTS stems. Cement-in-cement revision of the femoral component represents a widely practiced technique for a variety of indications. In this study we compare the clinical and radiological outcomes of two polished tapered stems. From our prospectively collated database we identified all patients undergoing cement-in-cement revision from January 2005 – 2013 who had a minimum of two years follow-up. All cases were performed by the senior author using either an Exeter short revision stem or the C-stem AMT high offset No 1. Patients were followed-up annually with clinical and radiological assessment. Ninety-seven patients matched the inclusion criteria. There were 50 Exeter and 47 C-stem AMT components. There were no significant differences between the patient demographics in either group. Mean follow-up was 9.7 years. A significant improvement in OHS, WOMAC and SF-12 scores was observed in both cohorts. Leg lengths were significantly shorter in the Exeter group, with a mean of -4mm in this cohort compared to 0mm in the C-stem AMT group. One patient in the Exeter group had early evidence of radiological loosening. In total, 16 patients (15%) underwent further revision of the femoral component (seven in the C-stem AMT group and nine in the Exeter group). No femoral components were revised for aseptic loosening. There were two cases of femoral component fracture in the Exeter group. Our series shows promising long-term outcomes for the cement-in-cement revision technique using either the Exeter or C-stem AMT components. These results demonstrate that cement-in-cement revision using a double or triple taper-slip design is a safe and reliable technique when used for the correct indications

Cemented total hip replacement (THR) provides excellent outcomes and is cost-effective. Polished taper-slip (PTS) stems demonstrate successful results and have overtaken traditional composite-beam (CB) stems. Recent reports indicate they are associated with a higher risk of postoperative periprosthetic femoral fracture (PFF) compared to CB stems. This study evaluates risk factors influencing fracture characteristics around PTS and CB cemented stems. Data were collected for 584 PFF patients admitted to eight UK centres from 25/05/2006-01/03/2020. Radiographs were assessed for Unified Classification System (UCS) grade and Arbeitsgemeinschaft für Osteosynthesefragen/Orthopaedic Trauma Association (AO/OTA) type. Statistical comparisons investigated relationships by age, gender, and stem fixation philosophy (PTS versus CB). The effect of multiple variables was estimated using multinomial logistic regression to estimate odds ratios (OR) with 95% confidence intervals (CI). Median (IQR) age was 79.1 (72.0–86.0) years, 312 (53.6%) patients were female, and 495 (85.1%) stems were PTS. The commonest UCS grade was type B1 (278, 47.6%). The commonest AO/OTA type was spiral (352, 60.3%). Metaphyseal-split fractures occurred only with PTS stems with an incidence of 10.1%. Aims

The aim of this study was to describe services available to patients with periprosthetic femoral fracture (PPFF) in England and Wales, with focus on variation between centres and areas for care improvement.

Methods

This work used data freely available from the National Hip Fracture Database (NHFD) facilities survey in 2021, which asked 21 questions about the care of patients with PPFFs, and nine relating to clinical decision-making around a hypothetical case.

Aims

Periprosthetic hip fractures (PPFs) after total hip arthroplasty are difficult to treat. Therefore, it is important to identify modifiable risk factors such as stem selection to reduce the occurrence of PPFs. This study aimed to clarify differences in fracture torque, surface strain, and fracture type analysis between three different types of cemented stems.

Methods

We conducted biomechanical testing of bone analogues using six cemented stems of three different types: collarless polished tapered (CPT) stem, Versys Advocate (Versys) stem, and Charnley-Marcel-Kerboull (CMK) stem. Experienced surgeons implanted each of these types of stems into six bone analogues, and the analogues were compressed and internally rotated until failure. Torque to fracture and fracture type were recorded. We also measured surface strain distribution using triaxial rosettes.

Aims

Periprosthetic proximal femoral fractures (PFFs) are a major complication after total hip arthroplasty (THA). Health status after PFF is not specifically investigated. The aim of this study is to evaluate the health status pattern over two years after sustaining a PFF.

Methods

A cohort of patients with PFF after THA was derived from the Brabant Injury Outcomes Surveillance (BIOS) study. The BIOS study, a prospective, observational, multicentre follow-up cohort study, was conducted to obtain data by questionnaires pre-injury and at one week, and one, three, six, 12, and 24 months after trauma. Primary outcome measures were the EuroQol five-dimension three-level questionnaire (EQ-5D-3L), the Health Utility Index 2 (HUI2), and the Health Utility Index 3 (HUI3). Secondary outcome measures were general measurements such as duration of hospital stay and mortality.

Recent NICE guidance recommends use of a well proven cemented femoral stem for hip hemiarthroplasty in management of fractured neck of femur. The Exeter Trauma Stem (ETS) has been designed based on the well proven Exeter hip stem. It has a double taper polished stem design, proclaimed to share geometry and surface finish with the Exeter hip. This study investigated the surface finish of the two stems in order to investigate the hypothesis that they were different. Two ETS and two Exeter stems were examined using a profilometer with a sensitivity of one nanometer. Macroscopic visual inspection showed that the two Exeter stems had significantly smoother surface finish than the ETS stems. The roughness average (RA) values on the ETS stems were approximately an order of magnitude higher than those of the Exeter stems, mean of 0.235μm compared with 0.025μm (p<0.0001). This difference in surface finish has implications for the biomechanical functioning of the stem. Previous change of the Exeter stem to a matt surface-finish in 1976 resulted in a significant increase in stem failure rates and an understanding of the importance of the polished surface-finish in order to function within a taper-slip philosophy. By changing the surface finish in the ETS stem, longevity of the implant may similarly be affected. Clinical results have yet to be published demonstrating this. We recommend the manufacturer reconsiders the surface finish of the ETS stem to ensure it functions as well as the Exeter primary stem with which it shares a design philosophy

Introduction. Varus malpositioning of femoral stems has been recognised as a poor prognostic feature with regard to loosening and failure of cemented composite beam implants, but there have been no published results of the long-term effect on taper-slip designs. Objectives. To determine the results of varus placement of a polished triple tapered femoral stem. Methods. We have prospectively analysed the results of 350 consecutive polished triple-tapered C-Stem implants performed on 322 patients between March 2000 and July 2004, using a standard posterior approach and Palacos-R cement. All patients underwent annual clinical and radiological review. Results. The average duration of follow-up in surviving patients is 140 months (120–172 months). There were 49 femoral implants in 49 patients implanted with more than 5 degrees of varus (14%) with respect to the long axis of the femur, and 14 in 14 patients in more than 5 degrees of valgus (4%). Alignment within 5 degrees of neutral was achieved in 82% of cases. The average age in the varus group was 69.2 years (41–92), which was higher than in the neutral group (67.4 years: 25–89), more patients were male (49% v 36%) but the average BMI was similar (29 v 28). The average duration of follow-up was the same. There was only one re-operation in the varus group, which was a revision for deep infection. There was one dislocation successfully treated by closed reduction and one acetabular component is currently loose. In the neutral group there have been four dislocations, one of which required application of a PLAD; five revisions for aseptic loosening of the acetabular component associated with high wear rates, with a further three currently being loose; four intra-operative, undisplaced fractures of the greater trochanter; two peri-prosthetic femoral shaft fractures treated by internal fixation and two temporary nerve palsies. There has been no aseptic loosening of the femoral components in either group. Subsidence of the femoral stem within the cement mantle was noted in 96% of the entire series. There was no significant difference between the groups with respect to the degree of subsidence. In the varus group 78% subsided less than 2mm compared to 75% in the neutral group. None of the femoral implants in either group subsided more than 4mm and none demonstrated evidence of aseptic loosening or negative bone remodelling. Conclusion. Varus implantation of the femoral component was more common in older, male patients, but there was no increased risk of subsidence or loosening at a minimum follow-up of ten years

Aims

Despite multiple trials and case series on hip hemiarthroplasty designs, guidance is still lacking on which implant to use. One particularly deficient area is long-term outcomes. We present over 1,000 consecutive cemented Thompson’s hemiarthroplasties over a ten-year period, recording all accessible patient and implant outcomes.

Methods

Patient identifiers for a consecutive cohort treated between 1 January 2003 and 31 December 2011 were linked to radiographs, surgical notes, clinic letters, and mortality data from a national dataset. This allowed charting of their postoperative course, complications, readmissions, returns to theatre, revisions, and deaths. We also identified all postoperative attendances at the Emergency and Outpatient Departments, and recorded any subsequent skeletal injuries.

While cemented THA has been considered the gold-standard, cementless THA has become a common and even preferred approach for younger individuals and those with acceptable bone stock. Which technology provides superior results and in which patients, however, remains controversial, and has not been systematically studied. The literature suggests that well-fixed cementless prostheses, and even composite beam cemented prostheses cause stress shielding and progressive osteopenia in the surrounding bone. This compromises and complicates subsequent surgery, particularly in young patients who are at risk for multiple revisions, and may increase the risk of periprosthetic fracture, component failure, and aseptic loosening. In contrast, polished tapered stems, by behaving according to a taper-slip philosophy, favorably load bone, converting shear stress into radially directed hoop stresses, through the medium of the surrounding visco-elastic cement and its resulting ability to creep. The C-Stem, (DePuy, Warsaw, Indiana, USA) a cemented triple tapered polished stem is the only stem with published data demonstrating positive remodeling of bone in as many as 20% of patients, and preservation of the critical proximal medial calcar bone. Additional anectdotal reports confirm these results. The long-term results of cemented stems, which necessarily involve the surviving youngerst cohort in any series, are outstanding. The tapered polished stem technology may improve these historical results, and, by limiting periprosthetic stress shielding and osteopenia, may revolutionize the application of cemented THA in younger patients for whom preservation of bone stock through favorable bone loading is so critical to sustained success of the primary and subsequent revision surgeries

The use of polished femoral implants employing the taper-slip philosophy now dominates the cemented portion of the hip arthroplasty market in the United Kingdom. Despite this fact, there have been very few published or presented series reporting the medium to long-term results of double tapered implants and only one previously reported series looking at the results of the triple tapered stem. We present the results of 500 consecutive polished triple tapered C-stem femoral components performed in 455 patients. All operations were performed using a posterior appproach, with cemented all polyethylene acetabular components and the use of third generation femoral cementing techniques, restrictors, centralisers and Pala-cos-R cement. There were 282 female patients (62%) and 173 males (38%). The average age at the time of surgery was 68.3 years (23 to 92), with an average duration of follow-up of 71 months (36 to 112). 47 patients (51 hips) died during the follow-up period at an average of 54 months (1–87). There were 3 dislocations, 2 on one occasion and one twice, at an average of 4 years. There were 2 deep infections, one of which required revision, and 4 non-fatal pulmonary emboli. There were 3 undisplaced trochanteric cracks treated by cerclage wiring and 3 nerve palsies: 2 femoral (temporary) and one sciatic (permanent). There was only one case of significant heterotopic ossification (Brooker Class 4). The stem was neutral in 89% of cases, varus in 7% and valgus in 4%. Subsidence within the cement mantle occurred in 79% of cases, with an average of 0.87mm. 9 stems subsided 2 to 4mm but all stopped at 2 years and there was no further subsidence thereafter. One hip was revised for deep infection but none of the remaining implants demonstrated any progressive radiolucencies in any Gruen zones and none demonstrated any features suggestive of current or future loosening. There was no evidence of negative bone remodelling. The data was collected prospectively and the study is ongoing

Aims

The Exeter V40 cemented polished tapered stem system has demonstrated excellent long-term outcomes. This paper presents a systematic review of the existing literature and reports on a large case series comparing implant fractures between the Exeter V40 series; 125 mm and conventional length stem systems.

Methods

A systematic literature search was performed adhering to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) criteria. In parallel, we performed a retrospective single centre study of Exeter V40 femoral stem prosthetic fractures between April 2003 and June 2020.
