Pain immediately following total knee arthroplasty (TKA) is often severe and can inhibit patients' rehabilitation. Recently, adductor canal blocks have been shown to provide adequate analgesia and spare quadriceps muscle strength in the early postoperative period. We devised a single injection motor sparing knee block (MSB) by targeting the adductor canal and lateral femoral cutaneous nerve with a posterior knee infiltration under ultrasound. Our primary objective was to evaluate the analgesia duration of the MSB in comparison to a standard periarticular infiltration (PAI) analgesia using patients' first rescue analgesia as the end point. Secondary outcomes measured were quadriceps muscle strength and length of stay.

We randomised 82 patients scheduled for elective TKA to receive either the preoperative MSB (0.5% ropivacaine, 2.5ug/ml epinephrine, 10mg morphine, and 30mg ketorolac) or intraoperative periarticular infiltration (0.3% ropivacaine, 2.5ug/ml epinephrine, 10mg morphine, and 30mg ketorolac). Duration of analgesia, postoperative quadriceps power, and length of stay were evaluated postoperatively.

Analgesic duration was found to be significantly different between groups. The MSB had a mean duration of 18.06 ± 1.68 hours while the PAI group had a mean duration of 9.25 ± 1.68 hours for a mean difference of 8.8 hours (95% CI 3.98 to 13.62), p<0.01. There were no significant differences between groups in quadriceps muscle strength power at 20 minutes (p=0.91) or 6 hours (p=0.66) after block administration. Length of stay was also not significantly different between the groups (p=0.29).

Motor sparing blocks provide longer analgesia than patients receiving periarticular infiltration while not significantly reducing quadriceps muscle strength or increasing length of hospital stay.

Total Knee Arthroplasty (TKA) is a successful treatment for end stage osteoarthritis of the knee joint. However, post-operative pain can lead to patient dissatisfaction and poorer outcomes. Cooled radiofrequency nerve ablation (CRNA) has reportedly been effective at treating pain osteoarthritic knee pain by targeting the periarticular nerves of the knee. We undertook a prospective, controlled pilot study to determine if CRNA provides effective post-operative analgesia when utilised intra-operatively during total knee arthroplasty. Participants were recruited from January 2019 to February 2020. Those meeting inclusion criteria underwent TKA with intraoperative CRNA to 6 target sites prior to the cementing of implants. The primary outcomes were pain scores and opiate usage in the first 4 days post-operatively, then weekly up to 6 weeks. A total of 62 patients were screened and allocated sequentially; 18 were recruited to the control group and 12 recruited to the study group. The two groups did not have any significant difference in demographics. There were no clinically significant differences between the two groups in terms of pain scores nor opiate usage. There were complications as a result of the intervention. This study demonstrated no benefit of using intraoperative CRNA for improving post-operative pain scores or reducing opiate use after TKA

This Blinded Randomized Clinical Trial outlines: how peri-articular intra-operative multimodal analgesia significantly reduces post-operative analgesia requirement. Sixty-four patients undergoing total knee replacement were randomised to receive a peri-articular intra-operative injection containing ropivacaine, ketorolac, epimorphine and epinephrine or nothing. Patients who received the injection demonstrated greater satisfaction and pain relief. Finally, patients in the injected group did not show any signs of cardio and central nervous system toxicity. Post-operative analgesia can be associated with troublesome side effects. Good peri-operative analgesia facilitates rehabilitation, improves patient satisfaction and may reduce hospital stay. The purpose of this study was to assess a novel cocktail for peri-articular analgesia after total knee replacement. Sixty-four patients undergoing total knee replacement were randomised to receive a peri-articular intra-operative injection containing ropivacaine, ketorolac, epimorphine and epinephrine or nothing. The anaesthetic analgesic regime was standardised. All patients received patient controlled analgesia (PCA) for twenty-four hours post surgery, followed by standard analgesia. VAS pain scores during activity and at rest and patient satisfaction scores were recorded pre and post operatively and at six week follow up. PCA consumption and overall analgesic requirement were measured. PCA use at six, twelve and over twenty-four hours post surgery was significantly less in patients receiving the injection (p< 0.01, p=0.016, p< 0.01). Patient satisfaction in PACU and four hours post operation was greater (p=0.016, p=0.013). VAS for pain during activity in PACU and at four hours were significantly less (p=0.04, p=0.007) in the injected group. The average ROM at six weeks was no different. Overall hospital stay and the incidence of wound complications were not different between the two groups. Peri-articular intra-operative multimodal analgesia significantly reduces post-operative analgesia requirement. Patient satisfaction and pain relief is greater in the injection group. No cardio and central nervous system toxicity was observed. Our novel cocktail of ketorolac, epimorphine, epinephrine and ropivacaine provides superior pain relief with no adverse side effects

Good peri-operative analgesia facilitates rehabilitation, improves patient satisfaction and may reduce hospital stay. Locally administered pre-emptive analgesia is effective, reduces central hyper-sensitisation and avoids systemic drug related side-effects and may be of benefit in minimally invasive joint replacement. Materials and methods: 64 patients undergoing total knee replacement were randomised to receive a peri-articular intra-operative injection containing ropiva-caine, ketorolac, epimorphine and epinephrine. The anaesthetic analgesic regime was standardised. All patients received patient controlled analgesia (PCA) for 24 hours post surgery, followed by standard analgesia. VAS pain scores during activity and at rest and patient satisfaction scores were recorded pre and post operatively and at 6 week follow up. PCA consumption and overall analgesic requirement were measured. Results: PCA use over 24 hours post surgery was significantly less in patients receiving the injection (P=0.013). Patient satisfaction at 4 hrs post operation was greater (P=0.003). VAS for pain during activity at 4 hrs and 24 hours were significantly less (P=0.001) in the injected group. The average ROM at 6 weeks was no different. Overall hospital stay and the incidence of wound complications was not different between the 2 groups. Discussion: Peri-articular intra-operative multimodal analgesia significantly reduces post-operative analgesia requirement. Patient satisfaction is greater in the injection group. We describe seven cases of permanent neurological damage following interscalene block used in post-operative analgesia after operations at the shoulder. MRI, Nerve Conduction Studies and Quantitative assessments of function confirmed that in all there was infarction of the anterior spinal cord, resulting in a spinothalamic and corticospinal tract defect especially at segments C7, C8 and T1. We think that these lesions were caused by injury to radicular arteries. Domisse has demonstrated the anatomy of the radicular vessels joining the anterior spinal artery to supply the anterior two thirds of the cord. They are branches of the vertebral, ascending cervical and deep cervical arteries which pass through the inter-vertebral foramina with the C7, C8 and T1 roots predominantly. Chakravorty has shown that radicular vessels contribute to the blood supply of the lower cervical cord. Injury to them can cause ischaemia, leading to Anterior Spinal Artery Syndrome. We suggest tamponade of the radicular vessels by infusion of fluid under pressure deep to the prevertebral fascia as the main mechanism but neurotoxicity and vasospasm can be other possible explanations. In a second group there was an additional interference with the vertebral artery presenting with transient bulbar and cranial nerve symptoms. We had 2 patients with such combined lesions. Complications of interscalene blocks are well documented but most are reversible and transient. In our cases the damage has been permanent and disabling. The innervation of the gleno-humeral joint is largely through the 4th, 5th and 6th cervical nerves and we suggest more appropriate placing of the blockade should be adapted and use of this technique for post-operative analgesia should be abandoned

Purpose: Post-operative analgesia using opioids or epidural analgesia can be associated with troublesome side effects. Effective peri-operative analgesia facilitates rehabilitation, improves patient satisfaction and may reduce hospital stay. Locally administered analgesia is effective, avoids systemic drug related side-effects and may be of benefit in minimally invasive joint replacement. This study compares the effects of a peri-articular injection cocktail in patients undergoing total knee and hip replacement surgery. Methods: 128 patients undergoing total knee (TKR) and hip (THR) replacement were randomised to receive a peri-articular intra-operative injection containing ropivacaine, ketorolac, epimorphine and epinephrine or nothing. The anaesthetic analgesic regime was standardised. All patients received patient controlled analgesia (PCA) for 24 hours post surgery, followed by standard analgesia. VAS pain scores during activity and at rest and patient satisfaction scores were recorded pre and post operatively and at 6 week follow up. PCA consumption and overall analgesic requirement were measured. Results: Both TKR and THR patients used significantly less PCA 6 hours after surgery (p = 0.02 THR, p< 0.01 TKR). TKR patients receiving the injection used significantly less PCA use over 24 hours post surgery (p = 0.013). VAS for pain at 4 hrs post operation was significantly lower in the both groups (p = 0.003 TKR)(p = 0.017 THR). VAS for pain during activity at 24 hours was significantly less (p = 0.001) in the injected TKR group. Overall hospital stay and wound complications were not different between the groups. Conclusions: Peri-articular analgesia significantly reduces post-operative analgesia requirement in TKR and THR patients. Greater satisfaction and pain relief was seen in TKR patients compared with THR

Bladder catheterisation following joint arthroplasty is not uncommon but delaying catheterisation in the postoperative period until the patient is symptomatic can produce an atonic bladder due to the distension. This can prolong catheterisation and increase the risk of urinary tract infection. We prospectively determined if we could identify patients needing pre-operative catheterisation. Method: 150 consecutive patients undergoing knee and hip arthroplasty were recruited. Pre-operative symptoms of frequency, nocturia, retention, incontinence and previous bladder or prostate surgery along with prior history of catheterisation were recorded. The type of anaesthesia and post-operative analgesia was noted. Details of catheterisation included duration, antibiotic administration, and reason for catheterisation and incidence of urinary tract infection. Results: Patients mean age was 67.7 years. 47 patients required catheterisation of which 56.6% were female. The mean age of patients catheterised was 70.6 years in comparison to 66.3 years (Mann-Whitney P< 0.01). The frequency of catheterisation was unrelated to the surgical procedure. Nocturia was significantly more common in-patients requiring catheterisation (Kruskal Wallis P=0.04) and its combination with pre-operative symptoms of frequency, retention or incontinence increased the significance further to P=0.001. Patient age of greater then 66 years had a 76.6% predictive value for the subsequent need of catheterisation. This further increased to 91.5% when combined with a previous history of either catheterisation or nocturia. The type of anaesthesia or the post-operative analgesia did not significantly influence catheterisation frequency. Conclusion: Patients aged greater then 66 years undergoing joint arthroplasty with previous history of catheterisation or nocturia may benefit from pre-operative bladder catheterisation. Peri-operative catheterisation of high-risk patients in theatre reduces patient discomfort caused by the observation period and avoids bladder atonia consequent of the distention, which may subsequently prolong catheterisation

Osseous first ray surgery is a common day case procedure. Patients are often given regional blocks. We sought to find if there is a difference between ankle block and metatarsal block in this group of patients. After ethical approval was granted and power analysis performed, 25 patients were recruited into each arm of the study. These patients were having either an osteotomy or arthrodesis. All patients had standardised general anaesthesia and received 20mls of 0.5% chirocaine for the blockade. The cohort having the ankle block had infiltration under ultrasound guidance in the anaesthetic room; and the cohort receiving the metatarsal block had infiltration at the end of the procedure. The timings of both the anaesthesia and the operation were recorded for each patient. Patients scored their pain level at 2, 6 and 24 hours. The amount of post-operative analgesia used in the first 24 hours was also recorded by the research nurse. All patients were discharged home with a standardised prescription of analgesia. Analysis of the pain scores showed that there was no difference between the two blocks at any measured time period. Nor was there a difference in the analgesic requirement in the first 24 hours. There was, however, a difference in the time taken for the whole procedure: with the ankle block taking an average of ten minutes more. We conclude that metatarsal blocks are as effective as ankle blocks in providing pain relief after osseous first ray surgery, and may be a more efficient use of time

Purpose. Ankle and hindfoot surgery is associated with severe post-operative pain, leading to a reliance on opiate analgesia and its side effects, longer hospital stays, and patient dissatisfaction. Popliteal sciatic nerve blockade has the potential to resolve these issues. We present our experience with using a continuous local anaesthetic nerve block delivered by an elastomeric pump in patients undergoing major foot and ankle surgery. Methods. All patients undergoing major ankle or hindfoot surgery during a one-year period under a single surgeon were eligible for a continuous popliteal block. An ultrasound-guided popliteal nerve catheter was inserted immediately before surgery and a bolus of bupivacaine infiltrated. Using a 250ml elastomeric pump, a continuous infusion was started immediately post operatively and terminated 48 hours later. Prospective data including post-operative analgesia, nausea and vomiting (PONV), length of stay (LOS), pain scores, and patient satisfaction were recorded daily for 48 hours post operatively. Results. Eighty-one patients (53 male, 28 female) with a mean age 60 years (24–84 years) were included. 66 patients received spinal anaesthesia with 15 having general anaesthetics. There were no complications associated with the nerve catheters. At day 1 post op, 49 (60%) patients reported having no or mild pain. 68 (84%) patients had no PONV. 27 (33%) patients did not require any opiate analgesia during their post op period. Average LOS for all patients was 54 hours, with 41 (51%) discharged within 48 hours. 74 (91%) reported good or excellent pain management in the post operative period. Conclusions. Continuous popliteal sciatic nerve blockade is a safe and effective method for controlling post-operative pain, reducing opiate-induced side effects, and optimising length of stay. Patient-reported outcomes support its use in major ankle and hindfoot surgery. Furthermore, reduced costs from early discharge in combination with a daycase tariff uplift can bring significant financial savings

Introduction. Peri-articular local anesthetic injections reduce post-operative pain in total knee arthroplasty and assist recovery. It is inconclusive whether intra-operative injection of peri-articular morphine is locally effective. The aim of this study is whether the addition of morphine to peri-articular injections in only unilateral knee improves post-operative pain, range of motion, swelling in patients with simultaneous bilateral total knee arthroplasty. Materials and Methods. A prospective single-center double-blinded randomized controlled trial was undertaken to assess the local efficacy of adding morphine to intra-operative, peri-articular anesthesia in simultaneous bilateral total knee arthroplasty. Twenty eight patients with 56 TKAs were randomly divided into 2 groups, unilateral TKA with intraoperative peri-articular injection with adding morphine and the other side TKA without adding morphine. The morphine group received an intraoperative, peri-articular injection of local anesthetic (Ropivacaine 150mg), epinephrine (50μg), ketoprofen (25mg) and methylpredonisolone sodium (20mg) plus 0.1mg/kg of morphine. The no-morphine group received the same amount of local anesthetic, epinephrine, ketoprofen and methylpredonisolone sodium without morphine. The operating surgeon, operating staff, patients, physiotherapists, ward nursing staff and data collectors remained blinded for the duration of study. All surgeries were performed by the same operating team. A standard medial parapatellar approach was used in all operations. Post-operative analgesia was standardized to all participants with celecoxib daily for 3 weeks. Primary outcomes included visual analog pain scores (VAS), ROM and swelling of the thigh. Secondary outcomes included WOMAC and adverse outcomes. Result. There were no significant differences between two groups for pre-operative ROM, pre-operative pain VAS or the circumference of the thigh. There were no statistically significant differences in primary and secondary outcomes between two groups (Figure 1, 2, 3). Discussion. Multiple studies have demonstrated the clinical efficacy of multimodal peri-articular injection of analgesics in TKA for pain relief. However, the opioids often lead to nausea as an adverse effect, which is reported from 25% to 56%. The mechanism of pain relief by morphine is mainly the efficacy through the opioid receptor in central nerve system, and the other mechanism through local opioid receptor (μ-receptor) is rarely revealed for pain relief. Our study used morphine in unilateral TKA and no-morphine in the other side TKA and showed no significant difference in primary and secondary outcomes. These results revealed that the efficacy for pain relief in peri-articular injection without morphine is the same as that in no-morphine group. In conclusion, adding morphine in peri-articular injection could not be locally effective for pain relief

Local infiltration analgesia is a relatively novel technique developed for effective pain control following total knee replacement, reducing requirements of epidural or parenteral post-operative analgesia. The study aimed to investigate the anatomical spread of Local Infiltration Analgesia (LIA) used intra-operatively in total knee arthroplasty (TKA) and identify the nerve structures reached by the injected fluid. Six fresh-frozen cadaveric lower limbs were injected with 180ml of a solution of latex and India ink to enable visualisation. Injections were done according to our standardised LIA technique. Wounds were closed and limbs were placed flat in a freezer at −20°C for two weeks. Limbs were then either sliced or dissected to identify solution locations. Injected solution was found from the proximal thigh to the middle of the lower leg. The main areas of concentration were the popliteal fossa, the anterior aspect of the femur and the subcutaneous tissue of the anterior aspect of the knee. There was less solution in the lower popliteal fossa. The solution was found to reach the majority of the terminal branches of the tibial, fibular and obturator nerves. Overall, there was good infiltration of nerves supplying the knee. The lack of infiltration into the lower popliteal fossa suggests more fluid or a different injection point could be used. The solution that travelled distally to the extensor muscles of the lower leg probably has no beneficial analgesic effect for a TKA patient. This LIA technique reached most nerves that innervate the knee joint which supports the positive clinical results from this LIA technique. However, there may be scope to optimise the injection sites

The aim of this retrospective study is to compare the different anaesthetic techniques in children undergoing circular frame application for lengthening and deformity correction. Data was collected from 68 cases on severe pain episodes, postoperative duration of analgesia, requirement for top-up analgesia, complications and side-effects and number of osteotomies. The three analgesic methods used were morphine infusion (M), epidural analgesia (E) and continuous peripheral nerve blocks (CPNB). There was a significant difference between the number of episodes of severe pain experienced by the patients receiving a morphine infusion when compared to the epidural and CPNB groups (M vs. E p<0.0001, M vs. CPNB p=0.018). CPNB were associated with the lowest incidence of severe pain episodes and top-up analgesia requirements. The difference in the incidence of nausea and vomiting between the methods was approaching significance (p=0.06). A significant difference was found when comparing epidural and morphine infusions (p=0.05). Epidural analgesia was associated with significantly more nausea and vomiting (p=0.023), and motor blockade (p<0.01) than CPNB. The results show the most effective method of post-operative analgesia for our paediatric patients, in light of pain episodes and associated side-effects, is sciatic nerve catheterisation with continuous infusion

We aimed to determine the impact of anaesthetic techniques on post-operative analgesic utilisation in people undergoing total knee arthroplasty. We conducted a prospective study at a district general hospital to assess post-operative analgesia use via patient controlled analgesia (PCA) in patients undergoing a elective total knee arthroplasty. Subjects were divided into three groups; group 1 (general anaesthetic and local nerve block, n = 39), group 2 (spinal only, n= 39) and group 3 (general anaesthetic only, n = 38). The primary outcome measure was post-operative morphine consumption through a PCA within the first 24 hours. All subjects were followed up post-operatively by a dedicated pain control nurse. Data was analysed using SPSS version 17 for Windows (SPSS Inc, Chicago, IL, USA). Results are reported as mean (SD) unless stated otherwise. No significant difference in the age or sex distribution between groups was demonstrable. There was no statistical difference in the average usage of morphine when comparing Group 1 compared to group 2, (77.57 (49.56) vs 65.80 (44.71), p=0.27), group 1 compared to group 3, (77.57 (49.56) vs 77.80 (45.52) p=0.98) and finally group 2 compared to group 3, (65.80 (44.71) vs 77.80 (45.52), p=0.25). In this cohort of patients undergoing total knee arthroplasty the anaesthetic technique used does not appear to have an influence on the post-operative pain management

Background. Post-operative pain following forefoot surgery can be difficult to control with oral analgesia so regional analgesic methods have become more prominent in foot and ankle surgery. It was the aim of this study to evaluate the efficacy of a combination of popliteal and ankle blocks and decide if they provide significantly better post-operative analgesia than ankle block alone in forefoot surgery. Methods. This was a prospective, randomised, controlled and single blind study. The total number of patients was 63, with 37 in ankle block only group (control) and 26 in ankle and popliteal blocks group. All patients underwent forefoot surgery. Post-operative pain was evaluated in the form of a visual analogue scale and verbal response form. Evaluations took place four times for each patient: in the recovery room, 6 hours post-operatively, 24 hours post-operatively and on discharge. The pain assessor, who helped the patient complete the pain evaluation forms, was blinded to the number of blocks used. The amount of opiate analgesia required whilst as an inpatient was also recorded. On discharge the patient was asked to rate their satisfaction with the pain experienced during their hospital stay. Results were analysed using Mann-Whitney tests. Results. Results show that pain is significantly less after 6 and 24 hours and on discharge for those patients with combined blocks. Satisfaction with pain relief is also higher for these patients. Significantly less oral codeine pain relief is also required. Conclusion. A popliteal block in conjunction with an ankle block does reduce post-operative pain significantly more than ankle block alone after forefoot surgery

Introduction. Renal homeostasis has been shown to influence mortality after hip fractures; this is true for patients with both chronic kidney disease, and those who develop acute renal dysfunction after surgery. We have examined the influence of impaired renal function upon mortality and length of stay. We investigated this relationship through accurate mathematical modelling of available biochemistry data on a cohort of hip fracture patients. Methods. Complete data were available for 566 patients treated over a 27-month period. All patients had urea and creatinine checked on admission, and at 24–48 hours after surgery. Post-operative analgesia, fluid therapy, transfusion protocols and orthogeriatric reviews were standardised. Generalised Linear Models and correlation matrices were used. Cox-proportional hazards analyses investigated the association between serum concentrations of urea and creatinine on admission and length of stay and mortality after surgery. Results. The cohort included 427 females and 139 males (mean age 80.6 years, mean post-operative length of stay 19 days). 1-year mortality was 19.1%. Urea and creatinine were significantly, positively correlated with age (more significant for urea). After adjusting for age and sex, risk of mortality was positively related to serum concentrations of creatinine and marginally so for urea. Increased age and a male gender were associated with a higher mortality risk. Risk of discharge from trauma ward, length of stay on trauma ward, and overall length of stay were not related to levels of creatinine and urea at admission. Conclusions. This study shows a quantifiable correlation between renal dysfunction and NOF mortality. The low risk of survival with both male sex and raised creatinine identified this subgroup as the ‘most at risk’. Both age and gender are non-modifiable factors, so this subgroup may require a more targeted approach to the management of their fluid and electrolyte homeostasis

Introduction. Brachial plexus blocks are used widely to provide intra-operative and post-operative analgesia. Their efficacy is well established, but little is known about discharging patients with a numb or weak arm. We need to quantify the risk of complications for improved informed consent. Objectives. To assess whether patients can be safely discharged from hospital before the brachial plexus block has worn off and record any complications and concerns. Study design. Prospective cohort study. Methods. 319 consecutive patients who had a brachial plexus block alone or combined with a general anaesthetic for upper limb surgery were assessed. The adequacy of the block and the outcome of the block was assessed. Results. 238 patients received a general anaesthetic as well as the block and 81 patients received a brachial plexus block alone. The mean time to discharge was 27.5 hours (2-308 hours). Sensation recovered in 15.8 hours (SD- 15.9) and motor power recovered in 15.6 hours (SD- 13.3). The most frequent complication was swelling of the limb which occurred in 8 patients. 7 patients (2.2%) developed Horner's syndrome, 4 patients (1.3%) developed phrenic nerve anaesthesia and 2 patients (0.6%) developed a pneumothorax. Eleven patients had prolonged numbness of >24 hours following the block without any untoward effect. 287 patients (90%) were happy to care for themselves following discharge. Conclusion. Patients can be discharged before the brachial plexus block has worn off with good advice. Patients should be warned of symptoms of Horner's syndrome and phrenic nerve anaesthesia

Purpose: Recent animal evidence has suggested that Bupivicaine may be harmful to articular cartilage. The purpose of this study was establish whether, following arthroscopy of the knee, infiltration of Bupivicaine around the portals is as effective as intra-articular infiltration for post-operative analgesia. Method: Consecutive patients attending for knee arthroscopy were consented and randomised to one of two groups. Following arthroscopy, Group I received 20mls 0.5% Bupivicaine infiltrated into the joint; Group II received 20mls 0.5% Bupivicaine infiltrated around the portals. A Visual Analogue Score (VAS) was collected at one hour post-operatively and rescue analgesia recorded. A power calculation was performed. Ethical approval was granted. Results: There were 68 patients in Group I (intra-articular) and 69 patients in Group II (portal). There was no significant difference in the age or sex distribution of patients in either group. The mean VAS score was 3.04 in Group I and 3.24 in Group II. There was no significant difference between the two groups (p=0.619). There was also no significant difference in the need for rescue analgesia (p=0.930). The study has demonstrated equivalence between the two groups, within one VAS point (Power = 80%). Conclusion: We would recommend that following knee arthroscopy, Bupivicaine should be infiltrated around the portals, avoiding intra-articular infiltration

Regional anaesthetic for foot surgery has been discussed as a method of post operative analgesia. Ankle block as the sole anaesthetic for foot surgery has not been extensively reviewed in the literature. We aimed to describe our experience of forefoot surgery under ankle block. Sixty-six consecutive forefoot procedures (59 patients) were carried out under ankle block. Patients were contacted post operatively and completed a standardised questionnaire including an incremental pain assessment ranging from 0-10 (0 no pain, 10 severe pain). Forty nine female and 10 male patients (age range 20-85y) were included. Procedures included 33 first metatarsal osteotomies, 15 cheilectomies, 3 first MTP joint replacements, 5 fusions, 4 excision of neuroma and 6 other procedures. 22 patients (33% of cases) reported discomfort during the block procedure (average pain score 1.5). 6 patients reported pain during their operation(s), average score 0.26. Average pain scores at 6, 12, 24 and 48 hours following surgery were 2.0, 3.2, 2.7 and 2.1 respectively. All patients were discharged home and walking on the same day. There were no readmissions. Each patient confirmed they would have surgery under regional block rather than general anaesthesia and would recommend this technique to family and friends. There are many advantages in being able to perform these relatively small procedures under regional anaesthesia. The anaesthesia obtained permits the majority of forefoot procedures and provides lasting post-operative analgesia. Combined with intra-operative sedation, use of ankle tourniquet and same day discharge; it has very high patient acceptance and satisfaction

Introduction: The aim of this study is to present the effect of various local anaesthetics, in particular solution concentrations, in peri- and post-operative analgesia in patients with carpal tunnel syndrome (CTS). Material and Method: 105 patients with CTS (81 female, 24 male, ages 27–79) underwent carpal tunnel release under local anaesthesia. The patients were divided into 5 groups (xylocaine 2%, ropivacaine 0.75%, ropivacaine 0.375%, chirocaine 0.5%, chirocaine 0.25%). A tablet of Gabapentin (Neurontin) 400mg was administered to some patients of each group (41 pts of the 105pts) 12 hours prior to surgery. All patients were evaluated immediately after surgery, in 2 weeks and 2 months postoperatively according to VAS pain score, grip strength, finger active motion and two point discrimination. Postoperative complications were also reported. Results: Anesthesia was immediate after the local injection. All patients improved postoperativelly regarding relief from pain and paresthesias. There was no statistically significant difference in grip strength before and after surgery. Only 10 patients used paracetamol immediately after surgery, without any statistically significant correlation to any group of patients. 1 patient developed complex regional pain syndrome 2 months after surgery. Conclusion: The use of local anaesthesia in carpal tunnel release surgery is beneficial in providing immediate intraoperative effect and recovery and mobilization after surgery. Rehabilitation seems to be irrelevant of the type of local anaesthetic that was used during the procedure. Small solution concentrations of local anaesthetics (ropivacaine 0.375%, chirocaine 0.25%) provide adequate analgesia during surgery and provide a normal postoperative course
