Purpose. The aim of the study was to assess the efficacy of a cementless acetabular cup without bone grafting in a cohort of patients with protrusio acetabuli who were treated with total hip arthroplasty. Methods. We retrospectively reviewed the records of a cohort of 45 patients with protrusio acetabuli who received elective total hip replacements between 2009 and 2011 at Tygerberg Academic Hospital. We reviewed pre- and post-operative radiographs and the most recent follow-up radiographs (minimum of three months) to assess cup osteointegration. We classified protrusio acetabuli according to Sotelo-Garza and Charnley into mild, moderate and severe, by evaluating pre-operative radiographs. Results. Our cohort of had a median age of 63 years and had a female of male ratio of 4 to 1. The mean follow up was 11 months. We had 9 mild, 29 moderate and 7 severe protrusio cases. At follow-up all cups showed signs of osteointegration with no signs of loosening. Conclusion. The use of a cementless, plasma coated, dual radius acetabular cup without bone grafting in primary THR in protrusio acetabuli provides good primary stability with satisfactory short term radiological outcome. Introduction

This study was to investigate the association of developmental dysplasia of the hip (DDH) and primary protrusion acetabuli (PPA) with Vitamin D receptor polymorphisms TaqI and FokI and oestrogen receptor polymorphisms Pvu II and XbaI.

Methods

45 patients with DDH and 20 patients with PPA were included in the study. Healthy controls (n=101) aged 18-60 years were recruited from the same geographical area. The control subjects had a normal acetabular morphology based on a recent pelvic radiograph performed for an unrelated cause. DNA was obtained from all the subjects from peripheral blood. Genotype frequencies were compared in the three groups. The relationship between the genotype and morphology of the hip joint, severity of the disease, age at onset of disease and gender were examined.

Protrusio acetabuli (arthrokatadysis or Otto pelvis) is a relatively rare condition associated with secondary osteoarthritis of the hip. Radiographically, protrusio acetabuli is present when the medial aspect of the femoral head projects medial to Kohler's (ilioischial) line. This results in medialization of the center of rotation (COR) of the hip. Protrusio acetabuli is typically associated with metabolic bone disease (osteoporosis, osteomalacia, Paget's disease) or inflammatory arthritis (RA or ankylosing spondylitis). Idiopathic acetabular protrusio can occur without the above associated factors however. Patients with protrusio acetabuli typically present with significant restriction of range of motion (ROM) of the hip due to femoral neck and trochanteric impingement in the deep acetabular socket and pain associated with secondary osteoarthritis (OA). Total hip arthroplasty (THA) in patients with protrusion acetabuli is more challenging than THA in patients with a normal hip COR. ROM is typically quite restricted which can compromise surgical exposure. Dislocation of the hip in the patient with a deep socket and medialised COR can be extremely difficult and associated with fracture of the femur if not carefully performed. Restoration of the hip COR to the normal more lateralised position is a principle goal of surgery. This restores more normal mechanics of the hip and has been associated with improved durability. A variety of techniques to accomplish this have been described including medial acetabular bone grafting with cemented cups, protrusio rings or porous coated cementless cups fixed with multiple screws. The latter technique has been shown to be more durable and associated with better outcomes. THA in protrusio acetabuli starts with templating of the preoperative x-rays to determine the optimal acetabular implant size and final position of the acetabular component that restores the hip COR to the normal position. Patients with protrusio acetabuli often have varus oriented femoral necks and the femur needs to be carefully templated as well to insure that an appropriate femoral component is available that will allow for restoration of the patient's anatomy. Cartilage covering the thinned medial wall needs to be carefully removed without disruption of the medial acetabular wall. The acetabulum is then carefully reamed with the goal of obtaining stable peripheral rim support of a cementless socket and at least 50% contact of the implant on good quality host bone. Unlike acetabular preparation in the normal hip, preventing the reamer from “bottoming out” is essential in order to obtain desired rim support and return of the hip COR to the normal lateralised position. When good rim support of the reamer is obtained, a trial component is placed and intraoperative x-ray obtained to confirm fit, position and restoration of hip COR. Limited addition reaming can be performed to obtain desired degree of press fit (1‐2mm) and contact with host bone. Morselised autograft from the femoral head and neck is then packed into the medial defect and reverse reamed. The cementless acetabular component is then impacted into position and fixed with screws. Weight bearing is determined by bone quality, size and containment of the medial defect, amount of contact of the cementless cup with host bone and stability of the acetabular construct. Incorporation of autograft bone in the acetabulum and stable long term fixation occurs reliably if stable initial press-fit fixation of the cementless cup is obtained. Restoration of hip COR to within 7mm of its normal location is associated with better implant survival

Detection of clinical situations are the most difficult for primary THA and factors which determine the complexity. Results of 2368 primary THA performed by one surgeon in 1923 patients with various hip pathologies from 2004 to 2016 were analyzed. The time of the surgery, the bloodloss, the features of the surgical technique, the implants used, and the incidence of complications and revisions were assessed and X-ray analysis was performed. Difficult cases of primary hip arthroplasty include severe dysplasia (types B2, C1, and C2 according to the Hartofilakidis classification), post-traumatic segmental acetabular defects and pelvic discontinuity, protrusio acetabuli, iatrogenic bone ankylosis and consequences of proximal femur fractures with significant shortening of the limb. X-ray signs of difficulty included an interruption of the Shenton line of 2 cm or more (except for acute fractures of proximal femur), the femoral neck-shaft angle less than 100°, and the horizontal distance from Kohler line to center of rotation less than 20% of the diameter of the femoral head. An additional burdening factor is the previous surgical interventions on the hip joint. The ten-year survival rate for standard cases was 94.9%, and for complex cases − 92.3%. The odds ratio development of complications in complex cases compared to standard cases is OR = 8.402 (95% CI from 4.614 to 15.300). In standard cases mean HHS increased from 42.9 points before surgery to 95.3 after surgery. In complex cases mean HHS before surgery was 38.9 and after surgery improved to 81.6 points, p <0.001. The complexity of the operation cannot be determined on the basis of only the etiological factor, it is necessary to take into account the severity of anatomical changes

Aim: To ameliorate surgical strategy of disabled rheumatoid hip joints, perioperative status and clinical features of the patients undergoing total hip arthroplasty (THA) were retrospectively evaluated. Materials and Methods: 150 joints of 106 patients were studied (male/female rate; 1:6, mean age; 60 years and duration of the disease; 15 years). All patients received cemented THA (mean follow-up period; 8 years). Mode of bone defect with acetabular reconstruction type, femoral bone quality, survivorship, steroid use and complications were surveyed. Results: In preoperative status, proturusio acetabuli was found in 37% with type I; 54%, II; 34% and III; 12% by Sotelo-Garza classification. Superior bone defect was recognized in 56%, collapse and/or defect of femoral head in 19%, and geode formation in 0.2%. Femoral medullar canal was classified as type A; 1%, type B; 53% and type C; 46% by Dorr classification. The presence of fracture before surgery was 5%. Anatomical reconstruction was achieved in all cases including application of 42% bone grafting (autogenous alone; 51%, application of artificial substitute; 39% and of cross-plating system; 10%). Acetabular revision rate due to aseptic loosening (%/years) was improved by graft methods (whole series; 5/8, any grafting; 6/8, autogenous alone; 8/8, artificial substitute; 4/8 and cross-plate system; 0/4). Revision rate for any reasons was 9% (aseptic acetabulum 5%, aseptic femur 5%, dislocation 2% and infection 1%). Dislocation (11%), infection (3%) and severe thrombotic events (1%) were experienced. Steroid use was found in 73%, associated with increased risk of protrusio acetabuli, superior bone defect with protrusio acetabuli and fractures. Discussion and Conclusion: The study indicated that steroidal medication significantly related to the perioperative status of bone defects and perioperative fractures of rheumatoid patients undergoing THA. Improved ace-tabular procedures could promise better survivorship of the implant

Summary Statement. Pincer deformities are involved in the genesis of femoro-acetabular impingement (FAI). Radiographic patterns suggestive of pincer deformities are common among general population. Prevalence of the pincer deformities among general population may be overestimated if only plain radiographs are considered. Background. Pincer deformities (coxa profunda, protrusio acetabuli, global retroversion, isolated cranial over-coverage) have been advocated as a cause of femoro-acetabular impingement (FAI) and early hip osteoarthritis (OA). Different radiographic patterns may advocate the presence of a pincer deformity. The prevalence of these radiographic patterns among general adult population, as their role in early hip OA, is poorly defined. Methods. From a database of 40.351 pelvic radiograms and CT collected at our institution between 2005 and 2010, we selected 118 caucasian individuals (56 females, 62 males), aged between 15 and 60 years, who underwent both plain radiographs and CT of the pelvis. A series of exclusion criteria were strictly applied to achieve a sample of adult general population as more representative as possible. In particular patients with presence of any disease involving hip joint, including: advanced hip OA (grade II or III of Tonnis scale), head necrosis, fractures, heterotopic ossifications, bone and soft tissue tumors, rheumatic pathologies, classic hip dysplasia with lateral center-edge angle (L-CEA) less than 20°, clinical diagnosis of FAI or hip pain, were excluded from the present study. We also excluded patients in which open growth plates, osteopenia, hardware or evidence of prior surgery were present. Radiographs were investigated for pelvic tilt, signs of retroversion, lateral center-edge angle (L-CEA), presence of coxa profunda or protrusio acetabuli. EAV was measured on CT scans at the equatorial plane of the acetabulum passing by the 3 o'clock position, while CAV was calculated at a more cranial level corresponding to the 1 o'clock position EAV and CAV were obtained in the axial plane by measuring the angle made by a line connecting the anterior and posterior rims of the acetabulum and a line perpendicular to the line connecting the ischial spines. A new parameter, Acetabular torsion (AT), has been introduced in order to discriminate between global retroversion and isolated cranial over-coverage. AT was defined as the difference between EAV and CAV. Cam deformity was assessed by calculating the alpha angle on the femoral side; an alpha angle > 55° was considered abnormal and suggestive of cam deformity. Radiological signs of chondrolabral degeneration were noticed. Results. Mean EAV and mean CAV were higher in females, mean AA was higher in males. L-CEA, EAV and CAV increased with age. Mean AT was 8.8±6.3. AT was inversely related to CAV (r=−0.799; p<0.0005) but independent from EAV (r=−0.076; p=0.244). EAV≤10.2° was defined as the marker of global retroversion, while AT≥21.2° was defined as the marker of isolated cranial over-coverage. Overall prevalence of pincer deformities was 21.6% (> females; p=0.02). Early OA changes were related to age (p<0.0005) and AA (p<0.0005), but not to pincer deformities (p=0.96). Radiological signs of retroversion showed good or excellent negative predictability but poor positive predictability. Conclusions. Radiographic patterns of pincer deformities are common among general population. Relationship with radiological signs of chondrolabral degeneration is poor. CT allows to discriminate between global retroversion and isolated cranial over-coverage. Prevalence of the pincer deformities among general population may be overestimated if only plain radiographs are considered

Protrusio acetabuli can be either primary or secondary. Primary or idiopathic protrusio is a rare condition of unknown etiology. Secondary protrusio may be associated with rheumatoid arthritis, ankylosing spondylitis, osteoarthritis, osteomalacia, trauma and Paget's disease. Challenges in surgery include: lack of bone stock, deficient medial support to the cup, difficulty in dislocating the femoral head, and medialization of the hip joint center. Several surgical techniques have been described: use of cement alone without bone graft; morselised impacted autograft or allograft with a cemented cup; metal cages, reinforcement rings, and solid grafts. We describe our technique of impaction grafting using autologous bone and a cementless porous-coated hemispherical cup without the use of acetabular rings or cages in patients with an average age of 46 years. Protrusion was graded depending on distance of medial wall from Kohler's line as mild (1–5 mm medial), moderate (6–15 mm medial) and severe if it was more than 15 mm medial to the Kohler's line. All patients were operated in the lateral position using a modified Hardinge's anterolateral approach. Adductor tenotomy may be required in cases of severely stiff hips. After careful dislocation of the femoral head, it was sectioned in situ into slivers to facilitate obtaining the graft. The periphery was reamed and care was taken to preserve the membrane lining the floor of the defect. Morselised graft was impacted with hemispherical impactors and the trial cup 1–2 mm larger than the last reamer placed in the desired position. The final socket was then inserted. Femoral preparation was performed in routine fashion. The mean pre-operative Harris Hip Score of 52 improved to 85 points at a mean follow up of 4 years. The average acetabular inclination angle was 42 degrees. Our results have shown incorporation of the graft in all cases. There was no evidence of progression of the protrusio or cup loosening in any of the cases. Thus far, our hips have not shown osteolytic lesions. The technique described is a satisfactory biological solution of restoring bone stock particularly in young and middle-aged patients

Aims

Femoroacetabular impingement (FAI) patients report exacerbation of hip pain in deep flexion. However, the exact impingement location in deep flexion is unknown. The aim was to investigate impingement-free maximal flexion, impingement location, and if cam deformity causes hip impingement in flexion in FAI patients.

Methods

A retrospective study involving 24 patients (37 hips) with FAI and femoral retroversion (femoral version (FV) < 5° per Murphy method) was performed. All patients were symptomatic (mean age 28 years (SD 9)) and had anterior hip/groin pain and a positive anterior impingement test. Cam- and pincer-type subgroups were analyzed. Patients were compared to an asymptomatic control group (26 hips). All patients underwent pelvic CT scans to generate personalized CT-based 3D models and validated software for patient-specific impingement simulation (equidistant method).

Aims

The aim of this study was to investigate whether anterior pelvic plane-pelvic tilt (APP-PT) is associated with distinct hip pathomorphologies. We asked: is there a difference in APP-PT between young symptomatic patients being evaluated for joint preservation surgery and an asymptomatic control group? Does APP-PT vary among distinct acetabular and femoral pathomorphologies? And does APP-PT differ in symptomatic hips based on demographic factors?

Methods

This was an institutional review board-approved, single-centre, retrospective, case-control, comparative study, which included 388 symptomatic hips in 357 patients who presented to our tertiary centre for joint preservation between January 2011 and December 2015. Their mean age was 26 years (SD 2; 23 to 29) and 50% were female. They were allocated to 12 different morphological subgroups. The study group was compared with a control group of 20 asymptomatic hips in 20 patients. APP-PT was assessed in all patients based on supine anteroposterior pelvic radiographs using validated HipRecon software. Values in the two groups were compared using an independent-samples t-test. Multiple regression analysis was performed to examine the influences of diagnoses and demographic factors on APP-PT. The minimal clinically important difference (MCID) for APP-PT was defined as > 1 SD.

Introduction. Computer navigation systems are quite sophisticated intra-operative support systems for the precise placement of acetabular or femoral components in THA. However, few studies have addressed the clinical benefits derived from using a navigation system to achieve precise placement of the implants. The purpose of this study is to investigate the early dislocation rate of navigation-assisted primary THA through a posterior approach in order to clarify the short-term benefit of using a computer navigation system. Methods. We retrospectively reviewed the early dislocation rate (within 12 months after surgery) of 475 consecutive primary cementless or hybrid THAs with femoral head sizes ≦32mm performed via posterior approach. There were 85 men and 390 women, with a mean age of 60 years (17 to 88) at operation. Preoperative diagnoses included osteoarthritis in 384 hips, osteonecrosis in 45 hips, and others in 46 hips (ex. RA, trauma, infection, congenital disease). All THAs were planned using a 3D templating system based on the combined anteversion theory, performed by single surgeon through a posterior approach with repair of the posterior capsule, assisted by a CT-based surface matching type computer navigation system for cup implantation. All patients were directly followed up at least 1 year after surgery. We classified all 475 joints into four groups: normal or mildly deformed hips (Group A; 308 joints, ex. primary OA, Crowe group 1, osteonecrosis), moderately deformed hips (Group B; 97 joints, ex. Crowe group 2, protrusio acetabuli, Perthes like deformity), severely deformed hips (Group C; 53 joints, ex. Crowe group 3 or 4, ankylosis, fused hip), and neuromuscular and cognitive disorders (Group D; 17 joints), and examined the dislocation rate for each group. Results. We had eleven early dislocations, for an overall dislocation rate of 2.3% (11/475). All dislocations occurred posteriorly within 6 weeks after surgery. Three joints were Crowe group 4 dislocated hips, three were Charcot joints, two were Girdlestone hips after pyogenic arthritis, two was a Crowe group 1 hip, and one was osteonecrosis. All 11 cups were implanted within 5 degree of error from the preoperative planning, and all were placed within the Lewinnek safe zone. The dislocation rates according to group were 0.6% for group A (2/308), 0% for B (0/97), 9.4% for C (5/53), and 23.5% for D (4/17). Discussion. The use of computer navigation system in patients undergoing THA improves the precision of acetabular cup placement by decreasing the number of outliers, which may result in reducing the risk of dislocation. In this series, most dislocations occurred in the highly risky or rare condition cases in groups C or D. In such cases, precise and appropriate cup implantation assisted by the navigation system could not completely prevent dislocation because of the patients' specific special backgrounds. On the other hand, early dislocation was prevented for the normal/mild to moderately deformed joints such as those in groups A or B. Computer navigation system was effective for prevent early dislocation in the normal or mild to moderately deformed joints

Aims

Young adults undergoing total hip arthroplasty (THA) largely have different indications for surgery, preoperative function, and postoperative goals compared to a standard patient group. The aim of our study was to describe young adult THA preoperative function and quality of life, and to assess postoperative satisfaction and compare this with functional outcome measures.

Methods

A retrospective cohort analysis of young adults (aged < 50 years) undergoing THA between May 2018 and May 2023 in a single tertiary centre was undertaken. Median follow-up was 31 months (12 to 61). Oxford Hip Score (OHS) and focus group-designed questionnaires were distributed. Searches identified 244 cases in 225 patients. Those aged aged under 30 years represented 22.7% of the cohort. Developmental dysplasia of the hip (50; 45.5%) and Perthes’ disease (15; 13.6%) were the commonest indications for THA.

Protrusio acetabuli can be either primary or secondary. Primary or idiopathic protrusio is a rare condition of unknown etiology. Secondary protrusio may be associated with Rheumatoid Arthritis, Ankylosing spondylitis, osteoarthritis, osteomalacia, trauma and Paget's disease. Challenges in surgery include lack of bone stock, deficient medial support to the cup, difficulty in dislocating the femoral head, and medialisation of the hip joint centre. Several surgical techniques have been described: use of cement alone without bone graft; morsellised impacted autograft or allograft with a cemented cup; metal cages, reinforcement rings, and solid grafts. We describe our technique of impaction grafting using autologous bone and a cementless porous-coated hemispherical cup without the use of acetabular rings or cages in patients with an average age of 46 years. Protrusion was graded depending on distance of medial wall from Kohler's line as mild (1–5 mm medial), moderate (6–15 mm medial) and severe if it was more than 15 mm medial to the Kohler's line. All patients were operated in the lateral position using a modified Hardinge's anterolateral approach. Adductor tenotomy may be required in cases of severely stiff hips. After careful dislocation of the femoral head, it was sectioned in situ into slivers to facilitate obtaining the graft. The periphery was reamed and care was taken to preserve the membrane lining the floor of the defect. Morsellised graft was impacted with hemispherical impactors and the trial cup 1–2 mm larger than the last reamer placed in the desired position. The final socket was then inserted. Femoral preparation was performed in routine fashion. The mean pre-operative Harris hip score of 52 improved to 85 points at a mean follow up of 4 years. The average acetabular inclination angle was 42 degrees. Our results have shown incorporation of the graft in all cases. There was no evidence of progression of the protrusio or cup loosening in any of the cases. Thus far, our hips have not shown osteolytic lesions. The technique described is a satisfactory biological solution of restoring bone stock particularly in young and middle-aged patients

Several hundred Joint Replacement Surgeries have been done by surgeons (Gustilo/Leagogo) of the Philippine Orthopedic Institute in the Philippines since its inception over 20 years ago. Revision surgery of failed cemented hip replacement, have been increasing in the last decade, which is the subject of this report. The average total cost of revision hip surgery in the Philippines ranges from PEP 800 000 to PEP 1 000 000 or USD 20 000 to USD 25 000. All medical records of the patients were reviewed to determine the following:. Etiology. Duration from index surgery. Common anatomical pathology and amount of leg length discrepancy encountered. Revision implants used and use of allograft. Postoperative course and complications. Preliminary results (How many are ambulatory with or without assistive device). Analysis of x-ray at last follow-up. A total of sixty nine (69) patients were included in the study. Sixty eight percent (68%) of the patients (47 out of 69) underwent revision due to aseptic loosening of components (femoral or acetabular). Nine (9) patients (13%) developed infection. Six (6) patients (9%) developed protrusio acetabuli that necessitated revision. Peri-prosthetic fracture was noted in four (4) patients (6%). Three (3) patients had hip dislocation (4%) prior to revision. Average number of years from index surgery is 9 years (2–18years). Fifty nine (59) patients (86%) underwent cemented total hip prior to revision while ten (10) patients (14%) underwent cemented partial hip replacement prior to revision. Fifty six (56) patients (81%) have shortening on the affected side with an average of 3cm (1–6cm). Forty five (45) patients (65%) had proximal femoral bone loss requiring structural allograft. Thirty one (31) patients (45%) had acetabular defect (Paprosky Types I & II). In sixty five (65) patients (94%), Active LockTM Cementless Hip Revision System was used. All patients were allowed full weight-bearing after revision surgery. Four patients (6%) developed acute post-op infection. Two patients (3%) had dislocation after revision surgery. At short-term follow-up of one year or more, only 5 patients were ambulating with assistive device. The rest are ambulatory, without aid and are symptom free. Follow-up radiographs showed implants in excellent alignment, no signs of loosening, migration or subsidence. The main reason for revision hip arthroplasty in the Philippines is aseptic loosening (68%) followed by infection (13 %). The use of long non-cemented, calcar-replacing curved revision stem, and strut allograft on the femoral side; jumbo, cementless acetabular socket and a high hip center on the acetabular side, addressed the problem of anatomic pathologies as a result of failed, cemented THR. There is no association between cause for revision with gender and age of the patients

Aims

Femoroacetabular impingement (FAI) describes abnormal bony contact of the proximal femur against the acetabulum. The term was first coined in 1999; however what is often overlooked is that descriptions of the morphology have existed in the literature for centuries. The aim of this paper is to delineate its origins and provide further clarity on FAI to shape future research.

Methods

A non-systematic search on PubMed was performed using keywords such as “impingement” or “tilt deformity” to find early anatomical descriptions of FAI. Relevant references from these primary studies were then followed up.

As our understanding of hip function and disease improves, it is evident that the acetabular fossa has received little attention, despite it comprising over half of the acetabulum’s surface area and showing the first signs of degeneration. The fossa’s function is expected to be more than augmenting static stability with the ligamentum teres and being a templating landmark in arthroplasty. Indeed, the fossa, which is almost mature at 16 weeks of intrauterine development, plays a key role in hip development, enabling its nutrition through vascularization and synovial fluid, as well as the influx of chondrogenic stem/progenitor cells that build articular cartilage. The pulvinar, a fibrofatty tissue in the fossa, has the same developmental origin as the synovium and articular cartilage and is a biologically active area. Its unique anatomy allows for homogeneous distribution of the axial loads into the joint. It is composed of intra-articular adipose tissue (IAAT), which has adipocytes, fibroblasts, leucocytes, and abundant mast cells, which participate in the inflammatory cascade after an insult to the joint. Hence, the fossa and pulvinar should be considered in decision-making and surgical outcomes in hip preservation surgery, not only for their size, shape, and extent, but also for their biological capacity as a source of cytokines, immune cells, and chondrogenic stem cells.
