Introduction. We perform PAO via a modified Smith-Petersen approach. The purpose of this study was to investigate the result of PAO via a modified Smith-Petersen approach at a minimum 10-years' follow-up. Methods. We retrospectively reviewed 209 hips in 179 patients with acetabular dysplasia who underwent PAO with a modified Smith-Petersen approach from August 1995 to April 2010. Exclusion criteria were as follows: under 10 year follow-up, incomplete clinical or radiographic data. Harris hip score (HHS) was investigated preoperatively, at the time of most improvement and at the final follow-up for clinical evaluation. Tönnis classification was investigated preoperatively and at the final follow-up for evaluation of osteoarthritis. Center edge (CE) angle and acetabular roof obliquity (ARO) were investigated preoperatively, postoperatively and at the final-follow up for radiographic evaluation. Tönnis classification and radiographic parameters were investigated on anterior-posterior radiographs. Patients of conversion of PAO to total hip arthroplasty (THA) were investigated for preparing Kaplan Myer survival analysis. The Wilcoxon signed-rank test was used to compare changes in HHS and radiographic parameters between the preoperative and the postoperative values. Statistical significance was defined a priori as p < 0.05. Results. Eighty-seven hips in 79 patients (44.1%) were included in this study. 100 patients were excluded from this study. The average age of the patients at the time of surgery was 39 years (rang, 15 to 65 years) and the mean follow-up period was 12 years and 2 months (range, 10 years to 18 years and 3 months). The mean HHS improved from 74 points (range, 38 to 98 points) preoperatively to 95 points (range, 62 to 100 points) at the time of most improvement (p < 0.01) and decrease slightly to 89 points (range, 32 to 100 points) at final follow-up. Tönnis classification was as follows: grade 0 was 4 hips preoperatively and 2 hips at the final follow-up, grade 1 was 55 hips preoperatively and 50 hips at the final follow-up, grade 2 was 25 hips preoperatively and 24 hips at the final follow-up, grade 3 was 3 hips preoperatively and 11 hips at the final follow-up. The mean CE angle improved from 5° (range, −19 to 24°) preoperatively to 30° (range, 2 to 56°) postoperatively (p < 0.01) and increased 38° (range, 12 to 68°) at final follow-up. The mean ARO improved from 24° (range, 6° to 45°) preoperatively to 6° (range, −14° to 48°) postoperatively (p < 0.01) and increased to 12° (range, −24 to 45°) at final follow-up. THA was performed on 5 hips in 5 patients (5.7%) after PAO. The mean duration between PAO and THA was 9 years and 6 months (range, 1 year and 4 months to 15 years 4 months). Ten-year survival rate was 97 % with conversion THA as the end point. Discussion & Conclusion. Clinical data and radiographic parameter were improved in patients who underwent PAO satisfactory. PAO was instrumental as time-saving surgical treatment of symptomatic acetabular dysplasia or slightly osteoarthritis because of 97% survival rate at 10 years

The purpose of this study was to determine the complications after Bernese periacetabular osteomy (PAO) performed by one experienced surgeon using a minimally invasive modified Smith-Petersen approach. Between May 2012 and December 2015, 224 periacetabular osteotomies (PAO) in 201 patients were performed. The perioperative complications were retrospectively reviewed after reviewing clinical notes and radiographs. The mean age was 28.8 years with 179 females and 22 males. The most common diagnosis was acetabular dysplasia with some cases of retroversion. The average lateral centre edge (LCE) angle was 16.5°(−18–45) and mean acetabular index (AI) 16.79° (−3–50). Postoperatively the mean LCE angle was 33.1°(20–51.3) and mean AI 3.0°. (−13.5–16.6). There were no deep infections, no major nerve or vascular injuries and only one allogenic blood transfusion. Nine superficial wound infections required oral antibiotics and two wounds needed a surgical debridement. There was one pulmonary embolus and one deep vein thrombosis. Nine (4%) cases underwent a subsequent hip arthroscopy and three (1.3%) PAO's were converted to a total hip arthroplasty after a mean follow-up of 22 months (3–50). Lateral femoral cutaneous nerve dysaesthesia was noted in 64 (28.6%) PAO's. In 55 (24.5%) an iliopsoas injection of local anaesthetic and steroid for persistent iliopsoas irritation during the recovery phase was given. The minimally invasive modified Smith-Petersen approach is suitable to perform a Bernese periacetabular osteotomy with a low perioperative complication rate. Persistent pain related to iliopsoas is a not uncommon finding and perhaps under-reported in the literature

Background

The increasing desire to protect the periarticular structures led the need of a Tissue Sparing Surgery. The accesses most widely used are the direct-lateral approach and the postero-lateral one, both with patient in lateral decubitus. Aim: This accesses require however an incision of tendons and muscles even in their minimally invasive technique, so we looked for an approach that would wholly protect the periarticular structures and allow us not to revise our experience in patient positioning, preparation of the operating field and surgeon's position during surgery. Our intent was to leave the acquired knowledge unchanged and to preserve unaltered the anatomical landmarks that we had previously identified and consolidated for the correct positioning of the components.

Methods

We have used this approach in more than 180 cases of primary hip arthroplasty. Clinical control includes: Oxford Hip Score, VAS and X-Ray.

Complex spinal deformities can cause pain, neurological symptoms and imbalance (sagittal and/or coronal), severely impairing patients’ quality of life and causing disability. Their treatment has always represented a tough challenge: prior to the introduction of modern internal fixation systems, the only option was an arthrodesis to prevent worsening of the deformity. Then, the introduction of pedicle screws allowed the surgeons to perform powerful corrective manoeuvres, distributing forces over multiple levels, to which eventually associate osteotomies. In treating flexible coronal deformities, in-ternal fixation and corrective manoeuvres may be sufficient: the combination of high density pedicle screws and direct vertebral rotation revolutionized surgical treatment of scoliosis. However, spinal osteotomies are needed for correcting complex rigid deformities; the type of osteot-omy must be chosen according to the aetiology, type and apex of the deformity. When dealing with large radius deformities, spread over multiple levels and without fusion, multiple posterior column os-teotomies such as Smith-Petersen and Ponte (asymmetric, when treating scoliosis) can be performed, dissipating the correction over many levels. Conversely, the management of a sharp, angulated de-formity that involves a few vertebral levels and/or with bony fusion, requires more aggressive 3 col-umn osteotomies such as Pedicle Subtraction Osteotomies (PSO), Bone Disc Bone Osteotomies (BDBO) or Vertebral Column Resection (VCR). Sometimes the deformity is so severe that cannot be corrected with only one osteotomy: in this scenario, multilevel osteotomies can be performed

Purpose: Fractures of the anterior acetabular wall with preservation of the pelvic inlet are rare. These lesions were not noted or classiþed by Judet and Letournel in their classiþcation system Ð Ç In fractures of the anterior wall, the anterior part of the articular horseshoe breaks off with a major portion of the middle segment of the anterior column È. The ilio-inguinal approach was recommended for the surgical treatment of these fractures. Method : We have encountered two cases involving purely the anterior wall with preservation of the pelvic inlet, rather than the anterior wall fracture described by Judet and Letournel. We have identiþed only two other cases in the international literature. The recognition that these fractures were not as that described by Judet and Letournel was essential, as an alternative surgical approach was necessary for reconstruction. The ilioinguinal approach of Judet and Letournel is the technique of choice in anterior fractures, but provides only very limited potential for intra-articular manipulation through the line of fracture. The Smith-Petersen approach was practically the only feasible choice as intra-articular exposure is difþcult or impossible to obtain with the classical approaches used for anterior acetabular fractures. Results: Both of our cases were reconstructed via the Smith-Petersen approach. Postoperative review at 18 months demonstrated painless, fully mobile hips with evidence of radiological union of the fractures. Conclusion: We propose designating as ñfracture of the anterior columnñ all the anterior fractures described by Judet and Letournel (column and wall) that justify the ilioinguinal approach. We would reserve the term ñanterior wallñ for large anterior acetabular rim fractures, with the extension exclusively lateral to and below the pelvic brim. We recommend the use of a Smith-Petersen approach for reconstruction once this fracture pattern is identiþed

The Bernese periacetabular osteotomy (PAO) is a well-established procedure in the management of symptomatic hip dysplasia. The associated Smith-Petersen exposure offers excellent visualization of the acetabulum and control of acetabular osteotomy and mobilization. The traditional exposure of the true pelvis involves osteotomy of the iliac wing in order to mobilize the sartorial and inguinal ligament insertion. However, full osteotomy of the iliac spine may necessitate screw fixation if a relatively large segment of bone is included. A known complication with screw fixation of the iliac wing osteotomy involves failure of fixation and screw back out. Moreover, the screw may be irritative to the patient even in the setting of adequate fixation. A larger osteotomy may also injure the lateral femoral cutaneous nerve as it travels near the anterior superior spine. To minimize the risk of these potential complications, a wafer osteotomy may be used to develop a sleeve of tissue involving the sartorial insertion. Markings may be made so that the curvilinear incision is centered about the anterior-superior iliac spine (ASIS). The sartorial sleeve also mobilizes the entirety of the lateral femoral cutaneous nerve medially as it runs and branches to varying degrees in a fatty tissue layer in the tensor-sartorius interval directly beneath the subcutaneous layer, thereby affording protection throughout the procedure. When the ASIS is first osteotomized as a several millimeter-thick mobile fragment and reflected, the sartorius attachment to the mobile fragment of the wafer osteotomy may be preserved. Furthermore, the wafer osteotomy may be re-fixed to the stable pelvis during closure with simple heavy suture fixation alone, avoiding screw insertion or associated removal. Because only a wafer or bone is taken during the spine osteotomy, more bone is available at the ASIS for fixation of the mobile fragment after repositioning. Over the past years there has been considerable interest, debate and controversy over the role of surgical approaches for total hip replacements. The leading role belongs to appropriate direction and anatomical structures mobilization during surgical approach. We strictly follow the paradigm that surgical approach must enable gentle handling vessels, nerves, muscles and fascias structures create good viewing of target field with possibilities of free manipulations, give opportunity for widening at any level. This experimental study was performed to quantitatively comparesomethemostcommonlyusedsurgicalapproaches to hip joint. Kocher-Langenbeck, Smith-Petersen, Moore, direct lateral approaches were investigated on twenty men cadavers. All bodies had normal constitution, average age 60+−5 years, body weigh 75+−10 Kg, no previous surgical interventions at the hip joint area. For named approaches wound length (cm), wound depth (cm), angle of operating action (degree), tilting of operating axis (degree), wound’s inlet square (relative units), wound’s bottom square (relative units), accessibility zone’s square (percents) were registered. Direct measure and measuring on digital images of wounds using free version of UTHSCCA Image Tools for Windows v.2 were performed. Data stored to database for further analysis. The average angle of operating action for posterior approaches was 75+−7 degrees, for lateral approaches – 60+−8 degrees. Tilting of operating axis for all approaches was between 55–70 degrees and was in close relation of body’s position. As a rule, supine position for lateral approaches restricted tilting of operating axis to 40–45 degrees, reducing wound’s review. The wound’s inlet square to wound’s bottom square ratio that characterizes the usefulness of approach was the worst (8–12%) for Smith-Petersen and the best for Kocher-Langenbeck (16–18%) approach. This feature for direct lateral approach was 13–15%. Some tricks (wide capsule detaching, retracting the gemelli and obturator internus and the tendon of the piriformis, for example), can increase the useful square of wound’s bottom, but not more than 3–5%.The data received is slightly disappointing. The efficiency of widely using approaches is too low. The risk of nerve palsy or injury appear to be higher on the direct lateral approaches, however, there were no significant differences when comparing this risk nerve by nerve for both posterior and lateral approaches, in particular for the sciatic nerve. One can reduce the operating trauma by using minimal invasive technique, but this approach demands special instrument sets and must be under thoroughly investigation to clarify the questions of heterotopic bone formation, hip prostheses surviving etc

Improvement in coverage achieved by double or triple osteotomies is limited by the size of the acetabular fragment and the ligaments connected with the sacrum. Correction is achieved with the notable asymmetry of the pelvis. In periacetabular Ganz osteotomy (PAO) the acetabular fragment has no connection with the sacrum, which creates enormous possibilities for correction, leaving the pelvic ring untouched. The aim of the study is to present our experience and early results of using PAO in the treatment of hip dysplasia in adolescents and young adults who were previously treated operatively in childhood, and to find the technical and clinical impact of previous operations on our Results: In the years 1998–2005 262 periacetaubular osteotomies were performed in our hospital. All the patients were operated by one surgeon (JC). From this group 41 patients (43 hips) had previously been operated in childhood for the treatment of hip dysplasia. The previous treatment consisted of: open reduction in 10 hips, DVO in 14 hips, pelvic osteotomy (Salter, Dega, Chiari) in 8 hips, combined: open reduction+DVO+pelvic osteotomy in 10 hips, greater trochanter transfer in 3 hips, bone lengthening in 4 hips, acetabular cyst removal in 1 hip. The age at the primary operation ranged from 1–20.. The follow-up period ranged from 1–7,5 ys av. 2 ys. In 31 hips the Smith-Petersen, and in 12 hips ilioinguinal approach were performed. Methods. In clinical pre-op, and post-op examination the following factors were considered: pain, limping, Trendelenburg sign, range of motion, length discrepancy. Radiographic pre-op and post-op examination consisted of: AP view of the pelvis, false profile and AP view with the leg in abduction. Classic and anterior CE angle were measured. During the last examination Harris Hip Score was used. Results. Flexion slightly decreased from pre-op. 90–140° (av. 118°) to 80–130° (av. 104°) post-op. abduction remained unchanged 15–60° (av. 40°) and 15–60° (av. 40°) respectively, adduction slightly increased 15–40° (av. 31) and 20–50° (av. 33°). The range of rotation did not change after the operation. The sign of Trendelenburg was found in 27 hips before operation and in 8 hips post-op. Pain (acetabular rim syndrome) was found in 40 hips before operation and in 4 hips after the surgery. Either classic or anterior CE angle increased after the surgery in all cases from −14° to 34° and from −10° to 35° respectively. We had a rather low complication rate. But in cases previously operated and in males we strongly recommend Smith-Petersen approach extended into the frontal part of the hip for a better exposure in the scarred and hardened tissues. We find the technique of PAO as a safe, and effective tool for treating hip joint pathology increasing treatment possibilities for hip joint preservation

Aims

Hyaline cartilage has a low capacity for regeneration. Untreated osteochondral lesions of the femoral head can lead to progressive and symptomatic osteoarthritis of the hip. The purpose of this study is to analyze the clinical and radiological long-term outcome of patients treated with osteochondral autograft transfer. To our knowledge, this study represents a series of osteochondral autograft transfer of the hip with the longest follow-up.

Methods

We retrospectively evaluated 11 hips in 11 patients who underwent osteochondral autograft transfer in our institution between 1996 and 2012. The mean age at the time of surgery was 28.6 years (8 to 45). Outcome measurement included standardized scores and conventional radiographs. Kaplan-Meier survival curve was used to determine the failure of the procedures, with conversion to total hip arthroplasty (THA) defined as the endpoint.

Direct anterior approach (DAA) is one of the best way to the hip joint for prevention of post-operative dislocation. We have applied this method as minimum invasive surgery (MIS) to more than two hundred developmental dysplastic hip of Japanese patients in total hip arthroplasty (THA) and there is no post-operative dislocation within three years of last observation carried forward (LOCF). The reason of this benefit is derived from the accuracy of cup positioning and keeping good muscle balance. But the learning curve is very important and some technical pitfalls are there in this approach. We have chosen thirty four patients that the duration of operating time more than one and half hours and loss of blood more than five hundred gram in hour series. The most important factors of the difficulties are the combinations of shortening of femoral neck, especially Perthes like deformity of developmental deformities of the hip joint (DDH) and widening of pelvic bone for the reason of insufficiency working spaces and the difficulties of broaching insertion (8/34). The second factor is the contracture of hip and knee joints combinations for the difficulties of lift up the proximal femur as broaching stem (3/34). The obesity, Body Mass Index (BMI) above thirty is not the reason of difficulties of women in our series. DAA can be extended to Smith-Petersen approach and very useful technique for primary THA in Japanese dysplastic hip patients

Introduction. Slipped upper femoral epiphysis (SUFE) predominantly affects males in early adolescence. Severe slips occur with an estimated incidence of 1 per 100,000 children. Controversy exists over optimal treatment of severe slips with debate between in-situ fixation versus corrective surgery. We report on our management of a large series of such patients using a sub-capital cuneiform osteotomy. Patients/Methods. Between 2001–2011, 57 patients (35M: 22F) with an average age of 13.1 years were referred to our tertiary referral institution with a severe slip. This was defined as an epiphyseal-metaphyseal displacement greater than half the width of the femoral neck on a Billing lateral radiograph. The affected limb was rested in slings and springs prior to corrective surgery which was performed via an anterior Smith-Petersen approach. More recently our series has included pre-operative MRI to look for avascular necrosis of the epiphysis. Data was analysed to compare complication rate with time to surgery and surgeon's experience. Results. 50 patients (88%) were complication free at a mean follow up of 7 years and their average Oxford Hip Score was 44 (range: 37–48) and median visual analogue pain score was 0 (out of 10). 6 patients (10.5%) developed AVN requiring further surgery and one patient developed chondrolysis (1.8%) but declined further intervention. Complication rate was closely related to time to surgery with 100% AVN rate for surgery within 1 weeks of symptoms onset or 5–7% thereafter. A learning curve was identified with fewer complications seen as the series progressed. Discussion/Conclusion. This is a technically demanding operation with variable outcomes reported in the literature. We have demonstrated good results but feel it should be reserved for specialist centres exposed to higher volumes of these rare cases

Aims

Although there are various pelvic osteotomies for acetabular dysplasia of the hip, shelf operations offer effective and minimally invasive osteotomy. Our study aimed to assess outcomes following modified Spitzy shelf acetabuloplasty.

Methods

Between November 2000 and December 2016, we retrospectively evaluated 144 consecutive hip procedures in 122 patients a minimum of five years after undergoing modified Spitzy shelf acetabuloplasty for acetabular dysplasia including osteoarthritis (OA). Our follow-up rate was 92%. The mean age at time of surgery was 37 years (13 to 58), with a mean follow-up of 11 years (5 to 21). Advanced OA (Tönnis grade ≥ 2) was present preoperatively in 16 hips (11%). The preoperative lateral centre-edge angle ranged from -28° to 25°. Survival was determined by Kaplan-Meier analysis, using conversions to total hip arthroplasty as the endpoint. Risk factors for joint space narrowing less than 2 mm were analyzed using a Cox proportional hazards model.

The pathologies of the hip (epiphisiolysis, Perthes, congenital dysplasia) cause invalidating outcomes in young patients. In the years many authors searched for the golden standard if total hip arthroplasty is necessary. From Smith-Petersen to McKee, to Amstutz the saving of the femoral neck has been the main objective, with the longevity of the system. Therefore the combination metal-metal has been used for articular components, because the friction of metallic components creates sub-micron metallic particles causing only minimal foreign body reactions andvery low wear rates. Our study group (Arthro Surgery Group) has implanted, from April 2003, 28 cementless resurfacing hip prosthesis with metal-metal articular components. The average follow-up has been of 6 months (from 3 months to 1 year). In all cases, the Harris Hip Score has been good or excellent (from 77 to 100), and radiographic exams didn’t show signs of radiolucence or resorption. This type of prosthesis allows moreover an minimum-invasive approach to the hip and a second line of defense in case of revision

Introduction. The anterior approach to primary total hip arthroplasty is an unfamiliar approach to most surgeons that is considered to be minimally invasive based on the premise that there is less soft tissue damage and quicker post-operative recovery time. We present our experience of using the anterior approach exclusively by a single surgeon at multiple surgical centers for a period of 3.5 years. Method. 709 consecutive patients undergoing primary hip arthroplasties from 8/2007 to 12/2010 through a direct anterior approach were performed by single surgeon with extensive training in the approach. The procedure was performed with the patient supine on a fracture table (Trumph arch table extension) through an anterior approach as described by Dr. Joel Matta through a Smith-Peterson interval. Intra-operative data and complications were collected prospectively and to avoid missing any complications, electronic medical records (Alteer) were retrospectively reviewed. Results. The demographic characteristics of patients are listed in Table 1 and intra-operative data collected presented in Table 2. The overall major complication rate was 2.81% (19/709). Overall revision rate due to any cause was 1.83% (13/709). Wound related complications were 6.67% which included any type of drainage noted during post op clinic visits, wound dehiscence, stitch abscesses, or superficial infections requiring irrigation and debridement. Discussion. The anterior approach through a modified Smith-Petersen approach provides preservation of the major hip muscles to facilitate recovery. However, the lack of familiarity with the approach has prevented widespread adoption of the method. Our overall major complication rate was in the lower end of the range of published complication rates (range, 1.36%-15.79%). Although the high incidence of wound complications is of concern, it can be explained by the location of the incision being in an area where large skin folds or moist skin make healing difficult. We have since implemented a preoperative protocol to sterilize the area near the inguinal area and included silver impregnated dressing to help decrease our wound related complications

Purpose. compare the radiological results in sagittal balance correction obtained with pedicle subtraction osteotomy (PSO) versus anterior-posterior osteotomy (APO) by double approach in adults. Material and Methods. between January of 2001 and July of 2009, fifty-eight vertebral osteotomies were carried out in fifty-six patients: 9 Smith-Petersen osteotomy (SPO), one vertebral resection osteotomy (VRO), 30 anterior-posterior osteotomies (APO) and 18 pedicle subtraction osteotomies (PSO), being the lasts two groups the sample studied (48 osteotomies). The mean age of the patients was 56.3 years (17–72). Initial diagnose was: 28 posttraumathic kyphosis, 7 postsurgical kyphosis, 7 adult degenerative disease, 4 ankylosing spondylitis and 2 congenital kyphoscoliosis. We evaluated the preoperative standing radiographs, the postoperative and at final follow-up by digital measurements with iPACS system viewer (© Real Time Image, USA, 2001). The mean follow-up was 54 months (6–98), and complications were analized. Results. The group APO had a mean preoperative thoracic kyphosis of 67 °, a mean lumbar lordosis of −42° and a mean sagital balance of 8.6°. The group PSO had a mean preoperative thoracic kyphosis of 41°, a mean lumbar lordosis of −22° and a mean sagital balance of 12.3°. The mean correction in the APO group was 29° in its thoracic kyphosis, 8° of lumbar lordosis and 6.5° in its sagital balance. The mean correction in the PSO group was 12° of the thoracic kyphosis, 25 in the lumbar lordosis and 8.4 cm in the sagital balance. The local correction obtained at the osteotomy level was 28° in the APO group and 25.3° in the PSO group. There were no statistically significant differences in the percentage of correction between both groups (p>0.05). In terms of complications, PSO group had lower complication rate (26.6%) comparing to ODV group (44.5%). Conclusions. APO and PSO are useful techniques to correct the global sagital balance in patients with a disturbance of the sagittal profile. The correction obtained with the PSO is similar to obtained with the APO. Patients undergoing an OSP had a lower complication rate than patients undergoing APO

Purpose of the study: Revision surgery for scoliosis in adults is a technical challenge. Indications include flat back, non-union, and syndromes adjacent to the instrumentation The purpose of this work was to evaluate the pertinence of the transforaminal lumbar interbody fusion (TLIF) method for revision surgery for scoliosis in adults. Material and methods: In our spinal surgery unit, 23 patients underwent revision surgery for thoracolumbar and lumbar scoliosis. A unique posterior approach was used. The TLIF was performed systematically at the lumbosacral level, at the non-union when it was present, and at the level of the Smith-Petersen osteotomies, as well as the levels above and below a transpedicular osteotomy. Seventeen patients presented flat back, ten non-union, five degenerative disease distal to the instrumentation and one degeneration proximal to the instrumentation. Nine patients had several indications for surgical revision. Five transpedicular osteotomies were performed in five patients. Results: Mean follow-up was 30 months (range 18–48). On average 2.3 levels (range 1–4) were involved in the TLIF. The fusion was extended to the sacrum in 22 patients. The mean operative time was 5h50m (range 3–8 hours). Mean blood loss was 2100ml (400–4500). Postoperative lumbar lordosis (L1S1) was 53.5°, giving an improvement of 23° copared with the preoperative lordosis. Among the postoperative complications, there was one neurological complications which recovered partially at last follow-up one case of deep infection of the operative site which require partial removal of the implants and one case of recurrent non-union. There was no loss of correction in the frontal or sagittal planes with the exception of one patient who developed an infection. None of the patients in the series required complementary anterior surgery. Conclusion: For revision surgery of scoliosis in the adult, a circumferential arthrodeis is needed to maintain the fusion. The TLIF method has the advantage of allowing intersomatic fusion via the posterior approach alone without opening the spinal canal. We consider that the TLIF technique is an alternative to two-phase procedures for revision surgery for scoliosis in adults. This method has given a good percentage of fusion in our series with little loss of correction

Total Hip Replacement (THR) in proximal, posterior iliac dislocation of the hip often represents a problematic issue. Reviewing their selected cases (70 patients between 3700 THR from 1986 to 2001), authors focalized some key points for this demanding surgery. The most important steps are acetabular positioning, implant decisioning and surgical approach (exposure and release). Acetabular cup positioning. The natural site (Paleoacetabulum), the ideal place to restore biomechanical and dynamic properties of the joint, many times gives few chances to achieve primary stability. So one site, at least the nearest possible to the natural site must be reached. A CT or MRI study is necessary to assess preoperative planning for cup positioning. We used two different cups, the Zweymuller and the Wagner cup, with good primary stability. A Conus stem (Wagner) or an Alloclassic stem (in less displastic femoral shape) was used. We always performed this surgery as a one step procedure. No preventing traction or release surgery was performed. An anatomic and wide (medial and lateral) exposure of the joint must be performed. We used the Smith-Petersen approach modified by Wagner. A meticulous periarticular release of soft tissue was performed. In same cases a shortening femoral osteotomy was performed to allow refractory reduction. Possible complications are discussed. Good clinical outcomes at more than ten years are shown

Hip fracture treatment strategies continue to evolve with the goal of restoring hip fracture victims to Pre-injury Functional levels. Strategies for improved treatment have focused on fracture exposure, reduction, provisional fixation and definitive fixation with implant designs optimised for fracture union with minimal implant failure as originally proposed by Lambotte. Multiple implant designs have been conceived based on perceived inadequacies of previous generational designs. To better understand this evolutionary process, it is necessary to review the predecessors of modern fracture treatment and understand their design concepts and results. It is interesting that the modern era of surgical treatment of hip fractures actually began in 1902, when Dr Royal Whitman advocated the necessity of a closed reduction of adult hip fractures under general anesthesia and stabilisation by hip spica cast. Dr Whitman predicted the evolution of stabilisation by internal fixation and commented on this in his 1932 JBJS editorial emphasising the importance of surgical treatment of fractures. Dr Smith-Peterson, also from New York, in 1925 developed the 1st commercially successful hip implant, a tri–flanged nail. These first surgeries were performed with an open reduction, through a Smith-Petersen approach without radiographic control. This nail device was rapidly modified in the 1930's to permit insertion over a guide wire with a radiographic controlled insertion technique, a minimally invasive procedure. Nail penetration and implant failure in pertrochanteric fractures led to the rapid development of side-plates and a refocus on reduction stability. This led to a period of primary corrective osteotomies for enhanced stability, but fell out of failure after the sliding hip screw concept took hold. Originally conceived by Godoy-Moreira and Pohl independently in the 1940s, it became rapidly accepted as a method to avoid nail penetration and implant failure, unfortunately at the expense of accepting malunion and collapse of the fracture. Even the importance of rotational stability was discarded as insignificant by Holt in 1963. The concept of reduction of the Antero-Medial cortex was forgotten in favour of the Tip-apex distance as the only important variable in reduction to avoid implant cut-out. The concept of malunion of pertrochanteric fractures was simply deleted from consideration with disregard for the possible association of impaired functional recovery. Several recent papers that improved functional recovery is possible when these new implants are coupled with successful reduction strategies. Further studies are needed to identify the correct choice of implant for the appropriate fracture configuration, which may lead to a revision of our current fracture classification systems and our concepts of stability

Introduction: Correction of lumbar spine deformity in ankylosing spondylitis (AS) can be achieved by pedicle subtraction osteotomy (PSO), polysegmental osteotomy (PO) or Smith-Petersen osteotomy (SPO). We report our results with these three techniques. Methods: 26 males and 5 females with AS and average age of 54.7 years (range 40–74 years) underwent surgery for loss of sagittal balance, horizontal gaze and back pain. 12 patients underwent PSO, 10 SPO, and 9 PO. Osteotomy was carried out at L3 in PSO and SPO with pedicle fixation from T11 to S1. 9 patients with PO had osteotomy from L2–5 and fixation from T10-S1. Sagittal translation during corrective manoeuvre was controlled in 21 patients by application of temporary malleable rods, which were substituted with permanent rods. TLSO was used post-operatively for average period of three months. Mean follow-up was 4.2 years (range 1–9 years). Radiographic and clinical outcomes (ODI, VAS, SRS-22) were analysed. Results: Mean kyphotic correction in PSO was 380 (range 250–490), in PO was 300 (range 280–400) and in SPO was 280 (range 240–380). The sacrohorizontal angle improved by 190(range 50–300) in PSO, 210 (range 80–280) in PO and 150 (range 50–180) in SPO. Outcome scores were better in PSO and PO as compared to SPO. Blood loss and transient nerve root palsy was slightly higher in PSO group. One patient with SPO had fatal bleeding as a result of aortic injury. Conclusions: Regular use of temporary malleable rods is recommended to prevent sagittal translation during correction reducing the risk of neurological injury. Better correction of deformity was achieved with PSO and PO at the expense of increased blood loss. SPO can increase the risk of vascular injury, therefore we recommend PSO and PO for correction of deformity in Ankylosing Spondylitis

Introduction: The anterior femoroacetabular impingement syndrome has so far been a great unknown in orthopedic surgery. It is typically characterized by pain when the hip is subjected to the flexion – adduction – internal rotation movement. This pain is provoked by the impaction of the head-neck interface on the anterior wall of the acetabulum. The reason for this may be a retroverted acetabulum, an excessively prominent anterosuperior femoral head-neck junction or a combination of both. For many years, patients have been diagnosed with “adductor tendinopathy” or “inguinal herniations”, when in fact they had a coxofemoral problem. Materials and methods: The first 14 cases operated were analyzed; all of them were young patients who played sports regularly. Using the modified Smith-Petersen approach, an osteoplasty was made in order to resect in the anterior wall and the superior walls of the acetabulum – the latter only in part – and the prominent head-neck junction of the femur. The result was an improvement in the joint balance and the disappearance of impingement. Unlike other authors (Ganz, Trousdale), we avoided an osteotomy of the greater trochanter as a surgical approach. Results: In 13 of the first 14 cases operated with the technique described, immediate pain relief was achieved on internal flexorotation. ROM went from −17 ° mean internal rotation (range: −14°–−28°) in one 80 ° flexion to +23° after one month postop (range: 14°–32°). After two months, there were no instances of Trendelemburg sign or osteonecrosis of the femoral head. Conclusions: We should wait to assess the pre-osteoarthritic development of these patients, although their clinical and functional improvement is evident
