Aims. Restoration of proximal medial femoral support is the keystone in the treatment of intertrochanteric fractures. None of the available implants are effective in constructing the medial femoral support. Medial sustainable nail (MSN-II) is a novel cephalomedullary nail designed for this. In this study, biomechanical difference between MSN-II and proximal femoral nail anti-rotation (PFNA-II) was compared to determine whether or not MSN-II can effectively reconstruct the medial femoral support. Methods. A total of 36 synthetic femur models with simulated intertrochanteric fractures without medial support (AO/OTA 31-A2.3) were assigned to two groups with 18 specimens each for stabilization with MSN-II or PFNA-II. Each group was further divided into three subgroups of six specimens according to different experimental conditions respectively as follows: axial loading test; static torsional test; and cyclic loading test. Results. The mean axial stiffness, vertical displacement, and maximum failure load of MSN-II were 258.47 N/mm (SD 42.27), 2.99 mm (SD 0.56), and 4,886 N (SD 525.31), respectively, while those of PFNA-II were 170.28 N/mm (SD 64.63), 4.86 mm (SD 1.66), and 3,870.87 N (SD 552.21), respectively. The mean torsional stiffness and failure torque of MSN-II were 1.72 N m/° (SD 0.61) and 16.54 N m (SD 7.06), respectively, while those of PFNA-II were 0.61 N m/° (SD 0.39) and 6.6 N m (SD 6.65), respectively. The displacement of MSN-II in each cycle point was less than that of PFNA-II in cyclic loading test. Significantly higher stiffness and less displacement were detected in the MSN-II group (p < 0.05). Conclusion. The biomechanical performance of MSN-II was better than that of PFNA-II, suggesting that MSN-II may provide more effective mechanical support in the treatment of unstable intertrochanteric fractures. Aims. This study aimed to determine whether lateral femoral wall thickness (LWT) < 20.5 mm was associated with increased revision risk of intertrochanteric fracture (ITF) of the hip following sliding hip screw (SHS) fixation when the medial calcar was intact. Additionally, the study assessed the association between LWT and patient mortality. Methods. This retrospective study included ITF patients aged 50 years and over treated with SHS fixation between 2019 and 2021 at a major trauma centre. Demographic information, fracture type, delirium status, American Society of Anesthesiologists grade, and length of stay were collected. LWT and tip apex distance were measured. Revision surgery and mortality were recorded at a mean follow-up of 19.5 months (1.6 to 48). Cox regression was performed to evaluate independent risk factors associated with revision surgery and mortality. Results. The cohort consisted of 890 patients with a mean age of 82 years (SD 10.2). Mean LWT was 27.0 mm (SD 8.6), and there were 213 patients (23.9%) with LWT < 20.5 mm. Overall, 20 patients (2.2%) underwent a revision surgery following SHS fixation. Adjusting for covariates, LWT < 20.5 mm was not independently associated with an increased revision or mortality risk. However, factors that were significantly more prevalent in LWT < 20.5 mm group, which included residence in care home (hazard ratio (HR) 1.84; p < 0.001) or hospital (HR 1.65; p = 0.005), and delirium (HR 1.32; p = 0.026), were independently associated with an increased mortality risk. The only independent factor associated with increased risk of revision was older age (HR 1.07; p = 0.030). Conclusion. LWT was not associated with risk of revision surgery in patients with an ITF fixed with a SHS when the calcar was intact, after adjusting for the independent effect of age. Although LWT < 20.5 mm was not an independent risk factor for mortality, patients with LWT < 20.5 mm were more likely to be from care home or hospital and have delirium on admission, which were associated with a higher mortality rate. A new and very unstable intertrochanteric fracture complex is described. The intertrochanteric fracture with extension into the femoral neck is rare but results in an extremely high failure rate because of its instability. A ten-year retrospectic analysis of patients (246) with intertrochanteric fractures treated with a sliding compression hip screw at Hennepin County Medical center was performed. Of these 246 fractures, 20 were classified as fractures with a major intertrochanteric component with extension into the femoral neck. These fractures were similar to intertrochanteric fractures type I-IV described by Kyle and Gustilo in demographics, osteoporosis, and surgical treatment, but this new fracture fracture now described as Type V had a statistically higher rate of mechanical complications 0.0001, reoperation 0.0002, and failure of fixation 0.0001. The overall failure rate was 50%. The majority of these fractures were the result of complete collapse of the hip screw. We feel this higher complication and reoperation rate is secondary to inherent instability in the intertrochanteric fracture which extends into the femoral neck. This instability leads to cmplete collapse of a sliding hip screw result ing in a rigid device that leads to failure of fixation. This fracture complex also has a higher rate of nonunion and avascular necrosis. Although rare, this fracture must be recognized in the fracture classification of intertrochanteric fractures because of its poor prognosis. Other forms of treatment than a sliding hip screw may be considered with this fracture complex because of its high failure rate with standard treatment

The reduction for unstable femoral intertrochanteric fracture should be extramedullary, which means that the proximal fragment protrudes for the distal fragment. However, only few articles have compared extramedullary and intramedullary reductions in a biomechanical study. Thus, we created unstable femoral intertrochanteric fracture models using imitational bone (extramedullary and intramedullary groups, each with 12 cases) and evaluated their biomechanical stabilities. The fracture type was 31-A2 according to the AO-OTA Classification of Fractures and Dislocations and greatly lacked bone on the posterior side. We performed compression examination and evaluated stiffness. The implant used for fixation was TFNA (DePuy Synthes). We applied axial compression with 20 adduction in the standing position. Statistical analysis was performed using the Mann-Whitney U test. No significant difference in initial loading force was found between the two groups. However, the axial stiffness of the extramedullary bone showed a significant increase (p < 0.05) in high loading force (800–1000 N). This means that the stability of the extramedullary reduction was superior to that of the intramedullary reduction in terms of high loading force in the standing position. We suggest that antero-medial bony buttress is important for unstable femoral intertrochanteric fractures. These data indicate that extramedullary reduction and fixation for unstable femoral intertrochanteric fractures increase stability

We report an intertrochanteric fracture in a patient who had undergone hip arthrodesis 30 years previously. A 53-year-old man was injured in a head-on car crash and was referred to our hospital for treatment. Plain radiographs showed an intertrochanteric fracture of the right proximal femur and deformity of the right hip joint. He had undergone hip arthrodesis surgery 30 years previously at another hospital. Computed tomography scan showed marked atrophy of the gluteus and iliopsoas muscles. He preferred undergoing total hip arthroplasty (THA) to internal fixation. THA was performed using the anterolateral approach with the patient in the supine position as he had undergone hip arthrodesis through the Smith–Petersen approach, and we were concerned about damaging the gluteus muscle and dislocation if we took the posterolateral approach. The femoral head was removed using curved chisels under fluoroscopy. A cementless THA (J Taper stem, Aquala polyethylene liner; Kyocera Medical Corporation, Osaka, Japan) was inserted and fixed appropriately. Full weight-bearing using a walking frame was allowed 2 weeks after the surgery. Six months after the operation, he was able to walk independently and had good range of movement of the hip joint but continued to have weakness in the abductor muscles. Very few cases of proximal femur fracture in a previously arthrodesed hip have been reported. Manzotti et al. reported a similar case but they performed open reduction and internal fixation. No previous reports in the literature describe THA for intertrochanteric fracture in an arthrodesed hip. The conversion of an arthrodesed hip to THA is technically challenging. It has a high risk of complications such as nerve injuries and hip instability. We were able to treat the patient successfully, but surgeons should carefully decide the treatment method depending on the case

Introduction. Fracture of the proximal femur frequently occur in children with osteogenesis imperfecta(O.I.) or fibrous dysplasia and may lead to progressive coxa vara and a “shepherds crook” deformity. In adults, these changes introduce difficulties that are not ordinarily encountered with routine osteosynthesis. There is minimal literature on this topic and the cases reported are few in number. Objective. The purpose of this case report was to describe a intertrochanteric fracture in a elderly woman with O.I. successfully treated by 115 degrees hip osteotomy plate and cannulated screws. Methods. We present a case of a 82-year-old female who was injured by falling. She had O.I. type â�£ A according to Sillence. Radiographs showed a intertrochanteric fracture of the femur with severe deformity. The femoral shaft had 25 degrees angular deformity and moderate rotation at the proximal. The angle between femoral neck and shaft was 105 degrees (severe coxa vara) and the proximal femur had a “shepherds crook” deformity (See Figure 1). She had presented 70 years previously ipsilateral fractures of the femur which had healed. These mal-united fracture involved anatomical changes such as medicalization of the femoral canal and intramedullary remodeling and sclerosis (See Figure 2). Recognizing the anatomical changes before and during surgery, standard dynamic hip screw or AO angled blade plate could not fit the femur and not provide stability. Using 115 degrees hip osteotomy plate and cannulated screws, osteosynthesis was performed (See Figure 3). Results. Twelve months postoperatively, the fracture united without complications and the patient felt comfortable and satisfied with gait. Conclusion. An unusual case was presented in which a 82-year-old woman was successfully treated with 115 degrees hip osteotomy plate and cannulated screws for a intertrochanteric fracture of the femur with osteogenesis imperfecta. Standard plate osteosynthesis was unlikely to provide sufficient stable fixation in this case

Introduction. The vast majority of intertrochanteric fractures treated with cephalomedullary nails (CMN) will heal. Occasionally even though bony union occurs excessive lag screw sliding can cause persistent pain and soft tissue irritation and return to surgery for hardware removal. The purpose of this study was to evaluate if fracture stability, lag screw tip-apex distance (TAD), and quality of reduction have any impact excessive lag screw sliding and potential cutout. Methods. As part of our level one trauma center's institutional hip fracture registry, a retrospective analysis identified 199 intertrochanteric fractures fixed with CMN between 2009 and 2015 with follow up to union or a minimum of three months. The mean follow-up was 22 months (3 to 94 months). Mean patient age was 75 years (50 to 97 years) and 72% were women. Postoperative radiographs were used to measure the TAD, quality of reduction, neck-shaft angle (NSA), and lateral lag screw prominence. Follow-up radiographs were reviewed to assess fracture union, translation, and progression of lateral lag screw prominence. Complications and reoperations were recorded. Results. The average lag screw sliding was 5±5 mm. Excessive lag screw sliding (defined as > 10 mm; one standard deviation above the mean) was present in 12% of patients. Lag screw sliding was more common in unstable fracture patterns (21% vs. 5%, p<0.01) and patients with calcar fracture gapping > 4 mm (26% vs. 4%, p<0.01). Lag screw sliding was not associated with age (p=0.9), sex (p=0.4), TAD (p=0.3), implant (p=0.8), distal interlocking screws (p=0.3), or NSA (p=0.2). There were seven (3%) patients with prominent lag screws that required removal. These patients experience more lag screw sliding than those that did not require removal (9 mm vs. 5 mm, p<0.01). The average TAD was 17±5 mm. 15 (7%) of patients had TAD of 25 mm or more. There were 2 cutouts (1%). The average TAD was larger in the cutout group (26 vs. 17 mm, p<0.01). Conclusion. In this series, the incidence of cutout was low and associated with a larger tip-apex distance. Excessive lag screw sliding was associated with unstable fracture patterns, calcar fracture gapping, and more reoperations for symptomatic hardware. Careful attention to calcar fracture reduction may minimize excessive lag screw sliding

The first case was that of an 89-year-old woman with advanced dementia. After falling onto the left hip, she was admitted to the emergency room. Standard x-rays revealed an unstable intertrochanteric fracture. Closed reduction and internal fixation was performed using the long PFNA with a 105-mm helical blade, the second patient treated with this implant in our series. The operation was performed by an attending surgeon who was experienced with treatment of trochanteric fractures with intramedullary devices. Six weeks later the patient presented again with severe pain after she had fallen onto her left hip for a second time. Follow-up x-rays showed a perforation of the helical blade through the cortex of the femoral head after a shortening of the femoral neck by 2 cm. CCD angle was still 129 degrees, and there were no radiological signs of rotational displacement. The acetabular cartilage appeared intact. At her second operation, a replacement of the blade was performed using a shorter 95-mm implant. Postoperatively the patient was again mobilized under full weight bearing, and at 12 weeks follow-up, we found cut through again, we replaced hip with biopolar hemiarthroplasty

Purpose. To observe the follow-up results of standard cemented bipolar hemiarthroplasty with double loop and tension band wiring technique for treatment of unstable intertrochanteric hip fractures in elderly patients with osteoporosis. Materials and Methods. From May 2000 to May 2006, 86 cemented bipolar hemiarthroplasties were performed in elderly patients who had unstable intertrochanteric fractures. The mean age at the time of surgery was 82 years old. The average follow-up period were 5.3 years. We evaluated post-operative results after operation by clinical and radiographic methods. Results. Clinically, the final follow-up of Harrsi hip score was noted 79.2. The mean time needed for full weight bearing following surgery was 4.2 weeks and 82.5% of patients regained their preoperative ambulatory level. All patients achieved union in lesser trochanter fracuture, but great trochanter displacement were observed in 4 cases. There was one case of acetabular erosion. Post-operative superficial infections were found in 2 cases. 1 case with stem subsidence(<5 mm) showed satisfactory results without further subsidence in follow-ups. Conclusion. If we properly apply indications in technique with cemented bipolar hemiarthroplasty in the treatment of unstable intertrochanteric hip fracture in elderly, we will achieve systematic postoperative rehabilitation, pain control and handy nursing which is its one of merits

Background. Hip fractures affect annually over 350,000 people in the USA and over 1.6 million worldwide. About 50% of these numbers are intertrochanteric fractures,. The surgeon should be able to minimize the morbidity associated with the fracture by: recognizing the fracture pattern, choosing the appropriate fixation device, performing accurate reductions with ideal implant placement and being conscious of implant costs. In this study we assessed the ability of the orthopaedic surgeons to recognize fractures pattern, and choosing the ideal implant for the recognized fracture. Methods. We assessed 134 orthopaedic surgeons with questionnaires that assessed 14 different intertrochanteric femoral fractures. We evaluated the fractures as stable or unstable. We chose for each fracture the appropriate fixation device: either a Dynamic Hip Screw (for stable fractures) or an Intra Medullary Nail (for unstable ones), taking into consideration fracture's stability and implants’ costs. We compared the answers of the assessed surgeons to ours. Results. 134 orthopaedic surgeons fulfilled our questionnaires. The average agreement among the assessed surgeons and the authors for fractures’ stability distributed as follows: 78.2% agreement for the stable fractures, 86.2% for the unstable fractures. The agreement for the appropriate fixator between both groups (authors and surgeons) distributed as follows: in choosing the dynamic hip screws for the stable fractures 79.6% agreed with our choice. When choosing the Intra Medullary Nail for the unstable fractures 72.4% of the surgeons agreed with our choice. Interestingly, surgeons that their subspecialty is orthopaedic trauma tended to use more the Intra Medullary Nails in the stable fractures compared to the other surgeons. Conclusions. The majority of the assessed surgeons know to recognise inertrochanteric fractures’ stability and to choose the appropriate fixation device. 20% of surgeons did not agree with our choices. Choosing an Intra Medullary Nail for the stable fracture is not a mistake, but the wrong fixation device for the unstable fracture may cause non-union, mal-union or hardware failure, and might complicate patients’ rehabilitation and cure. We believe that a team discussion should take place for each and any case before operation, and whenever there is a doubt about fractures stability, the Iintra Medullary Nail should be chosen

Introduction. Lag screw cut-out following fixation of unstable intertrochanteric fractures in osteoporotic bone remains an unsolved challenge. A novel new device is the X-Bolt which is an expanding type bolt that may offer superior fixation in osteoporotic bone compared to the standard DHS screw type device. Aims. The aim of this study was to test if there was a difference in cut-out using the X-Bolt implant compared with the standard DHS system. Methods. Specimens of low density surrogate bone (5pcf) were inserted into a simplified biomechanical fracture model and had either an X-Bolt or DHS implant inserted. There were eight samples in each group. The fracture model was tested with an incremental cyclical loading programme in a Material Test System. Displacement, cycle count and force exerted were continuously recorded until cut-out of the implant. Results. All of the specimens failed by varus collapse with superior cut-out and resulted in an automatic stop of the MTS. Specimens with the X-Bolt implant inserted lasted longer on cyclical count and withstood a greater force at cut-out compared with DHS specimens. The mean number of cycles to cutout in the DHS specimens was 4345 and in specimens with the X-Bolt inserted was 6898. The mean force at which cutout occurred in the DHS group was 1.025kN and in specimens with the X-Bolt inserted was 1.275kN. A statistically significant difference was observed with a P-value of 0.005 and a power of 87.2% with respect to cycle count and a P-value of 0.008 and power 84.8% with respect to force exerted at failure when comparing between the two groups. Conclusion. This study shows that the X-Bolt device demonstrated superior cut-out resistance and withstood greater loads compared to the DHS in low density surrogate bone in an unstable fracture model under cyclical axial loading

Introduction Intertrochanteric fractures are common and represent a major source of morbidity and mortality. As with all orthopaedic implants a DHS can fail. One of the most important predictors of failure has been shown to be the Tip-Apex Distance (TAD). An audit was carried to assess the following:. What was our rate of cut out and implant failure?. Where we achieving an acceptable screw position and TAD?. Was there any difference between TAD and grade of surgeon?. Methods An audit of the case notes and x-rays of 54 consecutive patients with hip fractures, treated with DHS, within a twelve-month period were reviewed. Demographic data, grade of surgeon, fracture stability, DHS position, mortality and implant failure were assessed. Findings Our rate of failure was 2 out of 54 patients, 3.7%. Both of the patients that failed had a TAD of greater than 20mm, and none of the patients with a TAD below 20mm required further surgery. There was no statistical correlation between TAD and grade of operating surgeon. Recommendations It is paramount importance to ensure that the basic principles of DHS position are well taught to surgical trainees in order to reduce the risk of failure. Following DHS fixation, patients should be followed up for a minimum of 3 months until evidence of radiographic union is evident. DHS failure rates and screw positions should be constantly audited to ensure that failure rates are minimised

Introduction

The objective of this study was to determine if a synthetic bone substitute would provide results similar to bone from osteoporotic femoral heads during in vitro testing with orthopaedic implants. If the synthetic material could produce results similar to those of the osteoporotic bone, it could reduce or eliminate the need for testing of implants on bone.

Methods

Pushout studies were performed with the dynamic hip screw (DHS) and the DHS Blade in both cadaveric femoral heads and artificial bone substitutes in the form of polyurethane foam blocks of different density. The pushout studies were performed as a means of comparing the force displacement curves produced by each implant within each material.

Aims. This work aimed at answering the following research questions: 1) What is the rate of mechanical complications, nonunion and infection for head/neck femoral fractures, intertrochanteric fractures, and subtrochanteric fractures in the elderly USA population? and 2) Which factors influence adverse outcomes?. Methods. Proximal femoral fractures occurred between 1 January 2009 and 31 December 2019 were identified from the Medicare Physician Service Records Data Base. The Kaplan-Meier method with Fine and Gray sub-distribution adaptation was used to determine rates for nonunion, infection, and mechanical complications. Semiparametric Cox regression model was applied incorporating 23 measures as covariates to identify risk factors. Results. Union failure occured in 0.89% (95% confidence interval (CI) 0.83 to 0.95) after head/neck fracturs, in 0.92% (95% CI 0.84 to 1.01) after intertrochanteric fracture and in 1.99% (95% CI 1.69 to 2.33) after subtrochanteric fractures within 24 months. A fracture-related infection was more likely to occur after subtrochanteric fractures than after head/neck fractures (1.64% vs 1.59%, hazard ratio (HR) 1.01 (95% CI 0.87 to 1.17); p < 0.001) as well as after intertrochanteric fractures (1.64% vs 1.13%, HR 1.31 (95% CI 1.12 to 1.52); p < 0.001). Anticoagulant use, cerebrovascular disease, a concomitant fracture, diabetes mellitus, hypertension, obesity, open fracture, and rheumatoid disease was identified as risk factors. Mechanical complications after 24 months were most common after head/neck fractures with 3.52% (95% CI 3.41 to 3.64; currently at risk: 48,282). Conclusion. The determination of complication rates for each fracture type can be useful for informed patient-clinician communication. Risk factors for complications could be identified for distinct proximal femur fractures in elderly patients, which are accessible for therapeutical treatment in the management. Proximal femur fractures are common in the elderly population. The aim of this study was to determine the relationship between fracture type and proximal femoral geometric parameters. We retrospectively studied the electronic medical records of 85 elderly patients over 60 years of age who were admitted to the orthopedic department with hip fractures between January 2016 and January 2018 in a training and research hospital in Turkey. Age, fracture site, gender, implant type and proximal femoral geometry parameters (neck shaft angle [NSA], center edge angle [CEA], femoral head diameter [FHD], femoral neck diameter [FND], femoral neck axial length [FNAL], hip axial length [HAL], and femoral shaft diameter [FSD]) were recorded. Patients with femoral neck fractures and femur intertrochanteric fractures were divided into two groups. The relationship between proximal femoral geometric parameters and fracture types was examined. SPSS 25.0 (IBM Corparation, Armonk, New York, United States) program was used to analyze the variables. Independent samples t test was used to compare the fracture types according to NSA, FHD, FND and FSD variables. A statistically significant difference was found in FSD (p=0,002) and age (p=0,019). FSD and age were found to be greater in intertrochanteric fractures than neck fractures. Gender, site, CEA, FNAL, HAL, NSA, FHD and FND parametres were not significantly different. In the literature, it is seen that different results have been reached in different studies. In a study conducted in the Chinese population, a significant difference was found between the two groups in NSA, CEA and FNAL measurements. In a study conducted in the Korean population, a significant difference was found only in NSA measurements. The FSD is generally associated with bone mineral densitometry in the literature and has been shown to be a risk factor for fracture formation. However, a study showing that there is a relationship between FSD and fracture type is not available in the literature. In this study; FSD was found to be higher in intertrochanteric fractures (p = 0.002). However, for the clinical significance of this difference, we think that larger patient series and biomechanical studies are needed


Aims. There are concerns regarding nail/medullary canal mismatch and initial stability after cephalomedullary nailing in unstable pertrochanteric fractures. This study aimed to investigate the effect of an additional anteroposterior blocking screw on fixation stability in unstable pertrochanteric fracture models with a nail/medullary canal mismatch after short cephalomedullary nail (CMN) fixation. Methods. Eight finite element models (FEMs), comprising four different femoral diameters, with and without blocking screws, were constructed, and unstable intertrochanteric fractures fixed with short CMNs were reproduced in all FEMs. Micromotions of distal shaft fragment related to proximal fragment, and stress concentrations at the nail construct were measured. Results. Micromotions in FEMs without a blocking screw significantly increased as nail/medullary canal mismatch increased, but were similar between FEMs with a blocking screw regardless of mismatch. Stress concentration at the nail construct was observed at the junction of the nail body and lag screw in all FEMs, and increased as nail/medullary canal mismatch increased, regardless of blocking screws. Mean stresses over regions of interest in FEMs with a blocking screw were much lower than regions of interest in those without. Mean stresses in FEMs with a blocking screw were lower than the yield strength, yet mean stresses in FEMs without blocking screws having 8 mm and 10 mm mismatch exceeded the yield strength. All mean stresses at distal locking screws were less than the yield strength. Conclusion. Using an additional anteroposterior blocking screw may be a simple and effective method to enhance fixation stability in unstable pertrochanteric fractures with a large nail/medullary canal mismatch due to osteoporosis. To conduct a meta-analysis for intertrochanteric hip fractures comparing in terms of efficacy and safety short versus long intralomedullary nails. A pubmed search of the last 10 years for intertrochanteric fracture 31A1-31A3 according to the AO/OTA classification was performed. Baseline characteristics of each article were obtained, complication measures were analyzed: Peri-implant fracture, reoperations, deep/superficial infection, and mortality. Clinical variables consisted of blood loss (mL), length of stay (days), time of surgery (min) and nº of transfusions. Functional outcomes were also recorded. A meta-analysis was performed with Review Manager 5.4. Twelve studies were included, nine were retrospective. The reoperations rate was lower in the short nail group and the peri-implant fracture rate was lower in the long nail group (OR 0.58, 95% CI 0.38 to 0.88) (OR 1.88, 95% CI 1.04 to 3.43). Surgery time and blood loss was significantly higher in the long nail group (MD −12.44, 95% CI −14.60 to −10.28) (MD −19.36, 95% CI −27.24 to −11.48). There were no differences in functional outcomes. The short intramedullary nail has a higher risk of peri-implant fracture; however, the reoperation rate is lower compared to the long nail. Intertrochanteric fractures are common, accounting for nearly 30% of all fracture related admissions. Some have suggested that these fractures should be treated in community hospitals so as not to tax the resources of Level One trauma centers. Since many factors predictive of fixation failure are related to technical aspects of the surgery, the purpose of this study was to compare radiographic parameters after fixation comparing trauma fellowship trained surgeons to non-fellowship trained community surgeons to see if these fractures can be treated successfully in either setting. Using our hospital system's trauma database, we identified 100 consecutive patients treated with cephalomedullary nails by traumatologists, and 100 consecutive patients treated by community surgeons. Quality of reduction, neck shaft angle (NSA), tip-to apex distance (TAD) were compared. The mean TAD for the trauma group was 10mm compared to 21mm for the community group (p<0.001). The mean postoperative NSA for the trauma group was 133 degrees compared to 127 degrees for the community group (p<0.001). The mean difference in the NSA of the fractured side compared to the normal hip was 2.5 degrees of valgus in the trauma group compared to 5 degrees of varus for the community group (p<0.001). There were 93 good reductions in the trauma group compared to 19 in the community group (p<0.001). There were no poor reductions in the trauma group and 49 poor reductions in the community group (p<0.001). Fellowship trained traumatologists achieved significantly more accurate reductions and implant placement during cephalomedullary nailing of intertrochanteric hip fractures

Failure of cephalomedullary fixation for proximal femur fractures is an uncommon event associated with significant morbidity to the patient and cost to the healthcare system. This institution changed nailing system from the PFNA (DePuy Synthes) to InterTan (Smith and Nephew) in February 2020. To assess for non-inferiority, a retrospective review was performed on 247 patients treated for unstable proximal femur fractures (AO 31 A2; A3). Patients were identified through manual review of fluoroscopic images. Stable fracture patterns were excluded (AO 31 A1). Pre/post operative imaging, demographic data, operative time and ASA scores assessed. Internal/external imaging and national joint registry data were reviewed for follow up. No significant difference was found in overall failure rate of PFNA vs InterTan (4.84% vs 3.23%; p = 0.748). Overall, short nails were more likely to fail by cut-out than long nails (7.5% vs 1.2%; p = 0.015). Nails which failed by varus cut-out had a higher tip/apex distance (TAD) (26.2mm vs 17.0mm; p < 0.001). Of concern, varus cut-out occurred in two InterTan nails with TAD of <25mm. The PFNA enjoyed a shorter operative time for both the short (59.1 vs 71.8 mins; p = 0.022) and long nails (98.8 vs 114.3 mins; p = 0.016) with no difference in 120-day survival rate. Overall failure rates of the PFNA and InterTan nailing systems were comparable. However, the failure rate of short nails in this study is concerning. Using long nails with a lag screw design for unstable intertrochanteric femoral fractures may reduce failure rates. Cumulative frequency analysis suggests stringent tip-apex distances of less than 21mm may reduce failure rates in lag-screw design cephulomedullary nails. This dataset suggests that unstable intertrochanteric fractures may be more reliably managed with a long cephalomedullary device

Aim. The most frequent mechanical failure in the osteosynthesis of intertrochanteric fractures is the cut-out. Fracture pattern, reduction quality, tip-apex distance or the position of the cervico-cephalic screw are some of the factors that have been associated with higher cut-out rates. To date, it has not been established whether underlying bacterial colonization or concomitant infection may be the cause of osteosynthesis failure in proximal femur fractures (PFF). The primary objective of this study is to assess the incidence of infection in patients with cut-out after PFF osteosynthesis. Method. Retrospective cohort study on patients with cut-out after PFF osteosynthesis with endomedullary nail, from January 2007 to December 2020. Demographic data of patients (such as sex, age, ASA), fracture characteristics (pattern, laterality, causal mechanism) and initial surgery parameters were collected (time from fall to intervention, duration of surgery, intraoperative complications). Radiographic parameters were also analyzed (tip-apex distance and Chang criteria). In all cut-out cases, 5 microbiological cultures and 1 anatomopathological sample were taken and the osteosynthesis material was sent for sonication. Fracture-related infection (FRI) was diagnosed based on Metsemakers et al (2018) and McNally et al (2020) diagnostic criteria. Results. Of the 67 cut-out cases, 16 (23.9%) presented clinical, analytical or microbiological criteria of infection. Of these sixteen patients, only in 3 of them the presence of an underlying infection was suspected preoperatively. A new osteosynthesis was performed in 24 cases (35.8%) and a conversion to arthroplasty in the remaining 43 (64.2%). A comparative analysis was performed between cases with and without infection. The groups were comparable in terms of demographic data and postoperative radiological data (using Chang criteria and tip-apex distance). Patients with underlying infection had a higher rate of surgical wound complication (56.3% vs 22%, p = 0.014), higher rates of leukocytes counts (11.560 vs 7.890, p = 0.023) and time to surgery (5.88 vs 3.88 days, p = 0.072). Conclusions. One out of four osteosynthesis failure in PFF is due to underlying FRI and in almost 20% were not unsuspected before surgery. In PFF osteosynthesis failures, underlying infection should be taken into account as a possible etiological factor and thus a preoperative and intraoperative infection study should be always performed
